The paper is a part of a general mycological monitoring study of nosocomial infections. Differences in the species composition and drug resistance of fungi isolated from in-patients and out-patients prompted an analysis of this topic. The studies were conducted with fungi from the collection of the Department of Mycology, University of Warmia and Mazury in Olsztyn, that were isolated from the oral cavity, sputum, bronchoscopic fluid, anus and skin from 100 in-patients and 100 out-patients. Laboratory analyses, including species categorization of fungi, were performed according to routine mycological diagnostics. Drug sensitivity to fluconazole and nystatin was tested with the disc diffusion method. In the group of in-patients, a wider taxonomic diversity of fungi (12 species) was found in comparison with the out-patients (7 species) and 31 cases of multifocal infections were recorded, while in the second group the number of the latter was only six. In all patients, C. albicans were predominant, constituting the largest proportion in focal infections in all patients and in multifocal infections in the in-patients. In the latter, over a half of the examined individuals were resistant to fluconazole (C. glabrata and C. krusei – 80%, C. tropicalis and S. capsularis – 60%, C. guilliermondii – 50%) and nystatin (T. beigelii – 80%, C. krusei and C. tropicalis – 50%). Substantially lower drug resistance of fungi was recorded in the out-patients. The hospital environment is an abundant reservoir of different fungal species with significantly greater expansiveness and aggressiveness compared to the environment outside a hospital.

Informal payments in health care. Polish perspective and experiences Informal patient payments are a common phenomenon for the formersocialist countries, though they are reported in other European countries as well. There are various definitions of informal patient payments as well as theories which explain this phenomenon (including fee for service theory, donation hypothesis, governance hypothesis and ethics hypothesis). The definitions of informal patient payments and the theories applied for their explanation determine the measures which are taken in order to eradicate informal patient payments.The topic of informal patient payments in Poland was discussed within a corruption debate which was neglected for a long time. Since the end of 90s, due to the pressure of the international organizations, presence of corruption generally and informal patient payments particularly have been acknowledged. It resulted in various studies on informal patient payments as well as actions undertaken by governmental and nongovernmental organisations in order to eliminate these forms of payments.This paper presents the review of empirical studies on informal patient payments and actions which have been carried out in Poland during last two decades. The types, scope and levels of informal patient payments as well as opinions on informal patient payments are analyzed. Time series data allow to study also a dynamics of informal patient payments and to draw some conclusions on the effects of measures which have been implemented by the Polish government to deal with informal patient payments.This study is carried out under Project ASSPRO CEE 2007 funded by the European Commission under the 7th Framework Programme, Theme 8 Socio-economic Sciences and Humanities, Project ASSPRO CEE 2007 (Grant Agreement no. 217431). Introduction: A sick person who trusts his or her physician feels safer and more easily complies with the physician’s recommendations. Purpose: To validate to validate a Greek-language version of the Trust in Physician Scale (TPS) for Greek patients. Materials and Methods: The validation of a Greek-language version of the TPS was performed with a group of 251 patients at Kavala Hospital in Kavala, Greece. Validation consisted of the translation and evaluation of the psychometric properties of the Greek TPS and its application among Greek-speaking patients. Results: The internal consistency of the Greek TPS was high (Cronbach’s alpha = 0.895). The highest mean scores were for the items “I trust my doctor very much, and I always try to follow his or her advice” (M = 3.63 ± 0.91), “If my doctor tells me that something is true, then it must be true” (M = 3.55 ± 0.89), “I trust my doctor’s judgment of my medical care” (M = 3.44 ± 0.86), and “My doctor is usually considerate of my needs and puts them first” (M= 3.41 ± 0.88). Conclusions: The Greek-language version of The Trust in Physician Scale fulfills all of the criteria of psychometric and functional validation with the original scale.

Sudden unexpected death in people previously considered healthy represents an important challenge for modern medicine. The causes of this phenomenon are constantly being sought and attempts are being made to understand its pathomechanism. Among those at great risk are patients with epilepsy. Every unexpected death in a patient with epilepsy who was generally healthy in normal life activity and no other cause of the death was identified, is considered sudden unexpected death in epilepsy (SUDEP). Despite the difficulty in predicting sudden death, special attention should be paid to those at high risk, especially affected by cardiovascular diseases and epilepsy.
Nagłe niespodziewane zgony u osób dotychczas uważanych za zdrowe stanowią istotne wyzwanie dla współczesnej medycyny. Wciąż poszukuje się przyczyn tego zjawiska i próbuje poznać jego patomechanizm. Wśród osób szczególnie narażonych znajdują się osoby chorujące na padaczkę. Każdy niespodziewany zgon u chorego na padaczkę, którego ogólny stan zdrowia był dobry, aktywność życiowa normalna i u którego nie znaleziono innej przyczyny chorobowej tego zdarzenia, określa się terminem nagłej niespodziewanej śmierci u chorych na padaczkę (sudden unexpected death in epilepsy – SUDEP). Pomimo trudności w przewidywaniu nagłych zgonów szczególnym nadzorem należy objąć osoby z grupy podwyższonego ryzyka, do których niewątpliwie należą chorzy z chorobami układu sercowo-naczyniowego oraz z padaczką.

The aim of the study was to show selected aspects of commercial offer of health resort facilities in Poland. The special attention was given to profile treatment, health services, average prices, additional offer of health and other services, including SPA&wellness as well as business services. The analysis of health resort treatment in 2010-2013 was conducted with special attention given to number of health resort facilities in general and by voivodeships. The activity of health resort facilities was presented in quantitative approach using number of inpatients including self-pay inpatients as well as number of beds in health resort facilities.
Celem opracowania było pokazanie wybranych aspektów oferty komercyjnych usług placówek lecznictwa uzdrowiskowego w Polsce. Szczególną uwagę skupiono na profilach leczenia, proponowanych zabiegach leczniczych, przeciętnych cenach za pobyt, ofercie dodatkowych usług leczniczych i usług pozostałych, włączając usługi SPA&Wellness oraz biznesowe. Przeprowadzono analizę lecznictwa uzdrowiskowego w Polsce w latach 2010–2013 z uwzględnieniem ogólnej liczby placówek, ich rodzajów i zróżnicowania terytorialnego. Działalność placówek lecznictwa uzdrowiskowego przedstawiono w ujęciu ilościowym, wskazując na liczbę łóżek oraz liczbę kuracjuszy, w tym ponoszących pełną odpłatność.

The article addresses the topic of alternative legal models for pursuing out-of-court claims for medical damages, taking as its starting thesis the inefficiency of the Polish model stemming from its faulty legal construction. The purpose of the article is to conduct a legal analysis of the Polish alternative model of medical damage compensation, with particular attention to de lege ferenda postulates presented in the legal doctrine. In addition, in order to show the possible directions of legislative changes, the article presents the characteristics of selected foreign alternative models of medical damage compensation, including the model operating in New Zealand, which is an example of a model that can be classified as ‘no-fault’, the German model and the French model. The article culminates with an analysis of the draft of the Law on Quality in Health Care and Patient Safety to the extent that the draft provides for a new legal regulation of the alternative model for the out-of-court settlement of medical damage claims. The purpose of this analysis was to see whether the legislator has recognized the demands for change articulated in the legal doctrine, and whether the draft law on quality in health care and patient safety is inspired by solutions adopted in foreign legislation in terms of the legal construction of the alternative model of compensation for medical damages. The analysis here confirms the initial thesis of the work and leads to the conclusion that the draft law on quality in health care and patient safety takes into account many proposals constructed in the legal doctrine in connection with regulating an alternative model for the compensation of medical damages. It is also clear that the proposed model is inspired by some solutions that have been successful in other countries. Nevertheless, the article distinguishes proposed regulations that may still raise doubts and reservations.

The right to use health services, including spa treatment, is one of the basic patient rights. The specificity of the issues undertaken stems from at least two sources. First of all, patients are a special category of people, because human health, as the highest value possessed by every human being, is subject to legal protection and requires their rights and freedoms are guaranteed. Secondly, spa treatment, which is an integral part of the health care system, corresponds to art. 68 of the Polish Constitution, assuming the fundamental principle according to which every citizen has the right to health care. On the other hand, this treatment means providing health services, which are conducted only in a separate area of the spa commune – a spa and in specially adapted medical entities – spa treatment centers. The study covers the concise genesis of patient rights and the notion of the patient in terms of doctrine and legislation. Moreover, it contains the basic concepts related to spas. Recognition of the health needs of society is associated with a presentation of the individual types of spa treatment centers and spa treatment facilities, as well as the necessary conditions that they should meet. Due to the existing different status of patients in spa treatment institutions, the analysis covers the basis for admission, for referring and qualifying patients to these centers, and also the rules for using services guaranteed in the field of spa treatment. The patient’s right to health services is also examined in the context of using these services. Despite the fact that patients’ interest in this form of treatment increases every year, as is shown by the available data, there are a number of legal and organizational problems related to access to treatment as part of spa services.

This paper presents the main assumptions of patient-centred care, which emphasises the importance of getting to know and taking into account the patient’s perspective throughout the treatment process. The aim of this study is to demonstrate how communication skills, which are taught as part of classes in medical communication, facilitate the provision of patient-centred care. The paper discusses selected communication skills and presents fragments of students’ assignments demonstrating the practical application of the acquired skills. The studies provide examples of utterances fostering the exchange of perspectives and building of a common look at the patient’s problem and reinforcing the patient’s sense of being in the centre of care.

What we usually call loneliness is surely something experienced by: Palamedes and Ajax at Troy – the victims of Odysseus’s envy, Antygone – after meeting her sister Ismene, Prince Hamlet – after speeking to his mother. The author added to K. Jaspers b order situations his own loneliness research model: about invasive modality in surgery. It consists of two obligatory components parts: 1. Surgeon’s decision given to the patient in the form of proposal – suggestion.2. Patient’s approval – consent. Without surgeon’s proposal – patient consent is meaningless. Without the patient’s approval, the operation cannot usually take place. Patient’s approval, or refuse, is this part of the surgical decision, were we can distinguish: 1. Man’s sovereignty in disposing of his health and life. 2. Existence of a distinct self – individuality of existence “I”. The research model reveals, perhaps even more clearly than the another ones, the ontic dimension of loneliness. Loneliness is the price of identity – oneselfness. In conclusions the author stressed: Necessity of discussing patient’s consent in conjunction with surgeon’s decision. Attempts to isolate the issue of patient’s approval lead to aporia, in particular cases to unhappiness. Whilst describing, by the surgeon, diagnosis and treatment plan, the patient does not gain any competence in the branch of surgery; he is preoccupied with examining the doctor. The most important thing is whether he can trust him. Even the best functioning institutions and community customs cannot remove loneliness completely, especially in border situation s. Therefore they may not be blamed for the fact that the loneliness exists. Time is a strong generator of tensions connected with loneliness. Agreeing with N. Hartmann, that the man exists in his achievements, the author adds: and in decisions made in solitude. There is also creativity here.

Although the constitutional right to health care itself has not been subject to legislative changes, the emergence of some other legal regulations and their impact on the current state of the healthcare system justifies additional examination. The authors of the article indicate, among others, areas where the most significant changes to the Polish healthcare system took place in the recent years, and wherever possible, they also assess the current situation. The analysis of the current legal basis of the right to health protection focuses particularly on such important values as: equality in access to health and solidarity, human dignity and patient rights, and professional ethics of person providing benefits.

Prader-Willi syndrome (PWS) is a developmental disorder caused by a deficiency of paternal contribution of the chromosome region 15q11.2-q13 arising from differently sized deletions, maternal disomy, or rarely imprinting mutations. We have analyzed 20 PWS patients using combined cytogenetic high resolution technique (HRT), fluorescence in situ hybridization (FISH) and molecular studies to identify parental origin (uniparental disomy) or molecular defect (deletion) of the Prader-Willi region. Lack of a paternal copy of 15q11.2-q13 resulting from its deletion was found in 16 patients. Using high resolution GTG banding on prometaphase chromosomes, deletion in the 15q11.2-q13 region was detected in only 8 patients. Application of FISH with different sets of PWS specific unique sequence probes (D15S11, SNRPN, D15S10, GABRß3) revealed microdeletions in 12 patients. In 12 out of 20 cases FISH confirmed HRT studies, while in 8 cases inconsistent results were obtained. No discrepancies between results of FISH and molecular studies were found, although the latter had a higher sensitivity. We conclude that FISH appears to be a rapid and reliable method of microdeletion identification and should be performed as a method of choice in cytogenetic diagnosis of Prader-Willi syndrome.

This paper argues that in the case of mental illnesses whose somatic bases are not known or do not exist, a promising route to understand mental illness is to see it as the lack of a patient’s engagement with some moral values that are necessary for a good human life. The paper explains how the first-person perspective, which is constitutive for mental illnesses, makes it impossible to provide an adequate, third-person explanation of the pathological. Because of its irreducible first-personal nature, mental illness must be understood (also) in terms of a moral harm to the patient, and so an integration of ethics and psychiatry (at least at the level of practice) is required. This view is further illustrated with A. Kępiński’s idea of psychiatry as therapy with moral values.
