Opis:
Role of E. granulosus-specific IgG antibody avidity in evaluation of a natural history of the parasite cysts in the liver, duration of infection and immunogenic cysts activity was studied. Thirty-one patients with liver cysts suspected of echinococcosis were examined; E. Granulosus infection was finally confirmed by a fine needle aspiration biopsy or surgery in 29 cases (93.5%) and two non-parasitic simple cysts in the liver were diagnosed. E. granulosus-specific IgG antibody was found in 21 patients (67.7%). High IgG avidity values were observed in 14 patients with liver cysts in a degcneration stage confirmed by imaging techniques. Low IgG avidity values were mostly detected in children with hypoechogenic echinococcal cysts being in a recent stage of infection. Determination of anti-E. Granulosus immunoglobulin G antibody avidity is a very usefull diagnostic tool for a determination of parasitic cysts activity and their developmental age.

Opis:
Immunocapture assays ISAGA PLUS IgA/IgM (bioMerieux) and IgE ISAGA were used to determine their usefulness in the diagnosis of acquired and congenital toxoplasmosis. Specific IgM, IgA and IgE antibodies were tested in 134 patients, namely pregnant women who seroconverted during gestation (n= 20), children with congenital toxoplasmosis (n= 5), patients with toxoplasmic lymphadenitis (n= 56) and immunocompetent individuals with chronic Toxoplasma gondii infection (n= 53). Altogether 172 sera were examined. Specific IgM antibodies were detected in all sera from pregnant women (100%) with recent T. gondii infection (1- 8 weeks after seroconversion), in all patients with toxoplasmic lymphadenopathy (1-3 months after onset of symptoms) and in their control examinations after 2 and 5 months (100%) and also in 35 (66%) out of 53 patients with chronic infection. In infants with congenital toxoplasmosis IgM were found only in one new-born; equivocal results were obtained in 3 children during the asymptomatic serological reactivation in the second year of life. Specific IgA antibodies were present in sera from 15 (75%) out of 20 women seroconverted during pregnancy; in 3 cases the results were equivocal. IgA antibodies were detected in sera from 30 (81.1%) out of 37 patients with toxoplasmic lymphadenitis examined once; in 19 patients examined 3 times IgA antibodies were present in all the cases in the first serological examination performed when clinical symptoms were first observed (100%), in 17 patients after 2 months (89.5%) and in 11 patients after 5 months (57.9%). IgA antibodies were also detected in 21 sera (39.6%) from patients with chronic T. gondii infection. In children with congenital toxoplasmosis IgA antibodies were found in 3 cases during serological reactivation after discontinuation of pyrimethamine-sulfadiazine therapy; in these cases equivocal results of IgM antibodies were present, and positive result of IgE antibodies in one case. Specific IgE antibodies were detected in sera from 17 (85%) out of 20 women with seroconversion and in 18 patients with lymphadenopathy (32.1%); in the last group IgE antibodies were not present in the follow-up examination after 5 months. IgE antibodies were detected only in 5 cases (9.4%) with chronic infection. IgA and IgE antibodies in ISAGA begin to appear about a week later than IgM antibodies; in sera collected between the 2nd and 3rd week after invasion the positive results were obtained in all cases (100%). Therefore, ISAGA PLUS IgA/IgM (bioMerieux) is useful for the diagnosis of recent T. gondii infection especially in women with suspected seroconversion during pregnancy. ISAGA PLUS IgA/IgM is more sensitive than any conventional method routinely used and so far is a specially eflicient technique for newborns and infants suspected for congenital infection and/or in diagnosing congenital toxoplasmosis during immunological recrudescence. This test has a limited value in toxoplasmosis with lymphadenopathy by reason of possibility of a long persistence of IgM and IgA antibodies detected by ISAGA. Detection of specific IgE antibodies using ISAGA technique may be useful for differential diagnosis of acute and chronic phase of T. gondii infection and also in some cases of serological reactivation of congenital toxoplasmosis.

Opis:
Occurrence of Pneumocystis carinii DNA aod anti-Pneumocystis antibodies in sera of infants at the age of humoral response natural degradation. The polymerase chain reaction (PCR) and indirect immunofluorescent test (IF) were used for examination of serum samples obtained from infants with respiratory tract infections. Sixty (11,9%) out of the 503 examined infant samples were positive for anti-P. carinii IgM and 354 (70,4%) contained anti-Pneumocystis IgG. P. carinii DNA was found in 6 (6,7%) sera from 90 of infected infants. Five out these 6 samples were for anti-Pneumocystis antibodies positive; 4 contained both IgG and IgM classes and one bad only IgG. The sixth sample had neither IgG nor IgM, despite of P. carinii DNA presence. The results of the studies indicated that for diagnosis Pneumocystis carinii pneumonia (PCP) in infants on serum specimens detection of antibodies by IF test is of greater value than Pneumocystis DNA amplification by PCR method.

Opis:
The effect of pleasant and unpleasant odor on human physiological state was preliminary investigated by assessing salivary substances. A term ”pleasant“ (or unpleasant) is rather showing one's subjective and thus mental state meanwhile the odor itself should give an impact to both human mind and body. This study was aimed at investigating an effect of pleasant/unpleasant odors on human body and mind. In the experiment the odor of isovaleric acid, as an unpleasant odor, and Japanese cypress, as a pleasant one, were exposed to subjects for 18 minutes. Salivary immune substance (Immunoglobulin A: IgA) and glucocorticoid (cortisol) were determined every three minutes as for an indices of the impact of the odors on human immune and endocrine secretion. As a developing result, IgA showed an increase by isovaleric odor whilst cortisol decreased, even though the concentration of that unpleasant odor was too low to be aware of. These changes in the secretion of the substances were discussed in relation with human physiological stress reaction mechanism.

The immune system, its cellular and humoral response, is engaged by the host organism to fight against parasitic invasions. The group examined consisted of 38 women aged 19-39 years infected with Toxoplasma gondii. The diagnosis was established basing on serologic examination. Blood for analysis was collected before antiparasitic treatment. Control group consisted of 40 healthy women aged 18-46 years. The concentrations of IgM, IgG and IgE were assayed using a set of VIDAS (bioMerieux) according to the ELFA method. The concentrations of IL-5, IL-6 were assayed using a set of Quantikine human (R&D Systems) according to the immunoenzymatic methods with labeled antibodies. The present study revealed that in toxoplasmosis the concentration of IgE, IL-5 and IL-6 contents in blood serum was 2-times higher, than in healthy controls.

Blood for analysis was collected before antiparasitic treatment. Control group consisted of 40 healthy people (22 women and 18 men) aged 20-45 years. The concentration of IgE were assayed using a set of VIDAS (the ELFA method). The concentration of IL-5, IL-6 were assayed using a set of Quantikine human (R&D Systems). The study demonstrated that in echinococcosis the concentrations of IgE, IL-5 and IL-6 contents in blood serum was higher 10-, 2- and 3-times, respectively than in healthy controls.

Opis:
Toxocara canis and Toxocara cati do not transform in human organism into a mature form and they circulate reaching various organs and tissues causing characteristic symptoms. Activated eosinophils, which play a significant role in parasitic invasion, contain in their granules eosinophilic cationic protein (ECP) of strong pro−inflammatory activity. The aim of the study was to estimate the concentration of immunoglobulin E, peripheral blood eosinophilia and serum concentration of eosinophilic cationic protein in children treated for toxocariasis and the analysis of their value as the markers of active invasion and the therapy efficacy. The study included 45 children, aged from 3 to 18 years with Toxocara canis infection diagnosed for the first time. The children were diagnosed and treated at the Department of Gastroenterology, in the Outpatient Gastrointestinal and Allergologic Clinic, Institute Polish Mother Health Centre. T. canis larva infection was diagnosed based on serological investigation using immunoenzymatic ELISA test (Bordier Affinity Products, Switzerland). The tests (percentage of eosinophilia in peripheral blood, ECP, IgE) were performed after diagnosis and every 3 months since the beginning of the therapy. In children with toxocariasis its covert form was diagnosed and mebendazole was administered. Among the children with toxocariasis eosinophilia was found in 14 (31.1%) before therapy. Mean percentage of peripheral blood eosinophils was 5.58% in children with toxocariasis. The concentration of IgE was elevated in these children and was significantly higher than in the control group (p=0.002). Mean IgE concentration after 3−and 6−month therapy decreased IgEI vs IgEIII (p=0.01), but it was still higher than normal value. In children with toxocariasis the ECP concentration was 30.19 µg/l before the therapy and was significantly higher than in the control group (p<0.05); after 6 months of the therapy it decreased significantly (p<0.05). Eosinophilic cationic protein and eosinophilia can be the markers of Toxocara canis infection activity. The determination of immunoglobulin E and eosinophilic cationic protein concentration may be useful for toxocariasis therapy monitoring.

Wstęp: Choroba IgG4-zależna (IgG4-RD) jest niedawno opisaną jednostką chorobową o nieustalonej etiologii i patogenezie charakteryzującą się występowaniem bogatych w komórki IgG4 dodatnie nacieków tkankowych z włóknieniem. Zmiany w jej przebiegu lokalizują się w różnych narządach, jednak w obrębie przewodu pokarmowego występują rzadko. Rozpoznany w naszej Klinice przypadek izolowanego zajęcia żołądka przez IgG4-RD stał się punktem wyjścia do wykonania przeglądu aktualnej literatury w celu przybliżenia lekarzom tej nowej jednostki chorobowej. Materiały i metody: Przeglądu literatury dokonano w oparciu o bazę PubMed. Do przeglądu włączono 8 prac dotyczących choroby IgG4-zależnej ograniczonej do żołądka opublikowanych w latach 2001–2017 oraz przypadek IgG-RD rozpoznany przez autorów pracy. Wyniki: U 3 z 9 chorych występowały objawy ze strony przewodu pokarmowego, w innych przypadkach zmiany zostały wykryte przypadkowo. U większości pacjentów miały one charakter guza podśluzówkowego. Tylko w jednym przypadku zaobserwowano zmianę w postaci owrzodzenia. Najczęstszą lokalizacją zmiany był trzon żołądka. Wszystkie zmiany zostały usunięte chirurgicznie z powodu podejrzenia nowotworu złośliwego, a diagnozę postawiono na podstawie typowego obrazu choroby w badaniu histologicznym i immunohistochemicznym. Podwyższony poziom IgG4+ w surowicy stwierdzono u jednej pacjentki. W żadnym przypadku nie zaobserwowano wznowy ani pojawienia się innych zmian o typie IgG4-RD. Wnioski: Choroba IgG4-zależna może manifestować się w postaci zmian ograniczonych do żołądka i powinna być brana pod uwagę w diagnostyce różnicowej. Postawienie ostatecznego rozpoznania przed wykonaniem badania histopatologicznego jest trudne, co przekłada się na pomyłki diagnostyczne i terapeutyczne. IgG4-RD charakteryzuje się dobrą odpowiedzią na glikokortykosteroidy. W tym aspekcie konieczne są jednak dalsze badania.
