Introduction. The term ‘subarachnoid haemorrhage’ (SAH) stands for bleeding into the subarachnoid space, regardless of its source. It may be of primary character when the source of bleeding is situated within the subarachnoid space. Subarachnoid haemorrhage is often described as spontaneous bleeding, mainly in order to differentiate it from post-traumatic bleeding. Objective. The aim of the study was to evaluate the concentration of ICAM-1 and VCAM-1 in the blood of patients in the early phase following subarachnoid haemorrhage in terms of searching for markers useful in subarachnoid bleeding diagnostics and monitoring a patient’s clinical state. Materials and method. The study comprised 85 patients (47 women, 38 men), aged 29–81 (average 53±12 years), suffering from subarachnoid haemorrhage. The control group comprised 45 healthy people selected according to gender and age corresponding with the experimental group. Results. The study revealed that the concentration of ICAM-1 and VCAM-1 was significantly higher in patients suffering from subarachnoid haemorrhage. Additionally, the concentration of fibrinogen decreased, aPTT was shorter and the concentration of D-dimers increased. The studied parameters did not differ with respect to the age or gender of the patients. It was stated that according to the Hunt and Hess scale, the concentration of ICAM-1 was considerably higher in the group of patients in the most severe neurological state, compared to other patients. It was also observed that the concentration of fibrinogen was significantly higher, aPTT was shorter, and the concentration of D-dimers increased in the afore-mentioned group. Conclusions. Evaluation of the concentration of adhesion molecules, as well as values of global tests concerning the coagulation system, may serve as a useful diagnostic tool for SAH.

Opis:
There is increasing evidence that genetic variability influences patients' early morbidity after cardiac surgery performed using cardiopulmonary bypass (CPB). The use of mortality as an outcome measure in cardiac surgical genetic association studies is rare. We publish the 30-day and 5-year survival analyses with focus on pre-, intra-, postoperative variables, biochemical parameters, and genetic variants in the INFLACOR (INFLAmmation in Cardiac OpeRations) cohort. In a prospectively recruited cohort of 518 adult Polish Caucasians, who underwent cardiac surgery in which CPB was used, the clinical data, biochemical parameters, IL-6, soluble ICAM-1, TNFα, soluble E-selectin, and 10 single nucleotide polymorphisms were evaluated for their association with 30-day and 5-year mortality. The 30-day mortality was associated with: pre-operative prothrombin international normalized ratio, intra-operative blood lactate, postoperative serum creatine phosphokinase, and acute kidney injury requiring renal replacement therapy (AKI-RRT) in logistic regression. Factors that determined the 5-year survival included: pre-operative NYHA class, history of peripheral artery disease and severe chronic obstructive pulmonary disease, intra-operative blood transfusion; and postoperative peripheral hypothermia, myocardial infarction, infection, and AKI-RRT in Cox regression. Serum levels of IL-6 and ICAM-1 measured three hours after the operation were associated with 30-day and 5-year mortality, respectively. The ICAM1 rs5498 was associated with 30-day and 5-year survival with borderline significance. Different risk factors determined the early (30-day) and late (5-year) survival after adult cardiac surgery in which cardiopulmonary bypass was used. Future genetic association studies in cardiac surgical patients should account for the identified chronic and perioperative risk factors.

Nurkowanie ma też wpływ na stan czynnościowy śródbłonka naczyń. Brak jest obecnie doniesień dotyczących wpływu nurkowań na ekspresję genów odpowiadających za funkcje śródbłonka. Celem przeprowadzonych pilotażowych badań była ocena wpływu kspozycji hiperbarycznych i dekompresji na ekspresję genów mających znaczenie w biologii komórek śródbłonka. W badaniach wzięło udział 5 zawodowych nurków. Zostali oni poddani krótkotrwałym ekspozycjom hiperbarycznym (0,7 MPa) odpowiadającym nurkowaniom na głębokości 60 m. Dekompresję przeprowadzano zgodnie z tabelami Polskiej Marynarki Wojennej. Ocenę ekspresji genów przeprowadzono metodą RT-PCR z użyciem zestawów RT2 PCR Profiler Array (Superarray Bioscience, a Qiagen Company, USA) oraz cyklera CFX 96 Bio-Rad. Badania ekspresji genów i analiza wyników wykazały zmniejszoną ekspresję genów BCL2 i BCL2A1 oraz ICAM-1 i PECAM-1. Wskazane są dalsze badania w celu ustalenia, czy wykazany w badanej grupie nurków spadek ekspresji łącznie genów PECAM-1, ICAM-1, BCL2, BCL2A1 może mieć udział we wzroście ryzyka wystąpienia patologicznych zmian kostnych po dekompresji.
Diving has some impact on the functional status of the vascular endothelium. There are currently no reports on the effects of diving on the expression of the genes responsible for the functions of the endothelial. The aim of this pilot study was to evaluate the effects of hyperbaric exposure and decompression on the expression of genes relevant to the biology of endothelial cells. The studies were carried out on five professional divers. They were subjected to short-term hyperbaric exposures (0.7 MPa), corresponding to diving at the depth of 60 m. The decompression was carried out in accordance with the tables of the Polish Navy. The evaluation of the gene expression was performed using the RT-PCR method, with sets RT2 PCR Profiler Array (Superarray Bioscience, a Qiagen Company, USA) and CFX cycler Bio-Rad 96. The studies of gene expression and the analysis of results showed a decreased expression of genes BCL2, BCL2A1, ICAM-1 and PECAM-1. Further studies are recommended to determine whether the decrease in the expression of genes including PECAM-1, ICAM-1, BCL2, and BCL2A1 may contribute to the increase in the risk of pathological bone changes after decompression.
