Opis:
Introduction: The liver is involved in the metabolism of xenobiotics and their metabolites and it is vulnerable to oxidative damage. Hyperglycaemia is highly implicated in the progression of diabetes mellitus, and adversely affects the liver. Though, conventional hypoglycaemic drugs may be effective in reducing blood glucose, they do not appear to be effective in attenuating the progression of diabetes and its complications. Objective: This study evaluated the ameliorative effects of Anchomanes difformis on hyperglycaemia and hepatic injuries in type 2 diabetes. Methods: Type 2 diabetes was induced in male Wistar rats with a single intraperitoneal injection of streptozotocin (40 mg/kg BW) after two weeks of fructose (10%) administration. Aqueous extract of A. difformis (200 and 400 mg/kg BW) and glibenclamide (5 mg/kg BW) were administered orally for six weeks. Blood glucose concentrations were measured. Serum levels of liver dysfunction markers (ALT, AST, and ALP), total cholesterol, triglycerides, HDL- and LDL-cholesterol were investigated. Total protein, albumin, and globulin were also assessed. Antioxidant parameters: ORAC, GSH, GSSG, SOD, CAT and FRAP were evaluated in the liver while ORAC, FRAP and lipid peroxidation were determined in the serum. Histological examination of the liver tissue was carried out. Results: Treatment with aqueous extract of A. difformis significantly (p<0.05) reduced blood glucose and reversed steatosis in the diabetic-treated rats. The antioxidant status of diabetic-treated rats was significantly (p<0.05) improved. Serum levels of liver dysfunction markers were significantly (p<0.05) reduced in diabetic-treated rats. Conclusion: The findings in this study revealed that 400 mg/kgBW Anchomanes difformis was more effective than 200 mg/kg BW in ameliorating diabetes-induced hepatopathy, however, both doses of Anchomanes difformis demonstrated more antidiabetic ability than glibenclamide. Anchomanes difformis may be a novel and potential therapeutic agent in the management of diabetes and resulted hepatic injuries.

Objectives: The roles of cardiorespiratory fitness (CF) in reducing cardiovascular disease (CVD) and allcause mortality risks are well established; however, little is known about the role of CF in reducing risk of metabolic syndrome (MetS), a cluster of CVD risk factors, particularly in Asian countries. This research examined associations between CF and MetS diagnosis and its five components in Indonesian middle-aged and older adults. Methods: This cross-sectional study included 161 participants (aged 63±8 years; 70% female). CF was assessed with a 6-minute walk test. MetS diagnosis and its components were assessed with the Adult Treatment Panel III. Logistic regression modelling was conducted to examine the relationships between CF and MetS diagnosis and its components, after adjustment for BMI and other confounders. BMI was categorized using cut-points for Asians. Results: In adjusted models, participants with low CF had a greater likelihood of being diagnosed with MetS than those with high CF (OR=4.79, 95%CI:2.17-10.62). They were also more likely to have low highdensity lipoprotein (OR=2.07, 95%CI:1.02-4.18) or hypertriglyceridemia (OR=2.37, 95%CI:1.15-4.86). There was also borderline significant findings that suggested that participants with low CF had greater likelihood of having abdominal obesity (OR=2.34, 95%CI:0.97-5.65, p=0.06) or hyperglycaemia (OR=2.07 95%CI:0.98-4.41, p=0.06). Conclusions: Low CF is associated with increased likelihood of being diagnosed with MetS. The adverse effects of low CF are mainly characterized by dyslipidaemia. Public health messages should emphasise the importance of improving CF for preventing MetS. Assessment of CF could be useful for targeting individuals most likely to benefit from intervention to prevent MetS.

Objective: To study the glycaemic profile of patients with severe malaria (SM). Methods: For this purpose, 110 SM patients were recruited. Pre-treatment random blood glucose and plasma insulin were measured in a subset of donors. An ex-vivo experiment was developed for estimation of glucose consumption by parasitized erythrocytes. Results: Hyperglycaemia was frequent in SM but more commonly associated with cerebral malaria (CM), while hyperinsulinaemia was recognized in severe-malarial-hypotension (median, 25 %-75 %, 188.2, 93.8-336.8 pmol/L). The plasma insulin level was positively correlated with age (CC = 0.457, p < 0.001) and negatively with parasitaemia (CC = -0.368, p = 0.045). Importantly, fatal-CM was associated with hyperglycaemia (12.22, 6.5-14.6 mmol/L), hyperinsulinaemia (141.0, 54.0-186.8 pmol/L) and elevated homeostasis model assessment (HOMA) values. However, there was a trend of higher glucose consumption by parasites in CM compared with that in uncomplicated malaria (UM). Conclusion: Hyperglycaemia, hyperinsulinaemia and elevated HOMA are evidence for insulin resistance and possibly pancreatic B-cell dysfunction in fatal-CM.

Końcowym celem badań, których fragment zaprezentowano w niniejszej pracy jest opracowanie inteligentnego algorytmu pozwalającego na automatyczne sterowanie osobistą pompą insulinową na podstawie sygnałów otrzymywanych z urządzenia do ciągłego pomiaru stężenia glukozy. Wstępne badania przeprowadzone zostały w formule in silico czyli przy użyciu modelu komputerowego odzwierciedlającego funkcjonowanie żywego organizmu. W rozpatrywanym przypadku zastosowano model opracowany przez zespół dr. Boris Kovatchev'a z University of Virginia, USA. Analizowano przebiegi odpowiedzi modelu - krzywe cukrowe BG, na zmieniane wymuszenia w postaci zróżnicowanych posiłków i dawek insuliny. W efekcie stwierdzono silne zindywidualizowanie odpowiedzi a ponadto konkretny jakościowy wpływ czasu posiłku i wielkości dawki insuliny (w tym także podziału tej dawki na części), na możliwość utrzymania poziomu glukozy w przedziale pożądanym - pomiędzy 70mg/dl a 180 mg/dl, umożliwiające uniknięcie występowania groźnych dla pacjenta stanów hipoglikemi i hiperglikemi. Zakładamy, że otrzymane rezultaty pomogą przy późniejszym wdrażaniu gotowego systemu sterowania pompy insulinowej kontrolującego poziom stężenia glukozy we krwi chorego.
The material presented in this paper describes one of the steps in design of our control algorithm for Closed Loop Insulin Delivery. Such a system would consist of Insulin Pump with Continuous Glucose Monitoring Device and Control Algorithm. The presented work gives information about the qualitative influence of treatment parameters on blood glucose (BG) traces. The study utilises computer model tests (in-silico) for obtaining the BG level information. The great advantage of such computer tests is possi-bility to conduct several similar examinations leading to the family of traces that are different by a single parameter, while the other parameters are constant. The used computer simulator was delivered by the University of Virginia [3, 4]. Fig. 1 shows interspecies differences for subjects, which proves the need for control algorithm individualisation. Fig. 2 presents the impact of the bolus delivery time regarding the meal delivery time for an adult subject. Delivering the insulin in advance to meal reduces the BG. In Fig. 3 the influence of the meal duration time on the BG response is shown. Extending the meal duration slightly lowers the BG, but does not influence noticeably the fall ratio. Fig. 4 depicts the changes due to the bolus value, a higher bolus causes the lower BG and faster fall rate of change. Fig. 5 illustrates the test result of bolus distribution into 3 doses delivered within some period. Scheme 10/20/70 every 60min means that a single bolus is split into 10%, then 20% and 70% doses within the period of 60 minutes. There can be noted the response similarity between 10/20/70 every 15min and 70/20/10 every 60min, in spite of the fact that the drug delivery schemes are significantly different . This proves the possibility of long term BG regulation. None of the above tests show noticeable changes of the BG rise ratio, thus it cannot be easily controlled by insulin or meal delivery. The main purpose of the control algorithm isto avoid harmful extreme values of the BG. The presented study allows the better algorithm planning because the influence of a different type of meal and insulin scenario on the BG trace is known.

Opis:
There has been an enormous interest in the development of alternative medicines for type 2 diabetes, specifically screening for phytochemicals with the ability to delay or prevent glucose absorption. The goal of the present study was to provide in vitro evidence for potential inhibition of α-glucosidase and α-amylase enzymes, followed by a confirmatory in vivo study on rats to generate a stronger biochemical rationale for further studies on the ethanolic extract of Andrographis paniculata and andrographolide. The extract showed appreciable α-glucosidase inhibitory effect in a concentration-dependent manner (IC50=17.2±0.15 mg/ml) and a weak α-amylase inhibitory activity (IC50=50.9±0.17 mg/ml). Andrographolide demonstrated a similar (IC50=11.0±0.28 mg/ml) α-glucosidase and α-amylase inhibitory activity (IC50=11.3±0.29 mg/ml). The positive in vitro enzyme inhibition tests paved way for confirmatory in vivo studies. The in vivo studies demonstrated that A. paniculata extract significantly (P<0.05) reduced peak blood glucose and area under curve in diabetic rats when challenged with oral administration of starch and sucrose. Further, andrographolide also caused a significant (P<0.05) reduction in peak blood glucose and area under the curve in diabetic rats. Hence α-glucosidase inhibition may possibly be one of the mechanisms for the A. paniculata extract to exert antidiabetic activity and indicates that AP extract can be considered as a potential candidate for the management of type 2 diabetes mellitus.
