Subject and purpose of the work: The commercial success of an organisation is based on its unique characteristics which make it stand out from its competitors. It can be the result of as many factors as there are many types of organisations and the goals they pursue. However, there are some invariants among them, which include leadership and management style associated with it. The purpose of this article is to demonstrate the possible ways of implementing the idea of a turquoise management model in organisations with strong knowledge-based leadership. Materials and methods: The subject of the study were medical entities of the SMEs group. A faceto-face and telephone interview with a person holding a leadership role in the treatment unit was chosen as the research method. Results and conclusions: An attempt was made to verify the thesis of the possible synergies between turquoise ideas and strong leadership. The paper demonstrates a natural evolution of the leadership model which has resulted from the generation Y and Z entering the labor market, which is characterised by features converging with the turquoise idea of management

Opis:
The main aim of the paper is to reveal the outcomes of a research based on the efficiency of primary health care providers. The scientific goal of the mentioned research was the development of an efficiency measurement model and verification of its usefulness in practice. Overall, the research found that it is possible to use the efficiency measurement model for health care providers. Besides, significant differences were discovered in the efficiency of public and nonpublic primary health care providers. The research was conducted in the West Pomeranian Voivodship in Poland. This paper contributes to the widespread debate on public and nonpublic ownership in the field of healthcare. Also, it has practical implications as the research findings may be useful for any healthcare sector stakeholder, from decision makers to patients. The research was based on the literature overview, which allowed to elaborate the efficiency measurement model. The empirical research (based on a form of questionnaires) allowed testing the proposed model. The described efforts allowed drawing conclusions on the efficiency of primary health care institutions in the West Pomeranian Voivodship. The following methods of data analysis are presented in the paper: synthetic measure of development (SMD), Ward’s method, and k-means method. According to the main conclusion of the research, it is possible to measure the efficiency of public and nonpublic health care providers of the Polish healthcare system. The proposed model for measuring the socioeconomic efficiency may be used as one of the tools used to measure the efficiency in the primary care. The verification of the usefulness of the model showed that nonpublic health care providers operating in the field of the public sector, outperformed public providers. The paper contributes to the theoretical field as it reveals a comprehensive approach to the efficiency measurement in the health care sector. The efficiency measurement model is based on the three major pillars of the healthcare sector, namely, income/resources, cost, and the social aspect. The elaborated efficiency measurement model for the healthcare sector was implemented and tested on a group of primary health care providers in the West Pomeranian Voivodship. The research allowed for positive conclusions regarding its usefulness in practice.

Opis:
The aim of this research is to understand how patients’ characteristics increase healthcare providers’ perceived workload. Patients’ characteristics and dependency, technical and relational complexities of care seem to increase healthcare providers’ workload. As workload is multidimensional, we examine which dimensions are affected by patients’ characteristics. Our methodology is based on 121 patients assessed with the NASA task load index (NASA-TLX) and a questionnaire filled in by 57 health providers in 2 emergency wards in French hospital settings, to evaluate their attitudes to different patients’ characteristics. Our results show that physical demand is the dimension most affected by patients’ behaviour and characteristics. Next, we observe that workload increases more due to patients’ behaviour than their social characteristics. We propose that a regulation mechanism be taken into account in further research, using methodology based on observations to identify how healthcare providers might adapt their activities to compensate for workload variations caused by patients.

Opis:
The aim of the paper is to reconsider Habermas’ discourse approach in terms of its usefulness in the realm of public healthcare where, on a microscale, intersubjective communicative situations arise between defined participants, i.e., patients and healthcare providers, patients’ family members, and further eligible contributors to patient-related decision making. A need for more “communicative interaction,” and explicative and practical discourse, is illustrated by two empirical examples of medical decision making which reveal both communicative and discursive deficits (Section I). To empower and enable the patient as a rational and autonomous speaker and discourse participant, a Habermasian emancipatory argument and ‘the power of the better argument’ is recalled (Section III). The possibility of equal communicative and discursive rights in the light of real inequalities is discussed in the context of a ‘competence gap’ between participants (Section IV). Further sections focus on the importance of informed consent on the side of the patient and the communicative competences as an important factor of healthcare system.

Opis:
This article has investigated the considerations of healthcare facilities to utilize reusable respirators as an alternative to disposable respirators during the COVID-19 pandemic. The decision to choose specific equipment should be based on the protection factors and also on the overall analysis of given conditions. International scientific databases, such as Web of Science, PubMed and MedLine, were searched on May 5, 2020, with the following key words: COVID-19, respiratory protection, surgical masks, filtering facepiece respirators (FFRs) and disposable respirators. The differences between various respiratory protective equipment, i.e., surgical masks, respirators such as FFRs, elastomeric half-facepiece respirators, elastomeric full-facepiece respirators and powered air-purifying respirators (PAPRs), were compared. Reusable elastomeric respirators (RERs) may provide a better adaptation to the face and may be more stable when used by healthcare providers (HCPs). Protection factors were found to be higher in FFRs compared to surgical masks. While FFRs provide a one-tenth decrease in the inhaled aerosol concentration, PAPRs diminish the inhaled aerosol up to one-twenty-fifth. Even with some full-face PAPRs and helmets, the protection factor assigned by the Occupational Safety and Health Administration can reach a value up to 1000. For HCPs, the evidence shown in this article provides an additional support for the utilization of RERs. Such equipment might be less prone to leakages, can provide a better fit, and indicates a better stability compared to disposable FFRs (N95 and similar). By providing higher protection factors, reusable elastomeric respirators are recommended to be used by HCPs under controlled cleaning and disinfection protocols.

Opis:
The aim of this study is to identify the legal characteristics of contracts for the health care services provided by a public payer, i.e. the National Health Fund (NFZ) as part of treatment covered by universal health insurance, as compared to those provided by the health care providers with public or private legal status. This issue is discussed in relation to the legal conditions for the treatment of patients on a commercial basis in those institutions (private or public) which have contracts for the provision of healthcare services under the general health insurance (so-called contracts). The discussion is presented based on author’s own observations, resulting both from his scientific studies in the field of medical law and his work in various entities operating in the health care system.

Opis:
Local government units (LGU) are nowadays facing the very difficult and complicated task of making reasonable decisions regarding the transforming of SPZOZs into capital companies. First, it seems necessary to carry out a simulation of costs and advantages of the assumed models and solutions together with an analysis of advantages and disadvantages of the new legal and organisational forms. The aim of this paper is to assess whether the process of transforming SPZOZs into capital companies is purposeful and reasonable, and to define a way to prepare hospitals for functioning in an altered legislative environment. The paper draws attention to the fact that transformation itself does not guarantee that the results achieved by the given entity will automatically improve. The transformation can bring financial advantages for the newly created company and the local government, from the subsidies and remissions in accordance with art. 197 of the act on medical activity. Yet the conditions for getting such help are quite restrictive and not in every situation can financial help from the central budget be counted on. Such aid could help improve the financial standing of a hospital considerably. Also, it must be remembered that a hospital transformed into a capital company acquires the capacity to go bankrupt. If the new entity generates a loss, it may result in the owner having to raise the initial capital in order to avoid filing a bankruptcy petition by the company. In practice, the financial consequences for the local government are the same as in the case of having to cover losses. The difference lies in the continuity of the provided medical services.
