Opis:
Purpose: The paper presents an application of the Theory of Constraints (TOC) aimed at defining and maximising the potential of hyperbaric chamber as a key resource in a specialist hospital. Design/methodology/approach: The authors developed their own TOC based method of process improvement, supported by tools of Lean Management. Various constraints including an external one were taken into consideration. As a follow-up a project using the method was carried out in a hospital’s Hyperbaric Oxygen Therapy laboratory. Findings: The developed and implemented practical solution has improved patients’ access to the service at about 40% per day, shortened patients’ waiting time at about 90% and helped to overcome the threat of financial underperformance. Moreover, in combination with strict safety rules, the solution has proven its high resistance against difficulties caused by the COVID-19 pandemic. Research limitations/implications: A hyperbaric therapy is a narrow branch of medicine, therefore application of the method in other healthcare domains will require an appropriate adaptation. Practical implications: Despite the narrow subject of the study, the described approach is universal and can be used to maximise the capacity utilisation of various critical resources in hospitals and other healthcare providers. Originality/value: This is the first known to the authors application of a TOC/Lean based systematic hybrid approach to improve performance of a healthcare provider, which is taking into account several constraints including an external one. Additionally, this is the first paper on TOC in healthcare published in Poland.

The purpose of this article is to present the results from a survey concerning first-line managers’ assuredness about the effects of Lean after two years of Lean application in a Swedish healthcare organization. The purpose is also to reflect about assuredness as a driving force for sustainable change. Questionnaires were sent to all first-line managers in a healthcare organization in order to investigate how these managers consider their role, ability and conditions to create change according to Lean. One of the questions included 17 statements about how assured these managers were about the effects of Lean. The results from this question will be presented in this paper. The study showed that the majority of the first-line managers in this particular healthcare organization were assured that developmental work supported by Lean contributes to a higher patient focus, supports first-line managers with useful tools and methods, contributes to the development of an improvement culture and that the Lean concept in general is a support in improvement work. The question can either be used separately or as a part of an entire questionnaire in healthcare organizations. Asking first-line managers about their assuredness about the effects of Lean on a regular basis is one way to follow the Lean process from their perspective. The survey question might encourage discussions about the process of Lean and hopefully contribute to a greater understanding about the importance of assuredness and about the desired effects when applying Lean.

Opis:
Modern technologies and technics nowadays play a very important role in optimizing many processes. Healthcare sector is a vital system of every country and it requires great investment and constant improvement, which makes integration with modern technologies and technics inevitable. Efficiency, speed and time savings are thus crucial for achievement of this integration. Lean RFID approach is completely new way of modeling and optimizing healthcare systems. The objective of this paper is to identify, describe and analyze recent trends in merging healthcare (Emergency Department) and Lean and RFID principles into a unique system. Obtained results indicate that the application of certain segments of Lean "thinking" significantly increases the efficiency of focused processes in healthcare. The main idea of the integration of medicine and Lean RFID approach is to constantly create and improve new values and reject all the activities that are categorized as "waste" in order to provide time savings, which are extremely important in this branch.
