A fundamental pre-requisite for the establishment of a scaleable regional health information network (RHIN) is the development of an architectural framework and tools for the integration of specialized autonomous systems and e-health service platforms supported by an underlying health information infrastructure (HII). In this context, HYGEIAnet, which is the RHIN of Crete in Greece, has identified and utilized a number of critical software components enabling integrated access to clinically significant information, based on an open architecture addressing successfully the various interoperability challenges at hand. HYGEIAnet provides the framework for the reuse of standardized common components and public interfaces, thus enabling integrated and personalized delivery of healthcare.

The demand for ubiquitous and efficient information delivery is increasing rapidly, as the majority of access to professional data, information and knowledge is increasingly relying on the use of technology. Mobile workers become more efficient, if equipped with access means similarly powerful to stationary workplaces. All types of work exhibiting inherently nomadic characteristics are even more affected by these developments. Healthcare personnel in a clinical environment are definitely one of the typical examples, where the access of information is vital and bound to location. Additionally the information needs to be processed in very short periods of time. For this purpose it is of great advantage to deploy advanced information visualization technologies in order to communicate larger amounts of data in a shorter period of time. In this work, we present an IT platform, which emerged from applications in the cultural heritage domain, that can be used to deliver context-aware services and advanced visualization of information to medical personnel in a clinical environment. The location combined with usage profiles for each member of the stuff are used to make the decision about the type and amount of information as well as the visualization type delivered to the handheld devices. Along with the description of the platform and its components, two application examples/medical use cases are presented.

The telecommunications industry in last decade went through the dramatic changes motivated by mobility, wireless technologies and miniaturization. The continuous increase in the complexity and the heterogeneity of healthcare telecommunications infrastructures requires reliable methodology to assess the quality of service provision. This article presents a cost effective methodology to assess the user's perception of quality of service provision utilizing the existing Staffordshire University network by adding a component of measurement to the existing model presented by Walker. This paper offers a cost effective approach to assess the QoS provision within the University campus network, which could be easily adapted to any campus network or healthcare organization in the world.

Croatian national health ICT implementation strategy is determined by Croatian national health strategy and plan, Croatian ICT development strategy for 21st century, and requirements specifications for the health information system. National health ICT implementation strategy components are accented: purpose of the ICT implementation strategy, information principles, needs and ICT enablement in domains of patients, healthcare professionals, policymakers and managers and public. Telemedicine and telecare positions and implementation steps are described. Based on the determinants, three organizational levels have been established - government, ministerial and project levels. General architecture of Croatian healthcare information system and respective pilot projects and results of pilot implementations as well as national ICT environmental accelerators for health ICT implementations are presented.

The integration of information systems represents one of the most urgent priorities of a regional healthcare authority in order to meet its clinical, organizational and managerial needs. Current practice shows that the most promising approach to achieve a regional healthcare information system is to use a health level 7 (HL7) message based communication system implemented by an asynchronous common communication infrastructure between healthcare sites. The system is a complete and integrated information system at a regional level that comprises all types of healthcare levels, that includes interoperability issues, that covers most of the needed components, and that is able to work efficiently in a secure wide area network to ensure data privacy and confidentiality. Another important feature of the proposed solution is that it creates an interoperability framework that can be replicated from one healthcare institution to another. In that sense, common interoperability messages can be used to interconnect heterogeneous information systems. In response to this strategy, more than 10 different consortiums have submitted proposals to the Greek government and the proposed interoperability framework seems to be widely accepted as a solution to enhance information and communication technologies developments in the healthcare sector in Greece.

The advantages of the introduction of information and communication technologies (ICT) in the complex healthcare sector are already well known and well stated in the past. It is common knowledge that in order to install any type of information system in healthcare, six main groups of issues have to be dealt with: organizational and cultural matters related to healthcare, technological gap between healthcare professionals and information science experts, legal requirements on the confidentiality of personal data, of patient related data and on data privacy, industrial and market position of healthcare informatics and interoperability complexity, lack of vision and leadership of the health care managers and health authorities and user acceptability and usability of the proposed information systems. In order to meet these issues stated above, a special focus group (Z3) performed an assessment of the situation of healthcare informatics in Greece and of the main key points that would lead to success. In that sense it is now common knowledge that Greece is lagging information and communication technology progress in healthcare because almost none of the above mentioned issues were dealt with. This assessment is the result of the interaction of more than 150 decision makers, medical informaticians, healthcare practitioners and other individual involved in healthcare. As a conclusion, this focus group resulted in 4 major propositions that will lead to healthcare informatics introduction with better success chances: focus on terminologies and standards, focus on interoperability and information systems sustainability, focus on clear goals and system metrics that can create a healthcare performance management cockpit, and focus on people and what they have to say, by creating a e-health forum. These conclusions were taken into consideration by the Greek government and are incorporated the IASYS project, the national healthcare informatics framework for the next ten years.

The primary role of the diet is to provide adequate quantities of nutrients to meet metabolic requirements and maintain optimal health. However, it has been shown that certain types of food or specific food components can affect a variety of body functions and provide specific health benefits. Diet containing such types of food may promote better health and play an important role in disease prevention by reducing the risk of certain chronic diseases. Vegetable proteins, especially legume proteins, have been recognized as factors influencing lipid metabolism. Hypocholesterolemic effects of different legume proteins have been documented. Hypotensive action of bioactive peptides derived from soy and lupin proteins has been reported. In soy a peptide of anti-mitotic activity was found and it may play a significant role in suppression of tumor development and cancer prevention. Diet rich in vegetables and vegetable proteins is related to lower risk of cardiovascular and cancer mortality.

Life in good health and health security prove the most significant values highlighted by moral philosophy in the time of the environmental crisis. The imperfect operation of healthcare poses a threat for humans. Administrative measures regulate insufficiently medicine and healthcare. They need to be backed up by ethics, which cannot be seen solely as ethics of an individual’s conscience. What is needed is professional, practice-oriented and institutionalized-within-healthcare-organizations ethics. Recently, there have appeared a great number of new international documents setting standards of medical procedures in compliance with the requirements of the new bioethical values. At the same time elements of the ethical infrastructure such as bioethics commissions or committees have been created. In the face of the specificity and the complexity of ethical issues and problems encountered within contemporary medical practice the requirement of high ethical competence of all healthcare workers often fails to be sufficient.

The typical hospital and operating theatre present multiple potential hazards to both workers and patients, and protection against some of these is provided through use of various forms of clothing and textiles. While many standards exist for determining the performance of fabrics, most tests are conducted under laboratory conditions and against a single hazard. This paper provides an overview of selected developments in the principal properties of fabrics and garments for use in these workplaces, identifies the key standards, and suggests topics for further investigation.

Opis:
W artykule przedstawiono i zanalizowano problem zarządzania zasobami w służbie zdrowia. Omówiono podstawowe zagadnienia w procesie alokacji zasobów medycznych. Zaprezentowano przykłady zastosowania wybranych metod: programowania matematycznego (liniowego, całkowitoliczbowego, celowego, sieciowego), teorii kolejek, modelowania symulacyjnego, systemów hybrydowych oraz standardowych metod kosztowo-efektywnościowych do analizy zagadnień, związanych z podziałem zasobów w systemach opieki zdrowotnej. Przedyskutowano użyteczność tych metod do rozwiązywania szczegółowych problemów dotyczących zarządzania zasobami medycznymi.
Various issues referring to general problems of resource allocation in the area of healthcare were discussed in the article. Fundamental questions regarding the process of allocating healthcare resources were pointed out. The topics were presented according to the categories of methods used to potentially solve these problems. The following approaches were considered: chosen methods of mathematical programming, queuing theory, simulation, hybrid algorithms combining, among others, artificial intelligence and other techniques, as well as standard cost-effectiveness methods. Such methods were applied to the following problems: analyzing the current state of health care units and presenting proposals of changes to such systems. The modifications suggested referred to: allocation of material resources, planning the work schedules of medical personnel and patient admission. The question of determining the effectiveness of various medical strategies was also discussed. The article clearly underlined the importance and complexity of the problem of managing healthcare resources.

This paper presents an application of mixed integer programming model for optimal allocation of workers among supporting services in a hospital. The services include logistics, inventory management, financial management, operations management, medical analysis, etc. The optimality criterion of the problem is to minimize operational costs of supporting services subject to some specific constraints. The constraints represent specific conditions for resource allocation in a hospital. The overall problem is formulated as a triple-objective assignment model, where the decision variables represent the assignment of people to various jobs. A reference point approach with the Chebyshev metric is applied for the problem solution. The results of computational experiments modeled on a real data from a hospital in Lesser Poland are reported.

Opis:
The development of advanced spatial-positioning systems for medical monitoring and emergency response using RFID and Wi-Fi technologies is a current research focus of many healthcare organizations. The goal of such system is to support real-time location tracking of medical equipments and personnel in hospital settings using RTLS technology. This paper presents a spatial-temporal data analysis based on data collected at the W21C of Foothills Hospital, Calgary, Canada. The collected data, comprising real-time observations and tracking paths, has been used to perform advanced statistical pattern analysis and visualization of work-flow patterns in health care system. From this analysis, we bring forward some insights into medical resource, asset and facility utilization in a clinical care ward of a regional healthcare organization. This gained knowledge could be used as one of the inputs in improving patient care, responsiveness, and infection control practices.

Healthcare systems managing Electronic Health Records (EHR) are being implemented around the world to facilitate access to patients' health data. The primary idea of the EHR is to gather all health information about a subject of care in one place. When a countrywide EHR system is fully implemented, it is assumed that all newly created documentation is stored in the system. Distributed healthcare systems, which connect national or regional healthcare system, are necessary to allow access to patient' health data in any of connected nationwide healthcare systems. In such complex systems, due to high commercial value and sensitiveness of medical data, security is a very important issue. The paper presents a solution for maintaining evidence records, which can ensure a non-repudiation of EHR content, stored in many nationwide systems. In other words, the verifier using the registry evidence records can proof the non-repudiation of EHR content, which is stored outside a patient home healthcare system.
Systemy ochrony zdrowia zarządzające elektronicznymi dokumentami zdrowotnymi (EHR) są wdrażane na całym świecie w celu ułatwienia dostępu do danych o zdrowiu pacjentów. Podstawową ideą EHR jest zebranie wszystkich informacji medycznych dotyczących jednego pacjenta w jednym miejscu. Gdy ogólnokrajowy system EHR jest w pełni wdrożony, zakłada się, że wszystkie nowo utworzone dokumenty są przechowywane w systemie. Rozproszone systemy ochrony zdrowia łączące narodowe lub regionalne systemy ochrony zdrowia są konieczne aby umożliwić specjalistom ds. ochrony zdrowia dostęp do wszystkich danych dotyczących jednego pacjenta. W tak skomplikowanych systemach, z uwagi na dużą wrażliwość danych medycznych i ich dużą wartość komercyjną np. na rynku ubezpieczeń, bezpieczeństwo jest jednym z głównych priorytetów. W artykule przedstawiono rozwiązanie pozwalające na zarządzanie rekordami poświadczeń rejestrów, za pomocą których można wykazać niezaprzeczalność zawartości EHR, przechowywanego w wielu ogólnokrajowych systemach EHR. Zewnętrzny weryfikator używając rekordów poświadczeń rejestrów może wykazać, że zawartość EHR jest oryginalna i niezmodyfikowana.
