The purpose of the article. The study brings a comparative analysis between health systems in Poland and Italy. It is aimed at fulfilling the subject literature using economic comparative analyses between different health systems as well as straight comparisons between Polish and Italian health systems. Moreover, another aim of the study is to find out some weak points and to point out some good practices of each of the analyzed health systems. The research question for the purpose of this study is as follows: what changes can be implemented to improve the efficiency of each of the analyzed health systems? Methodology. The study is carried out on the background of health systems' theory. The critical literature review is conducted. A comparative analysis using such indicators as percentage of GDP and GDP per capita spent on financing health systems, healthcare spending components, life-expectancy data or Euro Health Consumer Index indicators are applied and analysed in the study. Results of the research. Health systems in Poland and Italy in the latest decades were transformed in a completely different way. Healthcare in Poland is based mainly on health insurance premiums whereas in Italy financing of healthcare is based mainly on taxes. Among similarities between the systems a high level of responsibility designated to local authorities may be mentioned. The comparative analysis indicates that the situation of the Italian health system seems to be much better as compared to its Polish counterpart. Though, some solutions, aimed at improving health system efficiency, can be transferred from one system to another in case of both analyzed systems.

Introduction: Authors analyze aspects of social stratification proposed by Guy Standing with respect to key medical professions performing work in Polish publicly funded medical entities. Purpose: The aim of the paper is to assess how health care providers can be assigned to particular classes and if the precarity phenomenon occurs in their work environment. Materials and methods: An overview of statistical data was made on how health care providers performed their work in years 2005- 2014 and the pay rates in 2014. Results: The vast majority of medical staff employed on the basis of civil law contracts were doctors. A smaller number of civil law contracts has been concluded by nurses and midwives. The number of nurses performing work on this kind of contracts has increased considerably since 2005 and in 2014 there were 10.27% of them. A senior nurse earned PLN 2,600.00. It was very small in comparision with average gross remuneration in the national economy in 2014 amounted to PLN 3,783.46. Unemployment existed in population of nurses was 2.3–2.8%. It is called tyhe natural unemployment. Conclusions: It was found that precarity phenomena occur in the Polish healthcare system although none of the professions met all the criteria attributed to the precariat.

INTRODUCTION: Poland’s healthcare systems is based on insurance and universal and equal access. The first component of the system is primary healthcare. At night and on holidays, the medical help can be obtained by means of After Hours Medical Services. Private paid visits can also be arranged. In emergencies, and cases of risk to health or life, emergency medical team can be called at 112 or 999, or a patient can go to the hospital emergency department (ED) on their own. There are also pharmacies where selected over-the-counter medicines can be bought. MATERIALS AND METHODS: The study was conducted in the first quarter of 2023 using a proprietary questionnaire with 5 sociodemographic questions and 20 case descriptions for which the participants indicated the type of medical help which they would use. The expert panel prepared a set of cases (accidents and illnesses), taking into account patient’s age and gender, time of day and working days and holidays when the incident occurred. The group of participants was selected using the snowball sampling method. The results were subjected to statistical analysis using nonparametric tests, the Spearman’s rho test and the Wilcoxon test. The used significance level was p<0.05. RESULTS: The study comprised 233 participants (166 females and 66 males). The mean age was 25 years (SD ± 9.50). A significant correlation was showed between the decision made and the gender in case of stroke occurring on a weekday (p=0.042); stroke symptoms on a holiday (p=0.024); head injury in a child (p=0,024); chest pain in an adult (p=0.009), and also the place of residence in case of an infection in a child (p=0.008). The decision was also influenced by education in case of chest pain in an adult (p=0.048); stroke (p=0.046) and sudden weakness (p=0.009). Also performing a medical profession correlated with the decision in case of arterial hypertension (p=0.028) and chest pain (p=0.010) in adults. The type of day (working/holiday) and the time of day were also analysed, revealing statistical differences depending on the type and time of day for infection in a child (p<0.000 in each comparison); working day and holiday for abdominal pain (p=0.000), stroke (p=0.023), diarrhea (p<0.000) and weakness (p<0.000), and also depending on the time of day in arterial hypertension (p<0.000); however, no differences were found in case of chest pain depending on the time of day(p=0.073). CONCLUSIONS: Emergency medical teams are called both in urgent cases, such as stroke or chest pain, and also in cases that do not require an immediate responses, such as diarrhea or weakness. Solutions need to be sought which would provide information what healthcare system units should be used in specific situations. The EMTs are called more often during the day on weekdays than at night or on holidays. The ambulance service responds most often in emergency cases such as convulsions, stroke or chest pain, but a significant number of calls is related to cases of weakness or arterial hypertension where the medical help should be sought in other healthcare units.

Influenza is an acute respiratory disease caused by the influenza virus which often occurs in outbreaks and epidemics worldwide. The World Health Organization recommends annual vaccination of healthcare workers (HCWs) against influenza, because most of them are involved in the direct care of patients with a high risk of influenza-related complications. Given the significance of the disease burden, a targeted literature review was conducted to assess issues related to influenza vaccination among HCWs. The primary aim of this review was to assess the incidence of influenza among medical personnel and healthcare-associated influenza, and to outline the benefits of influenza vaccination for patients and HCWs themselves. Vaccination of HCWs seems to be an important strategy for reducing the transmission of influenza from healthcare personnel to their patients and, therefore, for reducing patient morbidity and mortality, increasing patient safety, and reducing work absenteeism among HCWs. The benefits of influenza vaccination for their patients and for HCWs themselves are addressed in literature, but the evidence is mixed and often of low-quality.

This study explores an alternative healthcare innovation project in its making using ethnographic research methods. The project is a confined space—a living lab—that cannot fully be described or explained in the same way we normally understand set-ups for healthcare innovation. By creating its own space, in the intersection between formal and informal structures, it draws our attention to a new way of organizing healthcare innovation. Taking an ethnographic research approach, it is suggested how a concept of a bubble can be used to describe the nature of the living lab as a partial and flexible object that constitutes multiple future possibilities. The concept of the bubble challenges the notion of the living lab as a cheese bell, which is the term used by the field participants, inspired by Clayton Christensen. Bringing in theoretical points from Bruno Latour regarding laboratories, this study explores the materiality of the laboratory and its political nature. The study contributes to the debate on innovation in healthcare and especially fuses to the discussion of how to organize healthcare innovation. It argues that we need to pay attention to new kinds of living labs—like the one introduced in this study.

Implementation of health protection requires effective quantitative methods of its evaluation. Assessment could be based on usage of synthetic indices which aggregate couple input variables into a single measure. In this paper, the exploitation of a new synthetic index (by the author called HAI-the Healthcare Aggregated Index) was proposed with the aim of the assessment and long-term interstate comparisons of healthcare systems of the EU countries. Using taxonomic methodology, HAI involves three variables: the number of hospital beds, the number of physicians and the public expenditures on healthcare. HAI utilisation includes dynamic interstate comparisons of national healthcare systems of the different exploitations of human, physical and financial resources. The HAI application to assessment of twenty European Union Member States' healthcare systems revealed an effect of substitution between health care resources within the slight international diefferntiation of the health protection level and the minor dynamic of changes in time series.

Croatian national health ICT implementation strategy is determined by Croatian national health strategy and plan, Croatian ICT development strategy for 21st century, and requirements specifications for the health information system. National health ICT implementation strategy components are accented: purpose of the ICT implementation strategy, information principles, needs and ICT enablement in domains of patients, healthcare professionals, policymakers and managers and public. Telemedicine and telecare positions and implementation steps are described. Based on the determinants, three organizational levels have been established - government, ministerial and project levels. General architecture of Croatian healthcare information system and respective pilot projects and results of pilot implementations as well as national ICT environmental accelerators for health ICT implementations are presented.

The article analyses damage claims for medical malpractice in Poland. Thearticle describes types of responsibility for medical malpractice. Although availabledata have been analyzed, the evidence suggests that official statistical dataunderestimates the number of claims. The reason for such underestimation layswith existing barriers that discourage patients from claiming the damages. Eventhough new methods of claiming damages were introduced, the barriers remainin place. We present analysis of admission/discharge data from insurance provider for Saxony and Thuringia (Germany) for years 2010-2016. Study of such data is necessary to derive a structure of healthcare system transfer network, as no patients' transfer data are available. Hospital network is a basis for simulation of multidrug-resistant bacteriae spread allowing to study the effectiveness of disease-control strategies. In this paper, the properties of the dataset under consideration are presented and discussed. Moreover, the resulting interhospital network structure is analyzed.

Transformations of independent public healthcare centres (SPZOZ) into limited companies was supposed to improve the effectiveness of the healthcare system. Until the 30th April 2014, 174 entities were subject to organisational and legal transformations, while in the years to come the number of entities that operate as companies is going to increase for sure. Considering the importance of the transformation process to the condition of medical entities, NIK has carried out the audit entitled “Activities of self-governmental hospitals transformed into limited companies”. The objective of the audit was to evaluate the impact of transformations on the number and scope of health services financed from the public funds (by the National Health Fund – NFZ), and on the financial situation of these hospitals in the years 2011-2014.

Background: Despite a growing body of research literature focused on public-private partnerships (PPPs) in healthcare, some critical issues still seem to be insufficiently investigated, especially due to the frequent lack of clarity in defining the goals for healthcare management and to an oversimplified framework for analysing PPPs. Resolving conf licts of interest between diverse PPP stakeholders in healthcare requires special attention. Effective solutions in this area are not well known in Central and Eastern Europe countries, which are trying to modernise their healthcare systems. Aims: The purpose of this article is threefold. Firstly, it is to establish an appropriate understanding of healthcare sustainability, currently understood as the ultimate goal for healthcare management. Secondly, it is to shift the context for analysing the performance of PPPs from a purely financial to a wider and well-framed one, comprising the pillars of healthcare sustainability. Thirdly, it is to identify the governance mechanisms intended to improve the impact of PPPs on healthcare sustainability. Methods: The methods employed include a broad conceptual analysis of the international literature as well as external online desk research of the materials published by PPP consortia, financial institutions and public authorities engaged in managing PPPs for healthcare in Europe. Results: Healthcare sustainability is a complex, multifaceted and multi-pillar problem. PPPs can enhance, or damage, all healthcare sustainability pillars due to a complex conf lict of interest between the parties involved. Embedding certain governance mechanisms in PPP contracting and management is necessary to foster both financial and nonfinancial sustainability in healthcare provided via PPPs. Some countries and some PPPs have managed to develop suitable mechanisms to govern the conf licting goals in healthcare management. Conclusions: The mechanisms governing PPPs can be programmed as sustainability drivers to improve the resilience of healthcare to the benefit of all stakeholder groups, including capital providers, society and the government. The experiences of PPPs in healthcare operating in the European context may inspire an appropriate design of the PPP framework and PPP contracts.
Niejasne definiowanie celów w zarządzaniu ochroną zdrowia oraz zbyt uproszczone podejście do analizy projektów w formule partnerstwa publiczno-prywatnego (dalej: PPP) utrudniają formułowanie mechanizmów zapobiegania i rozwiązywania konfliktów interesów w ramach PPP w sektorze ochrony zdrowia. Efektywne rozwiązania w tym zakresie są słabo znane w krajach Europy Środkowo-Wschodniej. Celem tego artykułu jest w pierwszej kolejności ustalenie poprawnego rozumienia zrównoważonego systemu ochrony zdrowia, następnie przesunięcie kontekstu analizy PPP z perspektywy czysto finansowej w kierunku poprawnie określonej perspektywy filarów zrównoważonego rozwoju ochrony zdrowia i ostatecznie zidentyfikowanie mechanizmów zarządzania PPP wspierających zrównoważoną ochronę zdrowia. Zastosowanymi metodami badawczymi są: szeroka analiza koncepcyjna literatury światowej oraz zewnętrzna, internetowa, analiza danych zastanych publikowanych przez konsorcja PPP, instytucje finansowe i agencje publiczne zaangażowane w zarządzanie PPP w ochronie zdrowia w Europie. Określono, że zrównoważona ochrona zdrowia jest problemem złożonym i wieloobszarowym. PPP mogą wspierać albo hamować rozwój każdego z obszarów zrównoważonej ochrony zdrowia. Jakość oddziaływania PPP na zrównoważoną ochronę zdrowia wynika ze złożonych konfliktów interesów. Wykorzystanie określonych mechanizmów zarządzania w kontraktowaniu i zarządzaniu PPP jest niezbędne do wspierania finansowego i pozafinansowego obszarów zrównoważonej ochrony zdrowia. Bez tych mechanizmów niemożliwe jest zapewnienie trwałości ochrony zdrowia z korzyścią dla dawców kapitału, społeczeństwa i sektora publicznego. Doświadczenie zdobyte w ramach niektórych projektów PPP w ochronie zdrowia inspiruje projektowanie systemu mechanizmów zarządczych.

Aim: The increasing staff shortages in Polish surgery incline towards posing the following questions: What are the reasons for the reluctance of Polish doctors in training and residents to specialize in this field of medicine? What measures could be pursued to raise the interest of students and trainees in specializing in general surgery? The aim of this paper is to attempt to answer these questions. Material and methods: From August 5 to December 30, 2019, an online survey of doctors in training and resident doctors was conducted. The study covered 2,498 people. Results: The key reason indicated by young doctors of both sexes was the disproportion between the level of remuneration and professional responsibility. Other most commonly reported reasons include: high overtime, physical and mental strain at work, and fear of not being allowed by senior colleagues to perform surgery. Conclusions: In view of the aging Polish surgical staff and a decreasing number of young doctors interested in this field of medicine, it is necessary to introduce changes into their mode of education.

The subjects were 134 final-year students of Warsaw Medical University, 1st Faculty of Medicine. The purpose of the study was to measure the dominant coping strategy, moderated by positive and negative emotions, which can be important in management. The variance analysis showed that activation of negative emotions resulted in a significant rise in emotion-oriented and avoidance-oriented coping. At the same time, the level of a task-oriented strategy decreased significantly, which seems to be very important in healthcare management.

Healthcare organizations play an important role in every society. Those organizations are run connecting two conflicted fields: economics and ethics, facing a problem of what is more important: economic accounts or human's life. This perspective makes organizational culture in healthcare institutions seem much more important. Ethical codex, values system, norms of behavior and, of course, management methods - all those factors shape doctor-patient relationships and, as a result, influence human health and life. Thus, creating the organizational culture maximizing the medical staff effectiveness of lifesaving, is of interest to every society. In this paper, the author tried to answer two essential questions - what kind of organizational culture would be the most adequate for healthcare organizations and how to diagnose the organizational culture - measure something immeasurable.

The first proposed amendment concerns the elimination of Article 120 of the Misdemeanours Code, what is justified by the fact that the concept of arms, ambition and explosive materials and devices does not encompass trees and coarse woody debris. The author justifies the necessity of that amendment analyzing inadequate regulations included in the provisions in their current shape. The second suggested amendment concerns a former replacement of the term “Health Maintenance Organisation” with the term “Healthcare Entity”. As a result of his modification it became more difficult to define the scope of criminalisation of acts under Article 147a of the Misdemeanours Code., i.e. running a healthcare maintenance organisation without required entry into a register or obligatory registration. The scope of both terms raises considerable doubts. In the past it was suggested that Article 147a of the Misdemeanours Codem should concern treatment performed by both doctors and nurses. It is necessary to amend the discussed provision in order to effectively define the activity which it concerns.

W opracowaniu przeprowadzono analizę skutków ekonomicznych i prawnych przekształceń własnościowych podmiotów leczniczych, wynikających z zapisów ustawy z dnia 15.04.2011 r. o działalności leczniczej [DzU 2013.217], która weszła w życie w dniu 1 lipca 2011 r. oraz podjęto próbę ich oceny w aspekcie sytuacji finansowej jednostek ochrony zdrowia, gospo-darki i dyscypliny finansowej samorządu województwa podkarpackiego oraz odpowiedzialności społecznej. Badania dowiodły, że proces przekształceń podmiotów leczniczych skutkuje licznymi konse-kwencjami, a prawdopodobne odejście od formy samodzielnego publicznego zakładu opieki zdrowotnej, dominującego na dzień dzisiejszy, na rzecz spółek kapitałowych, jest ogromnym wyzwaniem i może okazać się dla podmiotów leczniczych, jednostek samorządu terytorialnego oraz społeczeństwa zarówno szansą, jak i zagrożeniem.
Analysis of the effects of economic and legal privatization of the healthcare organizations, arising from 15.04.2011 regulations about healing activity [Journal of Laws 217, 2013] and an attempt of their evaluation undertaken in the aspect of the financial standing of the healthcare organizations, economic and financial discipline of Podkarpackie voivodeship local government as well as its social responsibility. Studies have shown that the process of transformation of healthcare organizations results in numerous consequences, and the departure from currently dominating forms of individual public healthcare institutions in favor of companies, is an enormous challenge and can turn out for the health entities, local government units as well as the entire community as both a chance and a threat.

Healthcare workers are representatives of occupations that are most exposed to high levels of stress in the work environment. These characteristics of work increase the probability of suffering from mental disorders. One aspect of mental disorder prevention in the workplace is the role of healthcare managers as those people who are responsible for minimizing the negative impact of work-related stress factors. Their role can be performed by creating effective initiatives supporting workers’ mental health. The need to support the implementation of such initiatives has been highlighted by the COVID-19 pandemic. The aim of the review is to summarize available types of managerial interventions in the field of mental health protection of medical staff, considering the assessment of their prevalence, determinants of effectiveness, and limitations from the perspective of healthcare managers. The article was prepared based on the literature review method and covered publications from original research in English and Polish, published until June 2023 in the following databases: PubMed, Google Scholar, and PsycINFO. In addition to the original research, the review also includes documents developed by international health organizations. The determinants of effective managerial interventions that can be used for the needs of managers and decision-makers in the field of mental health management in the workplace have been presented. The greatest widespread of mental health initiatives concerned the pandemic period, but now the key systemic task should be to maintain the frequency of impacts outside the pandemic period, due to the constant nature of stressors. The determinants of their effectiveness include, among others: incorporating elements of psychological knowledge into the process of educating managers, involving healthcare specialists in the development of programs, and examining the needs of the staff each time at the stage preceding interventions.

In healthcare the payer, covering the cost of health services, plays the key role. As a result, we do not observe a regular relation seller–buyer, but rather dependency purchaser–payer. In the paper we analyze costs generated by the way of allocation financial resources in healthcare and their distribution by the payer- so called transactional costs. While optimizing healthcare budget costs of intermediaries need to be taken into consideration.
W obszarze ochrony zdrowia zasadniczą rolę odgrywa płatnik, pokrywający koszty usług zdrowotnych. W rezultacie mamy do czynienia nie z normalną relacją rynkową: sprzedawca–nabywca, lecz z zależnością: nabywca–płatnik. Badamy koszty wynikające ze sposobu gospodarowania środkami w systemie ochrony zdrowia i ich rozdysponowaniem przez płatnika – tzw. koszty transakcyjne. Przy dyscyplinowaniu kosztów trzeba też zwracać uwagę na koszty pośredników.

ObjectivesIncreased life expectancy results in greater challenges posed to healthcare. Concurrently, a shortage of healthcare workforce, including nurses, has been observed. Thus, an urgent need exists to implement improvements in healthcare services based on sufficient evidence. The aim of the study was to evaluate the influence of the relative number of nurses/midwives on life expectancy, and the influence of selected economic variables: gross domestic product (GDP), health expenditure as a percentage of GDP, and health expenditure per capita, on this number. The aim of the study was to evaluate the influence of the relative number of nurses/midwives on life expectancy, and the influence of select economic variables: GDP, health expenditure as a percentage of GDP, and health expenditure per capita on this number.Material and MethodsA retrospective analysis based on data from 46 countries was performed. Correlations between the relative number of nurses/midwives and life expectancy as well as economic variables were evaluated. To trace the differences between the countries with different relative numbers of nurses/midwives, the countries were divided into groups as follows – group 1: <5 nurses and midwives/1000 nurses inhabitants, group 2: 5–10 nurses and midwives/1000 inhabitants, and group 3: >10 nurses and midwives/1000 inhabitants.ResultsCorrelations were found between the relative number of nurses/midwives and life expectancy (p < 0.001, r = 0.68), and economic variables (p < 0.001, r = 0.82; p < 0.001, r = 0.62, and p < 0.001, r = 0.8, respectively). Life expectancy was higher in group 3 vs. groups 1 and 2 (p < 0.001 and p = 0.036, respectively), and in group 2 vs. group 1 (p = 0.006). Economic variables were higher in group 3 vs. group 1 (p < 0.001 for all) and group 2 (p = 0.016, p = 0.025, p = 0.022, respectively), and in group 2 vs. group 1 (p = 002, p = 0.024, p = 0.002, respectively).ConclusionsThe relative number of nurses/midwives correlates with life expectancy and relies on the country’s income and level of healthcare system financing.

Objectives The aim of the study was to analyze the long-term burnout levels of healthcare professionals (HCPs) working in Slovenian nursing homes during the fifth wave of the pandemic; to compare the results of similar facilities in 2020 and 2013; and to examine the correlation between demographics and burnout and fatigue among HCPs. Material and methods The study used a descriptive, correlational cross-sectional method. Results In the fifth wave, HCPs suffered more from emotional exhaustion, depersonalization and lack of personal accomplishment than in the first wave of the pandemic and in the spring of 2013. The HCPs caring for COVID-19 patients and younger women had higher rates of burnout and fatigue than other occupational groups. There is a strong positive correlation between burnout and fatigue. Conclusions There is an urgent need to address the problem of fatigue and burnout with administrative measures.

The aim of this study is to evaluate and compare the financial and managerial efficiency of healthcare inpatient systems in one country. The research question is: What factors give rise to efficiency variations within the regional healthcare system (hospitals)? A two-step approach proposed by Simar and Wilson has been applied. The first step applies the Data Envelopment Analysis to estimate the unit (i.e., a Polish region in our research) efficiency scores, which is followed by truncated regression with double bootstrapping to examine the impact of uncontrolled variables on efficiency scores. The efficiency scores obtained in 16 regions from the output-oriented model with non-increasing returns to scale indicate a full effectiveness of five regions. The DEA scores of the other regions exceed the value of 0.8 and do not differ significantly. The bootstrap-corrected DEA scores do not considerably change the efficiency assessment. The direction of the impact of the ‘percentage of people above 65 years of age’ variable and the ‘growth rate of total liabilities on efficiency’ variable is opposite to that observed for the ‘average salary’ variable. The results obtained suggest that the regional healthcare system efficiency can be affected by the ageing population, pay pressure, and liabilities.

System ochrony zdrowia (OZ) odgrywa szczególną rolę tak dla społeczeństwa, jak gospodarki. Niezależnie od stopnia rozwoju i poziomu wydatków na zdrowie, krajowe systemy jego ochrony borykają się z problemem niedoboru środków. W Polsce owe zjawisko dotyczy także istotnych braków w innych obszarach, w tym przede wszystkim kadrowych. W świetle nieuchronnych i koniecznych zmian stwarza to poważne ryzyko zarówno dla bezpieczeństwa zdrowotnego Polaków, jak i niepowodzenia wdrażanych rozwiązań mających na celu modernizację obecnie działającego systemu. Badanie zrealizowane przez pracowników Szkoły Głównej Handlowej w Warszawie uwydatniło możliwość rozwiązania tego problemu poprzez wykorzystanie i rozwój kompetencji przywódczych liderów OZ. Artykuł stanowi punkt wyjścia do dyskusji o roli kadry zarządzającej w procesie zmian systemowych OZ.
Health plays a special role both for societies and economies. Regardless of the level of development of health systems and respective expenditures, they are struggling with shortage of resources. In case of Poland, this phenomenon also concerns significant deficiencies in other areas, including personnel. In the light of inevitable and necessary changes, this poses a serious risk both to the health security of Poles and successful implementation of modernization plans. Research carried out by employees of the Warsaw School of Economics highlighted the possibility of solving this problem through the use and development of leadership competences. This article is a starting point for a discussion on the role of the leadership in the process of changes in healthcare system.

Healthcare in Poland is mainly financed by public sector entities, among them the National Health Fund (NFZ), state budget and local government budgets. The task of the National Health Fund, as the main payer in the system, is chiefly currently financing the services. The state budget plays a complementary role in the system, and finances selected groups of services, health insurance premiums and investments in healthcare infrastructure. The basic role of the local governments is to ensure access to the services, mostly by performing ownership functions towards healthcare institutions.
