Objectives The aim of this survey was to assess the seroprevalence of antibodies against Coxiella burnetii (C. burnetii) in subjects at risk of exposure in Sicily, Southern Italy. Material and Methods Prevalence of IgG antibodies to C. burnetii phase II antigens was evaluated by ELISA in a group of 140 workers at risk of exposure (38 veterinarians, 38 slaughterhouse workers, 44 livestock handlers, 20 laboratory and technical personnel) included in a medical surveillance program and in 42 control subjects. Positive samples were classified as suggestive of prior exposure to C. burnetii. Results Antibodies against C. burnetii were detected in 88 out of 140 (62.9%) exposed workers and in 6 out of 42 (14.3%) subjects of the control group. The variables evaluated did not seem to have a significant effect on seropositivity to Coxiella with the exception of symptoms in the last 6 months preceding the survey. Conclusions Our study demonstrated a high seroprevalence of C. burnetii in the group of exposed workers in comparison to non-exposed subjects of the control group. Clinical illness appears to be rare; nevertheless, physicians should consider Q fever in patients with compatible symptoms and occupational exposure to animals and their products. As aerosols represent the main route of infection in animals and humans, these workers are strongly advised to wear respiratory masks. In addition, occupational physicians should consider routine serologic evaluation and vaccination of occupationally exposed workers.

Objectives A Workers’ Health Surveillance (WHS) program is an occupational health strategy used to detect and address the health of individual workers to improve their ability to work. This study aims to investigate the feasibility and acceptability of a new job-specific WHS for hospital physicians. Material and Methods All hospital physicians of the general surgery, radiotherapy and obstetrics and gynecology departments from 1 academic hospital were invited to participate in the WHS by the in-company occupational health service. An occupational physician and a medical assistant were trained to use the protocol. Feasibility was operationalized as the received and delivered dose, observed success factors and potential obstacles. Acceptability was assessed by asking whether the WHS was desirable and feasible for future use and by estimating the effects on health and work ability. Written questions and semi-structured interviews were conducted with the participating physicians, 5 department managers and the 2 occupational health professionals involved in the study. Results One-third of the hospital physicians (34%) participated in every part of the WHS. The delivered dose was 77/84 (92%). Almost all hospital physicians who received recommendations expected to adhere to this advice. The study participants appreciated the organization of the WHS. This WHS was positively graded (8 out of 10 max) in terms of acceptability. Positive effects of the WHS on health, work functioning and long-term work ability were perceived by 2/3 of the physicians. Conclusions The new job-specific WHS for hospital physicians showed good feasibility and acceptability among participating hospital physicians, occupational health professionals and medical managers.

The aim of the study was to investigate the occurrence of serum antibodies to C. psittaci in workers at 8 stock farms located in a rural area of eastern Sicily. Serum samples from 188 workers and 160 controls were tested for immunoglobulin IgA, IgG and IgM direct against C. psittaci by microimmunofl uorescent test (MIF). Seropositive subjects were defi ned as those with IgG titres of 1:16-1:256, and seronegative subjects as those whose titre was <1:16. To rule out the presence of cross reactive antibodies, testing was also performed for C. trachomatis and C. pneumoniae. A total of 28 (14.9%) subjects studied met the criteria for seropositivity to C. psittaci. Two of the 188 (1.06%) had an IgG titre of 1:32, 16 (8.51%) an IgG titre of 1: 64, 6 (3.19%) an IgG titre of 1:128 and 4 subjects (2.12 %) demonstrated an antibody titre of 1:256. The high prevalence rate of C. psittaci antibodies among farmers suggests that this infection is spread in those subjects living in areas with an high percentage employed in rearing activities. The authors stress the importance of carrying out health surveillance in subjects working in close contact with animals receptive to infection, and confirm the need to adopt a serological test, such as MIF as a preventive measure for activities at risk.

Objectives Although routine workers’ health examinations are extensively performed worldwide with important resource allocation, few studies have analyzed their quality. The objective of this study has been to analyze the medical practice of workers’ health examinations in Catalonia (Spain) in terms of its occupational preventive aim. Material and Methods A cross-sectional study was carried out by means of an online survey addressed to occupational physicians who were members of the Catalan Society of Safety and Occupational Medicine. The questionnaire included factual questions on how they performed health examinations in their usual practice. The bivariate analysis of the answers was performed by type of occupational health service (external/internal). Results The response rate was 57.9% (N = 168), representing 40.3% of the reference population. A high percentage of occupational physicians had important limitations in their current medical practice, including availability of clinical and exposure information, job-specificity of tests, and early detection and appropriate management of suspected occupational diseases. The situation in external occupational health services – that covered the great majority of Catalan employees – was worse remarkably in regard to knowledge of occupational and nonoccupational sickness absence data, participation in the investigation of occupational injuries and diseases, and accessibility for workers to the occupational health service. Conclusions This study raises serious concerns about the occupational preventive usefulness of these health examinations, and subsequently about our health surveillance system, based primarily on them. Professionals alongside health and safety institutions and stakeholders should promote the rationalization of this system, following the technical criteria of need, relevance, scientific validity and effectiveness, whilst ensuring that its ultimate goal of improving the health and safety of workers in relation to work is fulfilled. Other countries with similar surveillance systems might be encouraged by our results to assess how their practices fit the intended purpose. Int J Occup Med Environ Health 2017;30(6):823–848

ObjectivesEpidemiological data on cancer diseases are alarming. The workplace has become an increasingly important site for disseminating health information and implementing health promotion activities. Occupational medicine physicians (OMPs) have the opportunity to carry out primary and secondary preventive activities focused on civilization diseases, especially cancer. The aim of this study was to evaluate the potential of OMPs in cancer prevention, including the analysis of factors determining the implementation of preventive measures, as part of standard healthcare for employees.Material and MethodsThe study was conducted among 362 OMPs. The interviews were carried out by the computer assisted telephone interview (CATI) method.ResultsOver 60% of the surveyed OMPs are ready to implement cancer preventive activities among employees. The doctors with the longest seniority in occupational health services are more likely to declare unwillingness to implement cancer preventive activities. Patient’s consent, informing women about the program and adjusting the time of the medical visit are the most important conditions for introducing cancer prevention programs by OMPs. Neither seniority nor the number of examinations performed by a physician influenced the currently implemented cancer preventive activities as part of occupational health services (including the evaluation of cancer risk factors occurrence among employees).ConclusionsIn Poland, OMPs are willing to implement cancer preventive activities among employees, but their current activity in this area is limited and needs development. The most specific actions should be addressed to doctors with the longest seniority in occupational health services, who are frequently unwilling to implement cancer preventive activities. Strengthening the preventive potential of Polish occupational health services requires a systemic approach to the scope and way of action of healthcare professionals.

Keeping up with the Kardashians depicts the lives of the Kardashian clan through reality television. The unparalleled success of five sisters managed by their mother has only continued to increase over time along with their participatory self-surveillance through their formidable use of social media. In recent years, a focus on health and wellbeing has led the sisters to endorse products for weight loss and health, using their bodies as spaces of commodification and advertisement online. The family’s interaction with the camera, and the aesthetics of their social media cross-promotions combine to present an open “honest” front promoting the replication of their success and beauty for their audiences. The sisters engage with blackness in a way that bolsters their claims of capacitating and beautifying white feminine subjects, engagements now commonly termed “blackfishing.” This article analyzes how the Kardashians have created an intense regime of self-surveillance, even dabbling self-consciously in the carceral state's techniques for surveilling blackness, to construct themselves as both uncommonly, exotically sexual ('baring all') and respectable enough (white or white passing) to sell various remedies with dubious health value.

Infectious disease surveillance systems are a very important part of national and local efforts to prevent and control infectious diseases. In many countries, especially those with limited health systems, the role of primary care physicians in surveillance is essential, but their lack of knowledge about surveillance can often be a barrier for an effective surveillance system. A one day training seminar was provided to physicians in a primary care centre in AP Vojvodina, Serbia and notifications of infectious diseases by trained physicians were then followed. After the training, the incidences of registered infectious diseases rose 3.9 times (7.5 times with chicken pox excluded). This was mostly for infectious diseases which do not require laboratory confirmation (streptococcal infections, intestinal infectious diseases). The average number of reports per physician increased nearly 8 times with almost all the physicians who participated in the surveillance study.
Systemy nadzoru nad występowaniem chorób zakaźnych są bardzo ważnym elementem krajowych i lokalnych działań mających na celu zapobieganie i kontrolę nad rozprzestrzenianiem się chorób zakaźnych. W wielu krajach, w szczególności tych z ograniczonymi zasobami systemów ochrony zdrowia, podstawową rolę w nadzorze pełnią lekarze. Niedostateczna ich wiedza w tej materii często bywa barierą dla efektywnego systemu nadzoru. Przeszkolono lekarzy w systemie jednodniowego seminarium w Instytucie Zdrowia Publicznego w Vojvodinie w Serbii. Szkolenia przeprowadzili wcześniej odpowiednio przygotowani w tym kierunku lekarze. Po takim szkoleniu 3,9 razy wzrosła częstość zarejestrowania przypadków chorób zakaźnych (nie licząc ospy wietrznej gdzie częstość ta zmalała 7.5 razy). Dotyczyło to głównie chorób zakaźnych, które nie wymagają potwierdzenia laboratorium (infekcje paciorkowcowe, jelitowe choroby zakaźne). Średnia liczba sprawozdań w przeliczeniu na jednego lekarza powiększyła się prawie 8 razy i dotyczyło to prawie wszystkich lekarzy sprawujących nadzór na chorobami zakaźnymi i uczestniczących w niniejszym badaniu.

WstępPomimo wprowadzonego ponad 20 lat temu w Polsce zakazu produkcji materiałów zawierających azbest wciąż odnotowuje się nowe przypadki chorób azbestozależnych. Systematyczna kontrola wydolności układu oddechowego u osób narażonych na pył azbestu jest zatem niezwykle istotna ze względu na biologiczne właściwości tego minerału.Materiał i metodyProgram profilaktycznych badań lekarskich Amiantus wprowadzono w 2000 r. w celu realizacji ustawowych uprawnień byłych pracowników zakładów przetwórstwa azbestu do tego typu badań. Osoby zatrudnione kiedykolwiek w tych zakładach uzyskały uprawnienia do korzystania z profilaktycznych badań lekarskich do końca życia. Prowadzone badania mają charakter ciągły, rozłożony na lata, i są ukierunkowane w szczególności na ocenę układu oddechowego.WynikiOd rozpoczęcia programu przez 20 lat jego realizacji przebadano 8329 osób, w tym 5199 (62,4%) mężczyzn, dla których przeprowadzono łącznie 34 454 badań lekarskich. W okresie realizacji programu odsetek zdiagnozowanych patologii związanych z azbestem wzrósł z 8% w 2000 r. do 25% w 2019 r. Wśród byłych pracowników przetwórstwa azbestu zgłaszających się na badania lekarskie w ramach Programu Amiantus rozpoznano 2078 chorób azbestozależnych, co stanowiło 25% osób z tej grupy. Wśród chorób dominowała pylica azbestowa (1880 przypadków – 90,5%), a następnie rak płuca (121 przypadków – 5,8%) oraz międzybłoniak opłucnej (77 przypadków – 3,7%). Dodatkowo zmiany opłucnowe w postaci blaszek i rozlanych zgrubień opłucnej rozpoznano u 40% pacjentów, natomiast zacienienia miąższu płucnego dotyczyły ponad 65% byłych pracowników przetwórstwa azbestu.WnioskiProgram Amiantus – dzięki długiej obserwacji – umożliwił monitorowanie stanu zdrowia byłych pracowników narażonych na działanie azbestu oraz stworzył unikalną możliwość przeprowadzenia analiz epidemiologicznych. Badania te dostarczyły także wiedzy na temat naturalnej historii chorób związanych z azbestem.
BackgroundDespite the ban on the production of asbestos-containing materials, introduced in Poland over 20 years ago, new cases of asbestos-related diseases are still being recorded. Systematic control of respiratory function in people exposed to asbestos dust is, therefore, extremely important due to the biological properties of this mineral.Material and MethodsThe Amiantus preventive medical examination program was undertaken in 2000 to implement the legal rights of former employees of asbestos processing plants for this type of examinations. People who have ever been employed in such factories have been authorized to use preventive medical examinations for the rest of their lives. The research is continuous, spread over time and focused, in particular, on the assessment of the respiratory system.ResultsSince the beginning of the program, throughout 20 years of its implementation, 8329 people have been examined, including 5199 (62.4%) men for whom a total of 34 454 medical examinations have been carried out. During the program period, the percentage of diagnosed pathologies increased from 8% in 2000 to 25% in 2019. Overall, 2078 asbestos-related diseases were diagnosed among former employees of asbestos processing plants under the Amiantus Program, which accounted for 25% of this group. Among all diseases caused by exposure to asbestos, the most common were: asbestosis (1880 cases – 90.5%), lung cancer (121 cases – 5.8%) and pleural mesothelioma (77 cases – 3.7%). Diseases of pleura in the form of plaques and diffuse pleural thickening were diagnosed in 40% of the examined patients, while radiological pulmonary shadows affected over 65% of former employees of asbestos processing plants.ConclusionsThe Amiantus Program, thanks to the long observation period, enabled monitoring the health of former employees exposed to asbestos, and created a unique opportunity to carry out epidemiological analyzes. These studies allowed the authors to expand their knowledge of the natural history of asbestos-related diseases.

The farm animals, as ruminants and pigs, are primarily raised as food-producing animals, however they are also used for scientific and educational purposes. Given the wide range of diseases, they can suffer from, ensuring adequate animal health can be challenging. This article describes best practice recommendations focusing on cattle, sheep, goats and pigs healthcare and welfare, as well. It provides general and specific information useful in developing a health management program for farms, suppliers, and research centers, as well as guidance on selection of animals for research and educational purposes. Critical thinking, based on the areas of those animals use, is intended to help making informed and sensible decisions, to prepare exhaustive pathogen exclusion list. Implementation of best practices in animal health and welfare management, should be under the direction of a competent veterinarian with expertise and sufficient authority to take appropriate actions, as well as excellent communication skills. It is strongly recommended that veterinarians of the research facility, work closely with the farm/supplier veterinarians.
