A fundamental pre-requisite for the establishment of a scaleable regional health information network (RHIN) is the development of an architectural framework and tools for the integration of specialized autonomous systems and e-health service platforms supported by an underlying health information infrastructure (HII). In this context, HYGEIAnet, which is the RHIN of Crete in Greece, has identified and utilized a number of critical software components enabling integrated access to clinically significant information, based on an open architecture addressing successfully the various interoperability challenges at hand. HYGEIAnet provides the framework for the reuse of standardized common components and public interfaces, thus enabling integrated and personalized delivery of healthcare.

Opis:
Biology and genomics in the near future will play a major role in day to day activities related with health. Today biologists and doctors work together in order to decode and analyze large amount of data extracted from the DNA analysis of proteins. Advanced health applications will be needed in order to store, retrieve, process the large amount of data - being produced today by genomics and bioinformatics analysis - in order to extract useful results in a reasonable time frame. In this paper we present the results from our research regarding the use of grid technologies with health applications. We present the current status of standardization activities and working groups, which are currently involved with the specification of health applications and the standardization of needed components such us security, functionality, etc., which are being introduced by the use of grid technologies.

In this paper we present a distributed telematic platform which is implemented to support health information management and innovative services to people suffering from allergies, asthma and rhinitis. The developed system establishes a trans-European information network offering specialized services to health professionals, patients and the public, by collecting, processing and distributing specialized data and informational content. An integrated monitoring and reporting system of aero-allergens is used to collect Pan-European environmental data and produce allergy maps, forecasting and danger level alerts. Informational content and e-learning resources are also consolidated and combined with personalized health management services. Through the created network, health information is supplied to sufferers via WWW and SMS technologies, and informational and learning resources are offered to health professionals. Sets of services were implemented in pilot form and offered to real users for testing purposes. The results were encouraging towards expansion to fully blown service at a Pan-European level.

The paper reviews available literature on globalization processes in health services. The aim is to identify the main research challenges and basic trends in this area. The analysis is made with the use of SinceDirect, EBSChost, BMJ Journals, ProQuest Medical Library and MEDLINE-PubMed data and covers the 1990-2007 period. The author concludes that theoretical discussions and empirical studies on the globalization of health services are still at an early stage of development even though health services and related sectors have a considerable share in most countries’ GDP. The paper offers a modified version of a model for describing the influence of globalization on health. The author shows that the growing influence of globalization on national health care systems is accompanied by an increased share of the private sector in these systems. At the same time, countries are increasingly adopting measures to boost the role of private funds in financing their health care systems. Analyses show international medical services are almost exclusively provided by private businesses. This means that globalization is primarily benefiting private health service providers and private investors, the author says.

Opis:
The objective of the paper is an assessment of the level and structure of financing the public health services in Poland; the main causes of deaths and relations between the health conditions within Poles and the level of financing public health care system. The aim of the article is indication main negative and positive issues of running the public health care system in Poland. The analysis is based on the last 5 years and refers to changes in Polish health care system which happened in 1999, when the public health services are started to be financed by a health insurance premium. The part of the premium reduces the income tax. The purpose of article is the test of answer the question: what kind of consequences in the level of financing and in health conditions have happened after changes which was carried into effect in 1999.
Celem artykułu jest wskazanie głównych problemów funkcjonowania opieki zdrowotnej w Polsce po reformie ubezpieczeniowej. Począwszy od roku 1999 świadczenia zdrowotne w Polsce są finansowane ze składki na ubezpieczenie zdrowotne. Składka ta opłacana jest przez pracownika i w części pomniejsza podatek dochodowy od osób fizycznych. Celem artykułu jest wskazanie przede wszystkim negatywnych aspektów wprowadzonych zmian. W artykule została dokonana analiza infrastruktury medycznej, wynagrodzeń służby zdrowia, czasu oczekiwania na usługę medyczną, szarej strefy w tym sektorze. Zwrócono również uwagę na finansowy aspekt funkcjonowania regionalnych organizacji powszechnych ubezpieczeń zdrowotnych w Polsce.

Opis:
Underdiagnosis of childhood asthma is a well known problem in many countries. In Poland the estimated occurrence of this problem reaches 50% and is similar to fi ndings published in foreign literature. The causes of the problem are not unequivocally recognized and are addressed by few authors. Among potential causes there are circumstances related to organization and practice of pediatric health care, including application of standard diagnostic criteria of asthma, availability of diagnostic tests, nozologic preferences, etc. Another category of circumstances in charge of underdiagnosis of childhood asthma stems from environmental infl uences, including family-related factors. Findings published in foreign literature show that underdiagnosis is more frequent in children of low socio-economic status, in rural setting or in families burdened by stress. Evidence however is far from being complete. There is a lack of Polish data on the subject and it remains unknown to what extent underdiagnosis of childhood asthma in this country is related to the above mentioned factors. Scientific investigation into the problem is necessary for a number of reasons and justifies implementation of subject-oriented epidemiological studies. Identification of factors associated with an increased probability of underdiagnosis of childhood asthma should facilitate planning of proper preventive measures, above all involving screening programs addressed to selected groups of population.
Prawo swobodnego przepływu osób oraz usług, będące podstawą funkcjonowania Unii Europejskiej, spowodowało konieczność zapewnienia obywatelom Unii Europejskiej, przemieszczającym się wewnątrz Wspólnoty, dostępu do świadczeń wynikających z zabezpieczenia społecznego, a w szczególności usług medycznych na terytorium innego kraju niż kraj pochodzenia. W celu całościowego przedstawiania zastosowania przepisów o koordynacji systemów zabezpieczenia społecznego przez Narodowy Fundusz Zdrowia, dokonano analizy podstawowych aktów prawnych, ustanawiających obowiązujące rozwiązania formalno-proceduralne, niezbędne dla zrozumienia niniejszego zagadnienia. Opracowanie stanowi teoretyczno-empiryczną analizę faktów, w oparciu o ustawowe zadania i ich realizację prowadzoną przez Narodowy Fundusz Zdrowia, z wykorzystaniem danych statystycznych pochodzących z raportów przygotowywanych przez Centralę Narodowego Funduszu Zdrowia, zgromadzonych w Centralnym Wykazie Ubezpieczonych NFZ.

This paper presents a part of the results of the research carried out by its authors in the framework of a research project "Analysis and evaluation of the level of adjustment of entrepreneurs and OSH specialists from the region of Łódź to the changes in legal provisions and in market needs". In the framework of the project 200 OSH specialists professionally active in the region of Łódź were interviewed. The evaluation of the level of knowledge of OSH specialists and the analysis whether or not there are any differences in it depending on the level on which they obtained their vocational qualifications, on a secondary or on a tertiary level, was the main aim of this analysis.
Artykuł przedstawia wybrane wyniki badań przeprowadzonych przez autorów w ramach projektu badawczego "Analiza i ocena stopnia dopasowania łódzkich przedsiębiorstw i kadr bhp, do zmian w przepisach i potrzebach rynku". Analizą objęto grupę 200 pracowników służb bhp prowadzących swą działalność na terenie województwa łódzkiego. Głównym celem analizy była ocena poziomu wiedzy kadr bhp oraz poszukiwanie różnic lub ich braku w zależności od tego, czy uprawnienia służb bhp uzyskano na poziomie szkolnictwa wyższego lub innego.

The objective of the paper is the discussion of the health care systems in selected countries. The paper presents the four basic models of health service, i.e. the Bismarck’s model, the Beveridge’s model, the residual model and the Siemaszko’s model. In the following part, the paper presents information on the deficit in the health care service in Poland, as well as the deficit in selected European countries, that is in Germany, in the United Kingdom, in France, Italy or the Netherlands It has been emphasised that a battle is currently waged throughout Europe about reducing expenses for health care services without any radical limitation in the scope of the public health care benefits. The shortage of financial funds and ever increasing expenditures for medical care put in a difficult situation not only physicians, but also patients. The subject matter of the analysis is indication of the possibilities of reducing excessive costs of the public health care services, but without severe reducing of the scope of public health care benefits. It has been emphasised that real changes may occur from development of privatisation in the competitors. The summary presents expenditures for health care in the years 1999-2008 in selected European countries as % of GDP.
Celem opracowania jest omówienie systemów opieki zdrowotnej w wybranych krajach. Przedstawiono cztery podstawowe modele służby zdrowia, tj. model Bismarcka, model Beveridge’a, model rezydualny oraz model Siemaszki. W dalszej części przedstawiono informacje dotyczące deficytu służby zdrowia w Polsce, a także deficytu pojawiającego się w wybranych krajach europejskich, to jest w Niemczech, w Wielkiej Brytanii, we Francji, we Włoszech czy też w Holandii. Podkreślono, że obecnie w całej Europie toczy się batalia o zredukowanie wydatków na służbę zdrowia bez drastycznego ograniczenia zakresu publicznych świadczeń zdrowotnych. Niedostatek środków finansowych i wciąż rosnące wydatki na opiekę medyczną stawiają w trudnej sytuacji nie tylko lekarzy, ale również pacjentów. Przedmiotem analizy jest wskazanie możliwości redukowania nadmiernych kosztów publicznej służby zdrowia, jednak bez drastycznego ograniczania zakresu publicznych świadczeń zdrowotnych. Podkreślono, że rzeczywiste zmiany mogą jednak nastąpić na skutek rozwoju prywatyzacji konkurencji. Na zakończenie przedstawiono wydatki na ochronę zdrowia w latach 1999–2008 w wybranych państwach europejskich jako % PKB.

The aim of this publication is to briefly characterise and to emphasize the importance of health tourism, as well as to present selected, most important health resorts in Poland and show them as attractive tourist destinations. The fourth part is an afterword which includes a summary of the publication, as well as comments and conclusions that emerged while working on the subject of health tourism.

Rehabilitation of people with disabilities is a process aimed at enabling them to reach and maintain their optimal physical, sensory, intellectual, psychological and social functional levels. WHO, in “World Report on Disability”, defines rehabilitation as a set of measures that assist individuals who experience, or are likely to experience, disability to achieve and maintain optimal functioning in interaction with their environments. Lack of rehabilitation services creates a barrier to full inclusion and participation in all aspects of life. The aim of this article was to present the nature of medical rehabilitation, in particular medical rehabilitation of elderly people. The basic definitions of medical rehabilitation and physiotherapy were also presented. The medical rehabilitation system in Poland, its organization and funding system was shown based on publications and statistical data of the Central Statistical Office (GUS) as well the access to medical rehabilitation services and selected medical procedures was assessed based on available data from National Health Fund (NFZ) and Ministry of Health (MZ).

The Canadian health care system is a publicly financed system administered by ten provincial and three territorial governments. The purpose of this article is to provide an overview of the universal health care system in Canada, including its history, the health status of Canadians, health care funding and spending, and health research and data collection. Health care spending in Canada amounts to 11.6% of the country’s gross domestic product and is estimated to have been $200.5 billion Canadian dollars in 2011. Hospitals account for the largest source of health care spending (29%), followed by drugs (16%) and physician spending (14%). Of the total health care spending, 70% is paid for by the public system. Due to the Canadian population being covered by the universal health care system, health data are being collected and can be used to monitor the health care system and inform evidence-based medicine.

The article is an attempt at diagnosing training needs of the employees of units operating in health care sector in the Podkarpacie Province. In times of permanent changes affecting each sphere of economy, providers of health care services cannot afford to remain outside this trend. Improving qualifications, adaptability of the offer, influencing its quality, and above all, the awareness of the necessity of these changes, have become an element which is fully integrated also with this sphere of public sector operations. Taking into account the above, the article verifies not only training needs articulated by employees of Health Care Centers (HCC) operating in the Podkarpacie Province, but also the way they are perceived by the managers of these centers, the ability to define training needs and their compatibility with characteristic features of analyzed HCCs. Therefore special emphasis has been placed on demonstrating the variety of diagnosed training needs with reference to such criteria as the size of analyzed centers, the market serviced by them, and their location. These determinants allowed us to conduct a complex analysis of conditions and structure of voiced need for subject training, and as a consequence, contributed to diagnosing the expectations of the health care sector concerning initiatives improving the quality of public services in the health care services area.

The aim of the article is to present the scope of determinants influencing hospitals’ functioning in contemporary Europe with emphasize put on the health sector human recourses issue. Multiplicity of the functions realized by the hospitals units relates to the plurality of determinants which influence their present situation as well as long-term transformation processes. The determinants can be categorized into three main groups: these related to the demand side of the hospital services, their supply and determinants being the results of the social and economic changes. Regardless of the differences existing between health systems in specific countries – all European countries are facing similar problems of increasing health care costs, strong need of efficiency improvement and deficits of medical staff. Materials and Methods: In early 2010, we asked employees (n = 2429) working in the Finnish social services and health care industry to identify all the organizational changes that had occurred at their workplaces over the previous two years, and to evaluate their own work ability and whether different statements related to the elements of work ability were true or false at the time of the survey. For our method of analysis, we used logistical regression analysis. Results: In models adjusted for gender, age, marital status, professional education and managerial position, the respondents who had encountered organizational changes were at a higher risk of feeling that their work ability had decreased (OR = 1.49) than the respondents whose workplaces had not been affected by changes. Those respondents who had encountered organizational changes were also at a higher risk of feeling that several elements related to work ability had deteriorated. The risk of having decreased self-evaluated work ability was in turn higher among the respondents who stated they could not understand the changes than among those respondents who understood the changes (OR = 1.99). This was also the case among respondents who felt that their opportunities to be involved in the changes had been poor in comparison to those who felt that they had had good opportunities to be involved in the process (OR = 2.16). Conclusions: Our findings suggest that the organizational changes in social and health care may entail, especially when poorly executed, costs to which little attention has been paid until now. When implementing organizational changes, it is vital to ensure that the employees understand why the changes are being made, and that they are given the opportunity to take part in the implementation of these changes.

The work is devoted to the analysis of the process of human life’s economization Processes taking place in the economic sphere nowadays, affect the social changes in the essential way, affect both the condition of an individual and on relationships between people. The main force of the socio-cultural transformation is scientific-technological progress, especially in the communication and transport as well as promoting the principles of governing the liberal market economy by global economic actors. Neo-liberal market economy and the values and attitudes associated with it (freedom, individualism, competition, mobility, flexibility, striving at all costs to success, especially material) affect the political sphere (economization of politics, marketization of democracy) cultural (the dominance of the commercialized consumer culture) and social (destruction of traditional social structures, disintegration, dehumanisation and commercialization of human relationships and cultural patterns). Commercialized consumer culture is accompanied with marketization and economization of all spheres of life and social relations. Every-thing could be buying: education, safety, care, treatment and longer life. Life itself is also commercializing, insurance companies, employers, companies offering on the market different goods and services assign a specific monetary value to life. People are judged by perspective of consumer behaviour, and products suited to the cultural meaning. Traditional values are replaced by market, commercial values.
