It is expected that during 1980 – 2020 the population of older people worldwide will increase by 240%. The objective of the study was to review strategies undertaken in various countries in association with the ageing of society. This analysis may be used for the selection of actions, which are necessary for the construction and performance of social and health policy in Poland. A method of choice was an analysis of academic literature. The WHO specified health promotion as the most important goal of the policy for seniors. Individual countries adopt many varied strategies of action. In Poland, the introduction of nursing insurance and development of geriatric medicine are primarily proposed. In Japan, the emphasis is placed on enabling the elderly the longest occupational activity possible, and the development of primary health care. For the Swedish, the priority is guaranteed support for family caregivers, while in the United States - an increase in the accessibility of environmental care. There are many strategies developed on an international forum whose purpose is to provide care for the growing number of seniors. The strategies adopted by individual countries are much varied. The basic postulates are a development of geriatric medicine and integration of the systems of health services and social security. The literature lacks reports on the effectiveness of the implemented strategies.

Caregivers of persons suffering from dementia constitute a group displaying a great need for mental health support, while at the same time being largely neglected in terms of receiving this support. In this context, the aim of this article is to propose the Stepped Care 2.0 (S.C. 2.0) model as a mental health care systemic solution for carers of persons with dementia. In this article, we focus on presenting the argumentation that proves the need and validity of introducing the proposed mental health care model for carers of persons with dementia. For this purpose, we draw on the results of our own pilot study, as well as a review of the subject literature. As the literature review indicates, caring for individuals with dementia is a source of multidimensional burden and deterioration in well-being. Results obtained in our own study indicate that there exists a burden of care among carers of persons with dementia in Poland and their well-being is significantly lower compared to non-carers. Moreover, the results suggest that the degree of negative consequences of being a caregiver varies depending of the degree of the patient’s disability. The flexibility of support and client-centric approach assumed in the S.C. 2.0 model can provide a good response to the diverse and changing needs of the carers of persons with dementia. It can also serve as a kind of hub encompassing and organising existing forms of support for carers. Moreover, the effectiveness of the selected types of support included in this model is confirmed by existing research findings. The need for mental health support for carers of individuals with dementia is undeniable. In the light of the arguments provided, the S.C. 2.0 model is a suitable solution for the systemic organisation of such support.

In the following article there are presented current regulations concerning conscience clause in practical medicine. The author points to laws being the source of conscience clause and analyze their subject; points and discuss restrictions of possibility to apply the conscience clause and resulting duties. In this paper there are also presented circumstances where conscience clause can have practical applications. Selected, the most controversial problems were discussed from the current law point of view. In the last part of this study author showed important technical problems following effective law – question of information from the hospital as it will not provide specific services and also question of payment for medical advice, which resulted in use of conscience clause. The goal of this article was also to get most wide sight upon analyzed subject, thus it contain many references to outlooks presented by doctrine.

The common problems of learners that contribute to absenteeism in school include malaria, cough/catarrh, typhoid, headache and stomach ache. This led to the formulation of national school health policy in Nigeria to promote health services in schools. In view of the foregoing, this study assesses principals’ perception of health services in secondary schools. The study population consists of 551 principals of secondary schools (public and private) in Kwara State, Nigeria. Purposive and convenience sampling techniques were employed to select 20 principals. Instrument tagged “Interview Protocol on Health Services (IPHS)” was used to elicit data from the participants. Data collected was transcribed and analysed via thematic approach. Findings revealed that majority of the schools does not have school clinic to dispense health services to learners. Our fidings also show that most of the schools lack health personnel and equipment. This study concludes that comprehensive health services were not provided for learners. The study recommends that the government should come up with policy statement on school health services that will help to provide quality health services to the learners. Adequate budget should be made by the government for the implementation of the policy. Lastly, the study provides practical and methodological contributions to the body of knowledge in the fild of education.

Objectives: To explore if the Barcelona Integral Care Program for Doctors with mental disorders (PAIMM, in Catalan) has achieved its goal of enhancing earlier and voluntary help-seeking amongst sick doctors. Material and Methods: We conducted a retrospective chart review of 1363 medical records of physicians admitted to the inpatient and outpatient units of the PAIMM from February 1st, 1998 until December 31st, 2011. The sample was divided into 3 time periods: 1998-2004, 2005-2007 and 2008-2011 (477, 497, and 389 cases, respectively). Results: The mean age at admission decreased (F = 77.57, p < 0.001) from the first period (x = 54.18; SD = 10.28 years) to the last period (x = 44.81; SD = 10.65 years), while voluntary referrals increased from 81.3% to 91.5% (Chi² = 17.85, p < 0.001). Mental disorders other than substance use disorders grew from 71% during the 1998-2003 period, to 87.4% (2004-2007), and 83.9% in the last period (Chi2 = 29.01, p < 0.001). Adjustment disorders increased their prevalence, while inpatient treatment progressively represented less of the overall clinical activity. Conclusions: Sick doctors may feel encouraged to seek help in non-punitive programs specially designed for them and where treatment becomes mandatory only when there is risk or evidence of malpractice.

Objectives The aging of the workforce is an issue that calls for concrete measures to promote the health of older workers. This study reports on the results of the European “ProHealth65+” research project interventions conducted in Poland in relation to workplace health promotion for older workers (WHPOW) and the institutions involved in these programs. Material and Methods A three-stage search of peer-reviewed and grey literature on the WHPOW in Poland. Results A total of 59 WHPOW programs were retrieved in Poland in the observation period (2000–2015). Most of these aimed at improving the Qualification and Training or at the Work Climate and attitudes toward older workers. The promotion, organization, and funding of these activities were carried out mainly by supra-national and governmental bodies, enterprises and employers, and educational and trainee institutions. Conclusions Although there is great commitment to the medical surveillance of workers on the part of the Polish occupational health service, our search detected a relatively low number of the WHPOW initiatives. Objectives: A casual relationship between temperament, job stress and depressive symptoms has not been established yet. The purpose of this study was to assess the relationships between job stress, temperament and depressive symptoms in female nurses at a Japanese general hospital. Material and Methods: A self-report survey was conducted among 706 nurses. We measured job stress, temperament, and depressive symptoms using the Brief-Job Stress Questionnaire, the TEMPS-A and a screening scale of items from the Ministry of Health, Labour and Welfare of Japan. In order to examine the causal relationship between the measures the stepwise multiple regression and path analyses were used. Results: Depressive symptoms were modestly correlated with job stress (γ = -0.23-0.30). Except for hyperthymic temperament measures, the correlations between depressive symptoms and temperament types were significant and moderate (γ = 0.36-0.50). Overtime, job control as well as depressive and cyclothymic types of temperament were significantly correlated with depressive symptoms (β = 0.15, p < 0.05; β = 0.19, p < 0.01; β = 0.26, p < 0.001; β = 0.32, p < 0.001, respectively). Path-analysis revealed that depressive and cyclothymic types of temperament influenced depressive symptoms both directly (β = 0.67, p < 0.001) and indirectly via job stress (β = 0.35, p < 0.001 from temperament to job stress; β = 0.20, p < 0.05 from job stress to depressive symptoms). Irritable and anxious types of temperament and quantitative job overload did not contribute to the path-analytic model. Conclusions: Health care professionals should consider temperament, especially depressive and cyclothymic types, in order to help employees cope better with job stress factors. We need further research about the effective intervention to help employees better cope with their job stress.

A fundamental pre-requisite for the establishment of a scaleable regional health information network (RHIN) is the development of an architectural framework and tools for the integration of specialized autonomous systems and e-health service platforms supported by an underlying health information infrastructure (HII). In this context, HYGEIAnet, which is the RHIN of Crete in Greece, has identified and utilized a number of critical software components enabling integrated access to clinically significant information, based on an open architecture addressing successfully the various interoperability challenges at hand. HYGEIAnet provides the framework for the reuse of standardized common components and public interfaces, thus enabling integrated and personalized delivery of healthcare.

Objectives The automotive industry represents the most important industrial sector in Czechia. The objective of this study has been to analyze the occurrence of occupational diseases (OD) in the automotive industry during the period from 2001 till 2014. Material and Methods Data on OD cases was retrieved from the National Registry of OD. Further, we conducted a survey in automotive companies with focus on occupational health services and positions of the companies in global production networks (GPNs). An analysis of OD distribution in the automotive industry was performed (age, gender, company size and its role in GPNs, regional distribution of studied companies, and regional unemployment rate), and was accompanied by assessment of the quality and range of occupational health services. Results Employees older than 40 years old have nearly 2.5 times higher probability of OD occurrence as compared with employees younger than 40 years old (odds ratio (OR) = 2.41, 95% confidence interval (CI): 2.05–2.85). Occupational diseases occurrence probability was 3 times higher for women than for men (OR = 3.01, 95% CI: 2.55–3.55). Occupational diseases incidence rates increased with the size of the company (0 OD/10 000 employees in micro enterprises to 57 OD/10 000 employees in large enterprises). A particularly steep rise in OD incidents in the automotive industry was observed in the Plzeň Region between 2001 and 2011. An association between OD incidents and the unemployment rate was not statistically confirmed. Conclusions A statistically significant increase in OD incidents dependent on company size may be arguably attributed to a higher quality of occupational medical services in bigger companies, which ensures better detection and diagnosis of OD. In the Plzeň Region, the rapid increase in OD incidents was mainly caused by a change in the production process of automobile textiles in one factory due to the introduction of a glue containing isocyanates, which are potent allergising agents. This led to an increase in occupational allergic diseases – bronchial asthma in particular. Int J Occup Med Environ Health 2017;30(3):455–468

Rehabilitation of people with disabilities is a process aimed at enabling them to reach and maintain their optimal physical, sensory, intellectual, psychological and social functional levels. WHO, in “World Report on Disability”, defines rehabilitation as a set of measures that assist individuals who experience, or are likely to experience, disability to achieve and maintain optimal functioning in interaction with their environments. Lack of rehabilitation services creates a barrier to full inclusion and participation in all aspects of life. The aim of this article was to present the nature of medical rehabilitation, in particular medical rehabilitation of elderly people. The basic definitions of medical rehabilitation and physiotherapy were also presented. The medical rehabilitation system in Poland, its organization and funding system was shown based on publications and statistical data of the Central Statistical Office (GUS) as well the access to medical rehabilitation services and selected medical procedures was assessed based on available data from National Health Fund (NFZ) and Ministry of Health (MZ).

W niniejszym opracowaniu autorki w zwięzły sposób wskazały na istotne zadania ratowników medycznych wynikające z aktualnie obowiązującego stanu prawnego oraz przedstawiły projektowane w tym zakresie zmiany. Istotą niniejszego artykułu autorki uczyniły uprawnienia ratowników medycznych, ale zwróciły również uwagę na finansowanie świadczeń udzielanych przez zespoły ratownictwa medycznego, co wymagało sięgnięcia do zagadnień związanych ze świadczeniami opieki zdrowotnej finansowanymi ze środków publicznych, w szczególności rolą Narodowego Funduszu Zdrowia. Najistotniejsze w kontekście zarówno istniejących, jak i projektowanych zmian zdaje się być przybliżenie statusu ratowników medycznych do lekarzy czy też pielęgniarek, położnych, a to głównie za sprawą nałożonego na nich obowiązku zachowania tajemnicy zawodowej czy prowadzenia dokumentacji medycznej.
In this article the author briefly pointed out the important tasks of medical rescuers resulting from the current legal status and presented the changes planned in this regard. As the essence of this article, the author has made the rights of medical rescuers, but they also paid attention to the financing of the benefits provided by medical rescue teams, which also required the issue of health care services financed by public funds, in particular the role of the National Health Fund. The most important in the context of both existing and proposed changes seems to be the approximation of the status of medical rescuers to doctors or nurses, midwives, mainly due to the obligation imposed on them to keep professional secrets or keep medical records.

In eight regions audited by NIK in 2016 and 2017, the gynaecological and obstetrical services for outpatients were provided – respectively in the two audited years – in 1,204 and 1,170 clinics, out of which only 199 (16.5%) and 181 (15.5%) were located in rural areas, while 361 (30%) and 357 (30.5%) in the municipal-rural local units. In 2016, the number of gynaecological-obstetrical clinics was from 72 (Podlaskie Region) to 300 (Wielkopolskie Region), including from six in rural areas in the Podlaskie Region and seven in the Opolskie Region, to 50 in the Podkarpackie Region and 41 in the Łódzkie Region (22.2%). A year later, the access to the services of gynaecological-obstetrical clinics for rural citizens further deteriorated.

Opis:
The health care system should offer and provide a variety of services without undue delay. Due to numerous technical, financial and human resource constraints, not all services can be offered both without restrictions and in equal measure in places of different sizes of residence. As a result of qualitative and quantitative research, a map of accessibility to social and health services was drawn up, taking into consideration the division of the country into voivodeships with different population. Spatial analysis showed great diversity in terms of service availability. Voivodships with the highest accessibility of health and social services are Dolnośląskie, Opolskie and Świętokrzyskie, while the worst situation is in Wielkopolskie. Moreover, the article identifies the main problems that people with limited functionality encounter when using health and social services.

The main aim of this article is to analyse key issues concerning patient rights in their capacity as consumers. The healthcare services market has been subject to dynamic development in recent years. For that reason, it is very important to be open to new forms of risk. The present situation in the healthcare services market is important, because a consumer able to clearly identify the character of the these services, will be able to protect his rights. Summing up, there is no doubt that the relation between consumer and patient are quite complicated, but on the other side, very important from the legal point of view.

Objectives: Commercialization of occupational health services (OHS) and transition to a supplier market which started in 1995, has affected work and working conditions of occupational physicians (OPs) profoundly. OPs have lost influence on the organization of their work and managers took over control. This study explores the impact of commercialization on job satisfaction of occupational physicians. Material and Methods: Data were collected through a questionnaire completed by 797 OPs, members of the Netherlands Association of Occupational Medicine (response: 45%). A range of work satisfiers and dissatisfiers measures was included that, according to the Social Action Approach, could explain the variation in job satisfaction. Stepwise multivariate regression analyses were performed to explore to what extent the differences in job satisfaction can be attributed to the characteristics of the setting, biographical variables, satisfiers and dissatisfiers. Results: Occupational physicians in commercial settings had the lowest average score as regards job satisfaction (6.7) contrasting sharply with their colleagues in private practices (8.7) and in non-commercial settings (7.9). The variation in job satisfaction between delivery settings could largely be attributed to satisfiers and dissatisfiers. Biographical characteristics (age, gender) had no effect. The data suggested that not commercialization as such, but the ability of commercial OHS providers to integrate professional values was the crucial factor to bring about job satisfaction. Conclusions: The challenge for commercial OHS providers is to preserve the professional zeal in OPs by integrating professional values in their organization in order to improve the quality of the services and the attractiveness of the profession.

ObjectivesEpidemiological data on cancer diseases are alarming. The workplace has become an increasingly important site for disseminating health information and implementing health promotion activities. Occupational medicine physicians (OMPs) have the opportunity to carry out primary and secondary preventive activities focused on civilization diseases, especially cancer. The aim of this study was to evaluate the potential of OMPs in cancer prevention, including the analysis of factors determining the implementation of preventive measures, as part of standard healthcare for employees.Material and MethodsThe study was conducted among 362 OMPs. The interviews were carried out by the computer assisted telephone interview (CATI) method.ResultsOver 60% of the surveyed OMPs are ready to implement cancer preventive activities among employees. The doctors with the longest seniority in occupational health services are more likely to declare unwillingness to implement cancer preventive activities. Patient’s consent, informing women about the program and adjusting the time of the medical visit are the most important conditions for introducing cancer prevention programs by OMPs. Neither seniority nor the number of examinations performed by a physician influenced the currently implemented cancer preventive activities as part of occupational health services (including the evaluation of cancer risk factors occurrence among employees).ConclusionsIn Poland, OMPs are willing to implement cancer preventive activities among employees, but their current activity in this area is limited and needs development. The most specific actions should be addressed to doctors with the longest seniority in occupational health services, who are frequently unwilling to implement cancer preventive activities. Strengthening the preventive potential of Polish occupational health services requires a systemic approach to the scope and way of action of healthcare professionals.

Objectives In the contemporary Western culture, working population health issues, stressed in various publications, have been perceived as a crucial part of public health. To deal with occupational health issues properly, occupational health services (OHS), aiming at the protection of the workers’ health, are organized in many countries. The survey was to find the differences in occupational health systems that the European Economic Area countries use in order to execute prophylactic activities focused on diseases of affluence and how the OHS are used for health promotion actions. Material and Methods The survey was conducted with the use of an on-line expert questionnaire. No deep statistical analysis was performed as for the designed aims of the study simple statistics were sufficient. Results All the reviewed countries have organized OHS. The funding mechanisms are based mainly on the financial involvement of employers. In the majority of countries, the main goal of the OHS is certification of the ability to work. Workplace surveillance aiming at the occupational risk assessment and health promotion activities that focus on work-related and/or occupational diseases are the most popular services provided within national systems. Conclusions Occupational health professionals are well placed to perform actions directed at health promotion and prophylactics of a wide range of diseases – not only occupational/work-related, but also non-communicable, particularly life-style-related diseases of affluence. The engagement of the occupational medical services (OMS) in prevention of the non-work-related health issues would be valuable. Introduction and objective. The Polish health service is in need of improvement because of the increasing number of geriatric patients. Identifying the resources available to patients is important for ameliorating deficits. The goal of this research was to measure the health and wellbeing of seniors and to investigate to what extent health services meet their needs. Differences in access and scope of received services between patients from different localities (i.e villages, small towns and big cities) were analysed. Materials and method. Questionnaires (Satisfaction with Life Scale; VAS Numeric Pain Distress Scale) and a structured interview assessing the quality of healthcare were carried out in 2015 on a sample of 459 seniors. Each candidate gave consent for participation in the study. Statistica 12.5 software was used for analyses. Results. The overall measure of satisfaction with life of the respondents (M=22.34 SD=5.78) was good. Inhabitants of small towns had a significantly lower sense of quality of life than seniors from big cities (Tukey HSD = 0.047, p<.05). Multimorbidity and polypharmacy were present to a limited extent. The issue of discrimination of older individuals by medical personnel affected a fifth of seniors living in villages, and one- third of those living in big cities. Geriatric patients did not have equal opportunities for access to medical services. Conclusions. State-provided geriatric care is insufficient and does not meet the real needs of patients. A holistic care approach and/or individualized care for older adults are often indicated but are only theoretical constructs in Poland. This inefficient system places the burden of caregiving on the family of the ageing patient. Anxiety among geriatric patients is growing regarding exclusion and lack of care.

The article discusses the problems of health service in prisons in relation to health of prisoners. It refers the results of conducted empirical studies which assumed that the ailments of the imprisoned, specific to their age, actual health service needs, expectations and needs related to health service of prisoners, are indicators of their health. A conception of prisoners’ social service by penitentiary administration without pressure on corrective influence formulated by Henryk Machel was the theoretical basis of the study. The study aimed to answer what was health of the prisoners examined, defined by their general health, needs and expectations concerning health service and to what degree they are answered by the health service in prison. The study was conducted in prisons under District Inspectorate of Prison Service in Poznań, i.e. remand institution in Poznań and prison institutions in Rawicz and in Wronki. All three institutions are of a closed type. The study was conducted between January 2011 and November 2012. It included analysis and explanation of acts of law concerning health service in prisons and questionnaires and interviews with prisoners and prison staff . 279 of them were suitable for further study. Structuralized interviews included 30 doctors and 30 prison carers. The results were analysed statistically. They show opinions of the respondents on the problem of health service in prisons. Young and middle age adults were the majority of the respondents (67,03%). There was a statistically significant relation between general health declared by the respondents and their age. The younger a prisoner was, the better his own general health declared, and vice versa p> 0,001, x2 = 23,245). The data show that half of prisoners declared that imprisonment contributed to deterioration of their health. This is in contrast with a relatively low number of diseases found by doctors after imprisonment. Among various ailments declared by prisoners, psoriasis and sleep deprivation were the most frequent ones. The most detrimental factor to health was prison stress. Its role was indicated by as much as 17 doctors (62,97% of all doctors in the study). Every fourth doctor indicated bad living and sanitary conditions in prison (25,93%). The same number concerned other factor, i.e. nicotinism among prisoners. This shows that many prisoners assume detrimental health habits in prison conditions. Results of the research show that, in practice, response to the need of health protection in penal institutions was very diverse. Health protection was a secondary issue and it was a part of various modules of prisoner treatment. They were characterized by various intensity of health service. It was established that the modules were as follows: control and security (63,36% indications in general). In practice, in penal institutions included in the study, this module occurred in two forms: social and health (providing security and order, prisoners’ control under prison rules, social service and above-standard health care), social (providing security and order, prisoners’ control under prison rules, social service and standard health care), security and rehabilitation (66,67% indications, providing security and order, providing a minimum level of social service and basic health care, rehabilitation). Only the first of the above forms allows to fit the problem of health care fully into penitentiary actions. The study allows for a conclusion that, in the face of the crisis of penitentiary rehabilitation, the role of health care of prisoners will increase – as a consequence of “non-rehabilitationˮ modules of prisoner treatment being more and more frequent. Health care, along with ensuring suitable living conditions and social service, may thus become a part of good penitentiary practice. Health care, and to be exact, medical cal care and medical services for the imprisoned must fit their actual needs. In general, the research has shown that the objective health condition of the imprisoned included in the study is better than their declarations. However, their health varied from a person to another, most of all related to age. The greatest health needs were declared by seniors and their expectations concerning health care and medical assistance ensured by the prison were greatest too. A practical model of prisoners' medical treatment must take this into account. I was established that penitentiary practice employs, most often, modules with basic medical treatment. This is due to the relatively modest prison infrastructure but also to limited budget. Thus, a security - control - pro-health treatment module seems to be a good penitentiary practice. It includes ensuring security to the prison and the imprisoned, order maintenance, above standard health care and social care according to European Prison Rules. Thus, a good practice must be of an eclectic character. Health care and health protection have an important place in such practice. From this point of view, the research shows a diversified picture of penitentiary reality. On the whole, it is relatively positive.
