Objectives The specific job demands of the Prison Service (PS) may affect the health of officers. The job demands-resources model (JD-R) model was used to design a survey of the consequences of working subject to particular job demands. The aim was to gain an insight into the relationship between job demands, personal resources, occupational stress and burnout and selected health consequence indicators (such as behaviors associated with the consumption of alcohol, stress symptoms). Material and Methods A total of 1732 PS officers in Poland were surveyed. The following tools were used as part of the survey: the Copenhagen Psychosocial Questionnaire (COPSOQ II), the Multidimensional Inventory for Assessing Coping Responses (COPE), the Alcohol Use Disorders Identification Test (AUDIT) and a form with a respondent’s particulars. Path analysis using partial least squares structural equation modelling (PLS-SEM) was performed. Results The assumed hypotheses were partially confirmed by the results. Out of 4 job demands categories only work pace turned out not to be a significant predictor of burnout and stress. For alcohol related behaviors, stress level was the only significant predictor, both as a direct and indirect effect taking into account job demands. It transpired that support from superiors rather than support from colleagues or self-efficacy was a significant moderator in the emotional demands – stress relationship. Limitations of the study and perspectives for its continuation are also presented herein. Conclusions Based on the obtained results it may be concluded that job demands and support from superiors do have an impact on stress in the PS group. This is also consistent with available reports in literature. At the same time stress is a significant predictor of alcohol related behaviors. Coping through the use of psychoactive substances was not a significant factor in statistical analyses and it has still not been subject to sufficient scientific analysis.

Introduction: Sarcomas are a rare group of malignant tumours. This study highlights important findings in 91 cases of nonodontogenic sarcomas of the orofacial region. Materials and Methods: Patients who presented with orofacial sarcoma at the Oral and Maxillofacial Surgery Department of a regional University Teaching Hospital between January 1997 and June 2017 were retrospectively studied. Excluded were cases of Kaposi and odontogenic sarcomas. Data obtained were analyzed using Statistical Package for Social Sciences (SPSS) version 16 (SPSS Inc., Chicago, IL, USA). Results from descriptive statistics were represented in the form of charts. Results: A total of 91 cases were reviewed and consisted of 51 (56.0%) males and 40 (44.0%) females, with a male to female ratio of 1.3:1. The mandible (n = 47; 51.6%) and the maxilla (n = 26; 28.6%) were the major sites involved. Osteogenic sarcoma (n = 44; 48.4%) and rhabdomyosarcoma (n = 16; 17.6%) occurred more frequently. A total of 41 (45.1%) patients had surgery and the common hard tissue procedures were mandibulectomy (n = 26; 28.6%) and maxillectomy (n = 10; 11.0%). Discussion: Most recurrences were noted less than one year post treatment. There is a tendency for patients to present late and compliance with follow-up review is poor in this environment.

Kluczową rolę w tym procesie przypisuje się predyktorom poznawczym, takim jak poczucie umiejscowienia kontroli i przekonanie o własnej skuteczności.
Health resources (potentials) can be defined as specific functional properties of a person and his/her environment, acting as the regulatory and health prevention. Increasingly, the attention is drawn to the need of multiplying health resources and protecting them for a lifetime. Linking health with lifestyle requires providing the individual with the help of modifying individual behavior patterns and shaping a pro-health lifestyle based on conscious undertaking of specific activities that increase the resources of one's own health and eliminate threats. The key role in this process is attributed to cognitive predictors, such as the sense of location of control and belief in self-efficacy.

Introduction and objective. This study examines the geographical variation of amenable mortality in Poland, focusing primarily on the role of health care resources at the level of administrative districts and regions, and selected area socioeconomic characteristics as explanatory factors. The concept was used of amenable mortality, based on the assumption that deaths from certain causes should not occur in the presence of timely and effective health care. Materials and method. Standardized death rates (SDR) from causes considered amenable to health care and, separately, for ischaemic heart disease (IHD), were calculated for each of 379 districts (NUTS 4 level) in Poland in 1991–1995 and 2006–2010, using unit mortality data from the National Causes of Death Register. The analytical procedure involved spatial analysis of the distribution of amenable mortality rates, selection of explanatory variables and fitting multilevel regression models using area-level and regional-level characteristics. Results. The results indicate that mortality from conditions which have become amenable to medical intervention has generally decreased in all districts of Poland in the past two decades. Considerable territorial variation in mortality can be observed. Since the 1990s, these differences have been reduced for IHD-related mortality and have increased for amenable mortality. Conclusions. The presented analysis only partly confirms the correlation between variables reflecting the infrastructure of health care resources and the territorial variation in mortality from these two categories of causes of death. Significant correlations with variation in mortality are revealed for the number of primary care physicians (at district level) and the number of specialist practitioners (at regional level). However, after controlling for socioeconomic variables, such as education and low income, the effect of the health care infrastructure-related variables was considerably reduced. The multi-level models also revealed a substantial variation at the regional level, which implies that there are other unobserved contextual influences on amenable mortality at this level.

Health human resources planning – Canadian experience The article is based on the document „Health Human Resources Modelling; Challenging the Past, Creating the Future” published by Canadian Health Services Research Foundation. The article describes three separate but related projects, which are undertaken to link population health needs to health care staff planning, to present the value and challenges in using health human resoures data to inform policy makers on nursing productivity and needs. Project 1, using health data survey explored the level,distribution and patterns of health indicators by demografic and social aspects. During realisation of project 2, nursing productivity was studied by analyzing data for inpatient episodes of care and severity. Project 3 surveyed former nursrs and register nurses across six Canadian provinces. Policy makers can improve estimates health human resources by incorporating population health needs, productivity analyses and evidences based policy strategies tailored for providers. Models and strategies for health human resources planning that are need-based, outcome-directed and that recognize the complex and dynamic nature of the impact these decisions need to be developed and implemented.

The difficult current global situation in the aspect of Human Resources for Health was clearly seen during the COVID-19 pandemic. The spending on healthcare is still increasing and the rate of increase outpaces the growth rate of GDP. Only part of these funds is dedicated to the training of new staff and current healthcare employees migrate in search for better job conditions and worklife balance. Personnel migration combined with the demographic structure in the high-income countries simultaneously leads to increasing demand for healthcare services and limits the supply of specialists who can provide such services. The confrontation between the demand for medical personnel and its supply will lead to a reduction in the quality of care and accessibility of services. In the study based on the large group of Polish county hospitals in 2015–2018, differences and similarities between the hospitals in terms of employment, measured in full-time equivalents (FTEs) and in terms of wages were analyzed. Similarity and dissimilarity analysis was conducted, based on distance measures and cluster analysis. Bigger differences between the hospitals were found for wages than employment levels. The hospitals with an ED and efficient units were less similar to one another than their counterparts in terms of employment (FTEs), except for 2016. When it comes to wages and both types of variables (wages and employment) considered simultaneously, the hospitals with an ED and high number of beds were characterized by lower similarity to one another than their counterparts during the whole period. Clustering all the 3 approaches (FTEs, wages, FTEs and wages) the results were the same. One of these groups was characterized by a rather low employment level per bed, while the other one – by high.

Ankle sprains are common soft-tissue injuries that are often treated in emergency departments. These injuries can have significant consequences for the patient, including long-term morbidity and loss of productivity. The objective of this study was to examine the direct and indirect health resource utilization associated with ankle sprains. 296 adult patients with acute ankle sprains participated in the study in Kingston, Ontario, Canada. Data were collected using a one-month productivity questionnaire. Overall, 11% (95% CI, 8-15%) of the participants visited a physician following the initial emergency department visit. Almost all (95%; 95% CI, 92-97%) of the participants used medications or supportive treatments and 55% (95% CI, 50-61%) reported taking time off from work, school, or housework. The use of unpaid assistance was indicated by 56% (95% CI, 50-62%). Findings from this analysis highlight the significant patient-related and health care system burden of acute ankle sprains.

Objective. To examine disparities in access to the public in Poland, in association of the resources of health care, socioeconomic development of Provinces and their level of urbanization. Materials and method. The Provinces were divided into four groups, using the quartile method, according to the urbanization and level of socio-economic development. The socio-economic development level was identified by the Human Development Index (HDI) for each Province. The urbanization level of each Province was determined by the Index of Urbanization (URBI). Disparities level to access to public health care was identified by taxonomic measure of accessibility (TMA), calculated using resources of health care data. TMA index was compared in the different regions, depending on the level of HDI and URBI. Results. There was no linear relationship between accessibility to public healthcare and socio-economic development of each tested region, nor between accessibility to public health care in the Provinces and their urbanization level. During the study, the correlation between the TMA and HDI and URBI alsdo produced a negative result. Conclusions. An insufficient number of physicians and the limited value of contracts within the public health service may cause a drop in the availability of the public medical service sector, regardless of regional level of urbanization and socioeconomic development.

European Commission Green Paper on the European Workforce for Health – goals and main postulates of the documentGreen Paper on the European Workforce for Health in Europe is a very important document, and decisions made in its light will affect European Union health policy for years to come. Health policies across Europe should be coordinated so that recommended guidelines, designed to reduce inequities, with monitoring to promote their  se in a consistent manner across the EU, particularly but not exclusively by addressing the social determinants of health. Health services are one of the largest groups of employers in most developed countries, and therefore they constitute an important component of national economies. To improve the health of the populations of Europe, and equity of health status, public health education and research need to be a leading part of the health workforce development programme of the EU.EU health systems have to perform a difficult balancing act, firstly between increasing demands on health services and restricted supply; secondly between the need to respond to people’s health needs locally but also to be prepared for major public health crises. There are a number of challenges facing health systems in Europe.1) Policy makers and health authorities have to face the challenge of adapting their healthcare systems to an ageing population. 2) The introduction of new technology is making it possible to increase the range and quality of healthcare in terms of diagnosis, prevention and treatment, but this has to be paid for and staff need to be trained to use it. 3) There are new and re-emerging threats to health, for example from communicable diseases.4) All of this is leading to continually increasing spending on health and indeed is posing major longer-term issues for the sustainability of health systems in some countries.To respond adequately to these challenges requires health systems to have efficient and effective work forces of the highest quality as health services are very labour intensive. Challenges facing the EU public health situation population include ageing, migration, and include infectious and non-infectious diseases, including cardiovascular diseases and cancer, nutritional conditions, disaster preparation, and injury control; along with many other issues of public health, such as management and priorities of health care systems. All are crucial for the future quality of life in Europe. A professional public health workforce is essential for society to be able to meet these public health challenges with high standards of cost-effective interventions

The aim of the article is to present the scope of determinants influencing hospitals’ functioning in contemporary Europe with emphasize put on the health sector human recourses issue. Multiplicity of the functions realized by the hospitals units relates to the plurality of determinants which influence their present situation as well as long-term transformation processes. The determinants can be categorized into three main groups: these related to the demand side of the hospital services, their supply and determinants being the results of the social and economic changes. The aim of the study was to analyse the relationship between fi ve-factor personality traits and subjective health in retirement age, including the mediating role of personal resources. The sample comprised of 240 older adults approaching or of retirement age; aged 55-70 (M = 60.1 years). Subjective health was negatively associated with neuroticism and positively associated with extraversion and openness to experiences. Multiple mediation analyses indicated that personal resources (especially the sense of meaningfulness) mediated most of the analyzed associations between personality and subjective health. For conscientiousness suppressive effects were found.

Objectives Music educators are subjected to many physical and psychological stresses encountered in the workplace. These stresses could be counteracted by certain work-related behavior and experience patterns as personal resources to reduce the negative consequences of stress. The aim of the study was to determine the existing work-related behavioral and experiential patterns and the characteristics of the Work-Related Behavior and Experience Patterns (Arbeitsbezogenes Verhaltens- und Erlebensmuster – AVEM) questionnaire dimensions in the professional group of music educators according to age group. Material and Methods A total of 205 music educators (66.3% female) from various music schools in Germany participated in the online survey. The subjects were divided into 3 age groups (AG): AG I: ≤35 years, AG II: 36–45 years, AG III: ≥46 years. In addition to sociodemographic and occupational data, the standardized AVEM questionnaire was used according to Schaarschmidt and Fischer. The age and occupation-related data were evaluated in a correlation analysis with the expression of AVEM dimensions. Results A total of 71.4% of the music educators were ≥46 years old group. Another 12.8% belonged to AG II, and 15.8% belonged to AG III. The sex distribution in the 3 age groups was comparable (p = 0.261). The expression of all AVEM dimensions was within the reference range. The most pronounced dimension, with a stanine value of M±SD 5.2±2.15, was the willingness to spend. There was also no significant difference in the assignment to the 4 patterns in the 3 age groups (p = 0.669). Age showed a negative correlation with the experience of social support (ρ = –0.354). Conclusions The age-independent and high intervention-requiring expressions of the AVEM risk patterns A and B led to the recommendation of workplace prevention and health promotion measures. Therefore, it seems reasonable to promote appropriate stress management measures and resilience during studies.
