Opis:
Health policy as a process and as activities in the sphere of public responsibility may cause different evaluation problems but at the same time the proper and reliable assessment should be understood as the essential interest of the engaged stakeholders: government at different levels, payers, providers and patients as well. The paper concerns the problem of difficulties influencing the research focused on the health policy description aiming at the indication of the most important factors, effects, possible development dimensions that may significantly change the health system. The methodology in this case is also a complicated issue: quite often based on the instruments typical for social disciplines but not completely applicable for the presented subject. The paper describes the new innovative and universal tool for the purpose of the analysis aiming at reliable and comparable health policy assessment, it presents the stages and objectives of such evaluation and the perspectives of the HPA matrix development both for research and for didactic purposes.

Health policy and HTASet of procedures referred to as Health Technology Assessment (HTA) becomes more and more popular among health policy makers in many. There are two factors contributing to this process. Firstly, it is universal access to health services and their guaranteed financing from public  esources.Secondly, a pressure exerted by manufacturers of pharmaceuticals and medical equipment who looks for profit. The choice which is to be made by decision makers is difficult. If new technologies are implemented too late – there are losses in health and a dissatisfaction of citizens is growing. If they are implemented to hastily, without necessary tests, there are losses in health again and public payer has to pay – and waste – a lot of money. HTA opens a way to make more rational decisions.

Autorka przedstawia główne problemy polityki zdrowotnej w szerszym kontekście polityki społecznej. Przywołuje argumenty wskazujące, że rozwiązywanie problemów zdrowotnych może być mało skuteczne bez rozwiązywania innych problemów społecznych, ale też trudno jest rozwiązywać wiele problemów społecznych bez rozwiązywania problemów zdrowotnych. Po omówieniu pojęcia polityki zdrowotnej, jej celów i zasad oraz uwarunkowań autorka charakteryzuje główne problemy dotyczące wybranych instrumentów polityki zdrowotnej: ekonomicznych, prawnych, organizacyjnych i kadrowych. Artykuł kończy się rekomendacjami wskazującymi przede wszystkim na konieczność zapewnienia kompleksowych działań, zwiększenia roli podstawowej opieki zdrowotnej i lekarza pierwszego kontaktu, poprawy opieki zdrowotnej dla grup szczególnie zagrożonych, przede wszystkim dzieci i młodzieży oraz ludzi starych.
The author presents crucial problems of health policy in a broad context of social policy. She argues that solving health problems may prove insufficient without solving other social problems, though it may also be difficult to solve broader social problems without solving the problem of poor health. After discussing the concept of health policy, its aims, principles and determinants, the author characterises basic problems of chosen health policy instruments: economic, legal, organizational and human resources. The text ends with recommendations demanding comprehensive services, increasing the role of basic medical services and a family doctor as well as an improvement of health care for the most endangered groups, such as children and youth and old people.

In many health space analysis of the relationship between the State of health and the environmental factors that may be in many ways. As a rule, this issue, as in the scientific literature and in the form of various legal acts, is presented in the light of environmental threats to health. Beyond the research nature shot as a factor stimulating health, typically no direct references to health in documents that contain various aspects of nature protection law. In the article it was decided to discuss the health of the legal and administrative context of environmental policy tools and the scale of their impact on the level of awareness of the ecological and health societies. The health of homeless people is a serious public health and social problem, which is enhanced by the lack of interest of homeless people to care about their own health. Homeless people are more likely to succumb to various diseases. It is a consequence of their lifestyle, lack of hygiene, diet, drinking too much alcohol, not attending regular medical preventive examinations, mandatory vaccinations, and so on. Homeless people seek medical help late when their illness is unbearable, or when they be- come victims of an accident or a violent crime. The submitted paper in the theoretical and practical part explains the issue of the health status of homeless people. The aim of the paper is to identify the presence of diseases among homeless people and the mutual relationship between their health status and reintegration into the mainstream society. The research part of the paper presents the results of a survey conducted within the Streetwork project in Trnava through qualitative research. The research file consists of 10 homeless people aged 22 to 52 living in the city of Trnava. The group of participants includes 6 women and 4 men. The results of the survey showed the unfavorable health status of ho- meless people. The conclusion of the paper classifies the recommendations for practice directed to streamline health care of homeless people.

Policy-makers assign various objectives to the implementation of patient charges for public health care services. These charges impose prices on health care consumption and as such, they are expected to affect the quantities of health care service demanded, and to generate revenues. The actual ability of patient charges to achieve these objectives depends to a great extent on the patient payment mechanism implemented in a country, as well as on the health care system and context-specific factors. This paper reviews and discusses the theoretical and empirical evidence on the effectiveness of patient payment policies. The paper suggests that patient charges can be a successful policy tool for controlling the pattern of health care utilisation and improving the quality of health care provision. However, an additional condition for success is the appropriateness of the design of patient charges with respect to efficiency and equity in the public health care sector

Opis:
This article explores various factors that affect implementation of the Polish health strategy, namely National Health Programme (NHP). On the basis of the concept of the Health Policy Triangle, the authors identify several factors of effective implementation of the NHP for the years 2016–2020 under the current legislation and organisational conditions. Those factors are divided into two groups: internal (dependant on NHP policy document) and external ones (related to legal and organisational context of programme’s implementation). Next, each factor is briefly characterised to provide some general guidance for the NHP for the next period. The final part of the article contain a list of proposed actions and decisions to be taken in order to ensure higher effectiveness of the next NHP.

Concept of Health Impact Assessment and its application in European Union. Private sector dimensionAccording to the WHO definition Health Impact Assessment (HIA) refers to “procedures, methods and tools by which a policy, programme  or project may be judged as to its potential effects on the health of the population and the distribution of those effects within the population”. HIA has emerged from weather forecasts and is founding much  broader use. It helps to identify the factors, which have a potential impact on health. HIA may be relevant for all sectors to determine the effects, also unintentional, of their policies and actions on health and it has the potential to bring greater transparency to the decision-making process. Usually, it is very important to be able to foresee future health related effects, also by clarifying the nature of trade-offs in policy. Comparing and judging wanted and unwanted results is essential to choose a possibly best alternative and avoid harm. In European Union where modern tools to rationalize decision making process are broadly accepted HIA has been welcomed, at least in declarations. Although some approaches to assess actions targeted at environment have been formalized and are binding, but others, including HIA, still depend on good will of partners involved. Both member countries and institutions are a bit reserved to use HIA. Insufficient popularity of HIA may result from scarce educational opportunities but also from deficit in well grounded scientific evidence.

The aim of this paper is a systematic description and classification of the deliberative poll (DP) as a potentially innovative method of participation in health policy. The paper discusses the theory of deliberative democracy and a specific method of deliberative polling, as well as examples of its application, along with critical commentary. It summarises the characteristics of DP: advantages and disadvantages, as well as conditions for proper application. In this context an important distinc tion is made between two ideal types of participation: co-decision and consultation. A key feature of the latter is its advisory, and not decisive, character. DP is considered an example of such non-decisive, consultative participation.

In present situation Health protection arrangement is currently rising as an instrument to oversee people groups budgetary requirements towards their wellbeing administrations. Presently multi day, quantities of medical coverage strategies are accessible in the market and those approaches giving money related advantages to the individual and the whole relatives. Present research paper celebrating that approach holders mindfulness and fulfillment levels towards medical coverage. The purpose of the present study was to find out the awareness level of policy holders, followed by the influence of health insurance policy factors, satisfaction levels of holder and problem faced by the policy holders by the health insurance policies in selected area i.e. Hyderabad city. The study reported responses of 183 policy holders from Hyderabad city. The Convenience sampling method was adopted in the collection of data from individual Health policy holder responses and tested by ANOVA test used to help of the SPSS 20.0 Version. The results indicated that there was a strong impact of HIP on demographic factors, followed by HIP factors influence having greater influence on policyholders, finally policyholders having satisfaction towards HIP, even they faced little bit problems towards HIP. Accordingly, through Policy holders opinions, HIP companies should concentrate on the problems of the HIP, and they should create best satisfaction levels to policy holders for giving best competition to competitors in the market.

The role of health technology assessment in pharmaceuticalsreimbursement in PolandThe methodology of health technology assessment has been introduced into Polish health care system officially in 2005, and practically in 2006. It is provided by the Agency for Health Technology Assessment (AOTM, AHTAPol), the institution which role is advisory for the Minister of Health. AOTM collects evidence, performs analysis and independently delivers recommendations on financing health care benefits of public funds. The main part of AOTM tasks concerns drug technologies. We describe Agency’s procedures fulfilling the processes of the assessment of drug technologies, their background according to actual legislation as well as methodology. We explain the directions and the strength of potential impact of AOTM recommendations on the final administrative decision on financing the drug of public funds.

In the Second Republic of Poland health protection was insufficiently developed. There were very few doctors, nurses and hospitals. Only people who were insured or the wealthy had access to medical treatment. The majority of citizens, especially rural resi-dents, could not use health care. The health policy of the state was passive for many years. Changes appeared only in the mid-thirties. The aim of health policy was a public health service for all citizens. The authorities declared their support for this idea on a number of occasions and the parliament accepted the law of public health in June 1939. It was the basis for the construction of common access to healthcare in Poland. To achieve this aim, health centres were built and the compulsory medical practice for the doctors was introduced in villages. The beginning of the Second World War prevented the enforcement of this Act. However, the attempt to create a public health service made in the Second Republic of Poland was a proof of progress which has been made in state health policy.
