In 2009, following the election of Barack Obama to the Presidency of the United States, the American public entered into a fierce debate on how to reform its health care system. The intense debate on health care reform, however, is not a new phenomenon in American political life. Debate over health care has cycled its way into the American political discourse every twenty-years or so. History suggests that forceful opposition has prevailed against most major efforts to alter the health system in the United States. Yet, once again the Democratically-controlled Congress and Presidency in the United States aspire to break this cycle of history. With both houses in Congress passing a bill with significant reforms, it seems that this time may be different. While it is very likely some reforms will pass in 2010, no law has been signed yet56. In the end, only time will dictate the outcome of this round of health reform debates. Through a thorough literature review, this article provides the Polish reader a sense of health care reform efforts in the U.S. from the historical perspective and discusses the current proposed reforms. Particular attention is paid to the evolution of the American health insurance system, failed efforts of past health reform initiatives, their contrast with today’s efforts, and current health and economic indicators that could lead to reform in 2010.

Purpose – The purpose of the paper is to examine the relationship between the organisation of the state (determined by selected characteristics, i.e. the organisational structure model and the position of the SNG sector) and the organisation of the health care system, and an indication of the possible consequences for the sustainability of public finance. Research method – The author uses a method of descriptive analysis and the analysis of statistical data on health care expenditure in 12 EU Member States with an insurance model of health care. The quantitative analysis was based on the latest available statistical data (2017) from the OECD, the WHO and Eurostat databases (according to ICHA and SHA2011). Results – It was concluded that despite the legal separation and theoretical independence from the budget system, the insurance model of health care may pose a threat to the sustainability of public finance. This applies both to the state budget and the SNG sector, which is particularly vulnerable in countries where the decentralisation of public services is not accompanied by an appropriate design of the financing system. Originality /value / implications /recommendations – This paper discusses the problem of the compatibility of the health care system and state organisation, omitted in literature, including the position of the SNG sector, in the context of fiscal sustainability.

The smooth operation of the health care system largely depends on how it is financed. The analysis assumes that the Polish system is not enough financed and poorly adapted to the changing socio-economic trends. Results of the analysis confirm no sufficient polish health care financing. Therefore, changes are necessary in the financing of medical services and obtaining new sources and special funds. Meeting solutions used in functioning and the organization of the system of the health care in Germany can be used in the process of transformations of the system of the health care in Poland.

Medicine is the field of knowledge and set of clinical practices, characterized by the presence of politics and ethics on all the levels. Politics should be treated as a domain of power, which aims to make strategic decisions concerning health of the citizens leading to form socially accepted objectives of health care policy. Ethics sets axiological frames for morally just decisions both political and medical in the field of health care. The article analyzes the reasons of conflicts and moral dilemmas in the health care system. Decisions which are made within health care system should accomplish five criteria: medical, economical, social, legal and axiological. The term “moral panic” is used to illustrate situations concerning corruption and moral atrophy of doctors.

Introduction of patient payments for publicly financed health care services – opinions of the main Polish health care system’s stakeholdersDuring the last decades many European governments have introduced patient payments in their public health care system with the aim to improve efficiency of health care provision, contain overall health care expenditure, and also to generate additional resources. In Poland, since 1999 patients have met formal payment obligations when they use dental services. Though introduction of formal patient payments for primary care services, out-patient specialists’ services and hospital services has been discussed, such payments do not exist. Empirical evidence suggests that the successful implementation of patient payments, to a large extent, depends upon public acceptance and political consensus. The paper presents the results of study on attitudes towards formal patient payments for publicly financed health care services, among different groups of Polish health care system’s stakeholders (health care consumers, providers, insurers and policy makers). The data are collected via focus group discussions and in-depth interviews carried out in Poland in June–October 2009 as part of project ASSPRO CEE 2007. The results are used to out-line policy recommendations.

Health human resources planning – Canadian experience The article is based on the document „Health Human Resources Modelling; Challenging the Past, Creating the Future” published by Canadian Health Services Research Foundation. The article describes three separate but related projects, which are undertaken to link population health needs to health care staff planning, to present the value and challenges in using health human resoures data to inform policy makers on nursing productivity and needs. Project 1, using health data survey explored the level,distribution and patterns of health indicators by demografic and social aspects. During realisation of project 2, nursing productivity was studied by analyzing data for inpatient episodes of care and severity. Project 3 surveyed former nursrs and register nurses across six Canadian provinces. Policy makers can improve estimates health human resources by incorporating population health needs, productivity analyses and evidences based policy strategies tailored for providers. Models and strategies for health human resources planning that are need-based, outcome-directed and that recognize the complex and dynamic nature of the impact these decisions need to be developed and implemented.

The aim of this paper is to analyse whether the medical rehabilitation segment is an important part of the entire Polish healthcare system, and if the medical rehabilitation services are provided with adequate levels of financing and management. The study reviews published literature and legal acts, and undertakes an analysis of data acquired from international and national health data repositories. In Poland there exists no coordination between medical, vocational and social rehabilitation or between the rehabilitation delivered by the health resort facilities. There is an observed lack of coordination among public fund payers. The described lack of coordination influences not only patient treatments (it is difficult to measure outputs and outcomes), but also makes summarizing the total expenditures on curative rehabilitation more difficult. Even though numerous countries spend a smaller or comparable amount of money on rehabilitation (per patient), funds allocated to rehabilitation in Poland (expressed in PPS) are over seven times lower than in France, about five times lower than in Austria and Belgium, and three times lower than in the Netherlands.

The aim of the article is to show the aspects of health care policy, the main aims necessary for the entities corresponding to these aspects and, above all, the changes introduced by the reform of health service adopted by the Law and Justice government. The activities of the current government in the area of health protection clearly modify the system towards the National Health Service, whose model originates from Great Britain, where it has been in operation since 1948. The modifications introduced in the Polish system are aimed to streamline it, improve its effectiveness and accessibility for citizens. Designed and implemented transformations of the health care system in Poland, based on specific aspects as a result of government reform, introduce a system that in the long run will implement the principles of universal, unrestricted access to health services at the best possible level.
Celem artykułu jest ukazanie wymiarów polityki ochrony zdrowia – głównych celów koniecznych do realizacji przez podmioty odpowiedzialne w ramach tych wymiarów, a przede wszystkim zmian wprowadzanych reformą służby zdrowia rządu Prawa i Sprawiedliwości. Działania obecnego rządu w obszarze ochrony zdrowia jednoznacznie modyfikują system w kierunku Narodowej Służby Zdrowia, której wzór zaczerpnięto z Wielkiej Brytanii, gdzie model ten funkcjonuje od 1948 roku. Modyfikacje dokonywane w polskim systemie z założenia mają na celu usprawnić go, poprawić jego efektywność, a także stworzyć go bardziej przystępnym dla obywateli. Projektowane i wprowadzane przekształcenia sytemu ochrony zdrowia w Polsce, oparte w określonych wymiarach na reformie rządu, zaprowadzają system, który w perspektywie czasu ma realizować zasady powszechnego, niczym nieograniczonego dostępu do świadczeń zdrowotnych na jak najlepszym poziomie.

Rosnące zainteresowanie efektywnością usług publicznych skłania do stałego poszukiwania sposobów takiego zarządzania organizacjami, które realizują usługi publiczne, aby te potrafiły zaspokoić oczekiwania społeczne, zarówno w aspekcie ich dostępności, jak i jakości. Istotą podejmowanych działań jest efektywne realizowanie czynności społecznie uznanych za ważne. Zdrowie i jego ochrona odgrywają ważną rolę dla każdego człowieka, dlatego też stanowią przedmiot szczególnego zainteresowania. Funkcjonowanie ochrony zdrowia jest procesem wyjątkowo złożonym, zaś na państwie spoczywają obowiązki związane z jego organizacją. W artykule podjęto próbę zdiagnozowania czynników bezpośrednio wpływających na stan systemu opieki zdrowotnej w Polsce. Przedstawiono wszystkich uczestników systemu ochrony zdrowia, a następnie poszczególnym jego beneficjentom przypisano przysługujące im prawa i obowiązki. W kolejnym kroku przeanalizowano działania każdej ze stron, aby finalnie uwypuklić działania, które destabilizują system, czyniąc go nieefektywnym. Wykazano ponadto, że dla dwóch z trzech głównych uczestników systemu, pacjent stał się raczej sposobem zarabiania pieniędzy, aniżeli docelowym beneficjentem jego starań, w myśl zasad, dla których powstał.
The growing interest in the efficiency of public services leads to constant search of ways such management organizations; public services to those able to meet the social expectations, both in terms of their availability and quality. The essence of the measures taken is the effective implementation of activities considered socially important. Health and its protection play a special role for every human being, and therefore are of particular interest. The functioning of health care is particularly complex process, and the State have obligations associated with its organization. Thus, the article is an attempt to diagnose the factors directly affecting the state of the health care system in Poland. We present all participants in the health care system and its individual beneficiaries assigned their rights and responsibilities. In the next step we analyzed the action of each of the parties to finally highlight the actions that destabilize the system, making it ineffective.

European Commission Green Paper on the European Workforce for Health – goals and main postulates of the documentGreen Paper on the European Workforce for Health in Europe is a very important document, and decisions made in its light will affect European Union health policy for years to come. Health policies across Europe should be coordinated so that recommended guidelines, designed to reduce inequities, with monitoring to promote their  se in a consistent manner across the EU, particularly but not exclusively by addressing the social determinants of health. Health services are one of the largest groups of employers in most developed countries, and therefore they constitute an important component of national economies. To improve the health of the populations of Europe, and equity of health status, public health education and research need to be a leading part of the health workforce development programme of the EU.EU health systems have to perform a difficult balancing act, firstly between increasing demands on health services and restricted supply; secondly between the need to respond to people’s health needs locally but also to be prepared for major public health crises. There are a number of challenges facing health systems in Europe.1) Policy makers and health authorities have to face the challenge of adapting their healthcare systems to an ageing population. 2) The introduction of new technology is making it possible to increase the range and quality of healthcare in terms of diagnosis, prevention and treatment, but this has to be paid for and staff need to be trained to use it. 3) There are new and re-emerging threats to health, for example from communicable diseases.4) All of this is leading to continually increasing spending on health and indeed is posing major longer-term issues for the sustainability of health systems in some countries.To respond adequately to these challenges requires health systems to have efficient and effective work forces of the highest quality as health services are very labour intensive. Challenges facing the EU public health situation population include ageing, migration, and include infectious and non-infectious diseases, including cardiovascular diseases and cancer, nutritional conditions, disaster preparation, and injury control; along with many other issues of public health, such as management and priorities of health care systems. All are crucial for the future quality of life in Europe. A professional public health workforce is essential for society to be able to meet these public health challenges with high standards of cost-effective interventions

The role of the public and private sector in health care systems remains one of the crucial problems of these systems' operation. The purpose of this research is to identify the relationships between the performance of health systems in CEE and CIS (Central and Eastern Europe and Commonwealth of Independent State) countries, and the mix of public-private sector in the health care of these countries. The study uses a zero unitarization method to construct three measures of health system performance in the following areas: (1) resources; (2) services; and (3) health status. The values of these measures are correlated with the share of public financing that represents the public-private mix in the health systems. The data used is from World Health Organization’s Health for All Database for 23 CEE and CIS countries and comprises the year 2010. The results show that the performance of health systems in the countries investigated is positively associated with a higher proportion of public financing. The strongest relationship links public financing with performance in the area of services production. For policy makers, these results imply that health systems in post-communist transition economies could be susceptible to a decreasing role of the state and that growing reliance on the market mechanism in health care can deteriorate the operation of these systems.

This article analyses issues related to the financing and effectiveness of health care systems. The first section reviews the new concept of sustainable development adopted by the United Nations – Agenda 2030. The next two sections present the process of health care transformation in Poland and the basic indicators of health financing in the European Union member states (total expenditure on health, general government expenditure, private expenditure, out-of-pocket expenditure). The final section assesses the selected health indicators for Poland versus the EU member states, with particular focus on the Euro Health Consumer Index produced by the Health Consumer Powerhouse.

Our society is on the brink of health care system reorganization and implementation of new medical technology. Hospitals have to be a core component of the medical revolution so they have to be prepared for the upcoming leap in their development. If Poland wants to be a pioneer in providing new medical solutions, the current ineffective system has to be changed. The necessary action should be taken to deal with the financial problems Polish hospitals have faced for over 20 years. The current structure of hospitals - SPZOZ, is old-fashioned and cannot adapt to a turbulent social and economic environment. The hospitals should be commercialized and restructured. Being capitalized companies will give incentives and new tools to deal with financial problems. The article presents an example of the commercialized hospital in Łańcut. We make the observations that commercialization increases: a hospital’s profitability, its employment productivity, its capital investment spending and leverage. The case proves that the transformation of hospitals to capital companies proposed by the Ministry of Health may be an appropriate approach and it does work once a reasonable management board is in charge. However, the legal structure alteration should be treated as the first step in the overall restructuring process. The article highlights the problem of managers of Polish hospitals who do not only struggle with financial shortages, but barely know which business model they should follow after commercialization to successfully run the restructuring process. Having examined the LMC the authors are crafting a prelude to the overall research on already commercialized hospitals to find an appropriate business model.

Funkcjonowanie SOZ, a więc jakość i dostępność świadczeń zależy od wielu czynników, z których najważniejsze to starzenie się społeczeństwa, jednak system do radzenia sobie z nimi nie jest przygotowany. Dodatkowo kryzys finansowy spowodował zwolnienie tempa wzrostu gospodarczego i kłopoty finansów publicznych, co przekłada się na wielkość strumieni pieniądza płynących do systemu. W tych niekorzystnych warunkach szczególną rolę będą musiały odegrać przedsiębiorstwa. Jednakże praprzyczyną stanu, w jakim znalazł się polski SOZ jest niewłaściwa regulacja prawna, która wynika z braku odpowiedzialności politycznej zarówno koalicji, jak i opozycji.
Functioning of SOZ, so the quality and availability of services depends on many factors, the most important of which figured as the aging population, to deal with which it prepares the system. In addition, the financial crisis caused a slowdown in economic growth and public finance problems, which is reflected in the size of the streams of money flowing into the system. In these adverse conditions will have a special role to play business. However, the root cause of the state of a Polish health care system is wrong regulations caused by the lack of political accountability of both the coalition and the opposition.

The attitudes of health care system stakeholders towards official patient charges have not been studied in Ukraine although both the central and local governments have several times considered to introduce such charges. Instead, informal patient payments are widespread and wellestablished. Ukrainian patients pay either unofficially or quasi-officially (i.e. charitable contributions) to health care institutions. The reasonable solution for dealing with these types of payments would be the introduction of official patient charges. However, the legal base for such reform in Ukraine is ambiguous. The Constitution declares that health care provision is free-of-charge. Nevertheless, in our study,  representatives of stakeholders groups appear keen not only on discussing official charges but are also favor their introduction. The expectations regarding the possible objectives of these charges expressed by different  stakeholders are the focus of this paper.

India is a pluralistic, multilingual and multi-ethnic country located in Southeast Asia. It is the 2nd largest populated country which has the 7th position in the geographical area. Since 1990 India has emerged as one of the wealthiest economies in the developing world. Recently it is the second fastest growing major economy in the world. Despite the fact that economical development in India has been accompanied by increases in life expectancy, literacy rates and food security, India’s performance in the area of health care has been still far from satisfactory. The system of financing health care as a one of the most privatized in the world faces especially many problems. This paper would therefore attempt to describe and assess the system of financing health care in India. In order to outline to the readers the overall country context the first section will provide a brief description of the demographic, economic and health profile of the Indian population. The second and third sections will present the main methods of funding health care in India as well as and the budget of health care system. The concluding section would attempt to assess the financing system in India and offer a way forward for the better achievement of health care system goals.

Sprawne działanie sektora opieki zdrowotnej w dużej mierze zależy od sposobu jego finansowania, a tym samym od wyboru odpowiedniego modelu systemu ochrony zdrowia. Obecnie wyróżnia się trzy tradycyjne modele: model Bismarckowski opierający się na obowiązkowych składkach zdrowotnych, model Beveridge’a wykorzystujący podatki państwowe oraz model rezydualny, który charakteryzuje się dobrowolnymi prywatnymi ubezpieczeniami zdrowotnymi. Do lat 90. XX wieku wyróżniano model Siemaszki, opierający się na centralnym planowaniu oraz finansowaniu świadczeń zdrowotnych przez budżet państwa. Mimo że model Siemaszki obecnie nie występuje, pewne jego elementy są wykorzystywane. Przyjęcie przez państwo konkretnego wzoru finansowania ochrony zdrowia pociąga za sobą wiele innych konsekwencji, które są odczuwalne dla wszystkich uczestników systemu – nie tylko pacjentów, lecz także zarządu/dyrekcji placówek medycznych czy ministerstwa zdrowia. W niniejszym artykule przedstawiono w ogólnym zarysie obecnie funkcjonujące systemy ochrony zdrowia w trzech wybranych krajach. Celem artykułu jest przybliżenie tematyki finansowania oraz funkcjonowania sektora opieki zdrowotnej. Model systemu opieki zdrowotnej przyjęty przez państwo oraz reformy służby zdrowia wpływają na skuteczność procesów restrukturyzacyjnych realizowanych przez szpitale. Ze względu na powyższe należy przeanalizować modele systemu opieki zdrowotnej w wybranych państwach, jak również reformy podjęte w ramach danego modelu. Artykuł należy traktować jako przegląd literatury z zakresu podjętej tematyki.
The smooth operation of the health care sector largely depends on how it is financed , and thus the choice of an appropriate model healthcare system. Currently, there are three traditional models: Bismarck's model based on mandatory contributions of health, the Beveridge model based on state taxes and the model residual, which is characterized by voluntary, private health insurance. Until the 90s of the XX century were distinguished Siemaszko model based on central planning and financing of health services by the state budget. Although the model Siemaszko currently there is no certain elements are used. Adoption by the state specific model of health care financing entails a number of other consequences that are felt by all participants in the system not only patients, but also board/ directorate of medical institutions, and ministries of health. The following article presents – in general terms – currently functioning health systems in the three selected countries. The purpose of this article is to introduce the subject of finance and the health sector. Model health system adopted by the State and health care reform affect the effectiveness of the restructuring processes implemented by hospitals. Due to the above models should be examined health care system in selected countries, as well as undertaken in the framework of the reform model. The paper should be regarded as a review of the literature with regard to the undertaken subject.

The different definitions of efficiency (in their medical meanings) are presented as the result of meta-reviews found in scientific databases. Efficacy and efficiency are often mismatched with effectiveness in the research of healthcare systems in different countries. In addition to the classic Bismarck's and Beveridge's models the modern concepts of health systems include personalized medicine, recognition of health as economic value, and so called salutogenesis. However, the basic problem in the Polish healthcare system is the low quality of overly specific and often changed legislation.
