Opis:
Introduction: Damage to the tendon of the flexor pollicis longus (FPL) is a very frequent sequel of injuries in the region of the thumb. The mechanism of injuries can be very different; most often, these are cuts by sharp tools. Damage to other tissues frequently accompanies these injuries. In such cases, surgical junction of a lesioned tendon is the treatment of choice followed by specialistic rehabilitation.Aim of paper: The aim of this study is to present a therapy program and the results of rehabilitation treatment in patients after surgical FPL junction.Material and methods: The study involved patients with damage to FPL tendon, who were treated in the Malopolskie Hand Rehabilitation Centre of the L. Rydygier Hospital in Krakow. The investigation method included physiotherapeutic diagnostic studies comprising measurements of the range of movements, dynamometric measurements of muscle strength, examination of sensation and the NHP test of manipulative capacity. For the assessment of hand function, Swanson’s methodology was applied. Results: Based on twice repeated assessments (at the beginning and at the end of rehabilitation) and their analysis, effectiveness and correctness of applied rehabilitation program were confirmed. Conclusions: Surgical treatment FPL damage requires the immediate introduction of rehabilitation. A broad selection of resources and physiotherapeutic methods applied during such therapy increase the effectiveness of treatment.

Opis:
Degloving injury consists in tearing out the soft-tissue integument from skeleton of the hand, with accompanied nerves and vessels. The whole hand degloving has bad reputation and one of worst prognosis, even worse than total hand amputation. The range of possible salvage procedures in these cases is limited and their outcomes are unsatisfactory. One of the suitable methods is wrapping the skinned hand with pedicled or free greater omentum flap, retrieved from the abdominal cavity. The article reports outcomes of the treatment of 5 patients at a mean of 8 years after total degloving of their hands and coverage with omental flaps. All flaps healed uneventfully, but in none of the patients the whole length of the fingers was preserved. Division of stumps of 3 fingers was possible in one patient, two others had three-digital hands and remaining two had only separated thumb. Dexterity of injured hands was limited with a mean of score DASH questionnaire of 43 points. Quality of life as measured by SF-36 questionnaire was fair (58 and 53 points in physical and mental domain, respectively). Regardless this, all patients were satisfied with achieved outcomes and all returned to work, which was a confirmation of the effectiveness of the method used in their treatment.

Opis:
Introduction: Damage to flexor tendons constitutes the prevailing area of traumatic injuries to a hand. Without appropriate, instantaneous postoperative rehabilitation various secondary changes like adhesions to surrounding tissues occur and it makes complete restitution of skills in the hand impossible. Generally, traumatic tendons of flexors injuries are treated primarily. However, some patients for various reasons require repeated surgery procedures, for example: two-stage flexor tendon reconstruction.Aim of the study: To compare rehabilitation results in patients after two-stage flexor tendon reconstruction with the results of individuals following one-stage surgical treatment of injured flexor tendons in the second area.Material and methods: The research group was composed of 17 patients after second stage flexor tendon reconstruction from the second to the fifth finger, for whom there primarily appeared a tendon injury in the second area. The control group consisted of 17 persons, who had had surgical fusion of the corresponding tendons performed. Routine physiotherapeutic diagnostics constituted the method of examination, composed of details, as follows: range of movement measurements, dynamometric measurements, examination of sensory and manipulative skills. Functioning of hand assessment was based on Swanson’s methodology. All patients underwent an individual program of rehabilitation which was based on physiotherapeutic, kinesitherapeutic and apparating procedures individually matched for each patient. The program was completed by exercises performing by the patients at home. Results: Both in the research group and control group, significant improvement of hand functioning between the first and the last examination was noticed. However, when compared to the research group, the hand functioning defect in both examinations was lesser in the control group. The rehabilitation duration of patients from the research group was significantly longer than in the control group. Conclusion: Rehabilitation after two-stage flexor tendon reconstruction leads to a significant improvement of hand functioning, however the treatment duration is longer.
