Opis:
Introduction. Gastrointestinal stromal tumors(GIST) are tumors of mesenchymal origin which originate from the walls of gastrointestinal system (GIS) organs. Aim. In this case report we aim to discuss the clinical, labaratory and radiological presentation of distal duodenal GIST as a rare and overlooked cause of life-threatining GIS bleeding. Description of the case. A 76-year-old male patient was presented to the emergency department with massive gastrointestinal bleeding. Computerized tomography revealed a mass soft tissue density of 4x4cm at the level of the 3-4th segment of the duodenum. At the endoscopy, there was a deep ulcer in the proximal part of the 3rd segment of the duodenum with a diameter of 2 cm with a bleeding vessel protruding into the lumen. After endoscopic treatments, biopsies were taken from the edges of the ulcer. Histopathological examination revealed a sheet-like infiltration composed of mildly pleomorphic cells with oval-spindle nuclei and abundant eosinophilic cytoplasm in the duodenal lamina propria, as the patient was diagnosed of GIST. Conclusion. GIST and its clinical, labaratory and radiological presentation should be kept in mind in the approach to massive duodenal GIS bleeding.

Opis:
Ectopic pancreas is a rare congenital disorder defined as pancreatic tissue lacking vascular or anatomic communication with the normal body of the pancreas. Most cases of ectopic pancreas are asymptomatic, but it may become clinically evident depending on the size, location and the pathological changes similar to those observed in case of the normal pancreas. It is often an incidental finding and can be located at different sites in the gastrointestinal tract. The most common locations are: the stomach, duodenum or the proximal part of small intestine. The risk of malignancy, bleeding and occlusion are the most serious complications. Despite the development in diagnostics, it still remains a challenge for the clinician to differentiate it from neoplasm. In this report, we described a case of 28-years old woman who presented recurrent epigastric pain. The upper gastrointestinal endoscopy revealed gastrointestinal stromal tumor on the border of the body and antrum of the back wall of great curvature of the stomach. The histopathological examination after surgery showed heterotopic pancreatic tissue. Ectopic pancreas should be considered in the differential diagnosis of gastric mass lesions.

Opis:
Introduction. Gastrointestinal stromal tumor (GIST) is most often locate in the region of the stomach and the proximal part of the small intestine. Aim. The multiple histopathological examination is described. Description of the case. Multiple GISTs are rare neoplasms that originate from the interstitial cells are described. Conclussion. GIST can occur in any part of the gut, they are most common in the stomach and small intestine, and less frequent in the colorectum and esophagus. Although their pathogenesis and clinical manifestations are different, these tumor syndromes confer a high risk for developing multiple neoplasms.

Opis:
The article presents a case report of a patient with an intramural gastric hematoma. Diagnostic examinations were suggestive of a suspected gastrointestinal stromal tumor Normal image was observed in gastroscopic examination while abdominal CT scan revealed a nodular lesion along the greater curvature of the stomach extending from the posterior wall and adjoining the pancreas and the spleen. The patient was qualified for surgical treatment. Laparotomy was performed followed by total gastric resection and Roux-en-Y reconstruction of the gastrointestinal tract. Post-operative histopathological examination revealed the presence of an extensive hematoma penetrating the perigastric fat tissue along with numerous hemosiderinophages and segmental indicators of formation of inflammatory granulation tissue suggestive of a chronic nature of the lesion. Immunohistochemical GIST assays (CD117, DOG-1, CD34, CD31, SMA, S-100, CKAE1/AE3, Ki-67) were negative. No complications were observed in the post-operative course. Patient is subject to continued follow-up and observation. Follow-up gastroscopy and abdominal CT scan performed 6 months after the surgery revealed an unremarkable image.
