Primary adenocarcinoma in the esophageal gastric graft is a rare complication diagnosed in patients with long-term survival. Most data concerning the diagnosis and treatment of patients with metachronic cancer in esophageal grafts is derived from Japan and South Korea. The diagnosis of cancer in esophageal gastric grafts in the European countries is rare. The study presented a case of a 66-year old male patient who, 30 months after an esophageal squamous cell cancer resection, was diagnosed with adenocarcinoma of the esophageal gastric graft. Despite control follow-up after the esophagectomy, cancer in the esophageal graft was detected during the stage that prevented performing radical surgery. The study presented the recommended diagnostic procedures and treatment options for esophageal gastric graft cancer, as well as review of available literature data

The study presented a patient with asymptomatic gastric cancer, in whom the first symptom was metastasis to the brain. The patient was initially diagnosed by a neurologist and subject to surgical intervention in the area of residence, where he underwent craniotomy with the excision of the metastatic lesions located in the occipital lobe. The histopathological examination revealed the presence of adenocarcinoma metastases. Following complex diagnostics the patient was diagnosed with cardial carcinoma, being subject to cerebral radiotherapy and chemotherapy. The patient was then referred to surgery at the Wielkopolska Cancer Center in Poznań. After final exclusion of disease dissemination (by means of PET-CT) the patient underwent total gastrectomy with D2 lymphadenectomy, and gastrointestinal tract reconstruction by means of the Roux-en-Y method. The histopathological examination result was as follows: tubular-papillary G2 adenocarcinoma (intestinal type), pT2 pN0 (23 evaluated lymph nodes without cancer metastasis), vascular neoplastic emboli, and positive HER2 protein expression. After surgery the patient was subject to adjuvant chemotherapy. Control brain CT examinations revealed the presence of 4 recurrent metastatic lesions-the patient was disqualified from stereotactic radiation therapy and was subject to palliative chemotherapy. The discussion presented the problem of treating patients with stage IV gastric cancer, including current management guidelines, as well as literature review concerning the treatment of patients with diagnosed gastric cancer and brain metastases.

Gastric cancer with the annual mortality of 700 000 is the second cause of death in the world. Iran with the annual incidence of 37 in 100 000 is among areas with high risk of gastric cancer. In Iran, Khuzestan Province with the annual incidence of 10 -15 in 100 000 is among areas with moderate risk. The role of Helicobacter pylori infection in distal gastric cancer as one of the most important risk factors of gastric ulcer has been proved but its role in proximal gastric cancer is controversial. In recent decades, the incidence of proximal cancers has increased. The aim of the study was to survey the relative frequency of Helicobacter pylori infection in the people with proximal cancer and to compare it with this infection frequency in the distal cancer. Material and methods. Relative frequency of Helicobacter pylori infection was surveyed by pathology tests and biopsy gastric mucosa sample staining with H&E in the patients with proximal and distal gastric ulcer whose cancers were proved by pathology method and also in the people who were healthy in terms of a gastric cancer. Data were statistically analyzed by frequency tables and chi square test. Results. One hundred and seventeen people entered the study. Among 32 patients with the proximal cancer, 21 people were with the positive Helicobacter pylori (65.6%) and 11 people (34.4%) were with the negative Helicobacter pylori (p<0.01). Among 30 patients with the distal cancer, 26 people were with positive Helicobacter pylori (86.7%) and four people were with negative Helicobacter pylori (13.3%) (p<0.01); 55 people were without the cancer, which among them, 40 people were with the positive Helicobacter pylori (72.7%); and 15 people were with the negative Helicobacter pylori (37.7%) (p<0.01). In comparison of the frequency of Helicobacter pylori infection in the patients with the proximal cancer, a weak significant difference exists (p = 0.053). Comparing the frequency of Helicobacter pylori infection in the patients with the gastric cancer and people without the cancer, no significant difference existed (p = 0.703). Conclusion. A weak significant difference existed between the frequency of Helicobacter pylori infection in patients with the proximal gastric cancer and the frequency of these infections in the patients with the distal cancer. So, eradicating Helicobacter pylori infection can be effective in preventing the distal gastric cancers in addition to preventing the proximal gastric cancers.

The report presents the case of a patient treated surgically for perforated gastroenterocolic fistula with a concomitant abscess in abdominal integuments and symptoms of the digestive tract blockage. Many months before this surgery the patient had undergone gastric resection and hepaticoenterostomy (Roux-Y) due to inflammatory tumor causing pyrolostenosis and including the peripheral part of the common bile duct. After the surgery, the patient suffered from recurrent abdominal pain which resulted in many hospitalizations. After one of the episodes of complaints, the patient with symptoms of the digestive tract blockage was admitted again to our ward, prepared to the surgery and qualified for the surgical intervention. En bloc resection of the stomach, hepaticoenterostomy and partial resection of the transverse colon were performed. The continuity of the digestive tract was restored by gastroenterostomy with the isolated jejunal loop, anastomosis between the hepatic loop and side of the afferent loop and end-to-end anastomosis of the transverse colon. There were no postoperative complications. The authors point out circumstances affected on decision to postpone the surgery by the patient despite frequent recurrent complaints after primary surgery and numerous previous hospitalizations.

The article presents a case report of a patient with an intramural gastric hematoma. Diagnostic examinations were suggestive of a suspected gastrointestinal stromal tumor Normal image was observed in gastroscopic examination while abdominal CT scan revealed a nodular lesion along the greater curvature of the stomach extending from the posterior wall and adjoining the pancreas and the spleen. The patient was qualified for surgical treatment. Laparotomy was performed followed by total gastric resection and Roux-en-Y reconstruction of the gastrointestinal tract. Post-operative histopathological examination revealed the presence of an extensive hematoma penetrating the perigastric fat tissue along with numerous hemosiderinophages and segmental indicators of formation of inflammatory granulation tissue suggestive of a chronic nature of the lesion. Immunohistochemical GIST assays (CD117, DOG-1, CD34, CD31, SMA, S-100, CKAE1/AE3, Ki-67) were negative. No complications were observed in the post-operative course. Patient is subject to continued follow-up and observation. Follow-up gastroscopy and abdominal CT scan performed 6 months after the surgery revealed an unremarkable image.

Laparoscopic adjustible gastric binding (LAGB) is one of most common surgical methods of treating obesity. Gastric band migration (erosion) is a typical LAGB complication, with a frequency of about 1-4%. The aim of the study was to present the possibilities of endoscopic diagnosis and treatment of this complication. Material and methods. The study was carried out in the Department of Gastroenterological, Oncological and General Surgery in Łódź. Between 2008 and 2015, 450 gastric bands were implanted using the laparoscopic technique in 318 (71%) women and 132 (29%) men. In this period 7 cases of band migration were diagnosed – 3 cases in men (2.3%) and 4 cases in women (1.3%), what presents 1.56% of general number of complications. Five out of 7 eroded bands were qualified for endoscopic removal. Four out of 5 qualified eroded bands were removed using the gastric band cutting technique. In one case we used the musculo-mucosal incision technique. In order to diagnose early perforations all patients underwent control passage examinations with oral contrast (gastrografin) 3-6 hours after the procedure. Results. All 5 out of 5 qualified eroded gastric bands were successfully removed with the endoscopic method, which gives 100% success rate in own material. Two endoscopic methods were used: 1) endoscopic gastric band cutting, 2) endoscopic musculo-mucosal incision. Conclusions. Endoscopy gives a possibility of instant diagnosis of gastric band migration and early minimally invasive treatment. One of our endoscopic methods of removing the bands by making several incisions of the musculo-mucosal plicae has not yet been described in professional medical literature.

Introduction: EpCAM protein belongs to adhesion molecules of epithelial cells. It mediates in the homophile adhesion cell-cell reactions. EpCAM protein expression can be observed in the majority of healthy normal cells. However, mutations in EpCAM gene may lead to an increased risk of cancer development. The aim of the study was to assess EpCAM protein expression in the correlation with chosen clinical and histological parameters in gastric cancer. Materials and Methods: EpCAM protein expression was evaluated immunohistochemically in 88 patients diagnosed with gastric cancer. Results: An increase in EpCAM protein expression was demonstrated in cancer cells compared to normal gastric mucosa (59.3% cancers with the positive expression of EpCAM protein). The increased EpCAM protein expression was observed in patients with a histological type of adenocarcinoma without a mucinous component than in those with adenocarcinoma with a mucinous component (p=0.028). The higher expression of this protein was observed also in the intestinal type according to the Lauren classification (p=0.037). The expression of the protein was lower in the diffuse type of cancer. Additionally, an increase in EpCAM protein expression was revealed in cancers infiltrating to the blood vessels (p=0.013). Conclusions: A correlation between EpCAM expression and adenocarcinoma without a mucinous component as well as the intestinal type according to the Lauren classification may prove a role of this protein in the histogenesis of gastric cancer. Moreover, its positive expression is related to cancerous cells infiltrating to the blood vessels, which may suggest a role of EpCAM protein in the early stages of gastric cancer metastases.

Presence of free gastric cancer cells in the peritoneal cavity of patients who underwent surgical treatment for gastric cancer is a negative prognostic factor and caused rapid disease recurrence, manifested as peritoneal metastases. Positive peritoneal cytology despite lack of visible peritoneal metastases was regarded as M1 class in the TNM classification (7th edition) in 2010. The aim of the study was to analyze factors associated with positive peritoneal cytology and identify groups of patients in whom diagnostic laparoscopy plus peritoneal lavage in the diagnostic process could affect therapeutic decisions. Material and methods. The study enrolled patients with gastric cancer who underwent surgical treatment at the Department of Surgery, Wielkopolskie Oncology Center in Poznań. During the laparotomy, after opening of the peritoneal cavity, 200 ml of physiological saline at 37°C was administered in the tumor region. After this fluid was mixed, 100 ml of lavage fluid was collected. This fluid was subsequently spun many times to obtain sediment for cytology and immunohistochemistry investigation using anti-BerEp-4, CK 7/20, and B72.3. Results of peritoneal cytology were analyzed jointly with clinical factors – patient’s age, sex and pathology factors – tumor invasion, involvement of lymph nodes, histological grade, histological type according to Lauren and localization of the cancer in the stomach. Results. Analysis of the peritoneal fluid for presence of free cancer cells was done in 51 patients. Positive peritoneal cytology was found in 12 (23.5%) patients. In the group of patients with positive cytology, all patients had T3/T4 tumors and all were found to have lymph node metastases, while G3 cancer was found in 83.3% of patients. In patients with positive cytology, diffuse gastric cancer according to Lauren predominated (9 of 12 patients, 75%), while in patients with negative cytology – intestinal type (20 of 39 patients, 51.2%). In the group of patients with positive histology, the whole stomach was involved by the cancer process in 7 of 12 patients (58.3%), while in the group with negative histology, in 29 of 39 patients the tumor was located in the gastric body and prepyloric part (74.4%). Conclusions. Based on this study we can conclude that determinants of positive peritoneal cytology include: tumor stage T3/T4, N+, G3, cancer located in the whole stomach, diffuse histological type according to Lauren.
