Opis:
Worldwide children’s walking disabilities that are caused mainly by cerebral palsy display multiple conditions of both permanent and non-developing motor dysfunction that in turn upsets posture, mobility, or muscle tone in general. In order to estimate the influence of instrumented gait analysis on the walking ability in cerebral palsied children the Dynaport Minimod, based on a triaxial accelerometer was employed. A Dynaport Minimod was used for collecting the spatial and temporal gait data. An evaluation of the gait data carried out via online services application for 80 children with cerebral palsies from April 1 st , 2018 to October 30 th , 2019. The Dynaport Minimod is capable of capturing most of the spatiotemporal gait parameters which indicate that this technique is quite cooperative and objective in the detection of gait changes and the evaluation of consequences, respectively. The most useful practice for the managing of children with cerebral palsy is a gait analysis system in which its clinical utilization is considered to be a developing technology especially in providing guidance to service planning centres and hospitals.

Celem pracy jest termowizyjna analiza powłoki heterotermicznej jednostronnie obciążonej górnej części ciała ludzkiego podczas chodu. Badaniem objęto grupę siedmiu mężczyzn w wieku od 21 do 27 lat. Maszerowali oni przez 10 minut z obciążeniem 5,5 kg w jednej ręce. Następnie wykonali tę samą czynność, ale obciążając drugą rękę. W celach badawczych zarejestrowane zdjęcia podzielono symetrycznie na obszary pomiarowe (symetryczne do płaszczyzny strzałkowej), odpowiednio w brzusznej i grzbietowej części. Zmierzono średnie temperatury tych obszarów, a następnie wyznaczono różnice temperatur odpowiadających sobie stron obciąŹżonej i nieobciążonej. W wyniku przeprowadzonej analizy wykazano znaczący wpływ asymetrycznego obciążenia ciała w czasie marszu na rozkład temperatury.
The purpose of this study is the analysis of the heterothermic shell of unilaterally loaded upper part of the human body, during a gait. The group of seven men, aged 21 to 27 years, were examined. They walked for 10 minutes with 5,5 kg load in one hand. Then, they performed the same task, however loading the other hand. In this study, the registered thermograms of human body were divided symmetrically into areas symmetrically to the sagittal plane, respectively for the abdominal and back side. The average temperatures of these areas were measured and the temperature differences of the corresponding sides loaded and unloaded, were determined. The significant influence of the asymmetric loading of the body during the walk on the temperature distribution, was revealed.

INTRODUCTION Sedentary style of life characteristic of present civilization causes the low back pain development at many people. The pain can become the reason of habitual incorrect motor pattern. The aim of this study was an assessment of gait of low back pain sufferers and healthy people. MATERIAL AND METHODS 92 adults (64 with above mentioned ailments and 28 healthy people) was tested. They estimated their noticeable pain and 3 D gait analysis test, on moving track with ZEBRIS Co. ultrasonic measurement system, was applied on them. The pelvic movement symmetry, step length, stance phase time and double support time was analyzed. RESULTS Gait asymmetry regarding especially pelvis movement in transversal plane has been observed at both low back pain patients and healthy persons. Pelvis rotation and double support asymmetry was greater at the pain sufferers. Correlation between pain feeling and gait asymmetry degree has not been observed. CONCLUSIONS At the essential group as well as at the control one little gait asymmetry occurred. Its greater level at the essential group was not directly connected with pain intensity.
WSTĘP Charakterystyczny dla współczesnej cywilizacji siedzący tryb życia powoduje u wielu ludzi rozwój zespołów bólowych kręgosłupa, które mogą stać się przyczyną nawykowych, nieprawidłowych wzorców ruchowych. Celem pracy była ocena chodu u ludzi z przewlekłymi zespołami bólowymi dolnego odcinka kręgosłupa na tle osób zdrowych. MATERIAŁ I METODY Badaniu poddano 92 dorosłe osoby (64 z wyżej wspomnianymi dolegliwościami i 28 zdrowych). Oceniły one stopień odczuwanego bólu oraz dokonano u nich trójpłaszczyznowej analizy chodu na bieżni ruchomej, z wykorzystaniem ultradźwiękowego systemu pomiarowego fi rmy Zebris. Analizie poddano symetrię ruchów miednicy, długości kroków, czasów podporu i podwójnego podporu pomiędzy obydwoma stronami ciała. WYNIKI Zaobserwowana została asymetria chodu, dotycząca zwłaszcza pracy miednicy w płaszczyźnie poprzecznej, zarówno u osób z bólami kręgosłupa jak i zdrowych. Asymetria rotacji miednicy i podwójnego podporu była większa w grupie osób skarżących się na ból, niż u osób zdrowych. Nie zaobserwowano korelacji pomiędzy odczuwaniem bólu, a stopniem asymetrii chodu. WNIOSKI Tak w grupie zasadniczej, jak i kontrolnej wystąpiła niewielka asymetria chodu. Większy jej poziom w grupie zasadniczej nie był bezpośrednio związany z nasileniem bólu.

The paper aimed to assess the gait pattern in children and adolescents with juvenile idiopathic arthritis (JIA) treated at the rehabilitation center and to assess changes in this pattern after the end of treatment and 9 months later. Methods: 50 children with JIA were enrolled into the study. 35 healthy volunteers were enrolled into the study for a comparison. Spatiotemporal and kinematic gait parameters were obtained using a movement analysis system. The Gait Deviation Index (GDI) was calculated. The assessment was performed three times: on the day of admission to the rehabilitation center, after the end of a 4-week treatment period and 9 months later. Results: With regard to the majority of spatiotemporal and kinematic parameters, differences in their distribution were highly statistically significant between the study group and the control group ( p < 0.001). In two subsequent tests, differences were less significant when compared to the control group ( p < 0.01). In the study group, ranges of motion in the sagittal plane in the hip ( p < 0.01), knee ( p < 0.001) and ankle joints ( p < 0.01) increased significantly between tests 1 and 2, and 1 and 3. A significantly lower value of GDI was observed in the study group (right limb; p = 0.036). Conclusions: The gait pattern of children with JIA is significantly different from the one observed in healthy children. A rehabilitation program significantly improved gait in children with JIA, but differences compared to healthy children were still observed, and it indicates that the abnormal gait pattern became permanent in this group of subjects.

The objective of this paper is to develop feasible gait patterns that could be used to control a real hexapod walking robot. These gaits should enable the fastest movement that is possible with the given robot's mechanics and drives on a flat terrain. Biological inspirations are commonly used in the design of walking robots and their control algorithms. However, legged robots differ significantly from their biological counterparts. Hence we believe that gait patterns should be learned using the robot or its simulation model rather than copied from insect behaviour. However, as we have found tahula rasa learning ineffective in this case due to the large and complicated search space, we adopt a different strategy: in a series of simulations we show how a progressive reduction of the permissible search space for the leg movements leads to the evolution of effective gait patterns. This strategy enables the evolutionary algorithm to discover proper leg co-ordination rules for a hexapod robot, using only simple dependencies between the states of the legs and a simple fitness function. The dependencies used are inspired by typical insect behaviour, although we show that all the introduced rules emerge also naturally in the evolved gait patterns. Finally, the gaits evolved in simulations are shown to be effective in experiments on a real walking robot.

Many pregnant women suffer from pelvic girdle pain (PGP) during pregnancy. Etiologies are multifactorial and affect the joint stability of the sacroiliac joint. Pelvic belts could restore stability and reduce pain during gait. The center of pressure (COP) is a reliable parameter to assess gait and balance. The objectives of this study were to analyze the COP during gait in pregnant women with PGP, to evaluate the effect of pelvic belts and to compare two types of belts on COP parameters. Materials and Methods: 46 pregnant women with PGP, 58 healthy pregnant women and 23 non-pregnant women participated in the study. The motor task consisted of three gait trials at different velocities on an electronic walkway. Two pelvic belts for pregnant women were used. An analysis of variance was performed to determine the effects of the progression of the pregnancy, gait speed, presence of pregnancy and occurrence of pain on the COP parameters. Results: Compared to the control group, pregnant women with PGP had a higher stance time, but COP displacement and velocity were lower. The COP parameters varied between pregnant women with and without pelvic girdle pain, the use of a belt during pregnancy decreased the walking velocity. No difference was found according to the type of belt. Conclusions: Differences in COP parameters during gait between pregnant women with or without PGP were minimal. Pelvic girdle pain did not affect the center of pressure. Wearing a belt during pregnancy modified the center of pressure velocity during gait in pregnant women with PGP.

Physical and hormonal modifications occuring during the pregnancy, can lead to an increase in postural instability and to a higher risk of falls during gait. The first objective was to describe the center of pressure (COP) during late pregnancy at different gait velocity. Comparison of nulliparous women with postpartum women were conducted in order to investigate the effects of pregnancy. The second objective was to analyse COP variability between pregnant and non-pregnant women in order to investigate the effects of regnancy on gait variability. Methods: Fifty-eight pregnant women in the last four months of pregnancy, nine postpartum women and twenty-three healthy non-pregnant women performed gait trials at three different speeds: preferred, slow and fast. Results: In the last four months of pregnancy gait velocity decreased. During the pregnancy, gait velocity decreased by 22%, stopover time increased by 6–12%, COP excursion XY decreased by 5% and COP velocity decreased by 16% and 20% along the anteroposterior and transverse axes, respectively. After delivery, gait velocity increased by 3% but remained a lower compared to non-pregnant women (–12%). Intra-individual variability was greater for non-pregnant than pregnant women. Conclusions: COP parameters were influenced by pregnancy. This suggests that pregnant women establish very specific and individual strategies with the aim of maintaining stability during gait.

The objective of this study was to describe spatial and temporal parameters during gait in pregnant women, and to compare it with women in post-partum and with a control group. Methods: To investigate alteration in natural locomotion, we used an electronic walkway (GAITRite system). Fifty-eight pregnant women (four last months of pregnancy), nine post-partum women and twenty-three healthy nulligravidae women participated in this study. The women performed the motor task at three different speeds: preferred, fast and slow. Spatial and temporal parameters for pregnant and non-pregnant were compared. Results: In pregnant women, gait speed, step length and cadence were reduced. Consequently, cycle time was longer. The gait cycle was modified by an increase of stance phase and a decrease of swing phase. As a result, an increase of double support and a decrease of single support phases were observed. Step width increased by 15%. Conclusions: The pattern of gait displayed significant modifications during pregnancy as compared to nulliparous women. These changes favour a more stable and safe gait. After childbirth, women kept some characteristics of gait in pregnancy during 8 months.

The aim of this study was to compare gait stability and variability between walking conditions and age groups. Methods: Twenty-six healthy younger and older females participated. Trunk acceleration in the vertical (V), medial-lateral (ML) and anteriorposterior (AP) directions during 5 minutes walking overground and 3 minutes walking on the treadmill at self-selected speed were recorded. Root mean square and standard deviations of acceleration, stride time and its variability, Lyapunov exponents (LE), multiscale entropy (MSE) and harmonic ratios (HR) were computed. Results: Both age groups showed significantly higher stride time variability and short-term LE in all directions during overground walking. For the older group, overground walking showed higher V and AP standard deviation. Significantly lower values for overground walking were observed for long-term LE (V and ML for the younger group, ML for the older group), HR (ML for the older group) and MSE (V for the older group). Significant age-related differences were found for V long-term LE for overground walking. Conclusions: The present findings suggest that both linear and advanced computational techniques for gait stability and variability assessment in older adults are sensitive to walking conditions.

Simulating the complexities of lower limb motion can be useful for orthosis or rehabilitation planning. The aim of this study was to develop an artificial neural network (ANN) able to accurately simulate the changes in the angle of the ankle, knee and hip joints during the gait cycle, then to use it to simulate the impact of a restricted range of ankle and hip joint angle changes on the progression of the knee joint angle. Methods: Thirty four young healthy students participated in the study. Gait kinematics data were collected using the Vicon system, then analyzed with an ANN. Results: We developed an ANN able to accurately simulate the progression of joint angles of lower-limb motion; its simulation of the impact of restricted ankle and hip joint angular ranges in the on the knee joint indicate that the braking phase is critical. Conclusions: ANNs offer a useful research method in clinical biomechanics. Further research in this vein should expand our understanding of compensatory functions in the lower limbs.

Although gait symmetry is being evaluated and reported in the literature with increasing frequency, there is still no generally accepted standard for assessing symmetry, making it difficult to compare studies and establish criteria to guide clinical decision-making. The purpose of this study was to ascertain whether gait symmetry in healthy subjects is consistent when assessed using various coefficients (RI, SI, GA, and SA), and if possible to identify a gait symmetry coefficient with the highest diagnostic utility. The study involved a group of 58 healthy university-level students of physical education and secondary school students aged 20.03 ± 0.97. Measurements of spatial-temporal gait parameters were conducted using the ZEBRIS platform. Our analysis supports existing recommendations that the symmetry index (SI) should be used as the most sensitive assessment of gait symmetry on the basis of spatial-temporal parameters in healthy subjects. Moreover, we developed normative values of individual features for diagnostic purposes.

Backward walking (BW) is a common technique employed in the treatment of a variety of orthopedic and neurological diseases. BW training may offer some benefits especially in balance and motor control ability beyond those experienced through forward walking (FW). The purpose of this study was to determine whether BW represented a simple reversal of FW and, hence muscle force distribution is the same. The study involved one male healthy student of physical education (22 years, h = 185 cm, m = 80 kg). Measurements of spatial-temporal gait parameters were conducted using eight Vicon system cameras, and Kistler plates. Noraxon EMG was used to obtain muscles activity. OpenSim software was used to compute muscle force distribution during both types of gait. During FW and BW there is small difference for force curves produced by m. gluteus maximus (RMS = 0.04), m. biceps femoris short head (RMS = 0.19) and m. tibialis anterior (RMS = 0.16). Good validation by EMG signal was obtained for m. rectus femoris, m. biceps femoris short head, m. tibialis posterior during FW and BW. For m. iliacus, only during BW good validation was achived.

W pracy przedstawiono wyniki symulacji analizy wytrzymałościowej z wykorzystaniem MES (Metod Elementów Skończonych) modelu stopy protezowej. Zaprezentowano wyniki analiz w postaci rozkładów map naprężeń zredukowanych wg hipotezy H-M-H (Hubera-Misesa-Hencky'ego).
The paper presents the results of stress analysis simulation using FEA (Finite Element Method) model of the prosthetic foot. The analysis 3D models (three-dimensional) made using CAD systems (Computer Aided Design) taking into account different boundary conditions resulting from some phases of gait. The results of the analysis in the form of reduced stress distributions maps by HMH hypothesis (Huber-Mises-Hencky).

Opis:
The aim of this study was to evaluate how wearing one or two orthoses influence the gait parameters in children with hemiplegia. Four children with hemiplegic cerebral palsy participated in the study. Patients were from 3 to 7 years old; one girl and three boys; one patient with right side hemiplegia, three patients with left side hemiplegia. Patients underwent the VICON MX system gait analysis three times (sessions): one with bare feet, the other two with shoes and orthoses: one with orthoses used unilaterally on palsied limb, one with orthoses used bilaterally. The following gait parameters were analyzed: velocity, cadence, pelvis tilt, range of pelvic movement in sagittal plane, separately for affected and non-affected side: step length, stance time (as a percentage of the gait cycle), range of hip movement in sagittal plane, knee flexion at initial contact, maximum knee flexion in swing phase. Results of the study showed which and how many gait parameters changed while wearing one in comparison with two orthoses. The results did not give a clear indication which condition is better: wearing one or two orthoses, however they suggest that when the orthotic devices are considered for patients with hemiplegic cerebral palsy, the objective, instrumented gait analysis can be used to take the best decision whether one or two orthoses should be prescribed.

The objective of this study was to compare the ground reaction forces (GRFs) and the multi-segment foot motion between individuals with plantar fasciitis (PF) and healthy controls. Methods: Twenty-one individuals with PF and 21 matched-case healthy controls who passed the criteria participated in the study. Gait data were assessed during their self-selected comfortable speeds by the 3D motion analysis system. The multi-segment foot motions were determined by the Oxford Foot Model. Outcome measures included the vertical and antero-posterior ground reaction forces (GRFs) and the multi-segment foot motions [the dorsiflexion (DF), plantarflexion (PF), inversion (Inv), eversion (Eve), adduction (Add), and abduction (Abd) peak angles for the forefoot with respect to hindfoot (FFHF) and the DF, PF, Inv, Eve, internal rotation (IR), and external rotation (ER) peak angles for the hindfoot with respect to tibia (HFTB) as well as their ranges (R)]. Results: Comparisons between individuals with PF and healthy controls showed no significant differences in any of the GRFs. Significant reductions were found in the FFHF-DF, FFHF-DF-R, FFHF-Inv, and HFTB-Inv/Eve-R in individuals with PF. In addition, there were tendencies of the increased angles of the FFHF-PF, HFTB-DF, HFTB-Inv, and HFTB-ER, but not significantly for individuals with PF, compared to healthy controls. Conclusions: Adaptations of the intra-foot motion showed the reduction of some angles but no change for the GRFs in individuals with PF compared to the healthy controls when both groups walked at a similar gait speed.
