Soft tissue defects of the lower extremity pose an important surgical challenge, often because of inviability of primary covering. Therapeutic modalities of choice comprise of local perforator flaps, as well as of free flaps created with the use of microsurgical skills. However, these methods cannot be applied in patients with important comorbidities, and it is in such cases where the reverse sural flap proves to be the best solution. The aim of the study was to present the results of treating lower extremity defects with various modifications of the reverse sural flap. Material and methods. Sixteen patients, males aged 17-56 years, were operated on from 2007 to 2013. Diabetes with multiple complications, disseminated atheromatosis, and extensive soft tissue trauma involving the arteries crucial for microsurgica anastomoses were the most common reason disqualifying from the free and perforator-based flap techniques. Results. All of the patients achieved very good results; in two cases distal and marginal flap necrosis was observed which, once exiced, covered with granulation, and was successfully covered with a partialthickness skin graft. Conclusions. The reverse sural flap, technically easy, offers a viable, low-risk alternative to free and perforator-based flaps.

The use of microsurgery for oral reconstruction of cancer patients, has become standard treatment in restoring oral function. The free anterolateral thigh flap is one of the most preferred options in reconstruction after total, subtotal or hemiresection of the tonque due to squamous cell cancer. The aim of the study was to present the reconstructive method using anterolateral thigh free flap with evaluation of quality of live. Material and methods. Clinical material includes 46 consecutive patients with tongue cancer, who underwent complex surgical treatment between 2009 and 2011. There were 36 males and 10 females and the M: F ratio was 3.6: 1. All of them were reconstructed using the anterolateral thigh free flap. The quality of life was evaluated 6 months after completing the treatment, based on postoperative functional and aesthetic status. Results. The overall flap survival rate was 96%. Surgical complications were observed in 8 patients (17%). Donor site was closed primarly in 42 cases and in remaining 4 skin graft was required. In all 46 cases understandable speech and return to unrestricted diet mastication and swallowing were achieved. The mean follow-up period after treatment was 32 months. Analysis of aesthetic effects evaluated in 23 cases and shows generally very good results. According to average transformed scores the QOL can be characterized as excellent for >90, very good for 76-90, good for 51-75, moderate for 25-50 and bad for <25 points. Conclusions. Anterolateral thigh flap, with its versatility in design, long pedicle with a suitable vessel diameter, low donor site morbidity, and very good aesthetic effects, could be the ideal flap for functional tongue reconstruction.

Dehiscence of a median sternotomy wound is a potentially devastating and life-threatening complication of cardiac procedures.Depending on the localization, extensiveness, and profoundness of the defect a variety of muscle flaps may be used to cover the frontal mediastinum, in particular: pectoralis major, rectus abdominisor latissimus dorsi. In spite of several options for restoration of sternal integrity we cannot avoid following serious local complications increasing patients morbidity. The aim of this paper is to report a method of sternal dehiscence management. Surgical technique and its results are presented on the example of two patients treated in Plastic, Reconstructive and Aesthetic Surgery Clinic, Medical University in Łódź because of sternal dehiscence after cardiovascular procedure with sternotomy. Our experience indicates that modified bilateral pectoralis major flap seems to be effective surgical method of sternal dehiscence treatment. Also it is worth to remember that surgical procedure in this complication should be performed as soon as possible to decrease patient’s disability and to avoid following complications

The extended latissimus dorsi flap (LD) is used in breast reconstruction since the 70's. LD flap is often used in corrective surgery in cases of unsatisfactory cosmetic results after breast-conserving therapy. In our department LD flap has several uses. In addition to free microvascular flaps - which applies in breast reconstructive surgery is routine, there are clinical situations where the use of pedicled LD flap is justified. The main indications for its use are: the inability to apply microvascular flap, the general condition (diabetes, advanced atherosclerosis), smoking, previous surgery of abdominal wall, abdominal obesity, patient preferences. Approximately 30% of patients after breast reconstruction require corrective procedures. Group which uses extended LD flap account for 24 patients. In 16 cases it was used for elective breast reconstruction. In the remaining eight cases it was used in the corrective procedures symmetry and shape of the previously reconstructed breast. Based on our own experience it can be concluded that the LD flap with an alternative to microvascular techniques. Complication rate when using the LD flap is relatively low and includes: seroma and slight motor disability of the shoulder girdle.

Opis:
Emergency full thickness abdominal wall defects constitute a challenging problem for surgeons. The aim of reconstruction is prevention of evisceration and hernia formation. Staged surgery is recommended in such cases. The first stage consists of wound healing with skin grafts and creation of "planned hernia". Ultimate reconstruction is delayed until good general health state of the patient. In presented case final skin and subcutaneous tissue reconstruction was achieved with island fasciocutaneous antero-lateral thigh flap. Reconstruction of musculo-aponeurotic layer was completed with compartment separation technique and synthetic mesh. During follow-up no recurrence of deformity was observed.

In various languages of the world, the so-called rhotic consonants arouse great interest of linguists. This is because these sounds are usually attributed to certain peculiar articulatory, acoustic, auditive and other characteristics. In addition, the functions they perform in phonological systems are, relatively frequently, quite complex. Spanish, in this aspect, turns out to be a specific language. Indeed, in addition to having two consonants of this type, it is a language where these sounds work in such a way as to provoke numerous descriptive, methodological and other problems. Hence, according to our perspective, Spanish can serve as a starting point for contrastive works within the framework of the rhotic consonants. For this reason, in this contribution our aim is to concisely present certain problems linked to the Spanish rhotic consonants indicating, at the same time, the features that differentiate them from the sounds that work in other languages that we have selected for this work.

Purpose of the study: The aim of the study was to present the treatment outcomes after vertical partial laryngectomy with or without pedunculated sternothyroid muscle flap reconstruction following the resection of neoplasm-infiltrated vocal fold. The procedure was used in a patient with glottic cancer. Oncological outcomes, morphology of neo-vocal fold and the act of swallowing were evaluated. Material and methods: 45 patients with T1-T2 glottic cancer were subjected to vertical partial laryngectomy with 26 patients undergoing a procedure with pedunculated sternothyroid muscle flap reconstruction and the remaining 19 patients undergoing a procedure without such a reconstruction. Two female and 43 male patients aged 35-82 years (mean age of 62.5 years) were enrolled in the study. Local tumor spread and the condition of reconstructed vocal fold were assessed in sequential videofiberoscopy examination conducted each month after surgery whereas the regional spread was assessed in ultrasound scans. Postoperative aspiration was graded according to the Pearson’s scale. Results: Six patients experienced local recurrence while 2 patients experienced regional recurrence of the tumor. The pedunculated sternothyroid muscle flap neo-fold was structurally resemblant of the non-affected vocal fold. Episodic, daily dysphagia was observed in 1 patient while normal act of swallowing with no Pearson’s scale symptoms was observed in the remaining 44 patients. No necrosis of pedunculated flap was observed. Conclusions: Vertical partial laryngectomy with or without pedunculated sternothyroid muscle flap reconstruction is a good method for the treatment of low- or intermediate-stage glottic cancer, especially when endoscopic access to the tumor is limited and when CO2 laser cannot be used. No significant functional disorders were observed in operated larynges.

The article presents investigation of flow around wing of TS-11 Iskra airplane. The flow visualization around 3D printed model of wing with flow control surfaces was performed in a water tunnel. Two configurations were investigated: first with a flap and second with an aileron. The flow visualisation was performed with a use of a dye. The geometry of model was prepared with use Computer Aided Design (CAD) software basing on scans of real object and technical documentation. The model was built with use of additive manufacturing technology. Movement of the flow control surfaces was remotely controlled with servomechanisms incorporated in channels inside the model. In order to perform qualitative validation of the results the investigated flow was simulated with use of CFD commercial software. The article presents visualisation results of flow around wing section of TS-11 Iskra airplane and water tunnel model preparation. In order to perform qualitative validation of the results the investigated flow was simulated with use of CFD commercial software. The goal of the research was to investigate the complex flow field in the vicinity of flow control surfaces and provide aerodynamic characteristics at various deployment angles via numerical simulations. The results can be used for verification of water tunnel testing procedures and training.

The article presents the positive results of the works performed at the Institute of Aviation in Warsaw, associated with modernization of a highly manoeuvrable training jet aircraft to make it reach the standards of increased manoeuvrability. It is shown how the maximum increase of a standard training aircraft manoeuvrability was achieved, which was the result of a small modification of the structure, along with the minimal costs of modernization and qualification tests. The paper also presents the basic process of modernization related to the development of iterative methodology, which includes numerical selection of mechanization elements, that is, strakes, slots and flaps. The parameters of the performed aerodynamic model are demonstrated as well as the results of the tests of its properties with the selected types of strakes conducted in a low-cost aerodynamic tunnel of 1.5 m diameter with the use of a sixelement aerodynamic weight. Parameters of the selected type of strake model with Fowler flap and slots are shown, as well as the results of the tests performed on it in the aerodynamic tunnel of 5 m diameter, and also the results of the buffeting resistance tests conducted in a transonic tunnel. The paper also presents the parameters of the designed optimized strakes, flaps and slots in 1:1 scale, which were mounted onto the aircraft prototype. Finally, the article presents the results of the full cycle of in-flight tests, which confirmed the anticipated project objectives and obtaining of a significant increase in the aircraft manoeuvrability by 48%.

The role of the anterolateral thigh flap (ALTF) in reconstructive microsurgery grows systematically from mid-eighties of the twenty century until now. Significant anatomic variability of the perforators supplying the ALTF was described in literature.The aim of the study was ultrasonographic assessment of the ALTF perforators in terms of localization, symmetry, diameter and flow velocity.Material and methods. The study was performed using ultrasound machine with 12 MHz linear transducer. Both thighs of 30 healthy volunteers (15 men and 15 women) aged from 18 to 60 (mean 37.9) were examined. The line from anterior superior iliac spine (ASIS) to lateral border of the patella was traced, and divided into 10 equal segments. Point where perforator pierces the deep fascia was marked and its diameter was measured and recorded. Maximal flow velocity was measured and recorded. Symmetry of perforator location was confirmed as positive if difference in position of two perforators on both thighs was less than 1.5 cm in diameter.Results. Total number of 119 perforators supplying skin of 60 thighs was found (mean 1.98 perforator per thigh). No perforators were found in 4 thighs (6.6%). Perforators were most abundant in segments from 5 to 7 (74.6%). Perforators with largest diameters and maximal flow velocity reaching 30-47 cm/s were localized in segments 5 and 6. Perforators in segments 4 and 5 were more abundant in men (50.9%) than in women (36.7%). Septal perforators make up to 26.9% of the total. Eighty percent of the septal perforators were localized in segments 5 and 6. Perforator distribution was elicited in the middle of the ASIS - patella line. In the range of ±1.5 cm from the midpoint of the line 33.6% of the perforators were found.Conclusions. 1. Most of perforators with large diameter and big flow velocity are located in segments 5 and 6. 2. Perforators are more common in men in segments 4 and 5 than in women. 3. No perforators found in 4 thighs suggests that preoperative perforator mapping should become a preoperative routine, which can spare intraoperative dilemmas.

Degloving injury consists in tearing out the soft-tissue integument from skeleton of the hand, with accompanied nerves and vessels. The whole hand degloving has bad reputation and one of worst prognosis, even worse than total hand amputation. The range of possible salvage procedures in these cases is limited and their outcomes are unsatisfactory. One of the suitable methods is wrapping the skinned hand with pedicled or free greater omentum flap, retrieved from the abdominal cavity. The article reports outcomes of the treatment of 5 patients at a mean of 8 years after total degloving of their hands and coverage with omental flaps. All flaps healed uneventfully, but in none of the patients the whole length of the fingers was preserved. Division of stumps of 3 fingers was possible in one patient, two others had three-digital hands and remaining two had only separated thumb. Dexterity of injured hands was limited with a mean of score DASH questionnaire of 43 points. Quality of life as measured by SF-36 questionnaire was fair (58 and 53 points in physical and mental domain, respectively). Regardless this, all patients were satisfied with achieved outcomes and all returned to work, which was a confirmation of the effectiveness of the method used in their treatment.

CBCT is a relatively new diagnostic imaging method. It was first used in 1982 at the Mayo Clinic Biodynamics Research Laboratory [1], and has been commercially used in dentistry since 2001. It differs from multidetector computed tomography in terms of implementation technique and some parameters. A low dose of radiation is its greatest advantage, thanks to which many specialists, including ENT doctors and reconstructive surgeons, are more willing to use it. In otorhinolaryngology, CBCT is applied in pre- and perioperative diagnostics in both rhinology and otology. In reconstructive surgery, CBCT facilitates precise planning of the flap and intraoperatively allows a perfect match of the reconstructed tissue elements. In the article, the authors present current information on CBCT in ENT obtained on the basis of a review of Polish and foreign literature and share their own experience in its application in reconstructive surgery within the craniofacial region.

The paper presents a modelling procedure of the M-28 Bryza wing with extended flaps and Computational Fluid Dynamics (CFD) simulations results preformed in order to assess the performance of the investigated wing for various flap extension rates. The M-28 Bryza is a two-engine high-wing aircraft used in the Polish Air Force for short distance airlift of people and equipment. The aim of this work was to determine the aerodynamic characteristics of the investigated wing. The CFD simulations were performed in order to investigate the influence of flap extension on lift and drag coefficients at various angles of attack. In order to validate the results of two different commercial CFD packages were used. The findings are presented in the form of flow visualization and aerodynamic characteristics. The typical and non-standard extension rates were investigated for low, moderate and high angles of attack. The results correlated with the limitations specified in the aircraft manual. The outcome of the presented work confirmed the feasibility of the presented methodology for its use as a supportive tool for providing additional information about airplane performance in standard and non-standard conditions of flight such as landing with one engine working. The results of work might prove useful for M-28 Bryza pilots and maintenance personnel as well as for educational purposes.

Opis:
Extensive tissue defects resulting from tumor resection within the head and neck require simultaneous reconstruction. One of the most commonly used free flaps in reconstruction of the oral cavity is free anterolateral thigh flap. The work describes the case of a 57-year-old woman with squamous cell carcinoma of the cheek, who underwent reconstruction of full thickness of the cheek with a folded free anterolateral thigh flap. The authors provide treatment methods and the possibility of using ALT flap.
