Introduction. Bone trace elements levels including nickel (Ni) depend on biological and environmental factors: age, gender, remodeling state, exposure to occupational and environmental pollution, smoking, diet, and surgical implants. Objective. The aim of this study was to determine Ni concentrations in bones of femur heads obtained from patients with osteoarthritis. Materials and method. A total of 111 samples of hip joint bones (cartilage, cancellous bone and compact bone) collected from patients from north-western Poland were examined. Ni concentration was determined by ICP-AES (atomic absorption spectrophotometry). Results. It was found that differences in Ni concentrations were statistically significant between cartilage and compact bone. Cartilage Ni concentrations were higher in patients with fractured femur neck than patients with osteoarthritis. Furthermore, higher Ni levels were also found in samples obtained from smokers compared to nonsmokers. In cancellous bone, higher Ni concentrations were found in samples from patients who had received implants than those without them. Moreover, higher Ni concentrations in cancellous bone were found in patients occupationally exposed to heavy metals than in those not exposed. Conclusions. The bone tissue reflects long-term exposure to Ni and may be used as a bioindicator to study the process of Ni accumulation in the human body.

Objective. The aim of the study was to assess the impact of a fattening season on the level of airborne chemical and microbial pollutants in the pig fattening house. Materials and method. Evaluation of the air composition as well as the microbiological air quality were made during 2 series, i.e. in the summer and winter period of fattening. Airborne gaseous pollutants were analyzed by gas (organic compounds) and liquid (inorganic compounds) chromatography methods. Air microbial contamination was determined in compliance with the Polish Standard PN-EN 13098:2007. Results. The air/gas composition in the pig unit showed that irrespective of the fattening season, the level of determined pollutants – especially ammonia, volatile organic compounds (VOCs), including methanethiol and total dust – could have an adverse effect on the health of swine facility workers. The investigated pig building was found to be a source of chemical pollutants as well as biological agents. The total bacterial count in 1 m3 of air was 3.6×106 and 4.6×106 cfu, during the winter and summer periods of fattening, respectively. Gram-positive cocci predominated in both seasons. In the winter, fungi of the Trichoderma genus were represented equally often. In the summer period, up to 90% of the isolated fungus was Aspergillus spp. Conclusions. Workers engaged in swine rearing and production are occupationally exposed not only to a high concentration of microbial contamination, but also to irritating and odorogenous volatile compounds. Thus, activities directed at the promotion of health prevention strategies, as well as effective guidelines for safe and hygienic animal housing, should be implemented.

Introduction and objective. Brittany is the leading agricultural production region of France where many workers are exposed to the local flora and fauna. The aim of the study is to describe the different skin diseases found among the workers occupationally exposed to the terrestrial fauna and flora in Brittany, and to assess the possible over-representation of some work environments. Materials and methods This was a retrospective, descriptive study carried out between 1 October 2002 – 30 April 2017, based on the coding of files from a multi-disciplinary occupational dermatology clinic at the Brest University Hospital Results. The study population of 84 patients was drawn from a total 788 patients seen during this period. Allergic contact dermatitis was the most frequently coded diagnosis with 35 cases (42%). There were 13 cases of irritant contact dermatitis (15%). Infectious diseases were poorly represented with only 5 cases (6%).The 6 most frequently represented occupations were packers in the agri-food industry (12.0%), cattle farmers and tomato greenhouse workers (both 10.7%), maintenance workers of parks and gardens (9.5%), agri-food factory cleaning operatives (8.3%) and pig farmers (7.1%). Conclusions.There was a marked incidence of allergic contact dermatitis and irritative contact dermatitis, mostly occupational in origin. While no specific diseases were found among these workers, some of the allergens and irritants identified may be specific to certain work environments. Overall, there did not seem to be a major risk of occupational dermatoses among workers exposed to Brittany’s terrestrial fauna and flora in this population, compared with other professional sectors, such as hairdressing or care work. The incidence rate of the cases of occupational skin diseases in this population is assessed from 3 – 5 cases per 100,000 workers.

Lead is an environmental contaminant. The majority of epidemiological research on the health effects of lead has been focused on children, because they are more vulnerable to lead than adults. In children, an elevated blood lead (B-Pb) is associated with reduced Intelligence Quotient (IQ) score. This paper summarizes the current opinions on the assessment of the health risk connected with the children’s environmental exposure to lead. The B-Pb level of concern of 100 μg/l proposed by the US Centers of Disease Control in 1991 was for a long time accepted as the guideline value. In the meantime there has been a significant worldwide decrease of B-Pb levels in children and present geometric mean values in the European countries range from 20 to 30 μg/l. The recent analyses of the association of intelligence test scores and B-Pb levels have revealed that the steepest declines in IQ occur at blood levels < 100 μg/l and that no threshold below which lead does not cause neurodevelopmental toxicity can be defended. European Food Safety Authority (EFSA) concluded in 2010, on the basis of results of Benchmark Dose (BMD) analysis, that an increase in B-Pb of 12 μg/l (BMDL₀₁) could decrease the IQ score by one point. It seems that this value can be used as a “unit risk” to calculate the possible decrease of IQ and, consequently, influence of the low-level exposure to lead (< 100 μg/l) on the health and socioeconomic status of the exposed population.

The rising prevalence of overweight and obesity in children and teenagers is a major challenge for public health. Obesity is a complex and heterogeneous disorder, affected by many interacting genetic and non-genetic factors. The aim of this article was to focus on the environmental risk factors for childhood obesity. Among different factors contributing to an increase in BMI, we highlighted the role of exposure to cigarette smoke, DDT, bisphenol A, pesticides, and noise. The correlation between exposure to environmental toxins during prenatal period and obesity development in later life was underlined. According to obesogenic environment hypothesis, some features of distal and proximal neighbourhood also have a pivotal impact on children's behaviour and may contribute to increasing the risk for overweight. The area of residence (urban or rural) may affect access to sports facilities or other opportunities for physical activity. Therefore, for designing adequate prophylaxis, it is essential to take into account modifiable risk factors present in residential neighbourhood. Prevention of childhood obesity should integrate activities for both micro- and macro-environment surrounding the child.
Wzrastająca częstość występowania nadwagi lub otyłości u dzieci i nastolatków stanowi poważne wyzwanie dla zdrowia publicznego. Otyłość to złożone i heterogenne zaburzenie wynikające z wielu interakcji pomiędzy czynnikami genetycznymi i pozagenetycznymi. Celem artykułu było skupienie się na środowiskowych czynnikach ryzyka dziecięcej otyłości. Spośród różnorodnych czynników przyczyniających się do wzrostu BMI na- świetliliśmy rolę ekspozycji na dym tytoniowy, DDT, bisphenol A, pestycydy oraz hałas. Podkreślony został związek pomiędzy narażeniem na środowiskowe toksyny w okresie prenatalnym a rozwojem otyłości w późniejszym życiu. Zgodnie z hipotezą obesogennego środowiska niektóre cechy dalszego i bliższego sąsiedztwa mają bardzo ważny wpływ na zachowania dzieci i mogą przyczynić się do wzrostu ryzyka nadwagi. Miejsce zamieszkania (wieś lub miasto) może warunkować dostępność do obiektów sportowych czy innych możliwości wykonywania aktywności fizycznej. Dlatego też, niezwykle ważnym jest uwzględnienie przy projektowaniu odpowiedniej profilaktyki modyfikowalnych czynników ryzyka obecnych w lokalnym środowisku. Prewencja dziecięcej otyłości powinna integrować działania na rzecz mikro- i makrośrodowiska otaczającego dziecko.

Introduction. In animal models, gestational exposure to inorganic arsenic has been associated with higher corticosterone concentration and consequent impairment of stress control in offspring. An equivalent association relating cortisol, a glucocorticoid hormone, in humans has not been previously studied. Objective. The aim of the study was to explore the association between prenatal inorganic arsenic exposure and salivary cortisol in infants from Arica, Chile. Materials and method. A cohort study of 168 mother-child dyads was recruited. In the 2nd trimester of pregnancy, urinary inorganic arsenic was assessed; 18–24 months after delivery, salivary cortisol was measured in the children. Maternal cortisol, maternal depression, stress, and socio-economic status were also evaluated. Results. The adjusted association was estimated with multiple linear regression after evaluating confounding through a directed acyclic graph. Median urinary inorganic arsenic in pregnant women was 14.1 μg/L (IQR: 10.4–21.7) while salivary cortisol in the children was 0.17 μg/L (IQR: 0.11–0.38). Among children from the highest income families (> 614 USD/month), arsenic exposure was associated with salivary cortisol. Children in the third quartile of arsenic exposure had -0.769 units of the logarithm of salivary cortiso, compared with those in the first quartile (p = 0.045). Conclusions. In this sample, prenatal exposure to arsenic was associated with salivary cortisol (third quartile of inorganic arsenic), only in infants belonging the highest income strata (> 614 USD). More studies are needed to confirm these preliminary results.

Objectives: Epidemiologic studies suggest an association between environmental exposure to benzene and hematologic cancers, but the relationship is not fi rmly established. The aim of this study was to assess the potential association between residence near hazardous waste sites containing benzene and hospitalization discharge rates for persons having hematologic cancers. Materials and Methods: We determined the number of hospital discharges of people with hematologic cancers in New York State except for New York City for the years 1993 to 2008. Descriptive statistics and negative binomial regression models were used to compare the rates of hospitalization of residents in zip codes containing hazardous waste sites containing benzene to the rates of discharges from residents in zip codes without waste sites. Results: When adjusting for potential confounders we found a 15% increase in the rate of hospitalization for chronic lymphatic leukemia (CLL) [rate ratio (RR): 1.15; 95% confi dence interval (CI): 1.00–1.33], a 22% increase in the rate of discharges for total leukemia (RR: 1.22; 95% CI: 1.04–1.43) and a 17% increase in the rate of discharges for total lymphoma (RR: 1.17; 95% CI: 1.02–1.35) in the benzene exposed sites. We found greater effects of exposure in African Americans compared to Caucasians, females compared to males and people with higher socioeconomic status (SES) compared to those with lower SES for several of the diseases studied. Conclusions: After controlling for major confounders we found statistically signifi cant increases in discharge rates for several hematologic cancers in persons residing in zip codes containing benzene waste sites. These results provide additional support for a relationship between environmental exposure to benzene and risk of hematologic cancers.

A network of pediatric environmental health specialty units (PEHSUs) in the United States was formed in 1998 out of a recognized need for clinical expertise in children’s environmental health. Documented trends in a rise of pediatric diseases caused or exacerbated by environmental conditions, coupled with the failure of medical schools and residency programs to cover these issues in a significant way, leaves health care providers, parents, communities, and governments at a loss for this specialized knowledge. The PEHSUs fill this gap by providing: 1) medical education, 2) general outreach and communications, and 3) consultative services to communities and health care professionals. This paper presents examples of key situations where PEHSU involvement was instrumental in improved patient outcomes or advancing clinical expertise in children’s environmental health. Challenges and opportunities for future directions for the program are also discussed.
Sieć Pediatrycznych Specjalistycznych Poradni Medycyny Środowiskowej (PEHSU) w Stanach Zjednoczonych została utworzona w 1998 roku w wyniku uznania konieczności przeprowadzania ekspertyz w zakresie zdrowia środowiskowego dzieci. Udokumentowane trendy wzrostu chorób dzieci wywołanych lub zaostrzonych przez czynniki środowiskowe i nakładający się równocześnie niedostatek programów nauczania z tego zakresu w szkołach medycznych lub w czasie rezydentury spowodowały brak tej specjalistycznej wiedzy wśród pracowników opieki zdrowotnej, rodziców, społeczności i władz. PEHSU wypełniają ten brak wykonując: 1) edukację medyczną, 2) promocję zdrowia środowiskowego 3) porady konsultacyjne dla społeczności i dla pracowników ochrony zdrowia. W niniejszej pracy przedstawiono najważniejsze przykłady, kiedy udział PEHSU był instrumentem dla polepszenia stanu zdrowia albo postępu ekspertyzy klinicznej w zakresie zdrowia środowiskowego.

Objectives: The main objective of the study was to determine the levels of house dust mite (Der p1), dog (Can f1), cat (Fel d1) and cockroach (Bla g2) allergens in kindergartens localized in an urban agglomeration. Material and Methods: A quantitative analysis of allergens was carried out in settled dust samples collected by vacuuming the floor surface in three kindergartens (N = 84) and children's clothing (N = 36). The samples were collected in springsummer and autumn-winter periods as well as at the beginning and end of the week. The allergen dust concentration was determined by enzyme-linked immunoenzymatic assay (ELISA). Results: The mean geometric concentrations (±geometric standard deviations) of allergens Der p1, Can f1, Fel d1 and Bla g2 determined in kindergartens were: 0.02±3.21 μg/g of dust; 0.97±4.49 μg/g of dust; 0.30±4.43 μg/g of dust and 0.01±3.08 μg/g of dust, respectively. Younger classrooms (children aged from 3 to 4 years) were characterized by almost twice higher mean concentration of allergen Fel d1, as compared to older classrooms (children aged from 5 to 6 years) (p < 0.05). A significant impact of seasonality on the level of dog allergen Can f1 was found (p < 0.05). No significant weekly variation was found in average concentrations of the allergens. Children who had a dog and/or cat at home were characterized by high concentrations of allergens Can f1 and Fel d1 on their clothes (59.2±5.39 μg Can f1/g of dust; 3.63±1.47 μg Fel d1/g of dust), significantly higher than concentrations of allergens in children who did not have any pets (p < 0.001). Conclusions: Special attention should be paid to keeping the kindergarten rooms tidy and clean and to an appropriate choice of furnishings and fittings which would prevent the proliferation of the house dust mite and accumulation of allergens.

This review is aimed at summarizing the current state of knowledge about the relationship between environmental exposure to the bioaerosol emitted by intensive livestock farming and changes in the microbiome of people living in livestock farm vicinity. The PubMed, Scopus and Web of Science databases were searched by crossing keywords from the following 3 groups: a) “livestock,” “animal farms,” “animal breeding”; b) “microbiome,” “resistome”; c) “livestock vicinity,” “farm vicinity,” “neighborhoods and health” in 2010–2022. Literature screening did not reveal any paper related to the full microbiome composition in the population studied. In the study, the authors included 7 papers (5 from the Netherlands, 1 from the USA, and 1 from China). The studies confirmed the carriage of Staphylococcus aureus, including methicillin-resistant S. aureus (MRSA), livestock-associated MRSA (LA-MRSA MC398) and multidrug-resistant S. aureus (MDRSA) in the nasal microbiome of adults and children living within 500–2000 m from a livestock farm. Clostridium difficile, including LA-ribotype RT078 carriage, was detected in the intestinal microbiome of adults living within 500–1000 m. Extended-spectrum β-lactamase (ESBL) producing Enterobacteriaceae were confirmed in the intestinal microbiome of adults living within 500–6200 m. Knowledge on the composition of the microflora of people living in livestock farm vicinity is insufficient to conclude about changes in the microbiome caused by the environmental emission of bioaerosol. The carriage prevalence of the LA-bacteria, including both strains with antimicrobial resistance and antimicrobial resistance genes, confirms the presence of zoonotic bacteria in the human microflora in populations without occupational contact with animals. It cannot be ruled out that zoonotic bacteria, as a component of the microbiome, have a negative impact on people’s health.

The essential elements of the model are genetic and behavioral determinants of susceptibility, progression of the disease from precursor lesions through early (localized) tumors to disseminated disease, detection by various modalities, and medical intervention. Using model estimates as a foundation, mortality reduction caused by early-detection and intervention programs can be predicted under different scenarios. Genetic indicators of susceptibility to lung cancer should be used to define the highest-risk subgroups of the high-risk behavior population (smokers). The calibration and validation of the model requires applying our techniques to a variety of data sets available, including public registry data of the SEER type, data from the NCI lung cancer chest X-ray screening studies, and the recent ELCAP CT-scan screening study.

Policies can influence health of a population in various ways. Numerous epidemiological studies supported by toxicological investigations demonstrate a positive association between ambient concentrations of airborne particulate matter and increased adverse cardio-respiratory events, including morbidity and mortality. The aim of this paper was to present the concept of the top-down health policy risk assessment approach model developed to estimate the expected health risk reduction associated with policy aiming at attaining the new particulate matter ≤ 10 μm in diameter (PM 10) standards in Poland. The top-down approach guides the analysis of causal chains from the policy to health outcomes. In this case study we tried to estimate the predicted health effects of the policy change over the past 20 years. Since Polish annual standard for PM 10 changed from 50 μg/m³ in 1990 to 40 μg/m³ in 2010, we calculated the relative risk associated with decreasing PM 10 in diameter to 10 μg/m³ in the annual level of PM 10 for 6 adverse health effects. The relative risk slightly decreased for almost all adverse health effects, which means that the relative decrease in the incidence of health effects from the baseline incidence should range from about 0.5–0.6% for heart disease admissions to > 1% for respiratory admissions. The obtained results indicate that implementation of the new ambient air standards could influence improvement of the health status of Polish population. A top-down policy health risk assessment model can be one of the main tools in this process, providing harmonized guidance how to seek evidence-based information, which could serve policy-makers.

Problemy wynikające z gromadzenia się w środowisku plastikowych odpadów stały się globalne. Apele o zaprzestanie wykorzystywania jednorazowych słomek do napojów czy plastikowych sztućców nie pojawiły się bez powodu – rocznie produkuje się 320 mln ton wyrobów plastikowych, z których 40% to przedmioty jednorazowego użytku. Coraz więcej państw i prywatnych przedsiębiorstw rezygnuje z przedmiotów plastikowych na rzecz ich biodegradowalnych zamienników, np. tekturowych słomek do napojów. W środowisku plastikowe odpady podlegają wielu oddziaływaniom fizykochemicznym oraz biodegradacji, w której biorą udział bakterie. Bytując na odpadach syntetycznych, powodują zmniejszenie ich rozmiarów i zwiększają ich dyspersję w środowisku. Małe, niewidoczne gołym okiem cząstki plastiku noszą nazwę nanoplastiku. Nanoplastik nie jest obojętny dla organizmów żywych. Z uwagi na swoje rozmiary jest pobierany wraz z pokarmem przez zwierzęta i przekazywany w łańcuchu troficznym. Zdolność nanoplastiku do przenikania barier organizmu indukuje efekty biologiczne o rozmaitych skutkach. Wiele ośrodków prowadzi badania na temat nanoplastiku, jednak ich wyniki wciąż stanowią ułamek danych potrzebnych do jednoznacznego wnioskowania o jego wpływie na organizmy żywe. Brakuje także danych dotyczących bezpośredniego narażenia na zanieczyszczenie nanoplastikiem w miejscach pracy, szkołach i miejscach użyteczności publicznej, norm opisujących dopuszczalne stężenie nanoplastiku w produktach spożywczych i wodzie pitnej oraz badań in vitro na nanocząstkach innych niż polistyrenu. Uzupełnienie dostępnych danych pozwoli obiektywnie ocenić zagrożenia płynące ze strony ekspozycji organizmów na nanoplastik. Med. Pr. 2020;71(6):743–756
Problems arising from the accumulation of plastic waste in the environment have become global. Appeals to stop the usage of disposable drinking straws or plastic cutlery did not come out without reason – 320 million tons of plastic products are produced annually, of which 40% are disposable items. More and more countries and private enterprises are giving up these types of items in favor of their biodegradable substitutes, e.g., cardboard drinking straws. Plastic waste in the environment is subject to a number of physicochemical interactions and biodegradation in which bacteria are involved. By using synthetic waste, they reduce the size of plastic garbage while increasing its dispersion in the environment. Small plastic particles, invisible to the naked eye, are called nanoplastic. Nanoplastic is not inert to living organisms. Due to its size, it is taken up with food by animals and passed on in the trophic chain. The ability to penetrate the body’s barriers through nanoplastic leads to the induction of biological effects with various outcomes. Research studies on the interaction of nanoplastic with living organisms are carried out in many laboratories; however, their number is still a drop in the ocean of the data needed to draw clear-cut conclusions about the impact of nanoplastic on living organisms. There is also no data on the direct exposure to nanoplastic contamination at workplaces, schools and public utilities, standards describing the acceptable concentration of nanoplastic in food products and drinking water, and in vitro tests on nanoparticles other than polystyrene nanoparticles.
