An exemplary rare case of neurogenic pulmonary edema induced by intracranial hemorrhage was reported including diagnostic and therapeutic implications as well as management recommendations. A 35-year old man who was treated first by a neurosurgical approach because of a subarachnoid hemorrhage (bore hole trepanation) and subsequently on a surgical intensive care unit because of severe postoperative hemodynamic, cardiocirculatory, and pulmonary disruptions. To monitor cardiopulmonary condition and treatment effects, a Swan-Ganz catheter was placed in the pulmonary artery, since after trepanation, a critical cardiopulmonary status developed during postoperative mechanical ventilation and catecholamine administration. This condition was indicated by neurogenic pulmonary edema detected by control chest X-ray film and high oxygen load in the inspiratory air required for sufficient arterial oxygenation. After use of high positive end-exspiratory pressure (PEEP) (initially directed against neurogenic lesion), adaptation of initial dobutamine doses, initiation of norepinephrine administration, and substitution of fluids, the patient's blood pressure finally rose sufficiently to sustain regular cerebral blood perfusion and achieve better arterial oxygenation. Thus, the patient‘s cardiopulmonary condition stabilized and temporary cardiac insufficiency could be overcome. Subsequently, it became possible to decrease PEEP according to requirements to prevent or limit cerebral edema and to diminish catecholamine doses.

Introduction. Artificial intelligence is increasingly being used in the medicine, particularly in radiological diagnosis of diseases such as an axial spondyloarthritis (axSpA). The aim of this study is to compare the available algorithms designed to detect active sacroiliitis in patients with axSpA. Material and methods. Four algorithms, two semi-automated and two full-automated for the assessment of bone marrow edema (BME) on magnetic resonance imaging (MRI) of the sacroiliac joints (SIJs) were included in the study. They were described and compared in terms of specificity, sensitivity, and correlation of BME detection findings between AI and experts. Analysis of the literature. Among all automated algorithms, the one created by Bressem et al. had the highest number of examinations analyzed in the study, involving 593 MRIs of SIJs. The sensitivity and specificity, as well as the correlation between the AI’s detection of BME versus manual, were not calculated for each algorithm. Rzecki’s algorithm had the greatest sensitivity and specificity for BME detection reaching 0.95 and 0.96, respectively. In addition, its Speraman’s coefficient of correlation between manual and automated measurements was 0.866. Conclusion. Each of described algorithms is certainly useful in assessing BME in the MRI examinations of SIJs.

Using paw edema acute inflammatory model induced by carrageenan (1%) in Wistar rats, the immunoregulatory action of Lactobacillus sp., isolated from two locally fermented food products in Nigeria: Nunu (a yogurt-like milk product) and Ogi (guinea corn slurry), was investigated. The rats were distributed into seven groups (A–G). Rats in group A did not receive any therapy or carrageenan inflammation, whereas those in group B received a carrageenan injection only. Groups C–F were orally administered with lactic acid bacteria (LAB) strains (5 × 107 CFU/ml), whereas group G received diclofenac sodium (150 mg/kg body weight) following the administration of carrageenan. At regular intervals, paw thickness (mm) was measured. Microscopy was used to count the number of leukocytes; myeloperoxidase (MPO) activity was used to measure the neutrophil accumulation in the paw tissue; and rat serum samples were subjected to ELISA to identify cytokine assays for C-reactive protein (CR-P), interleukin-10 (IL-10), and transforming growth factor-β (TGF-β). All of the LAB-treated groups showed a statistically signiWcant decrease in paw thickness, and their neutrophil and monocyte infiltration was significantly affected. Compared with the control groups, oral administration with LAB significantly suppressed the MPO activity. Lactobacillus fermentum NBRC showed the most significant upregulation of serum levels of IL-10 and TGF-β though serum levels of CR-P were downregulated. Lactobacillus pentosus increased the production of TGF β, with no significant effect on the production of IL-10. This study presents the role of Lactobacillus sp. in regulating inflammation by modifying the production of anti-inflammatory cytokines IL-10 and TGF-β.

Opis:
Portable hyperbaric chambers are a therapeutic option for altitude disease in the mountains. By an increase of the oxygen partial pressure the patient’s symptoms ameliorate significantly. Portable hyperbaric chambers may be used as ‘stand alone therapy’ as well as in combination with other therapeutic options. Here we present the recommendation how to use the device and how to avoid problems. The recommendation has been agreed by all members of the medical commission of the world umbrella organization Union Internationale des Associations d’Alpinisme (UIAA MedCom).

Ethylene phosphorodifluoridite (C2H4F4O2P2) (CAS No. 3965-00-2) is a colorless corrosive fuming liquid that is used as a stabilizer in the electrolyte of rechargeable batteries. There are no previous reports of toxic effects following exposure to this compound. A 28-year-old male complained of respiratory distress after accidental inhalation of ethylene phosphorodifluoridite for 30 min. The patient developed acute lung injury with noncardiogenic pulmonary edema and was treated with supportive management. The patient fully recovered and was discharged after 7 days without any significant sequelae. The patient’s symptoms were attributed to non-cardiogenic pulmonary edema caused by ethylene phosphorodifluoridite exposure. The case study showed that ethylene phosphorodifluoridite should be added to the list of chemicals that can cause acute lung injury.

Objectiv. To evaluate the efficacy of subthreshold micropulse yellow (577 nm) laser photocoagulation in diffuse macular edema (DME) in rural and urban patients of south-eastern Poland. Materials and method. Seventy-five eyes of 75 patients with diffuse DME were treated with subthreshold micropulse yellow laser photocoagulation with a 5% duty cycle at an energy level. The laser exposure time was 20 ms and the spot diameter was 100 μm. Best corrected visual acuity (BCVA), reading vision (Snellen) and optical coherence tomography-determined central retinal thickness (CRT) were estimated before and 2, 4 and 6 months after laser treatment. There were no statistically important differences in: the advancement of DME, HbA1c (glycated hemoglobin) level, duration of diabetes mellitus (DM), the degree of vision damage between rural and urban patients. Results. The follow-up was 6 months later. The baseline BCVA was 0.20 and remained stable- 0.3 after 6 months. The Snellen at baseline was 1.0 and improved to 0.5 finally (p=0.0004). The CRT at baseline was 500 μm and changed to 346 μm (p=0.00000) at the final follow-up. Finally, no retinal damage was observed. Conclusions. Place of residence had no statistically significant effect on the demographics data, baseline visual acuity, reading visual acuity and central retinal thickness. Subthreshold micropulse yellow laser showed a highly significant efficiency in the treatment of DME. The effects of the treatment were more significant in rural patients than in urban ones.

Diabetic macular edema (DME) is the dominant reason of diabetic visual loss, so early detection and treatment of DME is of great significance for the treatment of diabetes. Based on transfer learning, an automatic classification method is proposed to distinguish DME images from normal images in optical coherence tomography (OCT) retinal fundus images. Features of the DME are automatically identified and extracted by the pre-trained convolutional neural network (CNN), which only involves fine-tuning the VGGNet-16 network without any user intervention. An accuracy of 97.9% and a sensitivity of 98.0% are acquired with the OCT images in the Duke data set from experimental results. The proposed method, a core part of an automated diagnosis system of the DME, revealed the ability of fine-tuning models to train non-medical images, allowing them can be classified with limited training data. Moreover, it can be developed to assist early diagnosis of the disease, effectively delaying (or avoiding) the progression of the disease, consequently.

Introduction and aim. Edema and pain may occur in the lower extremities due to the long-term standing work of nursing students and the stress they experience. In this study, the aim was to determine the effect of foot baths applied after clinical practice on foot pain and leg edema in nursing students. Material and methods. This randomized controlled study was carried out with second year nursing students. The intervention group received a foot bath (including immersion in water and massage). No intervention was performed on the control group. The data was collected using a survey form, edema follow-up form, visual analog scale, and foot bath equipment. Results. Edema levels in the right tibia decreased statistically on the 1st and 2nd days after the foot bath compared to the values measured after nurses performed clinical practice, while the edema level in the left tibia decreased significantly in the first and fourth weeks (p<0.05). The right and left foot pain scores in the intervention group increased in the evening of the day after the clinical practice compared to before clinical practice, and pain scores decreased statistically significantly after foot bath (p<0.001). The foot pain scores expressed by the students in the first week increased continuously (p<0.001). Conclusion. The study showed that a foot bath performed after clinical practice were effective for easing foot pain and leg edema.

The brains of 10 infected pigs were examined for histopathology and presence of African swine fever virus (ASFV) DNA ASFV infection induces inflamed meninges, cerebral edema and vascular thrombosis, as well as subdural hematomas. Slight tension in the dura mater, flattening of the gyri and narrowing of the sulci were also observed at four days post infection (dpi). Enlarged perivascular spaces were detected for most vessels of the brain after three to four dpi. Considerable lymphocytic infiltration of the brain tissue was observed at the terminal stage of disease. ASFV was present in all investigated areas of brain beginning from three to four dpi. The isolated virus do not differ from the used in present study Georgia 2007 strain.

Introduction: Chylous complications, which also occur in the profile of vascularsurgical interventions with considerable frequency, are challenging with regard to their adequate management. Aim & method: Short compact overview on epidemiological, classifying, symptomatic, diagnostics and therapeutic aspects of chylous complications in vascular surgery, based on •own clinical experiences, 
 •current selection of relevant scientific references, and 
 •representative case reports from clinical practice. Results (complex patient- & clinical finding-associated aspects): •Basic treatment of lymphedema / postreconstructive edema comprises a complex physical therapy to improve edematous swelling, which needs to be usually performed over years. •In case of lymphocele, a wait-and-see strategy can be initially pursued to observe spontaneous clinical course. If the lymphocele and its clinical complaints persist, puncture, placement of drainage or temporary instillation of doxycyclin or ethanol can be attempted. •In case of lymphatic fistula, vacuum-assisted closure dressing, radiation and selective ligation of lymphatic vessels after previous application of methylene blue dye can be used. •Chylascites and chylothorax should be primarily treated – as has been widely established in the meantime – with a consequently conservative approach initially comprising paracentesis / thoracocentesis, protein-enriched and low-fat diet containing middle-chain triglycerides (MCT) or total parenteral nutrition combined with the application of a somatostatin analogue (surgical approach as ultima ratio only aiming at ligation of the lesioned lymphatic vessel – if necessary, including preoperative consumption of cream). Summary: Chylous complications can be primarily treated with conservative measures, which should be exploited using a step-wise approach prior to surgical intervention as ultima ratio. Conclusion: The experienced vascular surgeon should be acquainted with a sufficient, finding-adapted management of chylous complications. This requires a well-experienced clinician and surgeon with great expertise regarding the interdisciplinary setting comprising of interventional radiology, vascular (abdominal) surgery and partially surgical intensive care.

Laser has changed a lot over the years, the newer laser can deliver energy in different ways, by reducing the duration to 10–20 ms, in a train of pulses of 0.1 ms as in the micropulse / subliminal laser, and extremely short duration of 0.0017 ms as in the nanosecond laser. Clinical studies have shown that micropulse laser is efficacious in multiple macular conditions. Reducing the power and duration using end-point management has yet to demonstrate clinical efficacy beyond CSR. The ability to reduce the progression of intermediate AMD is intriguing, after all the LEAD (Laser intervention in early stages age-related macular degeneration) trial failed the primary endpoint. Nonetheless, there is evidence to support that it may be effective in a highly selected sub-group of intermediate AMD patients. More studies are required to see whether other lasers such as micropulse laser can have the same effect, and whether micropulse lasers with a slightly higher energy leading to a few RPE cell deaths is needed to have the same effect.
Lasery okulistyczne do leczenia schorzeń siatkówki przeszły znaczną ewolucję w ciągu ostatnich lat. Najnowsze urządzenia zapewniają krótszy czas dostarczania energii (10–20 ms): w impulsach trwających 0,1 ms w laserze mikropulsowym/ podprogowym i w impulsach o ultrakrótkim czasie trwania – 0,0017 ms – w laserze nanosekundowym. Badania kliniczne wykazały, że laser mikropulsowy jest skuteczny w wielu stanach chorobowych plamki żółtej. Trwają badania nad określeniem skuteczności klinicznej przy zastosowaniu mniejszej mocy i krótszego czasu aplikacji, uzyskanych dzięki metodzie end-point management we wskazaniach innych niż centralna retinopatia surowicza. Możliwości spowolnienia progresji średnio zaawansowanego zwyrodnienia plamki żółtej (AMD) wydają się realne, mimo że w badaniu LEAD (Laser Intervention in Early Stages Age-related Macular Degeneration) nie udało się osiągnąć tego założonego pierwszorzędowego punktu końcowego. Dysponujemy jednak licznymi poszlakami wskazującymi, że sukces jest możliwy w precyzyjnie wyselekcjonowanej podgrupie pacjentów z AMD. Potrzebne są zatem kolejne badania, aby jednoznacznie stwierdzić, czy urządzenia takie jak laser mikropulsowy mogą zapewnić satysfakcjonujący efekt terapeutyczny i czy lasery mikropulsowe o nieco większej energii, które powodują zniszczenie większej liczby komórek RPE, są niezbędne do osiągnięcia tego efektu.

Automatic assessment of voice disorders is one of the most important applications of speech signal analysis. Various algorithms utilizing both sustained vowels and continuous speech have been successfully used to perform detection of many voice pathologies, e.g. dysphonia, laryngitis, and vocal folds paralysis. However, algorithms described in literature used for classification of Reinke’s edema - one of the most severe smoking-induced voice conditions - are scarce and rely mostly on speech signals containing sustained vowels. In this paper, a method incorporating gammatone frequency cepstral coefficients (GFCC) based x-vectors extracted from continuous speech is presented. The extracted x-vectors are used to train a SGD classifier performing Reinke’s edema detection. For validation folds, the proposed method yielded AUC ROC, accuracy, recall, and specificity of 0.96 (±0.03), 0.94 (±0.02), 0.92 (±0.03), and 0.94 (±0.02), respectively. For testing set, the method yielded AUC ROC, accuracy, recall, and specificity of 0.98, 0.89, 0.88, and 0.89, respectively.

Diabetes is an important social problem. Epidemiological studies show a worldwide increase in the incidence of type 1 and 2 diabetes. Increasing duration of diabetes significantly raises the risk of the retinal vascular changes and the development of the macular edema. Ranibizumab in intravitreal injection has revolutionized the treatment of ocular complications of diabetes. It is used as part of the first-line treatment for clinically significant macular edema with occlusion and worsening of visual acuity. Numerous studies have demonstrated greater efficacy of the treatment with ranibizumab (both in monotherapy and in combination with laser therapy) compared to laser therapy, which was the gold standard of therapy.
Cukrzyca jest chorobą stanowiącą duży problem społeczny. Badania epidemiologiczne dotyczące zapadalności na cukrzycę typu 1 i 2 na świecie wykazują ciągły wzrost liczby zachorowań. Wraz ze wzrostem czasu trwania cukrzycy wzrasta prawdopodobieństwo znacznych zmian naczyniowych siatkówki i rozwoju obrzęku plamki. Zastosowanie ranibizumabu w iniekcji doszklistkowej zrewolucjonizowało leczenie powikłań ocznych cukrzycy. Lek ten stosowany jest w ramach leczenia pierwszego rzutu w przypadku klinicznie znamiennego obrzęku plamki z zajęciem dołka i pogorszeniem ostrości wzroku. W licznych badaniach wykazano większą skuteczność jego leczenia ranibizumabem – zarówno w monoteraii, jak i w skojarzeniu z laseroterapią – w porównaniu z laseroterapią, która do niedawna jeszcze była złotym standardem leczenia.

Opis:
Novel aryl/alkyl azole derivative compounds C1 and C2 were screened for in vivo anti-nociceptive and anti-inflammatory activities by using the hot plate test and formalin-induced hind paw edema test, respectively. Ability of these compounds to inhibit cyclooxygenase-1/2 (COX-1/2) and 5/15-lipoxygenase (5/15-LOX) were evaluated in vitro by using a colorimetric method. C1 (30 and 50 mg/kg, i.p.) and C2 (3 and 30 mg/kg, i.p.) increased the latency to withdrawal in the hot plate test, indicating an anti-nociceptive activity. C1(30 and 50 mg/kg) and C2 (3,30 and 50 mg) were able to decrease formalin-induced edema, indicating their anti-inflammatory properties. C1 and C2 exhibited inhibitory effects on the activity of COX(1-2) and 15-LOX. However, neither C1 nor C2 showed an inhibitory effect on 5-LOX. This study demonstrates that C1 and C2 have anti-nociceptive and anti-inflammatory activities, that is partially mediated by inhibition of COX and LOX enzymes. Our results suggest that C1 and C2, novel aryl/alkyl azole compounds, could serve as lead compounds to develop novel therapeutic options for the treatment of pain and inflammation.
