Opis:
Tympanostomy tube insertion and tympanic membrane incision are two the most frequently performed surgical procedures in otolaryngology, especially in children. The tympanic membrane incision - paracentesis, or myringotomy – is an incision of the tympanic membrane for diagnostic purposes or to allow drainage of pathological secretion from the tympanic cavity. Tympanostomy tube insertion involves incision made in the tympanic membrane and insertion of a ventilation tube (various types and for various periods of time) to improve hearing and aeration of the tympanic cavity. Procedures are performed through the ear canal (transcanal approach), under local or general anesthesia. Complications may occur in some cases of paracentesis and tympanostomy tube insertion.

Dizziness is a symptom of many diseases. Patients very frequently come with such ailment to the hospital emergency department. It is a state which may last a few seconds or minutes and increase or recede with time. Therefore, the admitted patients frequently cannot assess their own illness in a precise and objective way. The dizziness definition is also quite ambiguous. Dizziness is defined as the sensation of one own’s body movement or spinning and movement of the surrounding. It is very important to record the patient’s medical history since the diagnostic procedure may depend on the symptoms’ character. Dizziness may be a symptom of a serious disease, although it is not easy to find its cause. It appears not only in case of the labyrinth and nervous system disorders, but also in the systemic and functional diseases. Dizziness and balance disorder are the direct cause of admitting one in every thirty patients. The symptom indicated the directly life-threatening disease only within the 3-8.5% of patients (cerebral circulation insufficiency - 6%, cardiac dysrhythmia – 1.5%, brain tumour < 1%). Analyzing the data concerning the problem of dizziness occurrence within the general population, it has been noticed that this symptom has been reported two or three times more frequent by women than by men. Dizzinesses are classified pathogenetically and clinically into labyrinthine and non-labyrinthine, paroxysmal and permanent, acute and chronic. Dizziness is hard to diagnose because the symptoms reported by patients are only their own subjective sensations. The data presented in the article implicate the increasing number of patients with such disorders. Apart from dizziness, the patients complain also about the hearing disorders and nausea, which make their proper functioning impossible. It is inappropriate to start the treatment without knowing the cause. Establishing, on the basis of patient’s medical history and physical examination, whether the dizziness is of peripheral or central origin, is essential for the further diagnosis. After establishing the main diagnosis, the casual and symptomatic treatment is implemented, in some cases - there is a vestibular rehabilitation or even a surgical treatment.

Objective: The aim was to determine the clinical characteristics and analysis of surgical treatment of patients with glomus tympanicum hospitalized in the Department of Otorhinolaryngology. Material and methods: Between 2000 and 2015, 27 patients were hospitalized. Age ranged 28-79 years old. The study analyzed data from the clinical examination and the type of surgical technique. Results: The most common symptoms were tinnitus and hearing loss. Resection of the tumor was performed by anterior tympanotomy in 3 (11,1 %), CWU mastoidectomy in 21 (77,8 %), CWD mastoidectomy in 1 (3,7 %), lateral petrosectomy in 2 (7,4 %) patients. Conclusion: Glomus tympanicum have a quite characteristic clinical picture with dominant symptoms of tinnitus, hearing loss and reddening of the tympanic membrane in otoscopic examination. This type of tumors require surgical treatment. Surgical access depends on the size of the tumor in the tympanic cavity. Most of these tumors can be removed using antromastoidectomy, with posterior tympanotomy and hypotympanotomy.

Objective: The aim of this study was to evaluate cases of of ear, nose and pharyngeal (ENT) foreign body (FB) in pediatric patients referred to the pediatric emergency department of the Department of Otorhinolaryngology in Tokat State Hospital (Turkey). Methods: The paper comprises a retrospective study of all ENT foreign bodies presented to the pediatric emergency Department of Otorhinolaryngology in Tokat State Hospital (Turkey) between January 1, 2012 and December 31, 2018. Subject to evaluation were also demographic and clinical characteristics of the patients, types and anatomic locations of FBs, locations of FB by age groups, the method used to remove FBs and the distribution of FB cases by months. Results: A total of 829 FB cases were evaluated in this study. The mean age of patients was 47.8 ± 31.4 months. Of all patients, 404 (48.5%) were male, and 425 (51.2%) were female. The nose (58.7%) and the ear (20.2%) were the most common anatomical locations followed by mouth/pharynx/tonsil (12.3%), esophagus (6.2%) and the laryngotracheobronchial tree (2.4%). The most common FBs in different locations were beads in the nose (30.8%), beads in the ear (32.1%), fishbones in the mouth/pharynx/tonsil (56.8%), nuts and peanuts in laryngotracheobronchial tree (70%) and a coin/disc battery in the esophagus (80.7%). Conclusion: FB in ENT is among the emergency conditions that require different diagnostic and treatment approaches based on anatomic localizations. Proper vision, adequate equipment and immobilization are important in the removal of ENT foreign bodies in children.

Congenital cholesteatoma is a rare ear disorder. The most common presentation is a pearly and white mass, visible with an intact tympanic membrane in individuals with no previous history of ear discharge, ear surgery or perforation of tympanic membrane. Based on a careful overview of literature, authors of this article present: the most probable theories of the cause of development of congenital cholesteatoma, diagnostic criteria of congenital cholesteatoma, its most common clinical symptoms, preoperative studies, methods of surgical treatments and goals of postoperative proceedings. Furthermore, authors present a comparison of histological, molecular and clinical features of congenital and acquired cholesteatomas.

Maize is the host for a large number of pathogens, which invade all of its organs from the germination until being harvest, ear and grain infection often persistent even during storage. Diseases, through their symptoms reduce significantly the quantity and the quality of yield, estimated between 7-17% but, in the favorable years for this disease, they can be much greater. Fusarium diseases reduce yield value and quality by massive accumulation of Fusarium mycelium biomass (about 85%) on grain and ears and by mycotoxin contamination such as deoxynivalenol (DON), zearalenone (ZEA) and fumonisins (FUM). In this paper are presented aspects regarding the reaction of some maize hybrids under Fusarium  spp. natural and artificial infections; the effect of Fusarium  ear infection on yield, grain chemical composition, and mycotoxin content; the correlation between ear rot disease degree and yield ability, starch, protein and fat content. ANOVA evidenced the significant influence of experimental factors: infection conditions with Fusarium spp., maize genotypes, and their interaction on expression of the disease degree, yield capacity, protein, starch, fat and DON content. Average yield losses ranged between 7,0-9,3% during the experimental period. The hybrids Turda Star and Turda Favorit were more resistant to Fusarium ear rot, and Turda 165 was the most susceptible one. The artificial infection of ear with Fusarium spp. determined significantly decrease of starch and fat content and increases the protein and DON content for the most part of maize hybrids. Between rot diseased kernels and DON content a positive correlation was determined.

The objective of this study was to assess the relationships between ear length, ear diameter, plant height and ear setting height in maize linear Pearson correlation coefficients and Spearman’s rank correlation coefficients. The plant height and ear setting height were positively linear and rank correlated in all four years of study. The correlation coefficients, both - linear and rank, between all four traits observed in 2018 were statistically significant. Ear diameter and plant height were linear and rank correlated in 2018, however in 2015 only Pearson correlation coefficient was statistically significant between these two traits.
Celem badań było zastosowanie współczynników korelacji liniowej Pearsona i rangowej Spearmana do oceny współzależności pomiędzy długością kolby, średnicą kolby, wysokością roślin i wysokością osadzenia kolb produkcyjnych kukurydzy. Wysokość roślin i wysokość osadzenia kolb produkcyjnych były dodatnio liniowo i rangowo skorelowane we wszystkich czterech latach badań. Oba współczynniki korelacji – liniowe i rangowe, pomiędzy wszystkimi czterema cechami obserwowanymi w 2018 r., były istotne statystycznie. Średnica kolby i wysokość roślin były liniowo i rangowo skorelowane w 2018 r., natomiast w 2015 roku tylko współczynnik korelacji Pearsona był statystycznie istotny pomiędzy tymi dwiema cechami.

Backgrounds Otosclerosis is an underlying disease of the bony labyrinth. The hearing loss is most often of conductive nature, in some cases the involvement of the bony part of the cochlea results in mixed hearing loss. Aims: The aim of the analysis was to answer the question whether a surgery on one of the ears affects the state of the other ear in the course of otosclerosis. Methods The analysis included 140 patients hospitalized and operated on between 2010 - 2016. Only patients who had not had a surgical operation within the middle ear due to otosclerosis prior to the study were included in it. An audiological assessment was performed with the use of pure tone threshold audiometry taking into account. Results In the group of patients with no Carhart's notch, the mean threshold of bone conduction was statistically lower than before the procedure for the frequencies of 500, 1000 Hz and statistically equal for the frequency of 2000 Hz. The same analysis in the group of patients with Carhart's notch present in the pre-surgical tonal audiogram of the non-operated ear showed a statistically significant lower value of the post-surgical threshold bone conduction value. Conclusion It was confirmed the possibility of improving the hearing of the non-operated ear after the stapedotomy of the opposite ear, in the author's own studies by an average of 5 dB in the low-frequency range.

Introduction. The aim of this work was to describe acoustic neuromas as the first signs of inner ear functional disorders. Material and methods. The study covered 3,456 audiological and otoneurological patients, who were treated in the Department of Otolaryngology, Laryngological Oncology, Audiology and Phoniatrics Military Medical Academy University Teaching Hospital in Lodz within the period of 2011–2016. Among the studied subjects, an acoustic neuroma on the vestibulocochlear nerve was diagnosed in 13 cases (5,16%), including 9 women and 4 men. Each patient underwent a medical interview and an objective examination with static and dynamic tests, pure tone audiometry and speech audiometry tests, impedance audiometry tests, characteristics of tinnitus, videonystagmography and auditory brainstem evoked potentials (ABR) in crack. In each case of an incorrect ABR recording, contrast-enhanced MRI was performed. Study results. The conducted tests showed an incorrect ABR recording in 252 patients (7,29%, including 54,37% women and 45,63% men). In the performed contrast-enhanced MRI, 13 patients (5,16%), including 3,57% women and 1,59% men, had an image typical to neuroma of the vestibulocochlear nerve. Each one of the studied patients showed signs and symptoms typical to an acoustic neuroma such as hypoacusia and balance disorders, tinnitus in 12 subjects, headaches in the temporal and occipital area in 4 subjects, trigeminal neuralgia symptoms in 2 subjects and vision problems like scotoma and blurred vision reported by 1 patient. Conclusion. Each case of sensorineural hearing loss, particularly unilateral sensorineural hearing loss, requires expanded hearing tests and auditory brainstem evoked potentials tests. An incorrect brainstem evoked potentials recording such as: an elongated latency of the 5th wave, an elongated interlatency of 1–2-3 and 1–2–3–4 waves, requires a contract-enhanced magnetic resonance imaging to diagnose cerebellopontine angle tumours.

Z perspektywy opisu zaburzeń mowy i trudności w czytaniu i pisaniu lateralizacja, w tym lateralizacja słuchowa, zajmuje ważne miejsce, ponieważ pozwala wyjaśnić mechanizmy zaburzeń. Ważne jest zatem przedyskutowanie sposobów diagnozowania lateralizacji słuchowej i wskazanie sposobów usprawniania tej funkcji.
Auditory lateralization is an increasingly common phenomenon taken into account in logopedic diagnosis and therapy. From the perspective of the description of speech disorders and reading and writing difficulties, lateralization, including auditory lateralization, occupies an important place because it allows explaining the mechanisms of disorders. It is therefore important to discuss ways of diagnosing auditory lateralization and to indicate ways to improve this function.

The term roughness is used to describe a specific sound sensation which may occur when listening to stimuli with more than one spectral component within the same critical band. It is believed that the spectral components interact inside the cochlea, which leads to fluctuations in the neural signal and, in turn, to a sensation of roughness. This study presents a roughness model composed of two successive stages: peripheral and central. The peripheral stage models the function of the peripheral ear. The central stage predicts roughness from the temporal envelope of the signal processed by the peripheral stage. The roughness model was shown to account for the perceived roughness of various types of acoustic stimuli, including the stimuli with temporal envelopes that are not sinusoidal. It thus accounted for effects of the phase and the shape of the temporal envelope on roughness. The model performance was poor for unmodulated bandpass noise stimuli.

The aim of this paper is to present latest studies on absolute pitch conducted over the past ten years. The author respectively discusses: results of the research on the origins of absolute pitch, relationship between absolute pitch and musical skills, dependence of the development of absolute pitch on applied aural training methods, anatomical and neural foundations of this ability, as well as infl uence of absolute pitch on the progress of other cognitive abilities. In conclusion the author submits: the key causes for the development of absolute pitch ability lay in innate predispositions (either genetic or stimulated in the prenatal period) — which manifest themselves in the asymmetry between left and right planum temporale — as well as in skills acquired by early musical practice combined with absolute method of ear training. It appears that using tonal languages is benefi cial for the development of absolute pitch. It has been proven that the ability of absolute pitch correlates with profi ciency in performing musical tasks. Examining brains of people with absolute hearing abilities shows that they have better developed circumferential networks, grey matter, greater thickness of cerebral cortex and improved white matter connectivity, what results in more effi cient networking between various areas of brain, better „small-world” functional networking and memory. Ability of absolute pitch can be understood as a phenomenon of unique perceptual enhancement, useful for the examination of brain plasticity.

This manuscript intends to review types, pathogenesis, associated risk factors, and potential methods of prevention and treatment of the retraction pockets in adults and children. The importance of retraction pockets (RP) lies in loss of original histological and anatomical structure which is associated with development of ossicular chain erosion, cholesteatoma formation and potentially life threatening complications of cholesteatoma. The trans-mucosal exchange each gas in the middle ear (ME) is towards equalizing its partial pressures with the partial pressure in the environment. MEs that have abnormalities in the volume and ventilation pathways in the epitympanic may be more susceptible to retraction pockets. Sustained pressure differences and/or inflammation leads to destruction of collagen fibers in the lamina propria. Inflammatory mediators and cytokines lead to release of collagenases result in viscoelastic properties of the lamina propria. The process of changes in the tympanic membrane structure may evolve to the cholesteatoma formation. There are many different staging systems that clinicians prioritize in their decision making in the management of RP. The authors discuss the management possibilities in different clinical situations: RP without and with ongoing or intermittent evidence of Eustachian Tube Dysfunction (ETD), presence of adenoid hypertrophy or re-growth of adenoids, presence or absence of effusion, invisible depth of RP without effusion. invisible depth of RP with effusion, ongoing RP after VT insertion, and finally suspicion of cholesteatoma in a deep RP with ME effusion. A decision algorithm regarding the management of TM retraction and retraction pockets is provided.
