Introduction and objective. Lyme borreliosis (LB) is a disease caused by the bacteria Borrelia burgdorferi. The most common symptoms are related to the skin, musculo-scelatal system, central and peripheral nervous system, rarely to the heart muscle and the eye, and may occur in the multistage course of the disease. LB may additionally be accompanied by psychopathological symptoms. The aim of the study is estimation of the cognitive and affective disorders occurence in patients with LB. Material and methods. The study was carried out in the group of 121 patients (61 females, 60 males) aged 18–65; mean age 46 years. All patients were diagnosed with late-stage of LB: 46 patients (38%) with Lyme arthritis and 75 patients (62%) with neuroborreliosis. Evaluation of the cognitive and affective functioning of patients was performed on the basis of a standardized interview and test methods: the Mini-Mental State Examination (MMSE), Clock Drawing Test (CDT) and the Beck Depression Inventory (BDI). Results. Cognitive disorders occurred statistically significantly more often in patients with neuroborreliosis (14.7%) than in patients with Lyme arthritis (4.3%). A group of females with neuroborreliosis and a group of males with the same diagnosis demonstrated cognitive deficits significantly more often (23.3% and 8.9%, respectively), compared to groups of patients with Lyme arthritis (6.5% in females and no cognitive deficits in males). A significantly higher percentage of depressive disorders was also noted in the group of males and females with neuroborreliosis (50.7%), compared to the group of patients with Lyme arthritis (39.1%). The symptoms of depression were particularly frequent in the females with neuroborreliosis (60%). The severity of depression measured by BDI was mild or moderate in most cases. In the examined groups, more patients with neuroborreliosis (44%), both in females (36.7%) and males (48.9%), demonstrated anxiety disorders. The obtained results showed a higher frequency of affective disorders compared to cognitive deficits, both in patients with Lyme arthritis and neuroborreliosis. Conclusions. An increased frequency of depressive and neurotic disorders was observed in patients with LB, particularly in patients with neuroborreliosis. Neurotic disorders, mainly adaptive, were most common in males with LB, while depressive disorders were more frequent in females. An increased frequency of cognitive deficits was observed in patients with neuroborreliosis, particularly in females.

The aim of the work is to present a clinical study of a development path using the theoretical model of developmental pathways (Pickles, Hill, 2006). The subject of the study, a three-year-old boy, came into the world through complicated labor which ended in cesarean section. He displayed characteristics of insecure-avoidant attachment. Moreover, he had a visual defect. The study describes the crisis at the beginning of preschool education. Adaptive irregularities arising from these burdens, but also the lack of competence and support from the preschool staffmeant that the boy began to show features of mental disorders functioning. At this time independent specialists diagnosed mental retardation and attention defi cit with hyperactivity disorder. Through correcting the vision problems, working on the relationship, changing the preschool facility and introducing cognitive-behavioral techniques the abnormal development of the boy returned onto the correct track. The aim of the work is to show the path of corruption in the development of mental disorder in the three-year-old boy. The developmental pathways approach takes into account the role of a wide range of internal factors (biological, social and psychological) as well as external ones – environmental and those concerning the effect of time and place, both in the development of pathology, and in turning to pathology as a way to return to normal development

Area disorders typically affect executive function, memory, attention, and verbal processing. Factors known to influence postoperative cognitive impairment are: age, preoperative cognitive functioning, and type of anesthesia. Current research into POCD is focused on cardiac and gastric surgery. A rapidly developing surgical technique is endoscopic surgery; specifically, endoscopic transnasal surgery, where a “dry” operating field is required. To date there is little research assessing the impact of this type of surgery on the development. POCD. In the current study we determine the possibility of occurrence of POCD, and investigate its prevention, in the aforementioned surgical situation. General ways to show existing research on POCD in the context of skull base surgery. Hypothetically, less tissue injury is associated with less inflammatory reaction and thus with the reduction of cognitive dysfunctions. Analysis of operating methods (such as derating RR and HR) and their potential impact on POCD. POCD is a temporary postoperative disorder and correlates with poor recovery after surgery. The causes of POCD are multifactorial; however, the immune response following surgery may be the initial factor to initiate the damage-causing ischemic response. Careful anesthesiological and surgical procedures lower the likelihood of POCD.

Autorki dokonały analizy teoretycznej i empirycznej mechanizmów zaburzeń adaptacyjnych (internalizacyjnych, eksternalizacyjnych i mieszanych) tkwiących w rodzinie pochodzenia. Jako podstawę dokonanych analiz przyjęły systemowe ujęcie rodziny, a w jej ramach typologiczne jej zróżnicowanie na podstawie koncepcji Davida Fielda (rodzina prawidłowa, chaotyczna, władzy, uwikłana i nadopiekuńcza), a także koncepcję zaburzeń emocjonalnych i behawioralnych Thomasa M. Achenbacha, w której wyodrębnia się zaburzenia internalizacyjne, eksternalizacyjne i mieszane. Dokonano próby analizy zależności pomiędzy typami i wymiarami (syndromami) zaburzeń adaptacyjnych a typami rodziny pochodzenia. Analiza wykazała, że jakość rodziny pochodzenia (wskaźnikowana przez jej typ) w niewielkim stopniu determinuje zróżnicowanie w zakresie typów zaburzeń adaptacyjnych. Zaobserwowano jednak pewne istotne tendencje, które pozwalają wnioskować, że specyficzne formy zaburzeń są powiązane z określonymi cechami rodziny pochodzenia (z uwzględnieniem zróżnicowania wynikającego z ról rodzicielskich: matki i ojca).
The authors conducted theoretical and empirical analysis of mechanisms of adjustment disorders (internalizing, externalizing and mixed) inherent in the family of origin. As the basis for the analyses, a systemic approach to the family was adopted, and within it, typological differentiation based on the concept of David Field (correct, chaotic, power, entangled and overprotective family), as well as the concept of emotional and behavioural disorders of Thomas M. Achenbach, which distinguishes internalizing, externalizing and mixed disorders. An analysis of relations between types and dimensions (syndromes) of adjustment disorders and types of the family of origin was attempted. The analysis showed that the quality of the family of origin (indicated by type) to a small extent determines differences in the types of adjustment disorders. However, certain significant tendencies were observed, which allow to conclude that specific forms of disorders are connected with particular features of the family of origin (taking into account differentiation resulting from parental roles: mother and father).

The article is a review of literature on language and communication difficulties in the course of psychotic disorders and mood disorders. Disorders related to non-linguistic aspects of statements, i.e. understanding of discourse, humour, irony, metaphors, as well as prosodic aspects of statements are described. Language problems are caused by improper functioning of the right cerebral hemisphere. In the course of psychotic disorders the most severe difficulties refer to generation and maintenance of the discourse and prosodic aspects of statements. Furthermore, problems with integration of information, related to understanding of jokes and sarcasm, are observed. Non-linguistic nature of statements is not consistent with the uttered contents, which causes communication disorders with the environment. In patients with schizophrenia the dynamics of communication changes depends on the duration of the disease as well as applied pharmacotherapy. Besides, differences are observed between schizophrenic women and men in their capabilities to read emotions. In the course of mood disorders the language and communication deficits are more discreet and seldom observed. The most extensively described language problems are disturbances in emotional prosody which significantly affects the correct perception of verbally communicated information. In addition, difficulties with understanding of metaphors and grasping of jokes and irony are described, besides a disturbed capability within understanding of discourse was found. The study compared also communication dysfunctions in the course of psychotic disorders and mood disorders. Similarly as in psychotic disorders, in mood disorders the language difficulties occur within understanding of discourse as well as the capabilities to grasp the jokes, irony and sarcasm.
Artykuł stanowi przegląd literatury na temat trudności językowych i komunikacyjnych w przebiegu zaburzeń psychotycznych i zaburzeń nastroju. Opisano zaburzenia dotyczące pozalingwistycznych aspektów wypowiedzi, tzn. rozumienia dyskursu, humoru, ironii, metafor, a także prozodycznych aspektów wypowiedzi. Występujące problemy językowe spowodowane są nieprawidłowym funkcjonowaniem prawej półkuli mózgu. W przebiegu zaburzeń psychotycznych najbardziej nasilone trudności dotyczą generowania i utrzymania dyskursu oraz prozodycznych aspektów wypowiedzi. Ponadto obserwowane są problemy z integracją informacji, dotyczące rozumienia żartu i sarkazmu. Pozajęzykowy charakter wypowiedzi nie jest spójny z wypowiadaną treścią, co powoduje zakłócenia komunikacji z otoczeniem. U chorych na schizofrenię dynamika zmian komunikacyjnych zależy od czasu trwania choroby, a także stosowanej farmakoterapii. Obserwowane są także różnice między chorymi na schizofrenię kobietami a mężczyznami w zdolności odczytywania emocji. W przebiegu zaburzeń nastroju deficyty językowe i komunikacyjne są bardziej dyskretne i obserwowane rzadziej. Najszerzej opisywanym problemem językowym są zakłócenia prozodii emocjonalnej, która znacząco wpływa na prawidłowy odbiór informacji przekazywanej werbalnie. Ponadto opisywane są trudności z rozumieniem metafor oraz uchwyceniem żartu i ironii, stwierdzono też zakłóconą zdolność w zakresie rozumienia dyskursu. W pracy porównano również dysfunkcje komunikacyjne w przebiegu zaburzeń psychotycznych i zaburzeń nastroju. Podobnie jak w zaburzeniach psychotycznych, w zaburzeniach nastroju trudności językowe występują w zakresie rozumienia dyskursu, a także umiejętności uchwycenia żartu, ironii oraz sarkazmu.

Autorki dokonały próby analizy mechanizmów zaburzeń internalizacyjnych, eksternalizacyjnych i mieszanych, związanych z postawami wychowawczymi (stylami wychowania) rodziców. Badania prowadzono wśród młodzieży gimnazjalnej o zidentyfikowanych różnych typach zaburzeń, a dotyczyły one percepcji dominujących stylów wychowania w rodzinie pochodzenia. Przyjęto klasyfikację stylów wychowania w ujęciu Marii Ryś, która wyodrębniła style: demokratyczny, autokratyczny, liberalny kochający i liberalny niekochający. Identyfikacja typów zaburzeń dokonana została na podstawie Arkusza Obserwacyjnego TRF Thomasa M. Achenbacha, a więc obserwacji przeprowadzonych przez nauczycieli. Analiza zależności między typami i wymiarami (syndromami) zaburzeń adaptacyjnych a stylami wychowania w rodzinie pochodzenia wykazała, że postawy wychowawcze rodziców w niewielkim stopniu determinują zróżnicowanie w zakresie typów zaburzeń adaptacyjnych, ale widoczne są specyficzne tendencje, wskazujące na powiązanie pewnych wymiarów zaburzeń ze stylami wychowania preferowanymi przez matkę i ojca: (1) zaburzenia internalizacyjne powiązane są istotnie z postawami wychowawczymi ojca (postawą autokratyczną i liberalną kochającą), powodując nasilenie się problemów w funkcjonowaniu interpersonalnym (problemy społeczne); (2) zaburzenia mieszane powiązane są istotnie z postawami wychowawczymi matki (postawą demokratyczną, autokratyczną, liberalną kochającą i niekochającą), wyzwalając zaburzenia o charakterze wycofania, lękowo-depresyjne i zachowania niedostosowane; zaburzenia eksternalizacyjne powiązane są z postawami wychowawczymi obojga rodziców (postawą demokratyczną i liberalną kochającą), wyzwalając głównie skargi somatyczne i zaburzenia depresyjno-lękowe.
Authors made the attempt of the analysis of the mechanisms of internalization, externalization and mixed disorders, attitudes related to upbringing (parenting styles) parents: mother and father. The study was conducted among junior high school students on the identified different types of disorders, and they relate to the perception of the dominant styles of education in the family of origin. Adopted the classification of parenting styles in terms from Maria Ryś, who singled styles: democratic, autocratic, liberal loving and liberal unloving. Identification of the types of disorders was made based on Teacher’s Report Form (TRF) Thomas M. Achenbach. Analysis of relationship between the types and dimensions (syndromes) disorders of adaptation and parenting styles in the family of origin, showed that the attitude of parenting a small extent determine the differences in the types of adjustment disorder, but specific trends are visible, indicating a link between certain dimensions of the problems with parenting styles preferred by the mother and father: (1) internalization disorders are associated significantly with father’s upbringing attitudes (attitude autocratic and liberal loving), causing the severity of problems in the functioning of interpersonal (social issues); (2) mixed disorders are associated significantly with mother upbringing attitudes (attitude democratic, autocratic, liberal loving and unlovely), triggering disorders of withdrawal, anxietydepressive and behaviour unadjusted; externalization disorders are associated with upbringing attitudes of both parents (democratic and liberal attitude of loving), releasing mainly somatic complaints and anxiety-depressive disorder.

In the general-somatic network there is a steady increase in the number of patients with psychosomatic disorders. Problems of providing adequate psychiatric and psychotherapeutic assistance to this category of patients are related to the motivation of patients to participate in psychological measures and the readiness of the medical system to provide comprehensive care on the basis of the biopsychosocial approach. Mental factors are involved both in the occurrence and course of a metabolic syndrome in the form of a patient's lifestyle and behavior patterns of healthy functioning, and is a consequence of somatic pathology. So mental disorders of metabolic syndrome are manifested in the form of psychosocial maladaptation, neurotic, affective, personality, and organic disorders. Desynchronosis which is a factor of the development of a metabolic syndrome and characterizes the complex chronobiological component of the regulation of psychophysiological functions in norm and under the influence of stress, deserves special attention.Addressing the diagnosis of mental disorders associated with metabolic syndrome is precisely aimed at determining chronobiological disorders of psychosomatic integrated areas and is supposed to improve diagnostic and treatment process and to shorten the treatment of these disorders.

Aim of the research: interdisciplinary description of functioning of lexical connections in a patient with spinocerebellar ataxia (SCA-type1). There is little research on lexical networks in patients with neurodegenerative diseases. Procedure: the subject was asked to give associations to 63 stimuli-lexemes. Tools used in the study: the Experimental lexical network of the Polish language, a digital recorder, an Olympus 650 DM. The Experimental lexical network was developed by Izabela Gatkowska (2017). It was created as a result of the study concerning the associations of 900 healthy Polish-speaking adults. Utterances of the subject were recorded, transcribed and compared with standard data. Results: the reactions of the subject were different from the associations presented in the Experimental lexical network. The most common verbal reactions were: creating definitions of stimuli-word meanings or making comment on them. Associations created by the subject were based on paradigmatic or syntagmatic relations. The authors explain how the specific verbal behaviour of the subject is related to the cognitive and language dysfunctions caused by the cerebellar damages. The results showed that language processing should not be regarded as separate from other cognitive functions. Verbal reactions of the subject are slightly similar to aphasic symptoms, but mostly they are related to cerebellar damage and executive disorders. The results obtained proved the validity of the connection between linguistic and psychological methods and tools in studies which concern the language behaviour of patients with neurodegenerative diseases.

In the work, we studied the parameters of the psychological and emotional state of patients with type 2 diabetes mellitus on the background of excessive body weight. Psychological and emotional state studies were conducted using the Patient Health Questionnaire-PHQ-9, questionnaire the hospital scale of anxiety and depression (HADS). A comparison of the frequency of occurrence of anxiety-depressive disorders and the study of the degree of their severity in the patients under study. The study revealed that negative changes in the indicators of psychological and emotional state in patients with type 2 diabetes are six times more likely than in healthy people.

Objectives To investigate the prevalence of work-related musculoskeletal disorders amongst dentists who work in public clinics in São Paulo, Brazil, to investigate their awareness of the presence of risk factors in the workplace, disability due to pain, and the influence of pain on this awareness and disability. Material and Methods A cross-sectional study was conducted among 204 dentists who work in public health clinics in the northwest of São Paulo, Brazil. The data was collected through interviews, using the Nordic Questionnaire and the Work-Related Activities that May Contribute to Job-Related Pain Questionnaire. In the case of workers who reported pain, the Pain Disability Questionnaire (PDQ) and the Numeric Pain Scale were also administered. Statistical analyses were performed using SPSS 21.0. Results Most dentists (81.4%) had musculoskeletal disorders, especially in the neck, shoulders and lower back. We found that the presence of symptoms in the neck (15.7%), shoulders (12.7%) and lower back (15.7%) were the major causes of absenteeism over the past 12 months. Occupational risk factors perceived as the most problematic ones were: bending or twisting the back in an awkward way, continuing to work when injured or hurt and working in the same position for long periods. Comparison between the symptomatic and asymptomatic dentists showed a statistically significant difference (p < 0.05) in the perception of occupational risk factors. The analysis of the intensity of pain and disability with PDQ in the symptomatic dentists showed an average pain intensity of 3.8. Mean scores of the PDQ total (11.46) and its dimensions – functional condition (7.1) and psychosocial condition (4.4) – suggest a moderate disability in the dental surgeons. There was a strong t correlation (r = 0.697) between pain intensity and the total score of disability caused by pain. Conclusions Pain and work-related musculoskeletal disorders interfere significantly with the dentists’ lives. In the case of dental surgeons there is a significant correlation between pain intensity and disability. Int J Occup Med Environ Health 2017;30(3):367–377

Objectives Shoulder pain associated with upper limb musculoskeletal disorders is an important health problem in clinical practice due to its high prevalence, frequent consultations in primary healthcare and occupational health. Preventive strategies, including information disseminated among workers, can be useful. The aim of this study is to analyze the associations between non-traumatic osteomuscular diseases of the shoulder and the workers’ knowledge of the risks at the workplace and preventive measures developed there, as well as the association with ergonomic requirements. Material and Methods An observational case-control study was carried out on an occupationally active population assisted during 1 year in 1 healthcare center, involving 690 participants. Data were collected through a questionnaire including sociodemographic variables, the workplace, the knowledge of prevention and ergonomic requirements. The independent effect of the variables associated with non-traumatic shoulder pathology was explored through logistic regression. Results In total, 66.7% of the participants stated that they had been informed of the occupational hazards related to their jobs. The following variables were associated with a lower probability of shoulder injuries: male gender, working hours > 9 h/day or > 40 h/week, as well as having information on the risks associated with the workplace, using personal protective equipment, the existence of an occupational risks prevention service and/or risk assessment, the knowledge of the prevention plan, periodic medical examinations, and using one arm or physical force at work. A multivariate analysis revealed that the risk increased with age and lower educational levels, forced postures, repeated gestures, monotony and temporary absences from work. Furthermore, being informed of workplace risks, and using a single arm as well as physical force were shown as independent protective factors. Conclusions Information on both the ergonomic requirements and how to prevent occupational risks is a useful tool for the prevention of non-traumatic shoulder diseases. Preventive policies including health education interventions in the workplace could benefit other developed preventive activities. Int J Occup Med Environ Health. 2019;32(6):825–40

Opis:
Street cleaning is an integral part of the solid waste management system. There are different ways to achieve clean streets depending on the availability of equipment, the type and magnitude of dirt, the surface conditions encountered or traffic conditions. In general, hand sweeping by an individual worker or a group, hose flushing, or machine sweeping or flushing are applied. In order to obtain information about the occurrence and relevance of occupational health hazards of street cleaners, the current international literature, as well as corresponding German regulations, were reviewed and evaluated. Street cleaning includes a variety of health hazards for employees. These can be subdivided into effects of occupational tasks and effects of working conditions such as weather or road traffic. The hazards result from physical, chemical and biological exposures, but may also be due to physiological and psychological burden or inadequate safety aspects. The most commonly reported work-related complaints are musculoskeletal and respiratory disorders, cuts, slips, and road traffic accidents. In developing countries, street cleaners seem to be still heavily exposed to dust and, in most cases, no suitable protective measures are available. Especially in industrialized countries there exist a number of standards and recommendations for waste workers that aim to reduce their occupational health impacts.

Background The aim of this work is to provide empirical evidence regarding types and increasing prevalence of mental disorders affecting Polish working population in the years 2014–2016. The research questions concerned the specific characteristics of the types of mental disorders and their prevalence as well as the differences between males and females. Material and Methods Types of mental disorders were investigated using a clinical method, a structured interview, as well as medical record data gathered in the years 2014–2016 in one mental health treatment center. The study was conducted in the population of 1578 working individuals aged 18–64 years old, in various forms of employment, including flexible employment (self-employment, task assignment agreement) and contract employment. The research population consisted of 998 females and 580 males, aged 18–64 years old. The study aimed at investigating types and the prevalence rate of mental disorders developed in the examined working Poles, also with reference to the sex of the study participants as well as the age at which they started seeking treatment. Results The prevailing disorders include neurotic disorders; diagnosed according to the 10th Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10) classification as a range of anxiety disorders, mixed anxiety-depressive disorders, stress-related and somatoform disorders; as well as personality disorders. The prevalence rate of the aforementioned disorders was found to be higher among working females than in the group of working males. Conclusions The overall study conclusions based on the research data analysis point to the fact that the prevalence rate of various types of mental disorders displayed by the examined working males and females increased significantly in the years 2014–2016. Introduction: Hyperkalemia is a common electrolyte disturbance that occurs within many patients. The more often prevalence of cardiovascular or renal diseases is, the more frequent medical issue hyperkalemia will be. An increasing quantity of entities requires taking medications that affect electrolyte hemostasis. Therefore, reasons for hyperkalemia should be deeply reflected. One of them is pseudohyperkalemia. Purpose: In this study diagnostics and treatment of hypokalemia were presented based on the case report of a 56-year-old man. Case presentation: A 56-year-old man was admitted to the Department of Invasive Cardiology of Medical Hospital of Białystok for complaints of strong chest pain associated with palpitations, cold sweats, feelings of general weakness and anxiety. The patient had no medical history of chronic diseases and neglected to take any medications at length. The laboratory tests performed at admission showed an increased serum concentration of potassium. Through the whole hospitalization, many medications were implicated to overcome hyperkalemia (diuretics, calcium resonium, inhalation with beta2-adrenergic agonists, intravenous infusion of glucose with insulin) with no effect. Hormone test was performed, the results excluded Addison’s or pituitary disease. Differential diagnosis with arterial blood draw showed normal potassium serum concentration. Conclusions: Hyperkalemia is encountered in a broad spectrum of patients. The severely elevated level of potassium could lead to life-threating conditions. Therefore, proper diagnosis making process is a matter of great importance. As clinicians, we need to base not only on laboratory but also examine the whole picture of the patient. Misdiagnosing pseudohyperkalemia might result in unnecessary medical management.

Problems of reactive mental disorders and of the simulation of mental disorders have lately been very poorly represented in both psychiatrist and criminological literature. The present contribution discusses the sources of a considerable number of difficulties which emerge in practice when discussing the question of “Reactive disorder or malingering?”, as well as the errors of diagnosis in diagnosing malingering. The contribution is based on a analysis of material which comprizes three hundred and fifty cases of reactive mental disorders, and ninety-nine cases of malingering (simulation), with the accused; such material has been obtained from the Department of Forensic Psychiatry of the Psychoneurological Institute and from fifteen mental hospitals in Poland, to which prisoners were sent for observation. When making use of the term of “ malingering” , the contents of that notion ought to be narrowed down so as to comprize behaviour of such kind, which consists in an individual who is not mentally ill consciously producing definite psychopathological symptoms. We could not possibly consider to be malingering in the true sense of the word the producing by a mental patient (e.g. one suffering from schizophrenia) of symptoms which are not characteristic of the disorder in question. What is described by the term of sursimulation, even though it contains elements of malingering, essentially differs from true malingering. On the other hand, the view is not correct which reads that we may only then speak of malingering, when the simulating of symptoms of mental disorders makes its appearance with persons who do not exhibit any abnormal traits. Malingering most frequently makes its appearance with prisoners who exhibit symptoms of psychopathy, encephalopathy, mental deficiency, etc. The problem of metasimulation deserves special attention. The fact that at a given moment we have to do with a behaviour which indicates malingering is not by itself evidence that previously, during the period immediately preceding such malingering, reactive disorders did not appear with same prisoner. The symptoms of reactive disorders during the period which preceded the sending of the prisoner to a mental hospital may have become almost entirely extinguished, while their place was taken by an attitude of malingering, greatly reminiscent of the recent symptoms of reactive mental disorders. Besides, in cases of that kind there also arises, as a rule, the question of whether, side by side with elements of malingering, there do not appear feebly marked symptoms of reactive mental disorders, as remnants of the reactive disorders from which the patient had previously been suffering. Neither should another difficulty, which jurisdiction finds in its path, be forgotten. When having to do with an attitude of obvious malingering, one ought to take into consideration the possibility of malingering being gradually transformed into reactive disorders. The mechanism of malingering becomes fixed in the prisoner’s mind, it undergoes automation, and sets into motion a hysterical mechanism, which, in its turn, acts independently, in the way proper to it, owing to which psychogenic disturbances arise. Such a state as that cannot be described as malingering, in spite of the fact that it was simulation that not only constituted the starting-point of the disorders arisen, but had actually provoked, and to some extent moulded, them. An individual in that state no longer exercises any control over the symptoms of reactive disorders which have appeared, he ceases to exercise any mastery over them; the former malingering mechanism has been driven out of his consciousness and has become transformed into a new, and considerably more complicated hysterical mechanism. The cases discussed above may still run a course complicated in another way, namely after the transformation of malingering into reactive disorders certain symptoms of the latter in their turn are subject to undergo, even after the extinction of the disorders, a conscious consolidation through the new manifestation of the malingering mechanism. Therefore in such cases malingering may be observed, not only at the beginning, but also after the recession of the state of reactive disorders, in the form of metasimulation. The mechanism of the arising of reactive disorders is analogous to that of the arising of malingering; at the basis of both the above mechanisms there lie certain common fundamental tendencies. In all probability malingering runs along the very same tracks as hysterical reaction, and mobilizes, through the intermediary of autosuggestion, analogous mechanisms, causing, as it were, the automation of certain attitudes. Malingering individuals, even though at first they control that mechanism and consciously steer it, may lose their control over it. This leads to the cases of a transformation of malingering into reactive mental disorders, discussed above. The knowledge of making use of a mechanism approximating a hysterical one, of producing and fixing certain symptoms which would constitute a good imitation of disorders, is - as is well known - a most difficult thing. This is why long-lasting and consistently carried out malingering is an extremely rare phenomenon. An individual who simulates in such a way must be equipped with peculiar features, in order to be equal to tasks of that kind. Hence the well-know saying that “ one can simulate well only that which is close to the simulating persons’s individuality” (Lassegue), and that “ a good malingerer must be born such” (Braun). Among psychiatrists there prevails, generally speaking, an agreement as to the view that long-lasting and consistent malingering happens, as a rule, only with persons whose personality exhibits clearly pathological features. The data obtained from sixteen mental hospitals for the period of 1953 - 1960 bear witness to the fact that, out of 5,967 male prisoners sent there for psychiatric examination, mental reactive disorders have been found to exist with 711 cases (11.9 per cent.), and malingering of mental disorders in a mere 99 cases (1.6 per cent.). In the case of the 793 women, sent from prisons to mental hospitals for psychiatric examination, reactive disorders were found to exist in 73 cases (9.2 per cent.), and malingering in a mere 7 cases (0.9 per cent). When we analyse the 99 forensic-psychiatric reports which diagnosed malingering, it appears that we may distinguish two different groups of cases among them. The first of them comprizes 70 prisoners,, with whom the diagnosis of mere malingering does not arouse any essentia] diagnostical reservations. On the other hand, in the second group, which comprizes 29 cases, we have to do with 19 cases of undoubted metasimulation, as well as with 19 cases which are doubtful. Doubts arise in connection with the possibility of the co-existence of reactive disorders with simulation (5 cases), as well as with the presence of reactive disorders during the period immediately preceding malingering (3 cases), or finally, because of data which speak in favour rather of reactive disorders than of malingering (11 cases). Thus it is only in seventy cases that the diagnosis of malingering does not arouse any serious doubts; neither should it be forgotten that, at the same time (i.e. during the same seven-and-a-half-year period) as many as 711 cases of reactive mental disorders were observed with prisoners in sixteen mental hospitals. Thus cases of malingering of long duration are an extremely rare phenomenon in forensic psychiatrist practice. For the purpose of establishing how do the data look which concern long-term malingering of mental disorders in prisons, data concerning the number of cases of malingering within the period of one year have been obtained from the psychiatrists employed in two large Warsaw prisons, which are, in principle, destined only for prisoners under investigation. It was found that the number of malingering prisoners amounted, in one prison to nine, and in the other to five. Taking into consideration the number of all the prisoners detained in those prisons in the course of twelve months, the “ co-efficient of malingering” , calculated as per one thousand prisoners, amounts to 1.86 and 0.96 respectively. After a correction has been introduced, because of the possibility of certain prisoners failing to report for examination, that co-efficient should not exceed 2 pro mille.[1] Among the 350 cases of reactive disorders, selected by lot out of the total number of reports with a diagnosis of “ reactive disorder” for the purpose of obtaining a representative sample, metasimulation during the period of clinical observation has been stated to take place in as many as 24.8 per cent, of the cases.  When examining the two groups of cases: those of “ pure” malingering and those of metasimulation, we can establish the essential differences which exist between them. Those prisoners with whom no reactive disorders have been found to exist during observation, simulate other symptoms of psychotic disorders than those prisoners, with whom malingering has made its appearance only after the extinction of reactive disorders in hospital.   In the group of the seventy “ pure” cases of malingering the most numerously represented is the simulation of memory defects and of mental deficiency, or else of dementia; apart from the above, prisoners also simulate symptoms of conversion hysteria, of hallucination or delusion, as well as, exceptionally, symptoms of stupor.  On the other hand, in the group of fifty cases of metasimulation, more than one-half of the total number consisted of prisoners who simulated symptoms of pseudodementia along with elements of puerilism (which were altogether absent from the group of “ pure” simulation). Of cases of con- fabulation with symptoms of pseudodelusions there were eight, while there were none of them in the “ pure simulation” group. Of individuals who simulated memory disorders there were three times less.  Deserving of particular attention are the twenty-six cases of “ pure” malingering, in which the whole manner of simulating, the contents of the pseudo-symptoms produced, and the prisoner’s entire behaviour are of such a kind, that it seems improbable that the simulating individual could suppose that he would succeed in deluding his environment. The attitude of such prisoners is one of playful contradiction, usually coupled with irony and mockery with regard to the medical personnel; their behaviour is characterized by elements of acting and indeed of clowning; the absurdity of their utterances is glaring. Periodically, however, states of a certain inhibition make their appearance, and from time to time sudden changes of mood are visible, considerable tension, violent attempts at aggressive behaviour, and tendencies to self-mutilation.  It was Mönkenmöller who, once upon a time, drew attention to that peculiar form of malingering, in which it is impossible to detect any intelligible purpose. In such cases malingering assumes the character of acting which gives the malingerer some satisfaction (“spielerische Simulation' 4, as Utitz called it); The picture of malingering gives one to think by its specific features, and is distinguished, from the other types of malingering, by its altogether exceptional primitivism and inconsistency. 92 per cent, of the prisoners who simulated in that way were recidivists with a considerable number of previous convictions to their names. In the anamnesis of nearly one-half of them alcoholism and brain trauma, as well as other chronical brain diseases, made their appearance. More than one-half of their total number have performed self-mutilation in prison. In the cases of “pure”, true malingering there appear, in the hospital material investigated, numerous prisoners with symptoms of encephalopathy (37.1 per cent.) and psychopaths (about 40 per cent.), as a rule described as impulsive, irritable, aggressive. Not a single malingering prisoner has been qualified as an individual with a normal personality. The prisoners who simulated mental disorders are recruited - 81 per cent, of them - from among recidivists, as a rule from among juvenile or young offenders: sixty-six per cent, of the investigated were under twenty-five years of age. They belonged to the category of offenders who commit common offences, mostly offences against property, with thefts predominating. Among the reactive mental disorders to be met with in forensic psychiatrist practice and in the prisons, two kinds of disorders may be distinguished. First of all, the group of disorders of the type of hysterical disorders, the majority of which has a more primitive character; they are: pseudodementia, Ganser’s syndrom, puerilism, states of incomplete stupor and of stupor, fancies with contents similar to those of delusions, and symptoms of conversion hysteria. It is precisely that category of disorders that oftentimes causes particular difficulties in practice, when it is a matter of distinguishing them from malingering. The second group of reactive disorders, with more psychotic symptoms, comprizes: reactive depressions, stupor, and syndroms with delusions and hallucinations and paranoid states. In this category of disorders disturbances of consciousness are much more clearly discernible than they are in the first. Bunyeyev, however, correctly emphasizes the fact that clinical experience points to the fact that in the several syndroms distinguished above there are frequently contained elements, of other reactive syndroms, and, moreover, in a considerable number of cases it can be observed, how, in the course of the disorders, one set of syndroms gives way to other symptom syndroms. Consequently, the clinical picture is usually considerably more complicated than would result from a description that would only take into consideration the most fundamental elements. Among the three hundred and fifty cases of reactive disorders with prisoners under investigation the several syndroms make their appearance In the following dimensions:   Pseudodementia                                                 90 cases    25.7 per cent. Pseudodementia, Ganser’s syndrom and puerilism between them account for 35.2 per cent, of the material investigated. Pseudodementia and puerilism frequently constitute the source of serious difficulties when it is a matter of distinguishing them from malingering, if hospital observation is of too short duration. Seventy per cent, of the above cases spent over three months on observation in hospitals, including nearly twenty per cent, who spent more than six months there.  After a syndrom of pseudodementia, it may be sometimes observed the malingering of the extinct symptoms of that syndrom (metasimulation). Among the cases of metasimulation in the material under investigation in fifty-five per cent, malingering was precisely connected with pseudodementia. Reactive depressions are the second set, as far as numbers are concerned, in the material under investigation (22.64 per cent.). Reactive depressions are of various character. The obvious colouring of the majority of such states with hysterical traits frequently lends a peculiar stamp to the clinical picture, and may incorrectly suscitate a suspicion of malingering.  Mental disorders with a stupor syndrom, as is well known, rarely arise as isolated type of reaction. Considerably more frequently stupor takes place after pseudodementia, Ganser’s syndrom and puerilism, not infrequently after a period of a seeming withdrawal of all reactive symptoms. What is more, after stupor there frequently appear once more symptoms of other reactive disorders, first and foremost those of pseudodementia (Bunyeyev, Pastushenko). In cases of incompletely developed stupor there frequently appear suspicions of malingering, even though such casses ought to be numbered undoubted mental disorders.  When discussing cases with a hallucination and delusion syndrome one ought to remember that even in such cases the suspicion of malingering occasionally makes its appearance. This is influenced by the fact that the contents of the hallucination are closely connected with the prisonner’s own situation, that his behaviour is characterized by lively emotional reactions, and that he not infrequently manifests interest in his further lot, his family, etc. In fact the suspicion of malingering as a rule proves to be unfounded. Morever, it should not be forgotten that, in cases with a hallucination and delusion syndrome there not infrequently emerge serious diagnostic difficulties in connection with the posibility of the existence of schizophrenia.  Among the reactive disorders observed with prisoners in the hospitals there were twelve cases of acute paranoid state. In this, relatively very infrequent, syndrom, which develops against a background of intensified fear and anxiety, and rapidly disappears under conditions of hospitalization, the existence of hallucinations, mainly visual ones, has also been found.  The symptoms which approach delusions include the so-called confabulation, with contents resembling those of delusions (“wahnhafte Einbildungen” ), which had been described by Birnbaum more than fifty years ago. The inventing of occasionally the most improbable and queerest facts takes place against a background of usually glaringly expressed hysterical traits; occasionally elements of pseudodementia and puerilism become visible. All this together may suscitate serious suspicions of malingering; prolonged observation, however, makes it posible to find the existence of clearly reactive disorders. Of such cases there were ten in the material under investigation. Predominant among them were cases of persecutory pseudodelusions (eight cases), with the most absurd and fantastical subject-matter. In the remaining two cases it was grotesque grandiose pseudodelusions that made their appearance. Both the attitudes and the behaviour of all such individuals were, as a rule, in complete contradiction with the contents of their utterances. Those prisoners who exhibited symptoms of reactive mental disorders differ in an essential way from those prisoners who simulate pathological symptoms. First of all, there are considerably less recidivists among them: the percentage of the latter did not exceed 33 per cent, while with the simulators it reached 81 per cent. Among the prisoners with reactive disorders there are less individuals who would exhibit organic changes of the brain (23 per cent., as compared with 37,1 per cent, with the malingerers), while, on the other hand, the percentage of persons of the schizoid type is considerably larger (36 per cent., as against about 10 per cent, with the malingerers), as well as that of psychopaths with obvious hysterical traits (31.4 per cent., as against about 20 per cent, with the malingerers).  A mere 4.5 per cent, of the total number of prisoners with reactive mental disorders under investigation were found to be persons whose premorbid personality did not suggest any suspicions concerning pathology; all the remaining ones figure, in the diagnoses, either as psychopaths, or else as persons with symptoms of encephalopathy. In spite of the lack of any exhaustive anamneses in a great many cases it was found possible to state that at least 17 per cent, of the prisoners sent to mental hospitals because of reactive mental disorders had already previously suffered from such disorders. The cases of reactive states of a protracted character, numerous in the material under investigation (32 per cent, among the cases dealt with in the Institute of Psychoneurology) make one realize the importance of a proper conception of the problem of reactive mental disorders with prisoners. In those cases states which could at first produce an impression of simulation were relatively numerously represented. Mistrust in such cases might well be increased by the fact that nearly one-half of them consisted of prisoners accused of the perpetration of homicide. A hospital observation which went on for many months on end, not only did confirm the diagnosis of a reactive mental disorder, but has also, over and above that, demonstrated that those mental disorders had, in a considerable number of cases, become so deep, that a large number of the patients had to be assigned for release from prison. Merely about 22 per cent, of the total of those suffering from protracted disorders recovered their health and could, later on, be prosecuted before a law-court.  A working hypothesis in both prisons and forensic-psychiatric practice should therefore be the premisse that a pure malingering of mental disorders going on for a longer period of time is an altogether exceptional phenomenon, and that, as a rule, we have to do, in such cases, with reactive disorders. A different approach not only does run counter to the present-day state of psychiatrist knowledge, but is also highly harmful for both forensic and prison practice, as well as being inhumanitarian.   [1] In order to avoid any misunderstandings it ought to be emphasized that we are here referring to cases of long duration, of a malingering of mental disorders going on for at least several weeks on end. Clumsy attempts at simulating pathological symptoms for a period of a few days, naturally, altogether elude a psychiatrist who is not permanently employed in the prison in question, and, in all probability happen much more frequently

We evaluated the effectiveness of three different rehabilitation programs for chronic behavioral disorders in traumatic brain injury (TBI) patients with orbito-frontal damage (OFC), incurred from 13 to 15 years prior to enrollment. From among 547 adult patients with severe TBIs treated neurosurgically from 1999 to 2003, 60 were selected for the study, divided into three groups of 20, matched for age and sex. Each group received one of three different rehabilitation protocols. Group A received a comprehensive program with two components: (1) a 6-month early neuropsychological rehabilitation program targeting cognitive deficits, beginning immediately post-injury, and (2) the “Academy of Life” program, with weekly meetings aimed at reducing behavioral disorders in social transactions. Group B received only cognitive rehabilitation, while Group C received only the “Academy of Life” progam. The Frontal Behavioral Inventory (FBInv) was used to assess behavioral disorders at baseline and again after completion of the study. At baseline, all 60 patients still displayed all or most of the common behavioral symptoms characteristic of frontal syndrome. The improvement achieved by group A (the comprehensive program) was significantly better upon completion of the study than that shown by the patients in groups B (cognitive only) and C (social skills only). A comprehensive program of rehabilitation, which combines early neuropsychological rehabilitation of cognitive deficits with the “Academy of Life” program, aimed at improving social skills, proved to be more effective than either of these two components administered without the other

Right hemisphere brain damage manifests itself in the language at the level of not only expression but also reception. Utterances of people with right hemisphere dysfunctions are often disorderly and illogical. As recipients, patients with right hemisphere brain damage, for instance, interrupt their interlocutor’s utterance, cannot understand jokes, mockeries, or ambiguous messages. The paper describes language and communication defi cits arising from right hemisphere brain damage and cases of patients suffering from right hemisphere disorder. The data obtained in the course of examining people with right hemisphere damage show that the most disturbed aspects of language include: lexical and semantic processing, processing complex language information, discourse, and prosody.

Objectives: Musculoskeletal disorders (MSDs) compose a large part of occupational diseases in dental professionals, prevention of which is dependent on assessment and improvement of job postures by means of ergonomic interventions. This study was aimed at evaluation of ergonomic conditions of the profession of dentists and also at assessing the relationship between MSDs and conditions of work. Materials and Methods: This cross-sectional study was performed among 65 dentists using the method of Rapid Entire Body Assessment (REBA). The prevalence of MSDs was obtained by the use of the Nordic Musculoskeletal Questionnaire (NMQ). Results: In this investigation, the prevalence of MSDs for different body parts was: 75.9% for the neck, 58.6% for the shoulders, 56.9% for the upper back, 48.3% for the lower back and 44.8% for the wrist. Job analysis by the use of REBA showed that 89.6% of limbs in group A and 79.3% of limbs in group B had a score > 4. Only neck and lower back pain have significant relationship with the risk levels obtained using the REBA method. Conclusions: It can be concluded that work postures of dentists need to be improved. In addition to education, work station design, rest period during work and regular physical activities should be taken into account.

Obesity is a growing public health concern in modern societies. This is a major health concern. It has been implicated as a risk factor for several physical illnesses, functional limitations and poor quality of life. However, while the physical consequences of obesity are well established, the relationship between obesity and mental health is still unclear. The question this study aimed to answer was whether the associations between obesity and mood disorders are occurring in the sanatorium patients population. The results found a significant association between obesity and mood disorders. It is still unclear whether the relationship between obesity and depression is causal, and if so, whether obesity causes depression or depression causes obesity. Implications for health care providers and suggestions for future research are discussed.

Głównym tematem artykułu jest problematyka symulowania zaburzeń funkcjonowania pamięci. DSM-IV (APA, 1994) definiuje symulowanie jako zamierzone tworzenie pozornych lub znacznie wyolbrzymionych objawów somatycznych i/lub psychicznych, motywowane czynnikami zewnętrznymi, takimi jak chęć uniknięcia służby wojskowej lub pracy, uzyskania rekompensaty finansowej, uniknięcia odpowiedzialności karnej lub zdobycia narkotyków. W pierwszej części artykułu przedstawiono trzy główne modele wyjaśniające zjawisko symulowania, które opierają się na analizie motywacji leżącej u podłoża zjawiska. Druga część została poświęcona problematyce wykrywania symulowania zaburzeń pamięci. Opisano tu podstawowe plany badawcze oraz metody wykrywania symulowania, oparte na analizach sposobu reagowania i wyników uzyskanych w standardowych testach neuropsychologicznych.
The article presents a theoretical analysis of the phenomenon of simulation of memory impairment. According to the DSM-IV (APA, 1994) the essential features of simulation involve intentional or grossly exaggerated presentation of physical symptoms, motivated by external incentives such as avoiding military duty, avoiding work, obtaining financial compensation, evading criminal prosecution, or obtaining drugs. First, three main explanatory models of malingering are presented, then the issue of detection of memory impairment simulation is analyzed. The main research designs employed in the studies on simulation are discussed and detection strategies based on the analysis of the results on standard neuropsychological tests are presented.

Infantile anorexia is one form of feeding behavior disorder. The group of these disorders usually affects infants and young children whose food consumption in some way deviates from the norm. Their main feature is the difficulty in establishing a regular eating pattern. Which means that the infant does not regulate its eating rhythm according to the physiological feeling of hunger or satiety. In the case of infant anorexia, a characteristic symptom is a persistent reluctance to eat. A sick infant does not signal hunger and is not interested in eating. This leads to a height/weight deficiency and other negative consequences for the child’s development and health. The diagnosis excludes traumatic experiences or a physical illness that could better explain the infant’s reactions. The causes of the occurrence of infantile anorexia are mainly seen in the mental factors related to dysfunctional interactions in the family system, although an increasing amount of research also points to a large role of biological factors in its etiology. Above all, it’s believed to have a very strong genetic component. Infantile anorexia is a potentially curable disease, provided it is diagnosed and appropriate treatment measures are taken. The latter consist in psychotherapy and the introduction of eating patterns. Failure to take such steps may result in the persistence of symptoms and the increased risk of acute or chronic child malnutrition, and in extreme cases may jeopardize the child’s life.

Pregorexia is the term used to describe anorexia nervosa in pregnant women. It is not a medical term, yet increasingly used by specialists nowadays. Probably this is because the cases of pregorexia they encounter in practice are increasing in number, affecting 1.5–5% of women. In addition, they emphasize the specificity of this eating disorder, which carries a double risk when the mother is expecting. However, the latest classification of mental disorders of the American Psychiatric Association, DSM-5, does not include a separate name and criteria for the diagnosis of anorexia nervosa occurring in pregnant women. The clinical picture of pregorexia, apart from some significant symptoms, is consistent with the picture of this disorder in other people. Women with this disorder show a strong fear of the natural consequence of pregnancy, which is weight gain. To this end, they reduce the amount of food consumed, reduce the caloric content of meals, and use fasts. They also often do intense exercise. Sometimes they have binge eating and purging episodes, provoke vomiting, and abuse laxatives. As a result, they lose weight, develop qualitative malnutrition and body exhaustion. This entails numerous negative effects on the health and sometimes life of both mother and child. Such a danger, however, is not a factor preventing the actions causing it. They are caused by the pathological mechanism of the disorder, related to the action of various factors. In this article, some selected ones seem to be peculiarly significant in relation to pregorexia.

The mutual mother – infant interaction activates simultaneously two systems: attachment system and caregiving system. It guarantees survival to a newborn and contributes to its optimal development – social, emotional and cognitive. According to the theory of John Bowlby (1969, 2007) a child has natural innate mechanisms such as crying, clinging, sucking, smiling, calling, and following. These mechanisms activates the caregiving system in the mother. In this theoretical paper, we aim to present how the neurodevelopmental disorder having its roots in brain damage can modify the original attachment behavior by causing difficulties in attuning to the caregiver and interfering with caregiving system activation. The authors’ clinical experience shows that FAS/FASD often correlates with attachment disorder. FAS can be both the cause of difficulties and reinforcement of the environmental factors which impair attachment. In a clinical picture FAS and attachment disorder influence each other, resulting in behavioural and emotional disorder.

This article discusses the problem of co-morbidity of posttraumatic stress disorder (PTSD) and substance abuse disorders. The author presents a review of epidemiological studies and theoretical conceptualizations on co-occurrence of PTSD and alcohol and drug abuse or dependence. In addition, a review of studies which explore the nature of the relationship between PTSD and substance abuse disorders is presented.

The article deals with one of the most important problems of modern life − children's aggression, which becomes apparent in passively-aggressive, emotionally-destructive behavior and also as a result of physical and verbal insults at school, social-living space, in the child's close surrounding – family, game surroundings, etc. The presence of aggression in the child's behavior is always the reason of great problems in the process of communication, but the aggressive tendencies aren't to be evaluated only as negative phenomenon. Aggression can rise as well as subside on the background of the child's activity increase. It can become apparent occasionally in all children and it can indicate the kind of child's personal sphere disorder. Thus, aggression can help a child to develop the spirit of initiative. However, it can provoke reticence and hostility as well. The detailed analysis of the scientific theoretical approaches as for the determination of aggression gives the reasons to confirm that the majority of modern and classical scientists have analyzed the concept of aggression from two points of view: "the aggression" is any form of behavior aimed to the insult, doing harm to another living being, or "the aggression" is one of the forms of activeness, which can have positive or negative manifestation. That's why our investigation is dedicated to the analysis of the children's aggression from two points of view: positive phenomenon, which corresponds to the vital interest, self-defense, as well as negative, which corresponds to the cause of some suffering (harm) to any person or oneself. According to the results of the summary of the scientific methods, three types of aggression and six subtypes of aggression, and also their symptoms were found out: self-controlled types of aggression include controlled and completed subtypes of aggression; latent type – protective and depressive; behavioral type – demonstrative and physical. There have been discoveries of sides of aggression and the level of aggression shown in the children with normal physical and psychological development and also in the children with phonetic speech disorders (henceforth we use abbreviation: phonetic speech disorders – PSD), phonetic-phonemic speech disorders (henceforth we use abbreviation: phonetic-phonemic speech disorders – PPSD) and mildly manifested general speech disorders (henceforth we use abbreviation: mild general speech disorders– MGSD). The dependence between the level of speech disorders and peculiarities of different types and subtypes of aggression in the tested junior schoolchildren with normal speech, with PSD, with PPSD and with MGSD is determined. The general level of aggression awareness in junior pupils with normal and disordered speech is studied.

This study used qualitative and quantitative methods, such as OWAS (Ovako working posture analysis system) and behavior observation, to analyze musculoskeletal disorder (MSD) risk factors of power line fixing work in China. Video-based sampling was used to record and analyze the frequency and posture of on-pole activities. Those key subtasks showed ergonomics characteristics of on-pole fixing tasks. Insulator-fixing was the longest subtask (33% of total working time). Bar-installing was the second longest (26% of total working time). It was evident that bar-installing and insulator-fixing were full of hazardous risks. The action categories of the 2 subtasks were higher than of the other ones. The literature points that attachment is key mechanism in the development of people. According to the attachment theory, the experience of neglect, abandonment and other early traumas have influence on forming the dysfunctions of emotions and personality pathology. The aim of this article is the verification of the thesis about relation between attachment style and personality disorders. The analysis of results of 155 participants has been conducted (persons in the similar age, similar educational level, similar number of females and males). The diagnosis of personality disorder was based on SCID-II (Structured Clinical Interview for Axis – II), and additionally confirmed by PDQ-4 Hyler Inventory. To examine the attachment style the Plopa’s Attachment Styles Questionnaire and the Revised Experiences in Close Relationships Inventory (ECR-R designed by Fraley et al., 2000), were used. The multiple regression model was used to test whether attachment styles maybe the predictors for personality disorders. The current results partly support the Lyddon and Sherry’ thesis. The main predictor for the relations between attachment and personality disorders is the anxious attachment style.

Introduction. Stroke is a common vascular disease encountered in the work of a practitioner. Despite this, his differentiation is difficult. This is due to a variety of diseases that can be a “mask” for stroke. These are neurological diseases other than vascular (brain tumors, epilepsy), head injuries and a number of metabolic disorders (fluctuations in glucose, electrolytes). Aim. This work aims to approximate the differentiation of this common disease entity. Case reports show both clinical view and diagnostic difficulties. Description of the cases. Case 1. The patient 59 years old treated for bipolar disorder, hypertension and alcohol abuse. Case 2. The patient 68 years old treated for type 2 diabetes, hypertension, ischemic heart disease, cholecystolithiasis – cholecystectomy. Conclusion. Stroke is a significant clinical and social problem both in Poland and in the world. Despite this, its diagnostics and especially differentiation is difficult. When analyzing the case of each patient with suspected sudden CNS vascular disease, all of them should be remembered. It should also be borne in mind that differential diagnosis cannot delay specific treatment for stroke

Aim of the study: Owing to its wide availability, relatively low cost and lack of negative effect on the patient, ultrasound has become the most commonly and readily used imaging modality. However, scanning for increasingly long periods of time on a given day and in a given week tends to negatively affect sonographers’ health, primarily resulting with the overuse of the musculoskeletal system, as multiple muscles and joints are engaged during scanning. This research has been aimed at evaluating the prevalence and type of musculoskeletal symptoms among diagnostic medical sonographers, as well as identifying their professional profile. Material and method: The study covered 553 sonographers who responded to an online survey comprising 27 questions, including branching questions allowing to provide more detailed information depending on the answers given, as well as open questions. The survey was geared towards identifying the type and frequency of the experienced symptoms, and determining additional contributing factors. Results: 83% of the respondents have experienced work-related musculoskeletal disorders (WRMSD). The study presents the detailed characteristics of the symptoms experienced by sonographers in their work, and their professional profile. Conclusions: A majority of physicians performing ultrasound experience musculoskeletal pain. Deeper analysis of the underlying causes and potential correlations with given contributing factors (variables) that could be effectively addressed may facilitate introduction of some preventive measures and occupational hygiene rules in the field of ultrasound diagnostics, as well as help to implement interventions aimed at relieving the experienced symptoms and improving the health of the examining specialists.
Cel pracy: Badania ultrasonograficzne, dzięki łatwej dostępności, stosunkowo niskim kosztom i brakowi obciążeń dla pacjentów, są obecnie najbardziej powszechną metodą diagnostyki obrazowej. Wykonywanie badań ultrasonograficznych w coraz większym wymiarze godzin może jednak mieć niekorzystny wpływ na zdrowie samych badających, prowadząc przede wszystkim do przeciążenia narządu ruchu, poprzez powtarzające się zaangażowanie szeregu mięśni i stawów. Celem pracy była ocena częstości występowania i rodzaju dolegliwości z zakresu układu mięśniowo-szkieletowego u diagnostów wykonujących badania ultrasonograficzne. Dodatkowo określono profil zawodowy tych lekarzy. Materiał i metoda: Grupę badaną stanowiło 553 lekarzy zajmujących się diagnostyką ultrasonograficzną. Za pomocą ankiety internetowej przygotowano zestaw złożony z 27 pytań, w tym pytań rozgałęzionych, umożliwiających rozszerzenie kwestionariusza w zależności od udzielonych odpowiedzi, oraz pytań otwartych. Ankieta miała na celu scharakteryzowanie rodzaju i częstości występowania dolegliwości oraz określenie dodatkowych czynników mogących mieć wpływ na ich pojawienie się. Wyniki: Wśród respondentów ankiety 83% lekarzy doświadczyło dolegliwości narządu ruchu związanych z wykonywaniem badań ultrasonograficznych. W pracy przedstawiono dokładną charakterystykę dolegliwości i profil zawodowy lekarzy zajmujących się diagnostyką ultrasonograficzną. Wnioski: Dolegliwości układu mięśniowo-szkieletowego dotyczą większości lekarzy wykonujących badania ultrasonograficzne. Dokładne poznanie przyczyn i ewentualnych zależności od czynników, które da się zmodyfikować, może pozwolić zarówno na opracowanie zasad profilaktyki oraz higieny pracy w badaniach ultrasonograficznych, jak i na wdrożenie programów naprawczych mogących przynieść ulgę i poprawę stanu zdrowia badających. Intestinal bacteria and the microbiota-brain-intestin axis have been the subject of intense research in the last decade. Until recently, it seemed unlikely to combine fecal microflora with mood disorders. The growing amount of animal research shows that one of the etiological factors of mood disorders may be irregularities in this area. Evidence indicates the existence of an extremely important two-way relationship between bacteria, intestines and the brain, and that this interaction is complex and takes place on many levels. Understanding and analyzing this dependency gives new possibilities in the therapy of mood disorders, such as the use of psychobiotics, prebiotics or drugs which selectively eliminate specific bacterial strains (antimicrobials). Psychobiotics are „good” bacteria which, when consumed in appropriate doses, have a positive effect on the intestinal axis and on the condition of patients with mental disorders. Studies on their use show good results in the treatment of diseases such as depression. In people suffering from depression, significant differences in the composition of the intestinal microflora were observed, which speaks for its essential role in this disease. Even greater role of bacteria in mood disorders favors research that proves that fecal microbiota transplant entails consequences in the form of behavioral changes.

Empathy and psychopathy seem to be two distant extremes, which only differ, with nothing similar. Therefore, the question that seems to be surprising is whether such a theoretical perspective is justified. Empathy exerts significant influence on social relationships and is associated with moral development, whereas psychopathy seems to be an opposite phenomenon, as it is associated with the lack of deep interpersonal bonds and the violation of legal norms. As studies from various disciplines and scientific areas indicate, such concepts as behavioral effectiveness, morality or altruism might help explain the complex nature of the interrelationship between psychopathy and empathy. The authors tried to explore and describe the complexity of the two presented concepts in the light of the conducted research, and the resulting theoretical and empirical implications.

Somnambulizm (sennowłóctwo) jest nieorganicznym zaburzeniem snu o typie parasomnii, występującym u około 15% dzieci i 3% dorosłych. Klinicznie objawia się wykonywaniem podczas fazy snu non-REM złożonych czynności ruchowych (np. chodzenie, jedzenie), bez pełnego wybudzenia się ze snu oraz przy niepamięci tego wydarzenia w dniu następnym. Wiedza zarówno psychologów, jak i lekarzy na temat somnambulizmu jest najczęściej fragmentaryczna, a w praktyce klinicznej znaczenie tego zaburzenia jest zazwyczaj lekceważone. Celem niniejszego artykułu jest zatem przybliżenie psychologom i innym zainteresowanym specjalistom aktualnego stanu wiedzy na temat somnambulizmu. Artykuł prezentuje zagadnienia częstotliwości jego występowania, obrazu klinicznego, patofizjologii, kryteriów rozpoznawania, współzachorowalności, etiologii, patomechanizmów, sposobów diagnozowania i prób terapii. Przedstawiono również wynikające z dokonanego przeglądu literatury implikacje dla praktyki klinicznej i badawczej psychologów.
Somnambulism (sleepwalking) is a non-organic sleep disorder of the parasomnia type with estimated prevalence of 15% in children and 3% in adults. Its clinical symptoms include performing complex motor activities (e.g. walking, eating) during the non-REM sleep stage, without full awakening and with amnesia of the event on the following morning. The knowledge about somnambulism is usually fragmentary both among psychologists and physicians, and the significance of this disorder tends to be neglected in clinical practice. The objective of this paper is, therefore, to provide psychologists and other professionals with an update on recent developments in the knowledge of somnambulism. The paper addresses the issues of prevalence of sleepwalking, its clinical manifestations, pathophysiology, diagnostic criteria, comorbidity, etiology, pathomechanisms, diagnostic methods and attempts of treatment. Finally, based on the presented review, clinical and research implications for psychologists are outlined.

The use and application of virtual reality techniques has become a standard in both everyday life (entertainment and utility), in training and research. Increasing use of virtual reality, on the one hand, allows specific skills to be practiced, especially in areas with high risk of health impairment or even loss of life (eg. vehicle, machine, or surgeon training), in areas with a difficult access (eg. astronautics) or very expensive ones. On the other hand, observations and research reports indicate the presence of threats in selected spheres of trainees’ life such as interpersonal, cognitive or somatic ones. The aim of the article is to: 1) make and synthesize data showing negative consequences of using virtual techniques for the health of trainees based on the literature, and 2) present the research proposals on the effects of real and virtual world interaction in the human-human-machine sequences on psychophysical and psychosocial functioning using three groups of tools (SSQ - Simulator Sickness Questionnaire, psychological testing battery and eye-tracking).

Introduction: Voice disorders in children especially in the school-age period comprise a major and challenging medical, social and educational issue. There is a lack of epidemiological data regarding children’s voice in Poland which limits the development of preventive and medical pediatric procedures in our country. Objective: The aim of study was to determine the prevalence of voice disorders in children starting school education in Mazovia. Material and method: The study was a retrospective analysis of survey data from parents of children attending the first grade of primary school. The survey consists of five questions describing the voice. A statistical analysis of 7891 questionnaires was carried out. Results: Analysis of the survey showed a prevalence of dysphonia in 12.8% children aged 7 starting primary school. In this study, voice disorders were observed more often in boys (14.6%) than girls (10.8%). The authors presented the correlation between voice disorders and the expression of emotions. Conclusions: Studies have shown that the percentage of children with voice disorders is significant in our country. The survey regarding voice in school children in Mazovia shows preliminary results of voice disorders in the pediatric population in Poland.

The study aims to analyse the risk of eating disorder emergence in secondary school students. Data were collected by questionnaire method. The questionnaire battery consisted of nominal variables and the EDE-Q Eating Disorder Examination Questionnaire scale. 776 secondary school students participated in the research. The mean age of respondents was 17. The empirical research identified 247 (31.8%) respondents displaying symptoms of eating disorders at least in one of four examined dimensions of the EDE-Q scale (Restraint, Eating Concerns, Shape Concerns, Weight Concerns). Significant differences were found in eating disorder symptoms regarding the preferred eating form. Respondents ticking the option “I buy my food” achieved the highest scores. Also, a significant difference in eating disorder symptoms was identified according to the accommodation type, where students living in “sublets” can be considered to be at the highest risk in this regard. A significant difference in the prevalence of eating disorder symptoms was also recorded in terms of gender, where girls scored statistically significantly higher than boys in the monitored dimensions.

The Rapid Upper Limb Assessment (RULA) survey is a posture-sampling tool used specifically to examine the level of risk associated with upper limb disorders of individual workers. This paper examines the relationship between RULA’s postural scoring system and measures of surface electromyography (EMG), self-reports of discomfort, and job attitude questionnaires. Twenty participants each performed a 30-min typing task on a computer in 3 working postures based on RULA’s scoring system. A statistically significant difference was found only in perceived discomfort. The perceived discomfort results demonstrated that RULA was able to identify “high risk” postures. The next question we need to ask is, does perceived discomfort result in tissue damage, or does tissue damage yield discomfort?

Aim. To study the way how Brief Solution Focused Therapy (BSFT) tackles emotional, cognitive and behavioural disorders. To provide basic definitions of such disorders and identify major approaches and techniques focusing on Brief Solution Focused Therapy aspects that may be applied in psychotherapeutic works with these people. Methods. Analysis and synthesis of scientific data regarding defining emotional, cognitive and behavioural disorders. Distinguishing basic diagnostic signs of such disorders in clients. Methods of BSFT psychotherapy. Results. The following BSFT techniques proved to be the most effective in the work on emotional disorders: problem-free talk, compliments, resource questions and scaling. These techniques can be called the most emotional ones – the techniques that call for your emotions, replenish your psychological resource, help rebuild your emotional attitude to the situation, look at it from a different angle. BSFT-based psycho-correction and psychotherapy of cognitive impairments and behavioural disorders is longer and more fundamental. Effective techniques are goal setting, resource identification, and also there are special turning points, such as the technique of life analysis, which is a variant of scaling as a method, use of coping questions. Conclusions. BSFT-methods are effective in addressing different disorders – both partial (emotional, behavioural, cognitive) and more complex ones. The fact that BSFT does not focus on the negative emotional conditions, the problem and its causes, enables the depressive clients, grieving clients and clients experiencing loss to relatively fast discover their resources and start building plans, demonstrating how it is possible to optimize the process of overcoming complicated life situations.

Ecto-purines and ecto-pyrimidines are present in the extracellular space of the central nervous system (CNS). Together with P1 and P2 receptors and nucleotides metabolizing ecto-enzymes, they make signaling system involved in neurotransmission, the modulation of sensory signals, including pain stimuli conduction, and the induction of apoptosis and necrosis of the cells. Purines and pyrimidines have a dual effect: positive (neuroprotective) of nucleosides, and negative (pro-inflammatory and pro-apoptotic) of nucleotides. Adenosine-5'-triphosphate (ATP) in the CNS triggers the pro-inflammatory reactions, predominantly by activation of the P2X7 receptor, which results in production and release of pro-inflammatory cytokines. In contrast to ATP, adenosine acts generally as an anti-inflammatory agent and plays an important role in neuroprotection. Currently, it is believed that the initiation of CNS diseases, including mental disorders, is caused by any imbalance between the concentration of ATP and adenosine in the extracellular space. Genetic tests provide also the evidence for the participation of purinergic signaling in psychiatric disorders. It is believed that any action leading to the effective increase of adenosine concentration: activation of nucleotide metabolizing ecto-enzymes (mainly NTPDases - nucleoside triphosphate diphosphohydrolases), inhibition of adenosine deaminase and/or adenosine kinase activity as well as therapies using P1 receptor agonists (adenosine or its analogues) might be beneficial in therapy of psychiatric disorders.

Głos pozostaje w ścisłym związku i zależnościach z narządem słuchu i mowy, bowiem jednym z najważniejszych uwarunkowań prawidłowego rozwoju mowy i głosu dziecka jest czynnościowo wydolny układ słuchowy, który z jednej strony umożliwia odbiór mowy ze środowiska, a z drugiej, dzięki słuchowej autokontroli, pozwala na doskonalenie wytwarzanej przez dziecko mowy i kontrolę głosu. Różna jest charakterystyka zaburzeń głosu ze względu na różne uszkodzenia/zaburzenia słuchu. Stopień zaburzeń głosu będzie uzależniony od głębokości niedosłuchu, momentu wystąpienia, czasu trwania, efektów dotychczasowej rehabilitacji, rodzaju protezowania słuchowego i indywidualnych kompetencji dziecka. Głos dziecka z uszkodzonym narządem słuchu można zbadać/ocenić w sposób subiektywny bądź w sposób obiektywny przez zastosowanie dostępnych metod analizy akustycznej. Rehabilitacja audiogennych zaburzeń głosu polega, po pierwsze, na zastosowaniu odpowiednich aparatów słuchowych czy implantów słuchowych, a po drugie, na wprowadzeniu odpowiednio dobranych dla potrzeb i możliwości pacjentów ćwiczeń.
The voice is closely related and dependent on the organ of hearing and speech, because one of the most important determinants of the proper development of speech and voice of the child is the functionally efficient auditory system, which on the one hand allows the reception of speech from the environment, and on the other, thanks to auditory self-control, allows for the improvement of the speech produced by the child and control The characteristics of voice disorders vary due to different hearing disorders. The degree of voice disorders will depend on the depth of hearing loss, the moment of occurrence, duration, effects of previous rehabilitation, type of auditory prosthesis and individual competences of the child. The voice of a child with a damaged hearing organ can be examined/assessed subjectively or objectively by using available acoustic analysis methods. Rehabilitation of audiogenic voice disorders consists, firstly, in the use of appropriate hearing aids or hearing implants, and secondly, in the introduction of exercises appropriately selected for the needs and capabilities of patients.

Opis:
Consumption of alcohol during pregnancy is a highly undesirable phenomenon, often causing miscarriages or many abnormalities of the child's development. Fetal Alcohol Spectrum Disorder (FASD) describes physical and mental disorders resulting from the effects of alcohol on health and behavioral disorders of the child, which are usually secondary to changes in the central nervous system caused by fetal alcohol intoxication during pregnancy. These changes are usually irreversible and evident throughout the life of a child affected by FASD, but if appropriate therapies are implemented, it is possible to minimize the symptoms of these disorders. Their nature and severity depend on several factors, including the time of exposure to alcohol and the health status of a pregnant woman. Clinical features of FASD include craniofacial anomalies, central nervous system disorders and growth retardation. Alcohol-induced structural changes include anomalies in the cardiovascular, skeletal, renal and urinary systems as well as in the organs of sight and hearing. Time of initial diagnosis plays an important role, because it allows the introduction of appropriate therapy for a child with FASD and the introduction of appropriate education of parents and other members of the family. It is possible to minimize the symptoms and disorders resulting from the Fetal Alcohol Disorders Spectrum, to the extent enabling them proper functioning of the child and his family.

Background. Assessment of the level of exposure to work-related musculoskeletal disorders (WMSDs) risk factors can be an appropriate basis for planning and implementing an interventional ergonomics program in the workplace. This study was conducted among workers of an Iranian sugar-producing factory to determine WMSD prevalence rate among production workers and to assess the level of exposure to WMSD risks. Materials and methods. In total, 116 workers were randomly selected from production workshops and included in the study. The Nordic musculoskeletal questionnaire was used to study prevalence of WMSDs and a quick exposure check (QEC) was used to assess physical exposure to risks. Required data were videotaped. Results. Most workers (87.1%) suffered from some kind of MSD symptoms during the 12 months prior to the study. The highest prevalence was reported in knees (58.6%) and the lower back (54.3%). In 99.1% of the workers, the level exposure to MSD risks established with QEC was high and very high. Awkward postures, manual material handling, and long hours of standing were the major ergonomics problems. Conclusion. There was a here was a high rate of WMSDs in this factory. The level of exposure to WMSD risk factors was high and corrective measures for reducing risk level were essential.

This article discusses the process of changes to a patient’s personality organization and God image in the course of long-term psychodynamically oriented psychotherapy. The patient remained a declared atheist, and so these changes did not involve his general views; rather, they only concerned his manner of experiencing his own and others’ faith. The patient’s personality organization and God image were examined twice: at the beginning of psychotherapy and after two years of contact. Those assessments used the personality organization diagnostic form and Ana-Maria Rizzuto’s God/Family Questionnaire. The process of changes in the God image and changes in approach to religious life is considered from the point of view of modern psychoanalysis, and especially from the perspective of object relations theories. Tis article addresses the maturity of different religious declarations and the relation to mental health from both a Freudian and a more modern psychoanalytical perspective.

The systemic lupus erythematosus (SLE) is a connective tissue disorder, being developed on the basis of the autoallergy. Clinical symptoms of SLE are connected not only with the occurrence of cutaneous exanthemata, but can also manifest itself with the row of symptoms on the part of internal organs, with haematologic distempers and symptoms about the neuropsychiatric character. The pathogenesis of neuropsychiatric systemic lupus erythematosus (NPSLE) is complicated and one tries to become clear her with the production of autoantibodies and with the postponement himself antigen–antibody complexes, what a succession is the cytotoxic damage of neurons, an irregular expression of inflammatory mediators and an influx of inflammatory cells, vascular distempers and an inclination to thrombuses. The entire frequency of the occurrence NPSLE is estimated on 56.3% patients with SLE. Most often mental disorders in patients with lupus erythematosus are mood disorder, anxiety disorder, cognitive disorders and consciousness clouding. An essential aspect there are also drug-induced mental trouble. In the therapeutic approach in the therapy of NPSLE about the slight activity of the disease and/or the slight degree of organ changes complies glucocorticoids in low doses and depending on needs medicines counterphobic, antidepressant, sometimes the valproic acid, the carbamazepine, the lamotrigine and anti-psychotic preparations. In the antithrombotic prophylaxis at ill from the risk population the acetylsalicylic acid complies.
Toczeń rumieniowaty układowy (systemic lupus erythematosus, SLE) jest chorobą tkanki łącznej, rozwijającą się na podłożu autoimmunizacji. Objawy kliniczne SLE są związane z występowaniem nie tylko wykwitów skórnych, ale również dolegliwości ze strony narządów wewnętrznych, a także zaburzeń hematologicznych oraz o charakterze neuropsychiatrycznym. Patogeneza NPSLE (neuropsychiatric systemic lupus erythematosus) jest złożona, próbuje się ją wyjaśniać wytwarzaniem autoprzeciwciał oraz odkładaniem się kompleksów immunologicznych, czego następstwem jest cytotoksyczne uszkodzenie neuronów, jak również nieprawidłową ekspresją mediatorów zapalnych i napływem komórek zapalnych, zaburzeniami naczyniowymi i skłonnością do zakrzepów. Całkowita częstość występowania NPSLE jest szacowana na 56,3% pacjentów z SLE. Najczęściej występującymi zaburzeniami psychicznymi u chorych na toczeń rumieniowaty są zaburzenia nastroju, lękowe, poznawcze oraz świadomości. Istotnym aspektem są także polekowe zaburzenia psychiczne. W podejściu terapeutycznym wobec pacjentów z NPSLE o nieznacznej aktywności choroby i/lub nieznacznym stopniu zmian narządowych stosuje się glikokortykosteroidy w niskich dawkach oraz – w zależności od potrzeb – leki przeciwlękowe, przeciwdepresyjne, niekiedy kwas walproinowy, karbamazepinę, lamotryginę i preparaty przeciwpsychotyczne. W profilaktyce przeciwzakrzepowej u chorych z grupy ryzyka stosuje się kwas acetylosalicylowy.

The aim of this study was to determine how nonverbal and verbal dysfunctions in preschool children are associated with severe speech disorders. In order to achieve the objectives of the research a special method was developed, which consisted of two modules - “Nonverbal” and “Verbal”. At the first stage, the study was conducted for each module, which allowed us to determine that children with speech disorders have significant deviations in auditory gnosis development, which causes speech disorders. However, at the second stage of the study a controversial issue arose: how do existing nonverbal auditory gnosis disorders stipulate the dysfunctions of verbal auditory gnosis development? The results of the correlation analysis show that there is a strong relationship between non-verbal and verbal auditory gnosis. Yet, if the development occurs spontaneously, without considering the “from simple-to-complex” principle, there is a significant imbalance in the formation of complex syntagmatic constructions. Therefore, during the corrective work, it is necessary to differentiate corrective tasks depending on the level of auditory gnosis development. A promising direction for further studies is the application of the developed diagnostic range of tools for creating special correction programs aimed at auditory gnosis development in children with severe speech disorders.
