Introduction and aim. Nursing fields begin to face a wide variety of challenges when they first enter clinical practice. The aim of this study is to determine the difficulties experienced in patient communication by nursing students taking the clinical practice course for the first time. Material and methods. Focus group interview design was used for this study. Focus group design is used to give information about the opinions and experiences of the sample group on any subject. It is reported as a convenient design to identify different perspectives on a subject within the scope of the sample. Focus group interviews are a widely used method as data collection technique. Results. The findings of the study continue on 4 main themes at level 3, by identifying the similarities and differences in the codes determined by the three experts (researchers consulted and the researcher conducting/reporting the study). The findings of the study were evaluated in comparison with the findings of the studies reached in the literature and conducted in regions such as Israel, Iran, Sri-Lanka, Kenya, and Africa. Conclusion. There are many problems in student-patient communication in many different dimensions. The difficulties arising due to these problems generally focus on unknown clinical environment, lack of knowledge, differences between theory and practice, and mentor interaction.

Diabetic retinopathy as a complication of diabetes is one of the most common causes of vision loss. It is a dangerous disease, which affects more and more people. It is important to know its different causes, background and to generate new, effective therapeutic threatments. One of the possible causes of diabetic retinopathy is the accumulation of advanced glycation products and over-expression of AGEs receptors (RAGE) leading to tissue damage, oxidative stress formation, and the promotion of other changes and disorders. There are many genetic variants that encode the AGEs receptor (RAGE). Some polymorphisms (Gly82Ser, 1704G/T, 2184A/G, -429T/C, -374T/A) may be associated with diabetic retinopathy. This paper aims to systematize knowledge in this field and to show the relationship between these compounds and diabetic retinopathy.

Opis:
One of the earliest critical secondary complications of diabetes is the opacification of the eye lens - a condition strictly associated with diabetic cataract. The study presented here was designed to investigate the effect of Ginkgo biloba extract (GbE), rutin and quercetin on streptozotocin (STZ) induced diabetic cataract (DC) rats. Ten weeks after administration of GbE, rutin and quercetin, the opacity of diabetic rats' lenses was graded under a slit lamp. Then, the levels of malondialdehyde (MDA), reduced glutathione (GSH), advanced glycosylation end products (AGEs), and the activities of aldose reductase (AR) were estimated. The DC-induced rats produced less GSH, higher levels of MDA and AGEs as well as elevated AR activity when compared to the normal group. Administration of GbE, rutin and quercetin remarkably inhibited the AR activity, stimulated the production of glutathione, and decreased the levels of MDA and AGEs in the lenses of DC-induced rats, which eventually delayed the progression of lens opacification in diabetic rats to various degrees. Our results revealed that quercetin had the highest significant (P<0.05) potential to delay the progression of STZ-induced diabetic cataract when compared with rutin and GbE. The mechanism dictating this interesting prowess of quercetin might be attributed to its AR inhibitory strength, anti-lipid peroxidation potential and anti-AGEs activity.

Introduction. Correctness of insulin treatment depends on both the experience and skills of the doctor and knowledge and behaviour of the patient. Objective. Evaluation the adequacy of insulin doses administered to diabetes patients in ambulatory conditions. Material and methods. The treatment of 59 patients hospitalized in the Diabetology Ward was evaluated at admission, discharge and 3 months after hospitalization. Results. The mean daily doses of insulin significantly differed at times of evaluation and were: 53.90, 39.31 and 43.34 units, respectively (p≈0.000001). A significant reduction of body weight, 90.86 vs. 88.25 kg (p≈0.000001), was obtained only during hospitalization, and was maintained 3 months after discharge (87.86 kg). Significant differences were also noted in the body mass index (33.44 vs. 32.48 vs. 32.37 kg/m2, p≈0.000001). The change in waist circumference was not statistically significant (107.87 vs. 104.89 cm; p≈0.06). A decrease in the number of hypoglycaemia episodes was observed, but were statistically insignificant (25 vs. 23; p≈0.7). Three months after hospitalization an insignificant decrease of HbA1c level was noted (8.41% vs. 8.03%; p≈0.07). Conclusions. During treatment in the Diabetology Ward the procedure of choice was more frequently a reduction than an increase in insulin doses. This management led to the reduction of the patients’ body weight, improvement of glycaemia, without any significant effect on the diabetes control determined by the HbA1c level.

In search for new markers of insulin-dependent diabetes (IDDM) susceptibility we studied the CATT tetranucleotide repeat in intron 1 of tyrosine hydroxylase (TH) gene on chromosome lip, the CA repeat at T-cell receptor a chain (TCRA) locus on chromosome 14q and two CA repeats at D10S211 and D10S213 loci in the chromosome 10 region containing glutamic acid decarboxylase (GAD2) gene. Alleles at these microsatellite loci were identified in a population of diabetic children and unrelated healthy controls originating from Wielkopolska, a midwestern region of Poland. We found significant association of certain alleles at TH, TCRA and D10S211 loci with diabetes in the population under study. On the contrary, none of the alleles at D10S213 locus was associated with the disease. Our findings indicate that typing of microsatellite markers may represent useful additional tool for identifying individuals at high risk of developing IDDM. Regarding loci on chromosome 10 our data and data published by other authors may suggest the extistence of two separate regions of association with IDDM susceptibility on this chromosome.

Objectives: The objective of the present study was to perform an analysis of the barriers to the practice of physical activity and diet in people with diabetes mellitus residing in Spain. Methods: Ninety participants (40 men and 50 women) with type 1 diabetes mellitus (71) and type 2 diabetes mellitus (19) residing in Spain, and aged between 12 and 80 years (average age 41.12 ± 17.04) were part of the present study. The participants were asked about the sociodemographic variables of sex and age, as well as the type of diabetes, the presence of other associated pathologies and the years elapsed since their diagnosis. The participants were also asked about the consumption of tobacco and alcohol. Afterwards, the barriers or difficulties perceived in relation to physical exercise were assessed through the Barriers Self-Report for the Practice of Physical Activity (BSPPA) and the diet with the Questionnaire of Adherence to the Mediterranean Diet (MEDAS-14). Then, the participants provided information regarding their last blood test with respect to the levels of glycosylated hemoglobin (HbA1c) and fasting glucose. A descriptive analysis was carried out to present the characteristics of the sample as well as the barriers for the practice of physical activity presented by the sample studied. Finally, a comparison analysis of means was carried out using the Student's t test for independent samples, showing the significant differences between the barriers for the practice of physical activity according to the characteristics of the sample. Findings: The data showed how the Obligations / Lack of time factor represents the main barrier to the practice of physical activity in this population, especially in women, in those under 65 and in people with type 1 diabetes. Conclusions: In general, it is observed that all those categories that may indicate a less healthy lifestyle have a greater degree of agreement with the perceived barriers.

In this study, 110 newly synthesized vanadium complexes from different structural groups were screened in three cell-based models representing the main target tissues for anti-diabetic drugs. In glucose utilization in C2C12 myocyte experiments, 93% of vanadium complexes were shown to have equal or greater activity than bis(maltolato)oxovanadium(IV) (BMOV), the methyl analog of bis(ethylmaltolato)oxovanadium(IV) (BEOV) which has been tested in clinical trials. Moreover, 49% and 50% of these complexes were shown to have equal or greater activity than BMOV in lipid accumulation in 3T3-L1 adipocytes and insulin secretion in RINm5F beta cell experiments, respectively. These results were the basis for the selection of compounds for the subsequent steps in the characterization of anti-diabetic properties. This study provides strong support for the application of screening cell-based assays with a phenotypic approach for the discovery of novel anti-diabetic drugs from the vanadium complex class. This is especially desirable due to the multiple and not fully defined mechanisms of action vanadium compounds.

Introduction. An important way of preventing type 2diabetes is by adopting a proper diet by which means appropriate control over blood glycaemia and lipids can be achieved. Objectives. To assess selected biochemical and haematological markers in overweight subjects or those suffering from type 2 diabetes in relation to their estimated dietary intake. Material and methods. The study was conducted in 2012 on n=86 overweight or obese subjects living in Warsaw or its environs, of whom n=43 had type 2 diabetes. Dietary intakes were compared between non-diabetics (control group) and diabetics (test group) by 3 day records, whilst the relevant blood biochemistry and haematology results were obtained from medical records; with patient consent. Results. Diabetic subjects had significantly higher serum glucose and CRP levels than controls, respectively; 190 vs 98 mg/ dl and 1.4 vs 1.1 mg/dl. Lipid profiles were however more significantly abnormal in controls, compared to diabetics with respectively; total cholesterol 220 vs 194 mg/dl, LDL-cholesterol 131 vs 107 mg/dl and triglycerides 206 vs 157 mg/dl. There were no significant differences in HDL-cholesterol; respectively 55 vs 51 mg/dl. In the diabetics, calorific intakes from carbohydrates, especially sugars, were significantly lower than controls i.e. 9% vs 13%. The proportional share of calories derived from dietary fats did not differ between groups, nevertheless a positive correlation was observed between dietary fat content with blood cholesterol concentrations in diabetics. Conclusions. Disorders of carbohydrate metabolism were confirmed in both overweight and diabetic (type 2) subjects. In addition, both groups demonstrated untoward lipid profiles that correlated with their improper nutrition.
Wprowadzenie. Prawidłowe żywienie, którego celem jest wyrównanie glikemii oraz profilu lipidowego odgrywa zasadniczą rolę w profilaktyce cukrzycy typu 2. Cel pracy. Ocena wybranych wskaźników biochemicznych krwi (glukoza, lipidogram, białko CRP) u osób z nadmierną masą ciała oraz cukrzycą typu 2 w aspekcie ich sposobu żywienia. Materiał i metody. Badanie przeprowadzono w 2012 roku, wśród 86 mieszkańców Warszawy i okolic, z nadwaga i otyłością, w tym u 43 osób z cukrzycą. Do oceny sposobu żywienia wykorzystano metodę trzydniowego bieżącego notowania. Dane dotyczące wskaźników biochemicznych krwi, za zgodą badanych uzyskano z ich kart zdrowia. Wyniki. U pacjentów z cukrzycą odnotowano istotnie wyższe średnie stężenie glukozy w surowicy krwi (190 vs. 98 mg/dl), jak również wyższe stężenie białka CRP (1,4 vs. 1,1 mg/dl). Biorąc pod uwagę wskaźniki gospodarki lipidowej stwierdzono większe nieprawidłowości w grupie kontrolnej (cholesterol ogółem 220 vs. 194 mg/dl; cholesterol LDL 131 vs. 107 mg/dl; triacyloglicerole 206 vs. 157 mg/dl). Stężenie cholesterolu frakcji HDL nie różniło się istotnie w obydwu grupach (55 vs. 51 mg/dl). Spożycie energii z węglowodanów, zwłaszcza prostych u chorych na cukrzycę było istotnie statystycznie niższe niż w grupie kontrolnej (9 vs. 13%). Procentowy udział tłuszczu w dostarczeniu energii nie różnił się znacząco pomiędzy grupami, przy czym w grupie z cukrzycą odnotowano dodatnią korelację pomiędzy ilością tłuszczu w diecie, a stężeniem cholesterolu we krwi. Wnioski. Uzyskane wyniki potwierdzają występowanie zaburzeń gospodarki węglowodanowej u pacjentów z nadmierną masą ciała oraz cukrzycą typu 2. Ponadto w badanych grupach zaobserwowano niekorzystny profil lipidowy korelujący z ich nieprawidłowym sposobem żywienia.

Introduction. Late complications of diabetes mellitus are a serious challenge for physicians treating diabetic patients. Micro- and macro-angiopathic changes are of the most severe sequels. However, it should be remembered that the effects of long-term diabetes are also associated with changes in the peripheral nervous system. One of the complications most troublesome for patients is diabetic autonomic neuropathy of the gastrointestinal (GI) tract. Objective. The review article presents the pathogenesis of diabetic neuropathy of the autonomous gastrointestinal tract, and advances in the diagnosis and treatment of this disease. The study used materials in the database which demonstrate current standards of conduct, based on the principles of EBM. Brief description of the state of knowledge. Diabetic neuropathy is one of the most common complications of diabetes mellitus. According to epidemiologists, it affects 10% to even 100% of patients with long-standing disease. Damage to the nervous fibres mainly results from hyperglycaemia, which leads to the typical symptoms of dysfunction of GI tract segments. The main additional examinations include abdominal ultrasound (US) and abdominal X-ray. In cases with oesophageal disorders accompanied by dysphagia, gastroscopy is found useful for excluding possible neoplastic lesions. The gold standard for evaluation of gastric emptying is radioisotopic scintigraphy, however it is available only in some specialistic departments. Other recommended methods include: manometry, EGG, insulin tests, NMR and expiratory tests. To date, the problem has been managed symptomatically; in special cases, surgical interventions have been required. Conclusions. At present, a wide array of treatment options is available, including new forms of drugs and less radical surgical procedures carried out in numerous centres worldwide.

Cukrzyca jest chorobą metaboliczną o złożonej i różnorakiej etiologii, która charakteryzuje się stanem przewlekłego, podwyższonego poziomu glukozy we krwi. Typ 2 cukrzycy stanowi około 90% wszystkich przypadków tej choroby. Cukrzyca jest chorobą cywilizacyjną, dotykającą osób coraz młodszych, dlatego też wraz ze wzrostem liczby nowych przypadków zwiększają się koszty nie tylko diagnostyki i leczenia cukrzycy i jej powikłań, ale i koszty wynikające z przedwczesnego zaprzestania działalności zawodowej, a także bezrobocia, które dotyka osoby chorujące. Diabetycy często mają problem ze znalezieniem odpowiedniego zajęcia, a ci, którzy pracują boją się utraty pracy ze względu na powszechnie panujące, a nieprawdziwe przekonanie dotyczące ich częstszej absencji w pracy czy niższej wydajności. Wykonywanie pracy zawodowej przez diabetyka jest zalecane i korzystne i tylko nieliczne zawody są niewskazane. Aby pracownik z cukrzycą mógł bezpiecznie pracować, należy stworzyć mu odpowiednie warunki pracy, a edukacja na temat cukrzycy musi dotyczyć wszystkich – nie tylko samego pacjenta, lecz także jego rodzinę, znajomych oraz współpracowników.
Diabetes is a metabolic disease with complex and varied etiology characterized by chronic elevated level of glucose in the blood. Type 2 diabetes accounts for about 90% of all cases of the disease. Diabetes is a civilization disease, affecting a person younger and younger, and therefore with an increase in the number of new cases of the disease increase not only costs of the diagnosis and treatment of diabetes and its complications, but also the costs resulting from premature cessation of employment and unemployment, which affects people suffering from diabetes. Diabetics often have trouble with finding the appropriate job and those who work are afraid of losing their jobs because of the false belief about their frequent absenteeism and lower productivity. Performance of work by a diabetic is recommended and beneficial, and only a few professions are not recommended for them. To employee with diabetes can safely work, you must create it with adequate working conditions and education in diabetes must apply to all - not only the patient but also his family, friends and co-workers.

Opis:
Neuropathic pain may be caused by a variety of damages or diseases of both the peripheral and central nervous system. The most common and best known syndromes of peripheral neuropathic pain are painful diabetic neuropathy, trigeminal neuralgia and postherpetic neuralgia, persistent post-operative and post-traumatic pain, complex regional pain syndrome, cancer-related neuropathic pain, HIV-related neuropathic pain and pain after amputation. The much less common central pain is primarily central post-stroke pain, post-traumatic pain in spinal cord, central pain in Parkinson's disease, in other neurodegenerative diseases, in syringomyelia and multiple sclerosis. A multidisciplinary team of Polish experts, commissioned by the Polish Association for the Study of Pain and the Polish Neurological Society, has reviewed the literature on various types of neuropathic pain, with special focus on the so far published international recommendations, and has formulated recommendations on their diagnosis and treatment, in accordance with the principles of evidence-based medicine (EBM). The number of randomised studies on the efficacy of various medicines and medical procedures in many neuropathic pain syndromes is small, which makes the strength of published recommendations poor.
Ból neuropatyczny może być wywołany różnymi uszkodzeniami lub chorobami, zarówno obwodowego, jak i ośrodkowego układu nerwowego. Do najczęstszych i najlepiej poznanych zespołów obwodowego neuropatycznego zalicza się bolesną neuropatię cukrzycową, neuralgię trójdzielną i popołpaścowa, przetrwały ból pooperacyjny i pourazowy, zespół wieloobjawowego bólu miejscowego, neuropatyczny ból nowotworowy i w przebiegu infekcji wirusem HIV, a także bóle po amputacji. Znacznie rzadszy ból ośrodkowy to przede wszystkim ośrodkowy ból poudarowy, pourazowy ból rdzeniowy, ośrodkowy ból w chorobie Parkinsona, w innych chorobach neurozwyrodnieniowych, w jamistości rdzenia i w stwardnieniu rozsianym. Wielodyscyplinarny zespół polskich ekspertów, powołany przez Polskie Towarzystwo Badania Bólu, oraz Polskie Towarzystwo Neurologiczne dokonał przeglądu piśmiennictwa na temat różnych rodzajów bólu neuropatycznego, ze szczególnym uwzględnieniem opublikowanych dotychczas międzynarodowych zaleceń i sformułował zalecenia dotyczące ich rozpoznawania i leczenia, zgodne z zasadami medycyny opartej na faktach (EBM). Liczba randomizowanych badań na temat skuteczności różnych leków i innych procedur medycznych w wielu zespołach bólu neuropatycznego jest niewielka, co powoduje, że siła publikowanych zaleceń jest słaba.

Opis:
W referacie opisano problem wykrywania oraz klasyfikacji stanu retinopatii cukrzycowej ze zdjęć dna oka przy pomocy głębokich sieci neuronowych. Retinopatia cukrzycowa jest chorobą oczu często występującą u osób z cukrzycą. Nieleczona prowadzi do uszkodzenia wzroku, a nawet ślepoty. W pracy badawczej opracowano system wykrywania retinopatii cukrzycowej na podstawie zdjęć dna oka. Opracowana sieć neuronowa przypisuje stan choroby w 5 stopniowej skali – od braku choroby do najbardziej zaawansowanego stanu choroby. Zaproponowano specjalny system kodowania klas w celu uchwycenia wielkości różnicy pomiędzy rzeczywistymi a predykowanymi stanami choroby. Uzyskano wysokie wyniki klasyfikacji na zbiorze testowym. W celu oceny skuteczności działania systemu wykorzystano miary statystyczne takie jak ważona Kappa i dokładność.
In the paper we described computer aided detection system of diabetic retinopathy based on fundus photos of retina. Diabetic retinopathy is an eye disease associated with diabetes. Non-treated diabetic retinopathy leads to sight degeneration and even blindness. Early detection is crucial due to provide effective treatment. Currently, diabetic retinopathy detection is time consuming process, done manualy by medical specialist. The disease is dangerous issue in places where the availability of phisicians is limited. We employed the computer system that detect diabetic retinopathy and assess a stage of the disease based on retinal photo of fundus. We used one of the best image classification system – deep neural networks. Employed system assess the stage of the disease in 5 level scale – from absence of disease to the most severe stage of disease. We employed transfer learning and data augmentation to enhance classification result. Moreover we proposed special class coding system to catch the difference between real and predicted stage of disease. We tested employed system using different statistical measures like accuracy, sensitivity, specificity and Kappa score.

Diabetes mellitus (DM) is a group of metabolic diseases which typically presents with frequent urination, increased thirst and increased hunger. DM be classified into three main types: type I (insulin-dependent DM), type-II (non-insulin dependent DM) and type III (gestational DM). Diabetes is a group of diseases of global health significance as 382 million people worldwide had diabetes in the year 2013 and this was projected to increase to an estimated 415 million in 2015. Damage to the nerves of the body (diabetic neuropathy) is the most common complication of diabetes. The signs and symptoms of diabetic neuropathy include numbness, diminished sensation, pain etc. Various types of electrotherapy, such as transcutaneous electrical nerve stimulation (TENS), pulsed-dose electrical stimulation, frequency-modulated electromagnetic neural stimulation, have been reported effective in managing diabetic neuropathy. This study is a systematic review of the evidence to enable the determination of the effectiveness of electrical stimulation and low-intensity laser therapy (LILT), and also aid their recommendation if proven to be effective. The outcome of this study was that TENS and other forms of electrical stimulation reviewed in this study may be effective and safe non-pharmacological treatment modalities in relieving the symptoms associated with diabetic neuropathy. The effectiveness of LILT couldn’t be determined due to the different parameters used to evaluate patients’ outcome and limited number of studies. Authors recommend that further randomized controlled trials with similar methodological parameters and studies with higher quality of evidences are needed to establish the true effectiveness of these modalities in diabetic neuropathy.

The leaves, stems, and bark of Eucommia ulmoides Oliv. (EU), also known as Du-Zhong, have traditionally been used to cure various diseases, such as liver, kidney, and muscle diseases, in Asia. Despite evidence for protective effects against renal complications, its precise effects and mechanisms of action are unclear. In this study, the effects of EU on advanced glycation end products (AGEs)-induced renal disease and its mechanism were examined. NRK 52E normal rat kidney tubular epithelial cells were treated with AGEs and an EU extract. Expression levels of TGF-β1, an indicator of renal cell damage, and catalase, an antioxidant marker, were examined. Nuclear factor-E2-related factor 2 (Nrf2), kelch-like ECH-associated protein 1 (keap1), and p65 regulation were examined to identify additional antioxidant mechanisms related to renal cell apoptosis and AGEs-induced renal cell damage. The effects of EU on mitogen-activated protein kinase (MAPK), Akt, and phosphoinositide 3-kinase (PI3K), which are involved in apoptosis, were also examined. TGF-β1 expression increased in response to AGEs and decreased by additional treatment with EU. Additionally, EU increased the expression of catalase. We found that EU increased Nrf2, keap1, and p65 and regulated the expression of RAGE (receptor for AGEs) and its downstream target Sirt1. EU also regulated the AGEs-altered phosphorylation of apoptosis factors. Based on these findings, we concluded that EU regulates AGEs-induced renal cell damage via antioxidant and apoptosis-related mechanisms.
