Introduction and aim. Nursing fields begin to face a wide variety of challenges when they first enter clinical practice. The aim of this study is to determine the difficulties experienced in patient communication by nursing students taking the clinical practice course for the first time. Material and methods. Focus group interview design was used for this study. Focus group design is used to give information about the opinions and experiences of the sample group on any subject. It is reported as a convenient design to identify different perspectives on a subject within the scope of the sample. Focus group interviews are a widely used method as data collection technique. Results. The findings of the study continue on 4 main themes at level 3, by identifying the similarities and differences in the codes determined by the three experts (researchers consulted and the researcher conducting/reporting the study). The findings of the study were evaluated in comparison with the findings of the studies reached in the literature and conducted in regions such as Israel, Iran, Sri-Lanka, Kenya, and Africa. Conclusion. There are many problems in student-patient communication in many different dimensions. The difficulties arising due to these problems generally focus on unknown clinical environment, lack of knowledge, differences between theory and practice, and mentor interaction.

Objectiv. To evaluate the efficacy of subthreshold micropulse yellow (577 nm) laser photocoagulation in diffuse macular edema (DME) in rural and urban patients of south-eastern Poland. Materials and method. Seventy-five eyes of 75 patients with diffuse DME were treated with subthreshold micropulse yellow laser photocoagulation with a 5% duty cycle at an energy level. The laser exposure time was 20 ms and the spot diameter was 100 μm. Best corrected visual acuity (BCVA), reading vision (Snellen) and optical coherence tomography-determined central retinal thickness (CRT) were estimated before and 2, 4 and 6 months after laser treatment. There were no statistically important differences in: the advancement of DME, HbA1c (glycated hemoglobin) level, duration of diabetes mellitus (DM), the degree of vision damage between rural and urban patients. Results. The follow-up was 6 months later. The baseline BCVA was 0.20 and remained stable- 0.3 after 6 months. The Snellen at baseline was 1.0 and improved to 0.5 finally (p=0.0004). The CRT at baseline was 500 μm and changed to 346 μm (p=0.00000) at the final follow-up. Finally, no retinal damage was observed. Conclusions. Place of residence had no statistically significant effect on the demographics data, baseline visual acuity, reading visual acuity and central retinal thickness. Subthreshold micropulse yellow laser showed a highly significant efficiency in the treatment of DME. The effects of the treatment were more significant in rural patients than in urban ones.

Introduction and aim. This study aimed to compare the hospitalization rate, mortality rate and morbidity status of patients hospitalized with stroke and diabetic ketoacidosis (DKA) before and during the COVID-19 pandemic. Material and methods. The data of 2522 patients who applied to the emergency department (ED) before and during the pandemic were evaluated. A Poisson regression analysis was used to examine the number of presentations between two different periods. Results. Stroke cases during the pandemic era were compared to those during the pre-pandemic period, and it was shown that the mortality rate for stroke patients during the pandemic period was much higher. Treatment-related ED presentations decreased significantly during the pandemic period, particularly among patients aged 75–84 years. Rates of ED presentation decreased by 84 percent (IRR: 0.14, 95 percent CI: 0.03-0.59) in those with DKA and by 37 percent (IRR: 0.67, 95 percent CI: 0.53- 0.75) in those with stroke during the pandemic period. Conclusion. Conclusion: Stroke and DKA admissions decreased during the pandemic, but the rate of stroke mortality increased statistically 3.375 times. Getting emergency medical care increases their chances of survival. Even in a COVID-19 outbreak, treatment is critical.

Introduction. High amputation of the lower limb not only causes immense physical disability but also the destruction of the patient’s mental state, and helps to shorten life of patients with diabetes. The incidence of amputations in diabetic patients is 10 times higher in comparison to non-diabetic subjects (2.8% vs. 0.29%). The purpose of the study is an analysis of the geographic variability of major non-traumatic lower limb amputation in diabetic and non-diabetic patients in Poland. Materials and method. All major non-traumatic lower limb amputations performed for the first time, in particular data between 1 January 2013 – 31 December 2013, and between 1 January 2014 – December 2014, were identified in the National Health Fund (NHF) database. In the presented study, the patients were grouped in relevant provincial departments of the NHF according to their place of residence, and not according to the hospital where lower limb amputation was performed. Results. In 2013 in Poland, 4,727 major non-traumatic lower limb amputation were performed in diabetic patients, and 4,350 in 2014. On the other hand, in non-diabetic patients, 3,469 major non-traumatic lower limb amputations were performed in 2013, and 3149 in 2014. The mean number indicator of major non-traumatic lower limb amputations in diabetic patients in Poland, compared to the average indicator of amputations in patients without diagnosed diabetes in Poland was 19.9-fold in 2013 and 19.4-fold higher in 2014. Conclusions. In populations of diabetic patients and individuals without diagnosed diabetes major non-traumatic lower limb amputations are performed over 19-fold more frequently.

The sarcoplasmic reticulum Ca2+-ATPase (SERCA) isoforms are normally expressed in coordination with the corresponding myosin heavy chain (MyHC) isoforms in the fibers of skeletal muscle but this coordination is often disrupted in pathological conditions. In the streptozotocin-induced diabetes of rats (stz-rats), the soleus muscle showed peripheral neuropathy and the SERCA2a level decreased in type I (slow-oxidative) fibers compared to the control muscles, whereas the expression of the corresponding slow MyHC1 did not change. No difference was found at the mRNA and protein levels of SERCA and MyHC isoforms in the whole soleus, except that the level of the SERCA2a protein specifically declined in stz-rats compared to the controls. This shows that the coordinated expression of SERCA2a and MyHC1 is disrupted at the SERCA2a protein level in the diabetic soleus. The results are in line with previous observations that regulators of the Ca-homeostasis may adapt faster to type I diabetes than the contractile elements.

Opis:
One of the earliest critical secondary complications of diabetes is the opacification of the eye lens - a condition strictly associated with diabetic cataract. The study presented here was designed to investigate the effect of Ginkgo biloba extract (GbE), rutin and quercetin on streptozotocin (STZ) induced diabetic cataract (DC) rats. Ten weeks after administration of GbE, rutin and quercetin, the opacity of diabetic rats' lenses was graded under a slit lamp. Then, the levels of malondialdehyde (MDA), reduced glutathione (GSH), advanced glycosylation end products (AGEs), and the activities of aldose reductase (AR) were estimated. The DC-induced rats produced less GSH, higher levels of MDA and AGEs as well as elevated AR activity when compared to the normal group. Administration of GbE, rutin and quercetin remarkably inhibited the AR activity, stimulated the production of glutathione, and decreased the levels of MDA and AGEs in the lenses of DC-induced rats, which eventually delayed the progression of lens opacification in diabetic rats to various degrees. Our results revealed that quercetin had the highest significant (P<0.05) potential to delay the progression of STZ-induced diabetic cataract when compared with rutin and GbE. The mechanism dictating this interesting prowess of quercetin might be attributed to its AR inhibitory strength, anti-lipid peroxidation potential and anti-AGEs activity.

W roku 2015 przeprowadzono badania wśród 86 klientów losowo wybranego gabinetu kosmetycznego w Krakowie (Południowa Polska). Celem pracy była ocena wiedzy respondentów na temat pielęgnacji i profilaktyki stopy cukrzycowej. Dodatkowym celem było wskazanie roli podologa w profilaktyce chorób stóp. Grupę badawczą stanowiło 36 kobiet oraz 26 mężczyzn powyżej siedemdziesięciu lat, 19 kobiet i 14 mężczyzn w przedziale wiekowym od 51-70 lat i 6 kobiet 3 mężczyzn w przedziale wiekowym od 30-50 lat. Przeprowadzona ankieta była anonimowa i składa się z 35 pytań. Dotyczyła wiedzy na temat pielęgnacji stóp: ich sposobu mycia, nawilżania, masowania, obcinania paznokci, usuwania nawarstwionego naskórka, używanego obuwia, a także świadomości kontroli podstawowych parametrów, ważnych w profilaktyce cukrzycy. Badania pokazały, iż respondenci podczas pielęgnacji stóp popełniają podstawowe błędy, których można byłoby uniknąć w przypadku kontaktu z podologiem. Najbardziej zainteresowaną grupą w samodzielnej pielęgnacji stóp okazali się chorzy z przedziału 51-70 lat, który mieli stosunkowo największą wiedzę na ten temat. Ankieta pokazała, że rola podologa polega na uświadomieniu i wdrożeniu u pacjenta prostych zaleceń, które uchronią go przed powikłaniami oraz nauczą właściwej pielęgnacji kończyn dolnych w zespole stopy cukrzycowej.

Troxerutin is a well and for a long time known substance, it is a semi-synthetic bioflavonoid derived from rutin. Original substance is supplied from flowers of Styphnolobium japonicum tree. The author discusses actual medical effects of troxerutin, including antioxidant, anti-inflammatory, antithrombosis and protecting endothelium of small vessels. Being water-soluble, it can be used as ophthalmic drops in treatment of subconjunctival haemorrhage, vitreous haemorrhage and early stages of diabetic retinopathy.

Introduction and aim. Non-ketotic hyperglycemia (NKHG), also known as hyperosmolar hyperglycemic state (HHS) is a serious metabolic complication of diabetes mellitus (DM).The mortality rate can be up to 20% and this is much more higher than that of diabetic ketoacidosis (DKA). It is usually precipitated by an event such as pulmonary/urinary infection, myocardial infarction (MI) or stroke. In this state of metabolic derangements, central nervous system (CNS) manifestations including altered mental status with or without focal neurological deficits are prominent clinical presentations. On the other hand, HHS may also be complicated with various other CNS events. Herein, a quite rare presentation of HHS with hemichorea ‒ hemiballismus in a 71 year old female patient with type 2 DM is presented. Description of the case. A 71-year-old female patient type 2 DM presented to our emergency department with progressive involuntary movements on the right upper and lower extremities accompanied by semiconsciousness during the last 24 hours. On neurological examination, cranial nerves and cerebellar signs were found to be normal, as the deep tendon reflexes. However, involuntary non-rhythmic writhing movements at rest were present on her right sided extremities. The fingerstick evaluation showed marked hyperglycemia (HG). The laboratory findings were characterized with high blood glucose level without obvious acidosis compatible with HHS. In urine analysis, glucosuria without significant ketonuria was detected. On head CT, subtle hyperdensity was noted in the left neostriatal regions without any mass effect or perilesional edema, compatible with left sided diabetic striatopathy (DS). Conclusion. Diabetic striatopathy is a quite rare presentation of HHS with hemichorea – hemiballismus. The characteristic computed tomograhy (CT) findings of associated striatopathy should be differentiated from vascular lesions that may also present with unilateral findings in the course of HHS and should not be overlooked in diabetic patients to recognise the ongoing HHS before the coma precedes.

Diabetes mellitus (DM) is a group of metabolic diseases which typically presents with frequent urination, increased thirst and increased hunger. DM be classified into three main types: type I (insulin-dependent DM), type-II (non-insulin dependent DM) and type III (gestational DM). Diabetes is a group of diseases of global health significance as 382 million people worldwide had diabetes in the year 2013 and this was projected to increase to an estimated 415 million in 2015. Damage to the nerves of the body (diabetic neuropathy) is the most common complication of diabetes. The signs and symptoms of diabetic neuropathy include numbness, diminished sensation, pain etc. Various types of electrotherapy, such as transcutaneous electrical nerve stimulation (TENS), pulsed-dose electrical stimulation, frequency-modulated electromagnetic neural stimulation, have been reported effective in managing diabetic neuropathy. This study is a systematic review of the evidence to enable the determination of the effectiveness of electrical stimulation and low-intensity laser therapy (LILT), and also aid their recommendation if proven to be effective. The outcome of this study was that TENS and other forms of electrical stimulation reviewed in this study may be effective and safe non-pharmacological treatment modalities in relieving the symptoms associated with diabetic neuropathy. The effectiveness of LILT couldn’t be determined due to the different parameters used to evaluate patients’ outcome and limited number of studies. Authors recommend that further randomized controlled trials with similar methodological parameters and studies with higher quality of evidences are needed to establish the true effectiveness of these modalities in diabetic neuropathy.

Diabetic retinopathy as a complication of diabetes is one of the most common causes of vision loss. It is a dangerous disease, which affects more and more people. It is important to know its different causes, background and to generate new, effective therapeutic threatments. One of the possible causes of diabetic retinopathy is the accumulation of advanced glycation products and over-expression of AGEs receptors (RAGE) leading to tissue damage, oxidative stress formation, and the promotion of other changes and disorders. There are many genetic variants that encode the AGEs receptor (RAGE). Some polymorphisms (Gly82Ser, 1704G/T, 2184A/G, -429T/C, -374T/A) may be associated with diabetic retinopathy. This paper aims to systematize knowledge in this field and to show the relationship between these compounds and diabetic retinopathy.

Objectives: The aim of this study was to analyze the level of physical activity in people with diabetes, depending on their characteristics. Methods: The sample consisted of 81 participants with type 1 and type 2 diabetes and belonging to a diabetes care association. First of all, the participants completed the short version of the International Physical Activity Questionnaire and the Mediterranean Diet Adherence Questionnaire online. Subsequently, they indicated their sex, age, type of diabetes, the presence of other complications and the years since diagnosis. In addition, data on consumption of tobacco and alcohol, glycosylated hemoglobin and fasting glucose were collected. For the analysis of the data, t-test for independent samples was used to compare means between categories, always with a level of significance of p <.05. Furthermore, effect size was calculated through Cohen’s d. Finally, the frequency and the percentage of the variables were calculated. Findings: The results showed in all the categories an average level of physical activity higher than the recommendations of physical activity for people with diabetes, being the level of physical activity predominantly moderate. Although women also complied with the recommendations for physical activity, their level of physical activity was significantly lower than the level of men Conclusions: It is recommended to expand the research on the barriers and motivations found by people with diabetes to practice physical activity, playing special attention to women with diabetes.
