Several chemical compounds found in plant products have proven to possess beneficial properties, being currently pointed out due to their pharmacological potential in type 2 diabetes mellitus complications. In this context, we studied the effect of Geranium robertianum L. (herb Robert) leaf decoctions in Goto-Kakizaki (GK) rats, a model of type 2 diabetes. Our results showed that oral administration of G. robertianum leaf decoctions over a period of four weeks lowered the plasma glucose levels in diabetic rats. Furthermore, the treatment with G. robertianum extracts improved liver mitochondrial respiratory parameters (state 3, state 4 and FCCP-stimulated respiration) and increased oxidative phosphorylation efficiency.

Background: Cardiovascular proteomics investigation reveals the characterization and elucidation of the novel therapeutic targets and strategies to prevent the development of heart failure associated diabetic complication by using 2DE and MS. Methods: The experimental animals were made diabetic with a single intraperitoneal injection of alloxan (150 mg/kg of bw). Albino rats were randomly divided into four individual groups: Group-I control (n=6), group-II alloxan-induced diabetic rats, untreated (n=6), group-III (n=6) and group-IV (n=6) alloxan-induced diabetic rats were treated with aqueous and ethanolic extracts of Cynodon dactylon for 15 days, respectively. Animals were euthanized to collect the heart tissues and blood samples. 2DE sample preparation, gel running and staining (n=6: each groups) were performed at the same time to avoid variation. The result of six gel images from each group were analyzed and evaluated as one match set with 2D software (P<0.05). Results: The above experiment revealed two up-regulated proteins in group-II i.e. NTF4 and ETFB. Conclusions: NTF4 is a neuro-protective agent for neuro-degenerative diseases. It will prevent diabetic secondary complications, such as diabetic polyneuropathy and cardiomyopathy. ETFB is active in the mitochondria, the energy-producing centres in cells. It is clear from the experiment that because of up-regulation of ETFB more energy is availabile and the electron transfer for heart during diabetes is possible, what leads to reduce the oxidative stress and free-radical formation. The up-regulated proteins reduced CVD that occurred just before overt hyperglycaemia due to administration of C. dactylon. This approach established the preliminary reference map for decoding cellular mechanisms linked between pathogenesis CVD and diabetes.

The activity of Cu,Zn superoxide dismutase in the fluid obtained from eye lens capsules after cataract surgery was investigated in samples obtained from patients with senile cataract and with senile cataract combined with diabetes mellitus. Two parameters were measured and compared: the frequency of occurrence of detected superoxide dismutase activity and the relative activity of the enzyme in samples derived from senile cataract patients versus those from the patiens affected additionally by diabetes mellitus. It was confirmed that the decrease of superoxide dismutase activity during cataract was additionally promoted by diabetes mellitus.

Introduction: According to the WHO, an estimated 422 million people are suffering from diabetes worldwide. Among them, the incidence of cholelithiasis is higher than in the healthy population. The aim of this literature review was to summarize the available evidence about acute cholecystitis in patients with diabetes. Materials and methods: This study adhered to the PRISMA guidelines. The course of hospitalization of patients with and without diabetes who underwent cholecystectomy due to acute cholecystitis was compared. Following information was abstracted from original studies: general study information, patient characteristics, complications, and recommendations for patients with diabetes. Results: Initial search provided 1632 results. After full text assessment, 40 studies met the inclusions criteria. Operative and postoperative complication rates were significantly higher among the diabetic patients. Diabetes is a risk factor for conversion from laparoscopic to open cholecystectomy method. The authors' opinions on elective surgery before the onset of acute cholecystitis symptoms are divided. Conclusions: Diabetic patients are at greater risk of developing complications. An individualized screening and treatment approach, as well as proper preparation of the diabetic patient for an elective cholecystectomy could have a positive effect on the outcome. However, the low quality of the data from the systematic review does not allow for meta-analysis, which is why we cannot draw strong conclusions.

Oxidative stress, capable of eliciting damage to various biomolecules including DNA, is a recognized component of diabetes mellitus and its complications. Metabolic syndrome (MetS) is associated with the development of type 2 diabetes mellitus (T2DM), as well as other unfavorable outcomes. The aim of this study was to elucidate the role of oxidative stress in the development of T2DM, by investigating association of oxidative DNA damage with metabolic parameters in subjects with MetS and early T2DM. Selected anthropometric and biochemical parameters of MetS, inflammation and oxidative DNA damage: body mass index (BMI), fatty liver index (FLI), waist circumference (WC), total cholesterol, HDL and LDL-cholesterol, gamma-glutamyl transpeptidase (GGT), uric acid, C-reactive protein (CRP), total leukocyte/neutrophil count, and urinary 8-hidroxy-deoxyguanosine (u-8-OHdG) were assessed in male subjects with MetS and both younger (≤55 years) and older (>55 years) subjects with T2DM of short duration without complications. BMI, FLI, WC, total and LDL-cholesterol and uric acid were higher, while the u-8-OHdG was lower in MetS group, when compared to older T2DM subjects. None of these parameters were different neither between MetS and younger T2DM, nor between two sub-groups of subjects with T2DM. Values of CRP, HDL-cholesterol, triglycerides, GGT, leukocytes and neutrophils were not different between all examined groups of subjects. Higher 8-OHdG in older subjects with T2DM suggests that both aging process and diabetes could contribute to the development of DNA damage. Oxidative DNA damage cannot serve as an universal early marker of T2DM.

Cukrzyca to „epidemia XXI wieku”, szacuje się, że w 2011 roku na cukrzycę chorowało 6,7% populacji Polski, z czego jedna czwarta nie miała świadomości choroby. Tak duże rozpowszechnienie cukrzycy sprawia, że coraz więcej lekarzy staje przed problemem prawidłowej kwalifikacji pacjenta z cukrzycą do amatorskiego uprawiania sportu, w tym nurkowania. W niniejszej publikacji przedstawiono najważniejsze informacje dotyczące oceny ryzyka, dopuszczenia do amatorskiego nurkowania pacjentów z cukrzycą oraz proponowanych protokołów postępowania przed i podczas uprawiania tego sportu.
Diabetes is an "epidemic of the 21st century" and it is estimated that in 2011 6.7% of Poland's population suffered from diabetes, one quarter of which was unaware of the disease. Such a high prevalence of diabetes mellitus results in an increasing number of physicians being confronted with the problem of qualifying patient with diabetes to partake in amateur sports, including diving. This publication presents the most important information concerning risk assessment, admission of diabetic patients to amateur diving and some proposed protocols of management before and during sport practice.

Background: Knowledge about diabetes mellitus (DM)-related complications, the direct and indirect costs incur red by patients with DM is still insufficient in Poland. Economedica DM-PL (Economedica) survey was a cross-sectional observational study that aimed to assess these issues. Material and methods: The survey was carried out in 2016 on a representative sample of 2032 diabetic patients recruited by 195 general practioners (GP) and 48 diabetologists (D) randomly selected from the national registry of physicians. Each GP and D recruited 8 to 10 patients with DM according to the specified protocol. The study included 1938 medical questionnaires (comple ted by the GPs and Ds), 1938 questionnaires about costs (completed by the patients) and 1532 treatment adherence questionnaires. Results: The Economedica survey provided new insights into DM epidemiological and cost data. 70% of patients with type-1 DM were diagnosed with the disease before the age of 40. For 76% patients with type-2 DM, it was diagnosed aeftr the age of 50. The average monthly out-of-pocket medical costs for DM patients in 2016 reached 66.2 EUR, while their non-medical costs were 36.8 EUR. Conclusions: The methodology applied in our study proved useful for providing new dimensions of DM epidemiological and cost data in Poland.

Background. There are few updated data on rates of hospital mortality of diabetic patients and length of their hospital stay on a country level. To determine such rates we provided analysis using claims data from a Nationwide General Hospital Morbidity Study carried out by the National Institute of Public Health – National Institute of Hygiene (NIPH- NIH) in Warsaw from 2010 to 2018. Objective. The aim of the study was to analyze the nine years changes of in-hospital morbidity and mortality in diabetic patients and length of hospital stay using a comparative approach by gender, age and place of residence. Material and methods. The data on all patients from general hospitals in Poland treated because of diabetes were taken from a nationwide database, kept since 1979 by the Department for Monitoring and Analyses of Population Health of NIPH-NIH. This database contains information gathered under the Statistical Research Program of Public Statistics. Hospitalization rates were used to evaluate the ‘hospitalized’ incidence of diabetes (number of hospitalization cases due to diabetes per year by the analyzed unit of population). In-hospital mortality was calculated as the percentage of deceased patients out of all patients hospitalized due to diabetes. Results. The number of cases and hospitalization rates of diabetic patients was rapidly declining by 18.8% for type 2 (E11) and 23.7% for type 1 (E10) diabetes. The downward tendency in the scope of hospitalization affected mainly older women and rural residents. Hospital mortality due to diabetes rose up dangerously to 3.77% exceeding the rates recorded eight years earlier. Conclusions. The recent reduction in hospitalization rates of people with diabetes in Poland may be associated with an unexpected increase in hospital mortality.

Introduction. Correctness of insulin treatment depends on both the experience and skills of the doctor and knowledge and behaviour of the patient. Objective. Evaluation the adequacy of insulin doses administered to diabetes patients in ambulatory conditions. Material and methods. The treatment of 59 patients hospitalized in the Diabetology Ward was evaluated at admission, discharge and 3 months after hospitalization. Results. The mean daily doses of insulin significantly differed at times of evaluation and were: 53.90, 39.31 and 43.34 units, respectively (p≈0.000001). A significant reduction of body weight, 90.86 vs. 88.25 kg (p≈0.000001), was obtained only during hospitalization, and was maintained 3 months after discharge (87.86 kg). Significant differences were also noted in the body mass index (33.44 vs. 32.48 vs. 32.37 kg/m2, p≈0.000001). The change in waist circumference was not statistically significant (107.87 vs. 104.89 cm; p≈0.06). A decrease in the number of hypoglycaemia episodes was observed, but were statistically insignificant (25 vs. 23; p≈0.7). Three months after hospitalization an insignificant decrease of HbA1c level was noted (8.41% vs. 8.03%; p≈0.07). Conclusions. During treatment in the Diabetology Ward the procedure of choice was more frequently a reduction than an increase in insulin doses. This management led to the reduction of the patients’ body weight, improvement of glycaemia, without any significant effect on the diabetes control determined by the HbA1c level.

Background. The aim of this work was to examine the awareness of patients with diabetes mellitus (DM) on the importance of physical activity (PA) in the prevention and treatment of their disease. Material and methods. The survey was completed by 178 respondents (47.3% women, 52.7% men), of whom 69.1% were dependent on exogenous insulin. Data was collected in April 2019 via a questionnaire given to patients at the Department of Diabetology and Internal Medicine at the Louis Pasteur University Hospital in Košice, Slovakia. Results. More than half of the respondents admitted having insufficient information about their health condition, and a similar proportion felt lack of knowledge regarding exercise for their illness. As many as 69.1% of the patients reported inadequate help by their health care providers with regard to the do’s and dont’s of their disease, and 41.9% of respondents reported having no knowledgeable health care provider with whom to share their concerns. Conclusions. Based on the results obtained, it is concluded that a substantial number of DM patients do not have adequate information about their health condition and how to improve it. On the other hand, nearly two-thirds of participants recognize that regular physical activity (PA) is an option to achieve positive changes.
Wprowadzenie. Celem pracy było zbadanie świadomości pacjentów z cukrzycą co do znaczenia aktywności fizycznej w procesie zapobiegania ich chorobie i jej leczenia. Materiał i metody. W badaniu wzięło udział 178 respondentów, z czego 47,3% stanowiły kobiety, a 52,7% mężczyźni. 69,1% badanych było zależnych od insuliny egzogennej. Dane zostały zebrane w kwietniu 2019 r. za pomocą kwestionariusza przekazanego pacjentom na Oddziale Diabetologii i Chorób Wewnętrznych Szpitala Uniwersyteckiego Louisa Pasteura w Koszycach na Słowacji. Wyniki. P onad połowa respondentów przyznała, ż e nie j est w ystarczająco p oinformowana w kwestii dotyczącej ich stanu zdrowia; podobna liczba ankietowanych odczuwała brak wiedzy o ćwiczeniach w związku z ich chorobą. Aż 69,1% pacjentów zgłosiło nieodpowiednią pomoc ze strony instytucji świadczących usługi medyczne w odniesieniu do nakazów i zakazów dotyczących ich stanu zdrowia. Ponadto 41,9% ankietowanych zgłosiło brak wykwalifikowanych pracowników służby zdrowia, z którymi mogliby podzielić się swoimi obawami. Wnioski. Na podstawie uzyskanych wyników stwierdzono, że znaczna liczba pacjentów cierpiących na cukrzycę nie ma wystarczającej wiedzy co do swojego stanu zdrowia i możliwości jego poprawy. Z drugiej strony prawie dwie trzecie uczestników badania postrzega regularną aktywność fizyczną jako drogę do osiągnięcia pozytywnych zmian.

Introduction. Several studies have suggested, that diabetes is very important factor of the risk of the chronic a renal disease. The paper aims to present a retrospective analysis of incidence of end-stage renal disease in subgroups of patients with and without diabetes in Poland. Material and methods. For assessing this problem an electronic search was performed using Polish National Health Fund data base from 01.01.2011 until 31.12.2013 in general population and from 01.01.2012 until 31.12.2012 in 2 populations: with diabetes and without diabetes mellitus. Results. The 43.70 % patients with end-stage renal disease was diabetic. The incidence rate in 2012 was: 292.48 ± 90.97 diabetic men / 100,000 diabetic population; 203.10 ± 90.97 diabetic women / 100,000 diabetic population; 23.44 ± 6.34 non-diabetic men / 100,000 non-diabetic population; 17.88 ± 6.33 non-diabetic women / 100,000 non-diabetic population. Conclusions. The incidence rate of end-stage renal failure in diabetics was multiple times higher than the incidence rate in non-diabetics. The incidence rate of new ESRD cases in Poland estimated to be 36.17 per 100,000 of general populations in 2011, 35.28% in 2012 and 30.46 per 100,000 of general populations in 2013. In 2012, the incidence rate of new ESRD cases in male diabetics was 292.48 ± 90.97 per 100,000 of diabetes men population, and in women diabetics 203.10 ± 66.06 per 100,000 of diabetes women population. In the same 2012 year, the incidence of new ESRD cases in men non-diabetics was 3.44 ± 6.34 per 100,000 of non-diabetes, and in women non-diabetes 17.58 ± 6.33 per 100,000 of non-diabetes women population.

Two recent studies found that RBMS1 gene rs7593730 and BCAR1 gene rs7202877 are related to type 2 diabetes. However, the association of these loci with type 2 diabetes mellitus (T2DM) has not been examined in Chinese. We performed a replication study to investigate the association of the 2 susceptibility loci with T2DM in the Chinese population. We genotyped 1961 Chinese participants (991 with T2DM and 970 controls) for each of the 2 single nucleotide polymorphisms (SNPs) rs7593730 in RBMS1 and rs7202877 near BCAR1 using SNPscan and examined their association with T2DM using logistic regression analysis. We also analyzed the correlation of the SNP alleles and clinical phenotypes. In additive model, genotype association analysis of BCAR1 rs7202877 loci revealed that the homozygous of rs7202877 GG carriers had significantly decreased T2DM risk compared to homozygous carriers of TT (P=0.038, OR 0.44, 95% CI 0.20-0.96). In the recessive model, the GG genotype GG had significantly decreased T2DM risk compared to GT+TT (P=0.043, OR 0.67, 95% CI 0.46-0.99). Allele G was statistically significantly correlated with TC (mmol/L) (P=0.036) and LDL-C (mmol/L) (P=0.007). As for rs7593730, the carriers of CT and TT genotype had significantly decreased T2DM risk compared to the carriers of CC genotype (CT: CC P=0.038, OR 0.71, 95% CI 0.51-0.98; TT: CC P=0.010, OR 0.32, 95% CI 0.13-0.76). In a dominant model, TT+CT: CC (P=0.013, OR 0.673, 95% CI 0.49-0.92) and in a recessive model, TT: CT+CC (P=0.019, OR 0.59, 95% CI 0.39-0.92). The T allele carriers had significantly decreased T2DM risk compared to the carriers of C (P=0.002, OR 0.65, 95% CI 0.50-0.86). Allele T was statistically correlated with FINS (P=0.010). In conclusion, our study showed that RBMS1 gene rs7593730 and BCAR1 gene rs7202877 were significantly associated with type 2 diabetes in the Chinese population.

UBE2E2 encodes ubiquitin-conjugating enzyme E2E2, which plays an important role in the synthesis and secretion of insulin. Two previous studies indicated that SNPs in UBE2E2 were associated with risk for type 2 diabetes mellitus (T2DM) in the Japanese and Korean populations, respectively. We examined the association of one SNP in this gene, rs7612463, with the risk of T2DM in 1957 Han participants in northeastern China, using an SNPscanTM Kit. rs7612463 genotype was significantly associated with risk for T2DM under various genetic models, including an additive model (P = 0.004), a dominant model (P = 0.024), and a recessive model (P = 0.008). The AA genotype was associated with a significantly decreased risk for T2DM (P = 0.004, OR = 0.513, 95% CI = 0.325-0.810) after adjustment for age, gender, and BMI. The heterozygous genotype, AC, was associated with increased risk for total cholesterol (mmol l-1; P = 0.031) and triglycerides (mmol l-1; P = 0.039) in control individuals. Our results show that rs7612463 is associated with T2DM, with homozygotes of the AA genotype at decreased risk for T2DM in the Chinese population. Additionally, heterozygotes may have decreased risk of T2DM due to insulin resistance.

In search for new markers of insulin-dependent diabetes (IDDM) susceptibility we studied the CATT tetranucleotide repeat in intron 1 of tyrosine hydroxylase (TH) gene on chromosome lip, the CA repeat at T-cell receptor a chain (TCRA) locus on chromosome 14q and two CA repeats at D10S211 and D10S213 loci in the chromosome 10 region containing glutamic acid decarboxylase (GAD2) gene. Alleles at these microsatellite loci were identified in a population of diabetic children and unrelated healthy controls originating from Wielkopolska, a midwestern region of Poland. We found significant association of certain alleles at TH, TCRA and D10S211 loci with diabetes in the population under study. On the contrary, none of the alleles at D10S213 locus was associated with the disease. Our findings indicate that typing of microsatellite markers may represent useful additional tool for identifying individuals at high risk of developing IDDM. Regarding loci on chromosome 10 our data and data published by other authors may suggest the extistence of two separate regions of association with IDDM susceptibility on this chromosome.

Background. Education of diabetic patients is integral to effective treatment. Material and methods. In the presented work, we studied Eastern Slovakian patients with diabetes type 2 with or without insulin treatment. We focused on their diabetic self-care and compared their attitudes, education on their disease, and treatment provided by professionals. There were 411 patients in the insulin-treated group. We used the standardized Diabetes Care Profile questionnaire (DCP). Results. The groups had very different attitudes toward diabetic self-care. Patients with provided professional self-care education scored higher in all areas of diabetic care. Appropriate education influenced knowledge and consequently the management and attitudes of diabetic patients toward their disease. Educated patients scored higher in the categories ‘Self-care ability’, ‘Importance of care’, ‘Self-care adherence’, ‘Diet adherence’, ‘Medical barriers’, ‘Exercise barriers’, ‘Monitoring barriers’ and ‘Understanding practice’ (p < 0.01). Patients who had not received diabetes education presented higher scores in emotional areas, i.e., negative and positive attitudes toward diabetes mellitus (p < 0.01). Conclusions. We concluded that it is beneficial for a structured e ducational process to be integrated in diabetes treatment.
Wprowadzenie. Edukacja pacjentów chorych na cukrzycę stanowi integralną część ich skutecznego leczenia. Materiał i metody. W prezentowanym badaniu porównano postawy w zakresie samodzielnej profilaktyki cukrzycy w dwóch grupach pacjentów cierpiących na cukrzycę typu 2, która wymaga insulinoterapii, w odniesieniu do szkoleń edukacyjnych dotyczących ich choroby oraz leczenia prowadzonego przez specjalistów. Badaną grupę stanowiło 411 pacjentów cierpiących na cukrzycę typu 2 leczoną za pomocą insuliny, którzy pochodzą ze wschodniej części Słowacji. Zastosowano standardowy kwestionariusz DCP (ang. Diabetes Care Profile), wykorzystywany do oceny czynników społecznych i psychologicznych wśród cukrzyków. Wyniki. Postawy dwóch porównywanych grup pacjentów wobec cukrzycy różniły się istotnie w zakresie samodzielnej profilaktyki. Pacjenci z zapewnionym profesjonalnym szkoleniem dotyczącym samodzielniej profilaktyki uzyskiwali wyższe oceny we wszystkich obszarach związanych z monitorowaniem i leczeniem cukrzycy. Prawidłowy proces edukacyjny ma wpływ na poziom wiedzy, a tym samym na działania podejmowane przez pacjentów i ich postawy wobec choroby. Pacjenci, którzy otrzymali odpowiednie przeszkolenie, lepiej kontrolowali sfery takie jak „Umiejętności dot. samodzielnej profilaktyki”, „Znaczenie samodzielnej profilaktyki”, „Przestrzeganie zasad samodzielnej profilaktyki”, „Przestrzeganie diety”, „Ograniczenia natury medycznej”, „Ograniczenia w wykonywaniu ćwiczeń”, „Ograniczenia w monitorowaniu” oraz „Rozumienie procedur” (p < 0,01). Z kolei pacjenci nieposiadający odpowiedniej wiedzy prezentowali lepsze wyniki w obszarach związanych ze sferą emocjonalną – negatywnym i pozytywnym nastawieniu do cukrzycy (p < 0,01). Wnioski. W kontekście uzyskanych wyników, zintegrowanie ustrukturyzowanego procesu edukacyjnego z leczeniem osób chorujących na cukrzycę wydaje się konieczne.
