RESEARCH OBJECTIVE: The aim of the present article was to make a narrative review about the relation of cognitive functions and neurotrophic factors with aging. THE RESEARCH PROBLEM AND METHODS: The research problem concerns the issue of aging and the changes in cognitive and biochemical functioning that may occur in this process. The article uses the method of critical analyzes as well as the analyzes of the reference literature. THE PROCESS OF ARGUMENTATION: The first section of the article discusses population aging, the second addresses changes related to cognitive functions during aging. The third part show biochemical aspects that can directly influence the cognitive functions already mentioned, and on aging in general. RESEARCH RESULTS: The result of this review points out that the world population is aging in an accelerated way and during this process of aging there are several changes in the organism of the elderly, among which stand out the cognitive and biochemical, one being directly related to the other. In this case, there is a relation of the neurotrophic factors, which may be responsible for the changes in cognitive functions, specifically spatial memory alterations. CONCLUSIONS, INNOVATIONS AND RECOMMENDATIONS: It is necessary that more studies be carried out with this population, which may trace the elderly with cognitive impairment that may be suggestive or conclusive of a neurodegenerative disease. In addition, biochemical data may contribute to the search for new therapeutic models or early identification for Alzheimer’s disease or other associated dementias.

Background: Both depressive disorders and nicotine use are proven and important risk factors of dementia. The purpose of this study was to verify if cigarette smoking and depression symptoms together are disadvantageous for the prognosis in mild cognitive impairment. Material and methods: A total of 43 patients with a diagnosis of mild cognitive impairment were included in the study. ADAS-Cog was performed upon inclusion in the study and again at least 2 years later. Additionally, patients with ≥18 points in MADRS were qualified as depressive. The Fagerström scale for nicotine dependence was administered to smokers. Results: Our study shows a relation between severity of depressive symptoms and further deterioration of cognitive functions according to ADAS-cog scale. Regression analysis revealed that smoking associated with severity of depressive disorders is also correlated with the progression of cognitive impairment. Conclusions: The results of our study are based on a small number of subjects and should be regarded as early findings. Moreover, nicotine dependency should not be regarded as an isolated factor affecting mood disorders and cognitive impairment progression. Further studies on larger groups of patients and using more sensitive methods of cognitive function assessment are needed.

Introduction and objective. Recently, data has indicated a higher incidence of mild cognitive impairment (MCI) in patients with diabetes. Old age is a risk factor for cognitive deterioration and dementia. The aim of the study was to find the factors associated with poor glycaemic control in type 2 diabetic elderly patients with MCI. Materials and method. A cross-sectional study was conducted on 87 diabetic patients with MCI in an outpatient clinic. All subjects were screened for MCI using the Montreal Cognitive Assessment (MoCA). Detailed medical history and collection of blood test samples were performed. Results. 83.9% of participants had poor glycaemic control. A positive correlation was found between HbA1c level and number of visit to a doctor per year, number of co-morbidities, duration of T2DM, triglycerides and fasting glucose level; and a negative correlation between HbA1c level and years of education, HDL cholesterol level and MoCA score. The univariate logistic regression models revealed factors which are associated with poor glycemic control are: less years of education, higher no of visit to doctor per year, increased number of co-morbidities, presence of CVD, retinopathy, higher levels of triglycerides and fasting glucose, lower level of HDL cholesterol, lower MoCA score. Multivariable model revealed that higher plasma levels of fasting glucose and triglycerides are significant predictors. Conclusions. There is a high prevalence of poor glycemic control patients among elderly diabetics with MCI. Higher plasma levels of fasting glucose and triglycerides seems to be the most important predictors of poor glycemic control, however father larger studies are needed to elucidate these relationships.

This study investigated the use of computer games to detect the symptoms of mild cognitive impairment (MCI), an early stage of dementia, in the elderly. To this end, three serious games were used to measure the visio-perception coordination and psycho-motor abilities, spatial memory, and short-term digit span memory. Subsequently, the correlations between the results of the games and the results of the Korean Mini-Mental State Examination (K-MMSE), a dementia screening test, were analyzed. In addition, the game results of normal elderly persons were compared with those of elderly patients who exhibited MCI symptoms. The results indicated that the game play time and the frequency of errors had significant correlations with K-MMSE. Significant differences were also found in several factors between the control group and the group with MCI. Based on these findings, the advantages and disadvantages of using serious games as tools for screening mild cognitive impairment were discussed.

The Mediterranean Diet has long been regarded as one of the promising strategies for the prevention of cognitive impairment and Alzheimer's disease. Aim: This study aimed to determine the association between adherence to a MedDiet and the risk of cognitive impairment (CI) among the elderly in Morocco. Due to their complete data, a sample of 172 (56.4% men) from 237 subjects were included in our study. Cognitive functions were assessed by the MMSE test (Mini-Mental State Examination). Adherence to a MedDiet was assessed using a validated questionnaire based on weekly consumption of the seven main MedDiet foods: non-refined cereals, potatoes, fruits (whole or in juice), vegetables (cooked or as salad), legumes, fish, olive oil, and three foods consumed less frequently in a MedDiet: red meats, poultry, and full-fat dairy products. A binary logistic regression was performed, with cognitive function as the dependent variable MedDiet adherence score as well as the aliment consumption frequency associated with CI in the bivariate analysis as an independent variable. The binary logistic regression adjusted for confounding variables revealed that adherence to a MedDiet is not associated with a lower risk of cognitive impairment (ORa= 0.928; 95% CI; [0.831-1.037]) (ORa: Adjusted Odd Ratio; 95% CI: 95% of Confidence Interval). However, only Olive Oil consumption was a protective factor against CI (ORa= 0.882; 95% CI; [0.815-0.953]).

We evaluated the effectiveness of three different rehabilitation programs for chronic behavioral disorders in traumatic brain injury (TBI) patients with orbito-frontal damage (OFC), incurred from 13 to 15 years prior to enrollment. From among 547 adult patients with severe TBIs treated neurosurgically from 1999 to 2003, 60 were selected for the study, divided into three groups of 20, matched for age and sex. Each group received one of three different rehabilitation protocols. Group A received a comprehensive program with two components: (1) a 6-month early neuropsychological rehabilitation program targeting cognitive deficits, beginning immediately post-injury, and (2) the “Academy of Life” program, with weekly meetings aimed at reducing behavioral disorders in social transactions. Group B received only cognitive rehabilitation, while Group C received only the “Academy of Life” progam. The Frontal Behavioral Inventory (FBInv) was used to assess behavioral disorders at baseline and again after completion of the study. At baseline, all 60 patients still displayed all or most of the common behavioral symptoms characteristic of frontal syndrome. The improvement achieved by group A (the comprehensive program) was significantly better upon completion of the study than that shown by the patients in groups B (cognitive only) and C (social skills only). A comprehensive program of rehabilitation, which combines early neuropsychological rehabilitation of cognitive deficits with the “Academy of Life” program, aimed at improving social skills, proved to be more effective than either of these two components administered without the other

Neuroinflammation is a known factor in the pathogenesis of recurrent depressive disorders. Depression is accompanied by activated immune-inflammatory pathways including increased levels of TNFα, sTNFR1and sTNFR2.The purpose of this study was to analyse the TNF-α, TNFRSF1A and TNFRSF1B genes on both mRNA and protein levels in patients with rDD, and to investigate the relationship between TNF-α,TNFRSF1A and TNFRSF1B gene expression and cognitive performance. The study comprised 158 subjects: patients with recurrent depressive disorder (n=89) and healthy subjects (n=69). Cognitive function assessment was based on: Trail Making Test, The Stroop Test, Verbal Fluency Test and Auditory Verbal Learning Test. Both mRNA and protein expression levels of all genes were significantly higher in rDD subjects when compared to healthy controls. No statistically significant correlations were observed between the analysed variables in both the rDD group and the HS test group. The only exception was noticed in the HS test group, where increased expression of TNFRSF1A and TNFRSF1B gene negatively affected the performance of the AVLT test. However, statistically significant correlations between TNF, TNFRSF1A, TNFRSF1B mRNA gene expression levels and all the neuropsychological tests used in the survey for the entire group were observed. Conclusions: 1.The results of our study show increased expression of the TNF, TNFRSF1A and TNFRSF1B genes on both mRNA and protein levels in depression. 2. Elevated expression of TNF-α, TNFRSF1A and TNFRSF1B negatively correlates with cognitive efficiency: working memory, executive functions, attention, auditory-verbal memory, effectiveness of learning processes and verbal fluency.

Demencja dotyka coraz większej liczby osób, co skłania do postrzegania jej jako istotnego problemu zdrowia publicznego. Zaburzenia poznawcze dotyczą przede wszystkim osób starszych, natomiast w praktyce stanowią również duże obciążenie dla ich opiekunów. Manifestacja otępienia nie jest jednak nieunikniona. Na funkcje układu nerwowego wpływa szeroki wachlarz czynników, począwszy już od tych na etapie życia płodowego. Do najistotniejszych determinant zaburzeń funkcji poznawczych należą obciążenia genetyczne, choroby, aktywność intelektualna, aktywność fizyczna oraz sposób żywienia. W prewencji podkreśla się rolę diety o charakterze przeciwzapalnym, ubogiej w cukier i produkty wysokoprzetworzone. Szczególne znaczenie mają kwestie takie jak prawidłowy stosunek kwasów tłuszczowych omega-3 do omega-6, odpowiednia podaż kwasów EPA i DHA oraz jak najniższe spożycie izomerów trans kwasów tłuszczowych. Natomiast do kontrowersyjnych aspektów należą m.in. dieta wysokotłuszczowa i spożycie alkoholu. Celem pracy jest analiza kontrowersyjnych czynników i strategii żywieniowych w prewencji zaburzeń poznawczych na podstawie dostępnych danych naukowych.
Dementia is affecting growing numbers of people. Consequently, we are inclined toper ceive it as a crucial public health issue. Although cognitive impairment relates mainly to the elderly, it also significantly burdens their caretakers. However, the development of dementia is not inevitable. The cognitive functions are influenced by a wide range of factors, starting with those at the beginning of the prenatal period. The most important determinants of cognitive impairment involve genetic defects, diseases, intellectual activity, physical activity and nutrition. The role played by anti-inflammatory, low in sugar and highly-processed foods is emphasized. Particular importance is attributed to the omega-6 to omega-3 ratio, proper amounts of EPA and DHA and the lowest possible trans fatty acids intake. The controversial aspects include, among others, high-fat diet and alcohol consumption. The aim of this paper is to conduct an analysis of controversial aspects and nutritional strategies related to cognitive impairment prevention, based on available scientific data.

Early diagnosis and monitoring of disease progression in patients with Vascular Dementia (VaD) have become vital in clinical practice, as disease modifying treatments for VaD become available. The goal of our research was to study cognitive impairment in a patient at an early stage of VaD. We evaluated latencies of the P3 GO and NOGO components of event-related potentials (ERPs), elicited in cued GO/NOGO tasks, as potential neuromarkers of cognitive im- pairment, as suggested by previous research. The patient, a right-handed, 53-year-old male with a college edu- cation, suffered a transient ischemic attack (TIA) in 2011. During this attack, which lasted for a few minutes, he was confused and had trouble speaking, trouble seeing in both eyes, difficulty walking, problems with balance and coordination, and strange behavior. Se - ven years later, in 2018, he was assessed using the HBI methodol- ogy, which consisted of recording (1) a 19-channel EEG in resting state (with eyes open and eyes closed), and (2) a cued GO/NOGO task, and then comparing the results with EEG spectra and Event- Related Potentials (ERPs) data from normative and patient data - bases. The patient died in 2019. Post-mortem studies confirmed cortical microhaemorrhages neuropathological criteria for VaD. We did not found deposits of hyperphosphorylated tau (HPτ) and Aβ, which fulfil the neuropathological criteria for AD. The parietal-temporal-occipital EEG power was significantly higher in all conditions in this subject in comparison to healthy controls, indicating idling of the corresponding areas. The amplitude and la- tency of the P3 GO wave were found to be intact in the subject, in- dicating normal posterior cortical functioning in the cognitive task. The latency of the P3 GO wave was found to be significantly higher in the subject, indicating impairment of engagement operations. In a GO/NOGO task, ERPs provide a useful tool for assessment of brain functioning in clinical settings.

In the clinical picture of sclerosis multiplex (SM) the neurological functioning of patients is affected, besides the neurological symptoms, by neuropsychological symptoms, which include emotional disturbances, cognitive dysfunction, and personality factors. Memory, attention or executive functions disturbances affect reduction of adaptability, which play a key role in the quality of life of patients. Prevalence studies using large, community and clinical samples indicate that roughly 45–60% of patients with SM are cognitively impaired. Cognitive disorders with mobility disabilities may hinder daily functioning, often they become obstacle to take life tasks, so there is a need to develop new and effective rehabilitation programs for this group of patients. Neuropsychological rehabilitation of patients with SM is covered by the various types of impacts, especially reduction of cognitive deficits as much as possible. There is increasingly use of computer programs to cognitive trainings in neuropsychologist work. The basis for this type of impact is evidence of neuroplastic changes in people with SM. The greatest therapeutic effects are achieved through the cooperation of an interdisciplinary team, including neurologist, psychiatrist, neuropsychologist and physiotherapist. Obtaining such assistance by patients with SM still remains difficult in Poland. An example illustrating the effective usage of neuropsychological rehabilitation with the help of computer programs is a case study of a patient with SM.
W obrazie klinicznym stwardnienia rozsianego (łac. sclerosis multiplex, SM) na funkcjonowanie chorych wpływają – poza objawami neurologicznymi – współwystępujące objawy neuropsychologiczne, do których zalicza się zaburzenia emocjonalne i dysfunkcje poznawcze oraz czynniki osobowościowe. Dysfunkcje w sferze procesów uwagi, funkcji wykonawczych czy pamięci mają wpływ na zmniejszenie zdolności adaptacyjnych, które są kluczowe dla jakości życia chorych. W badaniach dużych grup klinicznych udowodniono obecność dysfunkcji poznawczych u 40–65% pacjentów. Najczęściej na SM zapadają osoby młode, u których dysfunkcje ruchowe i zaburzenia poznawcze mogą utrudniać codzienne funkcjonowanie, a często też stają się przeszkodą w podejmowaniu zadań życiowych. Dlatego też istnieje potrzeba opracowania nowych i skutecznych programów rehabilitacyjnych dla tej grupy chorych. Rehabilitacja neuropsychologiczna pacjentów z SM obejmuje różnego rodzaju oddziaływania, których celem jest leczenie dysfunkcji poznawczych. W pracy neuropsychologa coraz częściej jako narzędzie terapeutyczne wykorzystuje się programy komputerowe służące do treningów poznawczych. Podstawą dla tego typu oddziaływań są dowody świadczące o zmianach neuroplastycznych u osób ze stwardnieniem rozsianym. Największe efekty terapeutyczne osiąga się jednak dzięki współpracy zespołu interdyscyplinarnego, w którego skład powinni wchodzić neurolog, psychiatra, neuropsycholog oraz rehabilitant. W Polsce uzyskanie takiej pomocy przez pacjentów z SM jest nadal bardzo trudne. Przykładem obrazującym skuteczne zastosowanie rehabilitacji neuropsychologicznej za pomocą programów komputerowych jest studium przypadku chorego ze stwardnieniem rozsianym.

Opis:
Carbon monoxide (CO) is a highly toxic, odorless, nonirritating gas. The most common cause of its exposure are faulty combustion heating systems and fires. CO competes with oxygen in the blood, binding to haemoglobin in place of it (the affinity of CO to haemoglobin is approximately 200 times greater than that of oxygen). Secondary hypoxia leads to damage of multiple organs especially those with high oxygen utilization such as the central nervous system. A case of a 26-year-old woman with prolonged deep cognitive impairment, which occurred after an accidental carbon monoxide intoxication, is presented. A course of over 2-year neuropsychological rehabilitation of the patient is thoroughly described by the authors and its positive effects are emphasized. In spite of all prophylactic measures carbon monoxide poisoning still remains a significant clinical issue. CO intoxication may produce a wide spectrum of neurological, emotional and cognitive disorders. Neuropsychological rehabilitation is an effective method of treatment of cognitive impairment after carbon monoxide intoxication. Positive effects of a systematic rehabilitation are observed even several years after poisoning.
Tlenek węgla (CO) jest bardzo toksycznym, pozbawionym zapachu, niedrażniącym gazem. Najczęstszą przyczyną zaczadzeń jest CO powstający w trakcie wadliwego spalania w instalacjach grzewczych oraz podczas pożarów. Tlenek węgla współzawodniczy we krwi z tlenem, wiążąc się w jego miejsce z hemoglobiną (powinowactwo CO do hemoglobiny przewyższa ok. 200 razy powinowactwo tlenu). Wtórna hipoksja prowadzi do uszkodzenia licznych narządów, w szczególności cechujących się wysokim zużyciem tlenu, takich jak ośrodkowy układ nerwowy. W pracy przedstawiono opis przypadku 26-letniej kobiety, u której w wyniku przypadkowego zatrucia CO wystąpiły długotrwale utrzymujące się, głębokie zaburzenia funkcji poznawczych. Autorzy szczegółowo opisują proces ponad 2-letniej rehabilitacji neuropsychologicznej chorej, podkreślając jej pozytywne efekty. Mimo działań profilaktycznych zatrucia tlenkiem węgla nadal pozostają istotnym problemem klinicznym. Zaczadzenie może spowodować różnorodne zaburzenia neurologiczne, emocjonalne i poznawcze. Rehabilitacja neuropsychologiczna jest skuteczną metodą leczenia zaburzeń poznawczych po zatruciu tlenkiem węgla. Korzystne efekty prowadzonej systematycznie rehabilitacji są obserwowane, nawet kilka lat po zachorowaniu.

Opis:
The aim of the study was a continuation of previously provided research on the tablets usability in the rehabilitation of old patients with cognitive disabilities after a brain stroke. First, the analysis of the pilot research results was provided. It gave a hope for possible rationalization of the rehabilitation process after proper improvement of the proposed mobile solution. Prototype examples of mobile rehabilitation applications were extended and integrated in the RehaMob - a single mobile solution. The mobile application includes six different types of tasks, which can be useful for stimulation and rehabilitation of language functions, short-term memory, etc. The RehaMob was made available again to the therapists in the rehabilitation center. Finally, the mobile application is used with good results in the therapy of patients with cognitive disabilities. The mobile solution is useful support for traditional rehabilitation process, but its influence to the rehabilitation progress is difficult to assess, because the RehaMob can be used only as additional tool.

The article is devoted to consideration of verbal fluency in MCI-PD. The VF test allows for the detection of dysfunctions within linguistic processes, semantic memory and dysfunctions of executive functions already in the early stages of Parkinson’s disease. The article compares the results of verbal fluency tests in selected categories of proper names and common names, and assesses the effect of the MCIPD stage on the test result. The comparison of the test group results with various degrees of MCI indicates a progressive decrease in verbal fluency. The experiment showed different profiles of the verbal task in terms of common and proper names.

Memantine, uncompetitive antagonist of NMDA receptor, is currently registered worldwide for the treatment of moderate to advanced dementia in the course of Alzheimer’s disease and is used either in monotherapy or in combination with cholinesterase inhibitor. Memantine’s efficacy has been documented in several clinical trials within cognition, behaviour and activities of daily living domains; the more advanced dementia the bigger effect size is usually seen. Adding memantine to an ongoing cholinesterase inhibitors exerts augmentation. Memantine efficacy in less advanced stages of Alzheimer’s dementia is the matter of controversy. It is highly unlikely to see an evident clinical benefit in the group of patients with initial MMSE above 20. Early treatment with memantine is thought to exert long-term consequences as probable disease-modifying effect may come to action. Data supporting this view (and, as a result, early treatment with memantine) is mainly of clinical nature and need to be confirmed with the use of validated disease progression biomarkers. Patients with mild dementia of Alzheimer’s type may be rationally treated with memantine monotherapy, even considering probable lack of symptomatic effect, when cholinesterase inhibitors are contraindicated or poorly tolerated. Data supporting the use of memantine in mild cognitive impairment are currently insufficient.
Memantyna, niekompetycyjny antagonista receptora NMDA, jest obecnie zarejestrowana do leczenia choroby Alzheimera, w otępieniu o nasileniu umiarkowanym do znacznego, w monoterapii lub połączeniu z inhibitorem cholinesterazy. Badania kliniczne udokumentowały jej skuteczność w zakresie funkcji poznawczych, zachowania oraz codziennego funkcjonowania. Efekt kliniczny jest zwykle tym bardziej widoczny, im większe jest nasilenie otępienia. Dodanie memantyny do uprzednio stosowanego inhibitora cholinesterazy wzmacnia efekt objawowy. Skuteczność memantyny w mniej nasilonych stadiach otępienia stanowi przedmiot kontrowersji. Dostępne dane kliniczne sugerują brak widocznego efektu objawowego u większości pacjentów z wyjściowym MMSE >20. Wczesne włączenie leku może mieć długoterminowe konsekwencje w postaci modyfikacji naturalnej progresji dysfunkcji poznawczych. Dostępne dane wspierające taki pogląd (i wczesne stosowanie memantyny) oparte są jednak tylko na badaniach klinicznych i wymagają potwierdzenia w analizach z wykorzystaniem neurobiologicznych markerów choroby Alzheimera. Niewątpliwie racjonalne, pomimo prawdopodobnie niewielkiego wpływu objawowego, jest włączenie memantyny u chorych z lekkim otępieniem, którzy nie mogą być leczeni inhibitorem cholinesterazy. Przesłanki, aby stosować memantynę w stadium łagodnych zaburzeń poznawczych, należy obecnie uznać za niewystarczające.

Opis:
Radiologically isolated syndrome (RIS) was defined for the first time in 2009 with an attempt to establish objective criteria of diagnosis for the patients, who underwent brain MRI scanning for a reason other than multiple sclerosis (MS), but were found to have white matter lesions in their central nervous systems (CNS) similar to those present in patients with diagnosed MS. RIS has been defined as separate entity with the presence of MRI findings strongly suggestive of MS in a patient with no neurological manifestations or other clear-cut explanation. Healthy patients may have an initial MRI procedure performed due to different reasons other than suspicion of MS, mainly because of headaches. However, a clinical examination does not reveal any signs of focal neurological deficits and there are no evidence for the focal damage in the CNS in these patients as well. Although RIS is not the first stage of multiple sclerosis in every patient, 30 up to even 45% of individuals diagnosed with this condition will present clinical symptoms in the future, within median time from 2.3 to 5.4 years depending on various researches. Most authors agree, that about 1/3 of patients with RIS will convert to clinically definite MS within 5 years of follow-up. There are some significant predictors of conversion, among others - presence of lesions in cervical and thoracic spinal cord. Moreover, patients with RIS, although asymptomatic in the meaning of classic clinical presentation of MS, are proved to experience early axonal loss, brain atrophy, increased anxiety and depression and subclinical inflammatory disease, as well as some signs of cognitive impairment. In this article we aim to make a review of the newest papers published in 2017 and 2018 concerning Radiologically Isolated Syndrome.
