The guideline titled “Prevention and treatment of pressure ulcers: clinical practice guideline” is the result of a collaborative effort between the National Pressure Ulcer Advisory Panel (NPUAP), European Pressure Ulcer Advisory Panel (EPUAP) and Pan Pacific Pressure Injury Alliance (PPPIA). A comprehensive literature review was conducted on pressure ulcer prevention and treatment. Rigorous scientific methodology was used to appraise available research and make evidence-based recommendations for the prevention and treatment of pressure ulcers. Draft guidelines were made available to 986 invited individual stakeholders and organizations or societies, and the feedback of the stakeholders was taken under consideration by the developers of the guideline. The guideline includes 575 explicit recommendations and/or research summaries for multidisciplinary pressure ulcer topics.

Aim/purpose - The purpose of this paper is to clarify the certification process of the Good Clinical Practice (GCP) competencies based on a comparison with the project management (PM) certification process. This aim was accomplished by model development. Originality/value/contribution - The paper raises research topics at the crossroads of project management, clinical trials, and GCP, topics that have been underdeveloped so far. The results might be significant for all organizations involved in conducting clinical research projects. The findings contribute to the quality of clinical trials and provide public assurance that the rights, safety, and well-being of trial subjects are protected and the clinical trial data are credible. Design/methodology/approach - The study was divided into three main phases: 1st phase - the identification of key characteristics of PM and GCP certification processes, 2nd phase - the development of certification models for PM and GCP, 3rd phase - the conclusions from in-depth interviews. Findings - As a result of the research, key characteristics of PM and GCP certification processes were identified, certification models for PM and GCP were developed. In addition, based on conclusions from in-depth interviews, solutions for organizing the way of confirming knowledge of GCP guidelines were proposed. Research implications/limitations - The proposed rules may be too complex and may exceed the needs and expectations of the clinical trial environment. The models focus on stakeholder relations, without the rules of certification granting procedures, to enable broader contextualization of the issues discussed in the paper. The analysis might be fragmented as it regards the sole certification process. The research refers to Poland only, hence it will be valuable to identify whether foreign authorities have the same attitude to the GCP competency certification model. It is recommended to increase the number of respondents' interviews to obtain results of higher validity and reliability.

Paraneoplastic syndromes consist of disorders that accompany benign and malignant tumours, but are not directly related to mass effects or invasion by the primary tumour or its metastases. The pathophysiology of mostly paraneoplastic syndromes is not well known. The usual mechanism of their development is the aberrant production of substances (protein hormones, hormone precursors or hormone-like substances) by tumour tissue or autoimmune disorder. The neurological paraneoplastic syndromes are autoimmune disorders that can affect almost any part of the nervous system. The endocrine paraneoplastic syndromes are often certain clinical syndromes like paraneoplastic hypercalcaemia or the aberrant production of hormones by cancers. Many haematologic conditions, including anaemia, leucocytosis, thrombocytopenia or thrombocytosis, coagulapathy have been reported in association with cancer. Also paraneoplastic rheumatic syndromes have been reported in association of cancer. Skin can be involved in cancer in different ways: by metastases, as a part of a genetic disorder with a cutaneous component in which there is an inherited predisposition to the later development of malignancy, as a part of an acquired syndrome due to the toxicity of a carcinogen that induces malignant change and accompanying skin changes, as a consequence of immunosuppression, or by the development of specific lesions that occur as a paraneoplastic syndromes The most frequently diagnosed dermatologic conditions include acrokeratosis paraneoplastica, Sweet’s syndrome and paraneoplastic pemphigus. The purpose of this article is to describe the most popular in internal clinical practice neurologic, endocrine, haematologic, rheumatologic and dermatologic paraneoplastic syndromes associated with cancer.

In the last decades, clinical evidence and expert consensus have been encoded into advanced Decision Support Systems (DSSs) in order to promote a better integration into the clinical workflow and facilitate the automatic provision of patient specific advice at the time and place where decisions are made. However, clinical knowledge, typically expressed as unstructured and free text guidelines, requires to be encoded into a computer interpretable form suitable for being interpreted and processed by DSSs. For this reason, this paper proposes an ontological framework, which enables the encoding of clinical guidelines from text to a formal representation, in order to allow querying, advanced reasoning and management in a well defined and rigorous way. In particular, it jointly manages declarative and procedural aspects of a standards based verifiable guideline model, named GLM-CDS (GuideLine Model for Clinical Decision Support), and expresses reasoning tasks that exploit such a represented knowledge in order to formalize integrity constraints, business rules and complex inference rules.

Despite the increasing popularity of the concepts known as person-centered care and the holistic approach, their implementation in real life is far from optimal. Patients’ priorities, preferences, and values are still too often neglected. The tendency to measure the outcomes of primary care just in terms of avoiding hospital admissions, reducing health care costs, and increasing adherence to treatment can cause problems and create distortion. Guidelines are too focused on single diseases and not patient-focused. Most guidelines have a “one-size-fits-all” mentality and do not build flexibility or contextualization into their recommendations. Quality indicators should be used with caution and wisdom, especially in primary care, as they are mainly related to a few common chronic diseases and this is not conducive to recognizing the vast range of health problems of our patients. Quality indicators can be useful as a starting point for discussions about quality in primary care but not all the data that we have in our electronic clinical records can be used to derive good quality indicators and they cannot reflect the broad scope of primary care. Some core values are difficult to measure because doctors and nurses are pushed to spend too much time on the registration and administration of the required data rather than dedicate this time to the actual care of the patient. Person-centered health care is certainly one the visions of primary care and primary care doctors need to step up and lead the change. Rural primary care doctors, who traditionally adopt a less biomedical and more holistic approach than their urban counterparts, could become the pioneers in the implementation of this process.

The aim of this study was to develop an action checklist for educational training of clinical nurses. The study used qualitative and quantitative methods. Questionnaire items were extracted through in-depth interviews and a questionnaire survey. PASW version 19 and AMOS version 19 were used for data analyses. Reliability and validity were tested with both exploratory and confirmative factor analysis. The levels of the indicators related to goodness-of-fit were acceptable. Thus, a model kit of work improvements in clinical nursing was developed. It comprises 5 domains (16 action points): health promotion (5 action points), work management (3 action points), ergonomic work methods (3 action points), managerial policies and mutual support among staff members (3 action points), and welfare in the work area (2 action points).

The article is an attempt to reflect on the current tasks faced by clinical psychology as a field of science and practice. These tasks stem from the constant need to improve the scientific level of this discipline and to solve problems connected with intensive social changes. We have considered the issues of globalization and the challenges it implies for clinical psychology, presenting selected theories of globalization and the patterns of reaction to these changes. We see evidence-based practice in psychology as a reflection of these transformations in science and clinical practice. In the article, we discuss the positive influence of this approach on the improvement of assessment practice standards and psychotherapy effectiveness, drawing attention to some negative consequences of universalization, particularly those that threaten human agency. Addressing and responding to these challenges requires close cooperation of researchers and practitioners, which was called for at the 1st National Clinical Psychology Conference in 2014.

The academic development of general practice (GP) within countries of Central and Eastern Europe began only after the political changes in the 1990s. From a research perspective GP has a reputation for being amongst the least intellectually rigorous clinical disciplines. This has several serious consequences; loss of self-confidence, lack of attractiveness as an intellectual discipline and less application of critical thinking in routine clinical work. The only solution is an effort to develop research in primary care, as this is the ultimate attribute of scientifically oriented disciplines such as medicine. GP research in the Czech Republic has a low level of awareness and support. Its development is slow and faces many barriers; lack of financial resources, lack of leading personalities within the field, lack of ideas, lack of scientific education, lack of experience, and insufficient research capacity. As GPs, we do not have enough skills to write and publish scientific papers. Research is also not recognised as an essential component of GP training. However, research in primary care is developing and here I present several successful research projects within our department. Research has been proposed as a vital agenda towards the new concept of general practice, as recently presented to the government by the Society of GPs.

The aim of this paper is to describe the most relevant aspects of a competency-based curriculum in communication sciences and disorders education. Competences to be developed are associated to cognitive, technical, relational, affective, integrative and contextual dimensions. The new curriculum has duration of five years with 49% of the work-load assigned to supervised practice. This way, expected learning outcomes by the end of the first year involve the students knowing what they learned about the basic sciences of the area. By the end of the second year, the students should know what they learned about specific areas of the field. By the end of the third year, the students should know how to use what they learned. At the end of the fourth year, the students should be able to demonstrate how to use what they learned. Finally, at the end of the program, the students should perform professional activities with expertise, ethics and critical thinking. To sum up, the new curriculum proposed provides the student with general and specific knowledge about communication sciences and disorders and different scenarios of supervised practice. Its main objective is to develop the adequate competencies to professional practice.
Celem niniejszego artykułu jest przedstawienie najbardziej istotnych aspektów nauczania opartego o wymogi kompetencyjne w naukach o ludzkiej komunikacji i edukacji specjalnej. Opracowano wymogi kompetencyjne w związku z uwarunkowaniami poznawczymi, technicznymi, relacyjnymi, emocjonalnymi i integracyjnymi pracy logopedy. Nowy program przewiduje okres 5 lat studiów z 49-procentowym udziałem nadzorowanej praktyki. W ten sposób, już przed końcem I roku studiów, studenci zaangażowani w pracę zdobywają wiedzę o podstawowych naukach wykorzystywanych na polu logopedii. Pod koniec II roku studenci powinni mieć bardziej szczegółową wiedzę o konkretnych obszarach tej dyscypliny. Pod koniec III roku powinni wiedzieć, jak wykorzystać to, czego się nauczyli. Pod koniec IV roku powinni być w stanie wykazać, jak korzystać z tego, co się nauczyli. Wreszcie, pod koniec programu, uczniowie powinni wykonywać czynności zawodowe z doświadczeniem, etyką i wykorzystaniem krytycznego myślenia. W podsumowaniu należy stwierdzić, że nowy program nauczania proponuje dostarczenie studentowi ogólnej i szczegółowej wiedzy na temat nauk o komunikacji i jej zaburzeniach oraz różnych rodzajach nadzorowanej praktyki. Jego głównym celem jest rozwijanie odpowiednich umiejętności, koniecznych do wykonywania praktyki zawodowej.
