Opis:
Introduction. Cervical cancer is one of the most frequent malignant neoplasms in females. Such neoplasms can be almost completely eliminated by means of adequate primary and secondary prophylaxis. The most developed countries focus their attention on the improvement of prophylaxis systems, test quality, and efficacy improvement, as well as on the expansion of the primary prophylaxis. In Poland, the discussions focus on the improvement of the malfunctioning population programme. Objective. Drawing on recent research findings, the article presents current Polish and global recommendations with regard to screening tests for cervical cancer. The results of the Population Programme of Prophylaxis and Early Detection of Cervical Cancer are discussed in the context of current trends of healthy behaviour among women inhabiting rural areas. Description of the state of knowledge. In the future, it will be relevant to increase the number of human papillary virus (HPV) tests as part of the prophylaxis programme, especially among the high risk patient group. In particular, there is a necessity to increase the number of vaccinations among young women, especially before the beginning of their sexual activity, as well as to establish new strategies of secondary prophylaxis in this group. At present, women who had been vaccinated should undergo routine screening. In Poland, the report based on SIMP registry (IT System of Prophylaxis Monitoring), shows that only 27% of the 3.3 million of invited women participated in the cytology tests. The data analysis shows that women living in rural areas more often respond and participate in the tests, compared to women living in the cities (39.3% vs. 16.8%).

Opis:
Breast and cervical cancer represent a significant health and economic issue for Polish society, although if detected early, both can be cured successfully. For this reason, since 2006, according to the National Cancer Combat Programme, populationbased screening programmes have been implemented, aimed at reducing the mortality and morbidity for breast and cervical cancer. The aim of this study is to determine which of the selected four environmental factors affect attendance for screening mammography and cytology. Analysis included data from questionnaires filled in during mammography by 582,959 women aged 50–69 years, and 288,142 women during cytology, aged 25–59 years, in 2007–2012 in the Wielkopolska Province of Poland. It was found that the impact of medical staff on the attendance for cytological screening was the strongest statistically significant factor (p = 0,0001). Invitation by name (p=0,001) and other factors (p= 0,0001) also affected the attendance. In the cytological screening, medical staff was the factor that had the greatest impact on attendance. Other factors, such as self-reporting, increased participation in the next screening rounds, although the factors that affect attendance changed over time. Their constant analysis is essential for the efficient and effective evaluation of screening programsme.

The research results presented in this article originate from the research and educational project conducted from January to June 2017, titled Prevention of cervical carcinoma, or how it is done in Tychy. Analysis of knowledge sources and the views on HPV vaccines, which was funded with a grant by the Polish Cancer League Foundation. The research material allowed for formulating the guidelines for the educational and preventive treatment programs addressed to young people and their parents. Despite free vaccinations against the HPV virus from some local governments, the percentage of people covered by vaccinations is decreasing. What is needed in order to reverse the negative tendencies is a set of new, innovative solutions, addressed at the needs and expectations of the beneficiaries. Therefore, people should be educated on the risk of falling ill, while HPV vaccinations constitute an important element of primary prevention.

Background. In the female population, cervical cancer is classified as one of the too late diagnosed cancers. Cervical cancer incidence and mortality rates in Poland are among the highest in the European Union. The aim of the paper is the assessment of women’s knowledge about cervical cancer based on the study of the feminine population in the county of Biała Podlaska. Material and methods. The study covered a group of 172 women living in the county of Biała Podlaska, Lubelskie Voivodeship, Poland. The research tool was an original questionnaire. The statistical analysis was performed using the STATISTICA v 10. program and the Pearson Chisquare test. Results. More than half of respondents (55.8%) indicated the following risk factors of cervical cancer: early onset of sexual life, many partners in a partner, infection of the cervix with HPV and herpes viruses, many deliveries, and smoking. The majority of respondents (62.2%) believe that cytology and gynecological examination are part of the preventive examination. Conclusions. 1. Respondents’ knowledge about the risk factors of cervical cancer is sufficient. 2. Respondents above 26 years of age possess knowledge about human papillomavirus (HPV) vaccination as a means of preventing cervical cancer. 3. Young women’s knowledge about HPV vaccination is insufficient.
Wprowadzenie. Materiał i metody. Background: Cancer, next to cardiovascular disease, obesity, diabetes, accidents, and mental illness, is one of the most common diseases of the 21st century. In the female population, cervical cancer is most often diagnosed at late stages. Aim of the study: The goal of the study is to determine the opinions of women who reside in Bialski poviat about midwife participation in cervical cancer screening and prevention. Material and methods: The study group was recruited from randomly selected patients from two physiotherapy offices in the Bialski poviat, as well as students and employees of the State University Pope John Paul II in Biala Podlaska. The research tool consisted of the author’s questionnaire concerning the role of midwives in cervical cancer prevention and consisted of 25 questions. Results: Almost half of the respondents or 44.2% (76) believe that the role of a midwife in cervical cancer prevention is based on health education given to women about cervical cancer screening and prevention. 27.3% (47) believe that the role of a midwife in prevention is based on the availability of cytological pap smears. Most or 56% (14) respondents from the age group over 55 and 29.2% (7) surveyed in the 18–25 age group knew about the important role of midwives performing cytological pap smear as part of a prevention strategy against cervical cancer. Conclusions: 1. The knowledge base of the women examined, regarding the role of a midwife in the prevention of cervical cancer, is lacking or insufficient. 2. The role of the midwife in the prevention of cervical cancer is unknown to young women in the 18–25 age group.

Opis:
Cervical cancer is the fourth leading cause of malignancy-related mortality in women worldwide, and effective advanced-stage therapies are urgently required. Berberine is a quaternary ammonium compound extracted from traditional Chinese medicinal herbs, including Phellodendron spp., with antibacterial and antitumor activities. Matrine, the main active ingredient of Sophora flavescens rhizomes, has not only traditionally described health effects but is also widely used for its anticancer, anti-inflammatory, immunoregulatory, antiviral, and hepatoprotective effects. We investigated the antitumor activities of berberine and matrine against human cervical cancer HeLa and SiHa cells using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, along with flow cytometry and western blotting analyses, to assess the effect of the compounds on the cellular status and apoptosis- and cell cycle-related proteins. The 24 h half-maximal inhibitory concentrations (IC50) of berberine and matrine were 123.633 ± 4.278 µmol/L and 9.625 ± 0.245 mmol/L against HeLa cells and 105.067 ± 3.745 µmol/L and 8.50 ± 0.23 mmol/L against SiHa cells, respectively. Berberine plus matrine inhibited cancer cell growth and caused cell cycle arrest. We observed an increased stimulation of apoptosis, which was likely mediated by enhanced levels of caspase-3, caspase-9, and B-cell lymphoma 2 (Bcl-2)-associated X protein (Bax), as well as decreased Bcl-2 protein expression. Cell cycle arrest in the G1 phase was probably mediated by p21 upregulation and cyclin-dependent kinase (Cdk)-4, Cdk-6, and cyclin D1 suppression. Combination treatment with berberine and matrine effectively inhibited human cervical cancer cell proliferation, most likely by stimulating apoptosis and inducing cell cycle arrest.

Opis:
Introduction. Human papillomavirus infections are frequent in on average 9% to 13% of the female population, and the risk of infection throughout life exceeds 50%. The HPV virus causes changes in the mucous membranes and the skin of the genital area in both women and men. Aim. This paper presents epidemiology, prophylaxis, diagnostics and treatment of HPV and cervical cancer infections. In addition, the knowledge of issues related to human papillomavirus infection among young women was assessed. Materials and methods. The diagnostic survey method was used to carry out the research. The research tool was a questionnaire of own authorship consisting of 41 questions. The research group consisted of 240 women. The research was carried out from June to September 2016 at the Chodźki Medical Center and the Luxmed facility in Lublin. Results. There is a relationship between the education of the respondents and their knowledge about the human papillomavirus infection. The studies did not show a significant relationship between the self-esteem of the studied women and whether they were vaccinated against the HPV virus (Chi-square = 0.362, p = 0.547). Conclusions. The general level of knowledge on issues related to human papillomavirus infection among young women is at the secondary level. A higher level of general knowledge was shown by women with better education, related to the medical and biological sciences. The self-assessment of the respondents coincides with the general knowledge, based on detailed questions, on issues related to HPV infection. Promoting knowledge about HPV infection is unsatisfactory

Introduction and aim. Reports have shown that there is alteration in haematological and inflammatory processes in patients with cervical cancer. However, there is the dearth of information on the pattern of alteration in Nigerian patients with cervical cancer at various stages of the disease. Therefore, haemocytometric profile and plasma levels of interleukin-6 (IL-6) and IL-12 were determined in Nigerian patients with cervical cancer at various stages of the disease. Material and methods. Eighty-nine adults consisting of 49 patients with cervical cancer and 40 apparently healthy controls were enrolled into this study. Haemocytometric profile was determined using automated haematology analyzer while the plasma levels of interleukin-6 (IL-6) and IL-12 were determined using ELISA. Results. Of the participants with cervical cancer, 6.12%, 24.49%, 53.06% and 16.33% were in stages I, II, III and IV respectively. The mean plasma IL-6 level was significantly higher in patients at stage IV of the cancer compared with those in stages I, II and III. No significant differences were observed in the mean plasma IL-12 level, and the haemocytometric profile when patients in different stages of the cancer were compared with one another. Plasma IL-6 had significant positive correlation with the lymphocytes count and cancer stage but had significant negative correlation with packed cell volume (PCV), haemoglobin and total white blood cells count (WBC) in patients with cervical cancer. Conclusion. Interleukin-6 appears to play an important role in the progression of cervical cancer and could be involved in cervical cancer-associated alteration in haemocytometric profile.

Opis:
Cervical cancer is the second commonest cause of cancer among women. According to the World Health Organization (WHO) data, in 2006 there were 1.4 million women who were suffering from cervical cancer and 80% of deaths among them were of women from developing countries. Genital infection with Human Papilloma Virus (HPV) is one of the major etiological factors for developing cervical cancers in almost all countries (World Health Organization, 2005). Papilloma viruses were first identified, cloned and sequenced from cervical tumor specimens and were subsequently established as important causative agents for the development of cervical cancer. Differences in prevalence rates are observed worldwide. Infection is more common in sexually active young women of 18-30 years and a reduction in acquisition of infection is observed after 30 years. The situation of Cervical cancer in Sri Lanka is identified as the second most common cancer among women in Sri Lanka and approximately 7.74 million women are included in the risk category. It is reported that nearly 850 die from cervical cancer each year.

Opis:
Aim of paper: Neuroendocrine cervical cancer is a significant diagnostic and therapeutic challenge. For many years, these neoplasms were not differentiated from small-cell cancer. It was known, that these cancers were associated with poor prognosis and, in spite of combination of several therapeutic modalities, treatment outcome was far below expectations. Material and methods: Based on retrospective analysis and using synaptophysin and chromogranin A staining and assessment of cellular proliferative activity using MIB1 antibodies, out of 14 small-cell cancers 5 cases were selected, which fulfilled the criteria for neuroendocrine neoplasms and clinical course of the patients affected was analyzed. Results: In all cases, the disease has been diagnosed in FIGO stage I. Treatment consisted in extended hysterectomy m. Wertheim-Meigs and adjuvant radiochemotherapy. In two patients (40%), treatment failure was observed after a relatively short follow-up (within less than 16 months). One patient died due to liver metastases and in the other, faced with failure of chemotherapy, we instituted somatostatin receptor treatment. Conclusion: Immature or small-cell cervical cancer requires differentiation with neuroendocrine cancer and confirmation of its true nature by immunohistochemical studies. Therapy should encompass modern chemotherapy protocols and (in selected cases) isotope therapy using radio-labeled somatostatin analogs. Somatostatin analog therapy is reserved for patients featuring expression of somatostatin receptor on the surface of cancer cells.

Objective: Gynecologic cancers represent a large health, social and economic burden worldwide. In low-income countries, particularly in Mozambique, little data is available and no effective policies are implemented to fight these diseases. Our objective was to trace the epidemiological profile of gynecologic cancers from 2010 to 2014 in Beira, Mozambique. Methods: We retrospectively reviewed the registers of the Department of Pathology of Central Hospital of Beira to identify all cases of gynecologic malignancies recorded from January 2010 to December 2014. Results: Most of the diagnosed female cancers (43.4%) were gynecologic and, among these, cervical cancer was definitely the most commonly reported cancer every year, ranging from 86.7% in 2013 to 93.3% in 2014. Conclusion: As in many low-income countries, the access to screening programs for gynecologic cancer is not effective in Mozambique; therefore urgent preventive policies are crucial to address this emergent issue.
Metoda: Autorzy dokonali retrospektywnego przeglądu rejestrów medycznych Oddziału Patologii Szpitala Centralnego w Beirze w celu wyodrębnienia wszystkich przypadków nowotworów kobiecych narządów płciowych odnotowanych w okresie od stycznia 2010 do grudnia 2014 roku.

Opis:
Introduction and objective. Cervical squamous cell carcinoma is one of the most common malignancies of women. Its incidence and morphology was analyzed based on the magnetic resonance (MR) data among rural and urban residents. Materials and method. The study involved 61 Caucasian women (58.26±9.63 years) preliminary diagnosed with a cervical cancer without any previous treatment. Standard MR examination, including diffusion weighted imagining, apparent diffusion coefficient (ADC) value measurement and dynamic contrast enhancement, was performed. Results. The rural residents (n=22) were insignificantly older. Their first and last menstruation were observed later and number of pregnancy was higher than in urban women (n=39). However, the incidence of miscarriage was insignificantly rarer. All the tumour linear diameters as well as its volume were insignificantly higher in rural women. The ADC value of the cervical tumor was insignificantly lower, while ADC of lymphatic nodules was higher in rural women. Insignificant changes in tumour grade between both examined groups were found in histological, clinical and radiological examinations. Place of residence did not influence any clinical symptoms nor tumour volume and its ADC. Colporrhoea and colpodynia were insignificantly more often observed in urban women, while parametrium, urinary bladder and rectal infiltrations were more commonly seen in rural residents. Higher risk of lymphatic spread to the internal iliac and parametral lymphatic nodes was reported in the rural community. Conclusion. Cervical cancer had similar morphology and growth pattern, regardless of the place of residence. However, a insignificantly larger tumour size among rural residents may suggest a higher incidence of lymphatic spread, probably as a result of less aaccess to modern health care.

Opis:
Introduction. Cervical cancer is the fourth most common cancer among women. It is related to persistent HPV infection. In order to improve diagnostic methods, a lot of research has been focused on detecting HPV DNA. A test known as a high- risk HPV test or HPV primary screening provides very encouraging results. Objective. The aim of this review is to present the actual knowledge about the possibilities of cervical cancer screening methods. Particular attention is paid to the question concerning the effectiveness of detecting viral DNA as a screening programme, compared to pap smear. State of knowledge. The HPV primary screening has higher sensitivity than the cervical smear test and it is able to detect lower-stage lesions, which are considered clinically irrelevant. Nonetheless, many HPV infections recede naturally. Therefore, relying only on the results of this test may expose women to unnecessary colposcopies and stress. Due to this fact, women under 30 years should not be screened with the hrHPV test. In view of its limitations, the HPV primary screening strategy is still tested worldwide as a pilot project. In Poland in 2019, a pilot project with the hrHPV test started at the National Institute of Oncology. Conclusions. Detecting viral DNA has its advantages and disadvantages. Further research is still required, but the hrHPV test has a great opportunity to become one of the main screening programmes worldwide, or at least, a valuable addition to cervical smear test.

Opis:
Objective: To determine the significance of histology and treatment modality on overall survival and cause-specific survival in stage IB1 cervical carcinoma. Methods: Cases of stage IB1 squamous cell carcinoma, adenocarcinoma, and adenosquamous cell carcinoma of the uterine cervix managed with either radical hysterectomy, definitive radiation therapy which may include external beam radiation therapy and/or vaginal brachytherapy, or total abdominal hysterectomy with adjuvant radiation therapy which may include external beam radiation therapy plus/minus vaginal brachytherapy were abstracted from the SEER database (2004–2008). Cause-specific survival was calculated using Kaplan–Meier, log-rank, and multivariable Cox regression analyses. Results: Five-year cause-specific survival for squamous cell carcinoma, adenocarcinoma, and adenosquamous cell carcinoma were 94.4%, 97.3%, and 85.7%, respectively (p = 0.001 by log-rank test). On multivariable Cox regression analysis, patients with squamous cell carcinoma were not more likely to die of cervical cancer than patients with adenocarcinoma (hazard ratio = 1.12, 95% confidence interval = 0.53–2.39); but patients with adenosquamous cell carcinoma were more likely to die of cervical cancer than patients with adenocarcinoma (hazard ratio = 3.65, 95% confidence interval = 1.41–9.44). Five-year cause-specific survival was 96.9%, 80.0%, and 92.4% for patients receiving radical hysterectomy, definitive radiation therapy, and total abdominal hysterectomy plus radiation therapy, respectively (p < 0.0001 by log-rank test). On multivariable Cox regression analysis, patients who received definitive radiation therapy and patients who received total abdominal hysterectomy plus radiation therapy were more likely to die of cervical cancer than patients who received radical hysterectomy. Conclusion: In patients with stage IB1 cervical cancer, on multivariable Cox regression analysis, patients with squamous cell carcinoma were not more likely to die of cervical cancer than patients with adenocarcinoma; patients with adenosquamous cell carcinoma were more likely to die of cervical cancer than patients with adenocarcinoma. Patients who received either definitive radiation therapy or total abdominal hysterectomy plus radiation therapy were more likely to die of cervical cancer than patients who received radical hysterectomy.
