The explanation as to why married cancer patients are characterised by better adjustment to illness than unmarried patients is not fully understood. This article aims to investigate a parallel mediation effect of meaning in life and sense of coherence in the relationship of spousal support and illness acceptance in breast cancer patients. A total of 213 women were included in this study. The Berlin Social Support Scales, the Personal Meaning Profile, the Sense of Coherence Scale, and the Acceptance of Life with the Disease Scale were used. Pearson’s correlation coefficient and mediation analysis were performed. The results showed significant correlations between spousal support, meaning in life, sense of coherence, and illness acceptance. Except for protective buffering support, meaning in life mediated spousal supports’ effect on illness acceptance. A sense of coherence mediated the relationship of perceived available support, actually received support, and protective buffering support in terms of illness acceptance. Meaning-based resources play a significant role in accepting the detrimental mental and physical consequences of breast cancer. Therefore, practitioners can incorporate these resources into rehabilitation programmes to improve adjustment to cancer.
Wyjaśnienie, dlaczego pacjentki z rakiem pozostające w związku małżeńskim charakteryzują się lepszym przystosowaniem do choroby niż pacjentki niezamężne, nie jest w pełni zrozumiałe. Celem niniejszego artykułu jest zbadanie równoległego efektu mediacji poczucia sensu życia i poczucia koherencji w związku wsparcia małżeńskiego i akceptacji choroby u pacjentek z rakiem piersi. W badaniu wzięło udział 213 kobiet. Zastosowano Berlińską Skalę Wsparcia Społecznego, Profil Znaczenia Osobistego, Skalę Poczucia Koherencji oraz Skalę Akceptacji Życia z Chorobą. Przeprowadzono analizę współczynnika korelacji Pearsona oraz analizę mediacji. Wyniki wykazały istotne korelacje pomiędzy wsparciem małżeńskim, poczuciem sensu życia, poczuciem koherencji i akceptacją choroby. Z wyjątkiem buforującego wsparcia ochronnego, poczucie sensu życia pośredniczyło we wpływie wsparcia współmałżonka na akceptację choroby. Poczucie koherencji pośredniczyło w relacjach między postrzeganym dostępnym wsparciem, faktycznie otrzymywanym wsparciem i buforującym wsparciem ochronnym w zakresie akceptacji choroby. Zasoby oparte na znaczeniach odgrywają istotną rolę w akceptacji negatywnych psychicznych i fizycznych konsekwencji raka piersi. Lekarze mogą zatem z powodzeniem włączyć te zasoby do programów rehabilitacyjnych w celu poprawy przystosowania do choroby nowotworowej.

The examination of the psychological functioning of cancer patients’ spouses implies that affect and personal resources play an important role in their psychological well-being. This is a consequence of the severe stress caused by the medical conditions of a spouse and of responsibilities related to caregiving. This study aimed to examine the role of affect and purpose-oriented personal resources (meaning in life and self-efficacy) in shaping psychological well-being in the spouses of cancer patients. The participants were 214 spouses of gastrointestinal patients who were either undergoing medical treatment in hospital units or attending scheduled clinic appointments at outpatient medical clinics. The Positive and Negative Affect Schedule (PANAS-X), Psychological Well-Being Scale (PWB), Meaning in Life Questionnaire (MLQ), and General Self-Efficacy Scale (GSES) were used. The results indicated that positive affect was positively correlated with meaning in life, self-efficacy, and psychological well-being. In contrast, negative affect was negatively correlated with positive affect, meaning in life, self-efficacy, and psychological well-being. The mediation analyses demonstrated that meaning in life and self-efficacy were mediators between positive and negative affect and psychological well-being. The mediating function of meaning in life and self-efficacy revealed the significant role of personal resources based on purpose and self-motivation in the relationship between affect and psychological well-being. Positive and negative affect can influence motivational processes that enable patients and their families to constructively deal with challenging and illness-related situations. Caregivers and medical staff could take into account the important role of purpose-oriented resources as they affect the functioning of both patients and their families.

A growing number of targeted anticancer agents has shown the unexpected ability to induce QT interval prolongation. In addition, standard chemotherapeutics and a variety of conditions such as electrolyte abnormalities, endocrine disorders, cardiac diseases, nutritional disturbances and other factors may be associated with long QT syndrome in cancer patients. Prolongation of the QT interval can lead to life-threatening ventricular arrhythmias, including ‘torsade de pointes’ (TdP). The association between long QT interval and ventricular arrhythmias remains the subject of many controversies. The QT interval represents the time interval of both ventricular depolarization and repolarization. Not only abnormalities of ion channels, but also changes in the myocardial microarchitecture and other factors and disorders frequently seen in cancer patients may participate in its prolongation and potential risk of ventricular arrhythmias. The aim of this review was to summarize current knowledge about QT prolongation in cancer patients with the special focus on targeted therapy.

INTRODUCTION: Cancer and its treatment with cytostatic drugs entails a number of negative experiences, emotions and side effects. Taking care of a oncological patient reveals a strong dependence between the quality of life and the patient's abilities and capabilities to meet their individual needs. OBJECTIVE: Evaluation of the quality of life and assessment of the influence of cancer treatment with cytostatic drugs on the quality of life of patients with blood cancers. MATERIAL AND METHODS: The study was conducted among 50 patients from the Department of Hematology, Blood Cancer and Bone Marrow Transplantation. In order to examine the quality of life of people in the course of treatment with cytostatic drugs in hematological cancers, a diagnostic survey method was implemented with the use of the author’s self-designed questionnaire and the EORTIC QLQ-C30 questionnaire in the Polish version. RESULTS: Analysis of the results of the study material reveals that: almost all the respondents suffered side effects of cytostatic medication; half of the respondents could not define their quality of life as good or bad; regardless of gender, the respondents function best in the cognitive aspect. CONCLUSIONS: The illness and its treatment had a greater negative impact on the sexual life in men than in women. 1. Women evaluated the quality of life significantly worse in terms of physical functioning, whereas men evaluated significantly worse the quality of life in terms of symptoms such as pain, shortness of breath and constipation. 2. Fatigue, as a symptom of the disease, was the most common ailment mentioned by the respondents. 3. Demographic factors, i.e age, marital status, professional activity, education level and place of residence correlated with the selected domains of life quality. 4. The overall assessment of patients' quality of life was reduced on average by 50%.
WSTĘP: Choroba nowotworowa i leczenie za pomocą leków cytostatycznych niesie ze sobą szereg negatywnych doznań, emocji oraz skutków ubocznych. W opiece nad pacjentem onkologicznym jakość życia pozostaje w silnej zależności ze zdolnościami i możliwościami chorego do zaspokajania jego indywidualnych potrzeb. CEL: Celem pracy była ocena jakości życia oraz ocena wpływu leczenia lekami cytostatycznymi na jakość życia chorych na nowotwory krwi. MATERIAŁ I METODY: Badania zostały przeprowadzone wśród 50 pacjentów Kliniki Hematologii, Nowotworów Krwi i Transplantacji Szpiku. W celu zbadania jakości życia osób w trakcie leczenia lekami cytostatycznymi w chorobach nowotworowych krwi posłużono się metodą sondażu diagnostycznego, z wykorzystaniem kwestionariusza ankiety własnego autorstwa oraz kwestionariusza EORTIC QLQ-C30 w wersji polskiej. WYNIKI: Z analizy materiału badawczego wynika iż: u prawie wszystkich badanych wystąpiły skutki uboczne przyjmowania leków cytostatycznych; połowa badanych oceniła jakość swojego życia ani dobrze, ani źle; bez względu na płeć ankietowane osoby najlepiej funkcjonują w aspekcie poznawczym. WNIOSKI: 1. Choroba i leczenie wywarły większy negatywny wpływ na życie intymne mężczyzn niż kobiet. 2. Kobiety istotnie niżej oceniły jakości życia w aspekcie funkcjonowania fizycznego, natomiast mężczyźni istotnie gorzej ocenili jakości życia w skali objawów takich, jak ból, duszność oraz zaparcia. 3. Zmęczenie, jako objaw choroby, było najczęściej zgłaszaną dolegliwością wśród ankietowanych. 4. Czynniki demograficzne, tj. wiek, stan cywilny, aktywność zawodowa, poziom wykształcenia oraz miejsce zamieszkania, korelowały z wybranymi domenami jakości życia. 5. Ogólna ocena jakości życia pacjentów była obniżona średnio o 50%.

Objectives: Religiosity may serve as a personal source of support when people face a life-threatening illness, but it can also elicit stress. The main aim of this study is to show how various religious dimensions interplay in predicting death anxiety in patients diagnosed as having cancer. Material and Methods: In the cross‐sectional, descriptive‐analytical research, 141 Polish patients who were hospitalized due to cancer were selected using sequential convenience sampling. Data were collected using the Centrality of Religiosity Scale, Religious Comfort and Strain Scale, and the Death Anxiety and Dying Distress Scale. Results: The authors’ results show that the effect of the centrality of religiosity on death anxiety is non-linear. The authors can also confirm the mediating role of religious comfort and struggles in the relationship between the centrality of religiosity and death anxiety. Conclusions: Thus, religious struggles appear to weaken the effect of religion on death anxiety, whereas religious comfort (contrary to expectation) does not enhance it.

Guidelines for the pharmacotherapy of pain in cancer patients were developed by a group of 21 experts of the Polish Association for the Study of Pain, Polish Society of Palliative Medicine, Polish Society of Oncology, Polish Society of Family Medicine, Polish Society of Anaesthesiology and Intensive Therapy and Association of Polish Surgeons. During a series of meetings, the experts carried out an overview of the available literature on the treatment of pain in cancer patients, paying particular attention to systematic reviews and more recent randomized studies not included in the reviews. The search was performed in the EMBASE, MEDLINE, and Cochrane Central Register of Controlled Trials databases using such keywords as “pain”, “cancer”, “pharmacotherapy”, “analgesics”, and similar. The overviewed articles included studies of pathomechanisms of pain in cancer patients, methods for the assessment of pain in cancer patients, and drugs used in the pharmacotherapy of pain in cancer patients, including non-opioid analgesics (paracetamol, metamizole, non-steroidal anti-inflammatory drugs), opioids (strong and weak), coanalgesics (glucocorticosteroids, α2-adrenergic receptor agonists, NMDA receptor antagonists, antidepressants, anticonvulsants, topical medications) as well as drugs used to reduce the adverse effects of the analgesic treatment and symptoms other than pain in patients subjected to opioid treatment. The principles of opioid rotation and the management of patients with opioidophobia were discussed and recommendations for the management of opioid-induced hyperalgesia were presented. Drugs used in different types of pain experienced by cancer patients, including neuropathic pain, visceral pain, bone pain, and breakthrough pain, were included in the overview. Most common interactions of drugs used in the pharmacotherapy of pain in cancer patients as well as the principles for the management of crisis situations. In the final part of the recommendations, the issues of pain and care in dying patients are discussed. Recommendations are addressed to physicians of different specialties involved in the diagnostics and treatment of cancer in their daily practice. It is the hope of the experts who took part in the development of these recommendations that the recommendations would become helpful in everyday medical practice and thus contribute to the improvement in the quality of care and the efficacy of pain treatment in this group of patients.

Types of oncological rehabilitation and its objectives are described. It includes information on many manifestations of oncological diseases that an internist has to deal with, which are conditioned not only by the oncological process itself, but by the cancer treatment conducted, as well as the attitude of doctors and patients to cancer pathology. The progress in improving the survival of cancer patients in Russia, as well as throughout the world, means that in the next years it is going to be an increase in the number of disabled people who has achieved remission, or who faces the consequences of high-dose polychemotherapy and/or radiotherapy between courses of cancer treatment. Until recently, the two main socially significant medical problems in Russia, cancer and cardiac, have been rarely united into one. Publications on this topic appeared only in specialized cancer journals and were unknown by a wide range of cardiologists and internists, therefore the involvement of these specialists in the study of theoretical and practical aspects of the management of patients with malignant tumors was limited. Creation of the International Society of Cardioncology (ICOS) contributed to the development of an interest in this interdisciplinary field of modern medicine. However, even today cardiac problems of cancer patients in Russia mostly are limited to the cardiotoxicity studying, but, according to the position of the authors a great attention should be paid to the systemic reactions, including cardiovascular reactions of the organism to the tumor, as well as to the therapeutic issues and cardiac rehabilitation of cancer patients.

Following the development of the sociology of illness and observed changes in relation to used theoretical frameworks and paradigms, special attention has been paid to cancer due to psychosocial consequences of cancer diagnostics, medical treatment and “living with cancer”. Based on empirical experiences coming from performed sociomedical studies, the role of illness behaviors in cancer patients and subjective assessment of health-related quality of life by survivals have been presented in the increasing “society of remission”.

The oncology package is the common name of set of legal provisions seeking to advance standard care of cancer patient. Due to the fact that cancers are second most frequent cause of deaths in Poland, it was necessary to introduce new regulations which would improve early detection of cancers and ensure fast track therapy. Since the introduction of new provisions in scope of oncology package, in the public system of health care a new category of patients, that is patients waiting for cancer therapy (cancer patients), has emerged. The intention of the legislator was to make diagnosis for patients with suspected malignant tumours more available. Hence, such patients were given an opportunity to be entered onto separate waiting list (list of cancer patients). The key issue was also to set diagnostic and therapy process in order. Thus, a cancer diagnosis and therapy form was introduced and coordinator, who oversees therapy process, was appointed. Other organizational changes in the health care system included in oncology packet cover: determination of maximum period for performance of individual stages of diagnosis and therapy, as well as abolition of limits for funding cancer diagnosis and therapy. The oncology package introduces positive changes, however there are some drawbacks. The most questionable issue is separation of group of oncology patients on the basis of disease type and favouring them, as it may seem, at the expense of other patients.

Colorectal cancer (CC) in Poland is the type of cancer with the highest dynamics of disease growth and is epidemiologically related to age. The analysis involved 353 patients operated on due to CC in senile and old age and compared with younger patients. It was found that people at this age are more often diagnosed with CC They were more often women, the patients did not differ in the stage of cancer, while they were significantly more often qualified for surgery due to urgent indications. In patients with colonic cancer, the resectability and radicality of the procedures in comparison with patients with rectal cancer was significantly higher, while there were more complications and deaths in the 30-day follow-up in this group. The overall survival in senile and old age was significantly worse. In the first year of follow-up after surgical treatment of patients in this group, complications and deaths were more frequently observed. However, in patients who survived 12 months after the operation, the overall survival rate did not significantly differ.

The aim of the study was to assess patients’ awareness of the prevention and treatment of colorectal cancer. Material and methods. Patients diagnosed with colorectal cancer, hospitalised at the Department of General and Colorectal Surgery of the Medical University in Łódź during the period from January 2015 to April 2015, were asked to complete a questionnaire concerning their families’ medical case record, factors predisposing them to the development of colorectal cancer, the tests applied in diagnostics, and the treatment process. The questionnaire comprised 42 closed-ended questions with one correct answer. A statistical analysis of all answers was carried out. Results. The study group consisted of 30 men and 20 women aged 27–94 years old. A strong, statistically significant negative correlation between a patient’s age and his/her awareness of the prevention and treatment of colorectal cancer was noted (p<0.001; r= -0.51). The study demonstrated a statistically significant relationship between the occurrence of neoplasms in a patient’s family (p=0.009) or, more specifically, the occurrence of colorectal cancer (p=0.008), and the awareness of the prevention programme. The women’s group was characterised by statistically significantly greater awareness of colonoscopy as a screening examination (p=0.004). Conclusions. Patients need more information on colorectal cancer, its risk factors, prevention, the treatment process, and postoperative care. Lack of awareness of the colorectal cancer issue can be one of the major factors contributing to the high incidence of this disease.

Opis:
Objective. The aim of the study is to compare the hope of rural and urban inhabitants in the terminal stage of a neoplastic disease covered by stationary hospice care. Materials and method. The study was carried out among patients in the terminal phase of a neoplastic disease. They were patients of both 24-hour and daily palliative and hospice care units throughout the country. The study group consisted of 246 patients, average age – 59.5, the youngest respondent was 18 and the oldest – 90. The B. L. Block (NCN-36) test, prepared for people struggling with serious life-threatening diseases, was used. Comparison of the results with regard to the place of residence was based on the test Friedman ANOVA and Kendall compatibility factor. The general comparison of hope in individual dimensions and globally with the division into the degree of urbanization, was based on the Kruskal-Wallis test. Results. On the basis of factor analysis, 4 scales constructed from 8 items branch were distinguished. The following scales are used to study hope in the situational dimension – health, the telek-temporal dimension – goals, the spiritual dimension – religious beliefs and in the emotional-affective (affective) dimension – motivations. Conclusions. The strength of hope in people in the terminal phase of cancer, residing in villages, settlements, small, medium and large cities, was similar and depended on its magnitude. Of all the manifestations of hope, the greatest variation in results occurred in the subjects when they encountered serious problems and difficulties. The inhabitants of medium-sized cities were characterized by a higher hope at that time.

Although contemporary medicine is able to fight many diseases yet it is helpless in the face of human suffering including cancer. Both the number of people getting infected and death rates are rising. Sadly, the cases of malicius tumour are diagnosed frequently. Medical diagnosis stating “cancer” is always received with a shock, disbelief or even rebellion. It is necessary to emphasize the care for the patients who cannot be cured. The sickness always challenges the state of personal and family equillibrium. This pain often leads to a truthful revelation of a meaning of human existence, which goes far beyond our earthly matters. Death is strictly interfered with life and is inevitably involved in the life of every human being. It demands respect and responsibility, especially from those who are in charge od medical services. It is the hospice movement that helps people overcome their fears and pain in the last stage of cancer.
