Research about health loss shows that the way we experience critical moments and build a new identity are important in giving meaning to disease. It allow to integrate the experience of illness into the whole of life. The aim of the research was to analyze the narrative identity reflected in the life history and to explore the factors contributing to the differences in the formed narratives. A research question was: what narrative about one’s life does a person with an oncological disease create? The Polish adaptation of Dan McAdams’ life story interview was used. The study was conducted in group of four people with cancer remission. The research material was subjected to McAdams’ proposed sequence and consistency analysis. Elements of hermeneutic analysis were also used. The patients identity is coherent and mature, their narrative include the time from childhood to the present. Narratives differ in the degree of paying attention on the description of emotional experiences, the level of detail and the way of moving to the next stages of the story. Patient include the disease in their narratives, doing so in an individual way. However, it is possible to distinguish two ways of storytelling. One of them is the location of the disease in the broader background of the life situation, the other – recognizing the disease as the main moment in a given part of the story, constituting its title. It seems interesting to perform comparative research in a group of people, who are during diagnosis of cancer and relate them to the results of people in remission stage.

Background. Dietary patterns (DPs) are defined as the amounts, types and combinations of various food products in habitual diets and the frequency of their consumption. Dietary pattern analysis is usually performed in order to assess the combined effect of consumed food products on health. Objective. The aim of the study was to assess and compare the nutritional value of dietary patterns identified in a group of patients staying on the oncological ward. Material and methods. The study group consisted of 100 patients (51 women and 49 men) aged 19-83 years. Dietary intake was assessed using a food frequency questionnaire (FFQ) validated for the population of Lower Silesian Voivodeship. Results. Factor analysis identified two main dietary patterns explaining 25.6% of variance. The “fruit and vegetables” DP consisted of vegetables, fruits, juices, unrefined grains and nuts, seeds and raisins. Instead, the “Western” DP was characterized by the consumption of high-fat and processed meat and poultry, fried fish, refined grains, honey and sugar, fats, sweets, beverages and chips. While higher scores for “fruit and vegetables” pattern were associated with increased intake of dietary fiber, antioxidant vitamins, folic acid and decreased glycemic load per 1000 kcal and sodium intake, for “Western” pattern observed relationships were opposite. Women were more likely to have higher factor scores for “fruit and vegetables” DP and lower factor scores for “Western” DP than men. Conclusions. Dietary patterns identified in the study group differed in terms of nutritional value, in spite of similar macronutrient content in the diet. “Western” DP was characterized by lower nutritional value than “fruit and vegetables” dietary pattern.
Wprowadzenie. Wzory żywienia (ang. dietary patterns) definiowane są jako ilości, rodzaje i kombinacje produktów spożywczych i napojów zawartych w całodziennej diecie oraz częstotliwość, z jaką są one zwyczajowo spożywane. Analiza wzorów żywienia jest stosowana w celu określenia synergistycznego efektu oddziaływania różnorodnych składowych diety na zdrowie. Cel. Celem pracy było porównanie wzorów żywienia wyodrębnionych w grupie pacjentów przebywających na oddziale onkologicznym. Materiał i metody. Grupa badana liczyła 100 pacjentów (51 kobiet i 49 mężczyzn) w wieku 19-83 lata. Sposób żywienia oceniono na podstawie kwestionariusza częstotliwości spożycia żywności (FFQ) walidowanego dla populacji dolnośląskiej. Wyniki: Za pomocą analizy czynnikowej wyodrębniono dwa główne wzory żywienia wyjaśniające łącznie 25,6% wariancji. Wzór „owocowo-warzywny” charakteryzował się wysokim spożyciem warzyw, owoców, soków, nierafinowanych produktów zbożowych, orzechów i nasion, a wzór „zachodni” - tłustego i przetworzonego mięsa i drobiu, smażonych ryb, rafinowanych produktów zbożowych, miodu i cukru, tłuszczów, słodyczy, słodzonych napojów i frytek. Wysokie wartości czynnikowe wzoru „owocowo-warzywnego” były związane ze zwiększoną zawartością w diecie błonnika pokarmowego, witamin antyoksydacyjnych i kwasu foliowego, niższą zawartością sodu oraz niższym ładunkiem glikemicznym w przeliczeniu na 1000 kcal diety. Dla wzoru „zachodniego” zaobserwowane zależności były odwrotne. W grupie kobiet uczestniczących w badaniu wykazano wyższe wartości czynnikowe wzoru „owocowo-warzywnego” i niższe wartości czynnikowe wzoru „zachodniego” niż w grupie mężczyzn. Wnioski. Wzory żywienia wyodrębnione w badanej grupie chorych znacznie różniły się od siebie wartością odżywczą pomimo podobnej zawartości makroskładników diety. Wzór „zachodni” charakteryzował się niższą wartością odżywczą diety, niż wzór „owocowo-warzywny”.

Most patients diagnosed with gastrointestinal carcinomas are older people. The above-mentioned fact may lead to an erroneous finding that the problem does not concern patients aged between 20 and 30 years. Unfortunately, this assumption is often the reason for late diagnosis and delayed treatment of these malignancies. The study presented an example of three patients subject to surgical management of gastrointestinal carcinomas at the II Department of General and Gastroenterological Surgery, Medical University in Białystok.

Incidence of female breast cancer in our population remains very high and the incidence increase every year. Breast cancer and following demanding oncological treatment extensively influence life of female patients. It intervene biological, psychical and social life of patient. Objective of our research was to check how women are informed about preventive precautions and how they are following them. Our research sample were randomly selected women of all age groups. Our results showed that it is necessary to increase activity in the area of education, increase personal responsibility in area of breasts self-examination and better inform about the influence of obesity to breast cancer incidence. Considering our previous experience in health care we were surprised that the role of nurse in education process is very low. We think that general nurses have enough opportunities to use daily contacts with patients to inform and lecture them.

High-dose chemotherapy currently used in the treatment of children with cancer may induce late effects on psychosocial and cognitive functioning in some of them. The aim of the study was to evaluate psychosocial functioning of childhood cancer survivors living in rural regions. In total, 29 children entered the study. The children were diagnosed as having cancer between 1993-1995 in a single centre. Each patient was examined 5 years after the completion of cancer therapy. The children had been diagnosed with leukemia and lymphoma (72.4%), and solid tumours. Self-report questionnaires, as well as standardized psychological test (WISC-R, WAIS-R-PL) were performed for the evaluation of psychosocial and cognitive functioning of the patients. Most of the studied cancer survivors living in rural areas did not reveal diffi culties with cognitive and psychosocial functioning. In some patients, however, we observed more diffi culties in verbal tasks and existing major learning diffi culties. Cancer survivors living in rural areas in our region may be more unprivileged due to poor additional supporting services.

Introduction and objectives. The aim of the study was describe the factors determining the evaluation of the hospital environment, especially satisfaction with care and individual needs of cancer patients. Material and methods. The study comprised 80 women with endometrial cancer diagnosed and treated surgery in the Clinic of Gynaecology and Obstetrics in Rzeszow, Poland, between 2011–2012. The study used 3 questionnaires: the Goals Attainment Scaling (GAS) questionnaires, and questionnaires developed by the EORTC Quality of Life group, i.e. the QLQ C-30 (general module) and the In- PATSAT-32. Results. Respondents indicated 36 goals/expectations and the most common (over 50%) concerned the normal course of the post-operative period. The overall index of all goals which were met was 7.0 points. General quality of life reported by respondents before surgery was at a medium level (52.3+16.8%). Emotional functioning received the lowest scores (61.0+18.8%). Most respondents assessed manual skills of hospital doctors and nurses as the best in the In-PATSAT 32 scale i.e. 69.9±14.7% and 67.3±16.1%, respectively. The worst ratings concerned access to hospital from the outside (50.8±16.9%) and easy orientation inside the buildings (55.9±16.0%). Conclusions. Analysis of correlations between GAS and the In-PATSAT32 scales proved that they cannot be used interchangeably since they measure different aspects of a patient’s satisfaction with hospital care. For this reason, the application of idiographic and nomothetic tests among cancer patients is helpful for evaluation of the hospital environment.

Purpose: The purpose of the study was to examine the effect of using videos during enteral feeding training on the level of relevant knowledge and practice. Materials and methods: A randomized controlled quasi-experimental, post-test research design was used. We included 33 patients and relatives in the video group while 31 patients their relatives in the control group. In video group, the video recording was shown to the relatives in addition to verbal training however only verbal training was given in the control group. The relatives’ level of knowledge and practice score was evaluated. Results: Mean enteral feeding knowledge scores of patient relatives were found to be 76.7 ± 7.97 in the video group and 42.1 ± 12.57 in the control group. Mean enteral feeding practice scores were as follows: 19.64 ± 3.3 for the video group and 13 ± 3.63 for the control group. Conclusions: It was concluded that the enteral feeding training provided by using the video method increased the knowledge and practice levels of patient relatives. Implications for Practice: Video material prepared for the clinic will be a guide for clinical nurses and help them in their enteral feeding training and care planning.

Introduction: The primary health care services holding the central role in the prevention of various diseases. Cancer is the second leading cause of death worldwide. Primary healthcare services, holding a coordinating role in staging, treatment and prognosis of cancer. Purpose: To present the role of primary health care services in primary and secondary prevention of cancer. Materials and methods: A search was made in the PubMed and Scopus databases for reports on primary health care services in cancer patients. Results: Monitoring and early detection are some roles of care. The preventive medicine focuses on health promotion and disease prevention, helping people to enhance their own health. Specifically, majority of researches revealed that mammography ranged 44% to 69%. Also, the screening of cervical cancer by early examination of Pap smear, the percentage ranged 24% to 36%. The screening of prostate specific antigen (PSA) test ranged about 55%. The health professionalsfocused on individual, group and community interventions aimed at health promotion and prevention through programs and counseling. Conclusions: The interest of the fact that the primary care services have multidimensional roles for eliminating the occurrence of cancer. Prevention and health promotion programs, informing the population, campaigns on the part of health professionals for more frequent screening and early diagnosis in general symptoms of cancer, are fundamental principles of these services.

Oncoplastic and reconstructive techniques are essential tools in the armamentarium of contemporary breast surgeons. The aim of the study was to identify oncoplastic reconstructive patterns in breast cancer centers across Poland. A questionnaire of 18 questions was sent by email to the members of the Polish Society of Surgical Oncology and the Polish Society of Plastic, Reconstructive and Esthetic Surgery via their dedicated websites. The numbers of breast cancer patients operated on in each center ranged from 120 to 904 per year. Breast-conserving surgery (BCS) predominated in all but one center (range 50-70%). Immediate breast reconstructions (IBR) accounted for 6-42% of procedures, The most frequent type of IBR was either a two-stage expander followed by a permanent implant or one-stage implant- based with or without synthetic mesh. The most frequent type of delayed breast reconstruction (DBR) was a two-stage expander followed by implant-based reconstruction. None of the surveyed cancer centers performed free flap reconstruction. Deep inferior epigastric perforator (DIEP) flaps were performed in the plastic surgery department. Reconstructions based on pedicled flaps were performed in cancer centers. Acellular dermal matrices (ADM) and fat transfer were used in selected centers. In the clinical scenario of adjuvant radiotherapy, delayed breast reconstruction was favored. The full range of oncoplastic BCS was performed. Patient-reported outcome measures (PROM) and complications were assessed. Our findings can act as a platform for further improvement in skills, certification, data collection and audit, including patient reported expectation measures. There is also an urgent need to address pan-European inconsistencies in procedural reimbursement.

Introduction: Microvascular free tissue transfer enables the reconstruction of complex head and neck defects. The aim of the study was to assess the results of treatment of patients undergoing reconstructive surgery and to identify factors affecting these results, with particular reference to patient’s age. Materials and Methods: All patients who underwent free-flap head and neck reconstruction in our institution between 2010 and 2017 were included in this retrospective study. A series of 66 patients met the inclusion criteria and were divided into 2 age groups: group G1 aged <65 years (n = 41) and group G2 aged ≥65 years (n = 25). Minor local complications and general complications as well as comorbidities were analyzed. Results: No correlation was found between advanced age and the risk of free flap failure as well as the incidence of local minor complications. General complications were more frequent in the G2 group (32%) than in the G1 group (19.5%), although this is not a statistically significant difference. A statistically significant difference was found between the age and the patient’s health status according to ASA (P = 0.010). In the younger low-risk group, 12 patients (29.3%) had general and local complications, while in the older low-risk group only 1 (4%). General and local complications were found in 5 (12.2%) high-risk G1 patients and in 7 (28%) high-risk G2 patients. Conclusion(s): Patients with advanced head and neck malignant tumors should undergo reconstructive microsurgery regardless of age.

Background. It is recognised that both nutritional status and an improper diet have significant effects on weakening the outcomes of treatment in cancer patients. As a result, a lowered response to therapy and an increase in untoward side effects is often observed leading to a deteriorating quality of life. The role of an adequately balanced diet is thus regarded as being vital in supporting recovery. Objective. To assess the dietary consumption of calories, macro-elements and selected vitamins and minerals for subjects diagnosed with cancers of the breast, lungs and bones or soft tissue. Material and Methods. A survey was performed on 100 subjects diagnosed with various tumours between the September and December months of 2011 consisting of 34 with breast cancer, 33 lung cancer and 33 with bone or soft tissue cancer. The questionnaire was devised in-house, which included a three day dietary record. Results. The average daily calorific intake was found to be inadequate at 1608 kcal. In addition, abnormal proportions of energy derived from macro-elements was seen, where the contributions made by fats and proteins were somewhat high at respectively 35.1% and 16.5%, but too low in the case of carbohydrates at 52.1%. Up to 78% subjects had insufficient protein intakes, 88% showed deficiencies in consuming carbohydrates, as were 89% for fibre, 85% vitamin C, 99% calcium, 98% magnesium and 81% for iron. Conclusions. Many dietary shortcomings were observed in the studied subjects. There is therefore a need to educate persons suffering from cancer to adopt an adequate and balanced diet as means of providing vital support for treatment to be more effective.
Wprowadzenie. Nieprawidłowy sposób żywienia oraz stan odżywienia osób z chorobą nowotworową wpływa na słabszą odpowiedź na leczenie przeciwnowotworowe, może nasilać jego objawy uboczne oraz wpływać na jakość życia. Prawidłowo zbilansowana dieta pełni wspomagającą rolę w powrocie do zdrowia. Cel pracy. Celem badań była ocena spożycia energii, makroskładników oraz wybranych witamin i składników mineralnych u osób z diagnozowaną chorobą nowotworową piersi, płuc oraz kości i tkanek miękkich. Materiał i metody. Badanie przeprowadzono w okresie od września do grudnia 2011 roku. Badaną grupę stanowiło 100 osób ze zdiagnozowaną chorobą nowotworową (34 osoby z nowotworem piersi, 33 osoby z nowotworem płuc i 33 osoby z nowotworem kości i tkanek miękkich). Badania przeprowadzono przy użyciu autorskiej ankiety oraz metody 3-dniowego bieżącego notowania. Wyniki. Spożycie energii przez osoby z chorobą nowotworową było niewystarczające i średnio wynosiło 1608 kcal/dzień. Procentowy udział poszczególnych makroskładników w dostarczaniu energii był nieprawidłowy, w przypadku tłuszczu i białka był zbyt wysoki (odpowiednio 35,1% oraz 16,5%), a w przypadku węglowodanów zbyt niski (52,1%). Aż 78% badanych nie realizowało normy na białko, w przypadku węglowodanów wartość ta wynosiła 88%, a dla błonnika 89%. Spożycie witaminy C było zbyt niskie u 85% badanych, wapnia u 99%, magnezu u 89%, a żelaza u 81% badanych. Wnioski. W ocenianych racjach pokarmowych pacjentów obserwowano liczne nieprawidłowości. Istnieje potrzeba edukacji osób z chorobą nowotworową w zakresie zasad prawidłowego żywienia i znaczenia bilansowania diety.
