Opis:
When taking water from pre-mountain rivers, for transferring of large amounts of river sediments, rich in mineral fertilizers, along with water to crop fields through irrigation networks requires high sediment transport capacity and deformation resistance from irrigation networks. The projecting and construction of irrigation canals with these features in the foothills requires concreting the canal. The high content of river sediments in the Sokh River (5 kg∙m-3) and the low efficiency of the Right Bank Irrigation Reservoir (10-15%) require high hydraulic efficiency of water intake canals from this system. The main challenge is to reduce costs in concreted canals and ultimately ensure technical superiority. In the research were used generally accepted research methods in hydraulics, in particular field research and consequently, mathematical analysis. Kokandsay, Kartan and Bachkir irrigation canals were accepted as the object of research, the canals were designed on the basis of the best hydraulic section, the canal side slope was taken as a variable parameter and the technical and economic efficiency was checked using computer software. As a result, it was found that the consumption of concrete raw material for 1 running meter can save 0.2-0.3 m3, depending on the adoption of the canal side slope, the acceptance of the slope of the canal wall at values 1-1.5 will increase up to sedimentation 10%.

Objective: The aim of the study was to evaluate risk factors influencing the results of tympanoplasties on the base of material taken from the Department of Otolaryngology and Laryngological Oncology Collegium Medicum of the Nicolaus Copernicus University in Bydgoszcz between 2004-2009. In this period, 98 operations were performed. The time from operations to hearing examination was 3 to 7 years, mean 5,43. Tympanoplastic operations were divided according to Tos classification. Measuring hearing results, tonal audiometry was done and mean air bone gap on four frequencies was assessed (500, 1000, 2000, 3000 Hz), according to AAO-HNS guidelines (1995). This parameter was compared between groups separated according to risk factors, that could potentially affect the results. Those risk factors were: disfunction of the Eustachian tube, location and size of the perforation of the tympanic membrane, damage of the ossicles, the state of the mastoid process, the number of operations, the presence of the cholesteatoma or granulating tissue, chronic otitis media in the opposite ear, smoking cigarettes, mastoidectomy, canal wall down technique. The results were analyzed using statistical test. Results: The most important risk factor affecting treatment results (besides discharge from the ear) is damage of the ossicles, especially the malleus and stapes. A properly performed operation ensures good hearing results irrespectively of the presence of cholesteatoma or granulating tissue, and also in case of reoperation. For all types of tympanoplasties neither the location, nor the size of perforation influence the hearing results in long-term observation.

The passage through the Panama Canal has become the usual waterway for all the ships that can navigate through the Canal. The traffic through the canal is limited by the size of a ship. The need for the expansion of the Canal has emerged due to the development of the global trade and the shipping industry. The new dimensions of the lock-chambers determine the size of the ships as well. The new generation of ships built to the largest specifications possible to transit the current locks of the canal are called the Post-Panamax vessels. The maximum dimensions of these ships are 366 meters in length, 49 meters in beam and 15.2 metres in draught. The paper analyses savings in the operational costs on three types of the Post-Panamax vessels after the Canal expansion.

Introduction: Surgical treatment of conductive hearing loss runs the risk of damage to the inner ear in the mechanism of acoustic trauma. Aim: The aim of this study was to evaluate the function of the organ of Corti, expressed as bone conduction threshold at the frequency of 4000 Hz for selected operations: mastoidectomy and canal-wall-down procedure. Material and methods: The material was collected from patients with chronic otitis media in the Department of Otolaryngology and ENT Oncology, Collegium Medicum of Nicolaus Copernicus University in Bydgoszcz in 2004–2009. All patients were examined with pure tone audiometry threshold before surgery and at least three years after surgery. The analyzed group of patients was divided into subgroups depending on the type of operation according to To classification and procedures for resection: mastoidectomy and canal-wall-down procedure. The results were statistically analyzed. Results: In the analyzed period of three years after surgery there was no statistically significant difference between groups, although there were higher values for tympanoplasty type 1 with mastoidectomy compared with tympanoplasty type 1 without mastoidectomy - respectively 25.67 dB and 18.53 dB. In the study, there was no statistically significant the difference in bone conduction threshold for frequency 4000 Hz within the type 2 tympanoplasty according to Tosa comparing canal wall-up and canal-wall-down procedure. Conclusions: Mastoidectomy or canal-wall-down procedure do not affect the bone conduction threshold for a frequency of 4000 Hz after tympanoplasty in long-term observation.

Opis:
External auditory canal neoplasms comprise less than 1 percent of all head and neck tumors. They are ceruminous gland-originated tumors in 2.4 percent of cases. In the past, all these tumors were called ceruminomas. In 1972 Wetli et al. classified these neoplasms into four groups: ceruminous adenomas, ceruminous carcinoma, adenoid cystic carcinomas, and pleomorphic adenomas. Ceruminous adenocarcinoma is very rare. It has no specific symptoms, therefore, an appropriate, quick diagnosis is difficult; it is very aggressive. Treatment should be based on tumor resection and followed by radiotherapy. We presented a case of a 56-year-old female with a lump in the external ear canal, who underwent diagnostic surgical excision of the tumor in the external auditory canal. The postoperative histopathological diagnosis was ceruminous adenocarcinoma (complete excision) and the patient received radiation therapy. Follow-up CT and MR – performed one year after surgery - showed a possible local recurrence of pathology. The patient underwent maximal surgical resection with subtotal petrosectomy but postoperative histological examination showed no malignant cells. Treatment of external auditory canal neoplasms should be based on radical surgical resection. Diagnostic imaging is very important in follow up examination but it can have limited value in terms of its specificity.

Opis:
Aims. Presenting our clinical experience with the postauricular island flap (pif) and estimation of the results following partial external auditory canal (eac) and/or auricular conchal bowl reconstructions with pif in patients after carcinoma resections. Methods. We have analyzed postoperative results of 19 patients after auricular conchal bowl (11), or auricular conchal bowl and eac (8) reconstructions with pif, following malignant tumor resections, between 2000-2015. The patients were reexamined and evaluated in respect of early and long-term results after surgical treatment considering both the plastic surgeon’s and the patient’s opinion. Results. The cancers were completely excised in all patients, and there were no recurrences within at least 2 years of follow-up. The observed complications after reconstructions comprised venous congestion in five cases (26.3%), pinning of the operated ear in four patients (21%), prominent earlobe in three (15.8%), and eac constriction in three cases (15.8%). The postoperative result was very good in all cases (both in the opinion of the plastic surgeon and the patients), except for patients with pinning of the operated ear, with a prominent earlobe (moderately satisfied). Conclusions. 1. Combined operations involving postauricular island flap reconstructions after partial (external auditory meatus and/or auricular conchal bowl) resections allowed for complete removal of malignant tumors with no evidence of recurrence, and for the preservation of a proper conchal shape in the reconstructed ear. 2. A retroauricular approach in cases with cancer involvement of the external auditory meatus allowed for proper visualization and estimation of lesions extent, as well as adequate surgical access.

Opis:
Background: Breaking an endodontic instrument is a complication that really hinders or even makes impossible complete disinfection and tight obturation of the root canal system. Case presentation: A 74-year-old patient suffered a fracture of the clinical crown of the second incisor in the maxilla. The examination revealed necrotic pulp and the presence of obliteration significantly limiting the lumen of the tooth canal. It was decided to perform root canal treatment and then to do a reconstruction reinforced by crown-root inlay. An endodontic instrument was broken in the lumen of the root canal at the beginning of the endodontic treatment. A broken piece tightly filled nearly 80% of the total length of the canal. It was decided to use endodontic forceps to remove the fragment. Conclusion: Choosing the right method is a key step in the complication described above. The management technique should entail the least risk of iatrogenic complications because they can greatly affect the long-term maintenance of the tooth. The article discuss on the basis of current literature standards of conduction in the case of tool breakage in the root canal system and presents the problem on the basis of own experience.

Opis:
About 94.8 per cent of Malaysian trade is depending on collective inter/intra-regional maritime networks. Straits of Malacca is an important strait and the only strait that connecting Indian Ocean and South China Sea which contributes to the nation's economic growth significantly. This strait plays a key character to enable the trade connection between east and west of the globe. In average, about 63,000 different types of vessels passing through the strait per year and the number of the vessels keep accelerating every year. Conversely, the introduction of Thai Canal to detour the trade voyage between Indian Ocean to South China Sea is expecting to reduce about three to four days journey time between these two passages. Although the development of this canal is predicted to provide substantial benefits to shipping lines, the grey area or disadvantages change to implications of this canal need to be explored especially on Malaysian trade and infrastructure. Therefore, a qualitative approach has been employed to achieve the aim of this paper. Face-to-face interviews with experts from various agencies had been conducted to gather information on the impact of Thai Canal on Malaysian trade and infrastructure. The outcome of this paper indicates that the implication of Thai Canal reduces the productivity of Malaysian seaports, transformation of seaports in northern region as a major hub of the nation, enforcing equal regional development between west and east coast Malaysia and administering dramatic progress on multimodalism in each state. The outcome of this research is important as an input for the preparation of Malaysian seaport system due to any significant changes in the trade route. Although, the development of Thai Canal reduce the overdependence on Malacca Straits, the impact of this this new canal will affects the trade performance of this region. Hence, this paper is significant to understand the implications that might occur to this nation due to dynamism of maritime logistics.

In the paper the basic questions connected with modeling of wastewater networks are presented. Methods of modeling basic sewage parameters and appropriate calculation algorithms are described. The problem concerns the gravitational networks divided by nodes into branches and sectors. The nodes are the points of connection of several network segments or branches or the points of changing network parameters as well as of location of sewage inflows to the network. The presented algorithms for networks hydraulic calculation concern sanitary or combined sewage nets. It is assumed that the segments parameters such as shape, canal dimension, bottom slope or roughness are constant. Because of these assumptions all relations considered concern the steady state conditions for the network. The calculation of flow velocities and the filling heights in the segments of the wastewater net are carried out for the known slopes and diameters of the canals.
