The aim of the study was to answer the question whether or not determination of C-reactive protein in patients after serious abdominal surgeries can be prognostic of septic complications.Material and methods. 36 patients who underwent elective surgeries were included in the study. The patients were included either in the group where no postoperative SIRS developed or in the group where postoperative SIRS did occur. In the seven-day period after the surgery, in 26 patients SIRS was found, and in 10 - sepsis was suspected (according to the ACCP/SCCM definitions). In patients who underwent abdominal surgeries blood concentration of C-reactive protein was determined prior to the surgery (measurement '0'), and then on postoperative days 1, 2, 3, 5 and 7.Results. The test for two variables (C-reactive protein on postoperative days five and seven) showed statistically significant difference, and for one variable (C-reactive protein on day three) - difference at the limit of significance. Thus, it was found that in the postoperative SIRS group the level of C-reactive protein is higher than in the non-SIRS group.Conclusions. Serial measurements of C-reactive protein are useful in the first week after surgery, as they can be prognostic of postoperative septic complications. Such complications can be anticipated if CRP on postoperative day 5 is higher than 1/2 of the maximum CRP concentration on day 2 or day 3, or CRP > 150 mg/L as of postoperative day 3. Unfortunately, the severity of the disease cannot be projected based on C-reactive protein level.

Introduction and aim. COVID-19 causes an uncontrolled and generalized inflammatory response of the host immune system. Early recognition of the disease and early prediction of the clinical course are of great importance. The aim of this study was to evaluate the predictive role of the C-reactive protein/albumin ratio (CAR) and the neutrophil/lymphocyte ratio (NLR) for mortality in patients hospitalized with the diagnosis of COVID-19. Material and methods. The patients, who were hospitalized for COVID-19 and whose CRP, albumin, neutrophil, and lymphocyte levels were documented within the first 24 hours after admission, were analyzed retrospectively. Patients were divided into survivors and non-survivors; the groups were compared. Univariate and multivariate Cox regression models were developed to evaluate the CAR and the NLR as risk factors for mortality in COVID-19 patients. Results. One hundred and thirty patients were included in this study. The mean age of the survivor group (n=114) was 60±16 and 52% were male. The mean age of the non-survivor group (n=16) was 75±13 and 56% were male. In the non-survivor group, the CAR detected at the time of admission to the hospital was significantly higher compared to patients in the survivor group (p=0.026). Conclusion. As a result, the CAR, the NLR and LDH are independent risk factor indicators of mortality in hospitalized patients.

Opis:
Open abdomen technique is a surgical treatment in which the fascia and skin are left open in order to reduce the value of the intra-abdominal pressure. According to the World Society of the Abdominal Compartment Syndrome (WSACS) normal values of the intra-abdominal pressure are between 5 and 7 mm Hg. Intra-abdominal hypertension occurs when the pressure value is equal to or exceeds 12 mm Hg. The aim of the study was to compare the results of the open abdomen treatment using standard methods and negative pressure wound therapy. Material and methods. The study was in the form of a retrospective analysis of the documentation of the patients treated with open abdomen technique. The study included 37 patients treated in the Department of General and Endocrine Surgery and Gastroenterological Oncology and in the Department of Anesthesiology and Intensive Care of the Medical Sciences since 2009-2012. Patients were divided into two groups: group 1 (n = 20) was treated with standard surgical procedures (laparostomy, repeated peritoneal cavity lavage) and group 2 (n =17) was treated using negative pressure wound therapy (NPWT). The analysed clinical data included the period of hospitalization and clinical outcome (survival vs death), the occurrence of enteroatmospheric fistulae, cyclical determination of the quantitative C-reactive protein levels. Results. The number of deaths during hospitalization in the group treated with NPWT was lower than in the group treated with standard methods (3 vs 9). The number of fistulae during hospitalization in the group treated with NPWT dropped as compared to the group treated using standard procedures (18% vs 70%). The decrease in the CRP levels was recorded in the group treated with NPWT and its increase - in the group treated with standard methods. Conclusions. The use of NPWT in patients requiring open abdomen treatment is reasonable due to the positive results with respect to survival rates and the decrease in the number of gastrointestinal fistulae. It is necessary to train the physicians in using this type of therapy in the form of workshops and in the clinical setting.

Hospital length of stay (LOS) and readmissions continue to be expensive and unexpected events following colorectal surgery (CRS) whether patients follow enhanced recovery pathways or traditional care. Predictors of these adverse events could facilitate identification and optimization of CRS patients. The aim of the study was to examine the impact of white blood cell count (WBC) and C-reactive protein (CRP) levels as predictors of delayed recovery or hospital readmission following CRS. Material and methods. Patients undergoing laparoscopic or open abdominal colorectal surgery by a single surgeon were managed using standardized enhanced recovery pathways. Those with postoperative day 2 CRP and white blood cell values were evaluated. Outcomes included 30-day hospital readmission rates and postoperative length of hospital stay. Results. CRP values were available for 193 patients (86 Male, mean age 58.6 years). Ninety-nine patients had surgery for colon cancer, 23 for Crohn’s disease, 19 for ulcerative colitis, 31 for diverticulitis and 18 for other reasons. Twenty patients (10.4%) were readmitted to the hospital within 30 days of surgery. POD2 CRP accurately predicted short length of hospital stay (p< 0.01). Average CRP was 6.3 in the LOS of < 3 days or less, and 11.7 in patients with LOS >4 days. The mean CRP of the readmission and non-readmission groups was 11.8 and 9.9, respectively (p=0.29). The average POD 2 WBC of the readmission and non-readmission groups was 10.6 and 9 respectively (p=0.01). Conclusion. A low POD2 CRP level was correlated with a shorter LOS, but it did not predict readmission. Conversely, POD2 WBC, and the difference in WBC from baseline were associated with readmission. These markers may be useful indicators to predict suitability of early discharge in an ERP. Further evaluation in prospective trials is warranted.

INTRODUCTION: Chronic Obstructive Pulmonary Disease (COPD) is a progressive disease that develops due to inflammation in the airways. The aim of this study is to demonstrate the effectiveness of c-reactive protein/albumin ratio (CAR) as a 30-day mortality indicator in COPD patients admitted to ICU. MATERIAL AND METHODS: A total of 235 COPD patients with available data between January 2018 and December 2018 were included in this retrospective cohort study. Demographics, APACHE II, Charlson comorbidity index (CCI), SOFA score, CAR and outcomes were evaluated. RESULTS: 87 (37%) of the cases were female and 148 (63%) were male. Their ages ranged from 26 to 95 years, with an average of 70.9± 11.4. The non-survivors had significantly higher APACHE II, CCI, SOFA score, procalcitonin, creatinine, mechanical ventilation (MV) time, WBC, CRP and CAR compared to the survivors (p˂0.05). Albumin and prealbumin were significantly lower (p˂0.05) in non-survivors. In the univariate model; age, sepsis, inotropic support, APACHE II score, CCI, SOFA, procalcitonin, creatinine, MV time, WBC, CRP, albumin, prealbumin and CAR were observed to be significantly effective (p˂0.05) in predicting 30-day mortality. In the multivariate reduced model; inotropic support, SOFA, WBC and prealbumin value exhibited significant independent (p<0.05) effectiveness in predicting 30-day mortality. Albumin, CRP, CAR, APACHE II, SOFA and CCI value were observed to be significant in predicting mortality (p=0.000). CONCLUSIONS: In the study, the predictive power of APACHE II score, CCI, SOFA score, albumin and prealbumin values alone was found to be significantly higher than that of the CAR.

Background. There is an increasing interest in the role of adipocytokines in cardiovascular pathophysiology. Aim. The aim of the study was to compare visfatin levels, a novel adipokine, in patients with heart failure (HF) due to the left ventricular systolic dysfunction with those in age- and body mass index (BMI) - matched healthy controls in relation to the parameters of glucose metabolism and high sensitivity C-reactive protein (hsCRP) levels. Material/Subjects and Methods. The study population consisted of 28 males with systolic HF referred for cardiopulmonary exercise testing, divided into two subgroups based on their NYHA class (HF patients NYHAI+II, n=17, and HF patients NYHAIII+IV, n=11), and 23 controls. The following indices were measured in a serum samples: visfatin, hsCRP, glucose and lipid metabolism parameters, and the insulin resistance index HOMAIR (homeostasis model assessment insulin resistance) was calculated. Results. Concentrations of visfatin and high-density lipoprotein cholesterol (HDL-cholesterol) in the HF subjects were significantly lower (p≤0.01) than in controls. The Kruskal-Wallis test showed significant differences between three groups (controls and both subgroups of heart failure patients) in mean levels of visfatin, hsCRP, glucose, HOMAIR and HDL-cholesterol. Conclusion. Serum visfatin concentrations in patients with systolic HF, particularly with more advanced NYHA classes, are significantly lower in comparison to healthy controls and are independent of age or anthropometric and metabolic parameters.

Despite the emerging evidence suggesting a proatherogenic role of C-reactive protein (CRP) in atherosclerosis, the contribution of CRP in pathogenesis of atherosclerosis and atherothrombosis has not been unequivocally defined. The role of CRP in pathophysiology/pathology seems to largely depend on its structure. Two CRP isoforms, the native pentameric and the modified monomeric one, differ substantially in their physiological functions, which is thought to origin from the considerable structural heterogeneity of the CRP molecule. The present review provides an overview of the experimental evidence with relevance to the clinical role(s) of various CRP isoforms. The biological role of the protein, its structure and distribution are discussed with particular emphasis on the diverse properties of the pentameric and monomeric forms of CRP. Some methodological aspects, related to experimental models and techniques of CRP preparation, are also critically reviewed.

Introduction: Neurogenic bladder (NB) most often is caused by meningomyelocele (MMC) and manifests with various lower urinary tract dysfunctions. Condition of NB is worsened by inflammatory process or oxidative status imbalance. Purpose: To estimate of urinary uric acid (UA), hs-CRP, thiol status in association with NB function in MMC patients.Materials/Methods: 33 MMC children and 20 healthy individuals were included into the study. The first daytime urine samples were collected from all examined participants and urinary thiol status, hs CRP and UA were measured.Results: MMC children presented higher urinary UA level. The median hs-CRP level were also higher in MMC patients compared to the reference. Thiol status were lower in MMC individuals compared to reference group. We found positive correlation between serum creatinine, serum UA and urine creatinine and negative between serum creatinine and GFR. Correlations between urinary UA and physical development parameters, renal function, hsCRP, thiol status and urodynamic findings in MMC and reference groups were found.Conclusions: UA is a marker potentially having direct effect on the bladder function. Disturbed oxidative status and increased markers of inflammation may be a potentially modifiable factors affecting function of lower urinary tract in MMC children.

Background: This study aims to examine the factors involved in anastomotic leak (AL) following low anterior resection and total mesorectal excision (LAR-TME) and to determine the usefulness of early measurement of the inflammatory biomarkers C-Reactive Protein (CRP) and Procalcitonin (PCT). Methods: One hundred patients undergoing LAR-TME with proximal diverting stoma were analysed between 2013 and 2016. Postoperative CRP and PCT levels were measured on the 3rd and 6th postoperative days. Results: There were 11 clinical leaks with a negative impact in univariate analysis on AL of male gender, larger and stenotic tumours, intraoperative blood loss > 200 mL, need for perioperative blood transfusion, postoperative anaemia and operating time exceeding 180 minutes. Upon multivariate analysis, only perioperative blood transfusion was an independent AL risk factor. Recorded CRP was higher in AL patients when compared with non-AL cases on both the 3rd postoperative day (152.4 mg/L vs 93 mg/L, respectively; P < 0.0001) and the 6th postoperative day (130.5 mg/L vs 68.2 mg/L; P < 0.0001). PCT levels also significantly differed between AL and non-AL cases on the 3rd postoperative day (0.5 ng/mL vs 0.2 ng/mL, respectively; P < 0.0001) and the 6th postoperative day (1.16 ng/mL vs 0.1 ng/mL, respectively; P < 0.0001). Both CRP and PCT showed high negative predictive values (NPV) for the diagnosis of AL on both postoperative days. Conclusion: Following low restorative proctectomy, high NPV of CRP and PCT measurements for the diagnosis of anastomotic leaks may assist decision-making for early hospital discharge.

Myeloperoxidase (MPO) and C-reactive protein (CRP) may play critical roles in generation of oxidative stress and the development of the systemic inflammatory response. The aim of the study was to determine the effect of atorvastatin therapy on the MPO gene expression and its plasma level in relation to lipids level lowering and an anti-inflammatory response in patients after acute myocardial infarction. The research material was represented by 112 samples. Thirty-eight patients with first AMI receiving atorvastatin therapy (40 mg/day) and followed up for one month were involved in the study. The relative MPO gene expression in peripheral blood mononuclear cells (PBMCs) was examined using RT-qPCR in 38 patients before-, 38 patients after-therapy and in 36 patients as the control group. The plasma concentrations of MPO and serum concentrations of biochemical parameters were determined using commercially available diagnostic tests. After one month of atorvastatin therapy, in 60.5% patients a decrease of MPO gene expression, whereas in 39.5% patients an increase, was observed. The plasma MPO levels behaved in the same way as the MPO gene expression. However, the serum lipids and CRP concentrations were significantly lower after one month of atorvastatin therapy in both groups of patients - with decreased and increased MPO gene expression. Atorvastatin exhibited a different effect on MPO gene expression and its plasma level. Short-term atorvastatin therapy resulted in lipid lowering and anti-inflammatory activity in patients after AMI, independently of its effect on MPO gene expression. The molecular mechanisms of this phenomenon are not yet defined and require further research.

Aim: The aim of this study is to examine uric asid (UA) and high sensitive C-Reactive protein (Hs-CRP) levels in patients with coronary artery ectasia (CAE). Materials and Methods: Ninety-eight patients with isolated CAE (mean age 57.5±10.3), (group-I), 110 patients with CAD but without CAE (mean age 56.3±10.7), (group-II), and 105 patients with normal coronary angiographies (mean age 58.1±10.8), (group-III) were included in the study. Blood samples of all individual were taken after coronary angiography from an antecubital vein, the patients uric acid and Hs-CRP levels were assessed. The severity of ectasia was evaluated and categorized according to Markis. Results: A significant difference was not seen in serum uric acid and Hs-CRP levels between CAE and CAD groups. However, relative to the control group, uric acid and Hs-CRP levels in CAE and CAD groups were higher to a significant degree (p=0.001, p<0.001, respectively), (p<0.001, p<0.001, respectively). The statistical significant was detected between subgroups type I and type IV, Hs-CRP and UA were statistically high in subgroup type I. (p=0.012, p=0.033, respectively) In multiple regression analysis, CAE and CAD were independently associated with UA (β=0.76; p<0.001, β=0.68; p<0.001, respectively) and Hs-CRP (β=0.66; p<0.01, β=0.62; p<0.01, respectively) along with diabetes mellitus (β=0.61; p=0.039, β=0.94; p=0.028, respectively). Conclusion: In conclusion, the blood uric acid and Hs-CRP values in our study have been observed to be higher in the individuals with coronary arteri ectasia in comparison to normal individuals, and the increase in these values were found to be parallel to the extent of the ectasia.

Background. Chronic obstructive pulmonary disease (COPD) is an airway inflammatory disease caused by inhalation of toxic particles, mainly cigarette smoking, and now is accepted as a disease associated with systemic characteristics. Objective. The aim of this work was to investigate and compare selected biochemical parameters in patients with and without COPD. Material and Methods. Observation group consisted of clinically stable patients with COPD (n = 60). The control group was healthy persons from the general population, without COPD, who were divided into two subgroups – smokers (n = 30) and non-smokers (n = 30). Laboratory parameters were investigated by automated clinical chemistry analyzer LISA 200th. Results. Albumin in our measurements showed an average value of 39.55 g.l-1 in the patient population; 38.89 g.l-1 in smokers and in non-smokers group 44.65 g.l-1. The average value of pre-albumin in the group of patients was 0.28 ± 0.28 g.l-1 and 0.30 ± 0.04 g.l-1 in smokers group. The average value of the orosomucoid in patients was about 1.11 ± 0.90 mg.ml-1. In the group of smokers, the mean value of orosomucoid was 0.60 ± 0.13 mg.ml-1. The level of C-reactive protein (CRP) in the patient group reached an average value of 15.31 ± 22.04 mg.l-1, in the group of smokers was 5.18 ± 4.58 mg. l-1. Prognostic inflammatory and nutritional index (PINI) in the group of patients showed a mean value of 4.65 ± 10.77 and 0.026 ± 0.025 in smokers. Conclusions. The results of this work show, that the values of index PINI in COPD patients are significantly higher than in smokers (P <0.001). This along with other monitored parameters indicative inflammation as well as a catabolic process that occurs in the organism of patients with COPD.
Wprowadzenie. Przewlekła obturacyjna choroba płuc (POChP) jest chorobą zapalną dróg oddechowych spowodowaną inhalacją toksycznych związków chemicznych, pochodzących głównie z palenia papierosów, która charakteryzowana jest jako choroba ogólnoustrojowa. Cel. Celem niniejszej pracy było oznaczenie we krwi chorych na POChP wybranych parametrów biochemicznych stanu zapalnego oraz porównanie ich stężenia w grupie zdrowych ludzi bez POChP.0 Materiał i metody. Grupa badana składała się z klinicznie stabilnych pacjentów z POChP (n = 60). Grupę kontrolną stanowili zdrowi pacjenci z populacji generalnej, bez POChP, którzy zostali podzieleni na dwie podgrupy – palaczy papierosów (n = 30) i niepalących (n = 30). Parametry biochemiczne oznaczano z wykorzystaniem automatycznego analizatora LISA 200. Wyniki. Średnie stężenie albumin w grupie chorych wynosiło 39.55 g.l-1 ; w grupie zdrowych palących 38.89 g.l-1 a w grupie zdrowych niepalących 44.65 g.l-1. Średnie stężenie pre-albuminy w poszczególnych grupach pacjentów i zdrowych palących wynosiło odpowiednio 0.28 ± 0.28 g.l-1 oraz 0.30 ± 0.04 g.l-1. Stężenie orosomukoidu w grupie chorych wynosiło 1.11 ± 0.90 mg.ml-1. W grupie palących zdrowych osób średnio 0.60 ± 0.13 mg.ml-1. Stężenie białka Creaktywnego (CRP) w grupie chorych na POChP wynosiło 15.31 ± 22.04 mg.l-1, a w grupie palących 5.18 ± 4.58 mg. l-1. Prognostyczny wskaźnik odżywienia i stanu zapalnego - PINI (Prognostic Inflammatory and Nutritional Index) w grupie pacjentów wynosił średnio 4.65 ± 10.77 a w grupie osób palących 0.026 ± 0.025. Wnioski: Wyniki niniejszych badań wykazały, że wartości PINI u pacjentów z POChP były statystycznie wyższe (p <0,001) niż w grupie osób palących. Uzyskane wyniki wraz z innymi monitorowanymi parametrami świadczą o stanie zapalnym jak również o procesie katabolicznym, który występuje w organizmie pacjentów z POChP.

This case study applied the Dahn Taekwondo Spondylitis Improvement Program (DTSIP) to a 30-year-old male with ankylosing spondylitis five times a week for 28 weeks. Materials and Methods: A 32-year-old male with ankylosing spondylitis underwent the Dahn Taekwondo Spondylitis Improvement Program (DTSIP), an amalgamated program of standard Western medicine practices and complementary and alternative medicine (CAM) exercises based in the oriental medicine practices of yoga and taekwondo. The subject performed the DTSIP 5 times a week for 90 minutes for 28 weeks. Results: First, blood C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and platelet levels decreased by approximately 29-71%. Second, blood leukocyte counts decreased by about 11%. Third, as a result of Bath Ankylosing Spondylitis Metrology Index (BASMI) measurement, tragus to wall (TWD), lumbar side flexion (LSF), LF (lumbar flexion), and intermalleolar distance (IMD) were improved substantially. The students' cervical vertebrae and thoracic and lumbar vertebrae ranges of motion were additionally improved. The range of motion of the shoulder and hip joint improved. Conclusions: Therefore, the DTSIP seems to have a positive effect on the inflammation, immune functions, and spine and joint range of motion in the subject of this case study. Future studies are required with a wider sample population to validate these results.

Introduction. An important way of preventing type 2diabetes is by adopting a proper diet by which means appropriate control over blood glycaemia and lipids can be achieved. Objectives. To assess selected biochemical and haematological markers in overweight subjects or those suffering from type 2 diabetes in relation to their estimated dietary intake. Material and methods. The study was conducted in 2012 on n=86 overweight or obese subjects living in Warsaw or its environs, of whom n=43 had type 2 diabetes. Dietary intakes were compared between non-diabetics (control group) and diabetics (test group) by 3 day records, whilst the relevant blood biochemistry and haematology results were obtained from medical records; with patient consent. Results. Diabetic subjects had significantly higher serum glucose and CRP levels than controls, respectively; 190 vs 98 mg/ dl and 1.4 vs 1.1 mg/dl. Lipid profiles were however more significantly abnormal in controls, compared to diabetics with respectively; total cholesterol 220 vs 194 mg/dl, LDL-cholesterol 131 vs 107 mg/dl and triglycerides 206 vs 157 mg/dl. There were no significant differences in HDL-cholesterol; respectively 55 vs 51 mg/dl. In the diabetics, calorific intakes from carbohydrates, especially sugars, were significantly lower than controls i.e. 9% vs 13%. The proportional share of calories derived from dietary fats did not differ between groups, nevertheless a positive correlation was observed between dietary fat content with blood cholesterol concentrations in diabetics. Conclusions. Disorders of carbohydrate metabolism were confirmed in both overweight and diabetic (type 2) subjects. In addition, both groups demonstrated untoward lipid profiles that correlated with their improper nutrition.
Wprowadzenie. Prawidłowe żywienie, którego celem jest wyrównanie glikemii oraz profilu lipidowego odgrywa zasadniczą rolę w profilaktyce cukrzycy typu 2. Cel pracy. Ocena wybranych wskaźników biochemicznych krwi (glukoza, lipidogram, białko CRP) u osób z nadmierną masą ciała oraz cukrzycą typu 2 w aspekcie ich sposobu żywienia. Materiał i metody. Badanie przeprowadzono w 2012 roku, wśród 86 mieszkańców Warszawy i okolic, z nadwaga i otyłością, w tym u 43 osób z cukrzycą. Do oceny sposobu żywienia wykorzystano metodę trzydniowego bieżącego notowania. Dane dotyczące wskaźników biochemicznych krwi, za zgodą badanych uzyskano z ich kart zdrowia. Wyniki. U pacjentów z cukrzycą odnotowano istotnie wyższe średnie stężenie glukozy w surowicy krwi (190 vs. 98 mg/dl), jak również wyższe stężenie białka CRP (1,4 vs. 1,1 mg/dl). Biorąc pod uwagę wskaźniki gospodarki lipidowej stwierdzono większe nieprawidłowości w grupie kontrolnej (cholesterol ogółem 220 vs. 194 mg/dl; cholesterol LDL 131 vs. 107 mg/dl; triacyloglicerole 206 vs. 157 mg/dl). Stężenie cholesterolu frakcji HDL nie różniło się istotnie w obydwu grupach (55 vs. 51 mg/dl). Spożycie energii z węglowodanów, zwłaszcza prostych u chorych na cukrzycę było istotnie statystycznie niższe niż w grupie kontrolnej (9 vs. 13%). Procentowy udział tłuszczu w dostarczeniu energii nie różnił się znacząco pomiędzy grupami, przy czym w grupie z cukrzycą odnotowano dodatnią korelację pomiędzy ilością tłuszczu w diecie, a stężeniem cholesterolu we krwi. Wnioski. Uzyskane wyniki potwierdzają występowanie zaburzeń gospodarki węglowodanowej u pacjentów z nadmierną masą ciała oraz cukrzycą typu 2. Ponadto w badanych grupach zaobserwowano niekorzystny profil lipidowy korelujący z ich nieprawidłowym sposobem żywienia.
