W artykule przedstawiono obowiązujące w Polsce normy prawne dotyczące budowy i użytkowania analizatorów wydechu. Wskazano na ograniczenia techniczne związane z dowodowym zastosowaniem analizatorów. Wypunktowano wymagania metrologiczne i użytkowe alkomatów przewidziane prawem. Opisano przykład rozwiązania konstrukcyjnego wraz z wynikami pomiarów prototypu. W pracy zawarto koncepcję zmian legislacyjnych pozwalających na budowę analizatorów o zastosowaniu masowym.
The article presents the regulations binding in Poland concerning the construction and use of breath analyzers. The technical limitations associated with the use of evidence analyzers are pointed. We classify the metrological requirements and functional breathalyzers required by law. Here is described an example of a design solution, along with the results of measurements of the prototype. This paper contains the concept of legislative changes to allow for the construction of analyzers to mass scale use.

Introduction The study aimed at measuring exhaled breath malondialdehyde (EBC-MDA) in workers exposed to dust containing silica and at its comparison with the non-exposed control group. Material and methods The cross sectional, case-control study (N = 50) was performed in a tile and ceramics production factory in Yazd, Iran. EBC-MDA was quantified in exhaled breath of the participants by a lab made breath sampler. Exposure intensity was measured according to the NIOSH 0600 method in selected homogeneous exposure groups. Additionally, spirometry test was conducted to investigate a correlation between EBC-MDA and spirometric findings in the exposed workers. Results There was no difference in the observed exposure intensities of silica containing dust in different units. However, “coating preparation” was the unit with the highest concentration of dust. Although, the level of EBC-MDA in the cases was slightly higher than in the controls, the difference was not statistically significant (U = 252, p = 0.464). A significant and positive correlation was found between dust exposure intensity in working units and the measured EBC-MDA of workers (r = 0.467, N = 25, p = 0.027). There were also no statistically significant differences among job categories in the exposed group for the values of FEV1% (F(3, 44) = 0.656, p = 0.584), FVC% (F(3, 44) = 1.417, p = 0.172), and FEV1/FVC% (F(3, 44) = 1.929, p = 0.139). Conclusions The results showed a significant correlation between respirable dust exposure intensity and the level of EBC-MDA of the exposed subjects. However, our results did not show a significant correlation between lung function decreases and EBC-MDA.

Background & Aims: Green tea is known worldwide for its high content of polyphenolic compounds and multifactorial beneficial effects on human health. The role of green tea as an inhibitor of lipid hydrolysis is widely discussed. The aim of the study was to assess the influence of green tea extract on lipid digestion and absorption. Methods: The study comprised 32 healthy volunteers aged 23 to 30 years with normal exocrine pancreatic function. In all subjects 13C-labelled mixed triglyceride breath test was performed twice with and without green tea extract ingestion. Cumulative percentage dose recovery was considered to reflect digestion and absorption of lipids. Values are expressed as medians and 1st-3rd quartile distribution. Results: In all subjects, cumulative percentage dose recovery values were normal in a placebo test (36.8% <30.1-43.3%>). These results were significantly higher (p=0.021) than those obtained in green tea extract test (28.8% <23.1-37.2%>). Results of six tests with GTE were abnormal. Conclusions: Single dose of green tea extract taken with a test meal decreases lipid digestion and absorption in humans.

The paper presents an analysis and practical study of the temperature and pressure influence on a non-dispersive infrared (NDIR) sensor for measuring the concentration of carbon dioxide in human breath. This sensor is used for monitoring patients’ carbon dioxide (CO2) in the exhaled air. High precision and accuracy of CO2 concentration measurements are essential in air sampling systems for breath analysers. They, however, require an analysis of the influence of the human exhaled air pressure and temperature on the NDIR CO2 sensor. Therefore, analyses of the changes in concentration were carried out at a pressure from 986 mbar to 1027 mbar and a temperature from 20°C to 36°C. Finally, corresponding correction coefficients were determined which allow to reduce the relative uncertainty of CO2 sensor measurements results from 19% to below 5%.

Opis:
Breath analysis has attracted human beings for centuries. It was one of the simplest methods to detect various diseases by using human smell sense only. Advances in technology enable to use more reliable and standardized methods, based on different gas sensing systems. Breath analysis requires the detection of volatile organic compounds (VOCs) of the concentrations below individual ppm (parts per million). Therefore, advanced detection methods have been proposed. Some of these methods use expensive and bulky equipment (e.g. optical sensors, mass spectrometry - MS), and require time-consuming analysis. Less accurate, but much cheaper, are resistive gas sensors. These sensors use porous materials and adsorption-desorption processes, determining their physical parameters. We consider the problems of applying resistive gas sensors to breath analysis. Recent advances were underlined, showing that these economical gas sensor scan be efficiently employed to analyse breath samples. General problems of applying resistive gas sensors are considered and illustrated with examples, predominantly related to commercial sensors and their long-term performance. A setup for collection of breath samples is considered and presented to point out the crucial parts and problematic issues.

Background: There are no data addressing the usefulness of non-invasive tests for the detection of Helicobacter pylori (HP) infection in celiac disease (CD). Aim: The aim of this study was to compare two most sensitive and specific tests - urea breath test (UBT) and fecal antigen test (FAT) in HP diagnosis in CD patients. Materials and Methods: The study comprised of 76 CD patients, 49 healthy subjects (HS) and 35 patients who underwent differential diagnosis due to abdominal pain (AP patients). The presence of HP infection was evaluated using the 13C isotope-labeled UBT and FAT (ELISA). Results: HP infection was diagnosed based on UBT and FAT in 8 (16.3%) and 7 (14.3%) HS, and in 8 (10.5%) CD patients and 12 (34.3%) AP patients, respectively, using both tests. The prevalence of conflicting results in comparison with positive results (obtained with any of the two tests) was distinctly higher (54.5%) in CD group than in other subjects (23.3%); however, due to low HP prevalence, it did not reach the level of significance (p<0.1759). Conclusion: CD may increase the risk of divergent results of non-invasive tests used for the detection of HP infection in children. Since UBT is the most reliable test, we suggest its standard use as a method of choice in pediatric CD - at least until new evidence emerges supporting a different approach.

Antibiotic therapy in the cystic fibrosis (CF) mouse model has been shown to result in reduced bacterial load of the intestine and significant body mass gain. The effect was suggested to be linked to the improvement of intestinal digestion and absorption. Therefore, we aimed to assess the influence of routinely applied antibiotic therapy in CF patients on fat assimilation. Twenty-four CF patients aged 6 to 30 years entered the study. Inclusion criteria comprised confirmed exocrine pancreatic insufficiency and bronchopulmonary exacerbation demanding antibiotic therapy. Exclusion criteria comprised: antibiotic therapy six weeks prior to the test, liver cirrhosis, diabetes mellitus, oxygen dependency, the use of systemic corticosteroids. In all enrolled CF subjects, 13C-labelled mixed triglyceride breath test (13C MTG-BT) was performed to assess lipid digestion and absorption, before and after antibiotic therapy. Sixteen subjects were treated intravenously with ceftazidime and amikacin, eight patients orally with ciprofloxacin. Cumulative percentage dose recovery (CPDR) was considered to reflect digestion and absorption of lipids. The values are expressed as means (medians). The values of CPDR before and after antibiotic therapy did not differ in the whole studied group [4.6(3.3) % vs. 5.7(5.3) %, p = 0.100] as well as in the subgroup receiving them intravenously [4.6(3.2) % vs. 5.7(5.3) %, p = 0.327] or in that with oral drug administration [4.6(3.4) % vs. 5.7(5.4) %, p = 0.167]. In conclusion, antibiotic therapy applied routinely in the course of pulmonary exacerbation in CF patients does not seem to result in an improvement of fat digestion and absorption.

Objectives Dyspnea is one of the most predominant symptom in clinical practice. There is a lack of data about incidents of dyspnea among Polish adults therefore it would be important to establish prevalence of this symptom before COVID-19 pandemic to assess the impact of this infection on the functioning of the adult Polish population in the future. The aim of the study was to establish prevalence of dyspnea in adult Polish population. Material and Methods It was an observational-cross-sectional study, with representative sample of adult Poles aged 18–79 years. The 2413 participants were surveyed. Responders were asked if and when dyspnea occurs and what is its severity in relation to one of four categories (A, B, C, and D) describing the impact of dyspnea on reduced exercise tolerance and daily activities. Results The 67.1% of the respondents answered negatively to all question about experiencing dyspnea (females (F) 61% vs. males (M) 74%, p < 0.05). Dyspnea only during intense physical exertion (A), was reported by 22.8% (F 26.2% vs. M 19.2%, p = 0.07). Dyspnea limiting daily activities (B, C and D) was reported by 10.1% (F 13.1% vs. M 7%, p < 0.05). Significant differences in the severity of dyspnea were found between the age groups. People diagnosed with chronic heart failure or lung diseases significantly more often reported dyspnea than people without these conditions. Conclusions Every tenth Pole reported dyspnea limiting performing activities of daily living. Additionally, about 20% of Poles experienced dyspnea considered as "gray area," only during intense physical exertion, that requires deepening and clarifying the medical history.

Background: Helicobacter pylori (Hp) is classified by the International Agency for Research on Cancer (IARC) as a Group 1 carcinogen. Its influence on the carcinogenesis of gastric cancer has been confirmed in many research studies. The conclusion is obvious - early detection and eradication of Hp can prevent the development of the disease. Methods: The objective of the study was to analyze the clinical and practical value of Carbon-13 urea breath test (UBT) in patients hospitalized due to pain complaints in the upper abdomen and dyspeptic symptoms. Fifty patients were enrolled in the study. Each patient underwent urea breath test according to the instruction included by the producer. Thereafter, each patient included in the study group was subjected to endoscopy of the upper gastrointestinal tract with the biopsy of the mucosa to determine the urease activity with rapid urease test (RUT). Results: In the study group, 14 patients (28%) achieved a positive urease test result which was confirmed in RUT. Four (8%) patients, despite a positive breath test, did not have a positive result in urease activity test from gastric mucosa. In 2 cases (4%) despite negative result of UBT, urease activity was confirmed in gastroscopic sections. The remaining 30 patients (60%) had a negative result in both studies. Conclusions: The limited availability of the gold standard for diagnostics of upper gastrointestinal tract diseases (gastroscopy) is the basis for the search for new methods of detection of Helicobacter pylori infections. The urea breath test is a method of high sensitivity and specificity. The positive result of urea breath test may be the basis for inclusion of eradication therapy.

Emotions are one of the most fascinating and difficult aspects of human experience, and have received significant attention in biblical studies. This paper explores how one Hebrew lexeme, rûaḥ, provides a point of entry into the complex world of how emotions are expressed in ancient texts. Drawing from some insights of Cognitive Linguistics, it examines the use of rûaḥ to express the experience of impatience and patience, and arrogance and humility. This paper then challenges a long-held but simplistic equation of rûaḥ with anger, and argues that a more nuanced and complex relationship exists between lexeme and emotion than most citations in scholarship suggest.

Introduction: Hydrogen breath test (BT) is commonly used as a diagnostic tool for the detection of small intestine bacterial overgrowth (SIBO). It was reported that colonic methane production is far more frequent in cystic fibrosis (CF) patients than in other subjects. Therefore, measuring exclusively hydrogen in the diagnostic breath test for diagnosing SIBO might be of limited value. We aimed to assess the usefulness of combined measurement of hydrogen and methane expiration for the diagnosis of SIBO in CF. Material and Methods: The study comprised 62 CF patients aged 5 to 18 years. Three-hundred-ninety subjects assessed due to gastrointestinal symptoms for the presence of SIBO served as a comparative group. In all subjects hydrogen/methane BT using glucose was performed. A positive BT was defined as fasting hydrogen ≥ 20 ppm or fasting methane ≥ 10 ppm or a rise of ≥ 12 ppm hydrogen or ≥ 6 ppm methane over baseline during the test. Results: In 23 (37.1%) CF patients and in 52 (13.3%) subjects from the comparative group abnormal BT results were found. In seven (11.3%) CF patients and 29 (7.4%) of the other subjects studied methane measurement allowed diagnosis of SIBO. Conclusions: Small intestine bacterial overgrowth is frequent in cystic fibrosis. For its detection in cystic fibrosis and other gastrointestinal patients, combined hydrogen and methane measurement instead of hydrogen breath test should be applied. Without the additional measurement of methane a significant percentage of SIBO will be missed.

The paper describes an integrated laser absorption system as a potential tool for breath analysis for clinical diagnostics, online therapy monitoring and metabolic disorder control. The sensors operate basing on cavity enhanced spectroscopy and multi-pass spectroscopy supported by wavelength modulation spectroscopy. The aspects concerning selection of operational spectral range and minimization of interference are also discussed. Tests results of the constructed devices collected with reference samples of biomarkers are also presented. The obtained data provide an opportunity to analyse applicability of optoelectronic sensors in medical screening.

Sensing technology has been developed for detection of gases in some environmental, industrial, medical, and scientific applications. The main tasks of these works is to enhance performance of gas sensors taking into account their different applicability and scenarios of operation. This paper presents the descriptions, comparison and recent progress in some existing gas sensing technologies. Detailed introduction to optical sensing methods is presented. In a general way, other kinds of various sensors, such as catalytic, thermal conductivity, electrochemical, semiconductor and surface acoustic wave ones, are also presented. Furthermore, this paper focuses on performance of the optical method in detecting biomarkers in the exhaled air. There are discussed some examination results of the constructed devices. The devices operated on the basis of enhanced cavity and wavelength modulation spectroscopies. The experimental data used for analyzing applicability of these different sensing technologies in medical screening. Several suggestions related to future development are also discussed.

Opis:
The ‘breath figures’ method was used to prepare nitrocellulose (NC) microspheres and N-butyl-N-(2-nitroxyethyl)nitramine (BuNENA) modified NC microspheres (BMNM). By using acetone as the solvent and non-solvent n-hexane as the atmosphere, NC microspheres and BMNM were obtained. The matching of solvents and non-solvents was the key factor influencing the final morphology. TG and DSC were employed to study the thermal decomposition characteristics of BMNM. The results suggested that there were two distinct stages of thermal mass loss from the BMNM: the first was the mass loss associated with partial volatilisation of BuNENA, and its activation energy was 68.54 kJ/mol. The second mass loss was the thermal decomposition of NC and residual BuNENA with a high activation energy (249.53 kJ/mol, calculated by the Kissinger method). According to the thermal decomposition kinetics model, the optimal kinetics model of BMNM was B1-A2. The addition of BuNENA did not influence the thermal decomposition of NC.
