Opis:
The authors presents a case of cribriform breast carcinoma in a cyst that clinically imitated an abscess. The case concerns a 71-year-old female patient treated for ankylosing spondylitis, with a positive family history of breast cancer. The patient presented at the surgical clinic for incision of an abscess of the mammary gland localized in the lower inner quadrant that was a consequence of previous trauma to the right breast. The abscess was incised and the serosanguineous contents were evacuated. The wound was drained and antibiotics (Dalacin with Metronidazol) were administered for the period of 10 days. During the treatment, a cutaneous fistula was formed. At the incision site, a hard thickening was palpable (tumor). Core needle biopsy of the clinically palpable tumor was performed and the purulent material from the fistula was collected for a culture test. Complete blood count did not reveal leucocytosis. In accordance with the obtained sensitivity report, the patient was started on antibiotics again. Breast ultrasound performed upon the completion of the antibiotic therapy, in the right breast, revealed two solidcystic oval lesions with thick echogenic walls and blurred margins. Both masses contained dense levels of fluid material and solid polycyclic structures. On sonoelastography, the lesions were heterogeneous with a high Young’s modulus. In the right axillary fossa, ultrasound examination revealed three abnormal lymph nodes enlarged to 31 mm length, which were rounded, hypoechoic and without visible sinuses. Histopathology of the core needle biopsy performed at admittance and after the antibiotic therapy indicated a breast abscess (presence of fibrinous and partly fibrinopurulent material). The mass was finally resected to confirm histopathology. The resected material revealed the presence of an invasive, moderately differentiated cribriform carcinoma, which developed within a cyst, with a 40% necrotic component. Eighteen months after the commencement of treatment, the patient remains under oncological supervision and continues hormonal therapy. There are no signs of relapse or foci of distant metastases. The occurrence of breast carcinoma within an abscess emphasises the need for comprehensive assessment and correlation of the clinical picture with imaging and histopathological findings. It also highlights the necessity to include breast abscess in the differential diagnosis of rare forms of carcinomas.
Przedstawiono przypadek raka sitowatego piersi w torbieli klinicznie imitujący ropień. Dotyczy on 71‑letniej pacjentki leczonej z powodu zesztywniającego zapalenia stawów kręgosłupa, obciążonej rodzinnie rakiem piersi. Kobieta zgłosiła się do poradni chirurgicznej w celu nacięcia ropnia gruczołu piersiowego, zlokalizowanego w kwadrancie dolnym wewnętrznym, który powstał w następstwie urazu prawej piersi. Nacięto ropień, ewakuując treść surowiczo‑krwistą. Ranę zdrenowano oraz włączono antybiotykoterapię (Dalacin z Metronidazolem) na okres 10 dni. W trakcie leczenia doszło do powstania przetoki skórnej. W miejscu po nacięciu ropnia było wyczuwalne twarde zgrubienie (guz). Wykonano biopsję gruboigłową klinicznie wyczuwalnego guza oraz pobrano treść ropną z przetoki na posiew. W badaniu morfologii krwi nie występowała leukocytoza. Włączono ponownie antybiotykoterapię zgodnie z otrzymanym antybiogramem. W badaniu ultrasonograficznym piersi, wykonanym po zakończonej antybiotykoterapii, w prawej piersi uwidoczniono dwie lito‑płynowe owalne zmiany, z obecnością grubych echogennych ścian, o zatartych brzegach. W obu zmianach występowały poziomy zagęszczonej treści płynowej oraz lite struktury o policyklicznych kształtach. W sonoelastografii oceniane zmiany były heterogenne, o wysokich wartościach modułu Younga. W prawym dole pachowym w badaniu ultrasonograficznym stwierdzono trzy nieprawidłowe węzły chłonne, powiększone do 31 mm długości, zaokrąglone, hipoechogeniczne, bez widocznych zatok. Wynik badania histopatologicznego z biopsji gruboigłowej wykonanej przy przyjęciu oraz po zakończonej antybiotykoterapii wskazywał na ropień piersi (opisano obecność mas włóknikowych, częściowo włóknikowo‑ropnych). Dopiero wycięcie całej zmiany celem weryfikacji histopatologicznej ujawniło obecność inwazyjnego, średnio dojrzałego raka sitowatego, który rozwinął się w świetle torbieli, z obszarami martwicy obejmującej około 40% masy guza. Pacjentka po 18 miesiącach od rozpoczęcia leczenia pozostaje pod opieką onkologiczną, kontynuuje hormonoterapię, nie ma nawrotu choroby oraz ognisk przerzutów odległych. Opisany przypadek raka piersi w ropniu wskazuje na potrzebę kompleksowej oceny i korelacji obrazu klinicznego z wynikami badań obrazowych oraz histopatologicznych, a także uwzględniania w diagnostyce różnicowej ropnia piersi rzadko występujących postaci raka.

Opis:
Colocasia esculenta (Taro) is an edible tuberous plant; however, corms are its most worldwide consumed part while the corm powder is widely used in food industries. In this work, a sulphated polysaccharide extract of C. esculenta corm (SCE) was prepared and its cancer chemopreventive properties was explored. The amending of carcinogen metabolism and radical scavenging affinity revealed that SCE is a strong tumor anti-initiation agent via suppressing cytochrome P450-1A and enhancing glutathione and the carcinogen detoxification enzyme; glutathione S-transferase. SCE exhibited a strong scavenging affinity towards critical radicals (hydroxyl and peroxyl). It induced lymphocyte growth and modulated the macrophage functions into an anti-inflammatory profile, via elevating macrophage proliferation and its binding affinity of fluorescein isothiocyanate-lipopolysaccharide (FITC-LPS) and inhibiting nitric oxide and tumor necrosis factor-α generation. Furthermore, SCE showed a potent cytotoxicity against human breast MCF-7 carcinoma cells (IC50 27.73 µg/mL), whereas SCE treatment inhibited the activity of histone deacetylase (HDAC IC50 37.70 µg/mL) and disturbed the pattern of cell cycle phases. An arrest in both S- and G2/M-phases was linked with shifted cell populations towards late apoptosis and necrosis, as detected by flow cytometry. SCE is a promising cancer chemopreventive agent to be used in healthy food industries and for high breast cancer-risk population.

Introduction. The proper negative margins (R0) breadth in the breast - conserving surgery for invasive breast cancer (IBC) and ductal carcinoma in situ (DCIS) is very important. The presence positive surgical margins (R1) is associated with the necessity of reoperation. It delays the adjuvant therapy and psychologically burdens the patient. The re-operation increases the costs of treatment. The introduction of mammography (MMG) increased detection of DCIS by 20%. With the increase in malignancy, cancer detection decreases in MMG, inversely in MRI groving. Effective preoperative and intraoperative diagnosis aims to reduce the number of R1 resections. Aim. The size of the tumor next to its biology, determines the clinical course of the tumor. The accurate analysis of imaging tests is important. Material and methods. This analysis was performed using a systematic literature search. Results. Adequate surgical margins in breast cancer surgery for breast cancer have been reviewed. It is important to know if the cancer is multifocal and what the extent of the tumor is. Conclusion. The adequacy of margins is important for adjusting the volume of excision. It is avoiding unnecessary resection of healthy breast tissue. It is essential for a good cosmetic result and the local recurrence rate. The combination of breast MRI with conventional breast imaging resulted in the lover rate of the R1 resectios and the lower rate of the re-operation.

The paper presents a case of primary neuroendocrine breast carcinoma that coexisted with a typical infiltrating carcinoma (BC) within the same mammary gland. It was diagnosed post-operatively on histopathological examination and confirmed by immunohistochemical analysis.
W pracy przedstawiono przypadek pierwotnego neuroendokrynnego raka piersi, który współwystępował z typowym rakiem naciekającym (BC) w tym samym gruczole piersiowym. Został zdiagnozowany pooperacyjnie w badaniu histopatologicznym i potwierdzony w badaniach immunohistochemicznych.

Opis:
Overtreatment means treatment that goes beyond current standards, and patients with any disease can be overtreated. Overtreatment is also given to patients with cancer, including those who need surgery. Overtreatment is closely related to the problem of overdiagnosis. In patients with cancer, unnecessary surgery may cause complications and generates unnecessary costs. The size of the problem of unnecessary surgery in patients with cancer can best be shown among patients with the most common cancers, which dedicated screening programs. Breast cancer patients, particularly those with pre-invasive types of the tumor, who typically have ductal carcinoma in situ (80%), are likely to undergo unnecessary surgery. We describe the most common clinical problems caused by overtreating patients with ductal carcinoma in situ.

Introduction and objective. αB-crystallin belongs to the ubiquitous family of small heat-shock proteins. It was discovered as a physiological protein of the eye lens, maintaining its liquid-like property. Furthermore, αB-crystallin was proved to playa bipolar role in both physiological and pathophysiological conditions. This review discusses current knowledge about the biology and genetics of αB-crystallin, and summarizes recent advances in understanding its role in ophthalmic and neurological disorders, as well as breast cancer, renal cancer and other malignancies. State of knowledge. α-crystallins are established as important elements of the protein quality control network, and consequently their defects are related to multiple human diseases. New studies highlight αB-crystallin’s involvement in proliferative diabetic retinopathy angiogenesis and point out its therapeutic potential in age-related macular degeneration. αB-crystallin is thought to be associated with the disease-causing protein aggregates, leading to its connection with such neurological disturbances as anaplastic astrocytoma, Parkinson disease, aging deficits in the peripheral nervous system and multiple sclerosis. In breast cancer, it was proven to be a marker of aggressive behaviur and cerebral metastases. Strong expression of αB-crystallin promoted growth and migration of clear cell renal cell carcinoma cells and was correlated with lower overall survival rate. Considering other malignancies, its various roles were established in colorectal and gastric cancers, head and neck squamous cell carcinomas and osteosarcomas. Conclusions. Further studies concerning αB-crystallin seem to be enormously promising, as they might improve our understanding of common human pathologies as well as contemporary diagnostics and treatment.

Diagnose of ductal carcinoma in situ (DCIS) requires, at least in a part of cases, assessment of auxiliary lymph nodes. The aim of the study was to analyze clinical value of Van Nuys Prognostic Index (VNPI) as the important criterion for qualification of DCIS patients to sentinel lymph node biopsy (SLNB). Material and methods. Analysis included patients diagnosed with DCIS and qualified to SLNB, operated in years 2004-2013. We performed a statistical analysis to assess correlation between VNPI value and positive pathological verification of excised sentinel lymph node (SLN). The influence of other clinical factors on presence of metastases in SLN was also determined. Results. 3,6% of DCIS patients were diagnosed with metastases in SLN. Metastases were diagnosed in 6.7% patients with VNPI > 10 points vs 3.4% in patients with VNPI < 10 points. In 75% of patients with metastatic SLN the value of VNPI was at least 10 points. In premenopausal patients metastases in SLN were diagnosed in 8.1% of cases vs 1.4% in postmenopausal patients. In patients diagnosed with cancer of the other breast metastases in SLN were found in 6.3% of cases vs 3.2% in one-sided cancer. Patients with multifocal cancer were over twice as often diagnosed with metastases in SLN (5% vs 2%). There was no correlation found between VNPI value and positive pathological verification of SLN. No differences between prevalence of SLN metastases depending on selected clinical features were found. Conclusions. Recommendation of VNPI as the criterion of qualification to SLNB starting with 10 points could be more favourable in patients with DCIS. Deciding on SLNB in DCIS patients, we should also take into consideration other clinical features.
