Introduction. Artificial intelligence is increasingly being used in the medicine, particularly in radiological diagnosis of diseases such as an axial spondyloarthritis (axSpA). The aim of this study is to compare the available algorithms designed to detect active sacroiliitis in patients with axSpA. Material and methods. Four algorithms, two semi-automated and two full-automated for the assessment of bone marrow edema (BME) on magnetic resonance imaging (MRI) of the sacroiliac joints (SIJs) were included in the study. They were described and compared in terms of specificity, sensitivity, and correlation of BME detection findings between AI and experts. Analysis of the literature. Among all automated algorithms, the one created by Bressem et al. had the highest number of examinations analyzed in the study, involving 593 MRIs of SIJs. The sensitivity and specificity, as well as the correlation between the AI’s detection of BME versus manual, were not calculated for each algorithm. Rzecki’s algorithm had the greatest sensitivity and specificity for BME detection reaching 0.95 and 0.96, respectively. In addition, its Speraman’s coefficient of correlation between manual and automated measurements was 0.866. Conclusion. Each of described algorithms is certainly useful in assessing BME in the MRI examinations of SIJs.

Objectives: The aim of the study was to evaluate the influence of acetylsalicylic acid (ASA) on benzene hematotoxicity in rats. Materials and Methods: The study was carried out on rats exposed for 2, 4 and 8 weeks to benzene vapour at a conentration of 1.5 or 4.5 mmol/m³ of air (5 days per week, 6 hours per day) alone or together with ASA at the doses of 5, 150 or 300 mg/kg body weight (per os). Results: Benzene at a concentration of 4.5 mmol/m³ caused a slight lymphopenia, granulocytosis and reticulocytosis in blood. In bone marrow traits of megaloblastic renewal, presence of undifferentiated cells and giant forms of granulocytes as well as an increase in myeloperoxidase and decrease in chloroacetate esterase activity and lipids content were noted. ASA (150 and 300 mg/kg b.w.) influenced some of hematological parameters, altered by benzene intoxication. ASA limited the solvent-induced alteration in blood reticulocyte count and in the case of bone marrow in the erythroblasts count. Traits of megaloblastic renewal in bone marrow were less pronounced. Besides, higher activity of myeloperoxidase and the decrease in the level of lipids in granulocytes were noted. Conclusion: Our results suggest that ASA limited the benzene-induced hematotoxicity.

The theoretical rationale was the author’s Functional Model of Health (FMH), where health is construed as a function of creative coping with stress. Participants in the study were 141 patients with blood cancer (80 men and 61 women) treated with bone marrow transplantation at the Hematology Clinic, Jagiellonian University Collegium Medicum. Besides a standardized interview the following instruments were used: STAI by Spielberger, CISS and CHIP by Endler and Parker, and SOC-29 by Antonovsky. Health status was operationalized using 10-point self-rating scales to assess the patients’ perceived health and sense of calmness, while their objective health was rated by a medical expert - hematologist. In the statistical analyses significant correlations were found between stress level and the patients’ subjective health (-0.34, p<0.001). The following variables predicted stress intensity: anxious personality (.585), distraction coping strategy (-.202), female sex (.201), and sense of meaningfulness (-.190). Subjective health predictors of leukemia patients are: objective health (ß = .413), level of stress (ß = -.285), by the patient’s sense of meaningfulness (ß = .244) and focusing on negative emotions (ß = -.193). To promote patients’ health moreover, their sense of meaningfulness in life and in being active should be supported and enhanced.

In the frame of a project aiming to the improvement of the properties of biodegradable polyester-based devices, designed for bone regeneration, we have prepared and characterized novel composites based on poly(ε-caprolactone) (PCL). In particular, we report in this paper the functional changes made through radical grafting on PCL to obtain the less hydrophobic derivatives, PCL-MA-GMA [maleic anhydride (MA)-glycidylmethacrylate (GMA)] and PCL-DMAEA [N-(dimethyl-amino)ethylacrylate]. In addition, we studied in vitro the biocompatibility and the osteogenic capacity of these novel polyesters on human bone marrow (BM) mesenchymal stem cells (MSC).

Epstein-Barr virus (EBV), a member of the family Herpesviridae, is widely spread in the human population and has the ability to establish lifelong latent infection. In immunocompetent individuals the virus reactivation is usually harmless and unnoticeable. In immunocompromised patients productive infection or type III latency may lead to EBV-associated post-transplant lymphoproliferative disorder (PTLD). The aim of our research was to investigate the utility of PCR-based methods in the diagnosis and monitoring of EBV infections in bone marrow transplant recipients. Thirty-eight peripheral blood leukocyte samples obtained from 16 patients were analysed, in which EBV DNA was confirmed by PCR. We used semi-quantitative PCR to estimate the viral load and reverse-transcription PCR (RT-PCR) to differentiate between latent and productive EBV infection. In 14 patients we confirmed productive viral infection. We observed a correlation between higher number of EBV genome copies and the presence of transcripts specific for type III latency as well as clinical symptoms.

Medical problems associated with the increasing number of patients suffering from brain diseases have resulted in a constant search for effective therapeutics. Considering the complicated pathological processes occurring in diseases of the central nervous system and the limited capability of the neural tissue to regenerate, therapy of neurological diseases is extremely difficult. The lack of efficient medical treatment results in complex problems associated with rehabilitation and thus in functional disturbances, which prevent patients from restoring their independence and returning to complete, also professional, activity, Cell therapy has recently been considered as a possible approach in the treatment of brain diseases. Its aim is to supply pathologically changed brain tissue with factors promoting regeneration and with cells that may replace the damaged ones. Bone marrow cells have become a potential source of cells in this type of therapy. Bone marrow contains at least two major kinds of stem cells: haematopoietic stem cells, which give rise to the blood cells and mesenchymal stem cells, which can differentiate into cells of mesenchymal lineage and produce an array of growth factors essential for repair. The review presents the achievements of studies on use of bone marrow cells in the therapy of various brain diseases of traumatic or neurodegenerative aetiology.

Opis:
Objective: The aim of this study was to evaluate the usefulness of ultrasonography in the diagnosis of hemorrhagic cystitis following bone marrow transplantation in children. Material and methods: The study involved an analysis of clinical material and the results of imaging tests performed in 334 patients who underwent hematopoietic cell transplantation. Ultrasonographic findings in 42 patients with hemorrhagic cystitis were analyzed in detail. The ultrasound images served to assess the severity of hemorrhagic cystitis and the results were compared with the clinical assessment of the disease on the Droller scale, as well as the laboratory and endoscopic tests. Results: In the studied group of patients hemorrhagic cystitis following allogeneic transplantation was diagnosed in 12.5% cases. 73.8% patients received transplants from unrelated donors, 26.2% – from compatible siblings. The study revealed a higher incidence of hemorrhagic cystitis in children above 10 years of age. Grade 3 according to the Droller was diagnosed in 42.9%, grade 2 – in 30.9%, grade 4 – in 14.3%, and grade 1 – in 11.9% patients. The number of ultrasound examinations depended on the clinical symptoms, severity, duration and co-occurrence of other complications following the transplantation and was within the 1–15 range (average: 4.6). Grades 3 and 4 were related to the poor clinical condition of the patients and to their longer hospitalization. During this period there was an increased risk of renal malfunction and acute renal failure, post-inflammatory narrowing of the ureters, hydronephrosis, and in grade 4 the fibrosis of the bladder with reduced bladder capacity. Analyses demonstrated a significant correlation between the ultrasound image of the bladder wall and the clinical severity. Conclusions: Ultrasound with Doppler options remains the primary diagnostic tool in the evaluation of hemorrhagic cystitis, and is useful in terms of its diagnosis, determination of the severity, and monitoring of the treatment.
Cel: Celem pracy była ocena przydatności ultrasonografii w diagnostyce krwotocznego zapalenia pęcherza moczowego, dość częstego powikłania po przeszczepach szpiku kostnego u dzieci. Materiał i metoda: W pracy przeanalizowano materiał kliniczny i wyniki badań obrazowych 334 pacjentów po transplantacji komórek hematopoetycznych. Analizie poddano wyniki badań ultrasonograficznych wykonanych u 42 dzieci z krwotocznym zapaleniem pęcherza moczowego. Na podstawie USG oceniano stopień zaawansowania krwotocznego zapalenia pęcherza moczowego, a wyniki porównywano z oceną kliniczną procesu w skali Drollera, badaniami laboratoryjnymi i endoskopowymi. Wyniki: W analizowanej grupie dzieci krwotoczne zapalenie pęcherza moczowego rozpoznano po transplantacjach allogenicznych u 12,5% badanych, u 73,8% był to przeszczep od dawcy niespokrewnionego, u 26,2% – od zgodnego rodzeństwa. Wykazano częstsze występowanie krwotocznego zapalenia pęcherza moczowego u dzieci starszych, powyżej 10. roku życia. Stopień 3. w skali Drollera stwierdzono u 42,9% dzieci, stopień 2. – u 30,9%, stopień 4. – u 14,3%, a stopień 1. – u 11,9% dzieci. Liczba badań ultrasonograficznych i rozpoznanie zależały od objawów klinicznych, stopnia zaawansowania, czasu trwania oraz współwystępowania innych powikłań po transplantacji i wynosiła 1–15 (średnio 4,6). Trzeci i czwarty stopień wiązały się ze złym stanem klinicznym chorych, a także wydłużeniem czasu hospitalizacji. W okresie tym wzrastało ryzyko uszkodzenia czynności nerek i ostrej niewydolności nerek, pozapalnego zwężenia moczowodów, wodonercza, a w końcowym stadium zwłóknienia pęcherza moczowego. Stwierdzono istotną zależność obrazu ultrasonograficznego zmienionej w przebiegu zapalenia ściany pęcherza moczowego, ze stopniem klinicznego zaawansowania. Wnioski: Badanie ultrasonograficzne z opcjami dopplerowskimi pozostaje podstawowym badaniem diagnostycznym w ocenie krwotocznego zapalenia pęcherza moczowego, jest przydatne w rozpoznawaniu, określaniu stopnia zaawansowania choroby i monitorowaniu leczenia.

W pracy przedstawiono przypadek spornego ojcostwa z udziałem pozwanego, który 2 i pół roku wcześniej przeszedł allogeniczną transplantację szpiku kostnego. Dziecko miało pół roku. Z uwagi na utratę owłosienia w wyniku terapii farmakologicznej, do badania pobrano wymaz z jamy ustnej i krew obwodową. We krwi uzyskano profil genetyczny dawcy (kobieta), natomiast w wymazie z jamy ustnej mieszaninę profili genetycznych dawcy i biorcy. Z mieszaniny wygenerowano profil genetyczny pozwanego. U dziecka we wszystkich badanych loci stwierdzono cechę wspólną z pozwanym. W żadnym z markerów nie uzyskano wyniku, który wykluczałby ojcostwo pozwanego. W badaniu loci z chromosomu Y u dziecka i pozwanego uzyskano pełną zgodność profili. Niniejsza praca podkreśla rolę wywiadu w prawidłowym protokole pobrania materiału, co chroni przed wydaniem fałszywej opinii.
In the Department of Forensic Medicine and Medico-Legal Toxicology, Medical University of Silesia, Katowice, Poland before blood or buccal swab sampling for disputed paternity testing the parties are routinely asked whether they had bone marrow transplantation or not. The authors report the disputed paternity case in which the male defendant had hallogeneic bone marrow transplantation. In the tested trio, the child was six months old. For disputed paternity testing blood and buccal swab samples were taken from the defendant. As he had lost all his hair because of antineoplastic therapy, no hair follicles were taken. A female genetic profile (donor) was determined from the blood sample of the defendant, whereas the mixture of two profiles ( donor and recipient) in the buccal swab one. From the mixture a genetic profile of the male defendant was finally distinguished. In all tested loci the child had genetic markers which appeared also in the defendant. There was no paternity exclusion in any of the tested markers. Moreover, there was full genetic agreement between profiles of the child and the defendant. The report emphasizes the importance of the detailed history taking before biological material sampling for disputed paternity testing, which can prevent forming a false opinion.

This article is focused on the data concerning properties of carbamazepine, an antiepileptic drug commonly used in everyday medical practice but also frequently misused leading to many reported cases of intoxication. Due to high prevalence of this drug detailed data concerning its pharmacokinetics and prahmacodynamics can be found as well as metabolic paths that have been proven or have not been confirmed yet. For the purpose of toxicological analyses of samples collected postmortem it is also essential to know the possible transformations of an analyte due to microbial metabolism and postmortem changes of the sample matrix. In forensic cases of unavailable or unusable blood samples when advanced stage of decomposition processes in the found body occurs, alternative matrices are required. Presented review describes three types of such matrices: bones, bone marrow and grease in the context of their postmortem changes and suitability as the source of toxicological information.

Opis:
There is a need for efficient stem cell-to-tenocyte differentiation techniques for tendon tissue engineering. More than 1 week is required for tenogenic differentiation with chemical stimuli, including co-culturing. Research has begun to examine the utility of mechanical stimuli, which reduces the differentiation time to several days. However, the precise length of time required to differentiate human bone marrow-derived mesenchymal stem cells (hBMSCs) into tenocytes has not been clarified. Understanding the precise time required is important for future tissue engineering projects. Therefore, in this study, a method was developed to more precisely determine the length of time required to differentiate hBMSCs into tenocytes with cyclic stretching stimulus. Methods: First, it had to be determined how stretching stimulation affected the cells. Microgrooved culture membranes were used to suppress cell orientation behavior. Then, only cells oriented parallel to the microgrooves were selected and evaluated for protein synthesis levels for differentiation. Results: The results revealed that growing cells on the microgrooved membrane and selecting optimally-oriented cells for measurement improved the accuracy of the differentiation evaluation, and that hBMSCs differentiated into tenocytes in approximately 10 h. Conclusions: The differentiation time corresponded to the time required for cellular cytoskeleton reorganization and cellular morphology alterations. This suggests that cells, when subjected to mechanical stimulus, secrete mRNAs and proteins for both cytoskeleton reorganization and differentiation.

Opis:
Alterations in the genetic apparatus of mice bone marrow cells and testicles caused by Hymenolepis nana, Ascaris suum and Toxocara canis metabolites have been investigated by means of micronuclear test application. The activity of spermatogenesis has been investigated at experimental hymenolepidosis, migrating ascariasis and toxocarosis with the use of ³H-timidine. Helminths metabolites have been established to exert a mutagenie effect on somatic cells of bone marrow, spermatogonies and also on the generative cells (spermatides) of helminths in invaded mice. The concurrent increase in micronucleus number in erythrocytes, spermatogonies and in spermatides (to a lesser degree) of invaded mice has been revealed. The decrease in spermatogenesis activity has been established in experimental hymenolepidosis, migrating ascariasis and toxocarosis in invaded mice.
