Opis:
A quiet revolution underway is leading to changes in healthcare for many countries of the world. The sentinels of health have always been physicians; the medical model, representing the research, education, and experience of medical doctors, has worked well for treating acute conditions of injury and physical illness and been very successful in reducing disease-producing morbidity and mortality. But pressing contemporary issues, such as spiraling healthcare costs, population aging and the need to manage chronic conditions, and recognition that mental health care is necessary for societies to be physically healthy, are forcing a reevaluation of existing conceptualizations of care. In response, physicians are increasingly working in integrated practices with other specialists to broaden care coverage to include social and behavioral conditions empirically demonstrated to influence medical care outcomes. Aiding the development of these new, more inclusive models of healthcare are advances in communication technologies. Practitioners are now using synchronous and asynchronous forms of communication to deliver physical and mental health services beyond the boundaries of traditional brick-and-mortar practices, into communities where clients live and work. This brief review of the potential of telehealth to address disparities in healthcare has two purposes: 1) examine an innovative model of comprehensive, integrated biopsychosocial services useful in single- and multiple-location practices; and, 2) identify challenges of using synchronous, virtual technologies for client-centered, mental health care service provision in rural, underserved areas.

In frame of the European Project CHANGE (Care of Health Advertising New Goals for Elderly people) the study of health behaviours regarding healthy nutrition and physical activity was conducted on patients in age of 60 and more covered with care of community nurses. Objectives: To assess association between physical activity and nutritional habits, stages of change of these behaviors among older patients and other psychosocial conditions regarding planning of health education intervention to change their health behaviours. Methods: 108 persons at age avg. 69.6 (SD 60–87) were interviewed by questionnaire (assessing the frequency of intake of particular food products, physical activity, motivation and stage of change these behaviors, satisfaction with physical condition and general wellbeing) by the trained community nurses. Results: Results showed sedentary style of life in most studied patients. Among physical activity the frequency of daily walking was the highest (69%), while 70% of older people did not exercise. In study group 42% were not aware of importance of physical activity for their health. But the awareness of importance of healthy nutrition was high, and about half of study group (48%) reported that they eat meals with reduced fat, and 80% eat high fiber food. The rules of healthy nutrition were not fully respected. Positive correlations were shown between intention to change, stage of change of health behavior and performance of physical activity and healthy eating. Those behaviors were positively related to well-being and satisfaction with physical condition. Conclusions: Health promotion programs for older patients should be planned in different way depending on their stage of change of health behaviors. There is a need to raise awareness of importance of physical activity, and to educate older patients about recommended types of exercises and recreation. In case of recommendations of healthy nutrition, the awareness is high and performance definitely more frequent, therefore health promotion programs should stimulate development of permanent social support to maintain change of these behaviors.

The use of multifunctional smartphones has changed lives in recent years and raised concerns around the world. The ability of smartphones to perform multiple tasks simultaneously is appealing. However, the total amount of time spent using smartphones every day is alarming, and excessive use might impact physical and mental health. The main goal is to determine the adequacy of the term “addiction” for excessive and problematic smartphone use. We present the case of Jalila, a 32-year-old woman who excessively uses her smartphone. We evaluate Jalila’s symptoms, noting that she spends about 9 hours per day on her smartphone. Jalila’s excessive cellphone use includes various addictive habits, such as instant chatting, watching YouTube videos, browsing the web, and visiting social media sites. The total amount of time she spends on these activities indicates a high degree of smartphone usage. When considered in its entirety, the smartphone, with all its functionalities, is a tool that can be used for all of these activities throughout the day. Jalila’s phone activities are primarily focused on community sites, and she uses her smartphone more than she intends to, indicating a loss of control that is affecting her health and well-being.

Contemporary world events are demonstrating the need to embrace and further develop tele-health options for assessment and delivery of biopsychosocial healthcare services. This is now possible, given advances in communication technologies allowing virtual connections of medical personnel with constituents, as well as necessary, in light of recent challenges posed by infectious conditions and growing needs for travel restrictions, social distancing, and isolation of large portions of populations. Moreover, the opportunity to virtually connect with persons through ubiquitous computer-based and handheld communication devices allows comprehensive care provision to include underserved areas, where restricted, walk-in access to brick-and-mortar establishments has long been recognized as a limiting factor in healthcare. This review examines evolving approaches of tele-healthcare, with a specific focus on telemedicine as a bridge between traditional, in-person approaches to diagnose and treat medical conditions and new healthcare opportunities developing to meet changing societal needs. The three purposes of the review are: discuss background information, with a brief look at policy and procedure guiding applications of tele-techniques in healthcare practices; identify relevant scientific studies to show the breadth of new evidence-based research for telemedicine practices; and, discuss challenges for the further development of telemedicine as healthcare systems in the United States evolve to meet current and projected healthcare needs.

Prezentowany artykuł porusza kwestie należytego kształtowania środowiska zbudowanego obiektu hospicjum przeznaczonego dla dzieci. Wpisuje się on w aktualny nurt tematyczny związany z projektowaniem obiektów zdrowia – health design. Rozważania opracowano na podstawie projektu koncepcyjnego dziecięcego hospicjum stacjonarnego wraz z centrum dziennej opieki zlokalizowanego w Katowicach. Opracowanie zwraca uwagę nie tylko na podstawowe potrzeby małych pacjentów, ale także na aspekty behawioralne decydujące o wysokich walorach jakościowych i użytkowych placówki. Prezentuje ono oddziaływanie przestrzeni hospicjum na emocje oraz odczucia użytkowników, które mają bezpośredni wpływ na ich stan zdrowia. Nowoczesnym przykładom obiektów opieki paliatywnej funkcjonujących zarówno w Polsce, jak i na świecie przyświeca dążenie przywrócenia życia do odhumanizowanej współcześnie medycyny, szczególnie tej związanej z opieką nad dziećmi chorymi terminalnie. Fundamentalnym kryterium przy tworzeniu środowiska hospicjum jest zapewnienie jego małym użytkownikom optymalnie zakomponowanej przestrzeni, która powinna dawać im poczucie bezpieczeństwa, prywatności, spokoju oraz tworzyć przyjazne środowisko, o wysokim komforcie użytkowania, które umożliwia specjalistyczną opiekę paliatywną na jak najwyższym poziomie.
The presented article tackles the issues of proper shaping of the environment of the constructed hospice facility intended for children. The article is part of the current theme related to the design of health objects - health design. The conlusions were presented on the basis of a conceptual design of a stationary children's hospice with a day care center located in Katowice. The study draws attention not only to the basic needs of young patients, but also to behavioral aspects that determine the high quality and utility values of the facility. It presents the impact of the hospice’s space on the emotions and feelings of users, which have a direct impact on their health. Modern examples of palliative care facilities operating both in Poland and in the world are guided by the attempt to restore life to contemporary dehumanized medicine, especially that related to the care of terminally ill children. The fundamental criterion in creating the hospice environment is to provide its little users with an optimally composed space, which should give them a sense of security, privacy, peace and create a friendly environment with high comfort of use, which enables specialist palliative care at the highest level.
