Background: Spinal pain is a common problem in developed societies. In recent years, more and more people have been found to complain about spinal pain. Therefore, it would seem that back pain remains a major problem, a burden for the patient, the family, and the economy. If the pain is severe, it can cause difficulties with performing even the most basic activities of everyday life. The quest for methods to reduce this complaint may contribute to improving the quality of life for many people. One of the more commonly chosen forms of therapy for back pain is therapeutic massage. Massage therapy has a long history with back pain treatment and has been found to produce various beneficial effects that are associated with endorphin release. It can also enhance local blood flow which could increase the efficacy of local pain mediators. Aim of the study: The aim of the study was to evaluate the effectiveness of therapeutic massage in the treatment of back pain in a 69-year-old woman. Case report: The subject was a 69-year-old woman reporting pain in the cervical and lumbar segments of the spine, BH = 154.2 cm, BM = 90.0 kg, BMI = 37.85 k/m² (obesity), waist circumference = 114.1, hip circumference = 118.2 cm, WHR = 0.96 (android obesity). The 11-point VAS scale was used in the assessment of her pain, where 0 means no pain, and 10 unbearable pain. The Laitinen Pain Scale (LPS) is a questionnaire which examines the nature of pain in four areas: the intensity of the pain, how frequently the pain occurs, the application of painkillers, and limiting motor activity. Before starting the therapy and immediately after the end of the therapy, researchers conducted a physical examination which included taking measurements of spinal mobility using the SFTR method. Before the first treatment, researchers conducted an interview was conducted about the need for the massage. 10 massages were performed after 60 minutes each, and were done every other day. Researchers performed a pain assessment with the VAS scale both before and after the procedures, which ultimately showed a reduction of the symptoms in the cervical and lumbar sections. The LPS scale showed a reduction of symptoms in three areas. The mobility of the spine had likewise increased. Conclusions: After a series of therapeutic massage treatments, there was a reduction in pain and improved spinal mobility.

Cel badań. Celem badań była ocena związku między nadwagą, otyłością a bólami kręgosłupa oraz weryfikacja korelacji pomiędzy tymi zmiennymi w grupie młodych kobiet kończących studia na kierunku fizjoterapii. Materiał i metody. Grupę badaną stanowiło 41 losowo wybranych studentek fizjoterapii III roku studiów licencjackich. Dokonano pomiaru wysokości i masy ciała oraz obliczono wskaźnik BMI. Do oceny dolegliwości bólowych części lędźwiowej kręgosłupa zastosowano Kwestionariusz Oswestry. Dla analizowanych zmiennych ilościowych wyznaczono podstawowe statystyki opisowe. Odpowiedzi na pytania zawarte w kwestionariuszu zestawiono i skorelowano ze wskaźnikiem BMI. W celu znalezienia związku między wskaźnikiem BMI a niepełnosprawnością zastosowano analizę wariancji oraz Test Levene’a jednorodności wariancji. Wyniki. Analiza wskaźnika BMI wykazała, iż nadmiarem masy ciała charakteryzowało się 29% badanych studentek (nadwaga 17%, otyłość 12%). Średnia wartość BMI wynosiła 24,8 kg/m2. Jak wykazała analiza kwestionariusza Oswestry, 78% badanych doświadczyło bólów kręgosłupa, a dolegliwości o charakterze dużym i bardzo dużym odczuwało 22% studentek. U większości stwierdzono niewielki (61%) i mierny (27%) stopień niepełnosprawności. Analiza wskaźnika BMI względem stopnia niepełnosprawności wykazała związek istotny statystycznie (Fniepełnospr. (2, 38) = 8.72, p < 0,001). Wnioski. Większość badanych studentek doświadczyła bólów kręgosłupa, a znaczny odsetek (30%) charakteryzował się nadmierną masą ciała. Wykazano istotny statystycznie związek pomiędzy wskaźnikiem BMI a stopniem niepełnosprawności. Konieczne wydaje się położenie większego nacisku na profilaktykę zdrowotną w programie studiów.
Background. The aim of the study was to assess the relationship between overweight, obesity, and back pain, and to verify the correlations between these variables in a group of young female physiotherapy graduates. Material and methods. The study group comprised 41 randomly selected physiotherapy third year students. Body height and body mass were measured, and BMI was calculated. Pain in the lumbar spine was assessed with the Oswestry Low Back Pain Questionnaire. Basic descriptive statistics were determined for the analysed quantitative variables. The questionnaire answers were compared and correlated with BMI. We used analysis of variance and the Levene’s test for homogeneity of variance to establish correlations between BMI and disability. Results. BMI analysis demonstrated that 29% of the students were characterized by excess body mass (17% overweight, 12% obese); mean BMI equalled 24,8 kg/m2. Furthermore, 78% of the respondents experienced back pain, with high and very high intensity complaints reported by 22%. Most students presented minimal (61%) and moderate (27%) level of disability. A statistically significant correlation was found between BMI and the degree of disability (Fdisability (2, 38) = 8.72; p < 0.001). Conclusions. Most physiotherapy students in the study group experienced back pain, and a substantial percentage (30%) were characterized by excessive body mass. A statistically significant correlation was demonstrated between BMI and disability level. It seems necessary to put more emphasis on health prevention in university curricula.

Introduction: Motorcycles in Poland become more and more popular also as convenient means of transport, especially in large cities. Factors influencing on driver during riding the motorcycle are assumed to cause spinal pain in motorcyclist. These factors may vary in motorcycles of different construction or purpose. Few information is available about spinal pain connected with riding a motorcycle. The aim of this study was the assessment of occurrence and intensity of spinal pain in motorcyclists depending on motorcycle type. Material and methods: The study group consisted of 2,124 motorcyclists aged 19-79 yrs., using motorcycle with an engine size bigger than 50 ccm and participating in minimum 1 riding season. An original, on-line available questionnaire composed of questions regarding riding a motorcycle and ailments of all parts of the spine was used. The results underwent statistical analysis using the Statistica 10.0. Results: The results showed that 58.4% respondents experienced pain of at least one part of spine during riding the motorcycle. The biggest percentage (51.1%) of persons with spinal pain in general was found out in cross/enduro type users. Neck pain was significantly connected with riding the sport motorcycle (p=0.001). Conclusions: Spinal pain in motorcyclists depends on the motorcycle type.

Introduction. Year after year, we spend an increasing amount of time in a sitting position. Often, we sit with poor posture, as indicated by numerous pain syndromes within the musculoskeletal system. Several reports confirm that body posture and the amount of time spent in a seated position have extensive implications for our health. Previous studies and a literature review suggest there is limited knowledge regarding an ergonomic sitting position. Objective. The aim of the study was to analyze the research relating to a proper sitting position and the consequences of incorrect sitting posture. A database search was conducted in Science Direct, Scopus, PubMed, Medline, and Google Scholar. Selection was made on the basis of titles, then abstracts and full texts of the studies. No limits were applied to the date of publication. Conclusions. Incorrect sitting posture contributes to many disorders, especially in the cervical and lumbar spine. It also determines the work of the respiratory system. Most authors suggest that maintenance of the physiological curvature of the spine is crucial for the biomechanics of the sitting position, as well as the location of the head and position of the pelvis. It raises awareness of work-related hazards and the introduction of education on the principles of proper seating. It is necessary to draw attention to the risks associated with work performed in a sitting posture, and education on the principles of ergonomical sitting.

Excessive physical activity is one of the main risk factors in the formation of degenerative changes of the intervertebral discs. Recreational and elite sport also represents a repeated increased physical load, and based on the type of sport, a more intense and sometimes less intense action of direct forces on the intervertebral discs. On the other hand, sport and training is in general regarded as beneficial for our health. Many doctors also regards sport as a form of prevention against back injuries. The authors analyze numerous sporting activities with different types of direct forces acting on the spine, with the goal to inform on realistic scientific facts regarding the impact of these activities on the degeneration of the intervertebral disc. They inform about the available facts, which confirm the positive effects of a certain type of sport load on the degeneration of the intervertebral disc, and its correlation with the clinically manifested back pain.

Introduction. Analysis of the medical literature shows that non-specific low back pain is a multifaceted affliction. Determining the unequivocal definition and classification of the ailment could be somewhat difficult. The following review presents a multiplicity of common low back pain nuances. The paper also shows necessity of unification of the definition and clarification, for placing non-specific low back pain among other musculoskeletal disorders. Aim. The author will attempt to provide the answers to basic questions about non-specific low back pain. In its form, the paper will have similarities to the prospect study with narrative review features. Although the reader should remember that the article is neither a result of expert team efforts nor non-specific low back pain leading authority opinion. Therefore the suggestions should be interpreted with necessary distance and scientific scepticism. Material and methods. Proper publications were searched in PubMed and EBSCO scientific articles databases, using terms: ‘nonspecific low back pain’ or ‘non-specific low back pain’, ‘definition’, ‘diagnostic triage’, and ‘classification’ in different combinations. Results. As a result of the review, subtle correction of the current non-specific low back pain definition has been proposed. Acknowledgments. Background: Low back pain is a common health problem among hospital nurses. However, the prevalence, characteristics, and work-related risk factors of low back pain have not been widely investigated in Taiwan. Materials and Methods: This study used a cross-sectional survey of 217 hospital nurses to gather self-reported information on the prevalence of back pain, demographic and pain characteristics, and work-related risk factors from 178 respondents who indicated a past history of back pain. The association between the characteristics of back pain and work-related risk factors was also examined. Results: The lifetime prevalence of back pain was 82.03%, and the point prevalence of back pain was 43.78%. The mean pain score is 41.67. The number of years at work was significantly associated with the pain score for an individual’s most recent episode of back pain, the extent of bothersomeness of back pain and leg pain, and the extent to which back pain interfered with normal work. Conclusion: Back pain is common among hospital nurses in Taiwan. Years at work are significantly associated with pain severity and disability caused by back pain.

Introduction and aim. This study compared the efficacy of core stabilization (CSE) and trunk balance exercises (TBE) with flexibility training on pain-related disability (PRD), psychological status (PS) and fear avoidance belief (FAB) in patients with non-specific chronic low back pain (NSCLBP). Material and methods. Twenty-eight (28) participants diagnosed of NSCLBP were randomly assigned into CSE, TBE, and control groups (CG). Participants in CSE (n=10); TBE (n=8) and CG groups (n=10) received core stabilization exercise, trunk balance exercise and back care advice respectively. All participants received flexibility training in addition to treatment in their respective groups. Assessment of outcomes were done at baseline, end of 4th and 8th week. Results. There was significant improvement in all outcomes in the CSE, TBE and CG at 8 weeks; PRD (p=0.005, p=0.008, p=0.005), PS: depression (p=0.005, p=0.008, p=0.007); anxiety (p=0.005, p=0.007) and FAB about work (p=0.005, p=0.007, p=0.005); about physical activity (p=0.005, p=0.018, p=0.006). Comparison of outcomes between CSE and TBE groups showed no significant difference (p>0.05). Conclusion. Both CSE and TBE with flexibility training are effective in improving PRD, PS and FAB of patients with NSCLBP.

Objectives This paper evaluates the efficacy of using the McKenzie and Vojta methods for patients with low back pain and the use of the DIERS Formetric 4D system as an objective diagnostic tool. Material and Methods The study enrolled 28 patients aged 15–17 years old. The patients were hospitalized at the Department of Orthopedics and Traumatology of the Świętokrzyskie Center for Pediatrics in Kielce with a diagnosis of back pain associated with a discopathy. The patients were rehabilitated according to the McKenzie and Vojta methods. Assessment by means of the DIERS Formetric system had taken place before the first therapy session and on the day that pain was eliminated to evaluate trunk inclination, angle of thoracic kyphosis, angle of lumbar lordosis, lateral deviation, trunk torsion, surface rotation and pelvic obliquity. Pain intensity and change in pain intensity over time were assessed by means of a numerical rating scale. Results Pain intensity was reduced to 0 over 3–12 days. The study participants demonstrated reduction in anterior trunk inclination of the mean value at 1.83°. The angle of thoracic kyphosis was also reduced by 7.95°. The angle of lordosis increased by 7.6°. The lateral spinal curvature was reduced by 8.92 mm. There was a reduction of 4.64° in trunk torsion. Surface rotation was reduced by 1.61° and pelvic obliquity was reduced by 3.78°. Conclusions In discopathic patients, postural parameters comprising trunk inclination, angle of thoracic kyphosis, angle of lumbar lordosis, lateral deviation, trunk torsion, vertebral rotation and pelvic obliquity fail to reach Hartzmann’s physiological reference ranges. A therapeutic intervention based on the Vojta and McKenzie methods may normalize the posture to physiological reference ranges and is effective in the treatment of patients with back pain. The DIERS system is an objective tool for tracing the effects of therapy in patients with back pain. Int J Occup Med Environ Health. 2019;32(1):33–41

Background: Extracorporeal Shock Wave Therapy (ESWT) has become a popular tool to treat musculoskeletal disorders and chronic low back pain. Aim of the study: To review the current scientific literature and assess the utility of ESWT in treating chronic low back pain. Material and methods: This systematic review was conducted from November 2019 to January 2020. Its purpose was to determine what the effectiveness is of the various forms of ESWT for the treatment of chronic low back pain. The critical review of the literature on the use of ESWT in chronic low back was made using the scientifically recognized medical databases PubMed, MEDLINE, Physiotherapy Evidence Database (PEDro) and Web of Science Core Collection. There was no restriction by date. Exclusion criteria were experimental, in vitro, animal, review, case reports, non-randomized clinical trials or studies with healthy participants. All articles written in languages other than English have also been excluded. Results: Six studies were included in the final analysis. According to the applied PEDro classification, the average scoring for the studies was 4.83, which indicates overall low quality of the presented reports. However, this result appeared closer to the moderate (acceptable) quality range (6-8 points) than to the unacceptable range (0-2 points). Conclusions: Based on the findings in the analyzed articles, ESWT promises to be an efficient and useful procedure in chronic low back pain treatment. Unfortunately, the level of evidence is relatively weak because there are a limited number of published studies related to ESWT and the final score in the PEDro classification was low. Together, these results indicate the need for further high quality randomized clinical trials.

Background Spinal overload among blue-collar workers occurs most frequently in the lumbar spine. Long-lasting spinal overload results in pain syndromes leading to limitations in everyday and professional life. Material and Methods The research included 106 adult males working as farmers for at least 10 years. The control group included 50 male white-collar workers. Four research tools were used: an interview questionnaire, a questionnaire assessing pain intensity and limitations in everyday functioning of study participants, a Numeric Rating Scale (NRS) measuring pain intensity and Oswestry Disability Index. Results The research revealed that as many as 86% of the farmers (91 individuals) experienced low back pain. In the control group only 64% of all the white-collar workers complained of pain in this part of the spine. The farmers defined their low back pain as constant pain or stiffness significantly more often. In addition, they reported hip pain radiating to one or 2 legs and experienced chronic and acute back pain more frequently. The total time in which they experienced this pain was usually 11–20 years. The correlation between pain intensity measured on a numeric scale and the level of disability of farmers was revealed. Conclusions The research revealed that individual farmers complained of low back pain significantly more often. The pain was usually defined as constant pain, chronic pain, acute pain or hip pain radiating to legs. Med Pr 2018;69(4):355–364

Occupational low back pain (LBP) remains a leading safety and health challenge. This cross-sectional investigation measured the prevalence of LBP in residential carpenters and investigated ergonomic risk factors. Ninety-four carpenters were investigated for LBP presence and associated risk factors. Ten representative job-tasks were evaluated using the Ovako Working Posture Analysis System (OWAS) and ErgoMaster™ 2D software to measure elements of posture, stress, and risk. Job-tasks were found to differ significantly for total lumbar compression and shear at peak loading (p < .001), ranging from 2 956 to 8 606 N and 802 to 1 974 N respectively. OWAS indicated that slight risk for injury was found in 10 job-tasks while distinct risk was found in 7 of the 10 job-tasks. Seven of the 10 job-tasks exceeded the National Institute for Occupational Safety and Health (NIOSH) action limit of 3 400 N for low back loading. The point prevalence for LBP was 14% while the annual prevalence was 38%.

Wstęp: Zespoły bólowe krzyża stanowią złożony problem medyczny oraz ekonomiczny. W związku z tym istnieje konieczność prowadzenia badań nad wyborem odpowiednich metod fizjoterapeutycznych skutecznych w zwalczaniu bólu kręgosłupa. Materiał i metody: Badania przeprowadzono w grupie 40 pacjentów poddanych tradycyjnej fizjoterapii i 40 pacjentów leczonych metodą Medical Taping. Oceniono stopień dysfunkcji przy użyciu zmodyfikowanego kwestionariusza Oswestry oraz natężenie subiektywnego bólu przy pomocy skali VAS przed leczeniem i zaraz po. Uzyskane wyniki poddano analizie statystycznej przy użyciu programu Statistica 6PL. Wyniki: Po zastosowaniu tradycyjnej fizjoterapii i metody Medical Taping uzyskano statystyczne (p<0,05) zmniejszenie się dolegliwości bólowych oraz stopnia niepełnosprawności pacjentów. Wnioski: Nie wykazano różnic statystycznie istotnych w porównaniu międzygrupowym po zastosowaniu obu form terapii. Otrzymane wyniki zachęcają do stosowania metody Medical Taping w rehabilitacji zespołów bólowych kręgosłupa.
Introduction: Back pain is a complex medical and economic problem. Therefore there is a need of research on appropriate and effective physiotherapeutic methods in the treatment of this disease. Material and methods: 40 patients treated with traditional physiotherapy and 40 patients treated with Medical Taping method participated in the study. The disability level was assessed on the basis of a modified Oswestra questionnaire and subjective pain level rated on the VAS scale before and after the treatment. The results were analysed with the Statistica 6.0 PL programme. Results: A significant decrease in the disability and pain levels was observed after the application of both treatments – traditional physiotherapy and Medical Taping method. Conclusion: There were no statistically significant differences between the two groups after application of both treatments. The results of the study show that the Medical Taping method can be beneficial in the treatment of back pain.

Introduction. Lumbar disc herniation (LDH) is a common cause of back pain and leg pain. For more than 60 years, standard discectomy by fenestration has been used to treat LDH. In his study, three commonly used scales were used to assess the outcome after microdiscectomy, such as the Low Back Pain Rating Scale (LBPRS), Oswestry Disability Index (ODI) and the Visual Analogue Scale (VAS). To the best of the authors’ knowledge, this is the first study analyzing treatment results at 3 months after microdiscectomy. Materials and method. During almost 5 years, a total of 327 patients underwent microdiscectomy operations for single level disc disease. Of these, 286 patients were available for 3-mounth follow-up. All patients were operated on at the Neurosurgery Department in the Specialist Hospital in Sandomierz, Poland. Before surgery and 3 months after burgery, all patients were asked to complete LBPRS, ODI and VAS scales. The statistical analysis used was p2, the effect size indicator for ANOVA. Results. Significant improvement was observed in all scales used 3 months after surgery. Conclusions. There are new, minimally invasive percutaneous techniques available, such as percutaneous laser disc decompression and intradiscal injection of radiopaque gelified ethanol. There are also trials with platelet-rich plasma or mesenchymal stem cells injected into the disc, aimed at the restoration of healthy disc properties. The hypothesis that metabolic products of bacteria propionibacterium acnes can contribute to disc degeneration inspired attempts at antibiotic therapy This study was conducted on a large group of patients and confirmed that discectomy is the recommended method of surgery which produces good treatment results in 3-mounths follow-up.

Objectives: Low back pain in pilots of military helicopters is widely discussed in the world literature. Vibrations and improper seat configuration cause that cervical, thoracic and lumbar spine might be exposed to overloading. The aim of the study was to determine the incidence and intensity of pain in the spine as well as to identify subjective risk factors of back pain and its effect on the pilot’s actions while flying a helicopter. Materials and Methods: 112 pilots, aged 25−56 years (mean age: 34.8±6.3 years), actively flying helicopters, participated in the questionnaire survey. The questionnaire containing five groups of questions, elaborated by the authors, was used. Results: 70% of the pilots reported pain complaints. Pain was localized in different parts of the spine. Uncomfortable body posture during prolonged flights resulting from the lack of lumbar support exerted the highest effect on pain provocation. Conclusions: This questionnaire survey enabled to determine the prevalence of pain, its type and intensity. Knowledge on low back pain imposes the necessity to eliminate the negative environmental effect on helicopter pilotage. Elimination of risk factors is possible through appropriate physical exercises and adjustment of pilots’ seats. The authors plan to conduct – in the future – detailed annual examinations among pilots with back pain and, based on the results, to elaborate a prophylactic program containing simple forms of isometric and isotonic training involving paravertebral and deep abdominal muscles designed specifically for pilots of military helicopters.

Objective: The purpose of this study was to examine the influence of gender and existing, recurrent low back pain (rLBP) on lower extremity and trunk mechanics, as well as neuromuscular control, during a lift task. Design: A multivariate design was used to examine the effects of gender and group on biomechanical and neuromuscular control variables in randomized symmetric and asymmetric lifting. Methods: 68 Males and females with rLBP and healthy performed symmetric and asymmetric weighted box lifting trials to a 1 meter height table. Results: Lifting style was different between gender and between the rLBP versus healthy groups during a 1m box lifting. A significant two-way interaction effect between gender and group was observed for multifidus muscle activity and knee rotation in asymmetric lifting. Several gender and group main effects were observed in pelvis obliquity, trunk flexion and side flexion, knee abduction angles in symmetric lifting; and in pelvis obliquity and rotation, trunk flexion and side flexion, hip abduction, knee abduction angles, external oblique and internal oblique muscles activity in asymmetric lifting. Conclusions: Females and individuals with rLBP appear to use different lifting styles that emphasize movement at the pelvis accompanied by poor kinematic control features at the hip, trunk and knee. Clinicians should be mindful of these changes when developing prevention and rehabilitation programs aimed at improving trunk control in preparation for lifting tasks during domestic and occupational activities.

The aim of this study was to assess the quality of life and health behaviours and to analyse the relationship between the quality of life and health behaviours in the group of patients with low back pain (LBP). Material and methods: The research was conducted on a sample of 52 patients with LBP (29 female and 23 male, M(age) = 48.96±15.86). The tool used for assessing the quality of life was the Quality of Life Questionnaire (WHOQoL-BREF, Polish version), whereas the methodological basis for investigating health behaviours was the Health Behaviour Inventory (HBI). Results: The analysis revealed that the General Index of Intensity of Health Behaviours (GIIHB) was high in 30.8%, average in 40.4% and low in 28.8% of the respondents. Statistically significant positive relationships were observed between: (1) Positive Thinking (HBI) and: Overall Quality of Life (R=0.42), General Health (R=0.29), Psychological domain (R=0.46) and Environmental domain (R=0.44); (2) between Proper Nutritional Habits (HBI) and Overall Quality of Life (R=0.38), Psychological domain (R=0.28), Environmental domain (R=0.30); (3) between Health Practices (HBI) and: Overall Quality of Life (R=0.31) and Psychological domain (R=0.28); (4) between Preventive Behaviours (HBI) and two domains: Psychological domain (R=0.34) and Environmental domain (R=0.34). GIIHB for the respondents in general was a factor that significantly differentiated quality of life in Psychological domain (p=0.031) and Environmental domain (p=0.026). Conclusion: In general, positive correlations concerning quality of life and health behaviours of the respondents were observed between the Psychological domain and all categories of health behaviours (HBI). Furthermore, positive correlations were found for Overall Quality of Life and Environmental domain with most of HBI categories. In light of these findings, it should be indicated that the focus during therapies for patients with LBP aimed to improve their quality of life should be on education in the field of health behaviours.

Objectives: The paper describes the case of a physical therapist with acute Low Back Pain (LBP) due to patient handling and the efficacy of Kinesio Taping (KT) around the trunk in the treatment of this occupational LBP. Materials and Methods: KT was applied around the trunk for 3 days, for an average of 10 h/day. Kinesio tape was applied with 130–140% stretch to the rectus abdominis, internal oblique, erector spinae, and latissimus dorsi muscles, which are activated in the process of lifting. Results: Following the KT application, the ‘Visual Analog Scale’ and ‘Oswestry Disability Questionnaire scores’ gradually decreased and active trunk range of motion limited by the LBP increased progressively. The physical therapist no longer complained of LBP and was able to handle patients without any pain. Conclusions: Hence, continuous application of KT around the trunk may be a supplementary treatment method for acute LBP in physical therapists and enable continuous patient handling without any loss of work time due to occupational LBP. In addition, KT may also be applicable for the prevention and treatment of occupational LBP in other professions involving lifting heavy objects.

Objectives: The purpose of this study was to investigate the relationship between daily physical activity (PA) level and low back pain (LBP) in young women. Material and Methods: Two hundred forty three female, desk-job workers aged 20–40 voluntarily participated in the study. The participants were assessed by the use of Oswestry Disability Index for measuring LBP disability and by the use of the short version of the International Physical Activity Questionnaire for PA assessment. The 1-way ANOVA test was used for comparing the mean values according to the physical activity level groups. Correlations between the average LBP disability score and all the other variables were obtained using Pearson’s correlation analysis. The level of statistical significance was p < 0.05. Results: Significant differences were found for LBP disability score between the results of 3 different PA groups (p < 0.05) (low, moderate and high PA groups). The correlation between the average LBP disability score and body weight (r = 0.187, p < 0.01), body mass index (r = 0.165, p < 0.01), vigorous MET score (r = 0.247, p < 0.01) and total PA MET score (r = 0.131, p < 0.01) were significant. Conclusions: The main finding of this study is that there is a U-shaped relationship between PA and LBP disability score in young women. A moderate level of daily physical activity and preventing body weight and fat gain should be recommended in young, female desk-job workers in order to prevent and manage low back pain.

Objectives: This study aimed to investigate the prevalence of Low Back Pain (LBP) among female nursing staff and explore the potential risk factors associated with LBP. Methods: An analytical cross-sectional study was conducted on randomly selected female nurses using payroll as a sampling frame in all public hospitals in Sana'a City, Yemen. Data was collected through face-to-face interview using a structured, pre-coded questionnaire that was available in Arabic and English. Weight and height of the nurses were measured using weight and height scales and body mass index was calculated. Multiple logistic regression was used to identify the factors associated with LBP. Results: Out of 696 female nurses selected, 687 (98.7%) responded. The life-time, the 12-month and one-week prevalence rates of LBP among female nurses were 512 (74.5%; 95% CI: 71.1-77.7%), 411 (59.8%; 95% CI: 56.0-63.5%) and 249 (36.2%; 95% CI: 32.6-39.9%), respectively. The prevalence was significantly lower in Indian nurses compared to other nurses. Three out of every 10 nurses with LBP had sick leave because of LBP in the last 12 months. Factors that showed significant association with LBP among nursing staff in the multivariate analysis were age, nationality, menstrual disorders and stress level at work. Conclusion: LBP is common among female nurses in Yemen. The role of menstrual disorders in developing LBP among female nurses seems to be important. Although sharing the same working conditions, Indian nurses were less likely to report LBP, which highlight the importance of cultural differences in willingness to report LBP.

Introduction. Exercises are important in the management of non-specific chronic low-back pain (NSCLBP). Aim. The study compared the effects of bridging and V-sitting exercises on pain and disability of patients with NSCLBP. Material and methods. 34 patients with NSCLBP recruited for the study were allocated into V-sitting (VSG) and Bridging Exercise group (BEG) equally. Participants in VSG and BEG groups performed V-sitting and bridging exercises respectively for 10 seconds, three times in a week for three weeks under a supervision of one of the authors. Each participants underwent ten sessions per a treatment regimen. Pain intensity and disability were assessed at the pre-intervention, second and third weeks using verbal rating scale and Rolland Morris Low Back Pain Disability Questionnaire prospectively. Data were analyzed using descriptive and inferential statistics, alpha level was set at 0.05 Results. There was a significant reduction in the third week (P < 0.001) in both VSG and BEG group of pain intensity and disability comparing the pre intervention, second and third week values. There was a significant reduction in the 3rd week VSG’s pain intensity (F=27.34 P<0.001) and disability (F=14.96, P<0.001) compared with BEG. Conclusion. V-sitting and bridging exercises were effective in management of patients with NSCLP, but V-sitting seems more effective.

Back pain is a common health problem. Natural degenerative processes reduce its strength, causing symptoms of sciatica, numbness, paraesthesia with muscle weakness and sensory surface, leading to muscle atrophy, inclusive. Degeneration of the intervertebral disc and hernia formation in the first place causing damage to the spinal cord and nerves and spinal facet joints and ligaments become a source of pain spondylogenic. In the case of advanced degenerative changes of the axial movement of the camera conservative treatment is sometimes ineffective, and the chronic nature of the disease and its neurological complications diminish the quality of life and often lead to disability. In order to reduce the time the patient recovers minimally invasive procedures are used, the effectiveness of which can sometimes vary depending on the location of the source of the pain. PURPOSE OF THE WORK is to present the possible neurological complications resulting from chronic spinal pain syndromes. MATERIAL AND METHODS: Data were from the PubMed database of the last 20 years, presented in a descriptive analysis. CONCLUSIONS: lower back pain make a huge therapeutic difficulties.

Opis:
Background: Sciatica carries a significant health burden in the world as the lifetime incidence of sciatica estimated between 13% to 40%. We used modern scientific tools to assess the online health interest of sciatica and lower back pain. Material and methods: Google Trends, Wikipedia statistics and PubMed data were used to gauge the online public interest in sciatica and lower back pain. Results: Over the last five years, (2015–2019) the Wikipedia page for sciatica has ranked high in all four categories it was included in; this shows that sciatica is a major concern for the public. Wikipedia page views on sciatica and low back pain show a 28% and 90% increase respectively from July 2015 to March 2019. In the last eleven years (2008–2019) Google Trends shows that sciatica has had a 2-fold increase in search frequency worldwide on the web, a 6-fold increase on YouTube and a 3-fold increase on Google images. Scientific interest in sciatica is low (only 140 PubMed publications in 2018). Conclusions: The public has a relatively high online interest concerning sciatica and back pain and this interest is rising. We suggest that hospitals clearly address the causes, pathophysiology, and management of sciatica to their patients to account for this increase.

Introduction: Spine pain syndrome, which significantly limits everyday functioning, is one of the most common symptoms of musculoskeletal disorders. The aim of this study is to assess the impact physical fitness has on the symptoms of back pain syndrome. Material Methods: A EUROFIT fitness testing battery was used to assess the fitness level of 34 participants with back pain. Measurements of aerobic fitness, lateral and sagittal mobility, strength, movement speed and precision were recorded, and the pain levels of the participants were taken into account. Results: No statistically significant impact of physical fitness on back pain was noted. Conclusions: Physical activity and fitness do not impact on the symptoms of back pain syndrome in any significant way. Further research into back pain syndrome is required in the future.
Wprowadzenie: Zespół bólowy kręgosłupa, który znacznie ogranicza codzienne funkcjonowanie, jest jednym z najczęstszych objawów schorzeń narządu ruchu. Celem pracy była ocena wpływu sprawności fizycznej na objawy zespołu bólowego pleców. Materiał i metody: Badaniami objęto 34 osoby cierpiące z powodu bólu pleców. W tym celu posłużono się baterią testów sprawnościowych EUROFIT. Pomiary wydolności tlenowej, ruchomości bocznej i strzałkowej, siły, szybkości i precyzji ruchu zostały zebrane i porównane z poziomem bólu uczestników badania. Wyniki: Nie stwierdzono statystycznie istotnego wpływu sprawności fizycznej na ból pleców. Wnioski: Aktywność fizyczna i sprawność fizyczna nie wpływają w znaczący sposób na objawy zespołu bólowego pleców. W przyszłości potrzebne są dalsze badania nad zespołem bólowym pleców.

Pain is a complex and challenging problem for patients with low back pain and for the psychologists and physiotherapist. One of the most common musculoskeletal pain problems is low back pain. Literature data allow us to assume that possible cause of decreased multifidus size include reflex inhibition and disuse atrophy. Increased fatigability (decreased endurance) of the multifidus muscle has been demonstrated in low back pain (LBP) patients in several research studies. Taking up physical activity belongs to one of the fundamentals of the therapy Formal exercise programmes used in conjunction with other methods of treatment and psychological therapy are one the main forms treatments in low back pain. Lifestyle, physical activity in particular is one on the factors influencing the health of youth and adults. Passiveness and apathy do not facilitate the persistence in realizing the aims of treatment.

Purpose: Low back pain (LBP) occurs among approximately 60-80% of the population, gradually contributing to long-term or recurring disability. We aimed to evaluate the efficiency of two physiotherapeutic treatments (namely, Träbert current and kinesiotherapy) among patients with LBP. Material and methods: Comprised of function tests and scales of pain and functionality, clinical tests were performed according to a prospective method in a regional hospital in Lomza. Forty patients randomly distributed into two groups: one group a subject to physical therapy with Träbert currents, the other group to kinesiotherapy following a proprietary programme. Physiotherapy was performed from Monday to Friday for two weeks in a series of ten treatments. Results: The treatment used in both groups significantly decreased pain and increased functionality of patients, evaluated by Laitinen’s survey, the Oswestry Disability Index (ODI), and the Roland Morris Disability Questionnaire. Results of the Schober’s and “fingertip-to-floor” tests also showed increased improvement. Conclusions: The physiotherapy facilitated a significant decrease in pain, an increase in the mobility in lumbosacral segment of the spine and a decrease in disability among patients with LBP.

Back pain is a symptom of a number of underlying diseases. The pain syndrome may be due to disorders of bone structures, apophyseal joints, ligaments, fascia, muscles or intervertebral discs, subsequently affecting or not the spinal cord and peripheral nerves. According to the pathophysiological mechanisms precipitating the pain, it can be classified as nociceptive or neuropathic pain. Nociceptive pain is precipitated by the mechanical irritation of nociceptors in ligamentous and muscular structures. Neuropathic pain is caused mostly by the compression of nerve roots. When acute nociceptive pain is not treated in a timely manner, it may develop into chronic neuropathic pain due to functional changes occurring in the central nervous system. Treatment of back pain involves a combination of myorelaxants, analgesics and non-steroidal anti-inflammatory drugs. It starts, as a first choice, with paracetamol because of a small number of its adverse effects. The combined administration of non-steroidal anti-inflammatory drugs and analgesics is preferable as it enhances the analgesic effect without increasing the toxicity. A similar combination is that of paracetamol with a mild opioid (tramadol). Neuropathic pain is influenced by several main groups of medications: antidepressants, anticonvulsants, membrane-stabilizing drugs, and GABA-B agonists. Early treatment of back pain is necessary in order to prevent pain chronification. Because of the different treatments of nociceptive and neuropathic pains, it is of particular importance to differentiate between them early.
Ból okolicy kręgosłupa to objaw towarzyszący wielu chorobom. Zespół bólowy mogą powodować nieprawidłowości struktur kostnych, stawów międzykręgowych, więzadeł, powięzi, mięśni czy krążków międzykręgowych, które następnie mogą negatywnie wpływać na rdzeń kręgowy i nerwy obwodowe. Na podstawie mechanizmów patofizjologicznych ból dzielimy na nocyceptywny i neuropatyczny. Ten pierwszy powstaje przez podrażnienie mechaniczne nocyceptorów w strukturach mięśniowych i więzadłowych. Z kolei ból neuropatyczny zwykle jest spowodowany uciskiem na korzenie nerwowe. Gdy ostry ból nocyceptywny nie jest leczony w odpowiednim czasie, może rozwinąć się przewlekły zespół bólowy spowodowany wtórnymi zmianami czynnościowymi w ośrodkowym układzie nerwowym. Leczenie zespołu bólowego kręgosłupa polega na podawaniu środków zwiotczających mięśnie, leków przeciwbólowych i niesteroidowych leków przeciwzapalnych. Lekiem pierwszego rzutu jest paracetamol ze względu na niewielką ilość reakcji niepożądanych. Preferowane jest leczenie skojarzone niesteroidowymi lekami przeciwzapalnymi i środkami przeciwbólowymi, ponieważ zwiększa efekt przeciwbólowy, nie nasilając toksyczności. W skojarzonym leczeniu stosowane jest połączenie paracetamolu z łagodnym opioidem (tramadolem). W przypadku bólu neuropatycznego podaje się różne grupy leków: leki przeciwdepresyjne, przeciwdrgawkowe, leki stabilizujące błonę komórkową i leki z grupy antagonistów receptora GABA-B. Ze względu na różne sposoby leczenia bólu nocyceptywnego i neuropatycznego ważne jest wczesne ustalenie rodzaju bólu, aby nie dopuścić do rozwoju przewlekłego zespołu bólowego.

Low back-pain is the most common pain syndrome. Radiofrequency facet denervation (RFD) is a minimally invasive method of treatment of chronic low back pain caused by facet joint syndrome. In this procedure electric current with frequency of radio waves are used to cause thermal injury to a small branch nerve that innervates painful facets. There are numerous studies confirming the effectiveness of this therapy. A good outcome of RFD can be sustained by appropriate and regular exercise. The aim of this article is to recommend such a program of exercises focused on strengthening the muscles and improving stabilization of the lumbar spine.

This investigation was conducted to determine the effects of a massaging lumbar support system on low back muscle activity. The apparatus included a luxury-level automobile seat, six 10-mm diameter bipolar surface electrodes, an amplifier, an anaiog-to-digital conversion board, data acquisition software, and a personal computer. Six experimental conditions, each involving a variation of massage time, were considered. The dependent variable was the change in the root mean square variation of the EMG signal. One minute of lumbar massage every 5 min was found to have a beneficial effect on low back muscle activity (as compared to no massage). This may prove to be an extremely important result in the quest to combat low back pain attributable to automobile seating.

Objectives Housewives are nucleus of families but as the working, living, and social architecture in the rural areas significantly differ from developed or urban area, the results of urban population cannot be generalized. Hence the purpose of the present study is to evaluate the prevalence of low back pain in non working rural housewives. Also an attempt has been made to determine the impact of social burden on low back pain (LBP). Material and Methods A sample of 301 non working rural housewives of Kanpur, aged between 30–70 years was selected. Hindi version of 3 appropriate scales Nordic musculoskeletal questionnaire, Oswestry disability index and Zarit burden interview measuring musculoskeletal discomfort, low back disability and social burden were given to all the housewives. Results Analysis of data reveals that both recent and yearly prevalence of LBP in rural housewives is 83%. More than 50% housewives have severe disability due to their LBP. Correlational analysis has shown that there is a significant impact of social burden on their disability due to low back pain. Conclusions The findings of the present study suggest that 83% of the non working rural housewives have low back pain and activity restriction due to their pain. They have significant impact of social burden on their low back pain. High prevalence (83%) of low back pain among rural housewives is an alarming sign for our society. Better health-care measures to enhance rural housewives education about good posture, ergonomic measures, health schemes, health awareness, and activity pacing could help rural housewives.

Objectives Increasing bilateral gluteus medius co-activation has been identified as one of the most important factors in developing low back pain due to prolonged standing in healthy people. This study aims to investigate the impact of an anti-fatigue mat on the bilateral gluteus medius co-activation pattern and to report the low back pain subjectively in 2 different standing positions on the normal rigid surface and on the anti-fatigue mat. Material and Methods While carrying out an easy simulated profession, 16 participants who had no low back pain background were requested to stand for 2 h in each position, with and without using the anti-fatigue floor mat, respectively. At the beginning of standing process and at every 15 min until the time of 120 min lapses, electric activities for the bilateral gluteus medius co-activation and subjective pain level in low back area were collected by the surface electromyogeraphy (EMG) and the visual analogue scale (VAS), respectively in each position. Results The obtained findings revealed that the anti-fatigue mat significantly decreased subjective pain level in low back area among 15 participants (p < 0.05). However, there was objectively no significant difference in the bilateral gluteus medius co-activation pattern among the participants between the position 1 and the position 2 (p > 0.05). The findings obtained under this study related to the impact of the anti-fatigue mat upon the low back pain based on the increase of > 10 mm on the VAS threshold, which showed that this intervention had no significant impact upon decreasing the number of patients suffering from the low back pain and also minimizing the bilateral gluteus medius co-activation in both pain developer groups (p > 0.05). However, 73% of the participants preferred to apply it. Conclusions It seems that the anti-fatigue mat may be useful in reducing the low back pain although it objectively didn’t significantly change the gluteus medius co-activation pattern related to the low back pain.

This study investigates changes in lumbar erector spinae (LES) muscle endurance, perceived low-back pain (LBP), and perceived exercise fatigue in older adults, and analyzes the trends of these changes during a 5-week lumbar exercise. Sixteen older adults with LBP were equally and randomly divided into two groups: the experimental group with incline-standing and the control group with the levelstanding positions. They were separately treated with lumbar exercise tasks and 10 seconds of muscle endurance tests using surface electromyography (sEMG). There was a trend of changes in both groups. The exercise tasks led to increase LES muscle endurance in the experimental group (53.7%) and the control group (45.4%) and decrease perceived LBP score significantly with the incline-standing position. There was no significant difference between the two groups in perceived exercise fatigue (p>0.05). Trunk flexion and extension with an incline-standing position can be an effective method to increase LES muscle endurance and reduce LBP in older adults.

Introduction. Exercises have been shown to relieve symptoms in non-specific chronic low back pain (NSCLBP) patients. Aim. This study compared the effects of cognitive behavioural therapy (CBT) and core stabilization exercises (CSE) on pain-related disability, psychological status and sleep disturbance in patients with NSCLBP. Material and methods. This randomized controlled trial involved a total of thirty-seven (37) participants. They were randomly allotted into three groups [CBT (11), CSE (14) and control (12)]. The intervention was done once per week for duration of 60 minutes for the CBT group, 30 minutes for CSE group and 10 minutes for the control group twice per week for 8 consecutive weeks. Assessment of outcome was done at baseline, 4 weeks and 8 weeks. Data were analyzed using statistical package for social science version 25 at alpha level of less than 0.05. Results. The results of this study showed that there was significant improvement in the level of pain-related disability (p= 0.001), level of anxiety (p =0.001), depression (p = 0.01, p = 0.001, p =0.001) and sleep disturbance (p = 0.001) in all the groups (CBT, CSE, control) post treatment. Conclusion. CBT and CSE are both effective in the treatment of pain-related disability, sleep disturbance, and psychological status of NSCLBP patients.

Idiopathic scoliosis (IS) is referred as a cause of a back pain in adults, probably on the basis of spinal overload and degeneration. Back pain in children and youths has been assessed only occasionally. The aim of the study was to determine the occurrence of back pain in adolescent with IS. MATERIAL AND METHODS Poll assessment and medical documentation analysis of 70 patients (65F and 5M) with IS was conducted. Mean age was 15 years (SD = 1.7), mean value of body mass index (BMI) was 18.7 (SD = 2.3), mean Cobb angle 26.4° (SD = 13.8), apical vertebra rotation (AVR) 13.4° (SD = 8.2), thoracic kyphosis 26.1° (SD = 12.2), lumbar lordosis 33° (SD = 9.4) lower limbs discrepancy 0.29 cm (SD = 0.64). Intensity of back pain was evaluated by visual analogue scale (VAS). RESULTS Back pain was present in 2/3 of subjects (65.7%). Intensity of pain was 1–7 points of VAS (mean 3.3, SD = 1.6). Pain arose usually in static positions. Cobb angle correlated positively with intensity of pain, reaching level of statistical signifi cance (r = 0.24, p < 0.05). No signifi cant correlation between AVR and pain severity was also observed (r = 0.13, p > 0.05), as well as between BMI and pain (r = 0.2, p > 0.05). Thoracic kyphosis (r = -0.2, p > 0.05), lumbar lordosis (r = -0.21, p > 0.05) and abbreviation of lower limb (r = -0.19, p > 0.05) revealed negative, insignifi cant correlation with intensity of pain. CONCLUSIONS Back pain occurs commonly in youths with idiopathic scoliosis. Statistically signifi cant positive relation between severity of back pain and Cobb angle was observed. Patients with IS and back pain should be diagnosed against other possible spine pathology.
Skolioza idiopatyczna (IS) jest częstą przyczyną występowania dolegliwości bólowych w dojrzałym wieku, prawdopodobnie na tle zmian przeciążeniowych oraz zwyrodnieniowych. Bóle grzbietu u dzieci i młodzieży znacznie rzadziej są przedmiotem badań. Celem pracy było określenie częstości występowania bólów grzbietu u młodzieży z IS. MATERIAŁ I METODY Przeprowadzono badanie ankietowe i poddano analizie dokumentację medyczną 70 pacjentów z IS (65 K i 5 M). Średnia wieku wyniosła 15 lat (SD = 1,7), średnia wartość wskaźnika masy ciała (BMI) 18,7 (SD = 2,3), kąta skrzywienia według Cobba 26,4° (SD = 13,8), kąta rotacji radiologicznej kręgu szczytowego (AVR) 13,4° (SD = 8,2), kifozy piersiowej 26,1° (SD = 12,2), lordozy lędźwiowej 33° (SD = 9,4), różnica długości czynnościowej kończyn dolnych 0,29 cm (SD = 0,64). Stopień nasilenia dolegliwości bólowych grzbietu określono za pomocą wzrokowej skali analogowej (VAS). WYNIKI Bóle grzbietu występowały u 2/3 badanych (65,7%). Stopień nasilenia bólu wahał się w zakresie 1–7 pkt VAS (średnio 3,3, SD = 1,6). Ból występował zwykle w pozycjach statycznych. Wielkość kąta Cobba korelowała dodatnio z nasileniem bólu, osiągając poziom istotności statystycznej (r = 0,24, p < 0,05). Zaobserwowano także słabą, nieistotną statystycznie korelację pomiędzy wielkością AVR i nasileniem dolegliwości bólowych (r = 0,13, p > 0,05) oraz BMI i nasileniem dolegliwości (r=0,2, p > 0,05). Wartości kifozy piersiowej (r = -0,2, p > 0,05), lordozy lędźwiowej (r = -0,21, p > 0,05) oraz skrótu kończyny dolnej (r = -0,19, p > 0,05) korelowały słabo ujemnie z nasileniem dolegliwości bólowych, również nie osiągając poziomu istotności statystycznej. WNIOSKI Dolegliwości bólowe grzbietu występują powszechnie wśród młodzieży z skoliozą idiopatyczną. Zaobserwowano istotną statystycznie zależność między nasileniem dolegliwości bólowych grzbietu a wielkością kąta Cobba. Pacjenci ze skoliozą idiopatyczną i bólem pleców powinni podlegać dalszej diagnostyce w celu wykluczenia innych przyczyn dolegliwości.

Objectives: This study aims to assess muscle fatigue for kitchen workers during various tasks in a kitchen. Material and Methods: This study consists of 2 phases: a) surface electromyography (EMG) to determine muscle fatigue of trapezius and erector spinae muscles bilaterally for 12 healthy male volunteers during 30 min of Indian flatbread (chapati) preparation, b) self-administered questionnaire study after 30 min preparation to determine the impact on performance. Results: Surface EMG results show that the right trapezius muscle had significantly higher fatigue during kneading, rolling and roasting tasks. Similarly, right erector spinae muscle showed significantly higher fatigue during rolling and roasting tasks. Conclusions: Both phases of the study conclude that, even though the dynamic postural adjustment may reduce fatigue during the work schedule; the combination of risk factors like the repetitive task, forceful exertion and prolonged standing during working results in a high risk of developing muscle fatigue. The aim was to evaluate if rehabilitation procedures including occupational health (OH) and workplace participation increase return to work (RTW) rates among patients with subacute and chronic low back pain (LBP). A systematic review of randomized controlled trials was conducted using the PubMed and Cochrane databases. Main outcomes were RTW and days of sick leave. Interventions needed to be multidisciplinary including both OH and active workplace involvement in rehabilitation. Out of 1073 potentially eligible references, 8 met the inclusion criteria. Three studies had OH and 5 case managers involved in rehabilitation. Rehabilitation involving both OH and workplace improved RTW and decreased the number of sick leave days among LBP patients. Having case managers involved had no effect in RTW. In order to improve RTW, workplace visits and work ability meetings (WAMs) between OH and workplace are essential components in the rehabilitation process among patients with chronic LBP. Based on the study results, the authors suggest utilizing these co-operative interventions with workplaces in OH. High quality research investigating only the effect of WAMs in OH setting is needed in future.

Psychologically and physically straining work conditions increase the risk of low back pain (LBP) development. According to recent recovery theories, leisure time recovery can counteract the negative influences of work stress on health. Similarly, a recent empirical work has indicated a moderating role of recovery on the link between stress and health issues. In this paper, a theoretical model is deduced to integrate the moderating effects of recovery on work stress and LBP development. Based on theoretical and empirical results, 2 separate recovery paths are distinguished: on the one hand, recovery can prevent the experience of stress because a well-recovered person can cope more easily with work demands; while on the other hand, recovery refills the depleted resources after confrontation with work strain and reduces stress experiences. Given that work strains is a main risk factor for LBP development, recovery in leisure time seems to be a highly relevant aspect, which has not been investigated to date in the field of LBP.

Background. The Back School program has been recommended in many countries around the world for patients with low back pain (LBP) to help improve self-efficacy to enhance their prognosis. However, few studies have reported on the application of the Back School in East Asia, including China. This study aimed to explore the Back School’s effect on Chinese adults with chronic LBP based on four areas: posture, knowledge of LBP, physical activity and body performance. Material and methods. There were 10 participants in the intervention group and 11 in the control group. Baseline data was collected prior to the intervention, including upper body physical examination, core and lower limb muscle examination, Roland Morris Disability Questionnaire, LBP Knowledge Questionnaire and Global Physical Activity Questionnaire. Physical indicators and questionnaires were retaken after the 8-week Back School intervention. The differences between the two groups were compared before and after the intervention. Results. There was a statistically significant increase in McGill trunk flexion test results and knowledge of LBP (especially basic knowledge and treatment sections) in the intervention group. Conclusions. The Back School-based intervention model has a positive impact on muscle performance in the core area and knowledge acquisition of LBP in Chinese patients with chronic LBP.
Wprowadzenie. Program „Back School” jest zalecany w wielu krajach na całym świecie w przypadku pacjentów z bólem dolnego odcinka kręgosłupa (LBP) w celu poprawy skuteczności i polepszenia rokowań. Jednak niewiele badań dotyczyło zastosowania programu „Back School” w Azji Wschodniej, w tym w Chinach. Niniejsze badanie miało na celu określenie wpływu działania programu „Back School” na dorosłych Chińczyków z przewlekłym LBP w oparciu o cztery obszary: postawę, wiedzę na temat LBP, aktywność fizyczną i wydolność organizmu. Materiał i metody. W grupie interwencyjnej było 10 uczestników, a w grupie kontrolnej 11. Przed rozpoczęciem interwencji zebrano dane wyjściowe, w tym badanie fizykalne górnej części ciała, badanie mięśni tułowia i kończyn dolnych, kwestionariusz niepełnosprawności Rolanda-Morrisa, kwestionariusz wiedzy na temat LBP i globalny kwestionariusz aktywności fizycznej. Wskaźniki fizyczne i kwestionariusze zostały wypełnione ponownie po trwającym 8-tygodni programie „Back School”. Różnice między obiema grupami porównano przed i po interwencji. Wyniki. W grupie interwencyjnej odnotowano istotny statystycznie wzrost wyników testu zgięcia tułowia McGilla i wiedzy na temat LBP (zwłaszcza w części wiedzy podstawowej i dotyczącej leczenia). Wnioski. Model interwencji oparty na programie „Back School” ma pozytywny wpływ na wydajność mięśni w obszarze rdzenia i zdobywanie wiedzy na temat LBP u chińskich pacjentów z przewlekłym LBP.

Introduction: Physical activity is considered to be one of the keystones of a healthy lifestyle and an indispensable condition for correct body posture. The purpose of the study was to check the body posture of young males with above-average physical activity and how the position of the trunk changes after extending the arms forward. The frequency and intensity of back pain among males in the study were also analyzed.Material and methods: The study involved 50 young, healthy males, whose high daily physical activity was confirmed by the IPAQ questionnaire. Body height and weight were checked, and BMI was calculated. The position of the trunk in three planes was examined using the ultrasonic Zebris Pointer system. The examination was performed twice: in the habitual standing position and in the position taken from the Matthiass test: standing position with the arms in front of the trunk (90o). The data was prepared in the Statistica v13 program.Results: The male participants were characterized by an incorrect depth of physiological curvatures of the spine and asymmetry of  the position of the shoulder and pelvic girdles in the frontal plane. Extending the arms forward in a standing position resulted in flatter thoracic kyphosis, rounder lumbar lordosis and tilting the trunk backwards. Periodic pain sensation in the lumbar spine was reported by approximately 60% of participants. It was a mild pain that did not require painkillers. There were no significant relationships between the quality of the trunk position and pains in the lumbar region. Conclusions: High physical activity turned out to be insufficient to ensure correct body posture and efficiency of core muscles. Postural education should be a permanent element of health promotion programs aimed at increasing the quality of life.
Wstęp: Aktywność fizyczna jest uważana za jeden z filarów zdrowego trybu życia i nieodzowny warunek prawidłowej postawy ciała.Celem pracy było sprawdzenie, jaka jest postawa ciała młodych mężczyzn o ponadprzeciętnej aktywności fizycznej i jak zmienia sięustawienie tułowia po wyciągnięciu ramion w przód. Analizie poddano również częstotliwość i nasilenie bólu kręgosłupa u badanychmężczyzn.Materiał i metody: Badaniu poddano 50 młodych, zdrowych mężczyzn, których wysoką codzienną aktywność fizyczną potwierdzonowynikiem kwestionariusza IPAQ. Zbadano wysokość i masę ciała, obliczono wskaźnik BMI. Ustawienie tułowia w trzech płaszczyznach zbadano za pomocą ultradźwiękowego systemu Zebris Pointer. Badanie wykonano dwukrotnie: w pozycji stojącej nawykowej oraz w pozycji zaczerpniętej z testu Matthiassa: pozycja stojąca z ramionami ułożonymi przed tułowia (90o zgięcia). Dane opracowano w programie Statistica v13.Wyniki: Dla badanych mężczyzn charakterystyczna była nieprawidłowa głębokość krzywizn fizjologicznych kręgosłupa oraz asymetria ustawienia obręczy barkowej i biodrowej w płaszczyźnie czołowej. Wyciągnięcie ramion w przód w pozycji stojącej  powodowało spłycenie kifozy piersiowej, pogłębienie lordozy lędźwiowej i odchylenie tułowia ku tyłowi. Okresowe odczuwanie bólu kręgosłupa lędźwiowego zgłosiło około 60% badanych. Był to ból o nieznacznym nasileniu, który nie wymagał zażywania środków przeciwbólowych. Nie zaobserwowano znaczących zależności pomiędzy jakością ustawienia tułowia a dolegliwościami odcinka lędźwiowego.Wnioski: Wysoka aktywność fizyczna okazała się niewystarczająca do zapewnienia prawidłowej postawy ciała i wydolności mięśniposturalnych. Edukacja posturalna powinna być stałym elementem programów prozdrowotnych, mających na celu zwiększenie jakości życia.

INTRODUCTION: Back pain syndromes constitute a serious health problem. They most often affect people between 40 and 60 years of age, but it also happens that they appear before the age of 30. They are a frequent problem in nurses. The etiology of these ailments is multifactorial. The main causes are physical stresses on the spine: lifting, a forced posture, and obesity. Human neurophysiological predispositions and social conditions also play an important role. The study attempts to determine whether the respondents know the cause of their ailments, and what the prevention of this disease, knowledge of the principles of ergonomics and pro-health measures should be. MATERIAL AND METHODS: In this work, the frequency of back pain syndromes in nurses in two hospitals in the Podkarpackie region was observed. The author’s own questionnaire and anthropometric research were used. RESULTS: Pain occurred in 89% of the respondents, and the main causes of these ailments were physical stress, overweight and low physical activity. The respondents knew the principles of ergonomics, but did not use them at work, and the length of service was not statistically significant. The lack of training and appropriate equipment was noted. Stress factors had an effect on back pain only in 13% of the respondents. CONCLUSIONS: Back pain occurs in 90% of the surveyed nurses in hospitals in the Podkarpackie region and has the features of an occupational disease. The respondents know the principles of ergonomics and express their willingness to deepen their knowledge on this subject. This knowledge should be supplemented with the basic understanding of active physiotherapy and neurophysiology due to the role of psychological factors in the treatment of chronic pain.
WSTĘP: Zespoły bólowe kręgosłupa są ogromnym problemem zdrowotnym. Dotyczą najczęściej osób pomiędzy 40 i 60 rokiem życia, ale zdarza się, że występują jeszcze przed 30 rokiem życia. U pielęgniarek są powszechnym zjawiskiem. Etiologia tych dolegliwości jest wieloczynnikowa. Zasadniczą przyczyną są obciążenia fizyczne kręgosłupa: dźwiganie, wymuszona postawa ciała oraz nadwaga. Istotną rolę odgrywają też predyspozycje neurofizjologiczne człowieka i uwarunkowania społeczne. W pracy podjęto próbę ustalenia, czy osoby badane znają przyczynę swoich dolegliwości, wiedzą, jaka powinna być profilaktyka tego schorzenia, znają zasady ergonomii oraz działania prozdrowotne w tym zakresie. MATERIAŁ I METODY: W pracy określono częstość występowania zespołów bólowych kręgosłupa u pielęgniarek w dwóch szpitalach na Podkarpaciu. Posłużono się autorską ankietą i badaniami antropometrycznymi. WYNIKI: Ból występował u 89% badanych, a zasadniczą przyczyną dolegliwości były obciążenia fizyczne, nadwaga i niska aktywność fizyczna. Osoby badane znały zasady ergonomii, ale nie wykorzystywały ich w pracy, a staż nie miał tu statystycznie istotnego znaczenia. Odnotowano brak szkoleń i odpowiedniego sprzętu. Jedynie u 13% badanych czynniki stresowe miały wpływ na dolegliwości bólowe kręgosłupa. WNIOSKI: Ból pleców występuje u 90% badanych w szpitalach na Podkarpaciu i ma cechy choroby zawodowej. Respondenci znają zasady ergonomii i wyrażają chęć pogłębienia swojej wiedzy na ten temat. Wiedzę tę należy uzupełnić o podstawowe wiadomości z zakresu aktywnej fizjoterapii i neurofizjologii ze względu na rolę czynników psychologicznych w leczeniu przewlekłego bólu.

Objectives Recovery processes in leisure time influence the effect of psychosocial work factors on health issues. However, this function of recovery has been neglected in research regarding the influence of work-related risk factors on low back pain (LBP) development. The aim of this prospective study was to examine the function of psychological detachment – a relevant recovery experience – concerning the influence of psychosocial work factors on LBP development. A moderating function of detachment for the interplay of work factors and LBP was assumed. Material and Methods Sixty pain-free administrative employees of German universities completed an online survey 3 times during a 6-month period. Generalized estimating equations were used to estimate risk-factors of LBP. Results Analyses revealed an increased chance of LBP development for smokers and a decreasing chance when work resources were high. Detachment had no direct influence on LBP development, although it moderated the influence of work stressors and work resources on LBP. On the one hand, high detachment values seem to protect against an increased chance of LBP development when employees were confronted with high work stressors, while on the other hand high detachment values enhance the protective effect of high work resources. Conclusions The results indicated a moderating role of detachment concerning the influence of psychosocial work factors on LBP development. Therefore, it is necessary to include recovery processes in future research regarding LBP development and consequently in LBP prevention concepts. Int J Occup Med Environ Health 2017;30(2):313–327

The aim of the study was to analyze the risk factors of back pain in M.A. students. The study involved 150 physical therapy students from the University School of Physical Education (AWF) in Krakow and 100 applied information technology (IT) students from the Cracow University of Technology. The study group was determined on the basis of dean’s lists and quota sampling. The study was conducted using a proprietary questionnaire. The results showed that the level of knowledge on prevention of back pain in physical therapy students is greater than in science students. Physiotherapy students experience back pain more often than IT students. The physical activity level of physical therapy students is higher than the IT students. The type of activity may signifi cantly affect the occurrence of back pain in students. The presented results may be indicative of the significant effect of stress on the occurrence of pain among students. Cite this article as: Pasiut S., Osińska A. Celem badań była analiza czynników ryzyka bólów kręgosłupa u studentów studiów magisterskich. W badaniu wzięło udział 150 studentów fizjoterapii AWF w Krakowie i 100 studentów informatyki stosowanej Politechniki Krakowskiej. Badaną grupę określono na podstawie list dziekańskich i doboru kwotowego. Badanie przeprowadzono za pomocą autorskiego kwestionariusza ankietowego. Wyniki wykazały, iż poziom wiedzy profilaktyki bólów kręgosłupa u studentów fizjoterapii jest większy niż u studentów informatyki. Studenci fizjoterapii częściej doznają bólów kręgosłupa niż studenci informatyki. Aktywność fizyczna studentów fizjoterapii jest większa niż studentów informatyki. Charakter zajęć, może w istotny sposób wpływać na występowanie dolegliwości bólowych kręgosłupa u studentów. Przedstawione wyniki mogą wskazywać, iż stres może w istotny sposób wpływać na występowanie dolegliwości bólowych wśród studentów.

The objective of this study was to prevent occupational musculoskeletal injuries. Our workers stacked boxes of ceramics weighing 10–27 kg, making low back pain common in our enterprise. In all the stacking stations, recommended weight limits (RWL) were separately calculated using the revised National Institute for Occupational Health lifting equation. Since the boxes weighed significantly more than the RWL, we developed a new ergonomic design that completely changed the stacking process. The load put on the workers’ waist vertebrae in the new and the old stacking methods was compared to evaluate the success of the new ergonomic design, using Newton’s third law of motion. Thanks to the new ergonomic design, the load on the workers’ vertebrae decreased by 80%. Due to its simple technology and its very low cost compared to robots, the new ergonomic design can be commonly used in enterprises with repeated and constraining stacking.

Wstęp: Do przyczyn powstawania zespołów bólowych kręgosłupa we współczesnym społeczeństwie zaliczamy zbyt małą aktywność fizyczną, nadwagę, brak elementarnej wiedzy na temat prawidłowych pozycji kręgosłupa. Do tego dochodzą jeszcze zmiany patologiczne tj. uszkodzenia i zwyrodnienia chrząstek międzykręgowych, pourazowe bóle kręgosłupa, czy wady postawy ciała. Materiał i metody: Na podstawie badań ankietowych przeprowadzonych w grupie 240 pacjen tów (120 kobiet i 120 mężczyzn) starano sie znaleźć odpowiedzi na pytania: w jaki sposób masa ciała, stopień aktywności fizycznej, charakter pracy zawodowej i poziom wykształcenia wpływa na częstość występowania zespołów bólowych kręgosłupa w nawiązaniu do wieku pacjentów. Ponadto ankietowani wskazywali na najbardziej urazowe odcinki kręgosłupa i podawali różne, stosowane przez nich formy leczenia. Wyniki i wnioski: Osoby z nadwagą częściej doświadczają zespołów bólowych kręgosłupa. Umiarkowana aktywność fizyczna wpływa korzystnie na profilaktykę bólu kręgosłupa. Wraz z wiekiem zwiększa się ryzyko występowania zespołów bólowych kręgosłupa. Najczęściej dolegliwości bólowe występują w odcinku lędźwiowym. Głównymi stosowanymi formami leczenia są: zażywanie ogólnodostępnych środków przeciwbólowych, bierny odpoczynek, a w następnej kolejności korzystanie z usług lekarza, fizjoterapeuty i masażysty. Powyższe stwierdzenia dotyczą zarówno kobiet, jak i mężczyzn. 
Intodruction: The most common reasons for back pain in modern society include inadequate physical activity, obesity and lack of basic knowledge on the correct positions of the spine. There are also other reasons such as pathological changes including damage and degeneration of annular fibers in the spinal disk, posttraumatic back pain and incorrect posture. Materials and methods: On the basis of a survey study on the population of 240 individuals (120 males and 120 females) we aimed to answer the following questions: how body index, physical activity, type of profession and education level correlate with the frequency of back pain in relation to age. Furthermore, responders reported the most commonly injured spine regions and types of treatments. Results and conclusions: Obese individuals more often suffer from back pain. Moderate physical activity is beneficial on spine prophylaxis. Aging correlates with higher risk of back pain. Back pain is the most commonly reported in the lumbar region. Common painkiller medications and restricted physical activity represent main treatments of back pain. Visiting a doctor, physiotherapy and therapeutic massage are less common. These conclusions hold true for both males and females.

Opis:
The submitted study addresses the weight of school bags as one of the risk factors of poor posture in pupils during mandatory school attendance. The research group included 680 children aged 6 to 14. The weight of the school bag was measured during one workweek from Monday to Friday. The results showed that many early school age pupils are overburdened with an inappropriate school bag weight. The weight of the empty school bag that exceeds the permitted weight according to the Czech national standard has a significant effect on the total weight of the school bag. Teachers should participate in the remedy of the situation. They should not only look for possible solutions, but also educate the parents of the pupils in this matter.

Objectives: To elucidate the association between metabolic syndrome (MetS) and disability due to low back pain (LBP) among care workers. Material and Methods: This cross-sectional study enrolled 656 care workers having experienced LBP in the year prior to the year of this study. The Roland-Morris Disability questionnaire (RDQ) and self-reported questionnaires regarding LBP, fear of movement, depressive symptom, psychosocial factors, intensity of pain, and duration of pain were administered, and a medical examination was performed. Metabolic syndrome was defined according to the international definition agreed in 2009. Out of the 656 care workers, we included 316 care workers (response rate: 48.2%) who had fully completed the questionnaires as the study sample (males: 13.6%, median age = 51 years old, range: 35–74 years old). To examine the association between MetS and the level of disability due to LBP, we used the Poisson regression analysis and estimated crude and adjusted prevalence ratios (PR). Results: Out of the 316 care workers, 52 (16.5%) were diagnosed as having MetS. Metabolic syndrome was significantly associated with the RDQ score (adjusted PR: 1.57, 95% confidence interval (CI): 1.17–2.11) after adjusting for covariates, such as age, sex, fear of movement, job demands, social support, intensity of pain, and duration of pain. Conclusions: This study showed that MetS was independently associated with disability due to LBP among care workers. A multidisciplinary intervention taking MetS into consideration may be an effective way to reduce disability due to LBP in people with both LBP and MetS. Back pain is a symptom presented in a broad range of disorders. Blurred or non-specyfic clinical presentation, excessive information from laboratory tests and image techniques, numerous therapeutic options of poor efficiacy evidence, explain difficulties in diagnosing and management of a person with low back pain. According to autor’s clinical observation there is a large number of back pain patients referred to specialized treatment with unrecognized cause and character of main and accompanying disorders. The paper presents an original pattern of diagnostic and therapeutic proceeding with patient apearing with back pain. Important and often neglected aspects of adequate diagnostic and therapeutic proceeding are discussed. The pattern is based on author’s clinical experience and literature review. The algorithm defines a logical consequence of proceeding and validates base for drawing clinical conclusions for an adequate therapeutic program design. Author believes that clinical proceeding according to the presented pattern could facilitate determination of a proper diagnosis and diminish the risk of incorrect treatment consequences.
Zespoły bólowe kręgosłupa są wbrew pozorom trudne w diagnozowaniu i leczeniu. Decydują o tym niespecyficzne objawy kliniczne i czasami bardzo bogate w treści badania dodatkowe. Okazuje się też, że czasami zbyt mało mamy informacji dotyczących rzeczywistego działania leczniczego niektórych zabiegów stosowanych w fizykoterapii, stąd pojawia się problem oceny skuteczności prowadzonego leczenia. W pracy przedstawiono propozycję algorytmu diagnostyczno-leczniczego w zespołach bólowych kręgosłupa. Omówiono najistotniejsze zagadnienia związane z diagnostyką i leczeniem bólów kręgosłupa. W algorytmie zwraca się uwagę na kolejność postępowania w zależności od uzyskanych informacji dotyczących diagnostyki, a także wskazano na najczęściej stosowane zabiegi terapeutyczne. Algorytm ma ułatwić postawienie rozpoznania choroby i ograniczyć do minimum błędy wynikające z niewłaściwej diagnozy, a co za tym idzie niewłaściwego leczenia. Obserwacje kliniczne dowodzą, że zbyt często rozpoznanie zespołów bólowych kręgosłupa traktuje się ogólnikowo, nie wnikając w szczegóły dotyczące określenia chorej struktury wywołującej objawy chorobowe.
