Background: The effect of sleeping positions during acclimatization to high altitude on Acute Mountain Sickness (AMS) and High Altitude Pulmonary Edema (HAPE) is unknown. We tested whether sleeping with the upper body raised by 5° reduces prevalence and severity of symptoms of AMS as well as of elevated pulmonary artery systolic pressure (PASP) values as a risk factor of HAPE. Methods: Randomly assigning trekking tourist volunteers n = 44 (25 m, 19 f; mean age 42.9 yr) sleeping at 4280 m or 5170 m to the experimental group (upper body elevated by 5°), or to the control group. After exclusion of other reasons for AMS-like symptoms those assumed to be related to AMS were rated by Lake Louise Score questionnaire in the evening and the following morning of the study. Transthoracic echocardiography was performed on both occasions to estimate PASP. Results: In the study group, symptoms of AMS were significantly reduced in younger subjects (p = 0.021), prevalence of AMS was reduced in women (p = 0.156), and PASP values were significantly reduced in older subjects and men (p = 0.032; p = 0.031 respectively). Conclusion: Results suggest that sleeping with the upper body in elevated position during a high altitude ascent may benefit those suffering from AMS or at risk of HAPE due to elevated PASP values.

In this study, the Soret and Dufour effects in a composite stenosed artery were combined with an analysis of the effect of varying viscosity on copper nanofluids in a porous medium. Blood viscosity, which changes with temperature, is taken into account using the Reynolds viscosity model. The finite difference approach is used to quantitatively solve the governing equations. For use in medical applications, the effects of the physical parameters on velocity, temperature and concentration along the radial axis have been investigated and physically interpreted. The results are graphically displayed and physically defined in order to facilitate comprehension of the various phenomena that occur in the artery when nanofluid is present. It is observed that the Soret effect increases the rate of heat transfer but decreases the rate of mass transfer. The new study enhances knowledge of non-surgical treatment options for stenosis and other abnormalities, hence reducing post-operative complications. Additionally, current research may have biomedical applications such as magnetic resonance angiography (MRA), which provide a picture of an artery and enable identification of any anomalies, and thus may be useful

INTRODUCTION: The paper discusses the techniques of modifying the atherosclerotic plaque in the left main coronary artery along with the results of the author’s own procedures using a scoring catheter (AngioSculpt) and a non-compliant (NC) balloon catheter. MATERIAL AND METHODS: 23 patients who met the inclusion criteria in the absence of exclusion criteria were enrolled in the study. 13 patients underwent coronary angioplasty using a scoring balloon catheter, and 10 patients underwent coronary angioplasty (percutaneous coronary intervention – PCI) with a NC balloon catheter. RESULTS: The groups did not differ in terms of demographics or comorbidities. In the study group, a larger diameter of the left main (LM) trunk was obtained, as assessed by the intravascular ultrasound (IVUS) technique. In the control group, the two-stent technique was used more often; however, due to the small number of participants, statistical significance was not achieved. In the control group, greater use of contrast was associated with more frequent use of the two-stent technique. Despite one myocardial infarction (MI) related to the procedure in the control group, the 2-year mortality rate between the groups was similar. Logistic regression did not reveal any of the studied variables to be significantly related to 2-year mortality. CONCLUSIONS: The preliminary results of our study suggest that there is no difference in early 30-day results and 2-year mortality in a randomized group of patients undergoing coronary angioplasty using a scoring balloon catheter and a non-compliant catheter. Selection of the optimal treatment strategy in LM stenosis is still a matter of research and the operator’s experience is of fundamental importance.
WSTĘP: W pracy omówiono techniki modyfikacji blaszki miażdżycowej w pniu lewej tętnicy wieńcowej wraz z przedstawieniem wyników własnych zabiegów z użyciem cewnika nacinającego (AngioSculpt) oraz cewnika niepodatnego (non-compliant balloon – NC balloon). MATERIAŁ I METODY: Do badania włączono 23 pacjentów spełniających kryteria włączenia przy braku kryteriów wy-łączających z badania. U 13 pacjentów przeprowadzono angioplastykę (percutaneous coronary intervention – PCI) z użyciem cewnika nacinającego, u 10 pacjentów z użyciem cewnika balonowego niepodatnego. WYNIKI: Grupy nie różniły się pod względem danych demograficznych ani chorób współistniejących. W grupie badanej uzyskano większą średnicę pnia lewej tętnicy wieńcowej (left main – LM) w ocenie za pomocą ultrasonografii we-wnątrznaczyniowej (intravascular ultrasound – IVUS). W grupie kontrolnej częściej stosowano technikę dwustentową; jednak ze względu na małą liczbę uczestników nie osiągnięto istotności statystycznej. W grupie kontrolnej użycie większej ilości kontrastu wiązało się z częstszym stosowaniem techniki dwustentowej. Pomimo jednego zawału mięśnia sercowego (myocardial infarction – MI), związanego z zabiegiem w grupie kontrolnej, śmiertelność 2-letnia między grupami była podobna. Regresja logistyczna nie wykazała istotnego związku żadnej z badanych zmiennych z 2-letnią śmiertelnością. WNIOSKI: Wstępne wyniki naszego badania sugerują, że nie ma różnicy we wczesnych wynikach 30-dniowych oraz 2-letniej śmiertelności w randomizowanej grupie pacjentów poddawanych angioplastyce wieńcowej pnia lewej tętnicy wieńcowej z użyciem cewnika balonowego nacinającego i cewnika balonowego niepodatnego. Wybór optymalnej strategii leczenia zwężenia LM jest nadal przedmiotem badań, a doświadczenie operatora ma fundamentalne znaczenie.

INTRODUCTION: : Coronary artery disease (CAD) leads to mortality and morbidity globally. Approximately, 18 million deaths occurred due to cardiovascular diseases. Objective: To determine the association between knowledge about coronary angiography and anxiety level among patients undergoing coronary angiography. MATERIAL AND METHODS: Analytical cross-sectional study was conducted in two major public-sector hospitals between September 2017 to March 2018. A total of 264 patients were recruited in the study Rusing a non-probability convenience sampling technique. A modified questionnaire about coronary angiography was distributed among participants and the HADS scale was used to assess the anxiety level of the participants. SPSS version 22 was used for data analysis. RESULTS: The mean knowledge score of the participants about angiography was 17.88 ± 4.047. The total knowledge score of both hospitals was good knowledge (6%), fair knowledge (42%) and had poor knowledge (52%). Regarding anxiety level, the participants of both hospitals experienced a high level of anxiety i.e. mild (20%), moderate (34%) & sever (10%). A significant association was found between knowledge and anxiety levels among the participants of both hospitals with a p-value of <0.001. CONCLUSIONS:This study found a significant association between patient’s knowledge about coronary angiography and anxiety level among patients undergoing coronary angiography. The study puts forwards that nurses shall assess the patient’s knowledge before coronary angiography procedure. The provision of accurate knowledge renders categorical purpose to this procedure for salubrious patients’ outcomes.

Fast-tracking in cardiac care refers to the complex intervention including early extubation, care during anesthesia, mobilization and hospital discharge to reduce perioperative morbidity, costs, and length of stay in the intensive care unit and the hospital. This review was designed to evaluate early extubation protocols, the diefrences in early and late extubation, the safety and efficacy of early extubation among the patients in surgical intensive care aeftr coronary artery bypass graft (CABG) surgery. The analyzed studies showed many significant diefrences in the mortality and postoperavtie complicaotins of mtie-directed extubaotin praccties and low dose-based general anesthesia in patients with low to moderate risk undergoing early extubation (fast-track) and the conventional care methods. Diefrent fast-track interventions could diminish extubation time, costs, and the length of hospital stay. However, several factors including patient's stay in the intensive therapy ward vs general ward, patient selection, skills and experience of the sta,f and fast-track anesthesia methods could be considered to perform safe fast-tracking in patients undergoing cardiac surgery. On the other hand, to achieve this safety for high-risk cardiosurgery patients multidisciplinary coordination is needed.

The petrosal and neighboring bones of the early Eocene palaeoryctid mammal Eoryctes melanus are described in tympanic and endocranial views based on CT scan data of the holotype. A second cranium of E. melanus has fragments of an osseous bulla, which have been interpreted as possibly formed by an independent entotympanic. The CT scans of the holotype reveal that the medial bullar wall is formed by an expanded rostral tympanic process of the petrosal, but the element(s) in the bullar floor remain unknown. The CT scans also allow for a comprehensive reconstruction of the cranial arterial and venous system. The arterial pattern differs from that in early eutherians by the absence of the arteria diploëtica magna and the bifurcation of the end branches of the stapedial artery dorsal to the tympanic roof. The venous pattern includes a large frontal diploic vein arising from the dorsal sagittal sinus on the midline and running through the frontal bone in a canal. Comparisons are made with other palaeoryctids, various Paleogene mammals (pantolestids, leptictids, apternodontids, apatemyids, and cimolestids) and the extant lipotyphlan Solenodon paradoxus. For the last taxon, the structure of the piriform fenestra and associated arteries is detailed. Cranial features support the monophyly of palaeoryctids and suggest possible lipotyphlan affinities.

The present paper analyzes an unsteady magnetohydrodynamic blood flow model of an visco-elastic fluid through an inclined porous stenosed artery with body acceleration and slip effect. Navier-Stokes equations have been used to describe the blood flow model. The governing equation of blood flow is solved by an analytic method by considering blood as an incompressible, visco-elastic fluid, and suspension of RBC’s in plasma. Axial velocity, blood acceleration, flow rate, and shear stress are derived numerically by using the finite Laplace and Hankel transformation and their inverse. The effect of parameters such as the visco-elasticity parameter, Womersley number, Hartmann number, inclination angle, parameter of slip, and body acceleration frequency is analyzed. Axial velocity reduces as the Hartmann number and visco-elasticity parameter enhance and it enhances with the enhancement of the slip parameter and inclination angle. The study is beneficial for finding the effect of slip parameter, porosity factor and Hartmann number when a human body is exposed to MRI and CT scan.

Introduction. Although there are lots of varieties of aorta arch, they seem to appear relatively rarely. Anomalies of aortic arch departures are mainly concerned with its location, course, the place of departure and number of its main branches. However, they warrant attention due to their importance in operative, diagnostic, and endovascular procedures. Aim. We want to present here a case of a female cadaver with rare aortic arch origin of the left vertebral artery. The aim of this article is to complete the frame of anomalies in aortic arch and to explore rare variability shown in this specific case. Description of the case. The present report describes an anomalous case of the left vertebral artery arising from the aortic arch between the left common carotid artery and the left subclavian artery in a female cadaver during dissection in an anatomical laboratory. Aortic origin of the vertebral artery is a rare anatomic variant. Conclusion. Thorough knowledge of anomalous origin is important for patients who undergo operation of an aortic arch or inferior part of the neck. Normally, the vertebral artery arises from the first part of the subclavian artery on both sides.

Coronary bifurcation remains a unique region of the coronary tree. The specific anatomy and blood flow determine the complex mechanisms of atherosclerotic plaque location. The treatment strategy should be chosen with caution as the failure of percutaneous coronary interventions (PCI) may expose a significantly larger area of the myocardium to ischemia than in the case of single vessel PCI. In order to understand the complexity of the clinical situation in patients after the treatment of coronary bifurcation, and the constantly evolving techniques of the procedure itself, this review presents basic information on the anatomy and classification, up-to-date step-by-step analysis of the optimal technique for PCI, beginning with qualification, planning and preparation for the procedure, through stent selection and ending with optimization techniques. Different PCI strategies are presented and explained, the indications for one- vs. two-stent techniques are evaluated, the available and recommended imaging and physiological methods for detailed bifurcation assessment are reviewed. Particular attention has been paid to left main stem bifurcation, a distinctive anatomical and physiological spot within the coronary circulation.
Bifurkacja wieńcowa jest miejscem szczególnym w obrębie krążenia wieńcowego. Charakterystyczna anatomia oraz cechy przepływu krwi w złożony sposób wiążą się z rozmieszczeniem blaszek miażdżycowych. Wybór strategii leczenia zmian bifurkacji wieńcowych wymaga szczególnej uwagi, gdyż wszelkie powikłania w tym rejonie narażają na niedokrwienie istotnie większy obszar miokardium niż w przypadku pojedynczego naczynia. Niniejsza publikacja stanowi przegląd podstawowych informacji o anatomii, klasyfikacji oraz kolejnych krokach aktualnie zalecanych technik leczenia przezskórnego zmian bifurkacji wieńcowych, poczynając od kwalifikacji, przygotowania, poprzez wybór stentu, po optymalizację efektu zabiegu. Podkreśla kliniczną złożoność problemu leczenia zmian bifurkacji wieńcowych, która wymusza stały rozwój technik zabiegowych. Przedstawione zostały strategie leczenia, wskazania do wyboru techniki z użyciem pojedynczego stentu oraz zamierzonego zastosowania dwóch stentów. Zaprezentowano również zalecane metody obrazowej oraz czynnościowej oceny istotności zmian w obrębie rozwidlenia tętnic wieńcowych. Szczególną uwagę poświęcono odrębnościom bifurkacji pnia lewej tętnicy wieńcowej.

This introductory essay revisits the multidimensionality of the river conceived of as a system of "communicating vessels," both literally and metaphorically. Drawing upon fine arts, poetry, biology, and philosophy, the argument organizing this text presents the river as a non-human, albeit often anthropomorphized, subjectivity, and serves to remind the reader of the universality of the neverending flow of essence and thought. Moored to the tides, humankind depends on the flow understood both in terms of the circulation of the ever-changing matter, but also in terms of the circulation of values. A human subject, dependent on other (not necessarily human, and not necessarily animate) subjectivities, recognizing the importance of the river as a living artery and as a principal agent of change, re-discovers the necessity to adopt the position of stewardship rather than that of ownership with respect to the world-in-flux that he or she inhabits.

The aim of the present study was to analyze the bilateral MCAs in patients with unilateral aneurysms. Material and methods: Images of computed tomography angiography of 45 patients were analyzed. The morphometric parameters of MCA were measured. The measured parameters of the normal MCAs were compared with MCAs containing aneurysms located in the main trunk division. Results: No significant differences in the length of trunk and angles between the middle and anterior cerebral arteries were found between the non-affected and aneurysmatic MCAs. Concerning arteries with aneurysms, more branches originating from the main trunk division were found more frequently. The coexistence of the more frequent trifurcation of the MCA and an aneurysm constitutes evidence of greater anatomical variability of this arterial area in cases with a concomitant vascular malformation. Conclusions: This finding is consistent with literature data, showing the relationship between aneurysms and arterial variability and the fact that aneurysms are most often located at arterial divisions.

Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome-coronavirus type 2 (SARS-Cov-2), injures multiple organs including the central and peripheral nervous system. We report the case of a 68-year-old patient diagnosed with COVID-19 infection in its moderate form who, 1 week after the onset of respiratory symptoms, developed a large acute ischemic stroke (AIS) in the area of the left middle cerebral artery (LMCA). The case shows that large artery stroke can occur even in moderate forms of COVID-19, and stresses the need for correct coagulation management. The source of cerebral stroke should also be sought by cervical computed tomography angiography (CTA) from the level of the aortic arch, and not only covering the cerebral vessels as thrombo-embolic events may appear already in large cervical vessels, even in mild forms of the disease.

Opis:
Introduction. Isolated common iliac artery aneurysms (CIAs) are rare. The rarer are rupture of the common iliac artery. We can treat them surgically: classic or endovascular. Open surgery has a main role in the management of aneurysms not suitable for repair by endovascular techniques. Aim. The aim of the study is to present the possibility of occurrence of such a disease at a young age, to indicate non-characteristic symptoms that may predict the above-mentioned disease entity at the level of a primary care physician or SOR. Review of medical publications from 2000-2019 regarding guidelines or methods of conduct in that cases. Description of the case. A patient 39 years old, admitted to the Department of Vascular Surgery, in an interview: about 15 years ago severely beaten, as a result of injuries total loss of hearing, memory and basic skills such as: speech, writing, reading, hospitalized over 6 months. Conclusion. Good rehabilitation and patient persistence restored cognitive skills. Advances in surgical has lowered the mortality and morbidity rates even in patients.

Background: Transfemoral access is regarded as the TAVI gold standard for the transcatheter aortic valve implantation (TAVI) procedure. However, other options for vascular access have developed in the last few years. Access via the carotid artery is one such alternative. Materials and methods: The study included 9 elderly patients who underwent transcarotid TAVI procedure at the Cardiac and Vascular Surgery Department of the Medical University of Gdańsk. Procedures were performed by a local Heart Team in a hybrid operating room under general anesthesia. Data was collected before the implantation and at discharge. Results: The mean patients’ age was 81 years of age (64-88) and the mean logistic EuroSCORE was 10.8 (7-16). Implantations were performed with 100% device success rate. Intra-operative valve-in-valve procedure was performed in one patient; there were no access-related and valve-related complications during the surgery. Post-procedural complications included minor bleeding, hematoma and pneumothorax. Echocardiographic parameters were significantly improving after the procedure. The mean hospital stay was 5 days (2-7 days). Conclusions: Transcatheter aortic valve implantation via the carotid artery appears to be safe and effective alternative to standard TAVI vascular access.
