Introduction: The patients with the aortic abdominal aneurysym of 55 mm in diameter are qualified to surgery. There are open repair (OR) by means of the vascular prosthesis implantation or the less invassive endovascular method by means of the stentgraft implantation through femoral arteries incision (EndovascularAorticRepair – EVAR). Aim: The aim of the study was the evaluation of the postoperative course in patients operated due to aortic abdominal aneurysm and the evaluation of the impact of the surgical method on the short-time results. Material and methods: 124 patients operated due to abdominal aortic aneurysm in Dept of Surgery of 4th Military Hospital in Wroclaw in 2014 were enrolled into the study: 53 patients with OR, 53 patients with EVAR, and 19 patients with ruptured aneurysm. Results: Mortality was 0% in EVAR and 6% in OR and 39% in ruptured aneurysm. Time of hospital stay was 5.8 days in EVAR vs 10 days in OR. The stay in ICU was 0% in EVAR vs 13% in OR. Blood transfusion was 9.4% in EVAR vs 66% in OR. Time of postoperative analgesia was 27 h in EVAR vs 76.8 h in OR. Cardio-respiratory decompensation was 1.9% in EVAR vs 7.6% in OR. Renal insufficiency was 2% in EVAR vs 9% in OR. The lower rate of organ complications was in EVAR. The ruptured aneurysm presented the most complicated postoperative course: hospital stay of 11.4 days, ICU stay of 78%, blood transfusion of 100%, painkillers of 136 hours, cardio-respiratory decompensation of 81% and renal insufficiency of 69%. Conclusions: The method of treatment, the conditions of the admision and the type of surgery influenced the postoperative course. The elective EVAR patients presented both the 0% of mortality and the lightest postoperative course. The ruptured abdominal aortic aneurysms operated as an emergency had the most complicated postoperative course.

In the current case report we present a novel case of a successful coil embolization of the left internal carotid artery aneurysm. The patient presented with neck pain and a palpable pulsating tumor and was admitted to the vascular surgery clinic where an angio-CT scan of the neck was performed. Angio-CT revealed a left internal carotid artery aneurysm with a narrow neck. The patient was admitted to the department of vascular surgery where she was enrolled into endovascular coil embolization. After the procedure, control angiography showed complete embolization of the aneurysm. Three months following the procedure, doppler ultrasonography of the carotid arteries showed no demonstrable flow into the aneurysm. Six months following the procedure, angio-CT confirmed complete aneurysm thrombosis. Based on this case, endovascular coil embolization of the carotid artery aneurysms is a safe and effective method of treatment.

Introduction. Bilatereal saccular cerebral aneurysms (SCAs) that developed symmetrically on the same named vessels are defined as mirror aneurysms and account for a small subset of multiple cerebral aneurysms. The internal carotid artery (ICA) bifurcation is a rare location for mirror aneurysms. Aim. We aimed to present the importance of risk status assessment for SCAs and screening in all ADPKD patients for timely detection and managementof SCAs before catastrophic complications occur. Description of the case. We present mirror aneurysms of bilateral ICA bifurcation that appear like a couple of dancing men on coronal computed tomography angiography (CTA) images, which were successfully treated with single stage coil embolization in a 45 year old female patient with medical history of autosomal dominant policystic kidney disease (ADPKD). Conclusion. SCAs are more frequent in patients with ADPKD than in general population and also these aneurysms are more likely to rupture at earlier ages. Mirror aneurysms of ICA terminus can be treated effectively and safely by single stage coil embolization.

Autosomal dominant polycystic kidney disease is the most common genetic cause of renal failure. Apart from kidney involvement, patients are at risk of extra-renal manifestations, including vascular lesions. The etiology of vascular changes is diverse and depends, among other factors, on polycystin gene mutation, increased activity of the renin-angiotensin-aldosterone system and the occurrence of hypertension. The observed vascular system complications include cerebral artery aneurysms, cervico-encephalic arteries' dissection, aortic aneurysm and dissection and intracranial arterial dolichoectasia. This article discusses the etiopathogenesis, symptomatology, principles of prevention and treatment of the aforementioned diseases of the vascular system accompanying polycystic kidney disease.

More than half of patients with subarachnoid hemorrhage die from the initial bleeding or its consequences, mainly vasospasm. However, within the six weeks after the bleeding onset, 40% of cases are fatal for patients. The main risk factors include: smoking, hypertension, alcohol, atherosclerosis, and body overweight. The paper presents a case study of rehabilitation and care for a 58 - year old patient after subarachnoid hemorrhage and surgery of the aneurysm of anterior communicating artery Patient’s difficulties in everyday functioning result from the current health condition and from the consequences of the post-operative treatment. The success in improving patient’s physical abilities depends on a systematic and long-term process of healing-care-rehabilitation and full commitment of the whole team. The aim of the study is to present the process of rehabilitation at home of the patient after surgery of the aneurysm of the anterior communicating artery and the effectiveness of activities performed by the therapeutic team.
Ponad połowa chorych po krwawieniu podpajęczynówkowym umiera z powodu pierwotnego krwawienia lub jego następstw, głównie skurczu naczyniowego. Natomiast w ciągu sześciu tygodni od wystąpienia krwawienia 40% przypadków kończy się zgonem pacjenta. Podstawowymi czynnikami ryzyka są: palenie papierosów, nadciśnienie tętnicze, alkohol, miażdżyca naczyń, nadwaga ciała. W pracy przedstawiono studium przypadku rehabilitacji i opieki nad 58 - letnim pacjentem po krwotoku podpajęczynówkowym i operacji tętniaka tętnicy łączącej przedniej. Trudności w codziennym funkcjonowaniu pacjenta wynikają z obecnej sytuacji zdrowotnej oraz konsekwencji przebytego zabiegu pooperacyjnego. Warunkiem sukcesu w usprawnianiu chorego jest systematyczny i długotrwały proces leczniczo – pielęgnacyjno - rehabilitacyjny z pełnym zaangażowaniem całego zespołu. Celem pracy jest przedstawienie procesu rehabilitacji w domu pacjenta po operacji tętniaka tętnicy łączącej przedniej oraz efektywności działań zespołu terapeutycznego.

Although visceral artery aneurysms are rare, mortality due to their rupture is high, estimated at even 25–75%. That is why it is significant to detect each such lesion. Visceral artery aneurysms are usually asymptomatic and found incidentally during examinations performed for other indications. Autopsy results suggest that most asymptomatic aneurysms remain undiagnosed during lifetime. Their prevalence in the population is therefore higher. The manifestation of a ruptured aneurysm depends on its location and may involve intraperitoneal hemorrhage, gastrointestinal and portal system bleeding with concomitant portal hypertension and bleeding from esophageal varices. Wide access to diagnostic tests, for example ultrasound, computed tomography or magnetic resonance imaging, helps establish the correct diagnosis and a therapeutic plan as well as select appropriate treatment. After a procedure, the same diagnostic tools enable assessment of treatment efficacy, or are used for the monitoring of aneurysm size and detection of potential complications in cases that are ineligible for treatment. The type of treatment depends on the size of an aneurysm, the course of the disease, risk of rupture and risk associated with surgery or endovascular procedure. Endovascular treatment is preferred in most cases. Aneurysms are excluded from the circulation using embolization coils, ethylene vinyl alcohol, stents, multilayer stents, stent grafts and histoacryl glue (or a combination of these methods).
Tętniaki tętnic trzewnych należą do rzadkich patologii. Śmiertelność z powodu ich pęknięcia jest wysoka, szacowana nawet na 25–75%, dlatego też wykrywanie każdego przypadku odgrywa ważną rolę. Tętniaki tętnic trzewnych są zazwyczaj asymptomatyczne i znajdowane przypadkowo podczas badania wykonywanego z innych wskazań. Badania autopsyjne sugerują, że większość bezobjawowych zmian pozostaje niezdiagnozowana za życia, a tym samym częstość występowania tętniaków trzewnych w populacji jest wyższa. Manifestacja pękniętego tętniaka zależy od jego lokalizacji i może przebiegać jako krwotok wewnątrzotrzewnowy, krwotok do przewodu pokarmowego oraz układu wrotnego z towarzyszącym nadciśnieniem wrotnym i krwawieniem z żylaków przełyku.Szeroki dostęp do takich metod diagnostycznych jak ultrasonografia, tomografia komputerowa czy też obrazowanie metodą rezonansu magnetycznego pozwala na ustalenie właściwego rozpoznania oraz zaplanowanie i wybór odpowiedniego leczenia. Po zabiegu metody te umożliwiają ocenę skuteczności postępowania, a w przypadkach niekwalifikujących się do leczenia służą do monitorowania wielkości tętniaków i wykrycia ewentualnych powikłań. Rodzaj leczenia zależy od rozmiarów tętniaka, przebiegu choroby, ryzyka pęknięcia oraz ryzyka związanego z leczeniem operacyjnym lub wewnątrznaczyniowym. Metodą z wyboru w większości przypadków jest zabieg wewnątrznaczyniowy. W celu wyłączenia tętniaków z krwiobiegu wykorzystywane są spirale embolizacyjne, EVOH (ethylene vinyl alcohol), stenty, stenty „gęsto plecione”, stentgrafty, kleje histoakrylowe (lub połączenie tych metod).

The aim of the study was to thoroughly evaluate the closure device ProStar XL in terms of its efficiency and safety in the percutaneous endovascular treatment of abdominal aortic aneurysms in the infrarenal section of the body. Additionally, it was crucial to assess if there were any occurrences of regional complications at any step of the procedure. It was also important to stipulate the estimated hospitalization period as well as the overall cost of the PEVAR treatment with the use of ProStar XL. Material and methods. The analysis included 21 cases with PEVAR performed in the infrarenal region. The final success was achieved in 98.2% of the cases. One PEVAR case (that constituted 0.2% of this group) was unsuccessful because of the bleeding while the placement of the sutures with the use of ProStar XL was being performed. Results. This resulted in the emergency treatment of the CFA with the continuous stitch (Prolene 5-0). During the postoperative period the above described patient was given 2 units of the Packed Red Blood Cells (PRBC). However, this complication (unexpected bleeding) did not influence the length of the hospitalization period in any significant way. Conclusions. Percutaneous vascular closure device ProStar XL used in the treatment of the common femoral artery (CFA) constitutes a necessary and safe supplement for Endovascular Aortic Aneurysm Repair (EVAR). The implementation of ProStar XL closure device proves to significantly shorten the hospitalization period after the EVAR treatment. Additionally, the safety of the entire procedure is exponentially linked to the experience of the operating surgeon.

In the subject literature there are only a few articles devoted to the Health-Related Quality of Life (HRQoL) of patients with large ex- tracranial aneurysms of the carotid artery segment. There are no reports on the quality of life of patients with rare Mega-Giant Carotid Artery Aneurysms (MGCAA) manifesting themselves as chronically large and growing neck tumors. The aim of the research was an evaluation of the health related quality of life (HRQL) of a patient following aneurysmotomy of Mega-Giant Carotid Artery Aneurysms (MGCAA). An 82-year-old patient was referred to the Department of Vascular Surgery at the John Paul II Hospital in Krakow, due to the presence of a megagiant, painful tumor covering the entire left side of the neck. A physical examination and angio-CT confirmed the pres- ence of a pulsating tumor, which extended vertically from the level of the angle of the mandible to the clavicle, and horizontally from the trachea to the cervical spine. An aneurysmotomy and recon- struction of LICA with cerebral protection using a shunt catheter was performed. The patient was awakened from anesthesia and extubated shortly after the procedure, without any Central Nervous System neurological defects. The surgery was complicated by left recurrent laryngeal nerve paresis and aphonia without dyspnoea, which was only partially resolved around 3 months after the oper- ation itself. The postoperative period was uncomplicated. Health related Quality of Life (HRQoL) was studied using the 36-Item Short Form Health Survey (SF-36) to study both the clinical symp-toms and how these symptoms would be reduced in a year follow up after the aneurysmotomy. Despite the increasing availability of endovascular options and techniques, open surgical repair of ICA aneurysms, especially Mega- Giant Carotid Artery Aneurysms (MGCAA), using cerebral protection (shunts), remains a basic option, providing an opportunity to avoid major intraoperative and postoperative complications. The aneurysmotomy, despite laryngeal nerve paresis and aphonia without dyspnoea, improves the patient’s health-related quality of life.

The article describes a case of a patient with aphasia, diagnosed with symptomatic epilepsy seizures after aneurysm clipping. A speech therapy diagnosis was made, consisting of patient observation, analysis of clinical documentation and speech test results. The subject was diagnosed with disorders of linguistic competence and skills caused by the presence of acoustic-mnestic aphasia. Their consequences are dysfunctions in terms of communication and interaction skills and abilities.

Introduction and aim. Aneurysms and arteriovenous malformations (AVMs) are both vascular abnormalities that can occur simultaneously or separately and increase the risk of intracerebral hemorrhage. The aim of this research was to characterize patients with intracranial aneurysms and AVMs. Material and methods. This retrospective research was based on analysis of Digital Subtraction Angiography. The research group consisted of 118 patients. StatSoft STATISTICA 13 was used for the statistical analysis of parameters, such as dimensions and vascularization pattern of vascular abnormalities. Results. The total number of aneurysms described was 168. In addition, 33 patients with cerebral aneurysms were also diagnosed with cerebral AVMs. The average dimensions of the aneurysms were 6.33mm, 95% CI (5.47 7.18) × 4.76mm, 95% CI (4.09, 5.43). The abnormalities were found in the extent of circulation of ACA (anterior cerebral artery) 50 (29.76%), ICA (internal carotid artery) 48 (28.57%), MCA (middle cerebral artery) 26 (15.48%), PCA (posterior cerebral artery) 23 (13.69%). There were 76 (64%) patients with diagnosed single aneurysms and 42 (36%) with multiple. Conclusion. Aneurysms are more likely to occur singly than multiply. The majority are located within the vascularization of the ACA. Aneurysms can coexist with AVMs, which increases the risk of rupture.

The study presented a case of a 29 year-old male patient with a pseudoaneurysm of the splenic artery as a complication of acute pancreatitis. The pseudoaneurysm was incidentally diagnosed during control angio-CT. The patient underwent immediate surgery due to deterioration of his clinical condition. During laparotomy the pseudoaneurysm, spleen and part of the pancreatic tail were excised. The postoperative course proved uneventful and the patient was discharged from the hospital after seven days.

Tętniaki aorty brzusznej (AAA, abdominal aortica neurysm) stanowią jedną z głównych przyczyn wykonywania zabiegów naczyniowych. W procesie destrukcji ściany naczynia i powstania tętniaka istotną rolę spełniają metaloproteinazy. Metaloproteazy macierzy posiadają zdolność trawienia kolagenu, fi bronektyny, elastyny, lamininy. Wzrost aktywności metaloproteinaz powoduje destrukcję macierzy pozakomórkowej. W następstwie oddziaływania ciśnienia tętniczego na osłabioną ścianę aorty brzusznej dochodzi do jej dylatacji i powstania tętniaka. W wielu pracach doświadczalnych i klinicznych wykazano wzrost ekspresji genów dotyczących metaloproteinaz macierzy zewnątrzkomórkowej w ścianie tętniaków aorty brzusznej. Uzyskane wyniki służą do porównania z wynikami ekspresji genów metaloproteinaz w innych tkankach lub zdrowej aorty. Dokładne poznanie roli metaloproteinaz w patogenezie tętniaków aorty brzusznej może zostać wykorzystane w profilaktyce i leczeniu tętniaków aorty.
The abdominal aortic aneurysms (AAA) constitute one of the main reasons for performing a vascular surgery. The metalloproteinases play an important role in the process of the aorta wall destruction and the aneurysm formation. The matrix metalloproteinases posses the ability to digest collagen, elastin, fibronectin and laminin. The increased activity of the matrix metalloproteinases causes the destruction of the extracellular matrix. In consequence of the blood pressure on the damaged wall of the abdominal aorta, it becomes dilated and the formation of the aneurysm occurs. In a series of experimental and clinical works an increased expression of genes for matrix metalloproteinases in the wall of abdominal aortic aneurysms has been demonstrated.. These results are used to compare with the results of metalloproteinase gene expression in other tissues or the healthy aorta. The precise knowledge of the role of MMPs in the pathogenesis of abdominal aortic aneurysms can be used in the prevention and treatment of aortic aneurysms.

Although many researchers have made the assumption that the abdominal aortic aneurysym (AAA) wall behaves as an incompressible and isotropic material, the experimental evidence for it is insufficient. Hence, the assumptions about the incompressibility and isotropy of the AAA wall were verified through analysis of stretch ratios of samples excised from the aneurysms walls. The stretch ratios were calculated on the basis of a real-time analysis of geometric dimensions of samples subjected to uniaxial tension. It was proved that the walls of abdominal aortic aneurysms can be modelled as an incompressible and isotropic material. Using histological techniques, the assumption concerning the negligence of shear stress in the analysis of AAA wall stresses was indirectly validated. The results were incorporated into a hyperelastic constitutive equation.

This study was undertaken to verify whether simvastatin modulates Cav-1/eNOS expression, and if this modulation is associated with changes in pro- and anti-inflammatory cytokine and Toll-like receptor 4 (TLR4) level in abdominal aortic aneurysm (AAA). It is a 1:2 case-control study of non-statin (n=12) and simvastatin-treated patients (n=24) who underwent open AAA repair. Simvastatin treatment decreased Cav-1 (p<0.05) and increased eNOS expression (p<0.01) in the AAA wall. These changes might be dose dependent. The changes in Cav-1 and eNOS were associated with a trend towards decreased IL-6 and IL-17 concentration (p>0.05) and increased IL-10 concentration (p=0.055); however, TLR4 expression was unaffected, suggesting that simvastatin influences Cav-1 and eNOS in the AAA wall by other mechanisms. Simvastatin may modulate Cav-1 and eNOS expression in the aneurysmal wall, indicating a potentially beneficial role for statins in AAA patients.
