Opis:
Oyster mushroom (Pleurotus ostreatus) is one of the most widely consumed mushrooms in the world. Its spores are highly potent allergens, but their main allergen, Pleo, has so far been very rarely reported. In indoor farms, fungal spores are often found to be floating in the air. This study presents a case of a non-atopic, healthy 32-year-old woman who opened an oyster mushroom farm with her husband. During the first harvest, after 30-minute exposure, she experienced dyspnoea. Similar symptoms occurred several more times while on the farm. A month later, during packing and sorting mushrooms, after 10 min, she again felt dyspnea, accompanied by weakness, rapid pulse, and skin itching with urticaria which occurred on her forearms. Immediately after one of the exposures to oyster mushrooms, spirometry was performed. The results showed a reduced airflow obstruction. Standard skin prick tests with an inhalant and food allergens were also performed – all rendering negative results. The diagnosis also included a prick-to-prick test with oyster mushrooms (a wheal size of 12 mm), this time with a positive result. Increased total IgE (78 iu/ml) in the serum was recorded. On the basis of her medical history, reported symptoms and diagnostic tests, the patient could be diagnosed with occupational anaphylaxis and work-related asthma. A constant exposure of indoor mushroom cultivators to Pleurotus ostreatus spores increases the risk of respiratory allergy development. An environmental monitoring of oyster mushroom spores is reasonable in the industry, as it may prove useful in the prevention of the disease.

Opis:
Indoor swimming pools can be used all year round and serve for one of the most popular sport pursued for recreation The positive effect of swimming arises in particular from the involvement of all the muscles of the body, decreasing the burden on the joints, as well as functional improvement of both the lungs and heart. Chlorine is typically used to disinfect swimming pool water and as a result the changes that take place lead to the formation of by-products, such as monochloramines (NH2Cl), dichloramines (NH2Cl2) i trichloramines (NH2Cl3), trihalogenometans (THM) or haloacetic acid (HAA). The highest concentration of these substances is just above the water surface and they may cause irritation of skin, eyes and mucosa of the respiratory tract. The toxic effect of high chlorine concentration and its side-products on the respiratory system is known, but the effect of low concentrations of these compounds is still not fully determined. Recent studies suggest that development of allergic diseases among swimmers may be increased by epithelial disorders driven by airway barrier dysfunction caused by chlorine irritation. Swimming in chlorinated water may be linked to symptoms of bronchial hyperreactivity, asthma and rhinitis especially in children, elite swimmers and employees of indoor swimming pools. Hypersensivity pneumonitis related to the use of swimming pools may manifest as a swimming pool or sauna user lung, most commonly caused by water polluting pathogens. The article summarizes recent data concerning the influence of chlorinated water on the development of allergic diseases.

Objectives. The objectives of this cross-sectional study were to investigate dust exposure and respiratory health of workers in duck hatcheries in western France. Method. Ninety volunteer workers, who work in sorting rooms and/or incubation rooms, participated in exposure assessments and medical examinations. Medical examinations were performed by occupational health practitioners. They filled-in a questionnaire with the workers, followed by a lung function test on each worker. General characteristics and prevalence of chronic respiratory symptoms were described in each type of working rooms. Associations between symptoms and exposure (working room or dust level) were studied in GEE multivariate models. Results. Overall prevalence of chronic respiratory symptoms (cough, phlegm) and chronic bronchitis were similar or lower than in the reference population. However, prevalence of these symptoms was higher for those working in sorting rooms, that were associated with an increased risk of respiratory symptoms and decreased lung function. Respirable dust was also significantly associated with an increased risk of respiratory symptoms. The prevalence of asthma and rhinitis were well above those in the reference population, but did not vary among working rooms. Descriptive data suggested an occupational origin for some cases. Conclusion. Hatchery workers were at increased risk of compromised respiratory health due to dust exposure, particularly those who work in sorting rooms. Asthma and rhinitis were in excess in this population of workers. Thorough clinical examination of these workers should be performed and all exposures assessed.

Opis:
Cel pracy: Celem pracy jest ocena częstotliwości występowania astmy i chorób alergicznych, głównych objawów alergicznych i oddechowych wśród miejskich i wiejskich dzieci w wieku 6–7 i 13–14 lat (w grupach wiekowych według badań ISAAC) w rejonie Grodna. Częstotliwość występowania alergicznego nieżytu nosa i atopowego zapalenia skóry wynosiła około 4% i 10%.
Background: Widespread, medical and social consequences, impact on quality of life and the significant cost of allergic diseases justify a number of measures for the prevention of diseases. What is clear is that official statistics may not match the true prevalence of allergic diseases in general, especially asthma. Aim of the study: Estimate the prevalence of asthma and allergic diseases, the main symptoms of allergic and respiratory among urban and rural children aged 6–7 years and 13–14 years in the Grodno region carried out in accordance with internationally recognized research methodology. Material and methods: The survey questionnaire (based on a survey ISAAC) 2187 was attended by parents of children 6–7 years and 13–14 years of Grodno and region (1091 children from the city, and in 1096 from the village). 955 children aged 6–7 years, 1232 at the age of 13–14 years. Knowing this with asthma may be suspected in patients, who feel shortness of breath accompanied by wheezing, tightness in the chest or cough and allergic diseases (allergic rhinitis and atopic der- matitis), analyzed the incidence of these symptoms and dis- eases among children. Results: Demonstrated a low incidence of asthma among chil- dren in the age group of standardized. Parents of 30 children (1.37% respondents) confirms the existence of disease in chil- dren under physician diagnose disease. Prevalence of asthma among urban children was 1.74% (19 people) and rural – 1.0% (11 children). With a deeper analysis of responses established with 60 children (2.74%) had in the last 12 months wheezing in the chest and coughing. The incidence of allergic rhinitis and atopic dermatitis is about 4% and 10%. These diseases often found only among children with asthma. Conclusions: Since the identification of asthma in children based on the diagnosis by the doctor is not high, can’t be excluded that this reflects a failure to recognize the disease. Noted marked differences in the prevalence of respiratory symptoms, manifesting linking asthma and asthma with allergic diseases. It seems necessary to continuously monitor these phenomena through the continuation of epidemiological studies as a basis for the development of information and implementation of prevention programs and epidemiological evaluation of the effectiveness of prevention at regional and international.

Background Radon-222-enriched hot spring therapy, which is characterized by a safe level of radioactivity, is used for the treatment of rheumatic disorders, and its efficacy has already been studied in several clinical trials. Radon-water inhalation therapy for the treatment of upper and lower airway inflammatory diseases is used in many hot springs centers. However, its application has not been reviewed to date. Methods We systematically searched the PubMed and Scopus databases for clinical trials published in the last 20 years in which objective parameters of upper and lower airway function had been tested before and after radon-enriched inhalation treatment. Results Four prospective studies were found: 1 asthma trial, 1 placebo-controlled chronic rhinosinusitis trial, 1 upper respiratory tract inflammation with nasal obstruction trial, and 1 case-control allergic rhinitis trial. Patients were treated with nasal inhalations of radon-enriched water for 12 to 28 days and were assessed at baseline and after therapy. After 2 weeks of treatment, nasal resistance decreased, flow increased, mucociliary clearance was enhanced, ciliated-to-muciparous cell ratio increased, and %FEV1 increased in asthmatic patients. Conclusion Radon-enriched inhalation therapy improves objective indicators of nasal function in allergic rhinitis and chronic rhinosinusitis, and causes relief of pulmonary obstruction in asthma.

Opis:
It is estimated that every third person living in Europe suffers from allergic diseases. Allergies are a growing health problem in Poland where 40% of the population have allergy symptoms, including 12% afflicted with asthma. The actual cost of allergic diseases is difficult to estimate due to the lack or incompleteness of the relevant data. The aim of this review is to present estimates of the indirect costs of allergic diseases in Poland and globally, using asthma, allergic rhinitis and atopic dermatitis as examples. The analysis also includes the impact of allergic diseases on the costs to the social welfare system and employers. The literature review of the indirect costs of allergic diseases shows that the indirect costs of a disease, which substantially exceed the direct costs, increase with the disease activity and severity. Interestingly, some studies have found that the indirect costs of lost productivity due to hours missed from work to take care of a sick child could be threefold higher than those of absence due to a worker’s own illness. The indirect costs of a disease can be significantly reduced by early diagnosis and appropriate treatment. Int J Occup Med Environ Health. 2019;32(3):281–90

Nail art (creation and decoration of artificial nails) is a growing fashion trend. Proportionally to its popularity, the number of allergic reactions to the materials used has recently increased. We report the case of a nail art operator who developed occupational allergy to acrylates, manifested with the unusual simultaneous presence of asthma and dermatitis. Cutaneous lesions only affected areas not covered by individual protection devices or clothes, even if such areas were not in direct contact with acrylates, suggesting airborne allergic contact dermatitis. While respiratory problems were solved with the correct use of a mask at the workplace, facial dermatitis proved impossible to avoid or control and, for this reason, the patient had to change her work.

Objectives: Diisocyanates (DIC) are highly reactive, low-molecular-weight chemicals which are the leading cause of occupational asthma (OA). The aim of the study was to analyze certain aspects of the pathogenesis of allergic infl ammation in the airways induced by toluene diisocyanate (TDI) in an experimental model in mice. Materials and Methods: The experiment was carried out on 50 female BALB/cJ/Han/IMP mice, which were exposed by inhalation (intranasal and in the inhalation chamber) to toluene diisocyanate (2,4-TDI). After the experiment, the bronchoalveolar lavage fluid (BALF) was collected from the animals, and the composition of the induced infl ammatory cells, and the concentrations of certain cytokines (IL-4, IL-5, TNF-α) were evaluated. Results: The total number of cells in BALF of the examined group of mice was signifi cantly higher compared to the control mice. There was also a signifi cant increase in neutrophils and eosinophils in the study group compared to the controls. The number of lymphocytes and macrophages did not differ signifi cantly between the two groups. A statistically signifi cant increase in the level of TNF-α was shown to occur in the group exposed to toluene diisocyanate in comparison to the control group. The concentration of IL-4 increased in the study group, compared to the control one, but the differences did not reach the level of signifi cance, p > 0.05. Such difference was not observed for IL-5. Conclusions: We developed a murine model of TDI-induced asthma which caused the infl ux of infl ammatory cells like eosinophils and neutrophils in the bronchoalveolar lavage fl uid (BALF) in the TDI-treated mice. The increase of the concentration of some proinfl ammatory cytokines (TNF-α, IL-4) in BALF from the exposed mice was also observed.

Objective: A number of studies show an association between traffi c-related air pollution and adverse respiratory health effects in children. However, most evidence relates to the regions with low or moderate levels of ambient air pollution. The study was undertaken to assess the impact of traffi c-related air pollution on respiratory health status in children living in the area of high levels of industrial and municipal ambient air pollution. Materials and Methods: Analyses involved data obtained from cross-sectional study on respiratory health in children (N = 5733), conducted between 2003–2004 in Bytom, one of the largest cities of Silesian Metropolis (Poland). Exposure to traffic-related air pollution was assessed by means of geographic information system and expressed as several measures of potential exposure to traffic-related air pollution, involving residential distance to major road and traffic density in the residential area. Logistic regression was used to examin association between reported respiratory health and traffic measures. Results: Statistically signifi cant association was found between doctor-diagnosed asthma and residential proximity to traffic. Results of multivariate logistic regression (logOR; 95%CI) confi rmed the effect of living in an area of a city with high-traffic-density on childhood asthma: 1.60 (1.07–2.39). Similar effects were found in case of allergic rhinitis and rhinitis symptoms, but the observed associations were not statistically signifi cant. Conclusion: The study fi ndings suggest that even in an area with poor regional ambient air quality, adverse respiratory health outcomes are more frequent in children living in a proximity to the high vehicle traffic flow

Bisphenols, endocrine disrupting chemicals, are widely used in daily life. Continued exposure during key developmental periods of life (pregnancy, infancy and early childhood) can contribute to adverse health consequences such as decreased lung function, wheezing/asthma, the occurrence of allergies or changes in immune system responses. The purpose of this review is to present the current state of knowledge on the effects of prenatal or postnatal exposure to bisphenol A (BPA), bisphenol S (BPS) and bisphenol F (BPF) on the development of allergic diseases in childhood. A comprehensive and systematic search of PubMed, Scopus and Web of Science databases was conducted. The review is restricted to studies published since 2015, in English in peer-reviewed journals. Based on keywords, 2648 studies were identified and reviewed for eligibility. Finally, 8 epidemiological studies were found to be appropriate for inclusion in this publication. The data collected in this review suggests that there is an association between maternal exposure during pregnancy or childhood to BPA and the development of allergic diseases. Most studies reported positive relationships between BPA exposure and at least one of the types of allergic disease. The paucity of studies and the observed differences in findings regarding the association between prenatal/postnatal exposure to BPS and/or BPF do not allow firm conclusions to be drawn. Further research is needed to identify the vulnerable population and the mechanisms responsible for the development of undesirable health consequences.

Opis:
Respiratory allergic diseases such as bronchial asthma, rhinitis, urticaria, atopic dermatitis have been steadily increasing all over the world, including India. Owing to its alarming trend, several aerobiological surveys have been undertaken in different parts of India to delineate the variety of pollen and spore load. In this review, we have reported the current state of aerobiological knowladge in India with particular reference to allergenic airborn pollen occurence in 2001–2015. Pollen have been found to contribute a significant proportion in the air and caused allergy symptoms in the local inhabitants. Aerobiological records, a questionnaire survey and hospitalization records have been employed for the analysis. Holoptelea integrifolia, Amaranthus spinosus in northern region, Sorghum vulgare, Pennisetum, Gynandropsis gynandra, Parthenium hysterophorus, Dolichandrone platycalyx in southern regions, and Parthenium hysterophorus from the western region; Cynodon dactylon, Cenchrus ciliaris in the central area; Acacia auriculiformis, Cleome gynandra, Catharanthus roseus, Phoenix sylvestris, Areca catechu, and Lantana camara in the eastern regions as potential aeroallergens in India. The statistical approach confirmed the correlation between hospitalization rate associated with allergy-related health troubles and the prevalent allergenic pollen in the air. The Poaceae group has been found to be dominant throughout India. Immuno-biochemical studies identified various protein with allergenic potential found in the pollen recorded. Epitope identification and homology of the major allergenic protein Cat r1 of Catharanthus sp and Par j 1 of Parietaria judaica have been found. Identification of allergenic pollen grains and the modern approach concerning cross-reactivity and epitope revelation of dominant airborne pollen have important clinical implications for the prevention, diagnosis and treatments of allergic diseases in India.
W regionach Indii północnych do alergenów pyłkowych należą Holoptelea integrifolia, Amaranthus spinosus, w regionach południowych Sorghum vulgare, Pennisetum, Gynandropsis gynandra, Parthenium hysterophorus, Dolichandrone platycalyx, w regionach zachodnich Parthenium hysterophorus, na obszarach centralnych Cynodon dactylon, Cenchrus ciliaris, a w regionach wschodnich Acacia auriculiformis, Cleome gynandra, Catharanthus roseus, Phoenix sylvestris, Areca catechu i Lantana camara.

Piperazine (CAS Registry No. 110-85-0) is a solid, crystalline substance used mainly as an antihelminthic drug in medicine and veterinary medicine, and also as corrosion inhibitor and polymerization accelerant in industry. Piperazine is a low-toxic chemical. In humans, exposure to piperazine led to both skin sensitization and allergic asthma. No mutagenic and carcinogenic effects have been found in the relevant epidemiologic and experimental studies. On the of human observations in industry, values of 0.1 mg/m3 and 0.3 mg/m3, have been proposed as MAC (TWA) and MAC (STEL) respectively.

Objectives The development of allergic conditions is largely dependent on the interactions between genetic (individual genetic predisposition) and environmental factors (exposure to risk factors). The aim of this study was an attempt to assess the influence of selected elements of the hygiene theory in the development of allergic diseases such as allergic rhinitis and asthma. Material and Methods The study group consisted of 5518 women and 3868 men. The method that was used was the European Community Respiratory Health Survey II and International Study of Asthma and Allergies in Childhood questionnaire validated and adapted to Central and Eastern European conditions. The project was conducted in 8 urban areas (Gdańsk, Wrocław, Poznań, Katowice, Kraków, Lublin, Białystok, Warsaw) and 1 rural area (Krasnystaw county). This study had 2 stages; the first stage involved grouping the 22 500 respondents based on their questionnaire responses with the use of a Personal Digital Assistant (PDA); the second stage involved 7000 subjects, who underwent additional assessments: skin prick tests (birch, grasses/cereals, Dermatophagoides pteronyssinus and Dermatophagoides farinae, molds [set I: Botrytis cinerea, Cladosporium herbarum, Alternaria tenuis, Curvularia lunata, Fusarium moniliforme, Helminthosporium], molds [set II: Aspergillus fumigatus, Mucor mucedo, Penicillium notatum, Pullularia pullulans, Rhizopus nigricans, Serpula lacrymans], cat, dog, molds Cladosporium herbarum, Alternaria tenuis) and spirometry tests. Results The age at which children attend the nursery school is critical to the development of allergic diseases; in allergic rhinitis, the risk of an IgE-dependent reaction is 2 times higher in the second than in the first year of life (p = 0.00147, p < 0.05), while in asthma, having a large number of siblings increases the risk of developing obstructive disease by almost 6 times (p = 0.00316, p < 0.05).The age at which children attend the nursery school is critical to the development of allergic diseases; in allergic rhinitis, the risk of an IgE-dependent reaction is 2 times higher in the second than in the first year of life (p = 0.00147, p < 0.05), while in asthma, having a large number of siblings increases the risk of developing obstructive disease by almost 6 times (p = 0.00316, p < 0.05). Conclusions The hygiene theory is particularly applicable and can explain the relationship of selected habits in the development of allergic diseases.

Opis:
Asthma, allergic rhinitis and idiosyncrasy to aspirin stand for an increasing epidemiologic. In Poland an epidemiological study has been carried out in 11 centres, showing the occurrence of asthma at 5.4% of general population, for allergic rhinitis and idiosyncrasy – 8.5% and 0.5%, respectively. Some publications suggest more often presentation of idiosyncrasy in atopic patients. Taking under consideration high and increasing occurrence of allergy and massive use of non-steroid anti inflammatory drugs, their coincidence is quite important and we decided to investigate it. In a retrospective analysis of patients diagnosed at the Department of Internal Medicine and Allergology, Wrocław Medical University, we determined a group of 1 557 patients suffering from at least of one of these conditions. Asthma has been recognized in 1 015 patients (65%), allergic rhinitis and aspirin idiosyncrasy in 495 (32%) and 363 (23%), respectively. Coincidence of asthma and allergic rhinitis has been reported in 255 patients, and coincidence of asthma or rhinitis and idiosyncrasy – in 3% and 6%, respectively. We conclude that asthma and allergic rhinitis coexist very frequently, confirming a mutual pathophysiologic background and the concept of united airways disease. Further, idiosyncrasy appears 10 times more often in allergic patients than in general population.
Współwystępowanie astmy i alergicznego nieżytu nosa stwierdzono u 255 pacjentów. Współwystępowanie którejś z powyższych chorób i idiosynkrazji aspirynowej zaobserwowano odpowiednio u 3 i 6% grupy.
