Opis:
Orbital infection occurs when pathogens pass from an infected sinus into the orbit. The inflammation is spreading onto the orbital structures from the surrounding tissues mainly from ethmoidal sinuses due to its specific anatomical conditions. The orbital complications are: preseptal cellulitis, orbital cellulitis, subperiosteal abscess, orbital abscess and intracranial infections. The diagnosis of subperiosteal orbital abscess is based mainly on clinical presentation. The symptoms depend on which sinus is affected. Orbital symptoms include periorbital swelling, proptosis, ophthalmoplegia, chemosis, and optic nerve compression. Computer tomography is mandatory. Early diagnosis and appropriate treatment can prevent from permanent vision loss and intracranial complication. The study discusses the course of the disease of 25-year-old woman who was admitted to Emergency Department due to inflammatory eyelid oedema and proptosis for 3 days. No symptoms of sinusitis were noted. The CT scans established diagnosis – subperiosteal orbital abscess. The patient was treated with broad spectrum antibiotics and functional endoscopic sinus surgery. Her treatment and recovery were uneventful. In follow-up nighter ophthalmological nor otorhinolaryngological complains were noted.

Splenic abscess is a potential complication of many disease entities which involve infections. Diagnostics are based on imaging studies. Treatment involves splenectomy and antibiotic therapy. In the case the abscess is limited, and particularly in young patients, percutaneous abscess drainage may be performed. This paper presents a case of a 66-year old female patient following coronary artery bypass grafting (CABG) complicated with sternum infection and cholecystectomy due to gallbladder abscess complicated with surgical site infection, who underwent long-term treatment in the ICU due to respiratory failure and persistent septic condition. Splenic abscess is a rare complication of abdominal cavity diseases and systemic infections.

Cervicofacial actinomycosis is a specific, primarily chronic inflammatory process caused by Gram-positive anaerobic, microaerophilic Actinomyces bacteria. The infection usually results from the skin or mucosal lesions. The following paper reports two cases of Cervicofacial actinomycosis in patients admitted in 2014 to Maxillofacial Surgery Department in Lublin, Poland. Diagnosis of culture-positive actinomycosis was based on histopathology findings and the presence of actinomyces colonies. Cervicofacial actinomycosis requires surgical treatment, oral cavity sanation, and in the case of presenting actinomycosis abscesses formation, combined surgical incision and drainage. Long-term penicillin antibiotic therapy is necessary.

This study presented a septic shock occurring with striking speed during the percutaneous drainage of a large abscess of the liver in an 18-year-old female patient after an appendectomy performed 3 months earlier using the laparoscopic method because of its phlegmonous and gangraenosus inflammation. The post-operative course was complicated by increased suppuration in places after removal of the trocars. After including the intravenous antibiotic therapy and under the USG control, the percutaneous drainage of the abscess was performed. While withdrawing the guide of the drainage catheter set, the symptoms of septic shock occurred. The pharmacological treatment was applied with continuation of the antibiotic therapy till a negative result of microbiological tests was obtained and the normalization of life parameters. The patient was not qualified for treatment in the ICU. She was discharged in a generally good condition after 24 days of hospitalization. In the case of this patient, the drain covered with the content of the abscess may have contact with the adjacent tiny blood vessels, which may have contributed to the systemic inflammatory response syndrome that was linked with fast-developing sceptic shock.

Introduction: Perianal abscess and fistula-in-ano are common findings in infants and children. The perianal abscess is usually a manifestation of a fistula-in-ano. Experience of our center indicates general lack of knowledge of the origin of the abscess and therefore, it is usually treated by incision and drainage, which leads to repeated recurrences. Aim: We aimed to present the optimal management of fistula-in-ano and perianal abscess in order to reduce or even eliminate the risk of recurrence. Material and methods: The retrospective study included 24 infants with perianal abscess treated at our center between 2013 and 2015. Patients were divided into two groups: group I (50%) was primary treated in our center, while group II had undergone prior surgical interventions in other hospitals. Fistula-in-ano was intraoperatively identified in all patients (100%) and fistulotomy was performed. Results: No fecal incontinence or recurrence of perianal abscess were observed in any of our patients. In group II, the disorder was associated with severe inflammation, some patients underwent an additional surgical intervention, such as incision and drainage of an extensive buttock’s abscess; patients required longer antibiotic therapy and prolonged hospitalization. Conclusion: Minimally invasive approach (sitz baths, antibiotic therapy, puncture or incision and drainage of the abscess) appears tempting due to its simplicity and lack of need for general anesthesia, but it is associated with a high recurrence rate. Fistulotomy and fistulectomy, which are slightly more invasive procedures, significantly reduce the recurrence rate of fistulain- ano and perianal abscess.

Introduction and aim: Intra-abdominal abscesses can lead to serious complications (e.g. sepsis) and death. The aim of the pilot study is to study if the occurrence of hepatic and splenic abscesses depends on the seasons. Material and methods: The study was based on patients with abscesses of liver or spleen in imaging studies (ultrasound and CT scan) performed in the period 2007-2016 in the Department of Diagnostic Imaging and Interventional Radiology of Pomeranian Medical University in Szczecin, taking dates of the examinations into consideration. Results: Out of the total number of studies abscesses in the abdomen (257), 52 studies were liver and spleen abscesses occurring in 42 patients. From the statistical analysis we obtained 17 studies carried out in winter, 8 - spring , 8 - summer and 9 - autumn. Chi-square test of goodness of fit showed a statistically significant increased value of abscesses in the number of tests diagnosed in winter in comparison to the other seasons (chi-square 5.212; p-value <0.1). Conclusion: The occurrence of hepatic and splenic abscesses is the highest in winter.
Wstęp i cele: Ropnie wewnątrzbrzuszne mogą prowadzić do poważnych powikłań (m.in. sepsy) i do śmierci. Celem badania jest określenie czy częstość występowania ropni wątroby oraz ropni śledziony zależy od pór roku. Materiał i metody: Przedmiotem badań są pacjenci z ropniami wątroby lub śledziony w badaniach obrazowych (USG oraz TK) wykonanych w latach 2007-2016 w Zakładzie Diagnostyki Obrazowej i Radiologii Interwencyjnej Pomorskiego Uniwersytetu Medycznego w Szczecinie. Pod uwagę wzięto daty wykonania badania. Wyniki: Z całkowitej liczby badań ropni wewnątrzbrzusznych (257) 52 badania stanowiły ropnie wątroby oraz śledziony występujące ogółem u 42 pacjentów. Z analizy statystycznej uzyskano 17 badań wykonanych zimą, 8 wiosną, 8 latem oraz 9 jesienią. Test zgodności chi-kwadrat wykazał statystycznie istotny wzrost liczby badań z ropniami wykonanych zimą w odniesieniu do innych pór roku (chi-kwadrat 5,212 ; p-value <0,1). Wnioski: Częstość występowania ropni wątroby oraz ropni śledziony jest najwyższa zimą.

Introduction: Endoscopic surgery of the paranasal sinuses is a surgical technique that has revolutionized the surgical treatment of patients with chronic sinusitis. Application of this method now goes far beyond the conditions of the nasal cavity. Aim: It allows among other operations within the eye socket, acting alternative technique for the operation of the external appproauch. Material and Methods: This work is devoted to the analysis of the effectiveness of endoscopic surgery in the treatment of patients with orbital complications in the course of sinusitis in the Department of Otolaryngology SU in Cracow in the years 2004-–2016, aim of this study is to search for optimal treatment of patients with the above disorders. Results: In the group of patients operated on because of orbital abscess from reaching the outside in all patients had symptoms of revocation on the part of the orbit and ocular, and postoperative complications were not observed. Among patients undergoing endoscopic trocar in one patient there was no improvement in the function of the eye. It could lead to a too long period of time from the first appearance of symptoms (over 48 hours), and sometimes surgical intervention, as well as the occurrence of orbital hematoma in the second postoperative day and the necessity of re-operation of the external handle.

This complication due to the high variability of clinical symptoms qualifies this disease to a group of emergencies, mainly due to the risk of vision loss. We present a case report a patient with orbital subperiosteal abscess in chronic sinusitis, in which in the age of admission to an experienced significant disorders of the ocular. The patient was performed orbital abscess drainage by intranasal, complicated by the occurrence of orbital hematoma and the need to re-perform the surgery.

Duodenal perforation is a rare and severe acute surgical condition which commonly follows the complications of endoscopic and laparoscopic procedures. Small degree of damage in this mechanism and an early diagnosis allow for an effective primary management. The most difficult surgical challenge is an effective management of retroperitoneal duodenal perforation together with coexisting pathological changes of its wall. In this work we present a case of duodenal necrosis with excessive necrosis of a fragment of its wall due to perinephric abscess, with an effective method of management of a defect in an isolated free small intestinal loop in association with gastroduodenal passage exclusion.

Laparoscopic cholecystectomy is the golden standard, considering treatment of cholelithiasis. During the laparoscopic procedure one may often observe damage to the gall-bladder wall, as well as presence of gall-stones in the peritoneal cavity, as compared to classical surgery. These gall-stones may be associated with the occurrence of various complications following surgery. The study presented a rare case of a retroperitoneal abscess, as a consequence of retained gall-stones, in a female patient who was subject to laparoscopic cholecystectomy two years earlier.

Objectives: In patients hospitalized due to deep neck infections (DNIs), to investigate the association between demographic parameters, etiology, and localization of abscesses and/or phlegmons, complications, comorbidities, treatment, and bacterial cultures. Methods: We analyzed data of 263 patients that were hospitalized from January 1, 2012, to December 31, 2015, due to deep neck space phlegmons and/or abscesses. We performed statistical analysis with the SPSS 22.0 software; statistical significance was set p<0.05. Results: Among the analyzed patients, dental infections were the most frequent, followed by acute phlegmonous pharyngitis. Submandibular space abscesses or phlegmons of the neck were the most frequent complications of these infections. Re-operation was performed in 19.8% of cases, and complications developed in 11% (mostly airway obstruction). DNI complications were less common in smokers than in non-smokers (OR=0.038, p=0.025). Discussion: In our study, dental infections accounted for 70.6% of DNI cases. Thus, our study explains why odontogenic DNIs are the most common DNI type studied in the literature. Poor oral health and odontogenic infections should not be underestimated because they can lead to uncommon but lethal diseases such as descending necrotizing mediastinitis, which requires aggressive surgical treatment and is associated with a mortality rate of 10%-40% despite treatment. The complication developed in 11.4% of cases. In conclusion, oral health and hygiene contribute to DNI development.

Actinomycosis is a very rare cause of inflammation of the labial soft tissue in girls. It is characterised by hard inflammatory infiltrates accompanied by fibrosis, abscesses and fistulas. The non-specific course of the disease makes a correct diagnosis difficult. In this paper, we have presented diagnostic and therapeutic difficulties in the inflammation of the labial soft tissue of probable actinomycotic aetiology in a 7-year-old girl. The patient was hospitalised twice, including three surgical procedures due to labial tissue inflammation and abscess. Methicyllin-resistant Staphylococcus aureus was cultured from the abscess content. Systemic antibiotics from six different classes were used; however, no improvement in the local condition was achieved. The abscess was irrigated with gentamicin solution; steroid ointments and physical therapy were used, but with no effect. A suspicion of actinomycotic infection was raised due to the chronic course of the disease and poor response to treatment. Penicillin was administered for a total period of 3 months, resulting in rapid improvement and, ultimately, a complete recovery of the child. Although the actinomycotic aetiology was not confirmed by cultures or histopathological examination, the course of the therapy has allowed to suspect Actinomyces infection.
Promienica jest bardzo rzadką przyczyną zapalenia tkanek miękkich okolicy warg sromowych u dziewczynek. Charakteryzuje się występowaniem twardych nacieków zapalnych z towarzyszącymi włóknieniem, ropniami i przetokami. Niecharakterystyczny przebieg utrudnia ustalenie właściwego rozpoznania. W artykule autorzy przedstawili trudności w diagnostyce i leczeniu zapalenia tkanek miękkich okolicy sromu o prawdopodobnej etiologii promieniczej u 7-letniej dziewczynki. Pacjentka była dwukrotnie hospitalizowana, w tym trzy razy operowana z powodu stanu zapalnego i ropnia w obrębie warg sromowych. W posiewach treści z ropnia wyhodowano Staphylococcus aureus MRSA. Stosowano leczenie ogólnoustrojowe antybiotykami z sześciu różnych grup, nie uzyskano jednak poprawy stanu miejscowego. Ropień płukano roztworem gentamycyny, stosowano maści steroidowe oraz fizykoterapię bez efektu. Ze względu na przewlekły przebieg oraz słabą odpowiedź na leczenie wysunięto podejrzenie zakażenia promieniczego. Podawano penicylinę przez łączny okres 3 miesięcy, uzyskując szybką poprawę i ostatecznie pełne wyleczenie dziecka. Pomimo braku potwierdzenia promieniczej etiologii zakażenia w posiewach oraz badaniu histopatologicznym przebieg terapii pozwala podejrzewać zakażenie Actinomyces.

Horseshoe kidney (HSK) is the renal fusion anomaly caused by disturbances in embryonic development when the kidneys are fused together in the lower or upper pole forming an isthmus. The most common disorders in urinary tract related to horseshoe kidney are ureteral pelvic junction obstruction, urinary tract infection and urolithiasis. In our study, we present a rare case of an abscess in the isthmus of horseshoe kidney after extracorporeal shockwave lithotripsy on the right kidney was performed. The patient has had recurrent urolithiasis and underwent 4 treatments on the left kidney in the past without complications.

Opis:
Autorzy przedstawiają u 73 letniej kobiety torbiel ogonowo-jelitową przestrzeni pozaodbytniczej, która uległą zezłośłiwieniu. Już w trzecim roku życia była leczona, jako przetoka ropna pośladka prawego, która się okresowo uaktywniała. W 2016 roku zgłosiła się do Poardni Chirurgicznej w Gorlicach z olbrzymim ropniakiem pośladka prawego z przetoką ropną . Na oddziale chirurgicznym w Gorlicach operacyjnie usunieto ropniak o ścianach dobrze ukształtowanych razem z przetoką. Torbiel zawierała ponad litr ropy. Po wygojeniu w bliźnie pojawiłą się przetoka śluzowa .Badaniem histologicznym rozpoznano retrocecal cyst hamartoma z komórkami mucinos adenocarcinoma. W czasie następonego pobytu chorej w szpitalu po wykonaniu rezonansu magnetycznego stwierdzono resztkowa zmianę w okolicy przedkrzyżowej sięgająca do trzeciego kręgu krzyżowego. Zmianę usunięto radykalnie potwierdzoną badaniem histopatologicznym. U chorej nawrotu nie zaobserwowano.
