Opis:
Laser ablation of graphite is studied both theoretically and experimentally. Plasma temperature and electron density in the early phase of expansion into vacuum are measured as a function of distance from the target. The experimental results agree well with the theoretical simulations. The simulation of ablation shows that plasma plume considerably affects the ablation rate.

In this report we present pulmonary vein and posterior box isolation together with the right superior ganglion plexus ablation using the Qdot Micro catheter without fluoroscopy. We describe different possibilities of this new technology for catheter ablation. The main advantages of this catheter to potentially increase ablation safety and effectiveness are discussed. Specifically, the possibility to perform high-density mapping with the lowest available distance between points. Furthermore, the possibility to decrease the risk of collateral tissue damage and to improve atrial linear lesions contiguity, transmurality and durability due to the dominance of resistive heating supported by the feedback temperature control. Finally, the possibility to shorten the procedure and fluoroscopy duration due to the high shortening of application duration to 4 seconds only.

Background Concentration of tumor necrosis factor alpha (TNF-alpha) might be useful in selecting patients with paroxysmal atrial fibrillation (PAF) who will benefit the most from pulmonary vein isolation. Material and methods We performed prospective cohort study among patients with PAF who had sinus rhythm prior to undergoing either radiofrequency ablation or cryoablation procedure. Blood samples were collected at the start of the procedure and 16-24 h after. TNF-alpha concentrations were measured. Follow-up data was obtained during a structured telephone interview and 24-hour ECG Holter monitoring 12 months after the ablation procedure. Results Thirty seven patients were enrolled. After 12-month follow-up 27 patients maintained sinus rhythm, 8 had recurrence of AF and 2 were lost to follow-up. There was no significant correlation between TNF-alpha concentrations in any of the samples and the recurrence of arrhythmia (for pre-procedural samples: 1.75 pg/ml vs 1.74 pg/ml; p=0.72; for post-procedural samples: 1.49 pg/ml vs 1.79 pg/ml; p=0.16). In patients who had a recurrence of AF, we observed a decrease in the periprocedural TNF-alpha concentration (-0.12 pg/ml vs 0.05 pg/ml; p=0.05). Conclusions Neither pre- nor post-procedural TNF-alpha concentrations are predictive of ablation outcome in patients with PAF. We observed a decrease in the periprocedural TNF-alpha concentration in patients who had AF recurrence.

The paper presents the selected results of tests on ablational laser removal of polymer lacquer coating with metallic filler (aluminium plates). The fibre laser with the Galvo head of the following parameters: the radiation wavelength λ = 1064nm, the exposition time of the laser impulse τ = 25ns, and the repetition frequency 30-500kHz was used. By changing the power thickness, repetition frequency, and scanning speed various effects of varnish, coating removal were obtained. The melting of the colourless lacquer coat was observed when small power thickness was applied. Multiple scanning with the laser beam of higher power thickness resulted in sequential removal of several to tens of micrometers of the lacquer coating. This technology brings very good perspectives for the renovation of lacquer coating systems in present vehicles (bodies, frames).

Opis:
Background: Atrioventricular nodal reentry tachycardia (AVNRT) is the most common paroxysmal supraventricular tachycardia. The relatively ineffective antiarrhythmic drugs and the predominant young age makes the catheter ablation the therapy of choice in many patients. This results in predominance of this arrhythmia in electrophysiological labs. The aim of the study was to analyze the gender-related differences among patients undergoing the radiofrequency catheter ablation of slow pathway entrance to the atrioventricular node. Material and methods: The study group comprised of 147 consecutive patients with diagnosed atrioventricular nodal reentry tachycardia, who underwent the radiofrequency catheter ablation (RFCA) of slow pathway. Patients have been divided into 2 groups, based on sex. Results: The overall 97.3% of effectiveness of RFCA was observed. Women were significantly younger than men (53.7+/-17.2 vs 57.7+/-9.8 years) with lower radiation dose (2383.5+/-1993.2 vs 2891.6+/-2377.1 cGyxcm2). Conclusions: Younger age of women in comparison to men during RFCA of AVNRT reflects earlier onset of symptoms in women. Gender does not affect the time of fluoroscopy, but the higher rate of inducible tachycardia after RFCA in women may suggest the existence of anatomical difficulties or the operator’s apprehensions. Sex-related difference in radiation dose that we have observed may result from the greater volume of the men’s chest.

Hydroxyapatite layers [formula] were deposited on Ti6Al4V substrate by use of pulsed laser deposition (PLD) technique using ArF excimer laser. In order to determine the influence of substrate temperature on structure of deposited films the experiment was performed for substrate temperatures ranged from 150 degrees of Celsius to 650 degrees of Celsius. The chemical analysis of deposited hydroxyapatite coatings were performed by Fourier Transform Infrared spectroscopy (FTIR). The morphology of deposited layers were characterized by means of Atomic Force Microscopy (AFM) method. The results indicate dependence of apatite structure and surface morphology on the substrate temperature.

Opis:
Osteoid osteoma (OO) is a benign bony lesion with a characteristic radiologic feature of x-ray focus (nidus). The most common symptom indicating diagnosis is strong nocturnal pain that eases with nonsteroidal anti-inflammatory drugs (NSAID). Computed tomography (CT) is a method of choice for diagnosing and localizing the tumor. Osteoid osteoma can regress spontaneously, thus conservative treatment is one of the methods of choice. In the past open resection surgery had been a standard management of osteoid osteoma until radiofrequency ablation (RFA) was discovered in the early 1990s. Nowadays, there are a few minimally invasive treatment techniques that are used. CT-guided radiofrequency ablation is considered a gold standard technique these days. Many cases occur with late diagnosis, and delayed treatment despite of characteristic symptoms. Therefore, it is crucial to be familiar with the clinical features of osteoid osteoma, proper diagnostic patterns, and treatment schemes.
W wielu przypadkach występuje znaczne opóźnienie pomiędzy początkiem charakterystycznych objawów a postawieniem prawidłowej diagnozy i skutecznym wyleczeniem. W związku z tym istotna jest znajomość obrazu klinicznego, prawidłowy dobór metod diagnostycznych i leczniczych.

Opis:
We present the first Polish experience with ablation performed using DiamondTemp catheter. The study was conducted with 3 male patients diagnosed with atrial fibrillation (AF). In the first 2 patients typical transseptal punctures were performed, followed by mapping with the Advisor catheter and EnSite-Precision system. One patient had a residual atrial septal leak, therefore ablation without fluoroscopy was attempted. High-power, short-duration ablation under temperature control was performed around pulmonary vein (PV) ostia. The power was 49-53 W, the temperature was 45-48 ͦC. Duration of procedures/fluoroscopy were: 146/8.9, 177/5.9, 132/0.0 min. In the reference group, 10 recent AF identical ablation procedures performed with traditional equipment resulted in 143.0±27.0/6.0±4.4 min. Duration of DiamondTemp applications were 14.7, 32.7, 30.8 min (reference group 37.3 ± 11.4 min). Procedural endpoints were achieved in all but one patient with incomplete isolation of the low segment of the right inferior PV. There were no procedural complications noted. In conclusion, the DiamondTemp saline-irrigated catheter is safe and effective for high-power short-duration ablation in patients with AF. Furthermore, this technology makes it possible to complete the procedure without fluoroscopy. However these findings must be confirmed in larger group of patients.

Patients with ventricular tachycardia are usually treated with antiarrhythmic drugs and ablation if the arrhythmia substrate is available for invasive treatment. Despite high efficiency of this treatment there is a significant group of patients who do not benefit from available treatment methods, either because they cannot be applied or do not allow for durable control of the disease. For that reason a novel treatment method, STereotactic Arrhythmia Radioablation (STAR) has been proposed and its safety and efficiency is extensively studied throughout the world. The method is based on irradiation of the arrhythmia substrate identified with electrophysiological examination with high-precision image-guided radiosurgical methods usually used for ablation of malignant tumors. Here we present the protocol of the first Polish study on STAR in patients with intractable ventricular tachycardia (STAR-VT, NCT04642963), designed to test the safety of the method. Secondary endpoints include measures of the treatment efficiency.

Opis:
On the base of literature data as well as authors works a review of the state-of-the-art in ablative laser cleaning of surface layers of various constructional materials with encrustation produced in physical and technological processes is presented in the paper. The process and mechanisms occurring in the ablative laser cleaning are described. Three types of the most frequently used lasers offer characteristic advantages understood as average power, pulse operation, wavelength, optical feedback with materiał processed, and fiber delivery. The process of surface layer cleaning of materials and machine elements can be carried out in various -ways. In general, when using pulse laser radiation, five different mechanisms of removal of particles, encrustations, or coatings can be applied. So, the process of removal can be performed via following physical processes: surface vibrations, particles oscillations, thermal expansion of particles (or encrustation), generation of so called detonation wave over surface being cleaned, and laser ablation. Lasers are especially useful for removal of layers from many types of substrates like wood, metals, plastics, composites, stones, ceramics, leather, and others. The important advantage of laser cleaning and surface modification is the capability of processing automation which is related to process repeatability, ease of parameters controlling, possibility of strengthening in zones difficult to access, high precision of interaction and high efficiency as well.

Background: Anatomy assessment using Computer Tomography (CT) and Magnetic Resonance (MRI) is performed in patients undergoing pulmonary vein isolation (PVI). The aim of this analysis was to investigate whether electroanatomical 3D map and CT/MRI image integration using the CartoMerge system improves efficacy, reduces procedure time or fluoroscopy usage. Materials and methods: 57 patients undergoing PVI were divided in two groups: “Merge” (n=45 pts) and “non-Merge” (n=14 pts) depending on usage of image integration. PV isolation during procedure (acute PVI), procedure time, fluoroscopy time, number of radio frequency (RF) applications and AF recurrence during follow-up (Merge group: 12-24 months, non-Merge group: 9-18 months) were analyzed. Results: Intra-procedural PVI was equal in both groups (93%). Long-term efficacy, defined as absence of AF recurrence, was insignificantly higher in the Merge group (69,8% vs 50%, p=0,11793). Procedure time was significantly longer in the Merge group – 239,1 (±55,5) min. vs 192,4 (±44,5). Fluoroscopy time was similar in both groups 29,9 (±12,23) vs 24,6 (±26,5) min, (p=0,579). Number of RF applications was significantly higher in the Merge group 48,5 (±25,2) vs 27,2 (±14,9). Conclusions: CartoMerge did not improve the rate of acute PVI, long-term effectivity or fluoroscopy time. In the non-Merge group the procedure time was shorter and the number of applications was significantly smaller.

Opis:
The work focused on developing functional coatings on titanium substrates that would facilitate the integration with the cardiac tissue and with a specific form of connective tissue like blood. Surface modifications consisted in the laser evaporation of part of the biocompatible layer, thus creating a suitable environment for a particular tissue. For the myocardium integration, the metal surface was refined by biohemocompatible coatings. Such surfaces were the starting point for further modifications in the form of channels. The channeled surfaces enabled a controlled cell migration and proliferation. The interaction of endothelial cells with the material was highly dependent on the surface characteristics such as: topography, microstructure or mechanical properties. The controlled cellular response was achieved by modifying the surface to obtain a network of wells or channels of different dimensions via the laser interference lithography. This technique determined a high resolution shape, size and distribution patterns. As a result, it was possible to control cells in the scale corresponding to biological processes. The surface periodization ensured the optimal flow of oxygen and nutrients within the biomaterial, which was of a key importance for the cell adhesion and proliferation. The work attempted at producing the surface networks mimicking natural blood vessels. To stimulate the formation of new blood vessel the finishing resorbable synthetic coatings were applied on the surface to act as a drug carrier. Therefore, the initial trial to introduce factors stimulating the blood vessels growth was performed.

Opis:
One of the most important parameters in the selection of composite materials used for the protection of avionics instruments exposed to high temperatures are ablative properties. They constitute the main criterion in determining the composition and thickness of the protective material. In this article, the authors determine the ablative properties of a polymer matrix with carbon reinforcement and check the effect of using the MMT additive (montmorillonite) on the change in the resistance to the impact of a high heat flux. The tested materials play an important role in the defence, aviation and space industry. Ablative materials are the only ones to protect the rear wall surface from an excessive temperature rise while using a thin insulation wall. For the sake of the research, we prepared a series of samples of the composite produced with epoxy resin LH 145, H 147 hardener, carbon fibre mats and the addition of MMT. The prepared samples were tested on a unique stand in laboratory conditions. The findings obtained from the experimental testing after a detailed analysis were tabulated and presented in the form of graphs. The authors determined the ablative loss of mass of the individual samples, compared their internal temperatures, which had been measured with thermocouples, as well as the temperature on the backside of the sample. In addition, in order to complement the experimental studies of determining the temperature rise on the rear surface, the authors used a thermal imaging camera. Besides, they took photos of different layers of the examined structure, which had been exposed, to a heat stream, by means of a scanning microscope.

Opis:
Background: Our aim was to determine if there is a difference in demand for analgesic and sedative medication according to the type of catheter ablation for atrial fibrillation (AF). Material and methods: We collected data from protocols of 1144pts, who underwent ablation of AF. We excluded 275pts, at most due to electrocardioversion during the procedure. We divided them into 4 groups: cryoballoon ablation group (CB, n = 101), single-point radiofrequency ablation group (RFth-, n = 541), single-point radiofrequency ablation group with thermocool catheter (RFth+, n = 156) and Multielectrode Pulmonary Vein Ablation Catheter group (PVAC, n = 71). We used fentanyl and midazolam for pain control. The dose was adjusted by the operator, accord-ing to patients’ request. Results: The median dose of fentanyl 0.04 mg (0.00-0.08) and midazolam 1.00 mg (0.00-2.00) in CB group was lower than in other groups (p < 0.001). The median dose of fentanyl 0.12 mg (0.08-0.17) was lower in RFth- than in in RFth+ group: 0.15 mg (0.1-0.2) (p < 0.001). The demand for analgesia was higher when PVAC was used, with median dose of fentanyl 0.15 mg (0.1-0.2) (p < 0.0024). Conclusions: The demand for analgesic/sedative medication was lower among patients who underwent CB. Among those who underwent RF ablation it was higher in groups with thermocool and multielectrode catheters.

Opis:
In this article a three-dimensional mathematical model of radiofrequency ablation during open-heart surgery is presented. It was developed to study temperature field distribution into myocardial tissue. This model uses an anatomically correct 3D model for the left atrium, obtained by magnetic resonance imaging (MRI) processing of a patient; takes into account thermoelectric characteristic differences depending on the area of electric current application; considers cooling by the air flow. An ex-vivo experiment on the pig’s heart was performed where the depth of myocardium tissue damage was measured for the model validation. It was shown that the deviation of the model data from the experiment is within the limits of instrumental measurement error. The developed model is proposed to be used for heart ablation procedures planning, or new equipment development.

Opis:
This article presents the problem of climate warming and the effect of melting ice caps. The problem of climate warming is discussed in two stages. In the first stage, the factors affecting global warming are discussed in detail and the effects and risks of ablation extensively described. Analyses were conducted on data available online from NASA and Carbon Dioxide Information Analysis Center. The Greenland area (Jakobshavn Glacier) was selected to visualize glacier calving front changes. The analysis of changes was performed on the selected satellite images covering the summer period (June to September) provided by the Landsat program. Then, the changes in the position of the calving front of the Jakobshavn Glacier were visualized for the period 1985–2020, with a repeatability of every 5 years. Thus, our results addressed the challenges of environmental changes to remote sensing data processing. In addition to the visualization, a surface summary of these changes was presented in the study. The results were discussed in the context of climate change data processed by means of the GIS method. Furthermore, an analysis of the effects of greenhouse gases on glacier surface changes was performed. In summary, the results reveal that satellite imagery is an excellent source of data on which to visualize glacier calving rates, comparing individual layers showing the position of the glacier calving front and calculating the area of calved ice.

Opis:
In order to solve the aluminum surface ablation issue of Al-Steel bimetallic pipes manufactured by the explosive welding technique, a novel explosive welding system has been designed to weld Al-Steel coaxial pipes. The stand-off distance was chosen according to an empirical formula. A special Explosive Cord with an aluminum coating was used as the explosive. Four experiments were performed using Explosive Cords 1, 2, 3 and 4, respectively. In each experiment, three reliable PVDF (Polyvinylidene Fluoride) piezoelectric film sensors were used to sample the impact pressure between the parent pipe and the flyer pipe along the detonation direction. p-t Curves were obtained at different points on the bimetallic pipe manufactured by Explosive Cord 1. In order to observe the inner surface and to judge the bonding interface, specimens were cut along the axial direction. BSE (Backscattering Electron) images of the interfaces were obtained. According to these pictures from all of the specimens cut along the axial direction, the surface ablation phenomenon has disappeared. The bimetallic pipe manufactured by the new welding system using Explosive Cord 3 has an irregular wavy interface, between micro and small wavy interface, which is one of the best bonding forms.
