Opis:: This paper shows the current state of research and development over the telemedical services system for family doctor's practices. The introduction of this telemedical system should result in improvement in accessibility to medical services for the patient and to actual medical information, publications and up-to-date consultations for the physician, regardless of the distance from physician's clinic to the consultation center. This system should reduce the total costs of basic medical services. The system consists of two autonomous subsystems, that work in two arrangements: patient ↔ family doctor's practice and family doctor's practice ↔ information & consultation center. The remote medical care system provides vital signs monitoring through telephone line - physician can observe electrocardiogram and oxidation of blood, while the patient is many kilometres away. In case of unstable state of health, this system is able to alert the physician. The teleconsultation system provides a videoconference service with shared multimedia document (audio-video streams, graphic, text, etc.).

Opis:: The expectation that computer networks combined with the Internet and associated technologies will be common in all businesses of the future has resulted in a flurry of investment in ICT around the globe, and in wide interest in the Internet, e-commerce and e-business. For example the Malaysian government has created a high-speed data highway, a federal capital that is a 'wired city', and a 'paperless hospital' [1]. The UK Government too has recently approved plans for major ICT projects [5, 10]. These projects are in healthcare, and involve large financial sums. It is important that the use of these public funds leads to successful outcomes. This paper analyses a large successful ICT project that uses e-business technologies. The aim of the paper is to identify the success factors in this completed project to see if these pointers indicate favourable prospects for the outcome of these new healthcare projects.

Opis:: The paper presents a strategy for achieving high dependability of a computer-based system devoted to endoscopic examination (ERS). Two levels of replication are used: hardware (3 computers) and database (redundant copies). Archivisation of documents describing patient examinations and films made during such examinations is described. The influence of the used techniques on performance and dependability of the replicated system is estimated.

Opis:: The problem of medical teleconsultations with intelligent computer system rather than with a human expert is analyzed. System for content-based retrieval of images is described and presented as a use case of telemedical tool. Selected features, crucial for retrieval quality, are introduced including: synthesis of parametric images, regions of interest detection and extraction, definition of content-based features, generation of descriptors, query algebra, system architecture and performance. Additionally, electronic business pattern is proposed to generalize teleconsultation services like content-based retrieval systems

Opis:: The electrocardiogram is a digital record of effective action potentials of heart muscle fibres, thus it contains an important information about the heart function. Unfortunately, the signal theory-based description of the ECG contents is not efficient in extracting these data, and the practical interpretation software is mostly based on heuristic statements. This conclusion was a motivation for our research on the expert-like machine reasoning based on ECG perception analysis, physiology-driven feature extraction and fuzzy logic. This paper highlights the aspect of optimal description of an electrocardiogram contents in a digital record.

Opis:: The combination of medical sciences with computer networks and computer telematics creates a wide spectrum of new applications called telemedicine. One of telemedical domains is making digitally represented medical data available to medical technicians and professionals from remote locations. It can improve health care and lower its costs. The application discussed it this article is remote access to patients' medical documentation, and DICOM (http://medical.nema.org/) images in particular, for consultation purposes. The paper presents the research done so far by Distributed Systems Research Group (DSRG) in the area of telemedicine. It shows the functional evolution of applications designed and developed by the group. Starting from relatively simple Web-based services through dedicated access and consultation applications, DSRG has created a comprehensive and sophisticated environment called TeleDICOM, allowing participants from the distributed consultation team to diagnose patients in collaboration. Teleconsultation services, demanding as they are, should be designed with deep knowledge and experience in the area of medical data, distributed systems, computer networks and many others. All these issues have been considered in constructing the applications and discussed in the article.

Opis:: Information technology applications in medicine are rapidly expanding, and new methods and solutions are evolving since they are considered pivotal in the success of preventive medicine. In this paper two different concepts will be presented, the PANACEIA-iTV and the e-Vital concept. PANACEIA-iTV is a home care service provision system based on interactive TV technology and supported by the IST programme of the European Commission. The e-Vital service, supported by the eTEN programme of the European Commission, regards an integrated home care and telemonitoring service chain aimed at large sensitive parts of the European population, the "at-risk" citizens, who are usually patients with a stable medical condition that allow a near normal life but may suddenly deteriorate and put life at risk.

Opis:: In the paper the features of Bluetooth wireless mobile communication technology important in biomedical applications are discussed. Comparison with other wireless technologies in this field is presented. Several examples of prototype medical systems using Bluetooth devices are also shown as well as prospective applications of this technology in wireless, wearable Bluetooth-compatible biomedical sensors. Some of the author's own concepts concerning the Bluetooth technology usage models for wireless health care systems are described. Currently developed own prototype Bluetooth systems for wireless pulse rate measurement, respirometry and Holter Electrocardiography are described as well as some other projects for home care applications.
W artykule przeanalizowano właściwości bezprzewodowej, mobilnej technologii komunikacyjnej Bluetooth, istotne z punktu widzenia zastosowań biomedycznych. Zaprezentowano porównanie z innymi bezprzewodowymi technologiami stosowanymi w tej dziedzinie. Opisano przykłady prototypowych systemów medycznych z wykorzystaniem urządzeń Bluetooth, a także możliwe zastosowania tej technologii w rozwiązaniach bezprzewodowych, zdatnych do noszenia na ciele biosensorów. Przedstawiono własne koncepcje autora związane z modelami zastosowań technologii Bluetooth w systemach opieki zdrowotnej. Opisane zostały opracowane ostatnio własne prototypowe systemy Bluetooth do bezprzewodowych pomiarów częstości pulsu, respirometrii, elektrokardiografii holterowskiej, a także inne projekty związane z opieką w środowisku domowym.

Opis:: The paper reviews available literature on globalization processes in health services. The aim is to identify the main research challenges and basic trends in this area. The analysis is made with the use of SinceDirect, EBSChost, BMJ Journals, ProQuest Medical Library and MEDLINE-PubMed data and covers the 1990-2007 period. The author concludes that theoretical discussions and empirical studies on the globalization of health services are still at an early stage of development even though health services and related sectors have a considerable share in most countries’ GDP. The paper offers a modified version of a model for describing the influence of globalization on health. The author shows that the growing influence of globalization on national health care systems is accompanied by an increased share of the private sector in these systems. At the same time, countries are increasingly adopting measures to boost the role of private funds in financing their health care systems. Analyses show international medical services are almost exclusively provided by private businesses. This means that globalization is primarily benefiting private health service providers and private investors, the author says.

Opis:: This paper presents some aspects of the on-line remote fetal monitoring system based on the GPRS data transmission service and WAN network. The remote signal recording instrumentation consists of bioelectrical signal recorder and tablet PC computer. The central surveillance system located in hospital has a capability of simultaneous monitoring of up to 24 patients, both remotely and within hospital. The system enables analysis, dynamic presentation and archiving of signals and medical data. The clinical interpretation is immediately available in hospital as well as at patient's home or attending doctor office.

Opis:: W pracy przedstawiono uwarunkowania oraz oddziaływanie społeczeństwa informacyjnego a także rozwój technologii informacyjno-komunikacyjnych (ang. Information and Communication Technologies ICT), na stwarzanie możliwości uzupełnienia modelu świadczeń zdrowotnych poprzez wprowadzenie telopieki. Społeczeństwo informacyjne w pełni wykorzystuje elektroniczne środki komunikacji, takie jak internet, zasoby informacyjne i inne formy usług dostępnych poprzez urządzenia mobilne, m.in. GPS, GSM, oraz inne formy tzw. e-usług. Wydłuża się czas aktywności zawodowej, a wielu ludzi coraz więcej czasu poświęca pracy. W wielu krajach obserwuje się zjawisko starzenia się społeczeństwa, wydłużenia aktywności zawodowej i narastania problemów opieki zdrowotnej. Według demografów proces starzenia się społeczeństwa rozpoczął się już w 2005 roku, a jego szczyt przypadnie na lata 2010-2020. W następnej dekadzie w wiek emerytalny (kobieta 60 lat, mężczyzna 65 lat) osiągnie prawie 2 mln ludzi są to osoby urodzone w powojennym wyżu lat 50. W 2020 roku co piąty Polak będzie emerytem (21%) i to pod warunkiem, że współczynnik narodzin w Polsce będzie utrzymywał się na wyższym poziomie niż obecnie. Sposobem złagodzenia problemów służby zdrowia może być teleopieka, co może przyczynić się do poprawy komfortu życiowego ludzi starszych.
Determinants, as well as the influence of information society on the development of Information and Communications Technologies (ICT) is discussed in this paper. It is pointed out that telecare technologies may have several medical benefits in the existing health care systems. Information society exploits electronic mass-media like internet communication and information, as well as various mobile technologies like GPS, GSM and other forms of so called e-services. Many people work more extensively than some years ago. From the other hand, the aging of societies in the developed countries is observed. Demographic data point out that the process of society aging started in 2005 and will be specially extensive in years 2010-2020. In the following decade, 2 million people will reach the retirement age (women: 60 years old, men: 65 years old). In years 2020 every fifth Polish society will be retired (21%), providing that current birth rate will increase. Telecare may help to solve some medical care problems and to improve life standard.
