Introduction. Combined aplasia of multiple sinuses is extremely rare. Agenesis of the paranasal sinuses is an uncommon clinical condition that appears mainly in the frontal (12%) and maxillary (5-6%) sinuses. Case report. In this paper, we present the case of a 74-year-old woman with combined frontal and shenoid sinus aplasia accompanied by unilateral maxillary sinus hypoplasia. The findings were confirmed by a computed tomography scan of paranasal sinuses. The reason for admission was persistent headache, numbness of the left cheek and left alveolar process, and occasional nasal blockage. Discussion. The uniqueness of our case is that the patient is an elderly female with combined aplasia of the frontal and sphenoid sinus with hypoplastic maxillary sinuses, whereas previously reported cases were found in children and in young adults. Summary and conclusions. These anomalies can be misdiagnosed as chronic sinusitis or neoplasm. All potential sinus anomalies will have clinical implications and will hinder conventional and functional endoscopic sinus surgery

Opis:
Silent sinus syndrome (SSS) is a rare entity characterized by ipsilateral chronic hypoventilation and atelectasis of the maxillary sinus, which may lead to facial or ocular asymmetry, hypoglobus, enophtalmos and diplopia. In this report, we present a case of a 21-year-old patient scheduled for an orthognathic procedure due to skeletal malocclusion. Patient complained about problems with nasal breathing. CT scans revealed an atrophic, inflamed, hypoventilated left maxillary sinus with obstruction of the left ostiomeatal complex, septal deviation and hypertrophy of the left interior turbinate. Furthermore atrophic and asymmetrical left zygomaticomaxillary complex with shifting of maxillary bite plane and left malar depression accompanied by slightly displaced inferior border of left lateral orbital wall was noted. Due to clinical symptoms of SSS, patients are often referred to the ears, nose and throat (ENT) department or to the ophthalmologist. Silent sinus syndrome diagnosis begins upon clinical examination and it is confirmed by radiological imaging (usually CT). The most characteristic radiological finding is maxillary sinus volume reduction as a result of inward retraction of the sinus walls. It is important to perform differential diagnosis, taking into consideration other conditions that present with enophtalmos. The main goal of surgical treatment is to normalize the ventilation of maxillary sinus by improving its drainage toward the nasal cavity and, in selected cases, restoration of the orbital floor.

Chronic sinusitis of the sphenoid sinus is a disease that can cause a number of complications, mainly intracranial and ophthalmic ones. The proximity of critical anatomical structures such as the internal carotid artery, cranial nerves, cavernous sinus, dura mater and cerebral lobes puts any or all of these at risk and challenge due to the disease process itself and in surgical management as well. Infiltration of bone structures or its damage in the mechanism of compression is recorded in cases of fungal rhinosinusitis (FRS), Wegener's granulomatosis or actinitis with manifestation in the area of the sinuses. The authors present five patients with chronic sinusitis with bone destruction of the sphenoid sinus. Suspicion of fungal etiology in all cases was based on computed tomography (CT) images. The authors pay particular attention to the role of functional endoscopic sinus surgery (FESS) in chronic sinusitis with bone destruction, especially of a fungal etiology.

Introduction: Pilonidal sinus is a disease affecting individuals at young age. Numerous surgical methods have been described in the treatment of pilonidal sinus disease. Currently, there is no treatment method providing a rapid recovery with low recurrence risk. Aim: In this study, we aimed to evaluate the results of our patients who underwent Karydakis surgery. Material and methods: Results of 112 patients operated on between 2012 and 2019 were retrospectively evaluated. Only the patients who underwent Karydakis surgery were included in the study. Results: Of all patients, 20 were female and 92 were male. The mean age was 21 years in the female and 20.8 years in the male patients. The mean BMI was 29.6 kg/m2 . The patients were followed up for a mean of 28 months. Postoperative length of stay in hospital was 2.1 days. The complications included wound dehiscence in 10.2%, infection in 3.6%, and bleeding (hematome) in 7.1%. Four patients developed recurrence during follow-up. Conclusions: Karydakis surgery is a preferable surgical technique with low rates of complications and recurrence.

Pilonidal cyst is a troublesome condition affecting usually young people. It significantly influences quality of life and causes low self-esteem. Despite its known pathophysiology and numerous therapeutic methods available, pilonidal cysts still constitute a significant problem in general surgery. A large number of surgical techniques indicates the lack of a single method that ensures therapeutic success, and frequent complications cause both patient’s dissatisfaction and frustration of the operator. In many cases, therapy is based on methods developed in a given healthcare center and their efficacy is usually not scientifically verified. Search for optimal strategy is also impeded by lack of an unambiguous clinical classification. In this article, we reviewed publications on various methods of managing pilonidal cyst, and we also presented surgical treatment used in our department. However, we did not manage to point out a surgical method with efficacy high enough to become standard treatment. It indicates the need for further search for new techniques that will give chances for successful treatment of pilonidal cyst.

Introduction: Computed tomography is an important imaging technique in Emergency Units. Thanks to its popularity, radiological changes are found in healthy children more commonly. The aim of this paper is to evaluate the incidence of maxillary sinus radiological changes in children with head trauma who admitted to the Emergency Unit of the University Children Hospital in Lublin. Material and Methods: A retrospective analysis of computed tomography scans of children suffering from head trauma admitted to the Emergency Unit of the University Children Hospital in Lublin was carried out. A group of 425 patients was analyzed. Results: Maxillary findings were present in 81 cases (19.06%); in 38 patients (8.94%) the changes were unilateral, while in 43 (10.12%) they were bilateral. Maxillary mucosal thickening was the most common radiological abnormality, present in almost 12% of the investigated cases (approximately 62% of all revealed changes). Maxillary total opacification as an isolated finding was found in younger children only. Retention cysts and maxillary polyps were found with a similar low frequency as maxillary opacification but in elder children only. Discussion: Asymptomatic radiological changes in computed tomography scans are common. Maxillary mucosal thickening is the most frequent asymptomatic abnormality. Maxillary polyps and pseudocysts are rare in the paediatric population. Maxillary opacification suggests other more significant pathologies and requires further diagnostics. Physicians should avoid diagnosing patients with sinusitis without proper examination and based on radiological abnormalities only. Paediatric patients with revealed maxillary changes should remain under regular laryngological control.

Introduction: Mucoceles of the paranasal sinuses are more common in the frontal sinus than other sinuses due to multifactorial causes. Their close proximity to the orbit can lead to complications like diplopia, severe proptosis or even vision diminution. CT being the modality of choice, clearly demonstrates bony walls of paranasal sinuses and the extension of a mucocele into the surrounding structures. Mucoceles can either be excised endoscopically or by external approach. Case report: We describe our experience of three frontal mucocele cases all of which showed advanced orbital involvement and were managed successfully by endoscopic approach with no complications or recurrence. All symptoms including diplopia and proptosis significantly improved in every case. Hence, we reiterate the management of paranasal sinus mucocele via endoscopic approach as the preferred surgical modality having advantage of shorter recovery time, easy access, lower morbidity and incidence of potential complications.

Introduction: Treatment of the pilonidal sinus, due to various surgical methods, remains the current topic of discussion during surgeon meetings worldwide. The newest methods of treatment consist in the excision and simultaneous closure of the wound with cleft lift procedures. One of such methods is Bascom II procedure which has been widely used in the world but is less popular in Poland. Aim of the study: To evaluate the results of treatment of pilonidal cysts using Bascom II procedure performed by one operator. Material and method: 50 patients (40 men, 10 women), Avg. 30.6 years of age. All patients treated with Bascom II procedure in one surgical ward by one operator (resident during specialisation training) with the use of uniform surgical care for all operational protocols. The following were assessed: BMI, average hospitalisation time, pain, post-operative complications, wound healing time, patient’s quality of life, and the recurrences rate after surgery. The follow-up period ranged from 12 to 52 months. Results: Average BMI 27.13 kg / m2. Avg. time of hospitalisation 2.95 days. Post-operative pain in the first 24 hours was on average 4.55 points. (+/- 2.24 points); on the 10th day on average 2.04 points (+/- 1.58 points); in the 30th day on average 0.76 points (+/- 1.1 points). Pain after healing 0.14 points (+/- 0.40 points). Post-oerative complications occurred in 28.57% of patients [partial wound dehiscence (16.32%), serum leak (10.2%), hematoma (6.12%), total wound dehiscence (0%), wound infection (0%)]. The average time of full healing was 2.94 weeks. Recurrence occurred in one patient (2.04%). Conclusions: Bascom II procedure is characterised by a low recurrence rate and short wound healing time. It is a a safe, effective and patient-accepted method of treatment. Statistically, it significantly improves the patient’s quality of life one year after surgery in terms of both physical and mental health.

Isolated sphenoid sinus mucocele (SSM) is a rare entity that can result in serious sequelae if diagnosis and treatment are inappropriately delayed. Typically, mucoceles are asymptomatic, and they are accidentally identified after computed tomography scan or magnetic resonance imaging of the maxillofacial area performed for other pathological issues. We report a case of isolated SSM that presented only with a headache for over a year, and also review the literature regarding surgical management of such an entity.

Introduction: The topographic relation between the maxillary teeth roots and the maxillary sinus floor is important for diagnosis and planning of many surgical procedures. The aim of the study was to assess the topographic relationship between posterior maxillary teeth and the sinus floor. Materials and methods: 50 pantomographs of patients aged 18-72, treated at the Medical University of Gdańsk, were analysed. We analysed only maxillary molars and premolars. Teeth were assessed using Kwak classification. Statistical analysis was carried out using STATISTICA 13.3. Results: We analysed 180 molars and 181 premolars (total 361). According to the root-sinus classification, most of the first molars were type V (55.96%), most of the second molars – type III (32.29%), most of the first premolars – type I (76.09%) and most of the second premolars – type III (35.96%). Non-parametric Spearman’s rank-order correlation coefficient revealed significant correlation between right and left molars and between right and left premolars (p<0.05). Conclusions: Proper assessment of the pantomograph allows for correct pre-operative planning, which may affect the course of the procedure and enables to avoid possible complications. These findings may have clinical applications.

Introduction: Nasal and paranasal sinus polyps are one of the most common laryngological problems. Often, despite surgical treatment of nasal and paranasal sinus polyps, they grow back and require surgical retreatment. It is very difficult to predict which patients are particularly exposed to it. Markers are still being sought to predict which patients are particularly exposed to regrowth of polyps and thus require increased clinical surveillance. Galectins are a group of glycoproteins that have been intensively studied recently. The sugar part of these proteins can play a role in transmitting intercellular signals. Laryngologists are especially interested in galectins-1 and -3. The determination of their increased content in cancer tissue is considered as a marker of malignancy, which worsens prognosis in patients. Recently, more and more attention has been paid to the role of galectins in benign lesions, and such are the nasal and paranasal sinus polyps. Materials and methods: In our work, the contents of galectin-1 and-3 were determined in the tissue of the surgically removed primary (n = 35) and recurrent polyps (n = 15). R esults: The content of galectin-1 and-3 showed no statistically significant differences between primary and recurrent polyps. Conclusions: The content of galectin-3 was lower in recurrent polyps, however the observed difference did not reach statistical significance (p = 0.07). Since the obtained „p” value is close to the significance limit, it is advisable to broaden the submitted studies to a larger group of patients in order to be able to fully assess whether the determination of the content of galectin-3 may be helpful in assessing the risk of recurrence of nasal and paranasal sinus polyps.

Cel: Klasyczne metody leczenia brodawczaka odwróconego jam nosa i  zatok przynosowych to zabiegi z  dostępu zewnątrznosowego. Postępy w technikach nawigowania obrazowaniem, rozwój narzędzi chirurgicznych oraz metod obrazowania śródoperacyjnego doprowadziły do stopniowego odejścia od dostępów zewnątrznosowych na rzecz chirurgii endoskopowej. Ze względów anatomicznych i technicznych, chirurgia endoskopowa w leczeniu brodawczaka odwróconego jam nosa i zatok przynosowych rozprzestrzeniającego się do zatok czołowych wciąż pozostaje wyzwaniem. Poniżej przedstawiono doświadczenia własne autorów dotyczące chirurgii endoskopowej w leczeniu brodawczaka odwróconego jam nosa i zatok przynosowych rozprzestrzeniającego się do jednej lub obu zatok czołowych. Metody: Zaprezentowano 10 przypadków brodawczaka odwróconego jam nosa i  zatok przynosowych rozprzestrzeniającego się do zatok czołowych, usuniętego skutecznie za pomocą wewnątrznosowego otwarcia obu zatok czołowych (Draf III) pod kontrolą endoskopu, bez dodatkowego dojścia zewnątrznosowego. Wyniki: Po zakończonym zabiegu chirurgicznym cała jama obu zatok czołowych mogła być w  łatwy sposób kontrolowana. Nie obserwowano ani wczesnych, ani późnych powikłań. Po średnim okresie obserwacji (39,5 miesiąca), nie odnotowano nawrotów. Wnioski: Zastosowanie dojścia endoskopowego z obustronnym otwarciem zatok czołowych w leczeniu brodawczaka odwróconego jam nosa i  zatok przynosowych rozprzestrzeniającego się do jednej lub obu zatok czołowych jest możliwe i wydaje się być skuteczne.

Introduction. Oroantral communication (OAC) between the maxillary sinus and the oral cavity is an infrequent post-surgical complication occurring most commonly after extraction of posterior maxillary teeth. Aim. To present the characteristics of OAC and predisposing factors as well as evaluate postoperative pharmacological therapy and complications in patients with an OAC. Material and methods. In this retrospective study, medical records of 63 patients with diagnosed OAC between 2011 and 2018 were analyzed. Results. The most frequent causes for tooth extraction leading to an OAC were periodontitis (n=34; 54%), carious destruction of the tooth (n=14; 22.2%), and tooth impaction (n=10; 16%). First molars (n=28; 44.4%), second molars (n=14; 22.2%) and third molars (n= 13; 20.6%) were the most frequently related teeth to OACs. The majority of OACs appeared in the fourth (n=22; 35%) and third (n=20; 31.7%) decades of life. Conclusion. OACs are rarely seen on an everyday basis by general practitioners; however, if left untreated, they may lead to further serious complications. Proper postoperative precautions must be taken in order to prevent further complications, and thus the evaluation of predisposing factors is of great importance.

Objective: Sinonasal inverted papilloma has been traditionally managed with external surgical approaches. Advances in imaging guidance systems, surgical instrumentation, and intraoperative multi-visualization have led to a gradual shift from external approaches to endoscopic surgery. However, for anatomical and technical reasons, endoscopic surgery of sinonasal inverted papilloma extending to the frontal sinuses is still challenging. Here, we present our experience in endoscopic surgical management of sinonasal inverted papilloma extending to one or both frontal sinuses. Methods: We present 10 cases of sinonasal inverted papilloma extending to the frontal sinuses and successfully removed by endoscopic median drainage (Draf III procedure) under endoscopic guidance without any additional external approach. Results: The whole cavity of the frontal sinuses was easily inspected at the end of the surgical procedure. No early or late complications were observed. No recurrence was identified after an average follow-up period of 39.5 months. Conclusion: Use of an endoscopic median drainage approach to manage sinonasal inverted papilloma extending to one or both frontal sinuses is feasible and seems effective.

Opis:
The dental implants are becoming more used by dental surgeons to perform prosthetic rehabilitation, therefore sinus lift procedure is more often performed. The complications after this procedure by the ENT specialists will be observed. So far a valid algorithm of candidates with naso-sinusal diseases for sinus lift procedure was not established. The interdisciplinary management, including ENT specialist, not only increases the expectation of a better procedural outcome, but also provides a better control while complications occur. The authors postulate that knowledge about sinus lift technique and possible postoperative complications should be well known, both by stomatologists and ENT specialists.
Wraz z rozpowszechnieniem rehabilitacji protetycznej pacjentów przy wykorzystaniu implantów zębowych zwiększyła się także częstość wykonywania zabiegu podnoszenia dna zatoki szczękowej. Z tego względu lekarz laryngolog częściej będzie spotykał się z powikłaniami pozabiegowymi u pacjentów po zabiegu sinus lift. Do tej pory nie ustalono konsensusu, co do algorytmu postępowania u pacjentów z chorobami nosa i zatok przynosowych przygotowywanych do zabiegu podniesienia dna zatoki szczękowej. Wydaje się, że podejście interdyscyplinarne pozwala na obniżenie ryzyka możliwych powikłań pozabiegowych, a w przypadku ich wystąpienia opanowanie trudnej sytuacji. Z tego względu autorzy są zdania, że dla lepszej współpracy pomiędzy lekarzem stomatologiem a laryngologiem procedura zabiegu podniesienia dna zatoki szczękowej oraz możliwe komplikacje pozabiegowe powinny być powszechnie znane.
