Introduction and aim. In real life, metastatic castration-resistant prostate cancer patients (mCRPC) had more complex clinical presentation than patients in the COU-AA-302 trial. This study primarily aimed to describe the overall survival of chemotherapy-naive mCRPC treated with abiraterone acetate plus prednisone (AAP). Other relevant outcomes and baseline characteristics of these patients were also evaluated. Material and methods. This retrospective, observational study collected data from chemotherapy-naive mCRPC patients treated with AAP in Vietnam. Kaplan-Meier curves were used to estimate time to treatment failure (TTF), and overall survival (OS). The impact of baseline characteristics on OS was explored using univariate and multivariate Cox proportional hazard models. Results. Data from 65 eligible patients were analyzed. The rate of PSA response was 73.8%, median PSA PFS was 10.5 months (95% CI: 7.4–13.6), median TTF was 15 months (95% CI: 11.1–18.9), and median OS was 24.9 months (95% CI: 18.9–30.9). Shorter OS was significantly associated with a higher Gleason score (≥8), shorter time from ADT start to mCRPC (<12 months), visceral metastases, and <50% PSA decline (p<0.05). Conclusion. Abiraterone acetate plus prednisone is well tolerated and effective for chemotherapy-naive mCRPC patients in clinical practice. Moreover, Gleason score, visceral metastasis, time from ADT start to mCRPC, and PSA response are the independent indicators for predicting the OS of mCRPC patients in both univariate and multivariate analyses.

A special form of prostate cancer is non-metastatic castration-resistant prostate cancer. Apalutamide is a non-steroidal selective androgen receptor inhibitor approved for the treatment of high-risk non-metastatic castration-resistant prostate cancer and metastatic hormone-sensitive prostate cancer. The paper describes a case of a patient diagnosed with prostate cancer with a long-term course of the disease. The patient was diagnosed with a non-metastatic castration-resistant stage 18 years after primary treatment. Systemic treatment with apalutamide was recommended. Initially, the treatment was carried out as part of the extended access to apalutamide program and from March 2022 as part of the B.56 drug program. The study presents the effectiveness and safety of the therapy in a 12-month follow-up period and discusses controversial aspects of the patient's previous treatments.

The treatment of patients with prostate cancer, who do not qualify for the radical therapy is based on the use of surgical or pharmacological castration methods. The effectiveness of the so-called androgen deprivation tends to decrease over time. Even in the absence of distant metastases on imaging studies, the resistance for the hormonal castration may be developed. At this moment, the use of new generation androgen receptor inhibitors may reduce the risk of metastases or death. The paper presents a case report of a patient with prostate cancer without distant metastases who started therapy with apalutamide at the moment of developing re-sistance to the pharmacological castration.

Bladder cancer (BCa) and prostate cancer (PCa) are genitourinary cancers which constitute significant health problems in men and in which environmental factors play an important role. Understanding the genetic susceptibility to BCa or PCa and occupational exposure is paramount to improving cancer prevention and early detection. The aim of this review article was to address the scientific evidence on the genetic risk factors and occupational exposure associated with the occurrence of BCa and PCa. The authors identified relevant original articles that have been published between 1994 and 2023. Variations of the following search terms: “gene” and “occupational” combined with one of the following terms: “bladder cancer” or “prostate cancer” were applied for the search purpose. The authors found 342 publications of which 50 population studies met their requirements for gene-occupation interactions. In total, 34 full-text manuscripts were about BCa and 16 about PCa. These research examines the genes involved in detoxification processes of xenobiotics (glutathione S-transferase, N-acetyltransferase, cytochrome P450, UDP-glucuronosyltransferase), oxidative stress (glutathione peroxidase 1, manganese superoxide dismutase, catalase), altering DNA repair capacity (X-ray repair cross-complementing 1, base excision repair, nucleotide excision repair), tumour suppression (TP53 gene), and vitamin D pathway (vitamin D receptor gene). The role of genetic factors in the occupational exposure has not been conclusively established, but it appears the possibility of genetic involvement. Determination of environmentally responsive genes provides important mechanistic implications for the etiology of occupational cancers, and valuable input in occupational exposure limits set by taking genetic susceptibility into account. Obesity and a lack of sufficient physical activity (PA) are recognized as risk factors for most civilization diseases, including cancer. This study synthesized the current evidence evaluating the relationship between excess body weight and prostate cancer (PCa) in the relation to the disease risk, progression, and mortality, and identifies biological plausibility of the association. We also estimated the importance of PA in intentional body weight loss. Several electronic major databases to identify eligible articles were searched until March 2022. A total 22 observational articles, the literature on the underlying biological mechanisms, and the crucial evidence of a role of PA in body weight maintenance and reduction were reviewed. The available knowledge suggests that association between body mass index and PCa is conflicting. However, the most research consistently shown that overweight/obesity was associated with higher risk of high-grade PCa and dying of PCa. Overweight/obesity can promote high-grade PCa through increased levels of secreted adipokines, increased formation of proinflammatory agents, and reduced concentration of adiponectin, among others. Being obese may by also linked with a higher risk of mortality. Exercise can decrease these health consequences related with obesity and may be effective in reduction of PCa-specific mortality, however, there are relative few studies on PA and PCa prevention among obese individuals.

For several years, prostate cancer has remained the most common malignancy in male patients in Poland. A large number of patients combined with rising costs of therapy translate into a significant socio-economic burden. In a clinical oncologist’s practice, we usually deal with patients with advanced prostate cancer. Taking into account the poorer prognosis in advanced disease, development of new therapeutic options as well we their adequate selection is of paramount importance. Enzalutamide is one of the second-generation androgen axis inhibitors, which has the ability to overcome resistance to androgen deprivation therapy by inhibiting the androgen-DNA signalling on several levels. Its efficacy and safety had been proven in numerous phase II and phase III clinical studies and it has been registered by regulatory authorities in the United States and European Union in the treatment of mCRPC, mCSPC and nmCRPC. In this article selected issues related to the treatment of prostate cancer are discussed, with particular emphasis on the role of enzalutamide (including its mechanism of action, indications, efficacy and safety).

Prostate cancer is one of the most frequently diagnosed cancers in men. Number of newly diagnosed cases is increasing due to several factors and the most important ones seem to be: population ageing and more sensitive diagnostic procedures. Secondary – the higher efficacy of treatment with its influence on improving patients’ overall survival and the specific mechanism of action of drugs used in systemic therapy lead to growing population of men suffering from prostate cancer in general and, specifically – patients with castration resistance. It is hormone therapy to play the key role in systemic treatment of prostate cancer with increasing significance of novel drugs focused on inhibition of molecular signal transduction mediated by androgen receptor. Abiraterone acetate is the representative of this therapeutic class. The paper describes the most clinically relevant data regarding the drug.

Modern antiandrogens: abiraterone acetate (inhibitor of CYP17 cytochrome) and enzalutamide (irreversible inhibitor of androgen receptor) are the drugs that are increasingly often administered in treatment of castration-resistant prostate cancer. Despite their clinical efficacy, especially in terms of prolonged survival and improved quality of life of patients, they pose of problem for a practicing oncologist such as possible cardiovascular complications (particularly arterial hypertension), which may lead to the cessation of this form of therapy. This article provides a brief overview of the mechanisms responsible for the above complications, including practical recommendations in the event such complications arise. A simple scheme of action for control of cardiovascular risk factors has been presented, which can improve the prognosis in this population of patients.

W ostatnich latach obserwujemy uobecnianie się „dyskursu prostatycznego” przez jego literackie zapisy, kulturowe reprezentacje, akcje świadomościowe czy wreszcie publiczne debaty. Ten moment jest interesujący dla badacza‑literaturoznawcy: „dyskurs prostatyczny” analizować trzeba na przecięciu kilku relatywnie nowych subdyscyplin nauk humanistycznych i społecznych: krytycznych studiów nad mężczyznami i męskościami, age studies i dyskursu maladycznego.Sięgając do tych trzech obszarów, autor poddaje lekturze powieść Duch wychodzi Philipa Rotha, powieść L’ablation Tahara Ben Jellouna (I wyd. 2014, nietłumaczona), esej/dziennik Philippe’a Petita: Philosophie de la prostate (wyd. 2018, nietłumaczona).Na podstawie przeprowadzonych analiz autor formułuje wniosek, że dokonujące się na naszych oczach rozpoznanie prostaty jako ważnego obszaru męskiej cielesności może przyczynić się do znaczących przesunięć w rozumieniu kulturowych i społecznych konwencji męskości.
The prostate cancer is one of the most widespread types of cancers among men. The disease attacks not only the male’s body, but it deeply concerns men’s social and cultural identity. As such, this type of cancer has been tabooed and its influence on men’s identity was silenced, even if Susan’s Sontag influential essay Disease as Metaphore aimed at redefining and demythologizing cancer. Literary and cultural representations of prostate cancer, identity andawareness‑rising actions among men have created in recent years a kind of “prostatic discourse.” This moment has a particular significance for a literary scholar: the newly established discourse could be analyzed intersectionally with the use of different methodologies: critical studies on men and masculinities, age studies, and maladic studies.On the basis of these fields of research I’m going to analyze Philip Roth novel Exit Ghost, L’ablation by Tahar Ben Jelloun, Philippe Petit’s Philosophie de la prostate.In conclusion I argue that the “prostate discourse” establishes the prostate as an important part of men’s bodies. It leads to a further conclusion that this recognition could result in important shifts in our understanding of cultural and social conventions of masculinity.
