Introduction and aim. Sub-centers (SC) are the first contact point with the community with auxiliary nurse midwife (ANM) as the instrument, delivering all the primary health care services. The SCs are under constant criticism for their inability to deliver quality services. This study assessed the preparation of facilities available at the SC to manage high-risk pregnancies (HRP) and to compare the same between rural and tribal blocks of the selected district. Material and methods. This health facility-based cross-sectional observational study was done for 6 months among 276 rural and tribal SC of the Udaipur district by a two-stage random sampling method using an observational checklist to assess the infrastructure and logistics of SCs. Data were analyzed using SPSS 20. Results. The study covered 264 (95.7%) non-24x7 SCs and 12 (4.3%) 24x7 SCs. Only one-third SCs, 93 (33.6%) were situated at the center of the village. Only 151 (54.7%) SCs had attached ANM quarters. All 24x7 SCs and 78.4% of non-24x7 SCs had adequate equipment and infrastructure. Conclusion. Most of the subcentres’ infrastructure and functional equipment was equipped to tackle HRP. Rural SC adhered more than tribal. Most HRPs were tracked and referred to higher centers. Unless we emphasize strengthening SCs, the dream of a healthy nation will remain obscure.

Primary Health Care (PHC) is implemented by the local governments in Nigeria based on the notion that healthcare service delivery is efficient when it is closer to people. With the adoption of analytical research through secondary sources of data, this study assesses the local government performance in PHC service delivery and examines the challenges it faced in the south-western Nigeria states of Lagos, Ogun, and Ondo in the period of 2010–2015. The findings showed various PHC programmes implemented by the local governments of the selected states, which include Maternal and Child Reduction, Investment Case and Eko Free Malaria in Lagos state, Araya and Gbomoro in Ogun state, as well as the Mother and Child Hospitals and Agbebiye in Ondo state. However, findings showed similar challenges encountered by the local governments, which include multiplicity of stakeholders in the disbursement of PHC funds, lack of fiscal decentralisation of revenue to the local government, diversion of PHC funds as compared to other projects, and poor community participation in PHC, irregularity of or interference in the local government elections by the state government, exclusion and deviation from the key principles of PHC and lack of continuity in PHC programmes implemented. The general conclusion from the study is that the local government performance in PHC service delivery is weak. In order to enhance the present local government performance in PHC, the study recommends for the application and implementation of local government constitutional role in the management of the PHC service delivery.

Artykuł przedstawia problematykę opieki zdrowotnej uczniów szkół podstawowych w kontekście zagadnień prawnych i organizacyjnych na podstawie Dzienników Urzędowych Ministerstwa Oświaty w latach 1945–1961. Na podstawie zarządzeń, instrukcji, okólników i komunikatów tego ministerstwa przedstawiony został model organizacyjny opieki higieniczno-lekarskiej, jaki ukształtował się na początku lat 50., doprowadzając po powstania szkolnej służby zdrowia. W 1952 r. została ona przejęta przez Ministerstwo Zdrowia. Ponadto pośredni wpływ na kwestię zdrowotności uczniów miało wiele innych czynników, takich jak: stan sanitarny szkół, wdrażanie zasad higieny u uczniów, promocja zdrowia i zmiana świadomości rodziców w sprawach związanych z higieną i zdrowiem. Wszystkie te problemy dostrzegało Ministerstwo Oświaty, odnosząc się do nich w swoich dziennikach urzędowych. Jednocześnie nie zabrakło w nich wymiaru propagandowego charakterystycznego dla polityczno-społecznej rzeczywistości Polskiej Rzeczypospolitej Ludowej. Przykładem tego była między innymi organizacja na terenie szkół ogólnopolskich wiosennych akcji sanitarno-porządkowych.
The article presents the issues of health care for primary school students in the context of legal and organisational issues based on the Official Journals of the Ministry of Education in 1945–1961. On the basis of the ordinances, instructions, circulars and communications of this ministry, the organisational model of hygienic and medical care was presented, which was shaped in the early 1950s, leading to the establishment of the school health service. In 1952, the Ministry of Health took over hygienic and medical care in schools. Moreover, a number of other factors indirectly influenced the issue of students’ health, such as: the sanitary condition of schools, the implementation of hygiene rules for students, health promotion and changing parents’ awareness of hygiene and health matters. All these problems were noticed by the Ministry of Education referring to them in its official journals. At the same time, they did not lack the propaganda dimension, characteristic of the political and social reality of the Polish People’s Republic. An example of this was, among others, the organisation of spring sanitary and cleaning actions in schools.

W związku z wybuchem pandemii COVID-19 cele systemu ochrony zdrowia musiały zostać dostosowane do zmieniającego się otoczenia, aby zrealizować potrzeby zdrowotne pacjentów i wypełnić oczekiwania personelu medycznego dotyczące zapewnienia bezpiecznych warunków pracy w zaistniałej sytuacji kryzysowej. Zmiany działalności personelu medycznego, które wynikają przede wszystkim ze zmian organizacyjno-finansowych, dotyczą systemów ochrony zdrowia na całym świecie i wpływają na funkcjonowanie wszystkich form opieki zdrowotnej. Celem tego artykułu jest wskazanie zmian organizacyjnych i finansowych wynikających z wprowadzonych od ogłoszenia przez WHO pandemii do 8 maja 2021 r. aktów prawnych i zaleceń wpływających na warunki pracy personelu medycznego podstawowej opieki zdrowotnej (POZ) w Polsce. Przegląd wprowadzonych działań w zakresie zapewnienia stabilności funkcjonowania POZ w warunkach pandemii pozwala stwierdzić, że sytuacja zagrożenia zdrowia publicznego ujawniła znaczną potrzebę wprowadzenia zmian organizacyjno-finansowych. Skutkiem zmian wynikających z legislacji oraz dobrych praktyk o istocie medycznej, organizacyjnej i finansowej są modyfikacje systemu ochrony zdrowia w Polsce, które mogą w nim pozostać. Warto jednak podkreślić, że jednym z istotnych wyzwań w zakresie przyszłych reakcji systemu ochrony zdrowia, w tym POZ, na stany zagrożenia zdrowia publicznego jest zachowanie spójności zmian organizacyjnych i finansowych wpływających na płynność oraz skuteczność działań podejmowanych przez personel POZ, a więc również na warunki jego pracy. Punktem wyjścia projektowania takich zmian powinna być analiza rozwiązań wprowadzonych od początku pandemii w Polsce. Można do nich zaliczyć nowe wymagania o charakterze organizacyjnym (warunki lokalowe, organizacja i stanowiska pracy, przepływ informacji czy sposób zaopatrzenia pacjenta) i zmiany finansowe (mobilizację dodatkowych zasobów finansowych w różnych formach). W artykule przedstawiono wiele pytań badawczych, które są warte rozważenia przy ustalaniu problemów i priorytetów w przyszłości, ponieważ zaistniałe zmiany i wnioski sformułowane na podstawie ich analizy mogą przyczynić się do wprowadzenia stałych modyfikacji funkcjonowania POZ w Polsce i ułatwić jej ewentualne dostosowanie do stanów ryzyka zdrowotnego. Following the outbreak of the COVID-19 pandemic, the objectives of the health care system had to be adapted to the changing circumstances, in order to meet the health needs of patients, but also the expectations of medical workers related to ensuring safe working conditions in the crisis situation. The activities of medical staff are greatly affected by organizational and financial changes in health care systems, which affect both the health care systems all over the world and the functioning of all forms of health care. The article examines the organizational and financial changes resulting from the introduction of regulations affecting the conditions of primary health care (PHC) workers in Poland from the beginning of the COVID-19 pandemic to May 8, 2021. The findings regarding measures taken to ensure the stability of PHC functioning during the pandemic highlight that the public health emergency exposed a significant need to introduce organizational and financial changes in PHC. The changes arising from legislation and good practices of medical, organizational and financial character resulted in health care system modernizations in Poland. It is worth stressing, however, that there is a great need to maintain coherence when implementing organizational and financial changes affecting the fluidity and effectiveness of the actions taken by PHC personnel, and thus their working conditions, when implementing future responses to public health emergencies. Such changes should be based on an analysis of the solutions introduced since the beginning of the pandemic in Poland: these include organizational changes such as housing conditions, organization of work and workplaces, flow of information and way of supplying the patient, and financial changes involving mobilization of additional financial resources. The article presents a list of future research questions that merit consideration when setting problems and priorities: these can be used to guide the introduction of permanent modifications to the functioning of PHC in Poland and to facilitate possible future adaptation in times of emergency. Introduction. Currently, malignancies are the most severe medical problems worldwide. Numerous, already known risk factors in carcinogenesis could be potentially avoided. Some cancer risk factors have been recognized and have become the targets of primary prophylaxis. Objective. The aim of the study was to ascertain the state of knowledge about risk factors, primary prevention and early detection of malignancies of gastrointestinal tract (GIT) in the urban and rural population of the Lublin province in Eastern Poland. Materialas and method. The study was cross-sectional. The originally designed questionnaire was applied to the group of 1,352 patients, representatives of both the rural and urban environments of the Lublin province during random appointments with their general practitioner (GP). Results. The study showed low awareness of the issues connected with GIT malignancies within the studied group. The problem was particulary apparent in the rural population. Conclusions. In order to raise general awareness of cancer, different means should be applied in urban and rural populations. GPs and the media were found to have the leading rols in the promotion of primary prevention.

Introduction and objective. For an effective and desired outcome of therapy to be achieved, the technical capacity of the healthcare worker is a key factor. The aim of the study is to assess knowledge and attitudes of primary healthcare (PHC) workers towards rational artemisinin-based combination therapy (ACT) prescribed in the treatment of uncomplicated malaria. Materials and method. This is a descriptive cross-sectional survey in which a self-administered structured questionnaire was used. A combination of stratified and multistage sampling techniques where utilized in the selection of the healthcare workers (HCWs). Results. Of the 422 respondents, 100 (23.7%) were males and 322 (76.3%) were females. Respondents with less than five years in practice accounted for 36%, while 64% had six years and above of practice. Community health extension workers (CHEW) accounted for 54%, while 46% were either community health officers (CHO), nurses, midwives, Bachelors of Science (BSc) in community health, or with other qualifications. A total of 390 (92.4%) were aware of the availability of ACTs and used the malaria treatment protocols, whereas 28 (6.6%) still used monotherapy for simple malaria treatment, and 2.7% did not use any form of test before initiating treatment. Co-administration of non-ACTs with ACTs accounted for 12%, with 88% prescribing paracetamol with ACTs. 67% of the respondents required capacity building in rational ACT use. Conclusion. The majority of the respondents had the requisite knowledge and skills coupled with positive attitudes in prescribing ACTs. Some respondents could not distinguish between an ACTs and a monotherapy. This may affect the exact application of the malaria treatment protocols, especially at the PHC level.

Contemporary world events are demonstrating the need to embrace and further develop tele-health options for assessment and delivery of biopsychosocial healthcare services. This is now possible, given advances in communication technologies allowing virtual connections of medical personnel with constituents, as well as necessary, in light of recent challenges posed by infectious conditions and growing needs for travel restrictions, social distancing, and isolation of large portions of populations. Moreover, the opportunity to virtually connect with persons through ubiquitous computer-based and handheld communication devices allows comprehensive care provision to include underserved areas, where restricted, walk-in access to brick-and-mortar establishments has long been recognized as a limiting factor in healthcare. This review examines evolving approaches of tele-healthcare, with a specific focus on telemedicine as a bridge between traditional, in-person approaches to diagnose and treat medical conditions and new healthcare opportunities developing to meet changing societal needs. The three purposes of the review are: discuss background information, with a brief look at policy and procedure guiding applications of tele-techniques in healthcare practices; identify relevant scientific studies to show the breadth of new evidence-based research for telemedicine practices; and, discuss challenges for the further development of telemedicine as healthcare systems in the United States evolve to meet current and projected healthcare needs.

Introduction. E-Health tools allow a medical facility to set a given patient’s data in order using ICT techniques, and the patient to use those techniques when contacting a given organisation. Materials and method. Secondary statistical data was used in the research. The study was carried out among primary health care patients. Mining for affinity rules was done in the R programme. The apriori and inspect functions from the arules package were used. Moreover, any redundant rules were removed from thoseobtained using the afero-mentioned method. Applying the general description of the affinity analysis method onto the survey described herein, it should be stressed that the aim of using affinity analysis was to discover the rules which contain the sub-transaction B={V_6=1} as a consequent. This was determined by the intention to discover associations regarding the knowledge about a uniform information system that the patients under study might have. Results. In the discovered rules, the antecedent most often contained an indication of the need for introducing a uniform solution as regards telemedicine. Moreover, according to the opinions of ‚conscious‘patients, a uniform IT system should improve the work at primary health care institutions, introducing an on-line booking system for visits should improve the productivity and comfort of doctors, and an IT system should provide unambiguous identification of a patient. Conclusions. There is potential in using affinity analysis within e-Health. The example of affinity analysis described in his study led to the discovery of interesting and important (from the point of view of a medical facility) regularities regarding the knowledge and expectations of patients as regards e-Health.

has dramatically increased. The number of visits to EDs can be an important indicator of the quality of primary health care. Objectives. To analyse: 1) the frequency of admissions to EDs, 2) the type and number of medical procedures performed in EDs, and 3) the type and number of services provided by PHC in the Kedzierzyn-Kozle district. Material and methods. A retrospective analysis of the statistical data regarding services provided by PHC in the Kedzierzyn-Kozle district within the National Health Fund (NHF) and data from the hospital emergency department in Kedzierzyn-Kozle (Opole Province, Poland). Results. The results showed an annual increase in the number of hospitalisations (13,815 – 2012, 14,192 – 2013, 15,123 – 2014) and the number of medical procedures performed in the ED (mainly those from I–III categories), as well as the low rate of admission from the ED to ICU (Department of Anaesthesiology and Intensive Care). There was an increase in the overall number of consultations given by a primary health care doctor and the total number of healthcare services provided at night and during holidays. Conclusions. Reducing the number of non-urgent visits to EDs can be achieved by making efforts to meet the needs of patients at the level of primary health care, in particular through better coordination of services provided by EDs and PHC and better motivation of primary care doctors to perform the role of gatekeeper to the health care system

In 2019, twenty-five years have passed since the implementation of the first legisla-tive act introducing for the first time family doctors (primary health care physicians) into the Polish model of basic health care. The beginning of changes in this area, falling in the nineties of the last century, was not easy and has not yet been complete but will continue until the end of 2024. Over the years, not only the conditions for the education and professional development of primary health care physicians, in-cluding family doctors, have undergone changes. Also the organisational and legal forms of providing basic health services by this group of doctors were changing, as well as the conditions allowing patients, the recipients of medical services, to choose their doctor from among those with whom health insurance institutions (health insurance funds, or the National Health Fund) concluded agreements on the provision of basic health care services, or those employed by a medical entity with whom the National Health Fund signed relevant agreements.The scope of changes introduced at that time was wide, and when it started at the beginning of the 1990s it was almost impossible to achieve the goals without the support of the World Bank and the PHARE programme, alongside the efforts of the Ministry of Health and Social Welfare and three regional health care consortia (unions). Their work and contribution of each of them and their activities were sig-nificant and together constituted a solid basis for further work on the improvement of the model of a universal primary health care after 1998.The Act on primary health care of 2017 has completed the process of changes in this respect. For the first time, the objectives of the basic, or primary health care have been clearly defined. Although the implementation of each of these objectives is to serve the patient who is the recipient of the services, the value of the services pro-vided by primary care physicians varies. Family doctors (general practitioners) are to provide the patient and the patient’s family with health care services, coordinating at the same time all the stages of the process of their provision. However, the social mission of their profession is to ensure the implementation of broader tasks, such as health promotion adapted to the needs of various groups of society, education of the medical services recipients in the field of responsibility for their own health, as well as shaping pro-health awareness, setting health priorities of the population covered by care and implementation of preventive actions.

Local-level challenges to implementing health education programs in rural settings: a qualitative study

Background. Health education is one of the main cores of primary health care (PHC). However, there is limited evidence on the difficulties of implementing health education programs. This study explored the barriers of implementing health education programs in Iranian rural communities. Objectives. A qualitative study with conventional content analysis approach was conducted. Applying purposeful sampling, 34 rural folks and health care providers were employed to participate in the study. Data were collected through individually, semi-structured interviews. Data analysis continued until data saturation, when no new theme or idea emerged. Material and methods. A qualitative study with conventional content analysis approach was conducted. Four themes, including “Ineffective teaching and learning processes”, “Lack of health educators’ motivation”, “Communication gaps”, and “Lack of resources and facilities for teaching and learning” emerged as the barriers of implementing health education programs in rural communities. Conclusions. Several executive and communicational problems were identified as the local-level obstacles of implementing health education programs in rural areas. Better understanding on the extensive range of health education barriers in rural areas may be helpful for rural health workers and stakeholders in designing and/or revisiting health education programs in rural communities.

Opis:
The article presents the risk of occupational burnout observed in health care, as well as possibilities of prevention in this regard. Its authors used their knowledge and experience obtained during the implementation of the international Time2Grow project financed from the European Social Fund. The project was dedicated to developing a burnout prevention model mainly in the area of broadly understood health care. The project was implemented in the Polish-Belgian-Finnish partnership, thanks to which it provided great opportunities to benefit from good European practices. The article presents the results of quantitative primary research carried out on Poland in the group of nurses and midwives, as well as selected results of quantitative and qualitative research conducted in Finland on a group of practicing nurses caring for elderly people.

Opis:
Quality in primary health care is gaining a predominant role. Both public and private providers of primary health care services increasingly focus on quality issues, which allows them to gain a competitive advantage. In the article, fifty attributes determining the quality of services identified in literature are considered and evaluated in the context of primary health care services. Both technical attributes relating to the medical process itself and functional attributes resulting from patient experience as a client are studied. An analysis aiming to rank the importance of attributes was made on the basis of surveys carried out among 98 primary health care PHC managers. The research allowed for the identification of the most and the least important primary health care service attributes determining quality from the perspective of PHC managers. Functional attributes relating to patients` experience of care are being noticed as important and desired by patients. The findings resulting from this study can be practically applied at primary health care entities in order to maximise the satisfaction of patients through proper adjustment of business models.

Background. In general practice, dysfunctions within the locomotor system are a recurring health issue. Most frequently, diagnoses and treatments relate to pain syndromes of the backbone, the shoulder girdle or the pelvic girdle. The authors believe that physiotherapy, along with other clinical disciplines, should be regarded as an important factor which influences the effectiveness of the therapeutic process in this area. In primary care, treatment of musculoskeletal disorders – especially at the stage of early clinical symptoms – should incorporate basic physiotherapy methods, e.g., massage, physical procedures, kinesiotherapy and the underrated education of the patient. Restoring appropriate spatial arrangement of tissues provides the right conditions for the regeneration and repair of muscles, ligaments and tendons, although it is a process that requires a long time. Therefore, it can be very important to introduce self-therapy in the form of systematically repeated, easy-to-replicate procedures in the scope of self-massage and self-kinesiotherapy. Objectives. This paper presents the impact of physiotherapy in treating selected disorders and pain syndromes of the locomotor system with particular attention to the role of massage. Emphasis is placed on the meaning of self-massage in the process of restoring structural balance of tissues. The model of active inclusion of the patient in the treatment process as preparation for self-therapy is presented. This paper aims to justify the need to reorganize health services provided through general practicioners within the National Health Fund network by incorporating physiotherapy in primary care.

Background: The National Health Fund (NHF) is the public payer solely accountable for securing and organizing access to health care services in Poland. The NHF is responsible for implementing a project entitled “Primary Health Care PLUS” which aims to introduce a primary care centered model, based on coordinated, proactive and preventive methods relevant to patients’ needs and furthermore, works to keep patients well-informed and active participants in health care decision-making. The implementation period of the project is July 1, 2018 through Dec. 31, 2021. Aim of the study: The purpose of this study is to outline patient demographics and staff structures of providers that took part in the PHC PLUS pilot program, as well as the status of the program throughout the first year of its implementation. Material and methods: Following an open and transparent recruitment process, 42 urban and rural primary health care providers were selected. The purpose of the providers’ geographic spread was to ensure the model was tested in all representative regions of the country. Results: 42 PHC PLUS providers attending to 288,392 patients are participating in the project. Approximately 1,100 medical staff members are involved in the project. PHC PLUS medical teams consist of specialists including physicians, coordinators, nurses, dietitians, psychologists, physiotherapists and health educators. Out of 41,022 health risk assessments declared to be conducted during the project, 18,058 (43.1%) were performed from July 1, 2018 to April 30, 2019, including 4,537 basic and 13,521 extended assessments. Furthermore, 15,020 patients in total, participated in the disease management programs, which are also paid from the project. Conclusions: A Polish health care pilot project that centers primary care can help shift the focus toward preventive interventions, rather than the current system, which often focuses on providing medical care to patients who have already been diagnosed with diseases, often in their more advanced stages.
