The authors deal with the problem of malaria induced by Plasmodium falciparum and imported to Poland by people returning from tropics. They stress significance of the variable clinical pattern involved and of chloroquine resistance. Basing on their own observations of a definitely diagnosed malaria (22 out 35 cases observed and suspected of malaria) the authors discuss diagnostic and therapeutic difficulties encountered in 7 patients with Plasmodium falciparum-induced malaria. Two representative cases of malaria (Plasmodium falciparum) have been discussed in detail. One of the cases with malaria imported from tropics, presented severe course and an atypical clinical pattern (with involvement of central nervous system) which made appropriate diagnosis difficult and delayed application of specific causal treatment. The other unusual case involved a nurse who contracted Plasmodium through a small skin wound with which patient's blood came into contact when the nurse was drawing blood for testing. Course of the disease was grave, with deep anaemia and central nervous system and kidney involvement. In both cases the disease had favourable outcome due to complex anti-malarial therapy and multispecific medical intervention.

Przemieszczanie się ludności między kontynentami jest coraz częstsze, szybsze i łatwiejsze. W ostatnich latach popularnym celem wypraw turystów są głównie kraje strefy tropikalnej. Bardzo gorący klimat oraz nie najlepszy stan sanitarno-higieniczny powodują ogromne zagrożenie nabycia wielu chorób, które są charakterystyczne dla rejonów egzotycznych. Malaria obecnie jest na trzecim miejscu wśród najważniejszych chorób zakaźnych na świecie, zaraz po AIDS i gruźlicy. Te choroby łącznie powodują więcej niż połowę wszystkich zgonów na całym świecie z powodu chorób infekcyjnych. Malaria jest najczęstszą chorobą człowieka w skali globu. Uważa się, że 40–45% populacji, w tym aż 40% dzieci, żyje na obszarach endemicznych malarii. Trudno o dokładne dane statystyczne pokazujące zachorowalność w Polsce na choroby tropikalne. Pierwszorzędowym zarażeniem pasożytniczym o globalnym znaczeniu jest malaria. Liczba przeniesionych z terenów malarycznych do Europy przypadków przekracza rocznie 10 000, a wywołana nimi śmiertelność stanowi kilka procent. W pracy przedstawiono kierunki rozprzestrzeniania się tej choroby na świecie z uwzględnieniem jej występowania na terytorium Polski.
Population movements between continents is becoming more frequent, faster and easier. In recent years, tropical countries are the most popular travel destination. Very hot climate, poor sanitary-hygienic state cause a huge threat of acquire many diseases that are characteristic of the exotic regions. Malaria is now in third place among the most important infectious diseases in the world, after AIDS and tuberculosis. In total, they cause more than half of all deaths worldwide caused by infectious diseases. Malaria is the most common human disease in the global scale. It is believed that 40–45% of the population, including 40% of children living in malaria endemic areas. It is difficult to obtain accurate statistics on the incidence of tropical diseases Poland. The primary parasitic infection of global importance is malaria. Total transferred cases from malarial areas to Europe exceed 10 000 and mortality mortality is a few percent. The paper presents trends in the spread of the disease in the world with regard to its presence on Polish territory.

Introduction and aim. Accurate identification of Plasmodium species is important because of the differences in their treatment. We aimed to investigate the role of hematological and biochemical parameters in the differentiation of Plasmodium falciparum and other plasmodium species. Material and methods. This is a retrospective study. Patients admitted to the emergency department with signs and symptoms of malaria were included into the study. Patients with malaria were grouped as P. falciparum and others. Hematological parameters of two groups were compared by univariate and multivariate analysis. Statistical analysis was performed using the Jamovi. Results. A total of 107 patients were included in the study. According to univariant and multivariant analysis there was no difference in between two groups in the terms of blood urea nitrogen, aspartate aminotransferase, total bilirubin, hemoglobin, hematocrit, white blood cell count, platelet count, and mean platelet volume (in univariate analysis p values were 0.029, 0.011, 0.019, 0.171, 0.870, 0.307, 0.042, and 0.276, respectively and in multivariate analysis p values for blood urea nitrogen, aspartate aminotransferase, total bilirubin, hemoglobin, and platelet count were 0.100, 0.535, 0.328, and 0.213, respectively). Conclusion. The investigated hematological and biochemical parameters were found to be not valuable in predicting type of malaria. On the other hand, we recommend confirming the results of our study with larger samples and multicenter studies.

As a result of widespread resistance to chloroquine (CQ) and sulphadoxine-pyrimethamine (SP), artemisinin-based combination therapy (ACT) has been recommended as a first-line anti-malarial regimen in Côte d’Ivoire since 2005. A thorough understanding of the molecular bases of P. falciparum resistance to existing drugs is therefore needed. The aims of this study were to analyze the in vitro sensitivity of P. falciparum field isolates from Abobo to CQ, pyronaridine (PYR) and dihydroartemisinine (DHA), and to investigate the polymorphisms associated with drug resistance. The standard in vitro drug sensitivity microtechnique recommended by the WHO was used to assess the sensitivity of Plasmodium falciparum isolates collected in December 2006. The Pfcrt haplotype 76 was analysed by PCR-RFLP while Pfatpase 6 amplification products were sequenced. Associations between drug sensitivity and parasite gene polymorphisms were evaluated with Cohen’s kappa test. The correlation between the IC50 values for different drugs was assessed by the coefficient of determination (r2). Significance was assumed at p<0.05. Of 128 in vitro tests performed, 112 (87.5%) were successful. Of the isolates, 56.2% were resistant for CQ and 48% for PYR. One isolate (3.6%) demonstrated reduced DHA sensitivity (IC50 higher than 10 nM). The mutant K76T pfcrt codon, present in 90% of DNA fragments analyzed, was associated with CQ-R (ĸ=0.76). The N669Y (16.1%), D734Y (28.6%) and D734H (1.8%) isolates were found to have mutant Pfatpase6, however, these mutations were not associated with diminished DHA sensitivity (k=0.01). These high levels of antimalarial drug resistance in Abobo (Côte d’Ivoire) demand further studies of drug efficacy across the whole country.

This paper is a study of G6PD activity in malaria-infected children in Owerri. Blood samples were collected from fifty-one (51) children hospitalized in the Federal Medical Center (FMC) and analyzed using standard Medical Laboratory methods. Results revealed that out of the 51 children examined (26 males and 25 females), 39, representing 76.47% of the total malaria infected children numbers, were G6PD deficient, while 12, representing 23.53%, had normal G6PD. Two different plasmodium species and their percentage occurrences were observed. These were: Plasmodium falciparum (78.43%) and Plasmodium malariae (21.57%). The work indicated that male children were more (P < 0.05) deficient than females, with percentage levels of 61.53% and 38.47%, respectively. In addition, children between the ages of 49-60 months was observed to be more (P < 0.05) G6PD deficient (with percentage of 25.64%), while those between 0-12 months were least G6PD deficient (with percentage of 8.3% (P < 0.05)). As being afflicted with malaria carries a high risk of Glucose-6-phosphate dehydrogenase (G6PD) deficiency in children, there is, therefore, urgent need for public enlightenment on public health implications, need for proper hygiene, as well as a need for strategies for preventing and controlling mosquito populations

Despite significant progress made in drug discovery and development over the past few decades, malaria remains a life-threatening infectious disease across the globe. Because of the widespread emergence of drug-resistant strains of Plasmodium falciparum, the clinical utility of existing drug therapies including Artemisinin-based Combination Therapies (ACTs) in the treatment of malaria has been increasingly limited. It has become a serious health concern which, therefore, necessitates the development of novel drug molecules and/or alternative therapies to combat, particularly resistant P. falciparum. The objective of the present study was to develop 1,2,4-trioxane derivatives as novel antimalarial agents that would be effective against resistant P. falciparum. In our study, 15 new trioxane derivatives were designed by molecular modification of the 1,2,4-trioxane scaffold as possible antimalarial agents. Molecular modeling studies of trioxane derivatives were performed based on the CADD approach using Biovia Discovery Studio (DS) 2018 software. The protein-ligand docking study was performed against P. falciparum falcipain 2 (FP-2) using the simulation-based docking protocol LibDock by the flexible docking method. The assessment of drug-likeness, ADMET properties, and toxicity was also performed. Furthermore, the compounds CC3 and CC7, which showed the best binding affinity against the target P. falciparum FP-2, were investigated by molecular dynamics (MD) simulation studies followed by the calculation of MM-PBSA binding free energy of protein-ligand complexes using DS 2020. Results of the docking study showed that among the 15 compounds, three trioxane derivatives were found to possess promising binding affinity with LibDock scores ranging from 117.16 to 116.90. Drug-likeness, ADMET, and toxicity properties were found to be satisfactory for all the compounds. Among the 15 compounds, two compounds, namely CC3 and CC7, showed the highest binding affinity against FP-2 with LibDock score of 117.166 and 117.200, respectively. The Libdock score of the co-crystal inhibitor was 114.474. MD studies along with MM-PBSA calculations of binding energies further confirmed the antimalarial potential of the compounds CC3 and CC7, with the formation of well-defined and stable receptor-ligand interactions against the P. falciparum FP-2 enzyme. Additionally, the selectivity of trioxane hits identified as potential inhibitors of P. falciparum cysteine protease FP-2 was determined on human cysteine proteases such as cathepsins (Cat K and Cat L), which are host homologous. Finally, it was concluded that the newly designed 1,2,4-trioxane derivatives can be further studied for in vitro and in vivo antimalarial activities for their possible development as potent antimalarial agents effective against resistant P. falciparum

Sequestration is the process by which erytrocytes infected with the mature forms of the malaria parasite Plasmodium falciparum disappear from circulation and accumulate within venules and capillaries of various organs and tissues. The molecular mechanism in sequestration is one of the most rapidly advancing fields in malaria research. The several specific aspects considered in this paper. Our electron micrographs show cytoadherence in own model of cerebral malaria in rats infected with Plasmodium berghei.

The recently identified erythrocyte binding antigen-140 (EBA-140) is a member of the Plasmodium falciparum DBL family of erythrocyte binding proteins, which are considered as prospective candidates for malaria vaccine development. The EBA-140 ligand is a paralogue of the well-characterized P. falciparum EBA-175 antigen. They share homology of domain structure, including Region II, which consists of two homologous F1 and F2 domains and is responsible for ligand-erythrocyte interaction during invasion. It was shown that the F2 domain of EBA-175 antigen seems to be more important for erythrocyte binding. In order to study activity and immunogenicity of EBA-140 antigen F2 domain, it is necessary to obtain recombinant protein of high purity and in a sufficient amount, which used to pose a challenge due to the high content of disulphide bridges. Here, we present a new method for expression and purification of Plasmodium falciparum EBA-140 antigen F2 domain in E. coli Rosetta-gami strain in fusion with the maltose binding protein (MBP). The truncated F2 domain formed by spontaneous proteolytic degradation of the fusion protein was purified by affinity chromatography on Ni-NTA resin followed by size exclusion chromatography. Molecular mass of this protein was confirmed by mass spectrometry. Its N-terminal amino acid sequencing revealed a proteolytic cleavage site within the F2 domain. The proper folding of the recombinant, truncated F2 domain of EBA-140 antigen was confirmed by circular dichroism analysis. The truncated F2 domain can specifically bind to human erythrocytes but its binding is not as efficient as that of full Region II. This confirms that both the F1 and F2 domains of EBA-140 antigen are required for effective erythrocyte binding.

Thrombocytopenia frequently appear in severe malaria. The reasons of low blood platelets count are ditferent and its results of hypersplenism, subclinical course of intravascular coagulation (DIC). Thrombocytopenia from "consumption" is consequence of sequestration of blood platelets in blood vessels of lungs and cerebral. We examination 29 years old men, who was as forest worker in islands on Indonesia. He was treated with recurrent, poliethiological malaria (Plasmodium falciparum, Plasmodium vivax) and severe thrombocytopenia (17.0 GIL) without hepatosplenomegalia. Antiplatelet antibody was examined in blood serum by ELISA methods (GTI - PAKPLUS®). In blood serum was detected IgG antibody against glicoprotein receptors on surface of blood platelets GPIIb/IIIa, GPIV, GPIb/IX, GPV, GPIa/IIa. Chronic infections of Plasmodium may conduct to autoimmune destruction of blood platelets.

Opis:
Thrombocytopenia frequently appear in severe malaria. The reasons of low blood platelets count are ditferent and its results of hypersplenism, subclinical course of intravascular coagulation (DIC). Thrombocytopenia from "consumption" is consequence of sequestration of blood platelets in blood vessels of lungs and cerebral. We examination 29 years old men, who was as forest worker in islands on Indonesia. He was treated with recurrent, poliethiological malaria (Plasmodium falciparum, Plasmodium vivax) and severe thrombocytopenia (17.0 GIL) without hepatosplenomegalia.
