A person with Parkinson's disease has bigger problems with maintaining the stability than the healthy person. Nordic Walking is a safe kind of physical activity for elderly people. The aim of this study is to present the influence of a 6-month therapy of the Nordic Walking on the imbalance and the risk of falls for 66-years-old patient with Parkinson's disease. On the basis of results of 4 tests, performed both at the beginning and at the end of the Nordic Walking therapy, we have made a comparison of patient’s changes before and after 6 months of rehabilitation. the DGI test, the PD patient received 18 points. After he got 22 points. In the Functional Reach Test ( FR), while attempting to reach forward, reached out for 23 cm before the therapy, and 31 cm after the therapy. In the test and go up the patient got a time of 63 seconds, after therapy the time was reduced to 45 s. Nordic Walking is a safe kind of physical activity for people with Parkinson's disease. It reduces muscle tension, and improves joint function.

The purpose of this article is to present speech disorders of 91 years old patient suffering from advanced Parkinson’s disease (it is the last stage of the disease when all symptoms are stronger) and to draw attention to the need of mentioning the impact of speech therapy while describing the disease. In the article an author is introducing the patient and her current condition. Mainly it is focused on the speech therapy diagnosis that was based on particular aspects of speech (such as: general condition of the patient, phonation, comprehension of speech and naming, repea- ting, dialogues and storytelling, reading, counting and writing). In the final part, the effectiveness of speech therapy is presented. This publication might become  a leading example for other speech therapist on how to manage the therapy for people suffering from these types of disorders in speaking and communication.

Neurodegenerative disorders are amongst the most dangerous diseases in modern society. At the end of 1990, adenosine receptor antagonists were used to block the adenosine A1 and A2A receptors causing less physical, cellular and molecular damage caused by Alzheimer’s and Parkinson’s diseases. In recent years, an increase in death rate caused by these diseases has been observed among people under 74. Caffeine, as NMDA receptor antagonist, prevents an uncontrolled influx of calcium ions into the interior of the cells exerting a neuroprotective effect and beneficial procognitive effects. There is much evidence that caffeine intake is associated with a reduced risk of Alzheimer’s and Parkinson’s diseases.

Optogenetics is a novel approach in neuroscience studies that enables to demonstrate a relation between an activity of particular neuronal circuits and a behavior. The discussed method is based on recent findings in genetic engineering and optical techniques allowing the introduction of light-sensitive membrane ion channels into neurons. A pulse of light opens the channel resulting in an ion flow, change of membrane potential and consequently, activation or inhibition of certain neuron. The main advantage of optogenetics is its application in vivo, which allows to control the activity of individual neurons in real-time, even in freely-moving animals during complex behavioral tasks. Our goal was to describe experimental protocols using optogenetic tools along with their advantages and review experimental studies which use optogenetics to examine neuronal bases of addiction, depression, anxiety and Parkinson’s disease. The selection of papers was made b y Pubmed search by keywords: optogenetics, channelrhodopsin and halorhodopsin. The synthesis of results allowed to wirupoint to the role of optogenetics in demonstrating causative relation between particular behavioral phenomena (e.g. reward) and (i) activity (e.g. phasic but not tonic) of defined neural population (e.g. dopaminergic neurons of the lateral ventral tegmental area), (ii) their projections (e.g. fibers projecting to the lateral shell of nucleus accumbens), and (iii) inputs (e.g. from the laterodorsal tegmentum). In summary, the results of optogenetic studies have revealed the role of specific neural systems and the dynamic of their activity in the modulation of the behavior that can be seen both in norma l conditions and in psychiatric disorders.
Optogenetyka jest młodą techniką badawczą, która znajduje obecnie szerokie zastosowanie w neuronaukach. Dzięki innowacyjnemu podejściu możliwe jest ukazanie związków pomiędzy aktywnością określonych układów neuronalnych a zachowaniem. Omawiana metoda badawcza wykorzystuje najnowsze osiągnięcia inżynierii genetycznej i technik optycznych. Polega na wprowadzeniu do komórki nerwowej egzogennych kanałów jonowych, które następnie ulegają wbudowaniu w błonę neuronu i reagują otwarciem na bodziec świetlny. Efektem tego jest przepływ jonów, a w konsekwencji aktywacja bądź inhibicja neuronu. Największą zaletą optogenetyki jest fakt, że można ją stosować in vivo, dzięki czemu uzyskano możliwość kontroli w czasie rzeczywistym aktywności zdefiniowanych populacji komórek nerwowych u zwierząt swobodnie poruszających się i wykonujących złożone testy behawioralne. Celem niniejszej pracy było (i) przybliżenie schematów eksperymentalnych wykorzystujących narzędzia optogenetyczne oraz (ii) przegląd prac badawczych z zakresu uzależnień, depresji, lęku oraz choroby Parkinsona, w których posłużono się optogenetyką. Wyboru prac dokonano na podstawie wyników wyszukiwania w bazie Pubmed haseł takich jak optogenetics, channelrhodopsin i halorhodopsin. Syntetyczne zestawienie wyników badań analizowanych prac wskazuje na rolę optogenetyki w odkrywaniu relacji przyczynowo skutkowych pomiędzy badanym zjawiskiem (np. efekt nagradzający) a (i) aktywnością (np. fazowa ale nie toniczna) poszczególnych populacji neuronów (np. neuronów dopaminergicznych w polu brzusznym nakrywki), (ii) ich specyficznych projekcji (np. do bocznej części skorupy jądra półleżącego), oraz (iii) włókien wstępujących tworzących z danymi neuronami funkcjonalne synapsy (np. z bocznogrzbietowej nakrywki). Podsumowując, wyniki badań z zastosowaniem optogenetyki wykazały rolę specyficznych systemów neuronalnych oraz dynamikę ich aktywności w modulowaniu zachowania charakterystycznego zarówno dla normy jak i zaburzeń psychiatrycznych.

Nowadays, methods are being searched in order to detect Parkinson’s disease at an early stage. This would aid faster diagnosis and a more objective assessment of the severity of the condition. This paper focuses on examining whether typing differs between healthy people and people with varying degrees of Parkinson’s disease. The analysis used publicly available data from the physionet.org database [1], which contains results from 227 people over several experimental days. Data from only 108 subjects were used, with a maximum of 3 days of use of the Tappy application by one participant with at least 200 clicks considered. Computer keyboard typing parameters, such as button hold time, latency and flight time were taken into account. Statistical analysis of the data obtained using non-parametric Kruskal-Wallis tests showed that there were no statistically significant differences (α < 0.05) between the study groups (healthy people and people with varying degrees of Parkinson’s disease). However, the reason for this may be the small study group and the lack of consideration of medications taken when matching to groups.

A robust and highly imperceptible audio watermarking technique is presented to secure the electronic patient record of Parkinson’s Disease (PD) affected patient. The proposed DCT-SVD based watermarking technique introduces minimal changes in speech such that the accuracy in classification of PD affected person’s speech and healthy person’s speech is retained. To achieve high imperceptibility the voiced part of the speech is considered for embedding the watermark. It is shown that the proposed watermarking technique is robust to common signal processing attacks. The practicability of the proposed technique is tested: by creating an android application to record & watermark the speech signal. The classification of PD affected speech is done using Support Vector Machine (SVM) classifier in cloud server.

Parkinson’s disease (PD) is one of most disabling disorders of the central nervous system. The motor symptoms of Parkinson’s disease: shaking, rigidity, slowness of movement, postural instability and difficulty with walking and gait, are difficult to measure. When disease symptoms become more pronounced, the patient experiences difficulties with hand function and walking, and is prone to falls. Baseline motor impairment and cognitive impairment are probable predictors of more rapid motor decline and disability. An additional difficulty is the variability of the symptoms caused by adverse effects of drugs, especially levodopa. Motor assessment of Parkinson`s Disease can be divided into clinimetrics, assessment of balance and posture, arm and hand function, and gait/walking. These are many clinimetric scales used in Parkinson`s Disease, the most popular being the Hoehn and Yahr stages of progression of the disease and Unified Parkinson’s Disease Rating Scale. Balance and posture can be assessed by clinimetric scales like the Berg BS, Tinetti, Brunel BA, and Timed Up and Go Test, or measured by posturometric platforms. Among skill tests, the best known are: the Purdue Pegboard Test, Nine-Hole Peg Test, Jebsen and Taylor test, Pig- Tail Test, Frenchay Arm Test, Action Research Arm Test, Wolf FMT and Finger-Tapping Test. Among motricity scales, the most popular are: the Fugl-Meyer Motor Assessment Scale and Södring Motor Evaluation. Gait and walking can also be assessed quantitatively and qualitatively. Recently, the most popular is three-dimensional analysis of movement. This review article presents the current possibilities of motor assessment in Parkinson`s disease.

Parkinson’s disease can lead to disability and reduce the quality of life of its patients. The purpose of this study is to evaluate the quality of life of a group of people with Parkinson’s disease. The clinical and progressive characteristics of the disease, its motor and neuropsychological impact were evaluated in each Parkinsonian subject included in the study. A quality of life assessment was performed and collected from 60 Parkinsonian patients followed and hospitalized at Hassan II University Hospital in Fez. Different instruments were used, the Hoenh and Yahr scale and the Parkinson’s disease questionnaire (PDQ-39) UPDRS engine, MMS, clinical fact sheet. According to our results, through the different tests and scale of evaluation, we observed an impaired quality of life in the areas of physical discomfort, cognitive disorder, activity of daily living, mobility, and emotional well-being, especially in patients with duration of evolution more than 5 years. There was no significant difference between the two sexes. In addition, the severity of the disease tended to give the impression of an impaired quality of life with respect to the dimensions of activities of daily living and cognition, which is relevant to improving the quality of life patient life and clinical interventions.

Communication difficulties in Parkinson’s disease (PD) arise not only as the result of the motor symptoms of the disorder, but also as a consequence of cognitive and affective impairments which are recognised as being part of the disease process. These changes are thought to account for much of the stigma associated with the condition, thereby complicating the ability of patients to inter-relate with others, including their closest family. This inevitably affects quality of life for both the patient and those family members involved in his/her care. The present paper presents an analysis of how the deficits in motor and cognitive function associated with PD in the form of reduced facial expressivity, altered language skills, motor and cognitive slowness and disturbances in the pragmatic aspects of language affect the communication abilities of patients with the disorder and give rise to stigmatisation, which in turn impacts the disability seen in PD.

Parkinson’s disease (PD) is a neurodegenerative disease. Due to a constantly growing rate of incidence and the lowering age of PD patients it is becoming a more serious social problem. The aim of this study is to assess the quality of life amongst care givers for people suffering from Parkinson's disease. The work includes research on the influence of physical rehabilitation of PD patients on the quality of life of their care givers. The research covered care givers (n=50) of PD patients diagnosed with third stage PD according to the Hoehn and Yahr classification. The diagnostics survey with a questionnaire addressed to care givers looking after PD patients was used. The questionnaire was created by the authors of this paper. The survey consisted of two parts, in which the first was to obtain basic information on the present status of the care giver, while the other was to verify life satisfaction level, using Jurczynski's Scale of Life Satisfaction. Statistical analysis showed significant differences in all analyzed statements between groups. Better effects were observed in the group participating in rehabilitation. Care for people suffering from PD affects the quality of life of caregivers. Participation in the process of rehabilitation of patients with PD improves the quality of life of caregivers.

With global life expectancy rising every year, ageing-associated diseases are becoming an increasingly important problem. Very often, successful treatment relies on early diagnosis. In this work, the issue of Parkinson's disease (PD) diagnostics is tackled. It is particularly important, as there are no certain antemortem methods of diagnosing PD - meaning that the presence of the disease can only be confirmed after the patient's death. In our work, we propose a non-invasive approach for classification of raw speech recordings for PD recognition using deep learning models. The core of the method is an audio classifier using knowledge transfer from a pretrained natural language model, namely wav2vec 2.0. The model was tested on a group of 38 PD patients and 10 healthy persons above the age of 50. A dataset of speech recordings acquired using a smartphone recorder was constructed and the recordings were labelled as PD/non-PD with the severity of the disease additionally rated using Hoehn-Yahr scale. We then benchmarked the classification performance against baseline methods. Additionally, we show an assessment of human-level performance with neurology professionals.

Parkinson’s disease (PD) is a progressive, neurodegenerative disease leading to the occurrence of movement disorders, postural instability, gait disorders, falls, non-motor symptoms and disability. Rehabilitation is an important component of multidisciplinary treatment in PD. The aim of the study is to present the benefits and effects of using dance in the rehabilitation of patients with PD. Dance can have a beneficial influence on the physical, psychological, emotional, intellectual, aesthetic, cultural and social spheres of human life. Positive effects, which can be obtained through regular dance classes, in particular when they accurately correspond to the therapeutic goals of rehabilitation in PD. Patients with PD benefit from dance training in the way other dancing individuals do. Dance training includes various postural, balance and gait exercises performed to rhythm and music. It uses strategies naturally used in dance which are particularly useful in the therapy of patients with PD: concentration on movement performed to music, imagination of movement before its execution, multisensory stimulation and using sensory cues. Music and dance facilitate the initiation of movement and its performance at a proper speed and amplitude. In the limited studies concerning the effects of dance rehabilitation programs on PD, positive results were obtained in balance, gait, functional status and the quality of patient life . The authors used various forms of dance: the elements of classical ballet, jazz dance, ballroom dancing, Argentine tango, group dances, folk dances, contemporary dance, contact improvisation, elements of choreography dance theatre. Dance in various forms can be the tool for multidisciplinary rehabilitation, joining the elements of physiotherapy, art therapy (dance and music therapy), rhythmic auditory stimulation as well as social integration.
Choroba Parkinsona (PD) jest postępującą chorobą neurodegeneracyjną, która prowadzi do wystąpienia zaburzeń ruchowych, niestabilności postawy, zaburzeń chodu, upadków, zaburzeń pozaruchowych i niesprawności. Rehabilitacja jest istotnym elementem kompleksowego leczenia chorych na PD. Celem pracy jest przedstawienie korzyści oraz efektów zastosowania tańca w rehabilitacji chorych na chorobę Parkinsona w oparciu o dostępne piśmiennictwo. Taniec wpływa korzystnie na sferę fizyczną, psychiczną, emocjonalną, intelektualną, estetyczną, kulturalną i społeczną człowieka. Pozytywne efekty, które można uzyskać poprzez regularne zajęcia taneczne szczególnie trafnie korespondują z celami terapeutycznymi rehabilitacji ruchowej w PD. Pacjenci z PD korzystają z treningu tanecznego, jak inne tańczące osoby. Trening taneczny obejmuje różnorodne ćwiczenia postawy, równowagi i chodu wykonywane do rytmu i muzyki. Wykorzystuje strategie naturalnie stosowane w tańcu i szczególnie przydatne w usprawnianiu chorych z PD, m.in.: koncentrację na ruchu do muzyki, wyobrażenie ruchu przed jego wykonaniem, wielosensoryczną stymulację i wskazówki sensoryczne. Muzyka i taniec ułatwiają rozpoczynanie ruchu i wykonywanie go z odpowiednią szybkością i amplitudą. W nielicznych badaniach dotyczących skuteczności tanecznych programów rehabilitacyjnych w PD uzyskano pozytywne efekty w zakresie równowagi, chodu, stanu funkcjonalnego i jakości Ŝycia chorych. Autorzy stosowali różne style i formy taneczne: elementy baletu klasycznego, jazz dance, taniec towarzyski, tango argentyńskie, tańce integracyjne, ludowe, taniec współczesny, kontaktową improwizację taneczną, teatr tańca oraz pantomimę. Taniec może być narzędziem kompleksowej rehabilitacji w PD, który łączy elementy fizjoterapii, terapii sztuką (tańcem i muzyką), rytmicznej stymulacji dźwiękowej i integracji społecznej.

INFLAMMATORY MARKERS AND NEUROPEPTIDES CHARACTERISTIC OF PARKINSON’S DISEASE AND THEIR RESPONSE TO VISUALIZATION AND SUGGESTION BASED MIND-BODY THERAPY

Progress in elucidating neuroimmune connections has created new opportunities for improving the treatment of Parkinson’s disease (PD). In recent years, mind-body therapies have been shown to have positive effects on the immune and nervous systems, but interactions at the molecular level have not been tested. Thus, the main aim of the present study was to investigate the effects of therapy based on visualization and suggestion on the concentrations of IL-6, CRP, DA, BDNF, CoQ10, and TAC in patients with advanced PD. Eight patients with PD and 8 elderly healthy men (control group) were enrolled in the study. The therapy lasted 19 days and consisted of three parts: individual sessions with the therapist, mixed (therapist and audio-file), and self-training (audio-file). Blood samples were taken before training and at the end of each part. The expected changes in the investigated markers was observed during therapy: the serum concentration of IL-6 and CRP decreased, whereas DA and BDNF increased, however, this change was observed only after the first part of intervention when the therapy was conducted by the therapist. In the subsequent stages, the levels returned to the baseline. Noteworthy, after the therapy, we observed a significant increase in the motor and intellectual skills of the PD patients. No such changes were observed in the control group. Mind-body therapy based on visualization and suggestions aimed at changing the concentrations of signaling molecules, which are crucial in the development or treatment of PD, may be an effective element in supporting pharmacological therapy.

Introduction: Nicotine is a widely known alkaloid synthesized from tobacco plants, being a main constituent of tobacco smoke and cigarettes. Nicotine has also gained eminence as the therapeutic option in managing smoking cessation and even other health conditions. However, the therapeutic potential of nicotine in other diseases has yet to be completely assessed. This information void stems from an inherent aversion from researchers in assessing nicotine’s risk-benefit, due to its toxicities. We present information on the current body of evidence relating to non-traditional therapeutic applications of nicotine to fill this literature void. The purpose of this work is to present current literature on the therapeutic uses of nicotine in treating various diseases. Methods: Electronic search in PubMed for relevant research relating to the therapeutic potential of nicotine in various diseases. Results/conclusions: Nicotine has gained therapeutic significance as an active compound of the nicotine replacement therapy. Research show nicotine may have other therapeutic applications in some diseases. We discussed application of nicotine in diminished prevalence of Parkinson’s disease, decreasing symptoms of the Tourette’s syndrome, psychiatric diseases like chizophrenia, depression and management of pain. We also reviewed nicotine dosing, type of formulations, and compliance which are crucial factors in the therapeutic applications of nicotine.

Introduction and objective. Continuous subcutaneous apomorphine (APO) treatment is one of the 3 therapeutic options for advanced Parkinson’s disease (PD), in addition to deep brain stimulation (DBS) and intrajejunal levodopa. Data from previously performed studies show that few PD patients can achieve APO infusion as monotherapy. The current pilot study presents the authors’ experience in achieving APO monotherapy. Materials and method. During the last 2 years, 9 patients with APO were treated in the Department of Neurology of the Medical University of Lublin; each patient was offered a 5-day duration APO treatment as monotherapy. The main indication for the APO therapy was advanced PD with motor fluctuations and the patient’s non-agreement for DBS therapy. Mean age of treated patients – 65.11 years, mean disease duration – 7.67 years, mean Hoehn-Yahr – 2.67, mean L-dopa equivalent before APO treatment – 1751.11 mg, mean daily dose of apomorphine as monotherapy – 106.11 ± 14.09 mg. Results. All treated patients managed to achieve APO monotherapy. A statistically significant reduction was found in the duration of the ‘off’ states in the observed PD patients on APO monotherapy (p<0.05). No significant improvement was observed in the III motor score of the UPDRS on APO treatment, compared to optimized oral therapy used before APO treatment. Conclusions. APO monotherapy can be achieved in advanced PD, and seems to be a good therapeutic option for this group of patients, especially in that it allows a significant reduction in the off-time which significantly simplifies the drug regime. Nevertheless, hospital admission with experienced neurologist supervision is recommended when establishing a PD patient’s APO monotherapy.
