Gunshots with non-powder weapons are relatively common in children, and most of them occur unintentionally. This paper describes the second known case of an adult male in whom a shot from an airgun resulted in the presence of a projectile within the maxillary sinus. A 47-year-old man was shot inadvertently about 10 years earlier and was asymptomatic at the time of reporting. The presence of a foreign body detected on a routine orthopantomogram was confirmed by a medical interview and a three-dimensional radiological examination. The pellet located in the right maxillary sinus was removed from intraoral access through the anterior wall of the sinus. The course of the surgical procedure was complicated by the projectile dislocation in relation to the position determined by radiological examinations. Postoperative care and a 4-year follow-up period were uneventful. The maxillary sinus may be considered a favorable location for a persistent projectile, which may reside in the maxillofacial region asymptomatically for many years. The foreign body may move loosely in the maxillary sinus, which should be taken into account when planning an operation.

Introduction: Computed tomography is an important imaging technique in Emergency Units. Thanks to its popularity, radiological changes are found in healthy children more commonly. The aim of this paper is to evaluate the incidence of maxillary sinus radiological changes in children with head trauma who admitted to the Emergency Unit of the University Children Hospital in Lublin. Material and Methods: A retrospective analysis of computed tomography scans of children suffering from head trauma admitted to the Emergency Unit of the University Children Hospital in Lublin was carried out. A group of 425 patients was analyzed. Results: Maxillary findings were present in 81 cases (19.06%); in 38 patients (8.94%) the changes were unilateral, while in 43 (10.12%) they were bilateral. Maxillary mucosal thickening was the most common radiological abnormality, present in almost 12% of the investigated cases (approximately 62% of all revealed changes). Maxillary total opacification as an isolated finding was found in younger children only. Retention cysts and maxillary polyps were found with a similar low frequency as maxillary opacification but in elder children only. Discussion: Asymptomatic radiological changes in computed tomography scans are common. Maxillary mucosal thickening is the most frequent asymptomatic abnormality. Maxillary polyps and pseudocysts are rare in the paediatric population. Maxillary opacification suggests other more significant pathologies and requires further diagnostics. Physicians should avoid diagnosing patients with sinusitis without proper examination and based on radiological abnormalities only. Paediatric patients with revealed maxillary changes should remain under regular laryngological control.

Introduction: Inflammatory myofibroblastic tumor (IMT) is a rare condition that can mimic potentially more dangerous states such as malignant tumors. The tumor itself can also show local malignancy as well as malignant transformation. The paranasal sinus IMT is quite a rare case in the literature. The manifestation of the disease can include face swelling, nasal obstruction, epistaxis, vision acuity worsening, numbness of the face, pain. Etiology of this type of lesion still remains uncertain but there are a few assumptions on the issue: viral and genetic, as well as posttraumatic and postinflammatory. We report a case of an adult woman with IMT detected in the right maxillary sinus after endoscopic sinus surgery. Wstęp: Zapalny guz miofibroblastyczny (IMT) jest rzadkim stanem, który może naśladować potencjalnie bardziej niebezpieczne choroby, takie jak nowotwory złośliwe. Sam guz może również wykazywać miejscową złośliwość, a także złośliwą transformację. IMT zatok przynosowych jest dość rzadko opisywany w literaturze. Objawem choroby może być obrzęk twarzy, niedrożność nosa, krwawienie z nosa, pogorszenie ostrości wzroku, drętwienie twarzy, ból. Etiologia tego typu zmian nadal pozostaje niejasna, ale rozważa się następujące przyczyny powstawania guza: wirusową, genetyczną, a także pourazową i pozapalną. Niniejszy opis przypadku dotyczy dorosłej kobiety z IMT wykrytym w prawej zatoce szczękowej po endoskopowej operacji zatok. Introduction. Oroantral communication (OAC) between the maxillary sinus and the oral cavity is an infrequent post-surgical complication occurring most commonly after extraction of posterior maxillary teeth. Aim. To present the characteristics of OAC and predisposing factors as well as evaluate postoperative pharmacological therapy and complications in patients with an OAC. Material and methods. In this retrospective study, medical records of 63 patients with diagnosed OAC between 2011 and 2018 were analyzed. Results. The most frequent causes for tooth extraction leading to an OAC were periodontitis (n=34; 54%), carious destruction of the tooth (n=14; 22.2%), and tooth impaction (n=10; 16%). First molars (n=28; 44.4%), second molars (n=14; 22.2%) and third molars (n= 13; 20.6%) were the most frequently related teeth to OACs. The majority of OACs appeared in the fourth (n=22; 35%) and third (n=20; 31.7%) decades of life. Conclusion. OACs are rarely seen on an everyday basis by general practitioners; however, if left untreated, they may lead to further serious complications. Proper postoperative precautions must be taken in order to prevent further complications, and thus the evaluation of predisposing factors is of great importance.

Introduction: The topographic relation between the maxillary teeth roots and the maxillary sinus floor is important for diagnosis and planning of many surgical procedures. The aim of the study was to assess the topographic relationship between posterior maxillary teeth and the sinus floor. Materials and methods: 50 pantomographs of patients aged 18-72, treated at the Medical University of Gdańsk, were analysed. We analysed only maxillary molars and premolars. Teeth were assessed using Kwak classification. Statistical analysis was carried out using STATISTICA 13.3. Results: We analysed 180 molars and 181 premolars (total 361). According to the root-sinus classification, most of the first molars were type V (55.96%), most of the second molars – type III (32.29%), most of the first premolars – type I (76.09%) and most of the second premolars – type III (35.96%). Non-parametric Spearman’s rank-order correlation coefficient revealed significant correlation between right and left molars and between right and left premolars (p<0.05). Conclusions: Proper assessment of the pantomograph allows for correct pre-operative planning, which may affect the course of the procedure and enables to avoid possible complications. These findings may have clinical applications.

Introduction. Combined aplasia of multiple sinuses is extremely rare. Agenesis of the paranasal sinuses is an uncommon clinical condition that appears mainly in the frontal (12%) and maxillary (5-6%) sinuses. Case report. In this paper, we present the case of a 74-year-old woman with combined frontal and shenoid sinus aplasia accompanied by unilateral maxillary sinus hypoplasia. The findings were confirmed by a computed tomography scan of paranasal sinuses. The reason for admission was persistent headache, numbness of the left cheek and left alveolar process, and occasional nasal blockage. Discussion. The uniqueness of our case is that the patient is an elderly female with combined aplasia of the frontal and sphenoid sinus with hypoplastic maxillary sinuses, whereas previously reported cases were found in children and in young adults. Summary and conclusions. These anomalies can be misdiagnosed as chronic sinusitis or neoplasm. All potential sinus anomalies will have clinical implications and will hinder conventional and functional endoscopic sinus surgery

The dental implants are becoming more used by dental surgeons to perform prosthetic rehabilitation, therefore sinus lift procedure is more often performed. The complications after this procedure by the ENT specialists will be observed. So far a valid algorithm of candidates with naso-sinusal diseases for sinus lift procedure was not established. The interdisciplinary management, including ENT specialist, not only increases the expectation of a better procedural outcome, but also provides a better control while complications occur. The authors postulate that knowledge about sinus lift technique and possible postoperative complications should be well known, both by stomatologists and ENT specialists.
Wraz z rozpowszechnieniem rehabilitacji protetycznej pacjentów przy wykorzystaniu implantów zębowych zwiększyła się także częstość wykonywania zabiegu podnoszenia dna zatoki szczękowej. Z tego względu lekarz laryngolog częściej będzie spotykał się z powikłaniami pozabiegowymi u pacjentów po zabiegu sinus lift. Do tej pory nie ustalono konsensusu, co do algorytmu postępowania u pacjentów z chorobami nosa i zatok przynosowych przygotowywanych do zabiegu podniesienia dna zatoki szczękowej. Wydaje się, że podejście interdyscyplinarne pozwala na obniżenie ryzyka możliwych powikłań pozabiegowych, a w przypadku ich wystąpienia opanowanie trudnej sytuacji. Z tego względu autorzy są zdania, że dla lepszej współpracy pomiędzy lekarzem stomatologiem a laryngologiem procedura zabiegu podniesienia dna zatoki szczękowej oraz możliwe komplikacje pozabiegowe powinny być powszechnie znane.

Functional endoscopic sinus surgery (FESS) is most commonly used in chronic rhinosinusitis treatment. This method is also applicable to other diseases, including the treatment of symptomatic ectopic teeth. Ectopic teeth are a quite rare phenomenon. They may appear within the facial region, as well as in other parts of the body. The article describes two cases of ectopic teeth appearing within the nose area and paranasal sinuses, there endoscopic surgery has been used in the extraction, which resulted in the remission of symptoms.

Silent sinus syndrome (SSS) is a rare entity characterized by ipsilateral chronic hypoventilation and atelectasis of the maxillary sinus, which may lead to facial or ocular asymmetry, hypoglobus, enophtalmos and diplopia. In this report, we present a case of a 21-year-old patient scheduled for an orthognathic procedure due to skeletal malocclusion. Patient complained about problems with nasal breathing. CT scans revealed an atrophic, inflamed, hypoventilated left maxillary sinus with obstruction of the left ostiomeatal complex, septal deviation and hypertrophy of the left interior turbinate. Furthermore atrophic and asymmetrical left zygomaticomaxillary complex with shifting of maxillary bite plane and left malar depression accompanied by slightly displaced inferior border of left lateral orbital wall was noted. Due to clinical symptoms of SSS, patients are often referred to the ears, nose and throat (ENT) department or to the ophthalmologist. Silent sinus syndrome diagnosis begins upon clinical examination and it is confirmed by radiological imaging (usually CT). The most characteristic radiological finding is maxillary sinus volume reduction as a result of inward retraction of the sinus walls. It is important to perform differential diagnosis, taking into consideration other conditions that present with enophtalmos. The main goal of surgical treatment is to normalize the ventilation of maxillary sinus by improving its drainage toward the nasal cavity and, in selected cases, restoration of the orbital floor.

Fungi are organisms which occur in the human environment. One of the potential pathogenic fungi is Aspergillus which belongs to mould, and is an etiological factor of non-invasive fungal paranasal sinusitis. Objective. Epidemiological analysis of aspergillosis of the maxillary sinuses. Material and methods. Retrospective analysis of the medical histories of 41 patients treated in the Maxillofacial Surgery Department of Medical University in Lublin, Poland between 2005–2014 due to non-invasive aspergillus maxillary sinusitis. The patients’ gender, age, and etiopathogenesis of the condition with signs and symptoms, and methods of treatment were analysed. Histological examination was crucial in the final diagnosis. Results. The majority of the patients constituted women aged 29–72. The most common complaints were suborbital pain, rhinorrhoea and impaired nasal ventilation. All the patients were treated surgically, and pharmacologically with Fluconazole. Conclusion. Fungal maxillary sinusitis should be taken into account in every case of chronic maxillary sinusitis resistant to standard treatment. Women are more susceptible to Aspergillosis, and the risk factors for the disease are endodontic treatment of the maxillary teeth and fistula antro-oralis post extractionem. Surgical treatment sometimes should be complemented by pharmacological antimycotic treatment.
